Abstract:OBJECTIVE:To study the clinical features and the pathogenesis of macular hole with and without retina detachment (RD) in high myopic eyes.METHODS:It was a retrospective series case study. The charts of high myopic patients with macular hole at our hospital from June 2006 to February 2007 were retrospectively reviewed and analyzed. Patients were divided into two groups (the RD group and non-RD group) depending on the presence of RD or not. Their clinical data and optic coherence tomography (OCT) results were further analyzed. SPSS 13.0 was used for the statistic analysis. When comparing the quantitative aspects like age, axial length and refraction, t-test was used. Categorical data, such as sex ratio, occurrence of vitreous traction, posterior staphyloma and retinoschisis were compared by using χ(2) test. Fisher's test was used in comparing eye laterality, incidence of white hole, visual acuity and posterior vitreous detachment (PVD).RESULTS:During this period, there were 43 patients fitting the including criteria. Among them, 36 patents (37 eyes) were in the RD group and 7 patients (7 eyes) in the no-RD group. In the RD group, the average age was 56.1, 24.3% of them (9/37) were male; percentage of left and right eyes was (11/37) and 70.3% (26/37), respectively; average refraction was (-8.9 ± 2.2) D; average axial length was (28.7 ± 2.0) mm. Visual acuity was ≤ 0.05 (72.2%) in 26 eyes and 0.05 - 0.2 (27.8%) in 10 patients. The incidence of complete and non-complete PVD was 89.2% (33/37) and 10.8% (4/37), respectively. White hole presented in 35.1% (3/37) patients. Vitreous traction and retinoschisis presented in 27.0% (10/37) and 35.1% (13/37) patients, respectively. In the non-RD group, the average age was 47.6; 16.7% of them (1/7) were male; left and right eyes were involved in 42.9% (3/7) and 57.1% (4/7), respectively. Average refraction was (-9.0 ± 1.9) D; average axial length was (28.9 ± 1.5) mm. Vision acuity was ≤ 0.05 in 3 patients (42.9%); between 0.05 - 0.2 in 3 eyes (42.9%) and ≥ 0.2 in 1 eye (14.3%). Incidence of complete and non-complete PVD was 85.7% (6/7) and 14.3% (1/7), respectively. White hole was observed in 14.3% (1/7) patients; 42.9% (3/7) patients were accompanied with vitreous traction and 71.4% (5/7) with retinoschisis. B-scan ultrasonography showed posterior staphyloma in all 44 eyes. The results of statistical analysis showed that the gender (χ(2) = 0.008) and eye laterality (χ(2) = 0.449) as well as refraction (t = 0.193), axial length (t = -0.25) and visual acuity (χ(2) = 4.509) of these two groups were similar (P > 0.05). The incidences of vitreous traction (χ(2) = 0.709), white hole (χ(2) = 1.179), PVD (χ(2) = 0.071) and retinoschisis (χ(2) = 3.207) were also similar (P > 0.05). But the age of the non-RD group is significantly younger than the RD group (t = 1.66, P < 0.05).CONCLUSIONS:Various pathogenesis may involved in the occurrence of retinal detachment in highly myopic eyes with macular hole. Further study is required to improve our understanding of this entity.
Preoperative Widefield Swept-Source Optical Coherence Tomography Versus Intraoperative Findings in Detecting Posterior Vitreous Detachment

