Abstract:Although breast conserving therapy and sentinel lymph node biopsy are widely used, complete mastectomy combined with axillary lymph node dissection, which was defined as modified radical mastectomy, still remains a viable option for patients with breast cancer and positive axillary lymph node.As a relatively more aggressive technique, modified radical mastectomy is associated with more adverse effect and worse cosmetic outcome. This topic is to address the evolving history, indications and surgical standardization of modified radical mastectomy.
Vascular anatomy and evaluation of the nipple-areola complex in nipple-sparing mastectomies

Abstract:Nipple-areola complex (NAC)-sparing mastectomies (NSM) have gained acceptance in the ifeld of breast oncology. The superior aesthetic outcomes of NSM explain their increased use and rising popularity. Technical considerations and challenges of this procedure are centered on nipple ischemia and necrosis. Patient selection, reconstructive strategies and incision placement have lowered ischemic complication rates. An understanding of the NAC vascular anatomy is, therefore, clinically relevant beyond NSM. In this paper, the relevant clinical anatomy is described, mainly focusing on the anatomy of the NAC. This article also covers how to identify and classify the in vivo blood supply to the NAC using breast MRI exams which provide valuable information for assessing vascular anatomy of the NAC. This includes the arterial iflling phase, venous drainage phase and 3-dimensional reconstructed maximum intensity projection (MIP) images. Finally, the indocyanine green (ICG) and a specialized infrared camera-computer system provide surgeons with a practical tool to assess real-time breast skin and NAC perfusion. Intraoperative evaluation of skin perfusion allows surgeons to detect ischemia and modify the operative approach to reduce the risk of nipple ischemia and necrosis.

Abstract:Objective: To evaluate the oncologic safety of nipple-sparing mastectomy (NSM) for breast cancer patients based on current literature. Data Sources: A comprehensive literature search of Medline, Embase databases was conducted for studies published through March 2014. Study Selection: Our search criteria included English-language studies that focused on NSM at nipple-areola complex (NAC) involvement, patient selection, and recurrence. Prophylaxis NSM, case series or reports that based on very small population were excluded. In the end, 42 studies concerning NSM and oncological safety were included into the review. Results: NSM is a surgical procedure that allows the preservation of the skin and NAC in breast cancer patients or in patients with prophylactic mastectomy. However, the oncologic safety and patient selection criteria associated with NSM are still under debate. The incidence of NAC involvement of breast cancer in recent studies ranges from 9.5% to 24.6%, which can be decreased through careful patient selection. Tumour-nipple distance, tumour size, lymph node involvement and molecular characteristics can be evaluated preoperatively by clinical examinations, imaging studies and biopsies to predict the risk of NAC involvement. Currently, there is no available standard protocol for surgical approaches to NSM or pathological examination of NSM specimens. The local recurrence (ranges from 0% to 24%) of NSM is not significantly higher than that of traditional mastectomy in selected patients based on long-term follow-up. The role of radiotherapy in NSM is still controversial and is not universally accepted. Conclusions: NSM appears to be oncologically safe following careful patient selection and assessment of margins.
[Improvement of Wound Treatment in Modified Radical Mastectomy of Breast Cancer].

Abstract:OBJECTIVE:To evaluate the efficacy of Dexon continuous intracutaneous sewing (DCIS) and Redon negative pressure absorbing drainage (RNPAD) in modified radical mastectomy of breast cancer.METHODS:The clinical data of 128 patients treated with DCIS and RNPAD, and 123 cases treated with traditional sewing and negative pressure absorbing ball were analyzed retrospectively.RESULTS:There were 16 cases (12.5%) of seroma formation,1 (0.8%) of skin flay necrosis among 128 cases treated by DCIS and RNPAD, and 47 cases (38%) of seroma formation, 16 (13%) of skin flay necrosis among 123 cases treated by traditional method. The statistically significant difference was found (P <0.001). The average length of post-operation hospital stay in DCIS and RNPAD group was 8.8 days compared with 14.7 days in traditional treatment group.CONCLUSION:The use of DCIS and TNPAD could decrease the complication rate of breast cancer operation.
Investigation of the current situation of nipple-sparing mastectomy: a large multicenter study in China (CSBrs-003)

