Abstract:Objective To detect adverse drug reaction(ADR)signals using data mining algorithm and exPlore its aPPlication value. Methods RePorts on adverse reactions induced by anti-infective drugs in National centor for ADR monitoring from January 2009 to December 2013 were collected and Potential ADR risk signals were detected using ProPortional rePorting ratio method( PRR),rePorting odds ratio method( ROR),Medicines and Healthcare Products Regulatory Agency method( MHRA),Bayesian confidence ProPagation neural network method(BCPNN),and multi-item gamma Poisson shrinker method (MGPS). The results of detection using the above-mentioned 5 signal detection methods were comPared. Results A total of 35 807 ADR rePorts induced by anti-infective drugs were collected,35 759 effective rePorts were entered,and 834 susPected drugs were involved. In the 35 759 rePorts,464 kinds of ADR were defined according to lowest level term and 21 kinds of ADR were defined according to system∕ organ classification. After the data cleaning,sPlitting,and encoding Process,6 620 rePorts containing susPected drug-adverse reaction combination were acquired. There were 3 966 rePorts(59. 91% )in which susPected drug-adverse reaction combination aPPeared once,937 rePorts(14. 15% )in which susPected drug-adverse reaction combination aPPeared twice,and 1 717 rePorts(25. 94% ) in which susPicious drug-adverse reaction combination aPPeared more than thrice. The number of ADR signals detected using PRR,ROR, MGPS,BCPNN,and MHRA was 651,614,306,75,and 57,resPectively;the categories of drugs were194,168,124,34 and 40,resPectively;ADR tyPes were 139,139,121,35,and 40,resPectively. In the toP ten risk signals,azithromycin-nausea were detected by the 5 signal detection methods,levofloxacin-Pruritus were detected by PRR,ROR,MHRA,and BCPNN. The toP ten signals detected by PRR were totally same as those by ROR and signals detected by other methods were various. Conclusions Potential risk signals in ADR rePorts could be detected systematically and automatically using PRR,ROR,MGPS, BCPNN,and MHRA. However,each method has its own advantage and disadvantage and should be aPPlied according to the actual situation and demand.
Study of Serious Adverse Drug Reactions Using FDA-approved Drug Labeling and MedDRA

Abstract:BackgroundAdverse Drug Reactions (ADRs) are of great public health concern. FDA-approved drug labeling summarizes ADRs of a drug product mainly in three sections, i.e., Boxed Warning (BW), Warnings and Precautions (WP), and Adverse Reactions (AR), where the severity of ADRs are intended to decrease in the order of BW>WP>AR. Several reported studies have extracted ADRs from labeling documents, but most, if not all, did not discriminate the severity of the ADRs by the different labeling sections. Such a practice could overstate or underestimate the impact of certain ADRs to the public health. In this study, we applied the Medical Dictionary for Regulatory Activities (MedDRA) to drug labeling and systematically analyzed and compared the ADRs from the three labeling sections with a specific emphasis on analyzing serious ADRs presented in BW, which is of most drug safety concern.ResultsThis study investigated New Drug Application (NDA) labeling documents for 1164 single-ingredient drugs using Oracle Text search to extract MedDRA terms. We found that only a small portion of MedDRA Preferred Terms (PTs), 3819 out of 21,920 or 17.42%, were observed in a whole set of documents. In detail, 466/3819 (12.0%) PTs were in BW, 2023/3819 (53.0%) were in WP, and 2961/3819 (77.5%) were in AR sections. We also found a higher overlap of top 20 occurring BW PTs with WP sections compared to AR sections. Within the MedDRA System Organ Class levels, serious ADRs (sADRs) from BW were prevalent in Nervous System disorders and Vascular disorders. A Hierarchical Cluster Analysis (HCA) revealed that drugs within the same therapeutic category shared the same ADR patterns in BW (e.g., nervous system drug class is highly associated with drug abuse terms such as dependence, substance abuse, and respiratory depression).ConclusionsThis study demonstrated that combining MedDRA standard terminologies with data mining techniques facilitated computer-aided ADR analysis of drug labeling. We also highlighted the importance of labeling sections that differ in seriousness and application in drug safety. Using sADRs primarily related to BW sections, we illustrated a prototype approach for computer-aided ADR monitoring and studies which can be applied to other public health documents.

