Abstract:BackgroundTo compare potential factors associated with deep infiltrating endometriosis (DIE) versus ovarian endometrioma (OMA) among endometriosis patients in China.MethodsA subgroup analysis of factors associated with DIE versus OMA was performed in Chinese women from the FEELING study. This study included 156 OMA patients and 78 DIE patients. Retrospective information on symptoms and previous medical history was collected via face-to-face interviews; patients also completed a questionnaire to provide information on current habits. Univariate and multivariate logistic regression analyses were conducted to identify potential factors that are associated with DIE versus OMA.ResultsFrom univariate analysis, women who were married, at older age, had any siblings, prior pregnancy, or longer time since age at menarche on the day of visit were more likely to be diagnosed with DIE (P<0.05). Also, the incidence of previous uterine surgery, menstrual and ovulatory disorders, deep dyspareunia, and gastrointestinal symptoms during menstruation were major factors that were significantly associated with the diagnosis of DIE (P<0.05). Multivariate analysis showed that women with any siblings, gastrointestinal symptoms during menstruation, or eating a greater number of fruit/vegetables per day were more likely to be diagnosed with DIE. Meanwhile, eating organic food and experiencing stress were major factors that are associated with the diagnosis of OMA.ConclusionsThe findings provide additional information on the potential risk factors that are associated with DIE, compared with OMA among Chinese endometriosis patients. The results may help to better understand DIE versus OMA, and aid in earlier risk stratification and diagnosis of the patients.Trial registrationNCT01351051. Registered 10 May 2011.

Abstract:At present, endometriosis remains a worldwide health burden, with the main symptoms of dysmenorrhea, chronic pelvic pain, and infertility, markedly reducing the quality of life (de Ziegler et al., 2010). Although there is no proof that the disease is associated with high mortality, this disorder can significantly contribute to the deterioration of women's general well-being (McPeak et al., 2018). The main current treatment for endometriosis is surgery to remove endometriotic lesions; however, the recurrence rate following surgical treatment is as high as 21.5% at two years and 40.0%‍-‍50.0% at five years post-surgery (Koga et al., 2015). To prevent recurrence, adjuvant treatment with drugs after surgery is recommended to prolong relapse-free intervals. However, it is inconvenient for patients to continuously use such medications in terms of adverse effects and cost (Turk, 2002).
Risk Factors for Coexisting Deep Endometriosis for Patients with Recurrent Ovarian Endometrioma.

Abstract:AimThe aim of this study was to assess the risk factors for coexisting deep endometriosis (DE) in patients with recurrent ovarian endometrioma (OE).MethodsWe retrospectively reviewed 151 recurrent OE patients who had been diagnosed of OE but not DE at the time of their first surgery and then received a second surgery for recurrent endometriosis with or without DE. Their clinical characteristics at the time of the first and second surgeries were collected. Univariate and multivariate logistic regression analyses were conducted to identify potential risk factors for coexisting DE in patients with recurrent OE.ResultsAmong the 151 recurrent OE patients, 46 were diagnosed of DE during the recurrent surgery and included in the DE group, while the remaining 105 patients were included in the non-DE group. In univariate analysis, there were significant differences in terms of uterine retroversion during the primary surgery and the follow-up time after the primary surgery between the DE and non-DE groups. The multivariate analysis also showed that both uterine retroversion and the follow-up time (≥5 years) were associated with the coexistence of DE during the recurrent surgery. The odds ratio (OR) for uterine retroversion was 3.72 [95% confidence interval (CI) 1.62–8.53], and the OR for follow-up time (≥5 years) was 5.03 (95% CI 2.29–11.02).ConclusionsOur study suggested that for recurrent OE patients, uterine retroversion during the first surgery and a follow-up time of at least 5 years are risk factors for the coexistence of DE in recurrent surgery, early prevention and full preparation before the recurrent surgery should be emphasized in these conditions.
Long-term Clinical Efficacy Evaluation and Prognosis Analysis of Deep Infiltrating Endometriosis in Young Patients

