Abstract:PURPOSE: To evaluate the clinical effects of vertical ramus osteotomy for treatment of condylar osteochondroma.METHODS: Nine patients with osteochondroma of the mandibular condyle from 2004 to 2009,treated by condylar resection and application of pedicled bone grafts gained by vertical ramus osteotomy to reconstruct the condyle,were observed and evaluated.Imaging examinations,occlusion and the function of temporomandibular joint were assessed after surgery.RESULTS: After following up for at least 8 months,good occlusion,symmetrical face,and satisfactory function of temporomandibular joint were regained.Imaging examinations showed that the upper ends of posterior ramus were in the glenoid fossa,and with time going on,it became a smooth and flat ball gradually.CONCLUSION: The posterior margin of ramus with a blood supply obtained by vertical ramus osteotomy is a relatively ideal method to reconstruct the mandibular condyle to be resected because of osteochondroma.

Abstract:Objective:To introduce the endoscopic-assisted vertical ramus osteotomy (VRO) for the treatment of condylar osteochondroma and discuss its indications and effectiveness. Methods: 5 cases with unilateral osteochondroma of mandibular condyle but without severe occlusal deviation were treated with endoscopic-assisted VRO in our department from April 2013 to May 2015. CT imagings were taken for diagnosis and treatment planning. Occlusion, facial contour and maximum mouth opening (MMO) were recorded and compared before and after operation to evaluate the effectiveness of the therapy. Results: 5 patients were treated with endoscopic-assisted VRO and rigid fixation. 1 patients accepted an additional mandibular contour shaping. The pathologic diagnosis were all confirmed as osteochondroma. A mean of 28.6 months follow-up for all 5 cases showed stable post-operative occlusion and facial situation. No functional disturbance were found according to the MMO. Conclusion: For osteochondrama patients without severe malocclusal, endoscopic-assisted VRO is a good option while orthognathic surgery might not be carry out due to patients' situation. This operation achieves two targets, tumor resection and occlusion rehabilitation, as well as reduces the trauma. It is a powerful supplement to the traditional surgical procedures.

Abstract:We report the outcomes of 12 patients with osteochondroma of the mandibular condyle who were treated by condylectomy with sagittal split ramus osteotomy (SSRO) between January 2011 and October 2015. Variables assessed before and after operation were imaging, appearance, maximum mouth opening, maximum mandibular protrusion, lateral excursion, and function of the temporomandibular joint (TMJ). Patients were followed up for a mean (range) of 21 (13 - 30) months. Outcomes were satisfactory with no complications or recurrence. Patients regained good occlusion and facial symmetry, and satisfactory function of the TMJ. Our results suggest that SSRO is a good option for condylar reconstruction after resection of mandibular condylar osteochondroma.

Abstract:We describe the use of computer-assisted three-dimensional surgical planning in condylar reconstruction by vertical ramus osteotomy for patients with osteochondroma, and its clinical effects. Seventeen patients with osteochondroma of the mandibular condyle who were seen from March 2005 to March 2009 were divided into 2 groups treated by condylectomy and condylar reconstruction using vertical sliding osteotomy of the mandibular ramus with and without three-dimensional simulation using Surgicase CMF Materialise software. Clinical examination, radiographs, photographs, and details of operation and outcome were used postoperatively to evaluate the clinical effects of the technique. Satisfactory mouth opening was achieved in all cases. Mean (SD) osteotomy and fixation time, duration of intermaxillary fixation, and degree of postoperative numbness of the lower lip were considerably reduced among patients who had three-dimensional simulation. The combined use of computer-assisted three-dimensional surgical planning and simulation with vertical ramus osteotomy to reconstruct the condyle for patients with osteochondroma after excision of the tumour makes the operation more accurate and more convenient, and avoids damage to vital structures.
Digital Cutting Guide and Endoscopically-Assisted Vertical Ramus Osteotomy to Treat Condylar Osteochondroma: a Long-Term Study.

