Clinical Results of Extracorporeal Membrane Oxygenation Support for Cardiopulmonary Failure

Abstract:OBJECTIVE To summarize the clinical experience of ECMO support for 8 cases of cardiopulmonary failure.METHODS The ECMO system was introduced for the treatment of 8 patients in which 4 cases were left ventricular failure following cardiac surgery with 1 for CABG and 3 for DVR;1 case was lung embolism during liver transplantation with V-V bypass;1 case was respiratory failure after kidney transplantation and other 2 cases were bronchoalveolar lavage for alveolar proteinosis.All ECMO systems consist of centrifugal pump and membrane oxygenator.The cannulation was accessed percutaneously in 6 patients and by cutting in 2 patients.Patients were cannulated via the left ventricular(right atrium) and aortic artery in 2 patients,3 patients were in veno-venous bypass and 3 patients were in veno-arterial bypass.RESULTS The duration of ECMO was 4~81 hours.Blood pressure was increased greatly and S_pO_2 was improved to 95% and more gradually following the start of ECMO.4 patients weaned successfully from ECMO and we were unable to wean the other 4 patients from ECMO because of heart arrest in 2 patients,persist bleeding in 1 patient and doubtful lung embolism in 1 patient.CONCLUSION ECMO is an effective device for cardiopulmonary failure.Although ECMO could improve the clinical results,selection of patients and suitable choice of devices is a more important consideration.