Abstract:The purpose of this study is to evaluate the long-term clinical efficacy of young patients with Deep Infiltrating Endometriosis (DIE) and analyze the related prognostic factors. Through retrospective analysis of a group of young patients, the survival curves of different treatment methods were compared, and the effects of age, course of disease, treatment methods and complications on prognosis were discussed by logistic regression model. The results show that surgical treatment has obvious advantages in improving the long-term prognosis of patients compared with drug treatment. At the same time, the prognosis of young patients with short course of disease and few complications is relatively good. This study provides a useful reference for clinicians to formulate personalized treatment plans and prognosis strategies, which is helpful to improve the quality of life and prognosis of young patients with DIE. However, there are still some limitations in the study. In the future, it is necessary to further expand the sample size, extend the follow-up time, and explore more potential prognostic factors in order to improve the understanding and treatment strategy of the disease.

Abstract:Deep infiltrating endometriosis (DIE) is typically characterized by multifocal locations. It has been shown that CCR1, combined highly with RANTES, contributes to the enhanced recruitment of inflammatory cells at endometriotic sites. As an estrogen-dependent disorder, estrogen receptors are also crucial to the growth of endometriotic tissues. In this study we report the immunohistochemical analysis of RANTES, CCR1, ER-α and ER-β in 48 histological lesions prepared from women with DIE undergoing surgery. Immunohistochemical analysis of RANTES, CCR1, ER-α and ER-β was conducted at different sites of DIE lesions. RANTES was immunolocalized in the cytoplasm and CCR1 in cytomembranes of endometriotic cells. ER-α and ER-β extensively immunostained the nuclei of endometrial glandular, and stromal cells. Immunoreactivity in DIE lesions, similar to the widespread ERs, showed higher expression of RANTES and CCR1 in three types of DIE lesions. There was a significant correlation, independent of cyclic changes, between the expression of RANTES/CCR1 and DIE lesions. RANTES/CCR1 increased significantly according to the severity of dysmenorrhea. RANTES and CCR1 together may provide a potential biomarker for DIE-related pain and inflammatory response in endometriotic lesions of patients with DIE.
The efficacy and safety of up to 24 weeks of triptorelin therapy following conservative surgery in patients with deep infiltrating endometriosis (DIE): a multicentre, prospective, non-interventional study in China

Abstract:OBJECTIVE: To evaluate the efficacy and safety of up to 24 weeks of triptorelin treatment after conservative surgery for deep infiltrating endometriosis (DIE). DESIGN: Multicentre, prospective, non-interventional. SETTING: 18 tertiary hospitals in China. POPULATION: Premenopausal women aged ≥ 18 years treated with triptorelin 3.75 mg once every 28 days for up to 24 weeks after conservative surgery for DIE METHODS: Endometriosis symptoms were assessed, using a visual analogue scale (0–10 cm) or numerical range (0–10), at baseline (pre-surgery) and routine visits 3, 6, 9, 12, 18 and 24 months after surgery. MAIN OUTCOME MEASURES: Change in symptom intensity over time. RESULTS: A total of 384 women were analysed (mean [SD] age, 33.4 [6.2] years). Scores for all symptoms assessed (pelvic pain, dysmenorrhoea, ovulation pain, dyspareunia, menorrhagia, metrorrhagia and gastrointestinal and urinary symptoms) decreased from baseline over 24 months. Cumulative improvement rates in pelvic pain, dysmenorrhoea, ovulation pain and dyspareunia were 74.4%, 83.6%, 55.1% and 66.9%, respectively. The 24-month cumulative recurrence rate (≥ 1 symptom) was 22.2%. The risk of symptom recurrence was higher in patients with ≥ 2 versus 1 lesion (odds ratio [OR] 2.539; 95% CI: 1.458–4.423; p = 0.001) and patients with moderate (OR 5.733; 95% CI: 1.623–20.248; p = 0.007) or severe (OR 8.259; 95% CI: 2.449–27.851; p = 0.001) pain versus none/mild pain. Triptorelin was well tolerated. CONCLUSIONS: Triptorelin after conservative surgery for DIE improved symptoms over 24 months of follow up. FUNDING: Sponsored by Ipsen

Fertility performance and the predictive value of the endometriosis fertility index staging system in women with recurrent endometriosis: A retrospective study.

