Abstract:Background HELLP syndrome is a rare disease in China, and 20% of patients with severe preeclampsia have been accompanied with HELLP syndrome, which is characterized by the presence of hemolysis, elevated liver enzymes and low platelet count. Case presentation In this case, we reported that a patient with preeclampsia was diagnosed with HELLP syndrome. Furthermore, hepatic infarction also was found via the computed tomographic (CT) images, which showed peripheral wedge-shaped inhomogeneous low attenuation in the right hepatic lobes via plain CT scan, and the low-density shadow and mottled appearance in the same areas where vessels were seen coursing through them via contrast-enhanced CT scan. Conclusions Besides typical clinical manifestations of the pregnant patient with preeclampsia, the typical laboratory evidences were elevated liver enzymes and thrombocytopenia. The abdominal CT scan showed imaging features of hepatic infarction, which was helpful to identify the rare complication of HELLP syndrome. Thus, we diagnosed a patient with HELLP syndrome with hepatic infarction, though the patient had no chance to do the liver biopsy.

Abstract:Ascites is an important indication of liver function discompensation,and also one of the main reasons of hospitalization for patients with liver failure.In China,hepatitis viral infection accounts for a large proportion of liver failure.Liver failure-related ascites is likely to develop into refractory ascites,resulting in the increased mortality of liver failure.This article reviews the progress on diagnosis and treatment of refractory ascites in patients with liver failure.

Abstract:The academic term "HELLP" syndrome is an abbreviation for a syndrome consisting of hemolysis (H), elevated liver enzymes (EL), and low platelet count (LP) that occurs in mid to late pregnancy or the early postpartum period. This event was first observed in 1954 and was formally proposed by Dr Louis Weinstein in 1982. [ 1 , 2 ] The syndrome is characterized by hepatic endothelial dysfunction, platelet aggregation and consumption, hepatocellular necrosis and death, or even the development of subcapsular hepatic hematoma or its rupture, ultimately contributing to both maternal and fetal mortality (24% and 60%, respectively). [ 3 , 4 ] It is considered one of the various manifestations of preeclampsia because approximately 70% to 80% of cases with HELLP syndrome coexist with it. [ 5 ] Herein, we describe a postpartum woman who suffered from a spontaneous subcapsular hepatic hematoma (SSHH) and experienced successful rescue, aiming to understand the natural outcome of SSHH under nonsurgical management, gain preliminary experience in rescue techniques, and provide tips for decision-making regarding therapeutic strategies through reviewing related literature.

Hypoxic hepatitis: a challenging diagnosis

Abstract:Hypoxic hepatitis (HH), one of the most common causes of acute liver injury, has a prevalence of up to 10% of admissions in intensive care units across the world. Inadequate oxygen uptake by the hepatocytes resulting in centrilobular necrosis associated with abnormally raised levels of the serum transaminases (ALT, AST) in patients with clinical history of cardiac, respiratory, or circulatory failures is the key feature of this condition. Abstracts, reviews, case reports, and research letters from various sources such as Pubmed, Proquest, Ovid, Google Scholar, and ISI Web of Knowledge dating from 1970 to 2011 were read and analyzed thoroughly. A study of 100 patients with HH, carried out from 2009 to 2010 at Tongji Hospital of Tongji University, Shanghai, People’s Republic of China, is also documented. The contributing factors leading to HH are passive congestion, ischemia, and arterial hypoxemia of the liver. Ischemia/reperfusion injury also has a major role in HH. Some of its complications are spontaneous hypoglycemia, a high level of serum ammonia, and respiratory insufficiency due to hepatopulmonary syndrome. The therapy of HH lies mainly in the treatment of the main underlying causes, and this leads to the successful reversion of HH. The aim of this review is to present a simplified concept about the etiology, pathophysiology, mechanism, clinical manifestations, diagnosis, and treatment of HH.