Abstract:Purpose:To assess the accuracy of swept-source optical coherence tomography (SS-OCT) in detecting complete posterior vitreous detachment (PVD) in comparison with intraoperative findings. Methods:The retrospective study included 145 eyes of 145 consecutive patients who underwent surgery for epiretinal membranes or macular holes. Within a week prior to surgery, PVD status was evaluated by SS-OCT with a depth of field of 3 mm and a capture window of 16 × 8 mm. Complete PVD was identified when the hyaloid condensation was visible clearly on any B-scan or when the vitreous cortex reflectivity was not visible on all 33 B-scans. Sensitivity, specificity, positive predictive value, and negative predictive value of SS-OCT for detection of complete PVD were then compared with those evaluated during a triamcinolone acetonide-assisted vitrectomy. Results:Of the 101 eyes diagnosed as complete PVD by SS-OCT preoperatively, 97 eyes were found to have complete PVD and four eyes were found to have attached vitreous intraoperatively. Of the 44 eyes categorized as attached vitreous by SS-OCT preoperatively, 43 eyes were graded as attached vitreous and one eye was graded as complete PVD during surgery. The sensitivity of SS-OCT for detecting complete PVD was 99.0% and the specificity was 91.5%. The positive predictive value and the negative predictive value were 96.0% and 97.7%, respectively. Conclusions:Widefield (16 × 8 mm) SS-OCT showed high accuracy for the diagnosis of complete PVD in patients with epiretinal membranes or macular holes. Translational Relevance:Widefield SS-OCT has great potential to evaluate PVD status preoperatively and explore the mechanisms of vitreoretinal diseases.
Degradation of Photopic and Mesopic Contrast Sensitivity Function in High Myopes With Partial Posterior Vitreous Detachment

Abstract:Purpose: The purpose of this study was to evaluate the effects of posterior vitreous detachment (PVD) on visual quality in patients with high myopia, as well as investigate the associated factors of photopic and mesopic contrast sensitivity function (CSF) in high myopia. Methods: Visual quality was comprehensively assessed in patients with high myopia. Visual acuity, contrast sensitivity (CS) at four spatial frequencies (3, 6, 12, and 18 cycles per degree [c.p.d.]) under photopic and mesopic conditions, as well as the modulation transfer function cutoff value (MTFcutoff), the objective scatter index (OSI), the Strehl ratio (SR), and internal aberrations, were measured in this cross-sectional study. Results: This study included 94 eyes from 47 subjects with bilateral high myopia, including 23 eyes with complete PVD (cPVD), 21 eyes with partial PVD (pPVD), and 50 eyes without PVD (nPVD). There was no significant difference in visual quality between the cPVD group and the nPVD group. Whereas in eyes with pPVD, there was a degradation of overall photopic CSF (versus nPVD, P = 0.048), photopic CS at 3 c.p.d. (versus cPVD, P = 0.009 and versus nPVD, P = 0.032), photopic CS at 18 c.p.d. (versus nPVD, P = 0.033), overall mesopic CSF (versus nPVD, P = 0.033), and secondary astigmatism (versus cPVD, P = 0.044). Under photopic conditions, the factors affecting CSF were pPVD and SR, whereas the factors affecting mesopic CSF were pPVD, OSI, and ganglion cell-inner plexiform layer thickness. Conclusions: The pPVD impaired visual quality in patients with high myopia compared to nPVD or cPVD, and pPVD could be a factor explaining CSF at both photopic and mesopic illumination. Translational relevance: Clinicians need to closely monitor patients with high myopia with pPVD due to the potential decline in visual quality and the development of vitreoretinal disorders.

Abstract:Background: High myopia is one of the major causes of visual impairment and has an ever-increasing prevalence, especially in East Asia. It is characterized by excessive axial elongation, leading to various blinding complications that extend beyond mere refractive errors and persist immovably after refractive surgery, presenting substantial public health challenge. Main text: High myopia-related complications include lens pathologies, atrophic and tractional maculopathy, choroidal neovascularization, peripheral retinal degenerations and retinal detachment, and glaucoma and heightened susceptibility to intraocular pressure (IOP) elevation. Pathological lens changes characteristic of high myopia include early cataractogenesis, overgrowth of lens, weakened zonules, and postoperative capsular contraction syndrome, possibly driven by inflammatory pathogenesis, etc. Dome-shaped macula and cilioretinal arteries are two newly identified protective factors for central vision of highly myopic patients. These patients also face risks of open-angle glaucoma and IOP spike following intraocular surgery. Morphologic alternations of optic nerve in high myopia can complicate early glaucoma detection, necessitating comprehensive examinations and close follow-up. Anatomically, thinner trabecular meshwork increases this risk; conversely lamina cribrosa defects may offer a fluid outlet, potentially mitigating the pressure. Notably, anxiety has emerged as the first recognized extra-ocular complication in high myopia, with an underlying inflammatory pathogenesis that connects visual stimulus, blood and brain. Conclusions: High myopia induces multiple ocular and potential mental health complications, underscoring the need to develop more effective strategies to improve both physical and emotional well-being of these patients, among which anti-inflammation might possibly represent a promising new target.
Relationship of choroidal thickness and axial length with posterior vitreous detachment in patients with high myopia