Abstract:We have introduced an effective treatment for mandibular condylar osteochondroma with a digital cutting guide and endoscopically-assisted vertical ramus osteotomy (VRO). Eleven patients with unilateral condylar osteochondroma, who did not require orthognathic surgery or had less than 3mm deviation of the chin and a stable occlusion, were treated during the period April 2013–January 2017 with a digital cutting guide and endoscopically-assisted VRO. Clinical data collected included the occlusion, facial contour, and maximum mouth opening (MMO). Computed tomographic (CT) scans were taken before and after operation. Two patients also had additional shaping of the mandibular contour. The pathological diagnosis was confirmed to be osteochondroma in all cases. A mean (range) 19 (12–40) months of follow-up for all 11 cases showed stable postoperative occlusion and facial aesthetics. There were no functional disturbances, recurrence, or condylar absorption. VRO is an alternative to orthognathic surgery for patients with osteochondroma who do not have severe malocclusions. The digital cutting guide and endoscopically-assisted VRO make it possible to achieve precise resection of the tumour and maintain the occlusion with minimal invasion.
One- Stage Treatment to Condylar Osteochondroma Combined with Secondary Jaw Bone Deformities by Sliding Vertical Ramus Osteotomy and Mandibular Contouring

Abstract:Osteochondroma of the mandibular condyle could cause secondary facial asymmetry; besides, it could affect the function of the temporomandibular joint (TMJ) and occlusion. Conventional treatments include total condylectomy or local resection of the lesion and condylar reconstruction. But it cannot effectively ameliorate the facial asymmetry. Thus, this article presents the application of combination surgeries including condylectomy, sliding vertical ramus osteotomy, and mandibular contouring in the treatments of this kind of disease. From 2007 to 2012, 12 patients with osteochondroma of the mandibular condyle were included. All of them accepted condylectomy, sliding vertical ramus osteotomy, and mandibular contouring at 1 stage. Mandibular contouring included mandibular inferior border ostectomy, mandibular outer cortex ostectomy, and horizontal osteotomy genioplasty according to the characters of jaw deformity. In addition, maximal mouth opening, pain in the TMJ, and numbness of the lower lip were recorded preoperatively and postoperatively to evaluate the therapeutic effects. All of patients obtained satisfactory results; TMJ dysfunction, facial asymmetry, and abnormal occlusion were improved greatly. No patients developed recurrence of the tumor during the follow-up period. In conclusion, after condylectomy, sliding vertical ramus osteotomy and mandibular contouring were operated at 1 stage, which effectively ameliorated the condylar osteochondroma patient's facial asymmetry and abnormal occlusion and achieved good therapeutic effects.

Abstract:The purpose of this retrospective study was to evaluate 10 patients with osteochondroma of the mandibular condyle who were treated by vertical ramus osteotomy. Three patients had resection of the condyle and reconstruction with free vertical ramus osteotomy grafts (free graft group) and seven had pedicled vertical sliding ramus osteotomy grafts (pedicled graft group). The mean (range) observation period was 30 months. All patients had satisfactory clinical outcomes, and facial symmetry and good occlusion were achieved during the first 10 months. However, slight facial asymmetry was observed in patients treated by free grafts at 11 months, as the mandible deviated to the operated side when the mouth was opened. Postoperative radiographs showed varying degrees of neocondylar resorption and reduction in height. Patients treated with pedicled grafts did not develop these symptoms, and had no bony resorption to speak of on radiographs taken during the follow-up period. In conclusion, the advantages of the pedicled graft over the free graft include less bony resorption and better long-term clinical outcomes.

