Glucocorticoid Resistance in Acute Lymphoblastic Leukemia:Molecular Mechanisms and Related Issues

Abstract:Glucocorticoids(GC) can specifically induce cell cycle arrest and apoptosis in lymphoid cells,and therefore for decades constitute a central component in the treatment of lymphoid malignancies,particularly acute lym-phoblastic leukemia(ALL).However,the development of GC resistance is a serious clinical problem that complicates the ALL treatment and contributes to treatment failure.The molecular mechanisms of GC resistance are poorly understood and likely vary with disease type,treatment regimen,and particularly the genetic background of the patient.The finding that activation of mTOR pathway can induce GC resistance in lymphoid cells and that rapamycin,the mTOR inhibitor can sen-sitizes ALL cells to GC indicate that the combination of rapamycin and GC has potential utility in ALL treatment.GC sensi-tivity is a major prognostic factor in childhood ALL.The shortcoming of the so-defined prednisone is certainly that poor response in patients with low or no blasts at diagnosis is missed,thus it is necessary to look for in vitro assays to define accurately and specifically GC sensitivity.