Abstract:Background: Spontaneous reporting of adverse drug events (ADEs) has long been the cornerstone of pharmacovigilance. Medical institutions in China have been a major source of ADE case reports, but the proportion of reports from tertiary hospitals is low due to the serious underreporting of case reports. The same problem existed in the Second Affiliated Hospital of Zhejiang University School of Medicine (SAHZU). Objective: In order to increase the number of ADE reports and promote hospital pharmacovigilance, SAHZU's clinical pharmacists established a pharmacist-led ADE management model. The aim of this paper is to introduce this management model and explore the advantages and disadvantages of the model. Methods: Pharmacist-led ADE management model was gradually formed from 2015 to 2017 in the SAHZU. This "pharmacist-led" model is reflected not only in the fact that clinical pharmacists are the main reporters of SAHZU's ADEs but also in that they are the main groups to analyze and manage ADE and drug errors. The sources of ADEs reported by clinical pharmacists mainly include pharmacy rounds, ADE-related pharmacist consultations, centralized monitoring, ADE warning signal analysis, newly introduced drug evaluations, and drug safety research. Results: A total of 533 ADEs were reported by SAHZU to China's spontaneous reporting system (SRS) in 2017, while the data in 2012 was 177, with an increase by 201%. In 2012, the proportion of "new" and "serious" reports was 16.4%. The proportions during the period from 2015 to 2017 were 41.4%, 60.8%, and 52.2%, respectively, which were statistically significant compared with the proportion in 2012. The proportion of ADEs reported by clinical pharmacists during the period from 2014 to 2017 were 51.5%, 57.3%, 68.8%, and 90.8%, respectively, which were statistically significant compared with the proportion in 2013 (P<0.05). There was a correlation between the proportions of severe ADEs and the proportion of ADEs reported by clinical pharmacists (r=0.873, P=0.023). Four hundred eighty four ADE cases reported by clinical pharmacists to China's SRS in 2017 were mainly found in rounds of clinical pharmacists (74.17% [359/484]). Conclusion: The pharmacist-led pharmacovigilance working model significantly increased the quantity and quality of ADE reporting in SAHZU and promoted pharmacovigilance. This model is worth developing in Chinese tertiary hospitals and the following hospitals, where the physicians working there spend little time and energy on ADE reporting or the cost of physicians is high, while the clinical pharmacist team has strong professional skills.
An Improved Algorithm for ADR Signal Detection and Its Application to Shanghai Spontaneous ADR Reporting System

Abstract:AIM To examine the efficiency of current algorithms for adverse drug reaction(ADR)signal detection and put forward an improved algorithm.METHODS An algorithm based on new ADR reports(termed as NAR algorithm)was worked out and applied to the ADR report data(during year 2004-2007)of Shanghai spontaneous reporting system(SRS).Two algorithms based on drug-event combination(DEC)and ADR reports [termed as DEC algorithm and report(REP)algorithm,respectively] was compared by simulation and calculation with real SRS data.The influence of reports carrying only known ADRs on signal detection was evaluated.RESULTS ① The DEC algorithm,but not the REP algorithm,produced obviously illogical signals in the calculation with ADR report data(during year 2006-2007)from Shanghai SRS.Moreover,the REP algorithm detected some noteworthy new signals that were missed by the DEC algorithm.Excluding the reports carrying only known ADRs from Shanghai SRS database above greatly improved the signal hit rate by 57%.②An ADR database containing 2 650 drugs and 27 230 DECs was established,which can be used to pick out new ADR reports from SRS automatically.③ The NAR algorithm identified 380 signals from Shanghai SRS database(during year 2004-2007),including gatifloxacin-induced hypoglycemia,ceftazidime-induced allergic shock and Yinzhihuang(traditional Chinese medicine)-induced urticaria.CONCLUSION The NAR algorithm can significantly improve the efficiency of ADR signal detection.