Abstract:Paroxysmal nocturnal hemoglobinuria is a rare intravascular hemolytic anemia, and thrombosis is the leading cause of mortality rate. The hepatic veins is the common site where Budd-Chiari syndrome usually occurs. We confronted a patient who simultaneously happened to have portal vein and superior mesenteric vein thrombosis leading to prehepatic portal hypertension and upper gastrointestinal bleeding. Percutaneous thrombolysis is an efficacious treatment.

Abstract:Patients with cirrhosis are vulnerable to acute hepatic insults and are more likely to develop rapid hepatic deterioration. The aim of this study is to describe the clinical course of patients with cirrhosis and hospitalized for acute hepatic deterioration (AHD).This is a prospective study involving 163 patients with cirrhosis and AHD. The occurrence of organ failures, systemic inflammatory response syndrome (SIRS), and infections during hospital stay were recorded and the relationship between organ failure and death or SIRS/infection was subsequently analyzed.Of 163 patients, 35 did not develop any organ failure during in-hospital follow-ups (90-day mortality: 0%); 84 had intrahepatic organ failures (IH-OFs, defined by liver and/or coagulation failure) (90-day mortality: 22.0%); and 44 patients developed extra-hepatic organ failures (EH-OFs, defined by kidney, cerebral, circulation, and respiratory failure) on the basis of IH-OF with a 90-day mortality of 90.9%. On multivariable analysis by a Cox proportion hazard model, age, WBC, presence of IH-OF, and EH-OF all predicted 90-day death. A logistic regression analysis identified SIRS being associated with the development of EH-OF. Furthermore, IH-OF at admission and infections occurred during the hospital stay were shown to be another 2 potential risk factors.The clinical course of cirrhosis patients with acute hepatic injury was characterized by 3 consecutive stages (AHD, IH-OF, and EH-OF), which provided a clear risk stratification. The PIRO criteria provided an accurate frame for prognostication of those patients. The systemic inflammatory response syndrome may be a target for blocking the progression to the EH-OF stage.

Abstract:Background Hemolysis-elevated liver enzymes-low platelet counts (HELLP) syndrome is a clinical condition occurring in middle and late stage pregnancy. It is characterized by hemolysis, elevated liver enzymes and low platelet counts. This study involves the analysis of the diagnosis, clinical characteristics and treatment of 59 cases of HELLP syndrome as well as the clinical classification, method of delivery and gestational age at delivery.Methods Clinical data from 59 cases of HELLP syndrome occurring from January 2000 to December 2009 were analyzed retrospectively. Thirty-five cases were classified as complete HELLP syndrome and 24 cases were considered partial HELLP syndrome. Results Twenty-six of the 59 analyzed patients (44%) with complete HELLP syndrome showed rapid onset, severe signs, symptoms, and complications in addition to a poor clinical outcome. Complications included multiple organ dysfunction syndrome (MODS) occurring in 18 cases, eclampsia (3 cases), placental abruption (3 cases), and perinatal death (4 cases). The remaining 33 cases (24 with partial and 9 with complete HELLP) were characterized by less severe signs, symptoms, complications and progression of the condition. Two of these cases were complicated with MODS (6.1%), and 1 with perinatal death (3.0%). Twelve non-radical-type cases received conservative treatment. The remaining 4 patients had recurring HELLP syndrome (6.78%).Conclusions HELLP syndrome is classified as the radical type and non-radical-type according to clinical characteristics and outcome. Classification of HELLP syndrome cases according to clinical features can help in the monitoring and treatment of the disease. Active termination of pregnancy should be considered for radical-type cases. Non-radical-type cases can undergo conservative treatment with close monitoring in an attempt to improve perinatal outcome without increasing maternal morbidity. Chin Med J2010;123(10):1273-1277

Abstract:Objective To investigate the diagnosis and treatment of HELLP syndrome through analyzing the clinical data of seven patients of HELLP syndrome.Methods We reviewed the study.There were three patients in the absolute HELLP group,four patients in the partial HELLP group.The laboratory data,clinical symptom,delivery method and the outcome of the pregnant women and infants of the two groups were analyzed.Results All the seven patients had clinical presentations of PIH.Total bilirubin(TB) value in absolute HELLP group was significantly higher than that in partial HELLP group(P<0.05).The other blood tests were not found significant difference between the two groups.All cases were delivered by cesarean section,including of one dead fetus and six immature labors.The average days of gestational age at delivery is 214.7±11.4 and there were three placental abruption,two postpartum hemorrhage and five severe asphyxia in newborn.Conclusion HELLP syndrome is a serious obstetric complication.Delivery is the most effective treatment.

