Abstract:Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disease that encompasses a spectrum of disorders with diverse clinical presentations.It may involve a single organ or multiple organs including bone,skin,and lung.Adult LCH occurs more rarely compared to its younger counterpart.This article mainly reviewed the treatment of LCH that affected skin in adult patient,to provide reference for clinical diagnosis and treatment.

Abstract:OBJECTIVES:To observe the clinicopathologic features of Langerhans cell histiocytosis (LCH), and to evaluate the values of langerin, CD1a and S-100 protein expression in diagnosis of the tumor.METHODS:Total 258 cases of Langerhans cell histiocytosis in the past 18 years (from 1992 to 2008) were collected, morphologic review and immunohistochemical staining were performed.RESULTS:In all 258 cases, the ages of patients older than 16 years or younger than 2 years were 126 (48.8%) and 37 (14.3%), respectively, in the remaining 95 (36.8%) of the cases, the age of the patients ranged from 2 to 16 years. For all of 258 cases, there were 364 diseased sites. Bony lesions accounted for 77.2% (281 cases), especially the skull (112 cases, 39.9%), followed by lymph node (25 cases, 6.9%) and skin (14 cases, 3.8%). Clinically, unisystem or unifocal disease was predominant (201 cases, 77.9%), followed by unisystem and multifocal disease (21 cases, 8.1%), multi-system disease (26 cases, 10.1%), isolated pulmonary LCH (2 cases, 0.8%), and unclassified (8 cases, 3.1%). Histologically, variable number of Langerhans cells was present in 265 samples of 258 cases. Multinucleated giant cells were found in 166 (62.6%) of the samples. Eosinophils were the major infiltrating non-neoplastic cells, and eosinophilic abscess was seen in 57 cases (21.5%). Coagulative necrosis and dead bone were detected in 29 (10.9%) and 124 (46.8%) of the cases, respectively. Immunohistochemically, the expression of S-100 protein, CD1a and langerin was 99.1% (209/211), 100% (206/206) and 98.5% (193/196), respectively, and the sensitivity of them had no statistical difference.CONCLUSIONS:In this group of LCH cases, the ratio of adult patients is high, but the proportion of multi-organ lesion is low. No significant difference of the sensitivity is found among langerin, CD1a and S-100 expression in diagnosis of LCH.
Detection of Clonal Lymphoid Receptor Gene Rearrangements in Langerhans Cell Histiocytosis

Abstract:Langerhans cell histiocytosis (LCH), also known as histiocytosis X, is a rare human disorder characterized by an abnormal accumulation and/or clonal proliferation of Langerhans cells (LCs) in various body organs. The cellular origin of LCs has been a subject of considerable debate since their discovery. As specialized dendritic cells strategically located in epithelia, LCs are generally considered to be of myeloid origin from the bone marrow, however, recent studies in mice have shown that LCs can be derived from lymphoid-committed CD4 precursors, suggesting a lymphoid origin. In human LCH, concomitant or sequential occurrence of a lymphoid or myeloid malignancy has been occasionally reported, suggesting the presence of lineage plasticity and/or the possibility of transdifferentiation of 2 otherwise morphologically and immunophenotypically different neoplasms. To gain a better understanding of the pathogenesis and cellular origin of human LCH, we retrospectively investigated 46 well-characterized LCH cases to detect clonal rearrangements of T-cell receptor gamma gene (TRG@) and immunoglobulin heavy chain and kappa light chain genes (IGH@/IGK@). The study included 25 males and 21 females, with ages ranging from <1 to 59 years. None (0/46) of the cases had a known history or concurrent B or T-cell lymphoma. Of 46 cases, 30% (14/46) cases had clonal IGH@ (4 cases), IGK@ (5 cases) or TRG@ (9 cases) gene rearrangements, respectively. Interestingly, of the 14 cases with at least one clonal rearrangement of lymphoid receptor genes, 3 LCH cases were shown to have both TRG@ and IGH@/IGK@ gene rearrangements, but failed to express T-cell or B-cell lineage specific or associated markers, suggesting lineage plasticity or infidelity of the neoplasm. Furthermore, all of the 14 cases were negative for t(14;18) by quantitative PCR analysis. In conclusion, our study shows that lymphoid receptor gene rearrangements can be detected in a subset of sporadic LCH cases, suggesting a possible lineage relationship between LCs and lymphoid cells or alternatively, derivation of LCs from lymphoid/myeloid precursors. The results provide genotypic evidence supporting the current notion of lineage plasticity of hematopoietic cells and their associated neoplasms.
Analysis of Clinical Manifestations of 43 Children with Langerhans Cell Histiocytosis at Diagnosis

