Abstract:Objective To explore the risk factors for recurrent laryngeal nerve paralysis(RLNP) after esophagectomy, analyze the influence of RLNP on the completion rate of adjuvant therapy and long-term survival. Methods The clinical and pathological data of 762 consecutive cases of esophageal squamous cell carcinoma in the department of thoracic surgery, Shanghai chest hospital from January 2013 to December 2015 were analyzed retrospectively. Results The incidence of RLNP in this study was 13.6%. Multivariate analysis showed that McKeown procedure and lymph node metastasis along the recurrent laryngeal nerve were independent risk factors for RLNP(P 0.05). The 3 years-OS of RLNP-delayed adjuvant therapy group was lower than that of no RLNP group (48.4% vs 57.1%), and 3 years-DFS of RLNP-delayed adjuvant therapy group was lower than that of no RLNP group (38.7% vs 47.3%), but the difference was not statistically significant (P>0.05). Conclusions Lymph node metastasis along the recurrent laryngeal nerve were independent risk factors for RLNP. Adjuvant chemotherapy was delayed by RLNP which may deteriorate long-term prognosis of histological node positive patients. Abstract:The hyperthermal liquid, spray and smog caused by energy-based surgical devices (EBDs) may be the potential risk factors for recurrent laryngeal nerve (RLN) thermal injury, but not been reported previously. We aim to demonstrated it both in thyroid surgeries and animal experiments. 321 thyroid patients and 32 rats were divided into two groups respectively. A type of endoscopic gauze was implemented in the protected group to prevent nerve from thermal risks. Electromyography and laryngoscope were recorded to determine nerve’s conductive ability. Microstructural morphological damage to nerves and muscles were evaluated by light and electron microscopy in the animal experiments. After thyroid surgeries, the incidence of transient vocal cord (VC) palsy was decreased in the protected group (1.4%) compared with the conventional group (4.7%) (P = 0.037). In animal experiments, the average nerve conductive velocity was significantly decreased in the conventional group (5.8 ± 2.1 vs. 25.5 ± 4.7 m/s, P < 0.01). Typical acute morphological thermal changes of edema, coagulation and homogenization in nerve tissues were recorded by light microscopy in the conventional group. Four weeks later, the average weight of gastrocnemius muscles in the injured limb was significantly decreased (1.3 ± 0.2 vs. 2.2 ± 0.2 g, P < 0.01), while no significant difference was noted in the protected group (2.0 ± 0.3 vs. 2.3 ± 0.4 g, P = 0.61). Hyperthermal liquid, spray or smog may represent risk factors contributing to RLN thermal injury caused by EBDs. Damage to the nerve’s conductive ability and morphological changes in nerves and muscles likely represent typical responses to thermal injury. Implementation of endoscopic gauze may be a promising method to protect the RLN from thermal damage especially in endoscopic thyroid surgeries.

Abstract:Abstract Development of thoracoscopic surgical skills and concepts in minimally invasive esophagectomy (MIE) and robotic-assisted (MIE) have provided us with a more wider anatomical understanding of lymphadenectomy and the relationship between lymph node dissection and recurrent laryngeal nerve paralysis (RLNP). However, RLNP correlates to postoperative complications even in prognosis. Early warning and novel diagnosis are urgent in the management of esophageal cancer patients. Taking a novel approach to researching the diagnosis of RLNP, we evaluated movement of the vocal cords and arytenoid cartilages using ultrasonography in patients who underwent thoracoscopic esophagectomy after extubating of trachea cannula at department of critical care medicine. Further ultrasonography combined endoscopy evaluation and verification were performed on day 5–7 before hospital discharge. RLNP occurred in 4 of the 20 patients’ cohort (20%). All of 20 patients were finished ultrasonography combined endoscopy evaluation. The diagnostic accuracy of RLNP by ultrasonography compared to endoscopy had a sensitivity of 100.0% (4/4), a specificity of 87.5% (14/16). The positive predictive value was 66.7% (4/6), negative predictive value was 100.0% (14/14), a false positive rate of 33.3% (2/6), and a false negative rate of 0% (0/14). Ultrasonography is a quite non-invasive and effective measure to diagnose and evaluate RLNP after esophagectomy compared to traditional examination of endoscopy. It also has a perspective future in evaluation for the recovery status of RLNP.

