Abstract:OBJECTIVE To investigate the effect of inhaled nitric oxide (NO) on surfactant protein A (SP-A) and mannose binding ability (MBA) in neonatal rats with hyperoxia-induced lung injury. METHODS Sixty-four neonatal rats were randomly exposed to room air (Control group), >95% oxygen for 6 days (Hyperoxia group), 10 ppm NO for 24 hrs (NO group), and >95% oxygen for 6 days along with 10 ppm NO for 24 hrs (Hyperoxia + NO group). After 2 and 6 days of exposure, the lung pathologic changes, gene and protein expressions of SP-A and MBA were measured. RESULTS The rats from the Hyperoxia group presented with obvious lung injuries. The SP-A expressions of mRNA (0.81 +/- 0.04 vs 1.53 +/- 0.25) and protein (59.45 +/- 18.37 vs 89.77 +/- 16.41) in the Hyperoxia group decreased significantly 2 days after exposure but increased significantly 6 days after exposure (SP-A mRNA 0.81 +/- 0.02 vs 0.63 +/- 0.03; SP-A protein 93.57 +/- 13.71 vs 47.73 +/- 21.69) compared with those of the Control group (P < 0.05). NO treatment alleviated the hyperoxia-induced pathologic injuries 2 days after exposure. The SP-A mRNA expression (0.55 +/- 0.91) in the Hyperoxia + NO group was significantly reduced as compared to both the Control and Hyperoxia groups (P < 0.05), and the SP-A protein expression (55.12 +/- 17.53) in the Hyperoxia + NO group was noticeably lower than that of the Control group (P < 0.01) 2 days after exposure. The SP-A protein expression in the Hyperoxia + NO group (67.33 +/- 18.59) was significantly lower than that of the Hyperoxia group 6 days after exposure (P < 0.05). Two days after exposure, the NO group had significantly higher MBA than the Control group (0.821 +/- 0.133 vs 0.58 +/- 0.158); the Hyperoxia + NO group had significantly higher MBA than the Hyperoxia group (0.43 +/- 0.175 vs 0.738 +/- 0.141) (P < 0.05). CONCLUSIONS Inhaled low dose NO may decrease SP-A protein expression and increase MBA of the lung tissue. This lessens the pathologic lung injury in neonatal rats with hyperoxia.
Effect of inhaled nitric oxide on aquaporin expression in newborn rat lung in acute hyperoxic lung injury

Abstract:AIM: To investigate the effect of inhaled nitric oxide on aquaporin expression and alveolar epithelial fluid transport in newborn rats with acute hyperoxic lung injury. METHODS: 32 newborn SD rats were randomized to breathe for 48 h room air (C), >95%O2 (O), >95%O2+10-5 NO (NO only in the first 24 h, ONO), room air + NO (CN). Then, the rats were killed, the lung wet-to-dry weight ratio (QW/QD), the histology, and AQP1, AQP5, α1-NKA, α-ENaC mRNA expressions in the lungs were measured. RESULTS: Compared with C group, the QW/QD in O group significantly increased (P<0.01), and AQP1 mRNA expression decreased significantly (P<0.01). Compared with O group, ONO group had a lower level of QW/QD (P<0.05), and AQP1 mRNA expression increased (P<0.05). AQP5 mRNA expression in all groups remained unchanged. CONCLUSION: In newborn rats with acute hyperoxic lung injury, inhaled 10-5 nitric oxide for 24 h may attenuate lung edema and increase AQP1 mRNA expression, suggesting that inhaled 10-5 nitric oxide for 24 h may promote the AQP1 expression in lung in this model of acute lung injury.

