Abstract:Objective: To evaluate the effects of disc height and degeneration on mechanical behavior of human lumbar spine segments. Methods: A new effective CAD method was used to accurately establish three finite element models, including normal height disc (NHD) model, pure height decreased disc (PHDD) model and degeneration combined height decrease disc (DHDD) model. The biomechanical parameters of three finite element models were measured under axial compressive load. Results: The height and degeneration of disc had obviously effects on the mechanical behavior of lumbar spine segment in terms of axial displacement, posterolateral disc bulge, the maximum stress in the peripheral anulus fibers. The stress distribution differed between DHDD and non-DHDD model. Conclusions: The stiffness of disc obviously increases after height decreasing. The happening of disruption of inner anulus fibers is the lowest in DHDD model. The ratio between the gap of facet articulation and axial displacement are crucial to determine the load strength of disc. The degenerated disc plays an effective role for stress distribution.
[Analysis of Lumbar Disc Degeneration Using Three-Dimensional Nonlinear Finite Element Method].

Abstract:OBJECTIVE To explore the effects of intervertebral disc degeneration on the biomechanical behavior of the lumbar motion segment. METHODS A three-dimensional nonlinear finite element model of L4-L5 segment was established using CAD technique based on CT images. A normal disc model and three degenerative disc models were established by changing the parameters such as disc material properties and disc height. The effects of disc degeneration on the biomechanical properties, including stiffness, nucleus pressure, maximum von Mises stress in the annulus, and force of posterior structure were studied under two moment loads (flexion and extension) and for three different direct forces (compression, and anterior and posterior shear forces), and the stress distribution of vertebral body and end- plate was also analyzed. RESULTS The stiffness of the lightly degenerative disc model was decreased compared with that of the normal disc, while it was increased in the moderately and severely degenerative disc models when compared with the normal disc. The force of posterior structure in the lightly degenerative disc model was increased while the values of force of posterior structure in the moderately and severely degenerative disc models were decreased gradually. The maximum von Mises stress in the annulus increased, and the nucleus pressure decreased as the disc degeneration progressed. Moreover, the stress of intervertebral disc, vertebral body, and end-plate was distributed more peripherally. CONCLUSION Light degeneration of intervertebral disc leads to instability of lumbar spine, while the stability restores with further degeneration of disc. There is a negative correlation between the force of posterior structure and the load on the intervertebral disc. With the disc degeneration progressing, the intervertebral disc load pattern changes, the stress of intervertebral disc, vertebral body, and end-plate is concentrated peripherally.

Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.

Abstract:The intravertebral vacuum phenomenon (VP) is usually associated with degenerative disc disease, which could be related to the low back pain. Various theories related to the pathogenesis of VP have been proposed, but these theories have not been critically examined and remain hypothetical. In this article, we review the possible role of endplate degeneration in the pathogenesis of VP, and discuss several pathways possibly linked to them. Due to the endplate calcification and activated cytokines, the transport pathway of the nutrition for the intervertebral disc was blocked, resulting in the metabolic unbalance and decrease of the synthesis of matrix structural proteins. It could promote the matrix decomposition, causing the decrease of the quantity of matrix and the changes of stress distribution in intervertebral disc. As a result, the structure of intervertebral discs became increasingly unstable. While compression happened, the intravertebral cleft could occur and be gradually filled with gas, which may cause low back pain and aggravate the intervertebral discs degeneration. As outlined above, we hypothesize that endplate degeneration might be the origination of the vacuum phenomenon.

Abstract:OBJECTIVE:To review the research progress of the vacuum phenomenon in spine.METHODS:The recent articles about the vacuum phenomenon in spine were reviewed; the features of imaging and the clinical correlation were summarized.RESULTS:The vacuum phenomenon has been described in every segment of the spine. This phenomenon occurs mostly to the disc space and represents a transparent area on radiograph.CONCLUSION:The causes of the vacuum phenomenon include degeneration of the anatomic structures of spine, trauma, and other pathologic changes. The vacuum phenomenon has important diagnostic and clinical significances.
Intervertebral Disc Vacuum Phenomenon is Associated with Modic Changes in the Adjacent Endplate: Implications for Discogenic Back Pain