Abstract:BACKGROUND: Mastectomy techniques have been extended to nipple-sparing mastectomy (NSM). This study aimed to assess the actual application of NSM in China and identify the factors influencing postoperative complications.METHODS: The clinical data of 615 patients (641 surgeries) undergoing NSM from January 1st, 2018 to December 31st, 2018 at 28 centers nationwide were retrospectively analyzed to obtain the rate of NSM and investigate factors related to NSM surgery.RESULTS: The proportion of NSM surgery performed in this study was 2.67% (17/641). Malignant breast tumors accounted for the majority of NSM surgery (559/641, 87.2%). A total of 475 (77.3%) patients underwent NSM combined with reconstructive surgery. The rate of reconstruction decreased with age in our study, and implants were the most common option (344/641, 53.7%) in reconstruction. Radial incision was the most selected method regardless of reconstruction. However, for those who underwent reconstruction surgery, 18.4% (85/462) of cases also chose curvilinear incision, while in the simple NSM surgery group, more patients chose circumareolar incision (26/136, 19.1%). The tumor-to-nipple distance (TND) influenced postoperative complications (P = 0.004). There were no relationships between postoperative complications and tumor size, tumor location, histologic grade, molecular subtype, nipple discharge, and axillary lymph nodes.CONCLUSIONS: NSM surgery is feasible and only TND influenced postoperative complications of NSM surgery. But the proportion of NSM surgery performed is still low in nationwide centers of China. The selection criteria for appropriate surgical methods are important for NSM in clinical practice. Perioperative care of nipple-areola complex-sparing mastectomy and one-stage breast reconstruction via endoscopic axillary approach for ductal carcinoma in situ: A case report

Abstract:Rationale: Breast cancer represents a prevalent malignancy that primarily impacts women, with pronounced consequences on their overarching health. The major therapeutic approach, encompassing surgical procedures, can often culminate in mastectomy, potentially inciting psychological turmoil and disorders. Patient concerns: A patient was admitted to our facility on May 5, 2023, precipitated by the discovery of bilateral breast masses during a routine physical examination conducted 3 days before admission. Diagnosis: The breasts were symmetric, with the right nipple inverted and a palpable mass in the upper outer quadrant of the right breast, measuring approximately 5 cm × 4 cm. The mass was firm with indistinct borders, relatively regular morphology, poor mobility, and no tenderness. Outpatient color Doppler ultrasound revealed heterogeneous echogenicity in the right breast, classified as Breast Imaging Reporting and Data System (BI-RADS) category 0, along with multiple ductal dilatations. The left breast exhibited a hypoechoic area (BI-RADS 3), indicative of proliferative changes. Radiographic mammography confirmed diffuse changes in the right breast (BI-RADS 0) and proliferative signs in the left breast (BI-RADS 2). Biopsy results reveal significant atypical ductal hyperplasia consistent with intermediate-grade ductal carcinoma in situ. This patient was diagnosed as ductal carcinoma in situ of the right breast (cTisN0M0 and Stage 0), accompanied by a left breast mass. Interventions: On May 15, 2023, the patient was readmitted for further surgical intervention. Following relevant auxiliary examinations, the patient underwent nipple-areola complex-sparing radical mastectomy for the right breast, sentinel lymph node biopsy in the right axillary area, prosthesis-based breast reconstruction for the right breast, and microrotatotomy of the left breast mass on the left side on May 17. Outcomes: The patient made a successful recovery under scrupulous perioperative supervision and was discharged 7 days post-surgery. Lessons: The axillary approach for endoscopic mammary gland excision and immediate implant reconstruction permits patients to preserve the esthetics of the female form while undergoing conventional medical treatment. This methodology considerably enhances the psychophysical health of the patients, thereby marking it as an advantageous practice worthy of broad dissemination in the medical community.
Safety and Feasibility of Minimally Invasive (Laparoscopic/Robotic-Assisted) Nipple-Sparing Mastectomy Combined with Prosthesis Breast Reconstruction in Breast Cancer: A Single-Center Retrospective Study