Abstract:Objective To evaluate the protective effect and mechanism of recombinant humansuperoxide dismutase (rhSOD) combined with inhaled nitric oxide (iNO) in infant rats with meconium aspiration acute lung iujury(ALI) .Methods 32 healthy male infant Sprage-Dawley rats were used. The model of ALI was established by infusing lml/kg of 20% newborn meconium suspension through endotracheal tube,and then randomized to 4 groups (8 in each): (1) Control group (C), exposure to room air; (2) iNO group,exposure to 20×10 -6 nitric oxide; (3) SOD group, infusing 20mg/kg rhSOD dissolved in l ml/kg 0 9% saline through endotracheal tube, followed by exposure to room air; (4) iNO/ SOD group,treatment with both 20mg/kg rhSOD and 20×10 -6 iNO. The animals were killed after 24h of treatments. The measurements included bronchoalveolar lavage fluid (BALF) cell counts, pulmonary myeloperoxidase (MPO) activity,malonyldialdehyde (MDA) and nitric oxide (NO) levels. Lung injury score was also evaluated.Results Compared with the C group, the rats in the iNO group, SOD group and iNO/SOD group had decreased BALF cell counts, MPO activity and lung injury score ( P 0 05 or 0 01 ) .Moreover, compared with the iNO group, iNO/SOD group had further decreased,BALF cell counts and MPO activity (all P 0 05) .Compared with the C group, pulmonary MDA and NO levels were decreased significantly in both SOD group and iNO/SOD group ( P 0 05 or 0 01 ) but not in the iNO group (all P 0 05).The iNO/SOD group hadlower pulmonary MDA and NO level than iNO group ( all P 0 05 ) .Conclusion 20mg/kg rhSOD intra-traeheal administration could enhance the effect of iNO in infant rats with meconium aspiration lung injury.
Inhaled Nitric Oxide in Preterm and Term Neonates with Hypoxemic Respiratory Failure and Persistent Pulmonary Hypertension

Abstract:AIM: To assess the effects of inhaled nitric oxide (iNO) as a rescue therapy for term and preterm neonates with hypoxemic respiratory failure (HRF). METHODS: Data were collected from retrospective studies at neonatal service of two university children's hospitals. Response to iNO at 5-20 ppm over time and outcome were assessed. RESULTS: Term, (n=40) and preterm (n=35) neonates did not differ in age at entry, initial or maintained iNO dose and duration, baseline blood pH and PaCO2. Oxygenation index (OI) at baseline was higher in the term, and significantly decreased after 30 min, 3 and 24 h after iNO. This effect was seen only at 24 h in the preterm. The decline in time weighted OI (TWOI) at 24 h of the treatment was greater in the term than in the preterm (P<0.01). In the subgroup neonates with baseline pH greater than or equal to 7.25, significant decrease in OI was found at 30 min, 3 and 24 h of iNO, but only at 24 It for the subgroup with pH<7.25 (P<0.05). Intracranial or pulmonary hemorrhage was found mainly in the preterm neonates. The survival was 65 % in all the patients. CONCLUSION: iNO improved oxygenation in both term and preterm neonates with HRF, and its response was in favor of term infants or those with high blood pH values at study entry.
Nitric oxide inhalation inhibits inducible nitric oxide synthase but not nitrotyrosine formation and cell apoptosis in rat lungs with meconium-induced injury

Abstract:Aim: To investigate the effects of inhaled nitric oxide (NO) on pulmonary inflammation, apoptosis, peroxidation and protein nitration in a rat model of acute lung injury (ALI) induced by meconium. Methods: Twenty-four healthy male Sprague-Dawley rats were randomly devided into 3 groups ( n =8): meconium-induced ALI with intratracheal instillation of 1 mL/kg saline (Mec/saline group), continuous inhalation of NO at 20 μL/L. (Mec/iNO), and the control group (control). Electromicroscopic examination was used to determine the extent of epithelial apoptosis. TUNEL was used to detect DNA fragmentation in pulmonary apoptotic cells, expressed as the apoptosis index (AI). Western blotting was used to detect pulmonary inducible NO synthase (iNOS) expression. RT-PCR was used to detect interleukin (IL)-1β mRNA expression. Cell count inbronchoalveolar lavage (BAL), myeloperoxidase (MPO) activity, as well as malondialdehyde (MDA) and nitrotyrosine formation, the markers of toxic NO-superoxide pathway in rat lung parenchyma specimens, were also examined. Results: Expression of iNOS protein and IL-1β mRNA were increased significantly in the Mec/saline group (both P <0.01) compared with the control group. BAL cell count, MPO activity, lung injury score, pulmonary AI, MDA level and nitrotyrosine formation were also increased significantly (all P <0.01). The meconium-induced iNOS protein and IL-1β mRNA expression were inhibited significantly by NO inhalation when compared with the Mec/saline group (both P <0.01). BAL cell count, MPO activity and lung injury score were also decreased significantly ( P <0.01 or P <0.05). However, there were no statistical differences in MDA level, nitrotyrosine formation or pulmonary AI between the Mec/saline and Mec/iNO groups. Electromicroscopic examination revealed a significant degree of epithelial apoptosis in both the Mec/saline and Mec/iNO groups. Conclusions: Early continuous inhalation of NO 20 μL/L may protect the lungs from inflammatory injury, but does not decrease epithelial apoptosis or lung nitrotyrosine formation. Inhalation of NO alone is not associated with a detectable increase in oxidant stress.
Caffeine Prevents Hyperoxia-Induced Lung Injury in Neonatal Mice Through NLRP3 Inflammasome and NF-κB Pathway.