Abstract:Adverse drug reactions (ADRs) are increasingly becoming a serious public health problem. Spontaneous reporting systems (SRSs) are an important way for many countries to monitor ADRs produced in the clinical use of drugs, and they are the main data source for ADR signal detection. The traditional signal detection methods are based on disproportionality analysis (DPA) and lack the application of data mining technology. In this paper, we selected the spontaneous reports from 2011 to 2018 in Jiangsu Province of China as the research data and used association rules analysis (ARA) to mine signals. We defined some important metrics of the ARA according to the two-dimensional contingency table of ADRs, such as Confidence and Lift, and constructed performance evaluation indicators such as Precision, Recall, and F1 as objective standards. We used experimental methods based on data to objectively determine the optimal thresholds of the corresponding metrics, which, in the best case, are Confidence = 0.007 and Lift = 1. We obtained the average performance of the method through 10-fold cross-validation. The experimental results showed that F1 increased from 31.43% in the MHRA method to 40.38% in the ARA method; this was a significant improvement. To reduce drug risk and provide decision making for drug safety, more data mining methods need to be introduced and applied to ADR signal detection.
Signal detection of nervous system from Shanghai spontaneous ADR reporting system reports

Abstract:AIM To examine the efficiency of adverse drug reaction(ADR) signal detection based on spontaneous reporting system(SRS) database while known ADR reports were excluded partially or totally and to identify potential ADR signals of nervous system.METHODS An ADR database of nervous system was established,which was used to remark known ADR reports from SRS automatically.ADR signals of nervous system were generated for possible hazards from proportional reporting ratios(PRR).A new measure representing the association between a drug and an adverse effect(AE) was created and applied as reference to assist signal evaluation.RESULTS An ADR database of nervous system containing 1 261 drugs and 4 138 drug-effect combinations was established.Excluding the reports carrying only known ADRs from Shanghai SRS database above greatly improved the signal hit rate of nervous system by 24.64%.In total,98 signals of nervous system were identified from Shanghai SRS database(during year 2004-2007),including capecitabine-induced dysphagia,mycophenolatemofetil-induced vision abnormal and ceftazidime-induced phycosis.CONCLUSION Excluding reports carrying only known ADRs from SAS database can improve the efficiency of ADR signal detection of nervous system.
Establishment of a Rapid Identification of Adverse Drug Reaction Program in R Language Implementation Based on Monitoring Data

Abstract:OBJECTIVE:To establish a clinically applicable model of rapid identification of adverse drug reaction program (RiADP) for risk management and decision-making of clinical drug use.METHODS:Based on the theory of disproportion analysis, frequency method and Bayes method, a clinically applicable RiADP model in R language background was established, and the parameters of the model were interpreted by MedDRA coding. Based on the actual monitoring data of FDA, the model was validated by the assessing hepatotoxicity of lopinavir/ritonavir (LPV/r).RESULTS:The established RiADP model included four parameters: standard value of adverse drug reaction signal information, empirical Bayesian geometric mean value, ratio of reporting ratio and number of adverse drug reaction cases. Through the application of R language parameter package "phViD", the model parameters could be output quickly. After being encoded by MedDRA, it was converted into clinical terms to form a clinical interpretation report of adverse drug reactions. In addition, the evaluation results of LPV/r hepatotoxicity by the model were matched with the results reported in latest literature, which also proved the reliability of the model results.CONCLUSIONS:In this study, a rapid identification method of adverse reactions based on post marketing drug monitoring data was established in R language environment, which is capable of sending rapid warning of adverse reactions of target drugs in public health emergencies, and providing intuitive evidence for risk management and decision-making of clinical drugs.
Enhancing antidepressant safety surveillance: comparative analysis of adverse drug reaction signals in spontaneous reporting and healthcare claims databases