Abstract:BackgroundMinimally invasive (robotic or laparoscopic-assisted) nipple-sparing mastectomy combined with prosthesis breast reconstruction (NSM–PBR) is associated with smaller scars and greater patient satisfaction. However, the oncological safety of minimally invasive NSM–PBR remains controversial.Patients and MethodsThis was a retrospective study of patients with breast cancer who underwent breast reconstruction between 1 January 2006 and 20 February 2021. Demographic and clinicopathological characteristics, operation information, postoperative complications, and survival outcomes were analyzed.ResultsIn all, 292 patients underwent minimally invasive NSM–PBR and 205 underwent open NSM–PBR for breast cancer. In the minimally invasive NSM–PBR group, 268 (91.8%) patients underwent laparoscopy and 24 (8.2%) patients underwent robot-assisted NSM–PBR. Mean operation time in the minimally invasive NSM–PBR group was significantly longer than that in the open NSM–PBR group (P = 0.023). Mean intraoperative blood loss was significantly less in the minimally invasive NSM–PBR group (P < 0.05). There was no significant between-group difference in total complications. Similarly, there were no significant between-group differences in overall survival, recurrence-free survival, and local recurrence rate (P = 0.450, P = 0.613, and P = 0.679, respectively).ConclusionsThe complication, recurrence, and mortality rates in minimally invasive NSM–PBR group were comparable to those in open NSM–PBR group. Our preliminary results are encouraging and suggest that minimally invasive NSM–PBR affords good cosmetic results and its oncological safety is comparable to that of open surgery.

Abstract:Background: Nipple-sparing mastectomy (NSM) and skin-sparing mastectomy (SSM) are challenging for surgical training among fellow trainees. We developed a surgical training course with novel concept of breast modular resection (BMR) for NSM/SSM procedure, and performed this study to investigate whether BMR could improve surgical outcomes compared to classical procedure resection (CPR). Methods: The records of 105 breast cancer patients undergoing NSM/SSM with immediate reconstruction performed by fellow trainees were reviewed. Clinicopathological characteristics and surgical outcomes were compared between 2 groups. Laser speckle contrast imaging (LSCI) was performed to intraoperatively evaluate the blood supply of the NAC, and the absolute perfusion unit (PU) values and relative perfusion unit (rPU) values were further compared. Results: Surgical training outcomes of BMR group (N = 52) were insignificantly improved compared to CPR group (N = 53). The rates of NAC necrosis, flap necrosis and implant removal all reduced respectively. Among the 60 NSM patients, the blood loss (P = .011) and surgery time (P < .001) was significantly reduced in BMR group (N = 30) and all the other outcomes were insignificantly improved. Both the absolute PU values and rPU values were significantly higher among patients without NAC necrosis (P < .001). The absolute PU values were significantly higher in BMR group (P = .002). Conclusion: Compared to CPR, the BMR-based surgical training course for NSM demonstrated the reduction in complications and operating time, offering a potential streamlined, efficient, and safe method for NSM procedure. LSCI was effective for intraoperative visualized evaluation of NAC blood supply and could provide effective real-time feedback for fellow trainees.

Abstract:Purpose: Breast-conserving surgery (BCS) plus radiotherapy and mastectomy exhibit highly comparable prognoses for early-stage breast cancer; however, the safety of BCS for T1-2N3M0 breast cancer remains unclear. This study compared long-term survival for BCS versus (vs.) modified radical mastectomy (MRM) among patients with T1-2N3M0 breast cancer. Methods: Data of patients with T1-2N3M0 breast cancer were extracted from the Surveillance, Epidemiology, and End Results database. Eligible patients were divided into 2 groups, BCS and MRM; Pearson's chi-squared test was used to estimate differences in clinicopathological features. Propensity score matching (PSM) was used to balance baseline characteristics. Univariate and multivariate analyses were performed to investigate the effects of surgical methods and other factors on breast cancer-specific survival (BCSS) and overall survival (OS). Results: In total, 2124 patients were included; after PSM, 596 patients were allocated to each group. BCS exhibited the same 5-year BCSS (77.9% vs. 77.7%; P = 0.814) and OS (76.1% vs. 74.6%; P = 0.862) as MRM in the matched cohorts. Multivariate survival analysis revealed that BCS had the same BCSS and OS as MRM (hazard ratios [HR] 0.899 [95% confidence intervals (CI) 0.697-1.160], P = 0.413 and HR 0.858 [95% CI 0.675-1.089], P = 0.208, respectively); this was also seen in most subgroups. BCS demonstrated better BCSS (HR 0.558 [95% CI 0.335-0.929]; P = 0.025) and OS (HR 0.605 [95% CI 0.377-0.972]; P = 0.038) than MRM in those with the triple-negative subtype. Conclusions: BCS has the same long-term survival as MRM in T1-2N3M0 breast cancer and may be a better choice for triple-negative breast cancer.
Neoadjuvant chemotherapy for primary invasive ductal carcinoma of the nipple: A case report

