Abstract:Liquid biopsies have piqued the interest of researchers as a new tumor diagnosis technique due to their unique benefits of non-invasiveness, sensitivity, and convenience. Recent advances in microfluidic technology have integrated separation, purification, and detection, allowing for high-throughput, high-sensitivity, and high-controllability detection of specific biomarkers in liquid biopsies. With the increasing demand for tumor detection and individualized treatment, new challenges are emerging for the ever-improving microfluidic technology. The state-of-the-art microfluidic design and fabrications have been reviewed in this manuscript, and how this technology can be applied to liquid biopsies from the point of view of the detection process. The primary discussion objectives are circulating tumor cells (CTCs), exosomes, and circulating nucleic acid (ctDNA). Furthermore, the challenges and future direction of microfluidic technology in detecting liquid biomarkers have been discussed.

Abstract:Microfluidic-based lab-on-a-chip technology is a multidisciplinary approach, which has evolved rapidly in the past decade and remains a hot research topic as a promising microanalysis platform for a plethora of biomedical applications. Microfluidic chips have been successfully applied in cancer diagnosis and monitoring, given that they can lead to the effective separation and analysis of cancer-derived substances such as extracellular vesicles (EVs), circulating tumour cells (CTCs) and circulating DNA (ctDNA), proteins and other metabolites. In particular, EVs and CTCs are two outstanding objects for cancer liquid biopsy, which share similar membrane structures but possess different sizes. Through molecular typing and concentration detection of EVs, CTCs and ctDNA, disease-related information can be well-learned, including the development stage and prognosis of cancer. However, the conventional separation and detection methods are often time-consuming with limited efficiency. In comparison, the use of microfluidic platforms can effectively simplify the separation and enrichment process and improve the detection efficiency significantly. Although review papers have been published on the application of microfluidic chips for the analysis of objects of liquid biopsy, generally they focused on a specific detection target, lacking a descriptive extraction of the commonality of LOC devices used in liquid biopsy. Thus, few of them present a comprehensive overview and outlook on the design and application of microfluidic chips for liquid biopsy. This motivated us to prepare this review paper, which is divided into 4 parts. The first part aims to elucidate the material selection and fabrication approaches of microfluidic chips. In the second part, the important separation strategies, including physical methods and biological methods, are discussed. The third part highlights the advanced on-chip technologies for the detection of EVs, CTCs and ctDNA by providing practical examples. In the fourth part, novel on-chip applications of single cells/exosomes are introduced. Finally, the prospective outlook and challenges for the long-term development of on-chip assays are envisioned and discussed.

Abstract:Liquid biopsy offers non-invasive and real-time molecular profiling of individual patients, and is thus considered a revolutionary technology in precision medicine. Exosomes have been acknowledged as significant biomarkers in liquid biopsy, as they play a central role in cell-cell communication and are closely related to the pathogenesis of most human malignancies. Nevertheless, in biofluids exosomes always co-exist with other particles, and the cargo components of exosomes are highly heterogeneous. Thus, the isolation and molecular characterization of exosomes are still technically challenging. Microfluidics technology effectively addresses this challenge by virtue of its inherent advantages, such as precise manipulation of fluids, low consumption of samples and reagents, and a high level of integration. Recent advances in microfluidics allow in situ exosome capture and molecular detection with unprecedented selectivity and sensitivity. In this review, the state-of-the-art developments in microfluidics-based exosome research, including exosome isolation approaches and molecular detection strategies, with highlights of the characterization of exosomal biomarkers in cancer liquid biopsy is summarized. The major challenges are also discussed and some perspectives for the future directions of exosome-based liquid biopsy in microfluidic systems are presented.