Abstract:Objective To analyze the clinical features of the disease by studying clinical manifestations of 43 children with newly diagnosed Langerhans cell histiocytosis(LCH).Methods The clinical manifestations and the laboratory data of the 43 LCH children diagnosed and treated in our hospital from October 2003 to January 2009 were collected and analyzed,the follow-up was done via phone calls.Results Thirty-one patients(72.09%) were younger than 2 years old at diagnosis with the most common complaint of superficial mass(51.16%,P 0.05);the blood system was the most frequently involved organ seen in 39(90.70%) patients at the first visit(P 0.01);11(25.58%) patients were found to be EBV IgG positive;CRP was elevated in 15 patients(34.88%);peripheral lymphocyte subpopulation was found to be abnormal in 9(20.93%) patients.Conclusions Langerhans cell histiocytosis can attack many organs of the human body,including reticuloendothelium tissue,and cause various clinical manifestations.Etiology and pathogenesis of the disease are still unknown.Thorough physical and accessory examination is vital in the diagnostic processure.

Abstract:Langerhans cell histiocytosis (LCH) is characterized pathologically by langerin-positive (CD207+) dendritic cell proliferation and is considered by some as a myeloid neoplastic disorder. Hemophagocytic lymphohistiocytosis (HLH) is associated with immune dysregulation characterized by the accumulation of activated macrophages and hypercytokinemia. However, these 2 histiocytosis rarely coexist. Currently, the etiology, risk factors, optimal therapy, and outcomes of LCH-HLH remain unclear. We reviewed the medical records of 7 LCH-HLH patients from our hospital and analyzed 50 LCH-HLH patients reported in scientific literature. The median age of LCH onset of these 57 LCH-HLH patients was 1 year, and 91% (52/57) of patients diagnosed as LCH were less than 2 years old. Fifty-six LCH-HLH patients belonged to the multisystem LCH category and 84% (47/56) patients had risk-organ involvement. Twenty-three LCH-HLH patients were complicated with infection and 3 patients had a primary pathogenic mutation of HLH. Overall, 90% of LCH patients developed HLH at the diagnosis or during chemotherapy. Of the 57 LCH-HLH patients, 15 died. Multisystem LCH patients with risk-organ involvement under 2 years old were most likely to develop HLH when complicated with infection at diagnosis or during chemotherapy. Identifying LCH-HLH patients during early stages and treating them with prompt chemotherapy, hematopoietic stem cell transplantation, or supportive therapies are important for better survival.

Abstract:Clinical categories of Langerhans cell histiocytosis (LCH) include single and multi-system disease. Pulmonary LCH is rare, which is an unusual interstitial lung disease with the characteristics of monoclonal proliferation and infiltration of Langerhans' cells to organs. We report our experience of a rare LCH case of multiple organs such as pulmonary and liver as the main clinical manifestation. The patient was treated with chemotherapy which included prednisone, vinblastine, methotrexate and 6-mercaptopurine for 52 weeks and follow up all along. The patient has a favorable clinical outcome.

Abstract:OBJECTIVE:To study the diagnosis and treatment for Langerhans cell histiocytosis (LCH) of head and face.METHOD:Clinical data of 19 cases suffering from LCH of head and face verified by pathology was retrospectively analyzed. The clinical presentation, diagnosis and treatment were discussed.RESULT:Of all the 19 cases, 5 patients had an involvement of the scalp or skull, 5 had the maxilla involvement and 9 temporal bone. Among the 19 patients, type I stage I was 8 cases(all adults), type II stage I 7 cases (all children), type ILL stage II 2 (1 adult, 1 child), stage III 1 and stage IV 1 (both child). Their main clinical manifestations included scalp mass, defect of skull, swelling or mass of facial maxilla and of ear or temporal bone, otorrhea, granuloma of auricular canal, and so on. Fifteen cases were cured by surgical curettage combined with radiotherapy and 4 by surgical curettage combined with chemotherapy. One patient had the sequelae of diabetes insipidus.CONCLUSION:Diagnosis of LCH should be based on synthetical analyses of clinical manifestations, features of imaging and histopathology. Surgical curettage, radiotherapy and chemotherapy are effective treatment regimens. The prognosis is generally good if the patient gets correct diagnosis and timely rational treatment.