Abstract:Breast cancer typically arises from the terminal duct-lobular unit of the mammary gland and rarely from the ducts inside the nipple. The present paper reports a rare case of primary invasive ductal carcinoma of the papilla, which was a locally advanced triple-negative breast cancer that was treated with 6 cycles of neoadjuvant chemotherapy with a nab-paclitaxel, epirubicin and cyclophosphamide regimen. Surgical pathology confirmed that a pathological complete response was achieved and adjuvant radiotherapy was performed postoperatively. No recurrence or metastasis occurred as of April 2024. A review of previous similar cases revealed that primary invasive breast cancer of the nipple has several manifestations. Changes in the nipple should be treated cautiously and a pathological biopsy should be performed in a timely manner. Breast cancer occurring in the nipple can be treated with reference to the same type of common breast cancer, and neoadjuvant chemotherapy can also be performed first if neoadjuvant chemotherapy is indicated.

Abstract:Endoscopic-assisted or robotic nipple-sparing mastectomy (E-NSM or R-NSM) with direct-to-implant breast reconstruction is becoming an increasingly popular surgical treatment for breast cancer patients. However, existing surgical methods such as traditional two-dimensional endoscopic-assisted or robot-assisted surgery are time-consuming and expensive. A new innovative three-dimensional videoscope system is efficient but needs special instruments; additionally, the incision over the extramammary region near the anterior axillary line is still too invisible. This study took place from May 2020 to April 2021. This technique is suitable for patients with small- to moderate-sized breasts (C cup size and below; grade 0 to grade I ptotic breasts). We have now performed 30 procedures, including eight bilateral reconstructions. The average operative time (including axillary operation) of a unilateral reconstruction is 179 ± 40.96 minutes, and for bilateral reconstruction, it was 271 ± 45.29 minutes. The median follow-up time was 9.93 months. For the complications, only one patient presented with a mild depigmentation of the nipple and one patient presented with local cellulitis but recovered after oral administration of antibiotics. All patients were satisfied with their postoperative aesthetic outcome. We present a new surgical technique of E-NSM with direct-to-implant subpectoral breast reconstruction to achieve better results.
Nipple-sparing mastectomy: initial experience at the Memorial Sloan-Kettering Cancer Center and a comprehensive review of literature