Abstract:Background Bronchopulmonary dysplasia (BPD) is a common chronic lung disease in premature infants and hyperoxia exposure is a major cause. In hyperoxic lung injury animal model, alveolar simplification and pro-inflammatory cells infiltration are the main pathophysiologic changes. Caffeine is a drug used to treat apnea in premature infants. Early use of caffeine can decrease the rate and the severity of BPD while the mechanisms are still unclear. The purpose of this study was to evaluate the effects of caffeine on inflammation and lung development in neonatal mice with hyperoxic lung injury and to explore the possible mechanism. Methods Following 14 d of 75% oxygen exposure in newborn mouse, the BPD model was established. Caffeine at a dose of 1 g/L was added in drinking water to nursing mouse. We measured the concentration of caffeine in serum and oxidative stress in lung by commercially available kits. Adenosine 2A receptor (A 2A R) expression and lung inflammation were measured by Immunohistochemistry and western blotting. Apoptosis and surfactant protein-C (SFTPC) levels were measured by immunofluorescence. The inflammasome and NF-κB pathway proteins were assessed by western blotting. Results We found that the caffeine concentration in plasma at present dose significantly decreased the expression of A 2A R protein in mice lung. Caffeine treatment significantly reduced oxidative stress, improved weight gain, promoted alveolar development, attenuated inflammatory infiltration and lung injury in hyperoxia-induced lung injury mice. Moreover, caffeine decreased the cell apoptosis in lung tissues, especially the Type II alveolar epithelial cell. The expression of NLRP3 inflammasome protein and NF-κB pathway were significantly inhibited by caffeine treatment. Conclusion Caffeine treatment can protect hyperoxia-induced mice lung from oxidative injury by inhibiting NLRP3 inflammasome and NF-κB pathway.

Abstract:We hypothesized that inhaled nitric oxide (iNO), a selective vasodilator for pulmonary hypertension, may exacerbate hyperoxia-related lung inflammatory injury by alteration of phosphatidylcholine (PC) synthesis in mature lungs. Healthy adult rats were allocated to 4 groups and exposed to: 95% oxygen, or 20ppm iNO, or both (ONO), or room air, all for 48h. 3H-choline chloride was injected i.v. at 10min, 8, 16, and 24h prior to the end of 48h exposure and the animal lungs were processed. In oxygen group, oxidative damage and inflammation were significantly induced compared to the room air group. In ONO group there were significantly elevated glutathione, attenuated malondialdehyde, myeloperoxidase, and wet-to-dry lung weight ratio in lung parenchyma, decreased white cell counts and vascular-to-alveolar leakage of albumin in bronchoalveolar lavage fluid. In both oxygen and ONO groups both total phospholipids and surfactant protein-A were significantly increased compared with the room air group. Newly synthesized 3H-PC was low in the lungs of NO group but high over time in both oxygen and ONO groups. Morphologically, lung injury was mild in ONO, but moderate in both oxygen and NO groups. We conclude that iNO alleviated oxidative damage and inflammation, and reduced alveolar leakage in hyperoxic injury of the mature lungs. Hyperoxia enhanced production of surfactant, whereas iNO did not attenuate this effect.
Inhaled Nitric Oxide Alleviates Hyperoxia Suppressed Phosphatidylcholine Synthesis in Endotoxin-Induced Injury in Mature Rat Lungs