Abstract:Langerhans' cell histiocytosis of the mandible is a rare disease in infant. This article reported a case of Langerhans' cell histiocytosis of mandible in a 2-year old female child. The clinical manifestation, radiographic characteristics, pathologic diagnosis, differential diagnosis and treatment were discussed. It is concluded that Langerhans' cell histiocytosis of the mandible in infant is rare without typical clinical manifestation and radiographic characteristics; final diagnosis is based on pathological examination with the characteristic hyperplasia of Langerhans' cell; immunohistochemistry examination of S-100 and CD1a are useful for diagnosis and differential diagnosis. Surgical resection with close postoperative follow-up is suggested as the main treatment for this disease.

Abstract:Objective:To analyze Langerhalls cell histiovytosis (LCH) clinical features for the sake of improving diagnosis level and decreasing misdiagnosis rate.Methods:Clinical data of 26 LCH patients was analyzed retraspectively. Results: The involved organs were bone (20 /26,76.92%),lymph nodes (2/26,7.69%),lung (2/26,7.69%),spleen (2/26,7.69%),ear (2/26,7.69%),musle (2/26,7.69%),liver (1/26,3.85%),stomach(1/26,3.85%),pituitary gland(1/26,3.85%),nasal organ(1/26,3.85%),cavitas oris(1/26,3.85%).Of the 26 patients,23 had 1 organ involved,1 had 2 organs involved,1 had 4 organs involved,1 had 6 organs involved.The clinical manifestation of Langerhans cell histiocytosis (LCH) is diversified,but it is short of specificity .Definitive diagnosis of LCH is based on pathology.Surgical operation or radiotherapy for unifocal lesions may improve the prognosis.Conclusion: The clinical manifestation of LCH varies widely,leading to high rate of misdiagnosis.Combined chemo-therapy for multisystem lesions,surgical operation or radiotherapy for unifocal lesions may improve the prognosis.
Langerhans Cell Histiocytosis: A Population-based Study of Anatomical Distribution and Treatment Patterns

Abstract:Background: Langerhans cell histiocytosis (LCH) is a rare monoclonal histiocytic neoplasm. Little is known about clinical factors associated with LCH single-vs multi-system involvement at the time of diagnosis.Methods: Data on 1549 LCH patients diagnosed between years 2010 and 2018 were extracted from the Sur-veillance, Epidemiology and End Results Program. Patterns of single-vs multisystem involvement were examined using multivariable logistic regression analysis. Odd ratio (OR) and 95% confidence interval (CI) were reported. Results: 968 children and adolescents (0-19 years; median: 4 years) and 581 adults (>= 20 years; median: 49 years) were included in the analysis. Multi-system LCH was reported for 30.9 % patients. Bone marrow (BM) (OR = 3.776; 95 %CI = 1.939-7.351; P < 0.001) and lymph node (LN) (OR = 3.274; 95 %CI = 1.443-7.427; P = 0.005) involvement were most commonly associated with multi-system LCH at the time of diagnosis; similar pattern was also observed in adult patients (OR = 17.780; 95 %CI = 6.469-48.867; P < 0.001 for BM LCH; and OR = 5.156;95 %CI = 2.131-12.471; P < 0.001 for LN LCH). Among pediatric patients, craniofacial osseous LCH was more likely to be treated with surgery (OR = 2.822; 95 %CI = 1.199-6.639; P = 0.018) compared to skeletal lesions in other sites, whereas vertebral body LCH was less likely to be treated with surgery (OR = 0.175; 95 %CI = 0.058-0.527; P = 0.002). In pediatric patients with bone LCH, the non-white patients were less likely to be treated surgically compared to the white patients (OR = 0.470; 95 %CI = 0.272-0.812; P = 0.007).Conclusions: BM and LN LCH are associated with the highest risks of multi-system disease, which may require active surveillance. Furthermore, active attempts are needed to mitigate the racial disparity in surgery utilization in pediatric patients with skeletal LCH.