Abstract:Success achieved with skin-sparing mastectomy has led surgeons to reconsider the necessity of nipple-areola complex removal. This study reports our short- and mid-term postoperative outcomes with nipple-sparing mastectomy (NSM) and an updated review of reported literature. Data were retrospectively abstracted from medical records at our institution. Patients underwent NSM based on patient preference, oncologic criteria, and cosmesis. A literature review was undertaken through a PUBMED search and selected based on title and abstract relevance. Twenty-five patients underwent 42 NSMs at our institution from July 2000 to October 2005. Patient mean age was 44 years (29-59 years). Indications for mastectomy were: 34 (81%) for prophylaxis, 5 (12%) for invasive ductal carcinoma, 2 (5%) for ductal carcinoma in situ, and 1 (2%) for a malignant phyllodes tumor. One prophylactic mastectomy specimen showed ductal carcinoma in situ in the retroareolar tissue, and the nipple-areola complex was removed at a second operation. Mean tumor size in cases with invasive carcinoma (n = 5) was 1.9 cm (0.7-2.5 cm). All tumors were peripherally located, and no cases showed occult nipple involvement. The nipple-areola complex was entirely preserved in 39 (93%) mastectomies. One nipple-areola complex was surgically removed, and two (5%) cases had partial loss due to infection or ischemia. Cosmetic result from surgeon's assessment was excellent in 30 mastectomies, good in 7, acceptable in 3, and poor in 2, with slight nipple asymmetry in 8 cases. At a median follow-up of 10.5 months (range, 0.4-56.4 months), the 39 nipple-areola complexes were intact and there were no local or systemic recurrences in cases treated for cancer. NSM represented approximately 1% of all mastectomies performed at our institution during the reported period. It was mostly used for prophylaxis and for the treatment of malignant tumors in few selected cases. NSM can be performed with a high success rate of nipple-areola complex preservation. Conclusions about the oncologic safety of this procedure cannot be drawn from our study due to small size series and short follow-up. However, available published data show that NSM can be safely performed for breast cancer treatment in carefully selected cases. Further studies and longer follow-up are necessary to refine selection criteria for NSM.

Abstract:BACKGROUND:This study was conducted to describe present and changing trends in surgical modalities and neoadjuvant chemotherapy (NACT) in female breast cancer patients in China from 2006 to 2015. METHODS:Data of 44 299 female breast cancer patients from 15 tertiary hospitals in Beijing were extracted from hospitalization summary reports. Surgeries were categorized into five modalities: breast-conserving surgery (BCS), simple mastectomy (SM), modified radical mastectomy (MRM), radical mastectomy (RM), and extensive radical mastectomy (ERM). RESULTS:In total, 38 471 (86.84%) breast cancer patients underwent surgery: 22.64% BCS, 8.22% SM, 63.97% MRM, 4.24% RM, and 0.93% ERM. Older patients (> 60) underwent surgery more frequently than younger patients (< 60). The proportion of patients who underwent BCS was highest in the age ≥ 80 (39.24%) and < 40 (28.69%) subgroups and in patients with papillary carcinoma (35.48%), and lowest in the age 60- subgroup (18.17%) and in patients with Paget's disease (19.05%). SM was most frequently performed in patients with Paget's disease (29.00%), and MRM for ductal (64.99%), and lobular (63.78%) carcinomas. During the study period, the proportion of patients who underwent MRM dropped by 29.04%, SM and BCS increased from 15.78% and 30.83%, respectively, and NACT increased in all subgroups, particularly in patients with lymph node involvement (26.72%). CONCLUSIONS:Surgical modalities varied significantly by age and histologic group. The use of BCS and SM increased dramatically, while MRM declined significantly. The proportion of patients treated with NACT has increased significantly, especially in patients with lymph node involvement.
Breast-conserving Therapy and Modified Radical Mastectomy for Primary Breast Carcinoma: a Matched Comparative Study.

Abstract:BACKGROUND:To compare two types of therapy for primary breast carcinoma, breast-conserving therapy (BCT) and modified radical mastectomy (MRM), in a matched cohort study.METHODS:A series of 1,746 patients with primary breast cancer treated with BCT or MRM in a single Chinese institute between January 2000 and February 2009 were analyzed retrospectively to compare their outcomes with respect to the incidence of local recurrence (LR), distant metastasis, and survival. The patients were matched with regard to age at diagnosis, spreading to axillary lymph nodes, hormone receptor status, the use of neoadjuvant chemotherapy and maximal tumor diameter. The match ratio was 1:1, and each arm included 873 patients.RESULTS:The median follow-up period was 71 months. The 6-year disease-free survival (DFS) and 6-year distant disease-free survival (DDFS) rates differed significantly between two groups. The 6-year local recurrence-free survival (LRFS) rates were 98.2% [95% confidence interval (CI): 0.973-0.989] in the BCT group and 98.7% (95% CI: 0.980-0.994) in the MRM group (P=0.182), respectively. DFS rates in BCT and MRM groups were 91.3% (95% CI: 0.894-0.932) and 86.3% (95% CI: 0.840-0.886) (P<0.001), respectively, whereas the DDFS rates in BCT and MRM groups were 93.6% (95% CI: 0.922-0.950) and 87.7% (95% CI: 0.854-0.900) (P<0.001), respectively.CONCLUSIONS:BCT in eligible patients is as effective as MRM with respect to local tumor control, DFS and DDFS, and may result in a better outcome than MRM in Chinese primary breast cancer patients.
Retrospective Analysis: Preserving More Subcutaneous Tissue Shows no Increase the Risk of Breast Cancer Recurrence