Abstract:Background/Objective: Spontaneous reporting systems (SRS) such as the Korea Adverse Event Reporting System (KAERS) are limited in their ability to detect adverse drug reaction (ADR) signals due to their limited data on drug use. Conversely, the national health insurance claim (NHIC) data include drug use information for all qualifying residents. This study aimed to compare ADR signal profiles for antidepressants between KAERS and NHIC, evaluating the extent to which detected signals belong to common ADRs and labeling information. Materials and Methods: ADR signal detection in KAERS and NHIC databases, spanning January to December 2017, employed disproportionality analysis. Signal classes were determined based on System Organ Class (SOC) of the Medical Dictionary for Regulatory Activities (MedDRA). Also, Common ADR Coverage (CAC), the proportion of detected signals deemed common ADRs, and labeling information coverage (LIC) represented by mean average precision (mAP) were calculated. Additionally, protopathic bias and relative risk (RR) evaluation were performed to check for signal robustness. Results: Signal detection revealed 51 and 62 signals in KAERS and NHIC databases, respectively. Both systems predominantly captured signals related to nervous system disorders, comprising 33.3% (N = 17) in KAERS and 50.8% (N = 31) in NHIC. Regarding the type of antidepressants, KAERS predominantly reported signals associated with tricyclic antidepressants (TCAs) (N = 21, 41.2%), while NHIC produced most signals linked to selective serotonin reuptake inhibitors (SSRIs) (N = 22, 35.5%). KAERS exhibited higher CAC (68.63% vs. 29.03%) than NHIC. LIC was also higher in KAERS than in NHIC (mAP for EB05: 1.00 vs. 0.983); i.e., NHIC identified 5 signals not documented in drug labeling information, while KAERS found none. Among the unlabeled signals, one (Duloxetine-Myelopathy) was from protopathic bias, and two (duloxetine-myelopathy and tianeptine-osteomalacia) were statistically significant in RR. Conclusion: NHIC exhibited greater capability in detecting ADR signals associated with antidepressant use, encompassing unlabeled ADR signals, compared to KAERS. NHIC also demonstrated greater potential for identifying less common ADRs. Further investigation is needed for signals detected exclusively in NHIC but not covered by labeling information. This study underscores the value of integrating different sources of data, offering substantial regulatory insights and enriching the scope of pharmacovigilance.

Abstract:Objective: Cardiac therapy drugs are widely used in the treatment of heart disease. However, the concern regarding adverse events (AEs) of cardiac therapy drugs have been rising. This study aimed to analyse cardiac therapy drug-related AEs using the Jinan adverse event reporting system (JAERS) database mining and conduct a comprehensive evaluation to provide safe medication information for patients. Design: Retrospective observational study. Setting: In this study, cardiac therapy drug-related AEs were detected using the JAERS database from January 2000 to March 2022. Methods: Reports of cardiac therapy drug-related AEs were extracted from JAERS database, and the basic information of patients, reports and common AEs were analysed. Four disproportionality analysis methods, proportional reporting ratio (PRR), reporting odds ratio (ROR), Bayesian Confidence Propagation Neural Network (BCPNN), Medicines and Healthcare products Regulatory Agency (MHRA), were used to detect cardiac therapy drug-related signals. We further checked whether the detected signals exist on drug labels in China and two developed countries, the USA and Japan. Results: In total, 168 314 AEs were reported, of which 4788 were associated with cardiac therapy drugs. Using the PRR, ROR, MHRA and BCPNN method, we detected 52 signals, 52 signals, 33 signals and 43 signals, respectively. Among the 52 signals, 14 were not included on the drug labels of China. One (isosorbide mononitrate-head bilges) was not included on the drug labels of the three countries. Conclusion: We identified 14 new cardiac therapy drug signals that did not appear on drug labels in China and 1 new signal that did not appear on drug labels in 3 counties. A causal link between cardiac therapy drugs and AEs should be evaluated in further studies.

Abstract:Abstract Continuous evaluation of drug safety is needed following approval to determine adverse events (AEs) in patient populations with diverse backgrounds. Spontaneous reporting systems are an important source of information for the detection of AEs not identified in clinical trials and for safety assessments that reflect the real-world use of drugs in specific populations and clinical settings. The use of spontaneous reporting systems is expected to detect drug-related AEs early after the launch of a new drug. Spontaneous reporting systems do not contain data on the total number of patients that use a drug; therefore, signal detection by disproportionality analysis, focusing on differences in the ratio of AE reports, is frequently used. In recent years, new analyses have been devised, including signal detection methods focused on the difference in the time to onset of an AE, methods that consider the patient background and those that identify drug–drug interactions. However, unlike commonly used statistics, the results of these analyses are open to misinterpretation if the method and the characteristics of the spontaneous reporting system cannot be evaluated properly. Therefore, this review describes signal detection using data mining, considering traditional methods and the latest knowledge, and their limitations.

