Abstract:OBJECTIVE To promote the clinical awareness of cross allergy and prevent the occurrence of medical errors.METHODS Prescribing information on cross allergy in "Contraindications" was derived from a software.Records of drug counseling,cases of medical errors and near misses in an online reporting system were analyzed.Data mining was performed.Clinical pharmacological interventions were conducted and the effects were evaluated.RESULTS Besides 27 kinds of drugs within pharmacologically similar class involving cross allergy,there were seven kinds of drugs with complete different pharmacological effects.Similarity in chemical structures was the main case of cross allergy.Excipients were related with cross allergy.Prescribing information in some drugs had unscientific descriptions on cross allergy associated contraindications,lacking clinical value in guiding safe use of medication.There were difference in descriptions on cross allergy in some drugs within pharmacologically similar clas,and some domestic and imported products.The problem of cross allergy of drugs involving different pharmacological effects was dangerous and not easy to be detected.After pharmacological interventions,awareness was greatly increased and the occurrence rate of medical errors and similar mistake decreased by 97%.CONCLISION Severe potential safety hazard might exist if more attentions are not paid to cross allergy aspect.The comprehensive pharmacological interventions are very effective.Applications of information technology and data maintenance are pivotal for continuous quality improvement in cross allergy aspect.

Abstract:Allergic reaction is one of the common adverse reactions in clinic. However, the clinical manifestation of anaphylactoid reaction is similar to allergic reaction, which makes it difficult to distinguish.Due to the wide variety of drugs related to anaphylactoid reactions and the incomplete study of their mechanisms, there is no standard model to evaluate the anaphylactoid reactions induced by drugs at home and abroad. This review summarizes the mechanism, the material basis of drugs, and the evaluation of anaphylactoid reaction, in order to provide references for further explorations.

Abstract:Effective strategies can now be developed in emerging economies to alleviate the large-scale allergy problems which are predicted. Such strategies may be initiated from the perspective of the potentially allergic individual and from the allergenic agents. Regarding symptom development and its alleviation, three levels of prevention can be applied. Primary prevention addresses symptom-free children at risk in particular. Secondary prevention focuses on sensitized individuals in their societal context. Symptom development can be prevented by avoidance of the offending allergens in the environment and food. Tertiary prevention includes medical treatment of the (chronic) symptoms. Since all major allergens are assumed to be known, high-throughput genomics and proteomics analyses can now be applied to diagnose a novel food for the presence, the molecular composition and the expressed amount of one or more of these allergens.

Abstract:Drug refers to immunological mechanism-mediated drug hypersensitivity reactions,which may be either IgE-mediated(immediate) or non-IgE mediated(delayed) hypersensitivity reactions.Small molecular drug can induce immune response by binding to carrier protein as prototype or metabolism product,and even can directly bind to T cell antigen receptor to activate CD4+ T cells or CD8+ T cells.Drug can induce type Ⅰ to Ⅳ hypersensitivity reactions after recognized by immune system.The main detection methods of drug allergy include in vivo skin tests,in vitro laboratory tests and drug provocation tests.

Abstract:Allergic reaction is also known as hypersensitivity reaction, refers to the abnormal specific immune response that occurs in the body after the sensitized body is exposed to the same antigen again. Allergic reaction is mainly physiological dysfunction or pathological damage to tissues and cells. In recent years, with the rapid changes in the human living environment, the incidence of allergic diseases has increased day by day. In the past 20 years, the incidence of allergic diseases has doubled in Western developed countries, and the global prevalence has reached as high as 22%. In recent years, many breakthroughs have been made in the diagnosis, prevention, and treatment of allergic diseases. But there are still gaps in many areas. This article analyzes the integration and analysis of allergy types, causes, treatments, and clinical symptoms. It provides basic knowledge and theoretical support for future research. The problem of preventing allergies has not yet been solved, and future research can focus on methods to prevent allergic diseases.

Abstract:Hypersensitivity conditions, a group of common disorders driven by the immune system, include a range of ailments such as hay fever (Allergic Rhinitis, AR), bronchial asthma, ocular allergies, eczema (Atopic Dermatitis, AD), and reactions to certain foods. These conditions typically originate from the body's immunoglobulin E (IgE) responses to harmless external substances, or allergens, with the specific response depending on the allergen's point of contact with the body. [ 1 ] Diagnostic techniques involve detecting specific IgE (sIgE) antibodies to these allergens either through skin (dermal puncture tests) or by identifying sIgE in blood samples (blood-sIgE assessments). [ 2 ] The immunoblot method is a commonly used semiquantitative approach to measure allergen levels. Globally, the prevalence of hypersensitivity conditions is increasing, presenting a significant health challenge that affects a substantial portion of the population. This rise adversely impacts the well-being of those affected. Hay fever, for instance, a widespread hypersensitivity condition affecting the nasal lining, is estimated to impact between 10% and 40% of people worldwide. [ 3–5 ]