Abstract:Abstract Background We investigated efficacy of inhaled nitric oxide (NO) in modulation of metabolism of phosphatidylcholine (PC) of pulmonary surfactant and in anti-inflammatory mechanism of mature lungs with inflammatory injury. Methods Healthy adult rats were divided into a group of lung inflammation induced by i.v. lipopolysaccharides (LPS) or a normal control (C) for 24 h, and then exposed to: room air (Air), 95% oxygen (O), NO (20 parts per million, NO), both O and NO (ONO) as subgroups, whereas [3H]-choline was injected i.v. for incorporation into PC of the lungs which were processed subsequently at 10 min, 4, 8, 12 and 24 h, respectively, for measurement of PC synthesis and proinflammatory cytokine production. Results LPS-NO subgroup had the lowest level of labeled PC in total phospholipids and disaturated PC in bronchoalveolar lavage fluid and lung tissue (decreased by 46–59%), along with the lowest activity of cytidine triphosphate: phosphocholine cytidylyltransferase (-14–18%) in the lungs, compared to all other subgroups at 4 h (p < 0.01), but not at 8 and 12 h. After 24-h, all LPS-subgroups had lower labeled PC than the corresponding C-subgroups (p < 0.05). LPS-ONO had higher labeled PC in total phospholipids and disaturated PC, activity of cytidylyltransferase, and lower activity of nuclear transcription factor-κB and expression of proinflammatory cytokine mRNA, than that in the LPS-O subgroup (p < 0.05). Conclusion In LPS-induced lung inflammation in association with hyperoxia, depressed PC synthesis and enhanced proinflammatory cytokine production may be alleviated by iNO. NO alone only transiently suppressed the PC synthesis as a result of lower activity of cytidylyltransferase.
CXCR2 Blockade Reduces Radical Formation in Hyperoxia-Exposed Newborn Rat Lung

Abstract:Inflammation contributes greatly to the pathogenesis of bronchopulmonary dysplasia. In previous studies, we showed that blocking neutrophil influx by treatment with SB265610, a selective CXCR2 antagonist, could partly reduce superoxide accumulation and preserve alveolar development in 60% O 2 -exposed newborn rats. The purpose of this study was to further investigate the role of neutrophils in the formation of reactive oxygen and nitrogen species mediating hyperoxia-impaired lung development. We found that hydroxyl radical formation and lipid peroxidation in rat lungs were significantly increased during 60% O 2 exposure. These increases were attenuated by the administration of SB265610. In addition, SB265610 largely inhibited protein nitration induced by hyperoxia. SB265610 partly prevented the hyperoxia-enhanced bronchoalveolar lavage (BAL) protein content in 60% O 2 -exposed animals. Our results demonstrate that neutrophils have a pivotal role in hydroxyl radical formation, lipid peroxidation and protein nitration. Taken together with our previous studies, the present findings show that blocking neutrophil influx protects alveolar development and improves lung function in part by preventing reactive oxygen/nitrogen species accumulation.
Inhaled nitric oxide and neonatal brain damage: experimental and clinical evidences.

Abstract:Inhaled nitric oxide (iNO) has been used not only for pulmonary vasodilation in term neonates with hypoxemic respiratory failure, but also in preterm ones at risk of chronic lung disease (CLD) with variable results in prevention and treatment of CLD and/or brain injury. However, meta analysis of clinical trials does not support that iNO should be used routinely in preterm infants with hypoxic respiratory failure as it has no convincing long-term follow-up data to show its advantages in neurodevelopment. Investigation of extra-pulmonary effects of iNO through nitrosothiol hemoglobin-associated hypoxic vasodilation, as well as its intra-and extra-pulmonary anti-inflammation effect, would have biological and physiological potential in the management of the lung and brain injury of prematurity. The eligibility and safety of iNO in these premature infants at high risk of neurodevelopmental disability require more clinical and follow-up effort to test its pharmacological benefit over harm.
Effect of Inhalation of Nebulized No Donor Substance on Acute Hypoxic Lung Injury in Newborn Piglets