Abstract:Abstract Background Back pain often arises from degenerative changes in lumbar intervertebral discs and their adjacent endplates. A painful endplate is not easy to identify in patients, but could possibly be revealed by inflammatory-like ‘Modic’ changes and by a ‘vacuum phenomenon’ within the disc. We hypothesize that Modic changes and a VP often co-exist in those lumbar levels most closely associated with back pain Methods We scanned 1023 consecutive inpatients of the Department of Orthopaedics from 2015 August to 2018 August, all patients suffered from lumbar degenerative diseases, whether the patients had back pain or not were evaluated, and the prevalence and location of vacuum phenomenon(VP) and Modic changes were compared at each spinal level. Results 5115 discs were studied from 1023 patients. The number of discs showed to have a VP was 430 using CT, of the 430 discs with a CT-diagnosed VP, 116 were L4-5, and 171 were L5-S1. 522 of the 5115 discs exhibited Modic changes, with prevalence of type I, type II and type III Modic changes being 1.6%, 8.2% and 0.4% respectively. Modic changes usually occurred adjacent to L4-5 discs or L5-S1 discs. The prevalence of back pain was higher in the VP group than no-VP group, VP were significantly associated with Modic changes Type II at L4/5 and at L5/S1. Conclusion VP are closely associated with back pain and Modic changes in the lower lumbar spine. Further investigations may be warranted when radiographs or CT identify a VP in a lumbar disc.
Assessment of Apparent Diffusion Coefficient Values in Normal and Degenerated Intervertebral Lumbar Disks: Preliminary Study

Abstract:Objective:To reflect the diffusibility of water in lumbar disks in adults by using the mean apparent diffusion coefficient(ADC) values and to determine if there was a relationship between ADC values and magnetic resonance findings of disks.Methods:Diffusion weighted imaging of lumbar spine was performed in 28 patients and 22 volunteers.ADC values of each disk were measured.According to the sagittal T_2-weigted images,all disks were graded into normal and degeneration.Mean ADC values of normal disks were compared with those of degenerated disks and were correlated with anatomic location.Results:The calculation of ADC values was possible in 47 of 50 participants.ADC values could not be calculated in three participants because of poor signal-to-noise and too great artifact.The mean ADC values of normal and degenerated disk were 2.32×10~(-3)±0.54(mm~2/s)and 1.87×10~(-3)±0.42(mm~2/s)respectively.There was significant correlation between mean ADC values and anatomic location(P0.05).Conclusion:Comparing with ADC values of normal disk,a statistically significant decrease was displayed in the degenerated lumbar disks,with lower ADC values seen in more caudal disks.

Abstract:Study Design. Observational serial CT analysis of the lumbar spine in a normal-aging population. Objective. Assess the natural history of IntraDiscal Vacuum Phenomenon and its role in disc degeneration. Summary of Background Data: The natural history of disc degeneration is well described but our understanding of the end stage of pathogenesis remains incomplete. MRI loses accuracy with advanced degeneration, becoming hyporesonant and indistinct. Cadaveric specimens display adaptive changes in the disc with loss of the hydrostatic capacity of the nucleus, increased intra-discal clefts and end-plate impermeability. IDVP is associated with advanced disc degeneration and CT is the optimal modality to visualise this, yet these insights remain unreported. Methods. Subjects only included historic CT abdomen scans of those over 60 years of age without acute or relevant spinal pathology, with a diagnosis of at least one level with IDVP on the original CT scan and all of whom had a similar scan >7 years later. A history of clinically significant back pain was also recorded. Results. CT scans included 360 levels in 29 males and 31 females (mean 68.9 years), displaying 82 levels of IDVP, with a second scan included after a mean of 10.3 years, Most levels displayed the same level of severity (persisted, 45) compared to where some progressed (26), regressed (8) and fused (3) ( P <0.01). There was also an increased incidence, 37/60 (62%) of developing IDVP at another level. Disc heights were reduced with increased severity of IDVP. A record of back pain was evident in 31/60 subjects, which was not significantly worse in those with worsening severity or additional level involvement over the study period. Conclusion. As disc degeneration advances, the associated IDVP persists in most cases, displaying a plateauing of severity over long periods, but rarely with progression to autofusion.