Abstract:Background: The aims were to assess the clinical and histopathological characteristics of Langerhans cell histiocytosis (LCH) based on percutaneous liver biopsy and to improve the technique’s diagnostic accuracy. Methods: Data from seven patients were collected. The clinicopathological features and immunophenotypes of hepatic LCH in the biopsied tissues were evaluated . Results: Five men and two women (median age: 21 years) were included. The clinical presentations and imaging findings were unremarkable. In terms of the histological findings, multifocal and solitary lesions were detected in six patients and one patient, respectively. All lesions were located in the portal area and exhibited either enlarged or normal portal tracts. In four patients, Langerhans cells (LCs) were distributed either in clusters or scattered among mixed populations of inflammatory cells.LC invasion of the bile duct epithelium was detected in all but one patient; that individual presented with sclerosing cholangitis (SC) in the absence of LCs. One patient had a similar inflammatory myofibroblastic tumor (IMT) or inflammatory pseudotumor-like follicular dendritic cell (FDC) tumor. One patient had suppurative cholangitis with abscess formation, accompanied by a small degree of LC invasion into the bile duct epithelium. Conclusions: The morphology of liver LCHs varied among patients. Bile duct epithelial damage accompanied by eosinophilic infiltration, a history of extrahepatic LCH, or central diabetes insipidus are clues that LCH may be present; however, H&E staining and IHC are crucial for its diagnosis. LCH should be differentiated from hepatic parasitic infections, primary SC, inflammatory pseudotumor-like FDC tumors, and inflammatory myofibroblastoma.

Abstract:[Objective] To explore the clinic and laboratory characteristics of Langerhans cell histocytosis(LCH),so as to improve its diagnosis and treatment level. [Method] In a retrospective study from 1990 to 2004, 18 cases of LCH were reviewed related to the clinic manifestations,physical signs, laboratory, imaging and pathologic results as well as the diagnosis and treatment. [Result] Because of its various clinic features and laboratory results, LCH is easy to be misdiagnosed. [Conclusion] LCH is a disease wih a variety of presentations, and X-ray examination plays an important role in helping diagnose it. Definitive diagnosis depends on pathological biopsy. No specific method has been found to cure LCH.

Abstract:Langerhans cell histiocytosis (LCH) is a rare proliferative disorder in which pathological Langerhans cells accumulate in a variety of organs. LCH usually affects the bone, skin and lymph nodes of children; however, LCH occasionally affects vital organs, including the liver, spleen and pituitary gland. The present study reports a case of an adult LCH patient with marked liver damage, splenomegaly and pituitary damage treated using a new therapeutic strategy. This case was misdiagnosed as liver cancer and pituitary tumor on the basis of abdominal ultrasound, abdominal magnetic resonance imaging (MRI) and head MRI. The final diagnosis was established by identifying the proliferation of cluster of differentiation 1a-positive LCs in liver tissues. A new regimen of combined 12-week therapy of prednisolone/desmopressin/vincristine and 10 months of maintenance therapy of prednisolone/vinblastine/6-mercaptopurine improved symptoms, liver function and blood cell tests.

Abstract:Objective: To discuss the diagnosis and treatment of Langerhans Cell histiocytosis (LCH). Methods: Clinical analysis was performed in 24 cases with LCH.Results: Among all the cases, 16 cases were Lettere-Siwe disease (LS), 2 Hand-Schuller-Chistian syndrome (HSC), 4 eosinophilic granuloma of bone (EGB) and the other 2 difficult to be typed. Sixteen cases were diagnosed by clinical manifestations, X-ray and pathologic changes. All the patients were treated with radiotherapy and chemotherapy and 3 died, 5 gave up treatment at the beginning, 7 discharged with better condition but lost contact then, 4 left hospital with no improvement, only 5 held on to the end of the therapy period and the following-up showed no recurrence. Conclusion: Attention should be paid to clinical manifestations, especially the special skin rash, and the skin print should be done routinely. Fully understanding of the typical X-ray changes will do good to early diagnosis. Early combined fulldose chemotherapy, radiotherapy and operation treatment can improve the prognosis. Chemotherapy composed of Vincristin, Etoposide and Predison (VEP) is effective and has light side-effects.
[A Clinicopathological Analysis of 160 Cases of Adult Langerhans Cell Histiocytosis].