Abstract:BACKGROUND:Birth asphyxia may result in multiple organ dysfunction such as lung injury. Inhalation of nebulized nitric oxide precursor can selectively reduce pulmonary hypertension. However, it is unknown whether such precursors can alleviate lung injury induced by hypoxia. We evaluated the effect of inhalation of nebulized nitroglycerine and sodium nitroprusside on acute hypoxic lung injury in newborn piglets.METHODS:Acute hypoxic lung injury was induced by inspiring 10% O2 for 1 hour. Twenty-four anaesthetized and mechanically ventilated piglets (5-7 days old) were randomly divided into four groups: (1) group S, not hypoxic; (2) group C, nebulized saline after hypoxia; (3) group NTG, nebulized nitroglycerine after hypoxia; (4) group SNP, nebulized sodium nitroprusside after hypoxia. Respiratory dynamic compliance and resistance of respiratory system were recorded at baseline, 0.5 hour and 1 hour of hypoxia; then 0.5 hour, 1 hour, 3 hours and 5 hours following hypoxia. After nebulization, arterial blood was collected for measuring methaemoglobin and nitrate/nitrite levels. Right lung tissue, wet-dry ratio and myeloperoxidase level were determined. White blood cell count (WBC), total surfactant phospholipids (TPL) and disaturated phosphatidyl choline (DSPC) of the bronchoalveolar lavage fluid (BALF) were calculated. Left lungs were used for examining pathological changes.RESULTS:No significant difference was observed in respiratory dynamic compliance, resistance of respiratory system, wet-dry ratio, levels of methaemoglobin and nitrate/nitrite after nebulization, TPL or DSPC/TPL among four groups. WBC in BALF in groups NTG and SNP significantly decreased as compared with group C: similarly for myeloperoxidase level in lung tissue. Lung histological findings showed infiltration of neutrophils in groups NTG and SNP decreased significantly as compared with group C.CONCLUSION:Inhalation of nebulized nitroglycerine or sodium nitroprusside can alleviate the infiltration of neutrophils, while it affects neither the metabolism of phospholipids nor water content in the lungs.
Experimental Research and Clinical Application of Inhaled Nitric Oxide in Premature Neonates

Abstract:One of the new indications of inhaled nitric oxide(NO) now is focused on efficacy of prevention and treatment of chronic lung disease(CLD) in prematurity.Results from large-sample randomized trials of single or multicenter based studies revealed that NO therapy reduced incidence of CLD as well as brain damage,especially in those with birth weight above 1000 g.The dosage of inhaled NO in most of the studies was below 5 parts per million,and usually supplied continuously for 1-2 weeks.As more evidence of safety is required based on follow-up studies,this therapy is not recommended for routine use.However,in patients with CLD and ventilator dependence,or patients at high risk of ventilator-associated pneumonia,NO therapy can be considered for investigational use only.
Inhaled Nitric Oxide Therapy for Hypoxic Respiratory Failure in Neonates

Abstract:Objective To evaluate the effects of inhaled nitric oxide (iNO) in hypoxic respiratory failure in neonates. Methods Tweenty eight full term and preterm newborns requiring mechanical ventilation for hypoxic respiratory failure, with oxygenation index (OI) 25, were received iNO at a concentration of 3~15 ppm. Results Before iNO, mean FiO 2 was 0.80±0.15, SpO 2 was (0.74±0.13) and OI was 26±16. At 3 and 24 hours after iNO, FiO 2 decreased to 0.72±0.15 and 0.52±0.11 (both P 0.001), SpO 2 increased to 0.91±0.05 and 0.93±0.04 ( P 0.001), and mean OI decreased to 17±9 ( P 0.05) and 12±5 ( P 0.01), respectively. Pumonary arterial pressure was decreased from (55±10) mmHg before iNO to (39±8) mm Hg 24 h after iNO ( P 0.001). No adverse effect of iNO was detected in respect with methemoglobin level and nitrogen dioxide formation.Conclusion Inhaled NO effectively alleviated hypoxic respiratory failure in the term and preterm neonates mainly through selective pulmonary vasodilation, without causing methemoglobinemia and other potential complications.
Influence of Hyperoxia on Ultrastructure in Damaged Lungs of Neonatal Rats