Emerging Nanotechnologies for Liquid Biopsy: The Detection of Circulating Tumor Cells and Extracellular Vesicles

Abstract:Liquid biopsy enables noninvasive and dynamic analysis of molecular or cellular biomarkers, and therefore holds great potential for the diagnosis, prognosis, monitoring of disease progress and treatment efficacy, understanding of disease mechanisms, and identification of therapeutic targets for drug development. In this review, the recent progress in nanomaterials, nanostructures, nanodevices, and nanosensors for liquid biopsy is summarized, with a focus on the detection and molecular characterization of circulating tumor cells (CTCs) and extracellular vesicles (EVs). The developments and advances of nanomaterials and nanostructures in enhancing the sensitivity, specificity, and purity for the detection of CTCs and EVs are discussed. Sensing techniques for signal transduction and amplification as well as visualization strategies are also discussed. New technologies for the reversible release of the isolated CTCs and EVs and for single-CTC/EV analysis are summarized. Emerging microfluidic platforms for the integral on-chip isolation, detection, and molecular analysis are also included. The opportunities, challenges, and prospects of these innovative materials and technologies, especially with regard to their feasibility in clinical applications, are discussed. The applications of nanotechnology-based liquid biopsy will bring new insight into the clinical practice in monitoring and treatment of tumor and other significant diseases.

Abstract:Applications of droplet‐based microfluidics in liquid biopsy for cancer diagnosis. Liquid biopsy, a noninvasive technique to obtain tumor information from body fluids, is an emerging technology for cancer diagnosis, prognosis, and monitoring, providing crucial support for the realization of precision medicine. The main biomarkers of liquid biopsy include circulating tumor cells, circulating tumor DNA, microRNA, and circulating tumor exosomes. Traditional liquid biopsy detection methods include flow cytometry, immunoassay, polymerase chain reaction (PCR)‐based methods, and next‐generation sequencing (NGS)‐based methods, which are time‐consuming, labor‐intensive, and cannot reflect cell heterogeneity. Droplet‐based microfluidics with high throughput, low contamination, high sensitivity, and single‐cell/single‐molecule/single‐exosome analysis capabilities have shown great potential in the field of liquid biopsy. This review aims to summarize the recent development in droplet‐based microfluidics in liquid biopsy for cancer diagnosis.

Abstract:Liquid biopsy, the technique used to shed light on diseases via liquid samples, has displayed various advantages, including minimal invasiveness, low risk, and ease of multiple sampling for dynamic monitoring, and has drawn extensive attention from multidisciplinary fields in the past decade. With the rapid development of microfluidics, it has been possible to manipulate targets of interest including cells, microorganisms, and exosomes at a single number level, which dramatically promotes the characterization and analysis of disease-related markers, and thus improves the capability of liquid biopsy. However, when lab-ready techniques transfer into hospital-applicable tools, they still face a big challenge in processing raw clinical specimens, which are usually of a large volume and consist of rare targets drowned in complex backgrounds. Efforts toward the sample preparation of clinical specimens (i.e., recovering/concentrating the rare targets among complex backgrounds from large-volume liquids) are required to bridge the gap between the proof-of-concept demonstrations and practical applications. The throughput, sensitivity, and purity (TSP performance criteria) in sample preparation, i.e., the volume speed in processing liquid samples and the efficiencies of recovering rare targets and depleting the backgrounds, are three key factors requiring careful consideration when implementing microfluidic-based liquid biopsy for clinical practices. Platforms based on a single microfluidic module (single-modal microfluidics) can hardly fulfill all the aforementioned TSP performance criteria in clinical practices, which puts forward an urgent need to combine/couple multiple microfluidic modules into one working system (i.e., multi-modal microfluidics, M3) to realize practically applicable techniques for the sample preparation of liquid biopsy. This perspective briefly summarizes the typical microfluidic-based liquid biopsy techniques and discusses potential strategies to develop M3 systems for clinical practices of liquid biopsy from the aspect of sample preparation.

Abstract:Urine biopsy, allowing for the detection, analysis and monitoring of numerous cancer-associated urinary biomarkers to provide insights into cancer occurrence, progression and metastasis, has emerged as an attractive liquid biopsy strategy with enormous advantages over traditional tissue biopsy, such as non-invasiveness, large sample volume, and simple sampling operation. Microfluidics enables precise manipulation of fluids in a tiny chip and exhibits outstanding performance in urine biopsy owing to its minimization, low cost, high integration, high throughput and low sample consumption. Herein, we review recent advances in microfluidic techniques employed in urine biopsy for cancer detection. After briefly summarizing the major urinary biomarkers used for cancer diagnosis, we provide an overview of the typical microfluidic techniques utilized to develop urine biopsy devices. Some prospects along with the major challenges to be addressed for the future of microfluidic-based urine biopsy are also discussed.