Abstract:OBJECTIVE:To observe the clinical features of Langerhans cell histiocytosis (LCH), and to improve its early diagnosis and treatment. METHODS:Retrospective analysis of 160 cases of adult LCH from pathology department, West China Hospital of Sichuan University and Union Hospital of Tongji Medical College of Huazhong University of Science and Technology from January 1992 to December 2013 were performed, and their clinical features were analyzed. RESULTS:Of 160 cases, there were 110 male and 50 female, the male to female ratio was 2.2:1. The mean age was 35(18-73) years. There were total 222 lesion sites, including 172(77.5%) osteal lesions, followed by 13(5.8%) lymph nodes and 8(3.6%) oral cavity lesions. The other involved organs were skin(5, 2.2%), liver(5, 2.2%), fossa orbitalis(4, 1.8%), lungs(4, 1.8%), sternoclavicular joint(3, 1.4%), gastrointestinal(2, 0.9%), ear(2, 0.9%), and thyroid (2, 0.9%), adrenal gland (1, 0.5%) and sublingual gland (1, 0.5%). Of 160 cases, 150 (93.8%) had one organ involved while 10 (6.2%) had two or more organs involved. Clinically, 77 cases (48.1%) were misdiagnosed as bone tumors (28 cases, including giant cell tumor, fibrous dysplasia, chondroblastoma, osteoblastoma and osteosarcoma), bone tuberculosis (13 cases), meningioma(9 cases), bone cysts (5 cases), chronic osteomyelitis (5 cases) and diabetes insipidus (5 cases) , skin (4 cases) diseases malignant lymphoma (4 cases), chronic skin ulcers (4 cases), chronic otitis media (1 case), lung (1 case) and oral cancer (1 case). CONCLUSION:In this group of the adult cases, the ratio of the male patients is higher. Adult LCH occurs predominantly in bone and presents mainly as unisystem single-focal disease, but multi-organ lesion and skin involvement are lower than that reported in the literatures. Just as LCH in children, adult LCH is also easy to be misdiagnosed. We should raise awareness of the disease and pathological examination is helpful for early diagnosis.

Abstract:Rationale: Langerhans cell histiocytosis (LCH) is a kind of rare disease in which dendritic cells proliferate abnormally. It often occurs in children and can involve any tissue and organ. The affected sites usually include bone, skin, pituitary gland, and lungs, while the thyroid gland and external auditory canal are rarely observed. The perineal and labial involvement of this disease has not been reported yet. Patient concerns: A 47-year-old female patient experienced a swelling of the anterior neck area without an obvious inducement. She noticed a quail egg-like mass on the left side, and the mass increased progressively within 3 months. The anterior neck area was found to be swollen, and some flaky red rashes were seen on the scalp and bilateral external auditory canals. Diagnoses: Imaging examination showed enlarged thyroid and cervical lymph nodes, multiple low-density nodules in the liver, and reduced signal in the posterior pituitary gland. The biopsy pathological result of the increased left cervical lymph node indicated that LCH was detected. Interventions: VP regimen (vincristine, dexamethasone per os) and related supportive treatments were given as inducing chemotherapy for 6 weeks. Outcomes: After the second chemotherapy, the rash on the scalp and external auditory canal improved, and the neck mass was significantly reduced. After the third chemotherapy, the rash was mostly disappeared, while the neck lumps increased during chemotherapy. Thus, clatribine chemotherapy was recommended as the follow-up. Lessons: Imaging examinations played an important role in the diagnosis and follow-up of the disease, especially 18F-FDG PET/CT, which could show multiple involving organs at the same time. When a patient suffering from diabetes insipidus, skin rash, or fever, has a high FDG uptake PET/CT result in multiple tissues and organs throughout the body, it is necessary to consider the possibility of LCH.
Isolated Langerhans Cell Histiocytosis of the Spleen A Case Report