Abstract:This study presents the postoperative pregnancy rate of women with recurrent endometriosis and evaluates the predictive value of the endometriosis fertility index (EFI) for the pregnancy. A total of 107 women who wished to conceive after surgery for recurrent endometriosis from January 2007 to December 2016 were included. The EFI score was calculated postoperatively. The receiver operator characteristic (ROC) curve was plotted to determine the most promising contributor to predicting pregnancy, and Kaplan-Meier (K-M) analysis was used to estimate the cumulative pregnancy rate (CPR). A total of 61 pregnancies were registered in 58 women and the remaining 49 patients failed to become pregnant. The EFI score was strongly associated with the postoperative fertility prognosis. The CPRs during the first 2 and 3 years postoperatively were 51.86% and 66.38%, respectively, and increased to 71.98% within the first 5 years postoperatively in patients with EFI scores >= 5. However, the CPR was 26.00% during the first 2 years after surgery in individuals with EFI scores <5, and there was no increase in the CRP thereafter. Women suffering from recurrent endometriosis still experienced a probability of natural pregnancy, especially patients with EFI scores >= 5. The EFI score had good predictive power for postoperative pregnancy in these patients.

Abstract:Background: This study aimed to summarize and analyze clinical characteristics and reproductive outcomes in postoperative deep infiltrating endometriosis (DIE). Methods: This retrospective cohort study included 55 reproductive-aged patients who were diagnosed with DIE, wished to conceive and underwent resection surgery at the Obstetrics and Gynecology Hospital, Fudan University, from January 2009-June 2017. Those with any plausible infertility factor or abnormalities in the partner's semen analysis were excluded. Patient characteristics, preoperative symptoms, infertility history, intraoperative findings and reproductive outcomes were followed up and recorded. Risk factors for reproductive outcomes were identified for women who became pregnant versus those who did not by univariate logistic regression. Additionally, pre- and postoperative endometriosis health profile questionnaire-30 (EHP-30), Knowles-Eccersley-Scott Symptom questionnaire (KESS), Cox Menstrual Symptom Scale (CMSS) and Female Sexual Function Index (FSFI) scores were used to evaluate the effect of DIE surgery on quality of life. Results: The average age was 30.22 ± 3.62 years, with no difference between the pregnancy and nonpregnancy groups. The average follow-up time was 26.57 ± 14.51 months. There were 34 pregnancies (61.82%): 24 (70.59%) conceived spontaneously and 10 (29.41%) by in vitro fertilization (IVF). Twenty-eight patients (82.35%) had term deliveries. The interval between operation and pregnancy was 10.33 ± 5.6 (1-26) months. Univariate analysis showed that a lower endometriosis fertility index (EFI) score (EFI < 8) was a risk factor for infertility (OR: 3.17 (1.15-10.14), p = .044). For patients with incomplete surgery, postoperative gonadotropin-releasing hormone agonist (GnRHa) administration improved the pregnancy rate (p < 0.05). Regarding quality of life, there was significant improvement (p < 0.05) in the postoperative EHP-30, KESS and CMSS scores compared with preoperative scores in both groups. Although there was no obvious difference in FSFI scores, significant improvement in dyspareunia was observed (p < 0.05). Conclusions: Overall, the postoperative pregnancy rate of DIE patients was 61.82%. Surgical management of DIE for patients with complaints of pain and with pregnancy intentions was feasible and effective. Long-term expectant treatment should not be advised for patients with lower EFI scores (EFI < 8), and postoperative IVF-ET may be a good choice. More cases should be enrolled for further study, and randomized studies are required.