Abstract:In this review, we explored the progress of the pathogenesis of Kummell's disease intravertebral vacuum. Using different expressions of the same disease including 'Kummell's disease', 'avascular necrosis after vertebral compression fracture (VCF)', 'post-traumatic vertebral osteonecrosis', 'vertebral pseudarthrosis', 'intravertebral vacuum (cleft or gas)', 'delayed vertebral collapse', 'VCF nonunion', and by conducting a search of the PubMed database, we analyzed the results to examine the pathogenesis of the intravertebral vacuum of Kummell's disease after referring to pertinent literature on intravertebral vacuum of ischemic necrosis after VCF, and exploring the progress of pathogenesis of this disease. A number of discrepancies were identified within the pathogenesis of the intravertebral vacuum after VCF. There were statements such as avascular necrosis of the vertebral body, bone biomechanics, gas forming and other types of claims, all of which obtained clinical and biomechanical supporting evidence. Collectively, most of the researchers believe that Kummell vertebral fracture syndrome was the comprehensive effect of multiple factors including osteoporosis, avascular necrosis of the vertebral body, and biomechanical changes following fracture. However, there are a number of discrepancies to be resolved and future studies are therefore needed.

Abstract:The term 'vacuum phenomenon' (VP), is characterized by gas-like density areas due to a rapid increase in the joint space volume ('acute VP') or represent a chronic gas collection. It can occur within a collapsed vertebral body, the spinal canal, joints but mainly the intervertebral disc. Studies support that VP is originated by a dynamic process involving the balance between tissues' liquid and gaseous components, influenced by the duration and the depth of mechanical and metabolic alterations, by the nature of neighboring tissues and the variability in both pressure and permeability of disc or vertebral or joint structures. Prevalence of VP in the general population is about 2%, reaching 20% in the elderly with disc degeneration. Although it's often a random finding in asymptomatic patients, VP is an eventually painful expression of disc degeneration, or disc or vertebral fracture, or bone lesions. In sporadic cases, intradiscal gas can be expelled (all-in-one or gradually), resulting in a gaseous cyst, causing pain and neurological symptoms. Considering that spontaneous resolution and recurrence after surgery are both possible, most of the authors recommend conservative treatment in patients with intradiscal and intravertebral VP; occasionally percutaneous CT(computed tomography) -guided aspiration or vertebral stabilization.

Abstract:The intervertebral disc (IVD) plays a main role in absorbing and transmitting loads within the spinal column. Degeneration alters the structural integrity of the IVDs and causes pain, especially in the lumbar region. The objective of this study was to investigate non-invasively the effect of degeneration on human 3D lumbar IVD strains ( n = 8) and the mechanism of spinal failure ( n = 10) under pure axial compression using digital volume correlation (DVC) and 9.4 Tesla magnetic resonance imaging (MRI). Degenerate IVDs had higher ( p < 0.05) axial strains (58% higher), maximum 3D compressive strains (43% higher), and maximum 3D shear strains (41% higher), in comparison to the non-degenerate IVDs, particularly in the lateral and posterior annulus. In both degenerate and non-degenerate IVDs, peak tensile and shear strains were observed close to the endplates. Inward bulging of the inner annulus was observed in all degenerate IVDs causing an increase in the AF compressive, tensile, and shear strains at the site of inward bulge, which may predispose it to circumferential tears (delamination). The endplate is the spine's “weak link” in pure axial compression, and the mechanism of human vertebral fracture is associated with disc degeneration. In non-degenerate IVDs the locations of failure were close to the endplate centroid, whereas in degenerate IVDs they were in peripheral regions. These findings advance the state of knowledge on mechanical changes during degeneration of the IVD, which help reduce the risk of injury, optimize treatments, and improve spinal implant designs. Additionally, these new data can be used to validate computational models.