Long-Term Outcomes of Recurrent Laryngeal Nerve Repair/Reconstruction in Oncological Settings

Abstract:Vocal cord paralysis results from involvement of the recurrent laryngeal nerve (RLN), either before the surgery or following excision. Coaptation of the resected edges utilising microsurgical techniques is the most promising therapeutic strategy available for RLN excision. The RLN can be repaired by direct epineural coaptation or using nerve grafts adhering to recommended microsurgical techniques. This article aims to convey our experience with RLN resections/injuries and their subsequent effects. We assessed the RLN repairs that our institute had completed from April 2018 to September 2023(5 years and 5 months) including follow-up of minimum 1 year. The Functional Oral Intake Scale (FOSI) was applied to assess dysphagia, aspiration risk, and glottic gap by laryngoscopy, and GRBAS (Grade, Roughness, Breathiness, Asthenia, Strain) comprised the basis for the assessment of voice quality by speech specialists. Surgical technique included using 9-0 Ethilon either for primary repair or for repair with a nerve graft. Ten patients were included in the study; six (60%) of them were male with the median age of 32 years. At 12-month follow-up, only 10% (n = 1) was found to have dysphagia. Only 10% (n = 1) had a mild harsh voice. This same patient was the only patient to show a minimal remnant glottic gap. Ten percent (n = 1) patient showed B/l cord mobility at 12 months, while 30% (n = 3) showed flickering movements of the affected vocal cord. In all the patients, the opposite vocal cord was found to be compensating. Thus, immediate repair of RLN is helpful along with the general physiological adaptation of vocal cords to improve phonation and reduce aspiration and dysphagia risks, thus helping to improve the quality of life. The right procedure should choose from the armoury after careful intraoperative assessment.
Analysis of Risk Factors for Acute Lung Injury/acute Respiratory Distress Syndrome after Esophagectomy

Abstract:OBJECTIVE:To explore the incidence and risk factors for the acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) after resection of esophageal carcinoma.METHODS:We retrospectively analyzed 422 consecutive patients admitted to the Department of Critical Care Medicine with esophageal carcinoma undergoing esophagectomy from January 2010 to December 2016 in Peking University Cancer Hospital. ALI/ARDS were diagnosed, the patients were divided into ALI/ARDS group and control group without ALI/ARDS, the differences of clinical features were contrasted between the two groups, and the multivariate Logistic regression modeling was used to identify the independent risk factors for ALI/ARDS.RESULTS:In the study, 41 ALI/ARDS cases were diagnosed, making up 9.7% (41/422) of all the enrolled patients undergoing esophagectomy. Comparisons of the ALI/ARDS group and the control group indicated significant statistical differences in the average length of their hospital stay [(18.9±9.7) d vs. (14.8±3.6) d, P=0.011], the proportion of the patients who needed mechanical ventilation support [51.2% (21/41) vs. 9.4% (36/381), P<0.001] and in-hospital mortality [31.7% (13/41) vs. 5.0% (19/381), P<0.001]. Univariate analysis showed significant differences between the patients with ALI/ARDS and without ALI/ARDS in smoking history (P=0.064), preoperative forced expiratory volume in one second/forced vital capacity (FEV1/FVC) (P=0.020), diffusing capacity of the lung for carbon monoxide (DLCO) (P=0.011), body weight index (BMI) (P=0.044), American Society of Anesthesiologists (ASA) physical status classification (P=0.049) and one lung ventilation duration (P=0.008), while multivariate Logistic regression analysis indicated that preoperative FEV1/FVC (OR=1.053, P=0.016, 95%CI 1.010-1.098), ASA physical status classification (OR=2.392, P=0.033, 95%CI 1.073-5.335) and one lung ventilation duration (OR=0.994, P=0.028, 95%CI 0.989-0.999) were the independent risk factors for ALI/ARDS after esophagectomy.CONCLUSION:ALI/ARDS was a serious complication in patients undergoing esophagectomy associated with increment in length of hospital stay and in-hospital mortality. Multivariate Logistic regression analysis indicated that preoperative FEV1/FVC, ASA classification and one lung ventilation duration were the independent risk factors for ALI/ARDS after esophagectomy. Carefully assessing the patient before operation, shortening one lung ventilation duration were the key points in preventing ALI/ARDS after esophagectomy.
Elevated Respiratory Complications Following Robotic-Assisted Lobectomy: A National Cohort Analysis of 26,140 Cases.