Predicting disease recurrence in patients with endometriosis: an observational study

Abstract:Background: Despite surgical and pharmacological interventions, endometriosis can recur. Reliable information regarding risk of recurrence following a first diagnosis is scant. The aim of this study was to examine clinical and survey data in the setting of disease recurrence to identify predictors of risk of endometriosis recurrence. Methods: This observational study reviewed data from 794 patients having surgery for pelvic pain or endometriosis. Patients were stratified into two analytic groups based on self-reported or surgically confirmed recurrent endometriosis. Statistical analyses included univariate, followed by multivariate logistic regression to identify risk factors of recurrence, with least absolute shrinkage and selection operator (Lasso) regularisation. Risk-calibrated Supersparse Linear Integer Models (RiskSLIM) and survival analyses (with Lasso) were undertaken to identify predictive features of recurrence. Results: Several significant features were repeatedly identified in association with recurrence, including adhesions, high rASRM score, deep disease, bowel lesions, adenomyosis, emergency room attendance for pelvic pain, younger age at menarche, higher gravidity, high blood pressure and older age. In the surgically confirmed group, with a score of 5, the RiskSLIM method was able to predict the risk of recurrence (compared to a single diagnosis) at 95.3% and included adenomyosis and adhesions in the model. Survival analysis further highlighted bowel lesions, adhesions and adenomyosis. Conclusions: Following an initial diagnosis of endometriosis, clinical decision-making regarding disease management should take into consideration the presence of bowel lesions, adhesions and adenomyosis, which increase the risk of endometriosis recurrence.
[Incidence of Deeply Infiltrating Endometriosis among 240 Cases of Pelvic Endometriosis and Analysis of Its Clinical and Pathological Characteristics].

Abstract:Objective: To evaluate the incidence of deeply infiltrating endometriosis (DIE) among patients of pelvic endometriosis confirmed by pathology and to make analysis of its clinical and pathological characteristics. Methods: From January 1, 2018 to December 31, 2018, clinical data of 240 cases of pelvic endometriosis diagnosed by laparoscopy and pathology hospitalized in Peking University First Hospital were analyzed retrospectively for the characteristics of symptoms, pelvic examination and anatomic distribution of endometriosis foci. Results: (1) Among 240 cases of pelvic endometriosis, 94 were diagnosed with DIE with an incidence of 39.2% (94/240); of them the diagnosis were made preoperatively in 44 cases (46.8%, 44/94). (2) Compared with those without DIE, patients with DIE had higher rates of secondary dysmenorrhea [53.2% (50/94) versus 38.4% (56/146), P=0.033], anal pain [43.6% (41/94) versus 28.1% (41/146), P=0.013], dyspareunea [39.4% (37/94) versus 18.5% (27/146), P=0.001] and frequent bowel movement [33.0% (31/94) versus 15.8%(23/146), P=0.002]. (3) Patients with DIE had higher rates of bad movement of uterus [21.3% (20/94) versus 6.8% (10/146), P=0.001], painful nodularity on uterosacral ligaments [26.6% (25/94) versus 6.2% (9/146), P<0.01], painful nodularity of posterior fornix [19.1% (18/94) versus 4.8% (7/146), P<0.01], blue nodule in vaginal wall [6.4% (6/94) versus 0 (0/146), P=0.003] by pelvic examination compared with those without DIE. (4) Ninety-four patients with DIE had a total of 162 nodules, of those 88 (54.3%, 88/162) located in uterosacral ligaments, 14 (8.6%, 14/162) in the rectum, 7 (4.3%, 7/162) in vaginal wall, 6 (3.7%, 6/162) in ureter, 4 in bladder (2.5%, 4/162), 2 (1.2%, 2/162) in Douglas pouch. Forty-three DIE patients (45.7%, 43/94) had more than one nodules. Patients with DIE had concomitant ovarian endometriosis in 69 cases (73.4%, 69/94), with a total of 103 endometrial cysts. (5) Patients with DIE had a higher rate of obliterated Douglas pouch [76.6% (72/94) versus 19.2% (28/146), P<0.01]. Conclusions: More than one third of patients with pelvic endometriosis have concomitant DIE with a lower rate of preoperative diagnosis. Pelvic pains, bad movement of uterus and painful nodulirity around cervix suggest the presence of DIE.
[Predictors of Recurrence and Prognosis in Patients with Stage I and II Endometrial Carcinoma].