Abstract:Objective To explore influence of hyperoxia on ultrastructure in damaged lungs of neonatal rats.Methods 48 SpragueDawley neonatal rats were randomly divided into hyperoxia experiment group(HG,n=24) and air control group(AG,n=24).The rats in HG were inhaled 95% oxygen to establish hyperoxia-damaged lung model and the rats in AG were permitted to breathe room air.Transmission electron microscope were used to examine changes in ultrastructures of pulmonary type II alveolar cell(AEC-II) and vascular endothelial cell in the two groups at the 3rd day,the 7th day,and the 14 day.14 days later,incidence rates of degeneration and necrosis of AEC-Ⅱ and vascular endothelial cell were observed in the two groups.Results The loosen lamellar bodies in AEC-Ⅱ and swollen partial pulmonary capillary endothelial cells were seen at the 3rd day of hypenoxia inhalation,the normal structures of AEC-Ⅱ and pulmonary capillary endothelial cells disappeared at the 14th day alone with prolongation of hypoxia inhalation,and at the 14th day,lethal damage of AEC-Ⅱ and pulmonary vascular endothelial cells were found,which shares morphologic similarities to human bronchopulmonary dysplasia.Compared with AG,HG,AEC-Ⅱ degeneration and necrosis rate were significantly higher(24.0% in AG and 68.0% in HG,χ2=39.32,P0.005) and pulmonary capillary endothelial cell degeneration and necrosis rate(15.0% in AG and 39.0% in HG,χ2=14.99,P0.005).There were significant differences between AG and HG.Conclusion Continuous hyperoxic exposure can cause damages of AEC-Ⅱ and vascular endothelial cells of neonatal rats.
Lung physiology and histopathology during cumulated exposure to nitric oxide in combination with assisted ventilation in healthy piglets.

Abstract:Inhaled nitric oxide (iNO) is routinely used for hypoxic respiratory failure and persistent pulmonary hypertension of the newborn, and investigation of its new indications requiring various levels of iNO is underway. Cumulated exposure of high level of iNO may exert adverse effects on lung function and morphology, which may be confounded with ventilator-associated lung injury. Sixteen healthy piglets (5.5–11 kg) were anaesthetised, intubated and mechanically ventilated at low FiO2 on continuous positive airway pressure and pressure support mode. The animals were randomly allocated to receive 40 ppm iNO (NO group, n=8) or no iNO (Control group, n=8). In both groups at 24 and 48 h, mild to moderate lung injury was observed, with mean values of PaO2/FiO2<300 mmHg. Compared to the Control, iNO at 40 ppm for 24–48 h did not adversely affect dynamic compliance or resistance of respiratory system, oxygenation, pulmonary and systemic hemodynamics. Neither did it affect composition and surface activity of surfactant phospholipids and white cell counts in bronchoalveolar lavage fluid. Inhaled NO resulted in elevated total serum nitrite/nitrate to 352±90μmol/l and methemoglobin (MetHb) to 5.0±3.4%, in contrast to 88±38μmol/l and 0.88±0.52% in the Control; 50% of the iNO animals having MetHb>3%. The lung injury scores as well as alveolar expansion were similar between the two groups at 24 h. At 48 h, low wet/dry lung weight ratio and lung injury score were found in the NO group. We conclude that no significant adverse effects on lung physiology and structure were found in the piglets receiving 40 ppm iNO for 24 or 48 h, on the contrary lung injury was moderately alleviated. The significantly impaired gas exchange over time associated with discrete morphological changes suggests adverse effects of prolonged positive pressure breathing and not necessarily exposure to oxides of nitrogen.
Inhalation of Nitric Oxide in Congenital Heart Defects Associated with Pulmonary Hypertension