Abstract:Purpose: Osteochondroma is a benign tumor that is rare in the craniofacial region. When it does occur, the condyle and coronoid process have been the most commonly affected sites. The secondary progressive malocclusion and facial asymmetry will be common physical signs in most cases. The traditional surgical treatment of osteochondroma of the mandibular condyle has been condylectomy, with or without reconstruction of the condyle. We believe more attention should be paid to restoring joint function, improving the facial appearance, correcting malocclusion, and re-establishing harmony among them.Patients and Methods: From January 2000 to March 2013, 27 patients (17 women and 10 men) who had been diagnosed with osteochondroma of a unilateral mandibular condyle underwent condylectomy and condylar reconstruction using a pedicled posterior mandibular border obtained by ramus osteotomy. The secondary dentofacial deformities were simultaneously corrected using orthognathic and facial contouring procedures, followed by orthodontic treatment, when necessary. Occlusion and temporomandibular joint pain and function, including the maximal mouth opening and maximal protrusion, were recorded preoperatively and postoperatively.Results: The patients were followed for an average of 13 months (range 24 to 48). The outcomes of, and feedback information from the patients showed apparent improved joint function, with no cases of osteochondroma recurrence. The secondary dentofacial deformities were corrected significantly. Satisfactory occlusion was achieved with orthognathic and orthodontic treatment.Conclusions: Our data suggest that condylectomy and condylar reconstruction, with simultaneous correction of the secondary dentofacial deformities using orthognathic procedures, might be a better approach to manage osteochondroma accompanied by dentofacial deformities. In addition to facial contouring procedures, orthodontic treatment should be considered for better improvement in facial esthetics and occlusion. Abstract:Objective To evaluate the combined application of condylectomy, sliding vertical ramus osteotomy, and mandibular contouring in treating condylar osteochondroma. Methods Eight patients with osteochondroma of the mandibular condyle were subjected to condylectomy, sliding vertical ramus osteotomy, and mandibular contouring at one stage. After the surgery, orthodontic treatment or intermaxillary fixation was applied to help treat those patients with facial asymmetry and abnormal occlusion. Results All the patients demonstrated satisfactory results. Their temporomandibular joint dysfunction, facial asymmetry, and abnormal occlusion were improved greatly. No patient developed a recurrence of the tumor during the follow-up period. Conclusion Condylectomy, sliding vertical ramus osteotomy, and mandibular contouring do not only remove the tumor, but also effectively ameliorate the facial asymmetry and abnormal occlusion of the patients as well as produce a favorable therapeutic effect.

Abstract:OBJECTIVE:Osteochondroma is rarely seen in the facial region, especially around the condyle. Here, we report a case of condylar osteochondroma, aiming to assist the diagnosis and treatment of the tumor.METHODS:A case of osteochondroma of the left mandibular condyle in a 49-year-old man was presented. Medical records with x-ray, computed tomographic scan, and bone scan of histologically proven osteochondroma of mandibular condyle were obtained.RESULTS:The patient underwent a surgical resection and had fewer functional changes as well as less dysfunction of the temporomandibular joint.CONCLUSIONS:The current study highlights the fact that, despite its rarity in the mandibular condyle, surgical resection is an effective treatment method. The decision, however, depends on how much swing of the mandible is required after surgery for correction of asymmetry and occlusion.

Abstract:The traditional approach for ankylosis is gap arthroplasty or interpositional arthroplasty followed by reconstruction of the condyle using, for example, costochondral grafts. As these are non-pedicled grafts, there is eventual resorption with subsequent decrease in height of the ramus, facial asymmetry and deviated mouth opening. The authors have applied the method of total and partial sliding vertical osteotomy on the posterior border of the mandibular ramus for reconstruction of the mandible condyle as a pedicled graft for the correction of temporomandibular joint (TMJ) ankylosis. From 2004 to 2008, 18 patients who were diagnosed with TMJ ankylosis underwent operations for resection of the ankylosed condyle. Two methods were performed depending on the level of osteotomy on the posterior part of the mandibular ramus. All patients were followed-up for an average of 36 months (range 24-48 months). All patients showed apparent improved joint function with no cases of re-ankylosis. The results showed that sliding vertical osteotomy on the posterior border of the mandibular ramus seems to be an alternative and promising method for condylar reconstruction in patients with TMJ bony ankylosis.