Abstract:Abstract Although accumulating evidence suggests a higher prevalence of posterior vitreous detachment (PVD) in highly myopic eyes, the relation between ocular biometric features and PVD stages in such eyes remains unclear. Therefore, we enrolled 170 patients with high myopia (axial length ≥ 26.0 mm) to investigate the status of PVD regarding subfoveal choroidal thickness and axial length. Utilising swept-source optical coherence tomography, we classified the PVD status into five stages. The distribution of PVD grades increased as the choroidal thickness decreased and axial length increased (P < 0.01). On adjusting for age and sex, decreased choroidal thickness and increased axial length were associated with more advanced PVD stages: odds ratios with the highest vs. lowest groups were 0.31 (95% confidence interval [CI] 0.09–1.01; P trend = 0.009) for choroidal thickness and 5.16 (95% CI 1.34–19.80; P trend = 0.002) for axial length. The inverse association between choroidal thickness and PVD status seemed stronger in women than in men (P interaction = 0.05). In conclusion, we firstly observed a significant trend of decreased choroidal thickness, along with increased axial length, with increased grade of PVD, particularly among women with highly myopic eyes, suggesting that advanced morphological myopic changes contribute to PVD in middle-aged adults.

Abstract:Myopic foveoschisis is a disease caused by abnormal vitreoretinal interface status and progressive posterior scleral staphyloma.Its occurrence and development are associated with centripetal traction (posterior vitreous cortex,internal limiting membrane and stiff retinal vessel)and centrifugal traction (increasing axial lengths and posterior scleral staphyloma).Currently vitrectomy is the major option to treat this condition as it can alleviate or eliminate centripetal and centrifugal traction.However as myopic foveoschisis is a life-long progressive degenerative disease,often with abnormalities in retinal pigment epithelium,choroid and sclera;the therapeutic effect of current surgical strategy (vitrectomy or scleral surgery,or combined surgery)is limited and unsatisfactory.A full assessment macular structure,function and related factors before surgery is helpful to predict the anatomical and functional prognosis.

Rhegmatogenous retinal detachment due to paravascular linear retinal breaks over patchy chorioretinal atrophy in pathologic myopia

Abstract:Objective: To characterize posterior paravascular linear retinal breaks over areas of patchy chorioretinal atrophy as a cause of retinal detachment among patients with pathologic myopia. Methods: In this retrospective case series, we evaluated 10 pathologically myopic eyes having rhegmatogenous retinal detachment associated with posterior paravascular linear retinal breaks. Results: Ten eyes with posterior paravascular linear retinal breaks and retinal detachment were identified from January 1, 2008, to July 31, 2009. The retinal breaks were most frequently found along the inferotemporal vascular arcade, followed by the superotemporal arcade. The length of the breaks ranged from 0.25 to 1 disc diameter, and their distance from the optic disc ranged from 1 to 5 disc diameters. These paravascular linear retinal breaks have distinct clinical characteristics, including a strong propensity to occur over areas of patchy chorioretinal atrophy, a linear shape that is oriented parallel to the adjacent retinal vessels, and a tendency to result in progressive retinal detachment. Vitrectomy with gas tamponade was performed in all cases, and retinal reattachment was achieved in 9 cases by a single operation. Conclusions: Paravascular linear retinal breaks over areas of patchy chorioretinal atrophy represent a distinct clinical entity that can result in a special category of retinal detachment among patients with pathologic myopia. These breaks are apt to elude detection before surgery, and a careful search along the posterior vascular arcades during vitrectomy may help to detect these abnormalities. Pars plana vitrectomy with photocoagulation and intraocular tamponade may lead to a resolution of such rhegmatogenous retinal detachments.
Ultra-Widefield Retinal Imaging for Analyzing the Association Between Types of Pathological Myopia and Posterior Staphyloma