Abstract:Assessment of a tumor's molecular makeup using biofluid samples, known as liquid biopsy, is a prominent research topic in precision medicine for cancer, due to its noninvasive property allowing repeat sampling for monitoring molecular changes of tumors over time. Circulating exosomes recently have been recognized as promising tumor surrogates because they deliver enriched biomarkers, such as proteins, RNAs, and DNA. However, purification and characterization of these exosomes are technically challenging. Microfluidic lab-on-a-chip technology effectively addresses these challenges owing to its inherent advantages in integration and automation of multiple functional modules, enhancing sensing performance, and expediting analysis processes. In this article, we review the state-of-the-art development of microfluidic technologies for exosome isolation and molecular characterization with emphasis on their applications toward liquid biopsy-based analysis of cancer. Finally, we share our perspectives on current challenges and future directions of microfluidic exosome analysis.

Abstract:There is growing awareness for the need of early diagnostic tools to aid in point-of-care testing in cancer. Tumor biopsy remains the conventional means in which to sample a tumor and often presents with challenges and associated risks. Therefore, alternative sources of tumor biomarkers is needed. Liquid biopsy has gained attention due to its non-invasive sampling of tumor tissue and ability to serially assess disease via a simple blood draw over the course of treatment. Among the leading technologies developing liquid biopsy solutions, microfluidics has recently come to the fore. Microfluidic platforms offer cellular separation and analysis platforms that allow for high throughout, high sensitivity and specificity, low sample volumes and reagent costs and precise liquid controlling capabilities. These characteristics make microfluidic technology a promising tool in separating and analyzing circulating tumor biomarkers for diagnosis, prognosis and monitoring. In this review, the characteristics of three kinds of circulating tumor markers will be described in the context of cancer, circulating tumor cells (CTCs), exosomes, and circulating tumor DNA (ctDNA). The review will focus on how the introduction of microfluidic technologies has improved the separation and analysis of these circulating tumor markers.

Abstract:Liquid biopsy serves as both a powerful non‐invasive diagnostic tool for early cancer screening and a prognostic tool for monitoring cancer progression and treatment efficacy. Exosomes are promising biomarkers for liquid biopsy, since these nano‐sized extracellular vesicles enrich proteins, lipids, mRNAs, and miRNAs from cells of origin, including cancer cells. Although exosomes are abundantly present in various body fluids, conventional exosome isolation and detection methods that rely on benchtop equipment are time‐consuming, expensive and involve complicated non‐portable procedures. As an alternative, recently developed microfluidic platforms can perform effective exosome separation and detection for liquid biopsy using a single device. Such methods offer advantages of integrity, speed, cost‐efficiency, and portability over conventional benchtop and early microfluidic‐based single‐functional methods which can only separate or detect exosomes separately. These advances have made exosome‐based point‐of‐care (POC) applications possible. This review outlines recent integrated microfluidic‐based exosomal detection strategies to guide future development of such devices for use in liquid biopsy for early cancer screening, prognostic monitoring, and other potential point‐of‐care applications.This article is protected by copyright. Abstract:This review provides a comprehensive review of microfluidic nanoparticle separation for precision medicine, covering the fundamentals of passive, active, and hybrid separation techniques. It focuses on recent advances in their application to liquid biopsy and nanomedicine. Additionally, the review discusses advancements in 3D printing, artificial intelligence, and advanced materials that support the practical translation of these techniques. A deeper understanding of disease heterogeneity highlights the urgent need for precision medicine. Microfluidics, with its unique advantages, such as high adjustability, diverse material selection, low cost, high processing efficiency, and minimal sample requirements, presents an ideal platform for precision medicine applications. As nanoparticles, both of biological origin and for therapeutic purposes, become increasingly important in precision medicine, microfluidic nanoparticle separation proves particularly advantageous for handling valuable samples in personalized medicine. This technology not only enhances detection, diagnosis, monitoring, and treatment accuracy, but also reduces invasiveness in medical procedures. This review summarizes the fundamentals of microfluidic nanoparticle separation techniques for precision medicine, starting with an examination of nanoparticle properties essential for separation and the core principles that guide various microfluidic methods. It then explores passive, active, and hybrid separation techniques, detailing their principles, structures, and applications. Furthermore, the review highlights their contributions to advancements in liquid biopsy and nanomedicine. Finally, it addresses existing challenges and envisions future development spurred by emerging technologies such as advanced materials science, 3D printing, and artificial intelligence. These interdisciplinary collaborations are anticipated to propel the platformization of microfluidic separation techniques, significantly expanding their potential in precision medicine.