Abstract:OBJECTIVE:To explore the high-risk clinicopathological features for the recurrence and prognosis of endometrial carcinoma diagnosed as International Federation of Gynecology and Obstetrics (FIGO) stagesIand II. METHODS:Three hundreds ninety-eight consecutive patients with clinical stage I, II endometrial adenocarcinoma underwent primary surgical therapy between Oct. 1990 to Oct. 2010 were studied retrospectively to analyze the correlation between clinicopathological risk factors and the disease recurrence or prognosis. RESULTS:Thirty-six patients (9.0%, 36/398) developed recurrence, 21 of them (58.3%, 21/36) relapsed within 5 years of treatment. The results shown that the disease recurrence were significantly associated with clinicopathological factors including: age [≤ 50 years versus > 50 years, 1.9% (2/103) versus 11.5% (34/295); P = 0.004];histologic types [adenocarcinoma versus serous papillary adenocarcinoma, 8.0% (27/338) versus 6/14; P = 0.000]; tumor grade [grade 1, 2 (7.4%, 25/338) versus grade 3 (17.9%, 10/56); P = 0.022]; depth of myometrial invasion [none (3.4%, 2/59) versus < 1/2 (7.5%, 18/240) versus ≥ 1/2 (16.2%, 16/99); P = 0.011];tumor size [ ≤ 2 cm (5.9%, 17/287) versus > 2 cm (17.1%, 19/111);P = 0.000]. peritoneal cytology [negative (6.9%, 22/317) versus positive (23.8%, 5/21); P = 0.019]; ER status [negative (20.8%, 11/53) versus positive (5.5%, 16/293); P = 0.000]; PR status [negative (30.0%, 15/50) versus positive ( 4.1%, 12/295); P = 0.031]. But lympho vascular space invasion (LVSI), p53 or PTEN status were not significant effect on recurrence and prognosis.Univariate analysis of survival indicated that age, histology, tumor grade, depth of myometrial invasion, tumor size, peritoneal cytology and PR status were correlated with overall survival (OS) and disease free survival (DFS; all P < 0.05), while ER status only impact on DFS (P < 0.05). CONCLUSIONS:The results showed that age, histologic type of carcinoma, myometrial invasion, tumor grade, peritoneal cytology, tumor size, ER and PR status are significant predictors for recurrence and prognosis of FIGO I, II stage endometrial carcinoma. Regular follow-up after treatment is essential for patients within 5 years.
Patterns of and Risk Factors for Recurrence in Women with Ovarian Endometriomas

Abstract:Objective: To identify risk factors for and the patterns of recurrence of endometrioma and of dysmenorrhea in women with ovarian endometrioma.Methods: We evaluated 710 consecutive patients operated on for ovarian endometriomas who were followed up for an average of 22.4 months. Twenty factors were examined to assess their effect on risk of recurrence of endometrioma and of dysmenorrhea using survival analysis. Hazard rate also was estimated to examine recurrence patterns.Results: For recurrence of endometrioma, the revised American Fertility Society (rAFS) score, younger age at surgery, and previous medical treatment of endometriosis were identified to be risk factors. For recurrence of dysmenorrhea, rAFS score was the only risk factor. For both recurrences, there was a constant hazard rate in the first 28-30 months after surgery, indicating that the recurrence in that period is completely random. After that period, the hazard rate increased dramatically.Conclusion: The total rAFS score, but not rAFS stage, is a risk factor for recurrence of both endometrioma and dysmenorrhea, indicating that the rAFS stage has little prognostic value. The existence of a completely random recurrence period may be a universal phenomenon, with its duration and the magnitude of recurrence risk determined by patient characteristics and quality of care. The second phase of much higher recurrence risk may reflect ectopic endometrium. Therefore, the identification of risk factors as well as patterns of recurrence should shed better light on possible causes for recurrence, which is now poorly understood.