Abstract:BackgroundRobotic-assisted lobectomy (RL) is increasingly used nationally, but little comparative data exist on its safety compared with open lobectomy (OL) or video-assisted lobectomy (VL). This study aimed to estimate the risk of perioperative complications for RL, VL, or OL.MethodsAdmissions were identified from the hospital administrative data collected between 2015 and 2019. Propensity score matching and inverse probabilistic weighting were used to account for selection bias. Logistic and quantile regression models were applied to determine perioperative outcome differences.ResultsWe identified 26,140 cases of which 5337 (20.4%), 12,680 (48.5%), and 8123 (31.1%) underwent RL, VL, and OL. RL and VL were associated with lower complication rates, shorter lengths of stay, and fewer mortality risks. RL was associated with significantly elevated risks for perioperative respiratory complications (adjusted odds ratio 1.10, p = 0.010).ConclusionRelatively low rates of perioperative complications for VL and RL, and higher respiratory complication rates in RL are concerning.
Occurrence and recovery of vocal cord paralysis after minimally invasive McKeown esophagectomy, risk factors, and clinical outcome

Abstract:Background: This study aimed to observe the occurrence of recurrent laryngeal nerve injury after McKeown esophagectomy for esophageal squamous cell carcinoma, as well as its recovery and influencing factors within 7 months after surgery. Methods: From July 2020 to July 2021, among all patients who underwent minimally invasive McKeown esophagectomy, 90 patients who developed vocal cord paralysis after surgery were included in the study. These patients underwent endoscopic vocal cord function assessment every 1 to 2 months and continued until 7 months postoperatively. Results: Among all 388 patients undergoing esophagectomy, 23.2% (90/388) of patients suffered postoperative vocal cord paralysis. Left, right, and bilateral injuries were confirmed in 73 (81.1%), 12 (13.3%), and 5 patients (5.6%), respectively. With a median recovery time being 183 days, the cumulative overall recovery rate was 65.4% at 7 months, 68.6% for the left side, 55.6% for the right, and 20.0% for bilateral injuries. In multivariable analysis, cervical paraoesophageal lymph node dissection and conventional thoracoscopic-assisted esophagectomy were demonstrated to be independent risk factors associated with non-recovery of vocal cord paralysis. Conclusions: After intensive endoscopic follow-up, a cumulative vocal cord paralysis recovery rate of 65.4% within 7 months was observed in patients after minimally invasive McKeown esophagectomy. Cervical paraoesophageal lymph node dissection and conventional thoracoscopic-assisted esophagectomy were demonstrated to be risk factors hindering vocal cord paralysis recovery.
Intraoperative Recurrent Laryngeal Nerve Monitoring for Esophagectomy: A National Cohort Study