Abstract:Background: Radical mastectomy remains the cornerstone procedure for the treatment of breast cancer (BC). However, traditional radical surgeries often lead to complications such as local numbness, pulling sensations, and atrophy of the pectoralis major muscle. In contrast, BC radical surgeries that preserve more tissue have shown potential in reducing these complications. This retrospective study aims to analyze case data from our institution, focusing on the methods of surgeries that preserve more tissue and evaluating the safety and reliability of the follow-up results. Methods: A retrospective observational study was conducted on cases diagnosed with BC between May 2018 and July 2019 at our institution. The cases were divided into three different surgical groups and followed up for a period of 5 years. The follow-up results were then discussed within each group. Results: A total of 315 cases diagnosed with BC underwent regular follow-ups. The statistical analysis revealed an average age of 45 years and an average tumor size slightly over 2.2 cm, with early-stage BC (Stage I and II) accounting for 90.2% of the cases. The overall survival (OS) and disease-free survival times in the group undergoing total mastectomy with tissue preservation were comparable to those in the traditional radical mastectomy group and the breast-conserving plus radiotherapy group. Moreover, the complication rate, particularly the incidence of chest wall numbness and pulling sensations, was lower in the total mastectomy with tissue preservation group compared to the traditional radical mastectomy group. The overall average follow-up time was 64.4 months, with a recurrence and metastasis rate of 15.6% and an OS rate of 92.7%. Conclusion: Based on our follow-up results, total mastectomy with more tissue preservation demonstrates comparable efficacy to breast-conserving surgery and traditional radical mastectomy. It can reduce some complications associated with traditional radical mastectomy and is beneficial for subsequent immediate and delayed breast reconstruction. This approach may be suitable for most patients with early to mid-stage breast cancer who do not wish to undergo breast-conserving surgery.