Abstract:Osteochondroma is one of the most common benign tumors of axial skeleton.Although osteochondroma is rarely seen in maxillofacial region,the cases in literature are usually in the mandibular region,especially around the condyle.Osteochondroma of the mandibular condyle is difficult to be diagnosed and treated.Osteochondroma of the mandibular condyle which has been confirmed to be benign tumor may cause signs and symptoms like those seen in patients with temporomandibular joint dysfunction.This study is a retrospective review of the newly-reported cases of osteochondroma of the mandibular condyle in the literature for the purpose of improvement of the understanding of the illness.
Evaluation of treatment with unilateral mandibular sagittal split ramus osteotomy and maxillary osteotomy in patients with condylar osteochondroma and mandibular asymmetry: A retrospective case series

Abstract:The aim of the study was to describe an approach where condylar resection with condylar neck preservation was combined with Le Fort I osteotomy and unilateral mandibular sagittal split ramus osteotomy (SSRO). Patients with a unilateral condylar osteochondroma combined with dentofacial deformity and facial asymmetry who underwent surgery between January 2020 and December 2020 were enrolled. The operation included condylar resection, Le Fort I osteotomy and contralateral mandibular sagittal split ramus osteotomy (SSRO). Simplant Pro 11.04 software was used to reconstruct and measure the preoperative and postoperative craniomaxillofacial CT images. The deviation and rotation of the mandible, change in the occlusal plane, position of the "new condyle" and facial symmetry were compared and evaluated during follow-up. Three patients were included in the present study. The patients were followed up for 9.6 months on average (range, 8-12). Immediate postoperative CT images showed that the mandible deviation and rotation and occlusion plane canting decreased significantly postoperatively; facial symmetry was improved but still compromised. During the follow-up, the mandible gradually rotated to the affected side, the position of the "new condyle" moved further inside toward the fossa, and both the mandible rotation and facial symmetry were more significantly improved. Within the limitations of the study it seems that for some patients a combination of condylectomy with condylar neck preservation and unilateral mandibular SSRO can be effective in achieving facial symmetry.

Abstract:Objective The objectives of this study were to introduce the classification of osteochondroma of the mandibular condyle based on computed tomographic images and to present our treatment experiences. Materials and Methods From January 2002 and December 2012, a total of 61 patients with condylar osteochondroma were treated in our division. Both clinical and radiologic aspects were reviewed. The average follow-up period was 24.3 months with a range of 6 to 120 months. Results Two types of condylar osteochondroma were presented: type 1 (protruding expansion) in 50 patients (82.0%) and type 2 (globular expansion) in 11 patients (18.0%). Type 1 condylar osteochondroma presented 5 forms: anterior/anteromedial (58%), posterior/posteromedial (6%), medial (16%), lateral (6%), and gigantic (14%). Local resection was performed on patients with type 1 condylar osteochondroma. Subtotal condylectomy/total condylectomy using costochondral graft reconstruction with/without orthognathic surgeries was performed on patients with type 2 condylar osteochondroma. During the follow-up period, tumor reformation, condyle absorption, and new deformity were not detected. The patients almost reattained facial symmetry. Conclusions Preoperative classification based on computed tomographic images will help surgeons to choose the suitable surgical procedure to treat the condylar osteochondroma.