Abstract:Background: Lymphadenectomy around the recurrent laryngeal nerve (RLN) is an essential part of curative surgery for esophageal cancer. Although several single-center studies have shown that intraoperative nerve monitoring (IONM) can avoid RLN palsy, this has not been confirmed in a large-scale multicenter study. This study used a national database to evaluate whether IONM can reduce postoperative RLN palsy during minimally invasive esophagectomy (MIE) for esophageal cancer. Methods: We retrieved data of patients with esophageal cancer who underwent 3-field thoracoscopic or robotic MIE with cervical anastomosis with IONM(+) (502 patients) and without IONM(-) (4353 patients) from April 2020 to March 2022 from the Diagnosis Procedure Combination database in Japan. We used propensity score-matching analysis to compare the frequency of postoperative RLN palsy and respiratory complications between the IONM(+) group and IONM(-) group. Results: The postoperative RLN palsy rate was significantly lower in the IONM(+) than IONM(-) group (odds ratio, 0.24; 95% CI, 0.13-0.46). The respiratory complication rate was also significantly lower in the IONM(+) than in the IONM(-) group (odds ratio, 0.66; 95% CI, 0.45-0.97). The anesthesia time was significantly longer in the IONM(+) group (regression coefficient, 60.1 minutes; 95% CI, 44.2-76.9 minutes). The length of postoperative hospitalization tended to be shorter in the IONM(+) than in the IONM(-) group (regression coefficient, -1.39 days; 95% CI, -3.91 to 1.14). Conclusions: This national cohort study showed that IONM during 3-field MIE for esophageal cancer was associated with a reduction of postoperative RLN palsy and respiratory complications.
Mechanisms of recurrent laryngeal nerve injury in endoscopic thyroidectomy for papillary thyroid carcinoma: A large data from China

Abstract:Purpose: To investigate the mechanisms of recurrent laryngeal nerve (RLN) injury during endoscopic thyroidectomy via breast approach (ET-BA) in patients with papillary thyroid carcinoma (PTC). Methods: The records of 416 PTC patients who underwent ET-BA with intraoperative neural monitoring (IONM) from May 2015 to May 2021 in Beijing Friendship Hospital affiliated to Capital Medical University were retrospectively analyzed. Results: All patients were women. Mean age was 37.80 (7.87) years. The ET-BA was performed in 416 patients. Overall incidence of RLN injury was 4.3% (18 patients). Injury was transient in 13 patients (3.1%) and permanent in five (1.2%). Macroscopic physical changes were apparent in the injured nerve in five patients (27.8%) and postoperative hoarseness or cough after drinking water were present in 11 (61.1%). Two RLN injuries occurred during nerve identification at the RLN laryngeal entry point into the surgical field, 15 during early nerve dissection somewhere between the first 0.5 and 2 cm of the nerve's course through the surgical field, and one occurred distal to 2 cm. The percentage of patients with separation, transection, traction and thermal mechanisms of injury was 27.8%, 22.2%, 22.2%, and 16.7%, respectively. The mechanism of injury was unknown in 11.1%. Conclusions: RLN injury may still occur during ET-BA despite endoscopic magnification and early nerve identification even when IONM is used. Separation, transection, and traction injuries were the most frequent causes of injury.
Development of a prediction model for the risk of recurrent laryngeal nerve lymph node metastasis in thoracolaparoscopic esophagectomy with cervical anastomosis