Abstract:Study Objective To analyze clinical characteristics and fertility outcome in postoperative deep infiltrating endometriosis. Design Retrospective cohort study. Follow-up of about 27 months. Setting Tertiary-care university medical center. Patients or Participants 55 patients of productive age who were diagnosed of deep infiltrating endometriosis (DIE), undergone resection surgery and wished to conceive in our hospital from January 2009 to June 2017. Any plausible infertility factor or abnormality of partner's semen analysis were excluded. Interventions Clinical characteristics, fertility and surgical outcome were followed up and analyzed. Measurements and Main Results There were 34 pregnancies (61.82%) with 24 (70.59%) spontaneously and 10 (29.41%) in-vitro fertilization (IVF). 28 patients (82.35%) were term deliveries. The interval between operation and pregnancy was 10.33±5.6 (1-26) months. The pregnancy group had higher EFI (endometriosis fertility index, EFI) scores and more patients had cautery procedure in non-pregnancy group(p<0.05). In univariate analysis, lower EFI score (EFI<8) was found to be a risk factor of infertility (OR:3.17(1.15-10.14), P=0.044). Meanwhile there was no difference in age, number and size of lesion, lesion location, lesion involvement (ovary/adenomyosis), lesion residual and postoperative usage of GnRHa between pregnancy and non-pregnancy groups. However, for the patients having in-complete surgery, administration of postoperative GnRHa improved the pregnant rate(p<0.05). For evaluation of life quality, there was significant improvement (p<0.05) in postoperative scores of EHP-30, KESS and CMSS compared with preoperative in both groups. Although there was no obvious difference in FSFI scores, significant improvement in postoperative dyspareunia scores was observed (p<0.05). Conclusion In our study, the postoperative pregnancy rate of DIE is 61.82%. Surgical management for DIE patients with painful complaints and pregnancy intention were feasible and effective. Once surgery is decided, complete excision of DIE could be the first choice. For incomplete excision cases, administration of postoperative GnRHa is suggested. Postoperative IVF-ET may be a good choice for the patient with lower EFI score (EFI<8). However, more cases should enroll and additional studies are required. Risk factors for postoperative recurrence of ovarian endometriosis: long-term follow-up of 358 women

Abstract:<h3 class="Heading">Objective</h3><p class="Para">To explore the risk factors for the recurrence of endometrioma and the risk factors for the recurrence of endometriosis-related pain after long-term follow-up.</p><h3 class="Heading">Methods</h3><p class="Para">This study retrospectively analyzed 358 women with endometriomas who had a minimum of 5-years follow up after laparoscopic endometrioma excision, which was performed at Peking Union Medical College Hospital from January 2009 to April 2013. All women were divided into recurrence group and nonrecurrence group. Analysis was performed with regard to preoperative history, laboratory analysis, findings during surgery, and symptoms during follow-up, including improvement and recurrence.</p><h3 class="Heading">Results</h3><p class="Para">The cumulative incidence rates of recurrence from 5 to 10 years after surgery were 15.4, 16.8, 19.3, 22.5, 22.5, and 22.5%, respectively. Significant differences were found between two groups in terms of age at surgery (RR: 0.764, 95% CI: 0.615–0.949, <em class="EmphasisTypeItalic">p</em> = 0.015), duration of dysmenorrhea (RR: 1.120, 95% CI: 1.054–1.190, <em class="EmphasisTypeItalic">p</em> < 0.001), presence of adenomyosis (RR: 1.629, 95% CI: 1.008–2.630, <em class="EmphasisTypeItalic">p</em> = 0.046), CA125 level (RR: 1.856, 95% CI: 1.072–3.214, <em class="EmphasisTypeItalic">p</em> = 0.021) and severity of dysmenorrhea. The severity of dysmenorrhea (RR: 1.711, 95% CI: 1.175–2.493, <em class="EmphasisTypeItalic">p</em> = 0.005) and postoperative pregnancy (RR: 0.649, 95% CI: 0.460–0.914, <em class="EmphasisTypeItalic">p</em> = 0.013) were significantly correlated with endometrioma recurrence in the multivariate analysis. No significant associations were found between the recurrence rate and gravida, parity, body mass index, infertility, leiomyoma presence, the size of ovarian endometrioma, the presence of deep infiltrating endometriosis, disease stage or postoperative medication.</p><h3 class="Heading">Conclusions</h3><p class="Para">The severity of dysmenorrhea and postoperative pregnancy were independent risk factors for the recurrence of ovarian endometriomas after surgery during the long-time follow up.</p>