Abstract:High myopia may develop to pathologic myopia, which brings severe visual impairment; however, the etiology is not fully understood. We, therefore, investigated the relationship between the presence of posterior staphyloma and posterior ocular disorders by assessing the patients with high myopia. A retrospective study was performed for the patients, who have more than 26 mm of the axial length and of whom fundus photography was taken with an ultra-widefield retinal imaging system. The objectives were 138 cases encompassing 229 eyes. In 138 cases, 91 were bilateral and 47 were unilateral. The averages ± SD of axial length of bilateral and unilateral were 28.8 ± 2.2 mm, 27.3 ± 1.2 mm, respectively, showing statistically significant difference. The number of eyes with and without posterior staphyloma were 107 (46.7%) and 122 (53.3%), respectively. Retinal detachment and retinal breaks are more observed in cases without posterior staphyloma (p = 0.017). Myopic choroidal neovascularization (mCNV) (p = 0.002), chorioretinal atrophy (p < 0.001), retinoschisis (p < 0.001), and optic neuropathy (p = 0.038) are more often seen in cases with posterior staphyloma. In conclusion, the prevalence rates of myopic choroidal neovascularization, retinal choroidal atrophy, and optic neuropathy were significantly higher with posterior staphyloma. The rate of periocular disorders such as retinal detachment was significantly higher without posterior staphyloma. These results indicate associations between types of pathological myopia and presence or absence of posterior staphyloma analyzed by ultra-widefield retinal imaging.

Factors Influencing Vitrectomy in Treating Penetrating Ocular Trauma Involving the Posterior Segment

Abstract:Objective To present the results of vitrectomy in penetrating ocular trauma involving the posterior segment and to analyze factors possibly influencing the final results.Methods Retrospectively reviewed records of 86 patients,87 eyes.Results In these 87 eyes,success was achieved in 67 eyes (77%),20 eyes(23%) were failed.Fourteen to thirty days after injury is the optimal time for surgical intervention.The success rate was lower in patients with pre operative VA0 02(72%) than in patients with pre operative VA≥0.02(11%).Retina detachmaent portended a poor result compared with eyes without retina detachment ( P 0.05). Conclusions Penetrating ocular trauma involving the posterior with good preoperative VA,has good prognosis.Fourteen to thirty days after injury is the best surgical time.Retina detachment combined with poor results.
The effect of posterior vitreous detachment on the prognosis of branch retinal vein occlusion

Abstract:Objective　To determine the effect of posterior vitreous detachment on the prognosis of branch retinal vein occlusion　(BRVO).　Methods　One hundred and sixteen patients (116 eyes) with BRVO who underwent vitreous examination were retrospectively studied.The relationship of vitreous conditions to posterior segment neovascularization and macular edema was statistically investigated.　Results　In 40 ischemic cases,12 of 25 eyes (48.0%) with no posterior vitreous detachment (PVD) or partial PVD developed retinal or optic disc neovascularization ,or both,but only one of the 15 eyes (6.7%) with complete PVD developed neovascularization during a mean follow-up period of 10.7±2.2 months　(P＜0.05).Diffuse macular edema was found in 45 eyes　(38.8%).The incidence of macular edema was significantly higher in eyes with vitreomacular attachment　(51.5%)　than in those with vitreomacular separation　(22.0%)(P＜0.01).　Conclusion　It was suggested that complete PVD may play a role in protecting eyes with BRVO from posterior segment neovascularization and macular edema.

