Abstract:Objeetive:To observe the clinical efficacy of "Jiedu Huayu Decoction" on active ulcerative colitis (UC) of damp-heat type and its influence on serum cytokines and coagulation indexes.Methods:Sixty-seven patients with active UC of damp-heat type were selected and divided into the treatment group (32 cases) and the control group (35 cases) according to the random number table.The patients in the control group were orally treated with mesalazin enter-coated tablets and the patients in the treatment group were orally treated with mesalazin enter-coated tablets combined with "Jiedu Huayu Decoction",with a course of 8 weeks.The clinical efficacy of the two groups was evaluated,the Chinese medical syndrome score,Baron endoscopic score,pathological score of colon tissue and inflammatory bowel disease questionnaire (IBDQ) score were assessed and compared before and after treatment,and the serum levels of tumor necrosis factor-α (TNF-α),vascular endothelial growth factor (VEGF),interleukin-6 (IL-6),fibrinogen (FIB) and blood platelet (PLT) were detected.Results:After treatment,the total effective rates of the treatment group and the control group were 90.63％ and 68.57％ respectively,and there was statistically significant difference on the clinical efficacy between the two groups (P ＜ 0.05).After treatment,the primary symptom score,secondary symptom score and total score in both groups were obviously decreased (P ＜ 0.01,P ＜ 0.05),and all the scores in the treatment group were lower than those in the control group (P ＜ 0.05).After treatment,the Baron endoscopic score and pathological score of colon tissue in the two groups were significantly decreased (P ＜ 0.01,P ＜ 0.05),and the scores in the treatment group were lower than those in the control group (P ＜ 0.05).After treatment,the scores of each dimension in IBDQ and the total score in the treatment group were all significantly increased (P ＜ 0.01,P ＜0.05),the dimension scores of intestinal symptom,emotion ability and social ability and the total score in the control group were also significantly increased (P ＜ 0.05),and the scores of each dimension and the total score in the treatment group were obviously higher than those in the control group (P ＜ 0.05).After treatment,the levels of TNF-α,IL-6,PLT and FIB were obviously decreased in both groups (P ＜ 0.01,P ＜ 0.05),the level of VEGF was obviously increased in the treatment group (P ＜ 0.05);the levels of TNF-α,IL-6,PLT and FIB in the treatment group were significantly lower than those in the control group (P ＜ 0.05),and the level of VEGF in the treatment group was higher than that in the control group (P ＜ 0.05).Conclusion:"Jiedu Huayu Decoction" combined with routine western medicine can significantly alleviate the clinical symptoms and signs and enhance the life quality of patients with active ulcerative colitis,and its mechanisms may be related to regulating the cytokines related immune abnormity and improving the intestinal mucosal microcirculation.