Abstract:BACKGROUND: There are no effective preoperative diagnostic measures to predict the probability of left and right recurrent laryngeal nerve (RLN) lymph node (LN) metastasis using preoperative clinical data in patients undergoing thoracolaparoscopic esophagectomy with cervical anastomosis.METHODS: We retrospectively reviewed the clinical data of 1,660 consecutive patients with thoracic esophageal cancer who underwent esophagectomy with cervical anastomosis at the Department of Thoracic Surgery at the First Affiliated Hospital of Zhengzhou University between January 2015 and December 2020.RESULTS: A total of 299 and 343 patients who underwent left (Cohort 1) and right (Cohort 2) RLN LN dissection were included in the final analyses. The analyses were conducted within each cohort. Among the 299 patients in Cohort 1, left RLN LN involvement was found in 41 patients (13.7%). A multivariable analysis showed that age, tumor location, and short axis were significantly associated with RLN LN metastasis (all P<0.05). Among the 343 patients in Cohort 2, right RLN LN involvement was found in 65 patients (19.0%). A multivariable analysis showed that computed tomography (CT) appearance, tumor location, long axis, and short axis were significantly associated with RLN LN metastasis (all P<0.05). Based on the results of the multivariable analyses, we constructed nomograms that could estimate the probability of RLN LN metastasis. Finally, we stratified the 2 cohorts into risk subgroups using a recursive partitioning analysis (RPA). The risk of left and right RLN LN metastasis was found to be 9.3% and 7.5%, 27.3% and 21.4%, and 52.4% and 47.3% for the low-risk, intermediate-risk, and high-risk groups, respectively.CONCLUSIONS: Our nomograms and RPAs appear to be suitable for the risk stratification of left and right RLN LN metastasis in patients undergoing thoracolaparoscopic esophagectomy with cervical anastomosis. This tool could be used to help clinicians to select more effective locoregional treatments, such as surgical protocols and radiation area selection.

Abstract:Esophageal cancer is a lethal neoplasm with frequent lymph node metastasis. Thoracic lymph node dissection is an essential part of radical surgery, especially the lymph nodes along recurrent laryngeal nerves (RLNs). In recent years, minimally invasive esophagectomies have been increasingly performed around the world. In particular, the robotic system provides great accessibility for subtle manipulations in lymph node dissection. This review focuses on the characteristics of intrathoracic lymph node metastasis, the impact of robotic system on intrathoracic lymph node dissection and in particular, RLN lymph node dissection, and lymphadenectomy-related complications.

Abstract:Abstract Background Surgical resection with radical lymphadenectomy is a pivotal component in the multidisciplinary therapy of esophageal cancer. Minimally invasive esophagectomy was shown to be effective in reducing the morbidity and was adopted increasingly. As a novel minimally invasive technique, robot-assisted esophagectomy remains in the initial stage ofapplication. This study describes the single-institution experience of robotic esophagectomy. Methods Between March 2016 and October 2017, 20 consecutive patients underwent robot assisted esophagectomy at our institute. The thoracic and abdominal mobilization were all performed with the assistance of the robot. We retrospectively collected the operative data and postoperative outcomes. Results The majority of patients were male (80%), and the median age was 62 years. The average operative time was 342 minutes (range 280–440). The average blood loss was 112 ml (range 50–400). No patient experienced conversion to a thoracotomy or laparotomy. R0 resection was achieved in all patients, the mean number of dissected lymph nodes was 19 (range 8–32). No 90-day operative mortality was observed, and postoperative complications were present in 8 of 20 patients (40.0%). Pulmonary complications were the most common event and were observed in 3 patients. Two patients experienced an anastomotic leak. Conclusion Our study demonstrated that robot-assisted esophagectomy is a safe and technically feasible alternative to conventional thoraco-laparoscopic esophagectomy. Abstract:Purpose: The recurrent laryngeal nerve lymph nodes (RLN LNs) are among the most common metastatic sites in esophageal cancer, and the dissection of these lymph nodes (LNs) is considered beneficial. The purpose of this study was to evaluate the characteristics of RLN LN metastases from esophageal squamous cell carcinoma, and the effects of these metastases on the prognosis of patients. In addition, we aimed to determine the reasonable range of dissection of regional LNs. Methods: The clinical data from 348 patients who underwent resection for esophageal carcinoma were retrospectively analyzed. Results: Recurrent laryngeal nerve palsy occurred in 37.6% of the patients. In a subgroup of patients with lower esophageal tumors, cervical LN metastases were significantly more common in patients with positive rather than negative RLN LNs. The primary tumor site, tumor differentiation, and tumor invasion depth were factors that significantly influenced RLN LN metastasis. Multivariate analysis revealed that RLN LN metastasis was a significant factor associated with overall survival (OS) and disease-free survival (DFS) (p<0.001). Conclusion: Metastasis to RLN LNs is a reliable indicator of cervical LN metastasis in middle/lower thoracic esophageal cancer. RLN LN metastasis may act as a prognostic indicator for patients with esophageal squamous cell carcinoma.