Abstract:BACKGROUND:Plasma exchange (PE)-centered artificial liver support system reduced the high mortality rate of hepatitis B virus (HBV)-related acute-on-chronic liver failure (ACLF). But the data were diverse in different medical centers. The present prospective nationwide study was to evaluate the effects of PE on patients with HBV-ACLF at different stages.METHODS:From December 2009 to December 2011, we evaluated 250 patients at different stages of HBV-ACLF from 10 major medical centers in China. All the laboratory parameters were collected at admission, before and after PE.RESULTS:Among the 250 patients who underwent 661 rounds of PE, one-month survival rate was 61.6%; 141 (56.4%) showed improvement after PE. Variables such as age (P=0.000), levels of total bilirubin (TB, P=0.000), direct bilirubin (P=0.000), total triglycerides (P=0.000), low-density lipoprotein (P=0.022), Na+ (P=0.014), Cl- (P=0.038), creatinine (Cr, P=0.007), fibrinogen (P=0.000), prothrombin time (PT, P=0.000), white blood cell (P=0.000), platelet (P=0.003) and MELD (P=0.000) were significantly related to prognosis. Multivariate logistic regression analysis showed that age, disease stage, TB, Cr and PT levels were independent risk factors of mortality among HBV-ACLF patients.CONCLUSIONS:PE can improve the clinical outcome of patients with HBV-ACLF. Levels of TB, Cr and PT, age and disease stage help to predict prognosis.

Abstract:Objective To summarize the incidence of patients with hemolysis,elevated liver enzymes and low platelet count(HELLP) syndrome.Methods The clinical data of 29 cases with HELLP syndrome were analyzed retrospectively during the past 6 years.Results HELLP syndrome happened around 34 weeks+ gestation.Most of patients had the complains of abdominal pain,nausea,emesis,and the complications of placental abruption,acute renal failure,acute heart failure,disseminated intravascular coagulation.The developmental change of platedlet count is probably the earliest index to indicate the incidence of the syndrome,while liver enzymes and LDH are the most reliable indexes reflecting the patients' condition.The prognosis of the neonates was related to the terminal gesta- tional age.Conclusion HELLP syndrome is serious and life-threatening.In order to decrease the maternal and perinatal mortality rate and the incidence rate of the complications,HELLP syndrome should be diagnosed and treated as fast as plssible,and terminate the pregnant in the right time.
A Case of Autoimmune Hepatitis Initially Manifesting As Hepatic Encephalopathy

Abstract:Autoimmune hepatitis (AIH) is a T-cell-mediated liver disease characterized by elevated transaminases, circulating autoantibodies, hypergammaglobulinemia, and interface hepatitis. A 66-year-old female patient visited our department due to recurrent episodes of altered consciousness, sleep-wake inversion, and asterixis, indicating hepatic encephalopathy (HE). Her liver biopsy results clearly demonstrated interface hepatitis. The patient's severe HE does not parallel her relatively stable liver function and was attributed to a wide retroperitoneal collateral vein shunting blood directly into the inferior vena cava, bypassing the liver, and allowing excess neurotoxins to enter the central nervous system. Due to the unfavorable benefit-risk ratio of embolization and the patient's stable liver function, non-invasive treatments were adopted, and prednisolone was discontinued. The patient experienced no further episodes of HE thereafter. To the best of our knowledge, this is the first AIH case with a spontaneous portosystemic shunt directly shunting blood into the inferior vena cava. A crucial lesson from this case is that when HE cannot be fully explained by liver dysfunction, abdominal CT scans should be carefully inspected for possible anatomical variations. This case also underscores the importance of a multidisciplinary approach in managing AIH in elderly patients, who may benefit more from a tailored treatment regimen rather than strictly following standard treatment guidelines.
Comprehensive Treatment of Acute-On-Chronic Liver Failure in A Patient with Hepatitis B: A Case Report

