Transdermal Ureteroscopic Nephrolithotripsy for Ureteropelvic Joint Calculi with Combined Holmium∶YAG Laser and Frequency Doubled Double-pulse Laser

Abstract:Objective To evaluate the feasibility and availability of transdermal ureteroscopic nephrolithotripsy with holmium ∶YAG laser and frequency-doubled double-pulse(FREDDY) laser in treatment of ureteropelvic joint(UPJ) calculi.Methods The data of 54 patients(29 males and 25 females;age range,21-88 years;mean age,42 years) who underwent transdermal ureteroscopic nephrolithotripsy with holmium ∶YAG laser and FREDDY laser were viewed.The longest diameter of the ureteropelvic joint calculi ranged from 1.2 to 3.2 cm with a mean of 1.7 cm.Among the 54 patients,hematuria and low back pain in different degree occurred in all cases,complicated moderate/severe hydronephrosis in 38 cases,complicated urinary infection in 8 cases and complicated renal and previous ESWL failure in 25 cases.During the operation,holmium ∶YAG laser and FREDDY laser were used simultaneously or successively according to the different size,distribution and character of the stone.The success criterion of lithotripsy was defined as the fragmented stones of less than 4mm in diameter.Results The ureteropelvic joint calculi in 54 cases were all fragmented after a single session without severe complications as hemorrhagia,water intoxication,perinephric abscess or obstructive pyonephrosis.The average procedure time of the ureteropelvic joint calculus was 12 minutes.The average postoperative hospital day was 1.7.The nephrostomy tubes were indwelled for 3.4 days in average.Follow-up for 6-24 months(mean,12.6 months) in the 54 cases showed no recurrence of calculi under plain KUB film.Conclusions The transdermal ureteroscopic nephrolithotripsy for ureteropelvic joint calculi with combined holmium ∶YAG laser and FREDDY laser is a kind of method with the characteristics of efficiency,effectivity,safety,less invasion and stone clearance,giving full play to their respective advantages of two lasers.