Abstract:ABSTRACT Problem To identify the predictive factors for the recurrence of chronic endometritis (CE) in infertile women. Method of study In this case‐control study, 1170 infertile women recovered from CE and underwent fertility treatment between December 2018 and August 2021. Among the 146 women (12.5%) who did not conceive or experienced pregnancy loss in 18 months after CE recovery, 105 consecutive women who underwent repeat endometrial biopsy for CD138 immunostaining and endometrial bacterial culturing were recruited. Thereafter, patients with and without CE recurrence were compared. Results The total recurrence rate of CE was 29.5% (31 women). Multivariable logistic regression analysis to determine predictive factors for CE recurrence revealed that hysteroscopic surgery (odds ratio [OR], 0.10; 95% confidence interval [CI], 0.02–0.56; p = 0.0009) and pregnancy loss (OR, 4.13; 95% CI, 1.31–13.05; p = 0.016) were significantly associated with decreased and increased CE recurrence rates, respectively. Also, reexamination with CD138 immunostaining after 16–18 months (OR, 9.75; 95% CI, 1.47–64.64; p = 0.024) was significantly associated with increased CE recurrence rates. Among 49 patients without a history of pregnancy loss, the cumulative CE recurrence rates after 6, 12, and 18 months were 5.6%, 13.5%, and 20.4%, respectively. Conclusions We recommend reexamination with endometrial CD138 immunostaining in patients with pregnancy loss or long‐term infertility during fertility treatment. Hysteroscopic surgery without antibiotic therapy for CE associated with intrauterine abnormalities is also recommended.

Abstract:This study addresses the challenges in the early diagnosis of deep infiltrating endometriosis (DIE) by exploring the potential role of the deubiquitinating enzyme USP14. By analyzing the GSE141549 dataset from the Gene Expression Omnibus (GEO) database, using bioinformatics methods and three machine learning algorithms (LASSO, Random Forest, and Support Vector Machine), the key feature gene USP14 was identified. The results indicated that USP14 is significantly upregulated in DIE and exhibits good predictive value (AUC = 0.786). Further analysis revealed the important role of USP14 in muscle function, cellular growth factor response, and maintenance of chromosome structure, and its close association with various immune cell functions. Immunohistochemical staining confirmed the high expression of USP14 in DIE tissues. This study provides a new molecular target for the early diagnosis of DIE, which holds significant clinical implications and potential application value.

Abstract:Purpose To explore the relationship between different artificial reproductive treatment (ART) strategies and tumor outcomes, by analyzing clinical data of patients with endometrial carcinoma (EC) and atypical endometrial hyperplasia (AEH). Methods This retrospective study was performed in a tertiary hospital. Patients (n=131) with EC or AEH, who underwent in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) treatment between June 2010 and June 2021, were divided into a recurrence group and a non-recurrence group. Clinical characteristics and tumor outcomes were assessed. Results 131 patients were followed up for 4-132 months; 33 patients had recurrence, the recurrence rate was 25.2%, 3-year recurrence-free survival (RFS) rate was 83.2 ± 3.4%, and the 5-year RFS rate was 72.9 ± 4.4%. Factors including the frequency of controlled ovarian stimulation (COS) and the total days of ovarian stimulation had no significant effect on the recurrence of tumor lesions (p=0.368 and 0.969, respectively). Histology type (HR: 4.94, 95%CI: 2.41-10.15, p < 0.001) and successful/un successful live birth (HR: 0.30, 95%CI: 0.14-0.65, p =0.003) were independent factors of recurrence. Twenty-two of the 82 patients who received a single COS had recurrence. Different COS protocols, the total dose of gonadotropin (Gn), and the serum E 2 level on the trigger day had no significant effect on recurrence ( p =0.326, 0.889 and 0.468, respectively). Conclusions The degree at which an endometrial lesion progresses into carcinoma is a key factor affecting the recurrence of EC/AEH in patients after IVF/ICSI treatment, and successful live birth is a protective factor for the recurrence of endometrial lesions. Different COS protocols and COS frequencies, as well as the dosage and duration of Gn used during IVF did not affect the recurrence of endometrial lesions.