Abstract:Objective To investigate the protective measures for recurrent laryngeal nerve in cervical esophageal reconstruction.Methods Clinical data of 31 patients who underwent cervical esophageal reconstruction without preoperative hoarseness from 1983 to 2005 were analyzed retrospectively.The patients included 23 cases of cervical esophageal carcinoma and 8 cases of severe pharyngoesophageal stenosis.The treatment was selected according to the patients' state of illness.Esophagectomy without thoracotomy was carried out in all cases.Reconstruction of the esophagus consisted of 17 stomach pulling-up,10 colon interposition,and 4 free jejunal graft.Results All patients succeeded in the deglutitory function and decannulation.The average course for rehabilitation of normal devouring function was 22.1days and that from operation to decannulation was 45.3 days.Complications included hoarseness,bucking,pulmonary infection,pharyngeal fistulas,cervical stomal fistulae,cervical stomal stenosis and splitting of abdominal wall.Conclusion The essential measures in cervical esophageal reconstruction can decrease nerval injure and elevate quality of life.

Abstract:Background: Whether extended robot-assisted esophagectomy with thoracic duct resection (RAE-TDR) has a favorable effect on esophageal cancer patients is not yet known. This study sought to analyze the safety and efficiency of RAE-TDR. Methods: From January 2019 to July 2020, 73 thoracic duct (TD)-resected and 127 TD-preserved consecutive patients who received standard RAE McKeown surgery were enrolled in this study. The perioperative-related indicators of recurrence-free survival (RFS) and overall survival (OS) at 1-year were compared between the 2 groups. Results: In relation to morbidity, the Clavien- Dindo (CD) classifications for grades greater than or equal to II-III were similar between the 2 groups (P>0.05). The number of retrieved total lymph nodes (LNs) and mediastinal nodes were significantly higher in the TD-resected group than in the TD-preserved group ( total lymph nodes: 29.0 +/- 11.1 vs. 25.1 +/- 8.5, P=0.006; mediastinal nodes: 19.5 +/- 8.0 vs. 16.1 +/- 5.5, P=0.002). Additionally, more metastatic TD-related LNs were harvested in cT3-4 patients (2.3 +/- 3.7 vs. 1.7 +/- 2.8; P=0.21). The rates of LN recurrence and local recurrence were similar between the 2 groups (LN recurrence: 6.8% vs. 7.1%, P>0.99; local recurrence: 1.4% vs. 2.4%, P>0.99). The OS and RFS at 1-year were equivalent regardless of the TD procedure at each stage (P>0.05). However, the rate of hematogenous metastasis in the TD-resected group was significantly elevated (17.8% vs. 7.9%; P=0.034). Conclusions: RAE-TDR may help to improve total and metastatic LN harvest, especially in patients with advanced esophageal squamous cell carcinoma (ESCC) without increasing the intra- and post-operative adverse events. However, RAE-TDR did not lead to a decrease in the local recurrence rate within the short-term follow-up period. Whether the increase in distant metastasis rate in the TD-resected group was associated with relevant immune system damage is unclear. Thus, nonselective RAE-TDR is not routinely recommended. Abstract:BackgroundRecurrent laryngeal nerve (RLN) invasion by thyroid carcinoma represents an advanced disease status with potentially significant co‐morbidity.MethodsIn a retrospective single‐center study, we included patients with invaded RLNs operated on while using nerve monitoring techniques. We studied pre‐, intra‐, and postoperative parameters associated with postoperative vocal cord paralysis (VCP); 5‐year recurrence‐free survival (RFS); and 5‐year overall survival (OS) in addition to two subgroup analyses of postoperative VCP in patients without preoperative VCP and based on source of RLN invasion.ResultsOf 65 patients with 66 nerves‐at‐risk, 39.3% reported preoperative voice complaints. Preoperative VCP was documented in 43.5%. The RLN was invaded by primary tumor in 59.3% and nodal metastasis in 30.5%. Papillary thyroid carcinoma was the most common pathologic subtype (80%). After 6 months, 81.8% had VCP. Complete tumor resection of the RLN was not associated with 5‐year RFS (p = 0.24) or 5‐year OS (p = 0.9). Resecting the RLN did not offer statistically significant benefit on 5‐year RFS (p = 0.5) or 5‐year OS (p = 0.38). Radioactive Iodine (RAI) administration was associated with improvement in 5‐year RFS (p = 0.006) and 5‐year OS (p = 0.004). Patients without preoperative VCP had higher IONM amplitude compared with patients with VCP. After a mean follow‐up of 65.8 months, 35.9% of patients had distant metastases, whereas 36.4% had recurrence.ConclusionPreoperative VCP accompanies less than half of patients with RLN invasion. Invaded RLNs may have existent electrophysiologic stimulability. Complete tumor resection and RLN resection were not associated with better 5‐year RFS or OS, but postoperative RAI was.Level of Evidence4 Laryngoscope, 2022