Abstract:Highly myopic optic nerve head (ONH) abnormalities encompass a series of complications resulting from the stretching of papillary and peripapillary structures during significant axial elongation. The morphological changes in the ONH typically initiate with disk tilting or rotation, progressing to PHOMS and PPA. Tissue defects in each layer manifest as focal lamina cribrosa defects (FLDs), peripapillary intrachoroidal cavitations (PICCs), and acquired pits of the optic nerve (APON). Anterior vitreous/vascular traction and posterior scleral protrusion may lead to prelaminar schisis as well as paravascular cysts and holes, which can potentially develop into retinoschisis. Traditional color fundus photography (CFP) is often insufficient for visualizing most of these lesions, yet their description and quantification benefit significantly from the advancements in optical coherence tomography (OCT) and OCT angiography (OCTA), complemented by fundus autofluorescence (FAF), indocyanine green angiography (ICGA), and three-dimensional imaging. The effective diagnosis and classification of ONH abnormalities heavily rely on a comprehensive understanding of their multimodal imaging features, as outlined in this review. These findings provide valuable insights into optic neuropathy in high myopia, establishing a solid foundation for future endeavors in disease monitoring and treatment guidance.

The Visual Outcome and Related Factors in Cataract Patients with Extreme Myopia

Abstract:OBJECTIVE:A prospective study was performed to analyze the visual outcome and factors related to the visual outcome in cataract patients with extreme myopia, in order to provide a principle for intraocular lens (IOL) diopter chosen.METHODS:43 cases were included in the present series (myopia over 10.00 D, axial length over 27.00 mm, with clear cornea and slight or moderate opacity of the lens). Distance and near visual acuity (NVA), best corrected visual acuity and the fundus were examined before and after the phacoemulsification and implantation of IOL. chi(2) test and stepwise regression analysis were used to identify the predictive factors.RESULTS:(1) Distance visual acuity, NVA and best corrected visual acuity were markedly improved in most cases. (2) NVA and fundus pathologic changes were two important factors for predicting post-operative visual acuity. (3) The prognosis was better in cases with NVA better than 20/200, and in cases with axial length below 29.00 mm. (4) Some cases with postoperative NVA worse than 20/80 complained of difficulty in reading.CONCLUSIONS:NVA is an important factor influencing the outcome of vision and is an important parameter for predicting the outcome of vision in cataract patients with extreme myopia. IOL diopter should be determined carefully by preoperative NVA and the patients' demand for near work.
Etiological factors and clinical outcomes in Non-Traumatic Non-Diabetec retinopathic vitreous hemorrhage

Abstract:Objective To analyze the etiological factors in non-traumatic non-diabetec retinopathic vitreous hemorrhage and to investigate the clinical outcomes of vitrectomy in such patients.Methods The etiologic factor and recovery rate of visual functions in a total of 174 patients (176 eyes) undergoing vitrectomy with follow-up records from March 2006 to July 2009 in our department were analyzed retrospectively.Results Vitreous hemorrhage was found in 73 eyes of retinal vein occlusion (RVO),25 eyes of Eales' disease,25 eyes of retinal hole or retinal detachment (RH/RD),22 eyes ofexudative age-related macular degeneration (EAMD),and 15 eyes ofpolypoidal choroidal vasculopathy (PCV),amounting to 90.9% of total eyes.The visual improvement rate after vitrectomy was more than 90%.Conclusions RVO,Eales' disease,RH/RD,EAMD and PCV are main causes of non-traumatic non-diabetec retinopathic vitreous hemorrhage.Vitrectomy is an effective way for vitreous hemorrhage.
Long-Term Prediction and Risk Factors for Incident Visual Field Defect in Nonpathologic High Myopia