Abstract:Food allergy is phenotypically an extremely heterogeneous group of diseases affecting multiple organs,sometimes in an isolated way,sometimes simultaneously,with the severity of reactions ranging from mild and local to full-blown anaphylaxis.Mechanistically,it is defined as a Th2-driven immune disorder in which food-specific IgE antibodies are at the basis of immediate-type adverse reactions.The sites of sensitization and symptoms do not necessarily overlap.Food allergy,which is the theme of this paper,is often confused with other adverse reactions to food of both animmune (e.g.,celiac disease) and nonimmune (e.g.,lactose intolerance) nature.To reliably diagnose food allergy,a careful history (immediatetype reactions) needs to be complemented with demonstration of specific IgE (immune mechanism) and confirmed by an oral challenge.Co-factors such as exercise,medication,and alcohol may help trigger food allergy and further complicate accurate diagnosis.Where food extract-based diagnostic tests are poorly correlated to symptom severity,new generation molecular diagnostics that measure IgE against individual food allergens provide clinicians and patients with more reliable symptom severity risk profiles.Molecular diagnostics also support establishing whether food sensitization originates directly from exposure to food or indirectly (cross-reactivity) from pollen sensitization.Epidemiological surveys have indicated that allergy to peach primarily originates from peach consumption in Europe,whereas in China it is the result of primary sensitization to mugwort pollen,in both cases mediated by an allergen molecule from the same family.Epidemiological surveys give insight into the etiology of food allergy,the size of the problem (prevalence),and the risk factors involved,which together support evidence-based strategies for prevention.Over the past decade,food allergy has increased in the affluent world.Economic growth and urbanization in upcoming economies are likewise expected to lead to increased prevalence of food allergies,sometimes to different foods due to dietary habits.Molecular allergology and biotechnology now offer the possibility to combat the increasing burden of food allergy by developing safe immunotherapies for food allergy,using hypoallergenic mutant recombinant molecules.The first clinical trials to evaluate such approaches are underway.Last but not least,the identification and clinical risk characterization of a more and more complete list of food allergens additionally provides the allergenicity risk assessment of genetically modified foods a firmer basis.

Abstract:Approximately 30% of the world population suffers from immunoglobulin-E (IgE)-mediated allergy. IgE-mediated allergy affects the respiratory tract, the skin and the gastrointestinal tract and may lead to life-threatening acute systemic manifestations such as anaphylactic shock. The symptoms of allergy are mediated by IgE-recognition of causative allergen molecules from different allergen sources. Today, molecular allergy diagnosis allows determining the disease-causing allergens to develop allergen-specific concepts for prevention and treatment of allergy. Allergen-specific preventive and therapeutic strategies include allergen avoidance, vaccination, and tolerance induction. The implementation of these preventive and therapeutic strategies requires a detailed knowledge of the relevant allergen molecules affecting a given population. China is the world ́s most populous country with around 1.4 billion inhabitants and an estimated number of more than 400 million allergic patients. Research in allergy in China has dramatically increased in the last decade. We summarize in this review article what is known about the dominating allergen sources and allergen molecules in China and what further investigations could be performed to draw a molecular map of IgE sensitization for China as a basis for the implementation of systematic and rational allergen-specific preventive and therapeutic strategies to combat allergic diseases in this country.

Abstract:China is a large country both in area and population size, with diverse landscapes, climates and environments, and its domestic economy, international trade and tourism is rapidly growing. Under such conditions and in line with the above growths, the occurrence of allergy problems is unexpectedly increasing. A scarcity of public, governmental and food-industry knowledge and awareness of allergies and allergenic sources in China may cause a serious burden to the national population as well as to foreign consumers. Numerous allergenic sources have been reported in China, most of which are however only available in Chinese literature and have not yet been systematically reviewed and summarized in English. Here, we present an overview of the most relevant allergen types as well as their sources and the most prominent examples in China.

Comprehensive Reviews in Food Science and Food Safety

Abstract:Food allergies are a global food safety problem. Peanut allergies are common due, in part, to their popular utilization in the food industry. Peanut allergy is typically an immunoglobulin E-mediated reaction, and peanuts contain 17 allergens belonging to different families in peanut. In this review, we first introduce the mechanisms and management of peanut allergy, followed by the basic structures of associated allergens. Subsequently, we summarize methods of epitope localization for peanut allergens. These methods can be instrumental in speeding up the discovery of allergenicity-dependent structures. Many attempts have been made to decrease the allergenicity of peanuts. The structures of hypoallergens, which are manufactured during processing, were analyzed to strengthen the desensitization process and allergen immunotherapy. The identification of conformational epitopes is the bottleneck in both peanut and food allergies. Further, the identification and modification of such epitopes will lead to improved strategies for managing and preventing peanut allergy. Combining traditional wet chemistry research with structure simulation studies will help in the epitopes' localization.