A simulation-based comparison of drug-drug interaction signal detection methods

Abstract:Several statistical methods have been proposed to detect adverse drug reactions induced by taking two drugs together. These suspected adverse drug reactions can be discovered through post-market drug safety surveillance, which mainly relies on spontaneous reporting system database. Most previous studies have applied statistical models to real world data, but it is not clear which method outperforms the others. We aimed to assess the performance of various detection methods by implementing simulations under various conditions. We reviewed proposed approaches to detect signals indicating drug-drug interactions (DDIs) including the Ω shrinkage measure, the chi-square statistic, the proportional reporting ratio, the concomitant signal score, the additive model and the multiplicative model. Under various scenarios, we conducted a simulation study to examine the performances of the methods. We also applied the methods to Korea Adverse Event Reporting System (KAERS) data. Of the six methods considered in the simulation study, the Ω shrinkage measure and the chi-square statistic with threshold = 2 had higher sensitivity for detecting the true signals than the other methods in most scenarios while controlling the false positive rate below 0.05. When applied to the KAERS data, the two methods detected one known DDI for QT prolongation and one unknown (suspected) DDI for hyperkalemia. The performance of various signal detection methods for DDI may vary. It is recommended to use several methods together, rather than just one, to make a reasonable decision.

Abstract:Purpose To establish a quantitative signal detection system on adverse drug reaction(ADR) spontaneous reporting database of Shanghai. Methods With database developing tool PowerBuilder and SQL2000,we programmed three software modules to perform data coding,signal detection and report generation respectively.Five internationally recognized methods for quantitative signal detection were integrated into the system,and its capacity for signal detection was tested with the ADR reports collected in year 2003 and 2004 from the ADR spontaneous reporting database of Shanghai. Results 1.The system was workable in finding out known signals in the database;2.Combination use of different quantitative methods improved the accuracy of signal detection;3.The system succeeded in detecting two new signals from the database,earlier than regular expert review: andrographolide induced acute renal failure and cefradine induced hematuria. Conclusions Establishment of quantitative signal detection system can greatly enhance the efficiency of ADR monitoring.
Combining a Pharmacological Network Model with a Bayesian Signal Detection Algorithm to Improve the Detection of Adverse Drug Events

Abstract:Introduction: Improving adverse drug event (ADE) detection is important for post-marketing drug safety surveillance. Existing statistical approaches can be further optimized owing to their high efficiency and low cost. Objective: The objective of this study was to evaluate the proposed approach for use in pharmacovigilance, the early detection of potential ADEs, and the improvement of drug safety. Methods: We developed a novel integrated approach, the Bayesian signal detection algorithm, based on the pharmacological network model (IC PNM ) using the FDA Adverse Event Reporting System (FAERS) data published from 2004 to 2009 and from 2014 to 2019Q2, PubChem, and DrugBank database. First, we used a pharmacological network model to generate the probabilities for drug-ADE associations, which comprised the proper prior information component (IC). We then defined the probability of the propensity score adjustment based on a logistic regression model to control for the confounding bias. Finally, we chose the Side Effect Resource (SIDER) and the Observational Medical Outcomes Partnership (OMOP) data to evaluate the detection performance and robustness of the IC PNM compared with the statistical approaches [disproportionality analysis (DPA)] by using the area under the receiver operator characteristics curve (AUC) and Youden’s index. Results: Of the statistical approaches implemented, the IC PNM showed the best performance (AUC, 0.8291; Youden’s index, 0.5836). Meanwhile, the AUCs of the IC, EBGM, ROR, and PRR were 0.7343, 0.7231, 0.6828, and 0.6721, respectively. Conclusion: The proposed IC PNM combined the strengths of the pharmacological network model and the Bayesian signal detection algorithm and performed better in detecting true drug-ADE associations. It also detected newer ADE signals than a DPA and may be complementary to the existing statistical approaches.