Abstract:AIM: To assess efficacy of inhaled nitric oxide (iNO) in postoperative cases of congenital heart defects who developed critical pulmonary hypertension and had no response to conventional pulmonary vasodilator. METHODS: From January 1997 to May 2002, 37 patients, age ranging 14 days to 14 years and body weight 3.1 to 32 kg, were enrolled in the iNO therapy. Indication for NO: ratio of pulmonary to systemic systolic arterial pressure (P-p/P-s)> 0.5, ratio of arterial oxygen tension to fraction of inspired oxygen (PaO2/FiO(2))<150 mmHg, and conventional pulmonary vasodilator failure. Initial iNO was started at 20x10(-6) vol/vol (ppm), adjusted according to blood gas and hemodynamic changes, maximal at 45 ppm, and maintained at 5-15 ppm. RESULTS: In 36 patients during iNO, Pp decreased from (58.5+/-14.7) to (44.8+/-13.9) mmHg (P<0.01) without significant change in Ps (77.2+/-18.0 vs 78.2+/-13.5, P>0.05), whereas PaO2/FiO(2) increased from 106.6+/-36.8 to 176.5+/-66.0 (P<0.01). Thirty four patients discharged at (5.5+/-5.4) d (range 2-18 d) and 3 died. Inhaled nitrogen dioxide was less than 1 ppm and methemoglobin less than 2 %. No hemorrhage and other adverse effects Were observed. CONCLUSION: iNO improved pulmonary hemodynamics and blood oxygenation in postoperative critical pulmonary hypertension without adverse effects.
Disruption of NO-cGMP signaling by neonatal hyperoxia impairs relaxation of lung parenchyma

Abstract:Exposure of immature lungs to hyperoxia for prolonged periods contributes to neonatal lung injury and airway hyperreactivity. We studied the role of disrupted nitric oxide-guanosine 3',5'-cyclic monophosphate (NO-cGMP) signaling in impairing the relaxant responses of lung tissue from hyperoxia-exposed rat pups. Pups were exposed to >/=95% O(2) or room air for 7 days starting from days 1, 5, or 14. The animals were killed, lungs were removed, and 1-mm-thick lung parenchymal strips were prepared. Lung parenchymal strips of room air or hyperoxic pups were preconstricted using bethanechol and then graded electrical field stimulation (EFS) was applied to induce relaxation. EFS-induced relaxation of lung parenchymal strips was greater at 7 and 12 days than at 21 days in room air-exposed rat pups. Hyperoxic exposure significantly reduced relaxation at 7 and 12 days but not 21 days compared with room air exposure. NO synthase blockade with N(omega)-nitro-l-arginine methyl ester diminished relaxant responses in room air but not in hyperoxic pups at 12 days. After incubation with supplemental l-arginine, the relaxation response of hyperoxic strips was restored. cGMP, a key mediator of the NO signaling pathway, also decreased in strips from hyperoxic vs. room air pups and cGMP levels were restored after incubation with supplemental l-arginine. In addition, arginase activity was significantly increased in hyperoxic lung parenchymal strips compared with room air lung parenchymal strips. These data demonstrate disruption of NO-cGMP signaling in neonatal rat pups exposed to hyperoxia and show that bioavailability of the substrate l-arginine is implicated in the predisposition of this model to airway hyperreactivity.
[Effect of melatonin on hyperoxia-induced oxidant/antioxidant imbalance in the lung of neonatal rats with chronic lung disease]

Abstract:Objective: To study the effect of melatonin, a potent antioxidant both in vitro and in vivo, on hyperoxia-induced oxidant/antioxidant imbalance in the lung of neonatal rats with chronic lung disease (CLD). Methods: Ninety neonatal rats were randomly divided into three groups (n=30 each): air-exposed, hyperoxia-exposed, melatonin-treated (4 mg/kg melatonin was administered 30 minutes before hyperoxia exposure and once daily after exposure). CLD was induced by hyperoxia exposure (FiO2=0.85). Lung specimens were obtained 3, 7, and 14 days after exposure (n=10 each) for histopathologic examination. The levels of total antioxydase capacity (T-AOC), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), myeloperoxidase (MPO), catalase (CAT), nitrite/nitrate, and malondialdehyde (MDA) in the lung were assayed by the spectrophotometer. Results: The histopathologic examination showed that lung damage was obviously alleviated in the melatonin-treated group. The levels of T-AOC, GSH-Px, SOD and CAT in the lung were significantly higher in the melatonin-treated group than those in the other two groups at all time points (p<0.05). The levels of MPO, nitrite/nitrate and MDA in the lung increased significantly in the untreated hypoxia-exposed group compared with those in the air-exposed group at all time points (p<0.05 or 0.01), while the levels of MPO, nitrite/nitrate and MDA in the melatonin-treated group were significantly reduced as compared with the untreated hypoxia-exposed group (p<0.05). Conclusions: Melatonin may reverse oxidant/antioxidant imbalance in hyperoxia-induced lung disease, thus providing a protective effect against CLD in neonatal rats.
Combined iNO and endothelial progenitor cells improve lung alveolar and vascular structure in neonatal rats exposed to prolonged hyperoxia