Abstract:Abstract Objective: With the development of the diagnosis and treatment of breast cancer, the pattern of multidisciplinary treatment has resulted a new tendency of breast conserving surgery and reconstruction. Nipple sparing mastectomy (NSM) preserves the aesthetically important nipple-areolar complex, improved appearance satisfaction and quality of life of patients after mastectomy, and had similar oncologic outcomes compared with traditional mastectomy techniques. The primary guiding principles of NSM are oncologic safety. Some studies have demonstrated its oncologic safety while others have demonstrated recurrent risk increasing because of the nipple-areolar complex. We sought to assess the overall survival and disease-free survival of NSM compared with breast-conserving surgery (BCS) and total mastectomy (TM). Key words: nipple sparing mastectomy, breast-conserving surgery, total mastectomy, the overall survival, the disease-free survival Methods: This retrospective matched-cohort study was conducted patients accepted breast surgery between January 2007 and December 2017 in the Department of Breast Surgery of Fudan University Shanghai Cancer Center. Cancer stages were identified depending on the American Joint Committee on Cancer [AJCC] TNM staging system. The inclusion criteria included (1) female breast cancer, (2) histopathology confirmed invasive breast cancer without distant metastasis or local relapses, (3) had complete clinical and pathological T&N stages. The exclusion criteria included in situ carcinoma, phylloides tumor, recurrent or metastatic breast cancer and pregnancy. The primary outcome was the overall survival (OS), the secondary outcome was the disease-free survival (DFS). Statistical methods included Kaplan-Meier survival analysis and COX regression analysis, all of which were completed by SPSS 20.0. Results: A total of 2757 patients were evaluated for eligibility, of which 430 were included (86 NSM vs 172 BCS vs 172 TM), the median follow-up is 61.57 months. Clinical T&N stage and other factors of the above three operations were balanced and comparable after matching. COX regression analysis reported that pT (pT3 vs pT1, P=0.00, HR 16.051, 95%CI:3.822-67.409) and pN (pN2 vs pN0, P=0.00, HR 16.626, 95%CI:4.581-60.337) were related factors for OS, and pT (pT3 vs pT1, P=0.021, HR 6.619, 95%CI:1.331-32.927) and pN (pN2 vs pN0, P=0.000; HR 13.320, 95%CI:3.250-54.591) are independent predictive factor of OS. In addition, pN (pN2 vs pN0, P=0.013; HR 3.448, 95%CI:1.304-9.118) was a related factor for DFS. Among 94.19% patients of the NSM group underwent breast reconstruction, 70 underwent implant-based breast reconstruction, 8 underwent autogenous flaps reconstruction, and 3 patients chose latissimus dorsi flap combined with prosthesis immediate breast reconstruction. Only 4 of 172 patients underwent autogenous flaps reconstruction in TM, and 1 chose implant-based breast reconstruction. Kaplan-Meier survival analysis found that the 5-year OS of NSM group, TM group and BCS group were respectively 100%, 95.8% and 99.2%. The 5-year DFS of three operations were respectively 95.2%, 92.1%, 93.5%. Patients who underwent NSM had no difference compared with BCS or TM regarding the OS (P=0.875, HR 0.938, 95%CI:0.421-2.090) and the DFS (P=0.882, HR 0.967, 95%CI:0.623-1.502). Conclusions: Patients undergoing NSM are not disadvantaged in terms of the OS and the DFS compared with patients who underwent BCS or TM. Patients with NSM tend to opt for breast reconstruction to keep good shape and it has the same prognostic outcome as patients with TM or BCS. Clinical Application of Surround-mammary Areola Incision in Surgeries of Mammary Fibroma

Abstract:Objective To discuss the feasibility of surround-mammary areola in surgeries of mammary fibroma.Methods 132 patients were divided randomly into two groups.The experimental group(67cases)use of central areolar incision therapy and the control one(65cases)use traditional radial incision treatment.Accomplish the regular visits by insisting the telephone consultation,the physical reexamination and the Bultrasonic reexamination.Estimate the effect of operation by observing the healing class of incision,double measured breast symmetrical situation,the feeling of nipple and areola,the cicatrization situation and the recurs situation.Results The experimental group compared with the control group,the basic symmetry of bilateral breast were 66 cases to 55 and the incision scar acceptable cases were 66 to 54(P0.01).Conclusion Surround-mammary areola incision is a safe and cosmetic procedure in treating mammary fibroma and easy to be handle and generalize in clinic.

Abstract:Objective To study the effect and safety of nipple-sparing mastectomy plus immediate reconstruction with implants( NSM-IBRWI) in early breast cancer. Methods From June 1999 through October 2006 ,14 breasts of 13 patients were treated with NSM-IBRWI. 180°arc incision along the supra-or infra-areola was used,the breast was completely resected,then silicone gel prosthesis was implanted immediately,meanwhile axillary lymph node dissection was performed. Results In the 14 breasts ,the successful rate of the operation was 92.9%. A severe complication occurred in 1 (7.1%) patient. The drainage liquid could not be reduced 2 months after implantation of the prosthesis,which resulted in nonunion of the wound,so the implant had to be removed. Two (14.3%) cases presented with ischemia in the nipple-areola complex,after conservative treatment they were both healed. Six months after operation the self-satisfaction was good in 9 cases,median in 3 (asymmetry in 2 and pain in 1),and poor in 1 (the implant was removed). Among the 13 patients,1 had carcinoma in situ,and 10 infiltrating ductal carcinoma. Median follow-up was 27 months,showing one patient (T 1 N 1 M 0 ) had lung,bone and brain metastasis 9 months after operation and died 15 months after operation. Ten patients had disease free survival and are still alive now. The main problems were non-union of wound,asymmetry and pain. Conclusion The NSM-IBRWI is oncologically safe and can be used in patients with early breast cancer