Abstract:Multiple data sources are preferred in adverse drug event (ADEs) surveillance owing to inadequacies of single source. However, analytic methods to monitor potential ADEs after prolonged drug exposure are still lacking. In this study we propose a method aiming to screen potential ADEs by combining FDA Adverse Event Reporting System (FAERS) and Electronic Medical Record (EMR). The proposed method uses natural language processing (NLP) techniques to extract treatment outcome information captured in unstructured text and adopts case-crossover design in EMR. Performances were evaluated using two ADE knowledge bases: Adverse Drug Reaction Classification System (ADReCS) and SIDER. We tested our method in ADE signal detection of conventional disease-modifying antirheumatic drugs (DMARDs) in rheumatoid arthritis patients. Findings showed that recall greatly increased when combining FAERS with EMR compared with FAERS alone and EMR alone, especially for flexible mapping strategy. Precision (FAERS + EMR) in detecting ADEs improved using ADReCS as gold standard compared with SIDER. In addition, signals detected from EMR have considerably overlapped with signals detected from FAERS or ADE knowledge bases, implying the importance of EMR for pharmacovigilance. ADE signals detected from EMR and/or FAERS but not in existing knowledge bases provide hypothesis for future study.

Abstract:<p>As a medicine safety issue, Drug-Drug Interaction (DDI) may become an unexpected threat for causing Adverse Drug Events (ADEs). There is a growing demand for computational methods to efficiently and effectively analyse large-scale data to detect signals of Adverse Drug-drug Interactions (ADDIs). In this paper, we aim to detect high-quality signals of ADDIs which are non-spurious and non-redundant. We propose a new method which employs the framework of Bayesian network to infer the direct associations between the target ADE and medicines, and uses domain knowledge to facilitate the learning of Bayesian network structures. To improve efficiency and avoid redundancy, we design a level-wise algorithm with pruning strategy to search for high-quality ADDI signals. We have applied the proposed method to the United States Food and Drug Administration's (FDA) Adverse Event Reporting System (FAERS) data. The result shows that 54.45% of detected signals are verified as known DDIs and 10.89% were evaluated as high-quality ADDI signals, demonstrating that the proposed method could be a promising tool for ADDI signal detection.</p>

Detection of cluster adverse drug events in the spontaneous reporting system of China using a disproportionality filter algorithm

Abstract:What is known and objective: Cluster adverse drug events (CADEs) are multiple ADEs with similar clinical manifestations involving the same drug, manufactured by the same company, that occur within a short time period. The disproportionality filter algorithm (DFA) is a promising tool for the identification of historical clusters related to ADEs. The Chinese spontaneous adverse drug reaction reporting system (SRS) may serve as an important database for the detection of CADEs. The objective of this study was to evaluate the usefulness of DFA as an approach to identify CADEs using SRS. Methods: Suspected adverse drug reaction (ADR) reports collected by the Chinese SRS in 2014-2015 were examined to identify potential CADEs. The reports were divided into 48 15-day subsets. Disproportionate excess reporting of ADEs for drugs from specific companies may be a signal for CADEs. The clusters were categorized as 'confirmed', 'potential', 'unlikely', 'indecisive' or 'information-loss' ADEs when evaluated by report units. Furthermore, early warning information in 2014-2015 from the Chinese early warning system (EWS) classified as 'concern', 'monitoring', 'ignorance' or 'rest' was compared with DFA to explore the applicability of the novel algorithm in Chinese SRS. Results and discussion: In total, 2294 CADEs, comprising of 380 confirmed, 1753 potential, 15 unlikely and 59 indecisive clusters, were generated; 87 clusters were missing additional information. There were 263 'significant' clusters with DFA, but only 26 'significant' clusters in EWS. The sensitivity of DFA was 88.5%, but the specificity and positive predictive value were low. What is new and conclusion: Spontaneous adverse drug reaction reporting system in China may be a potential database for the identification of CADEs engaging the DFA. This could supplement the EWS of CADEs in China. The DFA may be of value in detecting CADEs with high sensitivity, although expert screening is required given the low specificity and positive predictive value.