Abstract:Purpose: To investigate the long-term patterns and risk factors of visual field defect (VFD) development in nonpathologic high myopia (HM) over an 8-year follow-up. Methods: This was an observational cohort study. The VFD classification adhered to the Glaucoma Suspects with High Myopia Study Group. Logistic regression models with generalized estimating equations were used to identify risk factors for VFD development. Results: A total of 330 eyes from 194 patients were included. Among them, 49.4% of eyes developed VFD, with enlarged blind spot and nonspecific defect ranked as the most common VFDs, followed by partial arcuate defect, vertical step, nasal step, paracentral defect, and combined defects. Longer axial length (odds ratio [OR] = 1.43 per 1-mm increase; 95% CI, 1.04-1.95; P = 0.026), thinner central corneal thickness (OR = 1.01 per 1-µm decrease; 95% CI, 1.003-1.02; P = 0.013), worse mean deviation of visual field (OR = 1.51 per 1-dB decrease; 95% CI, 1.14-2.00; P = 0.004), and the presence of peripapillary γ-zone (OR = 5.57; 95% CI, 3.06-10.15; P < 0.001) at baseline correlated with the development of any VFD. By incorporating these factors, the prediction models achieved area under the curves of 0.789 (95% CI, 0.726-0.853) and 0.828 (95% CI, 0.714-0.943) for discriminating the development of any VFD and moderate/severe VFD, respectively, with good calibration power. Conclusions: The development of VFD occurred frequently in individuals with nonpathologic HM and can be effectively predicted using relevant metrics. The findings will aid in expanding our knowledge of optic neuropathy in HM.
Vitrectomy Outcomes in Eyes with High Myopic Macular Hole Without Retinal Detachment.

Abstract:Purpose: The purpose of this study is to evaluate the vitrectomy outcomes for high myopic patients with macular hole (MH) without retinal detachment and to compare these outcomes with those of control patients with idiopathic MH undergoing similar surgeries by the same surgeon.Methods: The study was designed as a matched, case-control, retrospective chart review. We performed a retrospective chart review of consecutive cases that underwent vitrectomy for MH without retinal detachment from 2000 through 2008. Twenty-four eyes of 24 patients were selected to participate in this study. They were divided into 2 groups: 12 study eyes with high myopic MH without retinal detachment and 12 control eyes with idiopathic MH. The main outcomes were preoperative, postoperative best-corrected visual acuity (BCVA), MH closure rates, and complications in both groups.Results: There was a closure rate of 100% in both the study and control groups, and no cases of reopening were reported during the follow-up visits. After surgery, the mean BCVA of the study group remained stable. There was no significant improvement of mean BCVA after surgery (P = 0.35) in the study group. The mean BCVA of the control group improved significantly (P = 0.00) after surgery. The mean postoperative BCVA in the study group was significantly lower than that in the control group in a comparison using the Student's t-test (P = 0.01). In the study group, the BCVA improved or remained stable after surgery in 8 eyes (67%). In the control group, the BCVA of all eyes improved or remained stable. The proportion of eyes with BCVA improvement after surgery was significantly higher in the study group than in the control group (P = 0.03). Disappearance of the outer nuclear layer and photoreceptor layer in the foveola and thinning of the foveola was showed in the postoperative optical coherence tomography images of four eyes in the study group while in none of the eyes in the control group.Conclusion: In summary, vitrectomy results in satisfactory anatomical improvement in patients with high myopic MH without retinal detachment. However, the visual acuity outcomes of the high myopic MH patients without retinal detachment were poorer than those for patients with idiopathic MHs. RETINA 32:275-280, 2012
Association Between Corneal Biomechanical Properties and Myopia in Chinese Subjects