Abstract:Fracture or defect of the mandible is a serious complication of mandibular angleplasty, and precise reconstruction for such defect is still a huge challenge. This case report provides a new method based on CAD/CAM and rapid prototyped titanium for individual design, fabrication, and implantation of a mandibular ramus and angle. A 25-year-old woman with a square-shaped face, who had undergone mandibular outer cortex split ostectomy (MOCSO) 3 months earlier, was afflicted by a series of symptoms: asymmetric face, collapse of the right face, masticatory problems, deviation during mouth opening, malocclusion, and TMJ clicks. These symptoms were caused by unintentional removal of the fractured ramus during MOCSO. By means of CT scan data, rapid prototyping, reverse engineering, 3D display, and CAD/CAM, the individualized titanium implant was designed and fabricated. The 3D demo system of operative scheme demonstrated the operative procedure, and determined the position of the implant so as to obtain a perfect fit. Postoperatively, the patient regained satisfactory morphologic symmetry, facial appearance, occlusion, and TMJ functions.
Treatment of Dentofacial Deformities Secondary to Osteochondroma of the Mandibular Condyle Using Virtual Surgical Planning and 3-Dimensional Printed Surgical Templates.

Abstract:Purpose: One-stage treatment for condylar osteochondroma and secondary facial deformities by resection and reconstruction of the mandibular condyle, orthognathic surgery, and mandibular contouring has been reported recently. This study investigated the clinical feasibility of treating osteochondroma of the mandibular condyle and secondary dento-maxillofacial deformities by virtual surgical planning and 3-dimensional (3D) printed surgical templates.Materials and Methods: A composite skull model with accurate dentition was obtained with data from spiral computed tomography (CT) and surface scanning of the dental arch. Virtual surgical simulation was performed using Dolphin Imaging 11.7 Premium and Mimics software after a comprehensive 3D diagnosis and surgery planning. Surgical templates were fabricated by 3D printing using data from virtual surgical simulation for guidance of excision of the mandibular condyle with osteochondroma, reconstruction of the mandibular condyle, mandibular contouring, and reconstruction of a normal occlusion. Le Fort I osteotomy of the upper jaw and genioplasty were performed when indicated. The linear difference between virtually simulated and postoperative skull models was evaluated.Results: All surgeries were successfully simulated using virtual surgical planning, and the guiding templates were successfully applied for all patients. Successful reconstruction of condylar function, normal occlusion, and symmetry of the facial profile was achieved. Postoperative CT scans and quantitative analysis showed that virtual surgical plans provided acceptable accuracy in the operating room. The linear difference of the incisors and first molars was no more than 1.4 mm, and the greatest difference was found for the menton landmark, which was up to 2.4 mm.Conclusions: Results from this study suggested that virtual surgical planning and guiding templates facilitated accurate diagnosis, treatment planning, accurate osteotomy, repositioning of bony segments, and contouring of the mandibular border in the treatment of condylar osteochondroma and secondary facial asymmetry. In this report, we describe the treatment of an OC on the left condyle in a 31-year-old Chinese female by conservative resection of the tumor into multiple fragments and preservation of most of the condylar area with minimal condylar reshaping. The postoperative cosmetic result was satisfactory.

Abstract:Objective: The objectives of this study were to introduce the classification of osteochondroma of the mandibular condyle based on computed tomographic images and to present our treatment experiences. Materials and Methods: From January 2002 and December 2012, a total of 61 patients with condylar osteochondroma were treated in our division. Both clinical and radiologic aspects were reviewed. The average follow-up period was 24.3 months with a range of 6 to 120 months. Results: Two types of condylar osteochondroma were presented: type 1 (protruding expansion) in 50 patients (82.0%) and type 2 (globular expansion) in 11 patients (18.0%). Type 1 condylar osteochondroma presented 5 forms: anterior/anteromedial (58%), posterior/posteromedial (6%), medial (16%), lateral (6%), and gigantic (14%). Local resection was performed on patients with type 1 condylar osteochondroma. Subtotal condylectomy/total condylectomy using costochondral graft reconstruction with/without orthognathic surgeries was performed on patients with type 2 condylar osteochondroma. During the follow-up period, tumor reformation, condyle absorption, and new deformity were not detected. The patients almost reattained facial symmetry. Conclusions: Preoperative classification based on computed tomographic images will help surgeons to choose the suitable surgical procedure to treat the condylar osteochondroma.