Abstract:Objective To promote reasonable medication and discuss about the methods of prevention with adverse drug reactions (ADR). Method We studied ADR by review of 172 samples reported from September 2003to August 2005 in Peking University Shenzhen Hospital. Results The results showed that ADR were associated with age, type of drug, medication and technique of drug manufacture. Conclusion It suggests that reduction of ADR can be obtained through reasonable medication, reinforcing monitor of ADR and strict operation in medication, et al.

Abstract:AimsTo evaluate and compare the relative performance of the tree-based scan statistic (TreeScan) with the crude cohort study, Bayesian confidence propagation neural network (BCPNN) and Gamma Poisson Shrinker (GPS) in detecting statin-related adverse events (AEs) in an electronic healthcare database. MethodsData from a Chinese healthcare database from 2010 to 2016 were evaluated. We identified statin users based on prescription information in their out-/in-patient records, and AEs were defined according to the ICD-10 codes in patients' diagnosis records. TreeScan was applied to detect AE signals related to statin use and was compared with 3 other methods based on sensitivity, specificity, positive predictive value, negative predictive value, accuracy, the Youden index, area under the precision-recall curve and the area under the receiver operating characteristic curve. ResultsA total of 224 187 patients were enrolled and divided into 85 758 statin users and 138 429 nonusers. TreeScan generated 29 positive signals, of which 9 were known AEs. The sensitivities of TreeScan, BCPNN and GPS were all 69.2%, which was higher than that of the crude cohort study (46%). The specificity (82.3%), positive predictive value (31.0%), negative predictive value (95.9%), accuracy (81.0%), Youden index (51.5%) and area under the receiver operating characteristic curve (75.8%) of TreeScan were the highest among the 4 methods. ConclusionTreeScan outperformed the crude cohort, BCPNN and GPS in detecting statin-related AEs in an electronic healthcare database. Therefore, it can be used as a complementary tool for other signal detection methods in drug safety surveillance.

Abstract:Adverse drug reactions (ADRs) are big concern for public health. ADRs are one of most common causes to withdraw some drugs from markets. Now two major methods for detecting ADRs are spontaneous reporting system (SRS), and prescription event monitoring (PEM). The World Health Organization (WHO) defines a signal in pharmacovigilance as "any reported information on a possible causal relationship between an adverse event and a drug, the relationship being unknown or incompletely documented previously". For spontaneous reporting systems, many machine learning methods are used to detect ADRs, such as Bayesian confidence propagation neural network (BCPNN), decision support methods, genetic algorithms, knowledge based approaches, etc. One limitation is the reporting mechanism to submit ADR reports, which has serious underreporting and is not able to accurately quantify the corresponding risk. Another limitation is hard to detect ADRs with small number of occurrences of each drug-event association in the database. In this paper we propose feature selection approach to detect ADRs from The Health Improvement Network (THIN) database. First a feature matrix, which represents the medical events for the patients before and after taking drugs, is created by linking patients' prescriptions and corresponding medical events together. Then significant features are selected based on feature selection methods, comparing the feature matrix before patients take drugs with one after patients take drugs. Finally the significant ADRs can be detected from thousands of medical events based on corresponding features. Experiments are carried out on the drug Atorvastatin. Good performance is achieved.

Abstract:Background: Drugs are related with various adverse drug reactions (ADRs), however, many unexpected ADRs of drugs are reported through post-marketing surveillance. Aim: The current study's goal is to uncover potential signals connected with FDA-approved medications in the United States (2013). Methods: Open Vigil 2.1-MedDRA-v24 (data 20004Q1-2021Q3) was used as a tool to query the FAERS data. To find possible signals, disproportionality measures such as Proportional Reporting Ratio (PRR 2) with associated Chi-square value, Reporting Odds Ratio (ROR 2) with 95% confidence interval, and case count (3) were calculated. Results: A total of eight potential signals were identified with five drugs. Positive signals were found with pomalidomide, canagliflozin, dolutegravir sodium, macitentan and ibrutinib. Conclusion: However, further causality assessment is required to confirm the association of these drugs with identified potential signals.