Abstract:With the aging of population, the incidence of osteoporosis and intervertebral disc degeneration increased remarkably.As a consisting part of spine, vertebrae play important roles in spinal diseases.The microstructure of vertebrae is closely associated with not only its biomechanical properties, but also its adjacent intervertebral discs.A detailed understanding of vertebrae's microstructure can help us understand the initial mechanism, progression and prognosis of vertebrae and its adjacent discs' diseases.On the other hand, traditional examinations are less effective in detecting vertebrae's microstructure.Being a noninvasive, high-resolution and bony-sensitive neo-technology, Micro-CT has obvious advantages in detecting vertebrae's microstructure.Many researches have reported the usage of Micro-CT in reveling microstructures while they also raised problems.Thus, herein we searched the related information of vertebrae's microstructure scanning with Micro-CT, explained its basic mechanism, reviewed its progress in technology and arithmetic, and concluded the latest developments.Depending on the microstructure revealed by Micro-CT, achievements in biomechanical properties of vertebrae and intervertebral disc degeneration have been made,and related problems have been summarized.Also, new applications of Micro-CT in spinal diseases are discussed for acquiring reasonable clinical data in the future.

Abstract:Background Chronic low back pain secondary to degenerative changes in the spine is a common cause of disability, and disc degeneration is one of the most frequent imaging findings. Intervertebral vacuum phenomenon (IVP) is usually observed in advanced degeneration. Recently, this phenomenon has gained interest due to a relatively new surgical technique called percutaneous discoplasty, aimed at treating low back pain secondary to degenerative disc disease in elderly patients. Purpose To analyze the prevalence and related factors of the vacuum phenomenon in adult patients. Material and Methods A retrospective cohort study was performed of patients who underwent abdominal computed tomography (CT) for non-spine-related reasons. Age, body mass index, smoking, and CT-based characteristics as presence of IVP, subchondral sclerosis, and facet joint degeneration at the lumbar spine from L1 to the sacrum were included in order to determine the prevalence of the vacuum phenomenon in this population and establish a relationship between this condition and patient demographics and other signs of spine degeneration, such as sclerosis and facet joint disease. Results A total of 238 patients were included in the study (114 men, 124 women; mean age = 75.6 ± 12.3 years. In total, 91 (38%) patients had at least one level of IVP; 59 (25%) patients exhibited subchondral sclerosis, and 235 (98%) facet joint degeneration. Among risk factors, age, smoking, and subchondral sclerosis were significantly associated with the presence of vacuum. Conclusion IVP was present in 38% of participants. Risk factors associated with vacuum were age, smoking, and subchondral sclerosis.

The relationship between MRI and histology in a rat model of intervertebral disc degeneration

Abstract:Objectives: To establish a slowly progressive, reproducible rat model of disc degeneration and investigate the relationship between MRI and histology in the long-term progression of disc degeneration. Materials and methods: The intervertebral discs of caudal spine in rats were punctured laterally, using 20-gauge sterile needles, 5 mm depth from the skin to the middle of the appropriate disc. The discs of tails were analyzed by in vivo MRI and histology before surgery and at 4, 8, 12, and 24 weeks post-surgery. Results: Based on histological grading system, the discs were categorized as normal, moderately degenerated, and severely degenerated, and histologic score of treatment groups was significantly correlated with the time post-stab. MRI measurements showed a progressive decrease in T2 signal intensity and MRI index starting at 4 weeks post-puncture. Furthermore, the degenerated discs did not recover spontaneously, as shown by decrease in T2 signal intensity and MRI index. We found significant correlation between the grade scale of histological score and MRI measurements. Conclusions: We demonstrated a correlation between grade scale of histological score and MRI measurements in a slow and progressive disc degeneration rat model. Therefore, MRI could provide a convenient, non-invasive, reproducible, and cost-effective option to monitor the progression of intervertebral disc degeneration.