Abstract:Liquid biopsy is a technology that exhibits potential to detect cancer early, monitor therapies, and predict cancer prognosis due to its unique characteristics, including noninvasive sampling and real-time analysis. Circulating tumor cells (CTCs) and extracellular vesicles (EVs) are two important components of circulating targets, carrying substantial disease-related molecular information and playing a key role in liquid biopsy. Aptamers are single-stranded oligonucleotides with superior affinity and specificity, and they can bind to targets by folding into unique tertiary structures. Aptamer-based microfluidic platforms offer new ways to enhance the purity and capture efficiency of CTCs and EVs by combining the advantages of microfluidic chips as isolation platforms and aptamers as recognition tools. In this review, we first briefly introduce some new strategies for aptamer discovery based on traditional and aptamer-based microfluidic approaches. Then, we subsequently summarize the progress of aptamer-based microfluidics for CTC and EV detection. Finally, we offer an outlook on the future directional challenges of aptamer-based microfluidics for circulating targets in clinical applications.

Abstract:Liquid biopsy allows for the quick and noninvasive detection of tumor-associated components. Various liquid biopsy components, which include circulating tumor cells, tumor-derived extracellular vesicles, circulating tumor nucleic acids, and tumor-educated platelets, can be analyzed. These components hold great potential for tracking the evolutionary dynamics of tumors and detecting the early emergence of therapeutic resistance and tumor recurrence. In recent decades, significant advances in nanotechnology have contributed to the development of blood-based liquid biopsy with higher sensitivity and specificity. This review discusses the key nanotechnologies that can be used to detect, isolate, and recover tumor-associated components, providing a brief overview of the downstream analysis of these components and then focusing on the different types of nanomaterials that can be applied for the treatment of circulating tumor cells to prevent metastasis. Taken together, these noninvasive nanotechnology-mediated diagnostic approaches can aid precise cancer assessment and guide modifications to current treatment strategies.(c) 2021 Elsevier Ltd. Abstract:Liquid biopsy is transforming cancer diagnosis through noninvasive methods, detecting biomarkers in various biofluids. This review explores its growing role in early detection, continuous monitoring, and personalized therapy. While highlighting its benefits, we address challenges such as accuracy and accessibility and look toward future innovations for broader personalized medicine applications. Liquid biopsy is emerging as a pivotal tool in precision oncology, offering a noninvasive and comprehensive approach to cancer diagnostics and management. By harnessing biofluids such as blood, urine, saliva, cerebrospinal fluid, and pleural effusions, this technique profiles key biomarkers including circulating tumor DNA, circulating tumor cells, microRNAs, and extracellular vesicles. This review discusses the extended scope of liquid biopsy, highlighting its indispensable role in enhancing patient outcomes through early detection, continuous monitoring, and tailored therapy. While the advantages are notable, we also address the challenges, emphasizing the necessity for precision, cost‐effectiveness, and standardized methodologies in its broader application. The future trajectory of liquid biopsy is set to expand its reach in personalized medicine, fueled by technological advancements and collaborative research.

Abstract:Liquid biopsy has emerged in the last ten years as an appealing noninvasive strategy to support early cancer diagnosis and follow-up interventions. However, conventional liquid biopsy strategies involving specified biomarkers have encountered unexpected inconsistencies stemming from the use of different analytical methodologies. Recent reports have repeatedly demonstrated that integrated detection of multiple liquid biopsy biomarkers can significantly improve diagnostic performance by eliminating the influence of intratumoral heterogeneity. Herein, we review the progress in the field of liquid biopsy and propose a novel integrated liquid biopsy framework consisting of three categories: elementary, intermediate, and advanced integration. We also summarize the merits of the integration strategy and propose a roadmap toward refining cancer diagnosis, metastasis surveillance, and prognostication.