Abstract:RATIONALE:Langerhans cell histiocytosis (LCH) is a relatively rare disorder characterized by the proliferation of abnormal Langerhans cells. Splenic involvement usually presents in children with multisystem LCH, and isolated LCH of the spleen is a very rare disease. Herein, we report a case of isolated splenic LCH in an adult man.PATIENT CONCERNS:We report a case of isolated splenic LCH that first manifested with recurrent left upper abdominal pain.DIAGNOSIS:Unenhanced CT revealed multiple nodular lesions located at the spleen. Magnetic resonance imaging speculated that these lesions were lymphomas. On the basis of histopathological and immunohistochemical findings, the diagnosis of isolated splenic LCH was confirmed.INTERVENTION AND OUTCOMES:The patient underwent splenectomy. Histopathologic examination revealed the proliferation of Langerhans cell. Immunohistochemical staining revealed that cells of the tumor were positive for S-100, CD1a, CD45RO, and Vimentin. The patient is alive without recurrence 9 years after operation.LESSONS:Isolated LCH of the spleen may have a favorable prognosis and splenectomy is an effective therapeutic method.

Abstract:Langerhans cell histiocytosis (LCH) is an inflammatory myeloid malignancy in the "L-group" histiocytosis. Mitogen-activated protein kinase (MAPK) pathway activating mutations are detectable in nearly all LCH lesions. However, the pathogenic roles of MAPK pathway activation in the development of histiocytosis are still elusive. This review will summarize research concerning the landscape and pathogenic roles of MAPK pathway mutations and related treatment opportunities in Langerhans cell histiocytosis.

Abstract:Objective To explore the clinical and pathologic features of Langerhans cell histiocytosis(LCH).Method Thirty-eight cases of LCH from both in hospital and outpatient department in dermatology and pediatry department from Jan.1991 to Jun.2006 were analyzed about the features of clinic,pathology and immunohistochemistry.Results The mean age of onset was 2.18 years old.The male /female ratio was 1.71.Skin lesions occurred in 78.9% of the patients.Among them,68.5% were as the first manifestation.The eruptions mainly distributed on trunk,90% of them presented as hemorrhagic maculopapules,nevertheless,3.3% of the eruptions showed as crust with hilar depression,which was similar to pityriasis lichenoides etvarioliformis acuta.Fever,hepatomegalia and splenomegia occurred in patients at a rate of 60.5%,68.4%,55.3% respectively.Thirty-one point six percent of the patients had got lymphadenectasis,the neck and inguinal lymph nodes were the common site to be affected.Ossature involvement occurred in 31.6% of the patients,which 8.3% got multiple injuries,howe-ver,91.7% got a solitary bone involved.Skull was the main site to be injured,else were lumbar,humerus,hipbone,an so on.Respiratory tract,auditory canal,mucosa were also the sites involved in this disease,but the incidence rate was lower than 10%,respectively.The laboratory data showed that 81.3% of the patients were anaemia,60.7% with abnormal subgroup of T-cell,and 32.1% positive for EBV-IgG.The skin histopathology data of 26/30 cases revealed that lichenoid infiltrates of Langerhans cells confined to the upper dermis.Cytologic features were cells with abundant eosinophilic cytoplasms,and longitudinally grooved or reniform nuclei.Lymphocytes and a few eosinophils also could be seen.Four cases of thirty showed that the proliferative Langerhans cells were with pale cytoplasms,besides,there were numerous eosinophils,and sometimes a few multinucleate cells were scattered.The immunity test of 20 cases of thirty displayed that CD1a(+)S100(+)KP-1(-).Biopsy of lymphaden and tumor of the skull of the rest 8 patients were all diagnosed as eosinophilic granuloma through both hematoxylin and eosin-stained section and immune marks.Conclusions Multiple systems can be involved in LCH.Hemorrhagic maculopapules,fever and splenohepatomegalia are common presentations in this disease.The morphous of nucles of histiocytes is particular,and to diagnose definitely,both CD1a(+) and S-100(+) are needed.