Abstract:To examine the relationship between corneal biomechanical properties and the degree of myopia. Chinese subjects (n=172, age: 11−65 years) were divided into diagnostic groups with non-myopia (spherical equivalence (SE)>−0.50 D), low (−3.00≤SE≤−0.50 D), moderate (−6.00≤SE<−3.00 D), and high myopia (SE<−6.00 D). Only the right eye of each subject was analyzed. Central corneal thickness (CCT) was measured by optical coherence tomography. An ocular response analyzer was used to measure corneal hysteresis (CH), corneal resistance factor (CRF), intraocular pressure (IOP), and corneal compensated IOP (IOPcc). Refraction was measured by both automated and subjective refractometry and expressed as SE. CH was significantly lower in high myopia compared with both low and non-myopia (P≤0.002). CCT was 1.5 times more correlated to CH variation compared with refraction. Similarly, CRF was four times more dependent on CCT than refraction. CH (P<0.001) or CRF (P=0.005) was positively correlated to refraction. Both IOP and IOPcc were negatively correlated to refraction (P<0.001), respectively. CH decreases only in high myopia. Refraction is positively correlated to both CH and CRF but negatively correlated to both IOP and IOPcc. These results indicate that the mechanical strength in anterior segment of the eye is compromised in high myopia. In addition, high myopia may increase the risk of glaucoma.

Abstract:Objective The objective is to use optical coherence tomography ( OCT) to observe the features of the retinoschisis in the fundus posterior pole of high myopia with posterior staphyloma, and to analyze the reasons to causthe disease.Methods The research is in the way of non-intervene and observation.212 eyes for more than 6.00D in 126 ca-ses were checked with type-B ultrasonic to find the patients who have posterior staphyloma.Further checks were made to them with split lamp, fundus check, mydriasis funduscopy check, Type-A ultrasonic check and OCT check.Then the OCT imagine with retinoschisises are analyzed.Results 37 eyes among the 212 eyes were found the the retinoschisis in fundus posterior pole of high myopia with posterior staphyloma.The location of the retinoschisis is corresponding to the location of the posterior staphyloma.Among the 37 eyes 26 eyes have single or interior and/or middle-level retinoschisis.29 eyes of the 37 eyes have the phenomenon of having vitreous body pull the retinas, including 22 eyes to have the location of the pos-terior hyaloids membrane be corresponding to the highest points of the retinoschisis or retinal detachment.Four eyes have macular holes and two eyes of them have full-thickness ceasma holes and two have pseudo-macular holes.The 175 eyes which have no retinoschisis shows that the thickness of their macular epiretinal membrance is negative corresponding to the eye axial lengths.Conclusion The retinoschisis has relationship with the vitreous body of the fundus posterior pole of high myopia with posterior staphyloma, pulling force of the internal limiting membrane and the stretching of the posterior staphy-loma.The shrinking of the choroid membrances for the high myopia weakens the closeness of the layers of the retinas to make the retinas be easy to form retinoschisis.The hight myopia people should have a regular check with OCT to have the former check and diagnosis of retinoschisis and/or retinal detachment.

Abstract:Background To determine the levels of connective tissue growth factor (CTGF) and hepatocyte growth factor (HGF) in the vitreous of patients with high myopia, in comparison with those with a vitreomacular interface disease (VMID). Methods Patients with either high myopia (high myopia group) or a VMID (VMID group) were included in this study. Each of the two groups were further subdivided into two subgroups: group A (high myopia with macular hole), group B (high myopia with macular retinoschisis), group C (idiopathic macular hole), and group D (idiopathic epiretinal membrane). Vitreal specimens were collected during vitrectomy, and enzyme-linked immunosorbent assay was used to quantitatively measure the CTGF and HGF levels in the vitreous. Results The average axial length was markedly longer in the high myopia group than in the VMID group. The vitreal CTGF level was significantly higher in the high myopia group than in the VMID group. Subgroup analysis revealed significantly higher vitreal CTGF in group A than in the other three subgroups. The vitreal HGF level was not significantly different between the high myopia and VMID groups, but was significantly higher in group D than in group C in the subgroup analysis. Correlation analysis showed that the vitreal CTGF level was positively correlated with the axial length. Conclusions The vitreal CTGF level is elevated in highly myopic eyes and may be related to the pathogenesis of high myopia, whereas increased expression of HGF may be involved in the development of idiopathic epiretinal membrane.