Abstract:The clinical data of a patient with acute-on-chronic liver failure were analyzed retrospectively. The patient has suffered from hepatitis B for 30 years. His liver function deteriorated, yielding Child-Pugh grade C and reaching a model for end-stage liver disease score of 33 points within a short period; this condition was complicated with highly active variceal bleeding and coagulation system failure (PT > 100 s). The patient also presented hepatocellular carcinoma. Comprehensive treatments included effective inhibition of hepatitis B virus replication and intensive care support. Piggyback orthotopic liver transplantation was performed as the final treatment. The patient recovered uneventfully and was discharged after surgery.
Acute Pancreatitis Complicated by Sheehan's Syndrome: A Case Report and Literature Review

Abstract:A 44-year-old woman was transferred to the ICU of the First Affiliated Hospital of Jinan University for 2 days of persistent epigastric pain and 7 hours of unconsciousness. Her admission diagnosis was severe acute necrotizing pancreatitis (hypertriglyceridemia type) with multiple organ dysfunctions. The results of CT revealed a small area of necrotizing pancreatitis, which was not consistent with the severe clinical manifestations. Considering lack of hair and history of postpartum hemorrhage, hormone examination was carried out. According to the results of the examination, she was further diagnosed as Sheehan's syndrome and pituitary crisis. After hormone replacement therapy, her condition improved rapidly.

Abstract:Background Hemolytic anemia, elevated liver enzymes and low platelet count (HELLP) syndrome is a severe obstetric complication which usually resolves in most patients after delivery. Methods We report a rare case of aggravation of HELLP syndrome after delivery. Results The patient underwent the treatment for HELLP syndrome, including glucocorticoid therapy. The symptoms of HELLP syndrome reappeared and became more severe than before the termination of pregnancy. The patient also had severe and persistent hypoproteinemia, hyponatremia and hypocalcemia. Conclusions HELLP syndrome is an acute and critical obstetric syndrome which can have heterogeneous presentations and variable prognosis. Subcapsular Liver Hematoma Rupture in HELLP Syndrome During Pregnancy: A Case Study and Multidisciplinary Approach

Abstract:BACKGROUND Spontaneous hepatic hematoma and liver capsule rupture is a rare but severe complication of Hemolysis, Elevated Liver Enzyme, and Low Platelet (HELLP) syndrome, with a high mortality rate. We report a case of a pregnant woman with HELLP syndrome and liver subcapsular hematoma rupture that was diagnosed during surgery. CASE REPORT A 35-year-old woman with 34+1 weeks of pregnancy came to the emergency department due to abdominal pain for 4 days. She was diagnosed with HELLP syndrome after a blood test. She was transferred to the obstetrics department, and an emergency cesarean section was performed under general anesthesia, due to fetal distress. During the surgery, non-clotting blood was found flowing out during the suturing of the incision. We suspended the surgery and organized an emergency multidisciplinary consultation. Subcapsular liver hematoma was diagnosed after intraoperative ultrasound detection. Emergency upper abdominal laparotomy was performed, and a ruptured liver capsule and active bleeding were found. The liver capsule was sutured and blood products were infused before the patient was sent to the intensive care unit. She recovered and was discharged 12 days after surgery. No special discomfort was reported during the 30-day follow-up after surgery. CONCLUSIONS Our case emphasizes that all parturients with abdominal pain and HELLP syndrome must be screened for spontaneous hepatic hematoma, and clinicians should pay attention to whether there is rupture of the liver capsule. Multidisciplinary consultations can increase the probability of successful rescue for such patients.
Subcapsular liver hematoma complicating HELLP syndrome: A case report and review of management strategies