Abstract:In past 20 years, the average prevalence of asthma in China has been growing up from 0.9 % to about 3.0 %. We carried out these studies to find out the reasons and relative molecular mechanisms. Data multiple-regression was used for prevalence and risk factor investigation (1990-2000) and animal experiments were used for mechanism study. According to multiple-regression, 10 statistical items, in which some of the items are building related factors, were found to be the asthma risk factors significantly; in the animal model of formaldehyde induced asthma, vanilloid receptor, thymic stromal-lymphocytes protein and S-nitrosoglutathione reductase were found to have important roles in asthma initiation and promotion.

Abstract:Drug allergy (DA) remains a complex and unaddressed problem worldwide that often deprives patients of optimal medication choices and places them at risk for life-threatening reactions. Underdiagnosis and overdiagnosis are common and due to the lack of standardized definitions and biomarkers. The true burden of DA is unknown, and recent efforts in data gathering through electronic medical records are starting to provide emerging patterns around the world. Ten percent of the general population engaged in health care claim to have a DA, and the most common label is penicillin allergy. Up to 20% of emergency room visits for anaphylaxis are due to DA and 15%-20% of hospitalized patients report DA. It is estimated that DA will increase based on the availability and use of new and targeted antibiotics, vaccines, chemotherapies, biologicals, and small molecules, which are aimed at improving patient's options and quality of life. Global and regional variations in the prevalence of diseases such as human immunodeficiency virus and mycobacterial diseases, and the drugs used to treat these infections have an impact on DA. The aim of this review is to provide an update on the global impact of DA by presenting emerging data on drug epidemiology in adult and pediatric populations.

Abstract:Traditional Chinese Medicine (TCM) treating allergic diseasesis effective and with few adverse reactions. The effects of TCM compound, TCM extracts and Chinese herbal monomers played the anti-allergic role in multiple stages. Such as reducing and preventing target cell degranulation, releasing biologically active mediators, and inhibiting the conduction of signal pathways. Recent studies have found that in addition to anti-allergic effects by regulating cellular and humoral immunity, TCM can also enhance the body’s non-specific immune function to different degrees and help eradicate certain intractable allergic diseases. The development of new therapeutic agents for allergic diseases with good effect and less adverse reactions from Chinese herbal medicine will become a trend in the future research and have broad medical prospects.

Abstract:Research on drug hypersensitivity is in a rapid development stage and an emerging trend in reports of anaphylaxis to polyethylene glycols is identified. Developing algorithms for understanding the standardization process of culprit drugs, clinical manifestations, and diagnostic methods will be the focus of future direction. In addition, a better understanding of the mechanisms to culprit drugs with immunological precise phenotypic definitions and high‐throughput platforms is needed. Background Drug hypersensitivity is a major global public health issue with a significant increase in prevalence in populations. Here, we provide a deep insight into the frontier hotspot and future direction in the field of drug hypersensitivity. Methods A knowledge map is portrayed based on publications related to drug hypersensitivity from Web of Science Core Collection using CiteSpace. Co‐occurrence relationships of countries, institutes, authors, journals, references, and keywords are constructed. According to the co‐occurrence relationships, hotspots and future trends are overviewed. Results The United States ranked first in the world and China with the second highest publications was the only developing country. Torres, Mayorga, and Blanca were highly productive authors. Harvard University was the institution with the most research publications. Keywords co‐occurrence analysis suggested applications in emerging causes, potential mechanisms, and clinical diagnosis as the research hotspots and development frontiers. Conclusion Research on drug hypersensitivity is in a rapid development stage and an emerging trend in reports of anaphylaxis to polyethylene glycols is identified. Abstract:The specialties of allergy and clinical immunology have entered the era of precision medicine with the stratification of diseases into distinct disease subsets, specific diagnoses and targeted treatment options, including biologicals and small molecules. This article reviews recent developments in research and patient care and future trends in the discipline. The section on basic mechanisms of allergic diseases summarizes the current status and defines research needs in structural biology, type 2 inflammation, immune tolerance, neuroimmune mechanisms, role of the microbiome and diet, environmental factors, and respiratory viral infections. In the diagnostic challenges section, clinical trials, precision medicine and immune monitoring of allergic diseases, asthma, allergic and non‐allergic rhinitis and new approaches to the diagnosis and treatment of drug hypersensitivity reactions are discussed in further detail. In the third section, unmet needs and future research areas for the treatment of allergic diseases are highlighted with topics on food allergy, biologics, small molecules, and novel therapeutic concepts in allergen‐specific immunotherapy for airway disease Unknowns and future research needs are discussed at the end of each subsection.