Abstract:Background: Stem cells or inhaled nitric oxide (iNO) are reported to improve lung structures in bronchopulmonary dysplasia (BPD) models. We hypothesized that combined iNO and transplanted endothelial progenitor cells (EPCs) might restore lung structure in rats after neonatal hyperoxia. Methods: Litters were separated into eight groups: room air, hyperoxia, hyperoxia + iNO, hyperoxia + iNO + L-NAME, hyperoxia + EPCs, hyperoxia + EPCs + L-NAME, hyperoxia + EPCs + iNO, and hyperoxia + EPCs + iNO + L-NAME. Litters were exposed to hyperoxia from the 21st day, then, sacrificed. EPCs were injected on the 21st day. L-NAME was injected daily for 7 d from the 21st day. Serum vascular endothelial growth factor (VEGF), radial alveolar count (RAC), VIII factor, EPCs engraftment, lung VEGF, VEGFR2, endothelial nitric oxide (eNOS) and SDF-1 expression, and NO production were examined. Results: Hyperoxia exposure led to air space enlargement, loss of lung capillaries, and low expression of VEGF and eNOS. Transplanted EPCs, when combined with iNO, had significantly increased engraftment in lungs, compared to EPCs alone, upon hyperoxia exposure. There was improvement in alveolarization, microvessel density, and upregulation of VEGF and eNOS proteins in the hyperoxia-exposed EPCs with iNO group, compared to hyperoxia alone. Conclusion: Combined EPCs and iNO improved lung structures after neonatal hyperoxia. This was associated with the upregulation of VEGF and eNOS expression.
Plasma Biomarkers of Oxidative Stress: Relationship to Lung Disease and Inhaled Nitric Oxide Therapy in Premature Infants

Abstract:OBJECTIVES. Inhaled nitric oxide treatment for ventilated premature infants improves survival without bronchopulmonary dysplasia. However, there has been no information regarding possible effects of this therapy on oxidative stress. We hypothesized that inhaled nitric oxide therapy would not influence concentrations of plasma biomarkers of oxidative stress. PATIENTS AND METHODS. As part of the Nitric Oxide Chronic Lung Disease Trial, we collected blood samples at specified intervals from a subpopulation of 100 infants of <1250 g birth weight who received inhaled nitric oxide (20 ppm, weaned to 2 ppm) or placebo gas for 24 days. Plasma was assayed for total protein and for 3-nitrotyrosine and carbonylation by using immunoassays. RESULTS. The demographic characteristics and primary outcome for the infants were representative of the entire group of infants who were in the Nitric Oxide Chronic Lung Disease Trial. For all infants at baseline, before receiving study gas, the concentration of total protein was inversely correlated with the respiratory severity score, and plasma carbonyl was positively correlated with severity score, supporting an association between oxidative stress and severity of lung disease. Infants who survived without bronchopulmonary dysplasia had 30% lower protein carbonylation concentrations at study entry than those who had an adverse outcome. At each of 3 time points (1–10 days) during exposure to study gas, there were no significant differences between control and treated infants for concentrations of plasma protein, 3-nitrotyrosine, and carbonylation. CONCLUSIONS. Inhaled nitric oxide treatment for premature infants who are at risk for bronchopulmonary dysplasia does not alter plasma biomarkers of oxidative stress, which supports the safety of this therapy.