Abstract:Subcapsular liver hematoma is a life-threatening complication of pregnancy. It is associated with preeclampsia and HELLP syndrome. We present the case of a 36-year-old Caucasian nulliparous woman who was diagnosed at 29 weeks and 6 days of gestation with mild preeclampsia. After brief hospitalization she was discharged. During a daily follow-up, at 31 weeks and 3 days of gestation, she complained of mild abdominal pain and blood tests revealed HELLP syndrome. The cervix was unripe. A healthy baby was delivered by emergency cesarean section. The following day, the patient complained of persistent abdominal pain, and at the same time the hepatic cytolysis worsened dramatically. A computed tomography (CT) scan revealed a significant subcapsular hematoma without any active bleeding or breach of Glisson's capsule. We treated the patient conservatively and she was discharged home 10 days after the diagnosis was made. The symptoms of subcapsular liver hematoma are non-specific. They include nausea, vomiting and epigastric pain, and pain in the right upper quadrant or shoulder. Biological analyses can show hepatic cytolysis, haemolysis and coagulation disorders. Medical imaging can confirm the diagnosis. The management of subscapular liver hematoma may depends on whether there is hemodynamic stability, active bleeding or breach of Glisson capsule's. If the patient is stable and in the absence of active bleeding, management should be purely symptomatic.

Abstract:Objective:To analyze the risk,clinical characteristics,prognostic factors,pregnancy outcomes and treatment on SPE (severe preeclampsia) complicated wtih HELLP (hemolysis,elevated liver enzymes,and low platelet count) Syndrome,which can help us to improve the outcomes of pregnant and fetus in ICU.Method:Retrospective study the clinical data of 125 patients with severe preeclampsia in ICU of Shanghai General Hospital from January 2007 to December 2016.26 cases of SPE complicated with HELLP syndrome.99 cases of SPE without HELLP syndrome were collected and analyzed retrospectively.General clinical data,laboratory data,treatment and pregnancy outcomes of patients and their fetuses were compared by chi-square test and ANOVA.Result:The patients were divided into two groups according to whether or not they had HELLP syndrome.The incidence of SPE complicated with HELLP was 20.8％.There were significant differences of aged,pregnancy history,prenatal check-up,platelet count,D-dimer,serum Cr,serum total bilirubin,serum AST,serum ALT,serum glucose,Apgar score,ICU LOS and transfusion of blood products between the two groups (P＜0.05).No maternal death occurred in the two groups.No statistical difference was found between neonatal survival rate.Conclusion:Aged,pregnancy history,prenatal check-up are risk factors,which were related to the SPE with HELLP syndrome.Enhancing the antenatal care and dynamic monitoring laboratory data such as complete blood count,hepatic function,renal function,and coagulation function of preeclampsia patients,which are useful to discover H ELLP syndrome earlier and reduce the incidence of complications and mortality of patients and fetuses.Transfusion of blood products is an effective treatment to HELLP syndrome.
Successful Rescuing a Pregnant Woman with Severe Hepatitis E Infection and Postpartum Massive Hemorrhage.

Abstract:AIM:To sum up the experience of the successful therapy for the severe hepatitis of pregnant woman with postpartum massive hemorrhage.METHODS:The advanced therapeutic methods including the bilateral uterine artery embolism, hemodialysis and artificial liver support therapy were performed with comprehensive medical treatments and the course of the successful rescuing the patient was analyzed.RESULTS:Through the hospitalization of about two mouths the patient and her neonatus had gotten the best of care in our department and pediatric department separately. Both of them were discharged in good condition.CONCLUSION:The key points for a successful therapy of the pregnant woman with severe hepatitis are termination of the pregnancy and the control of their various complications. It was suggested that the proper combination of these measures of modern therapy would race against time for renewing of hepatic and renal functions.
