Prediction of Left Atrial Thrombi in Non-Valvular Atrial Fibrillation

Abstract:Objective To study the relation of plasma C-reactive protein to left atrium thrombus formation in patients with non-valvular atrial fibrillation.Methods Ninety eight patients with AF were included and all of them received examination of plasma C-reactive protein(CRP) and D-dimer.All patients received transesophageal echocardiography(TEE) and trans-thoracic echocardiography.Thrombus formation in the left atrium was found in 22 patients(thrombus group) but absence in the remaining 76 patients(non-thrombus group).Results The plasma concentration of CRP in the thrombus group and the non-thrombus group was 1.60 mg/L and 0.80 mg/L respectively(P=0.003) and the concentration of D-dimer of the 2 groups was 170.50 μg/L and 92.00 μg/L respectively(P=0.004).Left atrium diameter(LAd) and left ventricle ejection fraction(LVEF) of the thrombus group compared with the non-thrombus group were 55.20±12.94 mm vs 46.77±12.31 mm(P=0.002) and 57.46±9.10% vs 62.81±8.67%(P=0.006),respectively.Fractional shortening(FS) of the thrombus group and the non thrombus group was 29.82±5.26% and 35.24±5.41% respectively(P0.001).Plasma CRP concentration and left atrium diameter were found to be independent risk factors of thrombosis in patients with non-valvular atrial fibrillation by Logsitic regression.Conclusion CRP and left atrium diameter are independent risk factors of thrombosis in patients with non-valvular atrial fibrillation.Inflammation may play a role in thrombus formation in non-valvular atrial fibrillation.