Abstract:As the innovation of medical imaging and the development of equipment,the research on the MRI of intervertebral disc degeneration has been greatly improved these years.The new MRI imaging strategies include diffusion weighted imaging,diffusion tensor imaging,magnetic resonance spectroscopy,functional magnetic resonance imaging,dynamic magnetic resonance imaging,T2 relaxometry and spin-lock T1ρ-weighted MRI.Dynamic magnetic resonance imaging can distinguish the active spine;diffusion weighted imaging can quantitatively study the degree of living tissue inland molecular diffusion.It shows potential application value in the aspects of intervertebral disease pathogenesis,effects of non-invasive treatment and prognosis.Diffusion tensor imaging may effectively show early degeneration of intervertebral disc degeneration.Magnetic resonance spectroscopy imaging shows great value of early disc degeneration.There is tiny difference between T2 relaxation time of normal and intervertebral disc degeneration.It needs further study on applying T2 relaxation time in the quantitative analysis of IVDD.All of the above new imaging techniques of MRI have been heatedly studied because they can non-invasively and quantitatively evaluate early intervertebral disc degeneration and provide more reliable evidences of imaging for clinical decisions by stages.This article summarizes the development and prospects of current magnetic resonance imaging of IVDD.

Abstract:Objective To observe the imaging characters of CT-guided ozone target-injecting technic for lumbar intervertebral disk hernia.Methods Thin layer(3mm)CT scanning on intervertebral disk of L4-5(38 cases)and L5-S1(48 cases)was performed in 123 cases with lumbaral intervertebral disk hernia.Puncture approach was choosen according to different types of prominence.O3 was injected and destribution of ozone was observed by means of CT scanning.Results Puncture of L4-5 intervertebral disk via posterolateral route was successsly used in 75 cases.Punture of L5-S1 interveretebral disk via lateral recess route was successsly used in 85 cases.The The accumulative destribution of ozone in disk was seen in 43 cases,crack-like destribution in 64 cases,and diffuse distribution in 62 cases.The disk prominence of 46 cases was greater than 1/3 saggital drive,of which distribution of ozone was inner in 29 cases and on the surface of the prominence in 17 cases.Conclusion The CT-guided ozone target-injection of lumaral intervetebral disk and prominence has the advantages of accurate location,target-injection and easy observation of ozone distribution in diseased area.

Abstract:Objective:To scan and observe the degenerative lumbar facet joints through computed tomography and sum up the pathological changes.Methods:CT images of 88 patients with degenerative facet joints were collected.The facet joint and relative structure were observed.The incidence of every pathological changes were calculated and recorded respectively.Results:The pathological changes were in sequence:joint space narrowing,osteoporosis or sclerosis of the subchondral bone,osteophyte formation,vacuum joint,joint capsule calcification,sagittal articular orientation change,hypertrophy of articular processes,partial dislocation. Conclusion:It's convenient and reliable to observe the structure of the facet joint of lumbar spine through CT scanning,and summarizing pathological changes will be helpful for increasing the knowledge of the degenerative diseases of the lumbar spine.
Symptomatic epidural gas cyst associated with discal vacuum phenomenon

Abstract:Study design: Four cases of computed tomography and magnetic resonance imaging documented lumbar nerve root compression associated with a vacuum disc communicating with the epidural space are presented. Objective: To give an insight into the mechanism of nerve root compression associated with vacuum disc phenomenon. Summary of background data: Few cases of lumbar radiculopathy resulting from epidural gas have been reported and illustrated using cross-sectional imaging studies. Methods: The medical records, imaging examinations, and surgical findings (when available) of 4 symptomatic patients with epidural and intradiscal gas collections were reviewed. Three patients underwent surgery with needle aspiration of the epidural gas, and 1 patient was treated conservatively. Results: Clinical symptoms of epidural gas cyst were similar to those of other more common causes of nerve root compression. Computed tomography and magnetic resonance imaging findings showed intraspinal gas collection associated with a vacuum disc. One patient had an additional vertebral pneumatocyst. Although evacuation of intraspinal gas resulted in complete cure or major neurologic improvement in 3 patients, an initial period of observation is warranted because gas may disappear spontaneously as in 1 of our patients. All 4 patients recovered full motor and sensory functions of the lower extremities. One year later, patients were completely free of symptoms. Conclusion: Although the vacuum disc is considered a common imaging finding of no or not much pathologic significance, occasionally intradiscal gas may leak into the spinal canal. Detection of an epidural gaseous collection at the same level with a vacuum disc strongly suggests a communication between the intervertebral disc and the epidural space. Epidural gas collections can be implicated as a possible cause of radicular symptoms and should be considered in the differential diagnosis of lumbar radiculopathy.