Abstract:The selection of circulating tumor cells (CTCs) directly from blood as a real-time liquid biopsy has received increasing attention over the past ten years, and further analysis of these cells may greatly aid in both research and clinical applications. CTC analysis could advance understandings of metastatic cascade, tumor evolution, and patient heterogeneity, as well as drug resistance. Until now, the rarity and heterogeneity of CTCs have been technical challenges to their wider use in clinical studies, but microfluidic-based isolation technologies have emerged as promising tools to address these limitations. This review provides a detailed overview of latest and leading microfluidic devices implemented for CTC isolation. In particular, this study details must-have device performances and highlights the tradeoff between recovery and purity. Finally, the review gives a report of CTC potential clinical applications that can be conducted after CTC isolation. Widespread microfluidic devices, which aim to support liquid-biopsy-based applications, will represent a paradigm shift for cancer clinical care in the near future.

We expected that the in‐depth summary of microfluidic devices–assisted analysis of exosomes will greatly facilitate the discovery of biomarkers, thereby realizing highly efficient and accurate diagnosis, and prediction of cancers. Moreover, in view of the significance of this interesting approach in exosomes‐based liquid biopsy, this review will contribute to the translation of microfluidic technology into clinical applications.

Abstract:Liquid biopsy, a technique used to improve general tumor diagnostic methods, is gradually attracting increasing attention. By determining the temporal and spatial complexity of tumors, it represents a promising solution to diagnose and monitor cancer. It is noninvasive or minimally invasive, sensitive, and convenient and provides specific, accurate, and personal molecular information. Herein, the features of liquid biopsy and its applications in oncology, with a focus on diagnosing early cancers, monitoring prognosis, and minimal residual disease and monitoring treatment responses of cancer, are discussed. Furthermore, an overview of some of the challenges and prospects to translate liquid biopsy into clinically applicable tools for diagnosis and monitoring of cancer is presented.

Abstract:Circulating tumor cells (CTCs) are important biomarkers of liquid biopsy. The number and heterogeneity of CTCs play an important role in cancer diagnosis and personalized medicine. However, owing to the low-abundance biomarkers of CTCs, conventional assays are only able to detect CTCs at the population level. Therefore, there is a pressing need for a highly sensitive method to analyze CTCs at the single-cell level. As an important branch of microfluidics, droplet microfluidics is a high-throughput and sensitive single-cell analysis platform for the quantitative detection and heterogeneity analysis of CTCs. In this review, we focus on the quantitative detection and heterogeneity analysis of CTCs using droplet microfluidics. Technologies that enable droplet microfluidics, particularly high-throughput droplet generation and high-efficiency droplet manipulation, are first discussed. Then, recent advances in detecting and analyzing CTCs using droplet microfluidics from the different aspects of nucleic acids, proteins, and metabolites are introduced. The purpose of this review is to provide guidance for the continued study of droplet microfluidics for CTC-based liquid biopsy.

Abstract:<p>Liquid biopsy, screening cancer non‐invasively and frequently by detecting and quantifying molecular markers in physiological fluids, would significantly improve cancer survival rate but it remains a distant goal. The key obstacles presented by the highly heterogeneous samples are rapid/high‐yield purification and precise/selective marker capture by their antibody and oligo probes. As irregular expressions of these molecular biomarkers are the key signals, quantifying only those from the cancer cells would greatly enhance the performance of the screening tests. The recent discovery that the biomarkers are carried by nanocarriers, such as exosomes, with cell‐specific membrane proteins suggests that such selection may be possible, although a new suite of fractionation and quantification technologies would need to be developed. Although under‐appreciated, membrane microfluidics has made considerable contributions to resolving these issues. We review the progress made so far, based on ion‐selective, track‐etched and gel membranes and advanced electrophoretic and nano‐filtration designs, in this perspective and suggest future directions.</p><p>This article is protected by copyright.