Abstract:Objective To investigate the risk factors for postoperative complications and anastomotic leakage after robotic surgery for mid and low rectal cancer and their influence on long-term outcomes. Methods A total of 641 patients who underwent radical mid and low rectal cancer robotic surgery at Zhongshan Hospital Fudan University from January 2014 to December 2018 were enrolled in this study. The clinicopathological factors of the patients were collected. The risk factors for short-term outcomes of complications and anastomotic leakage were analyzed, and their influences on recurrence and overall survival were studied. Results Of the 641 patients, 516 (80.5%) underwent AR or LAR procedures, while 125 (19.5%) underwent the NOSES procedure. Only fifteen (2.3%) patients had stoma diversion. One hundred and seventeen patients (17.6%) experienced surgical complications. Anastomotic leakage occurred in 44 patients (6.9%). Eleven patients (1.7%) underwent reoperation within 90 days after surgery. Preoperative radiotherapy did not significantly increase anastomotic leakage in our study (7.4% vs. 6.8%, P = 0.869). The mean postoperative hospital stay was much longer with complication (10.4 vs. 7.1 days, P<0.05) and leakage (12.9 vs. 7.4 days, P < 0.05). Multivariate analysis showed that male sex (OR = 1.855, 95% CI: 1.175-2.923, P < 0.05), tumor distance 5 cm from the anus (OR = 1.563, 95% CI: 1.016-2.404, P < 0.05), and operation time length (OR = 1.563, 95% CI: 1.009-2.421, P < 0.05) were independent risk factors for complications in mid and low rectal cancer patients. The same results for anastomotic leakage: male sex (OR = 2.247, 95% CI: 1.126-4.902, P < 0.05), tumor distance 5 cm from the anus (OR = 2.242, 95% CI: 1.197-4.202, P < 0.05), and operation time length (OR = 2.114, 95% CI: 1.127-3.968, P < 0.05). The 3-year DFS and OS were 82.4% and 92.6% with complication, 88.4% and 94.0% without complication, 88.6% and 93.1% with leakage, and 87.0% and 93.8% without leakage, respectively. The complication and anastomotic leakage showed no significant influences on long-term outcomes. Conclusion Being male, having a lower tumor location, and having a prolonged operation time were independent risk factors for complications and anastomotic leakage in mid and low rectal cancer. Complications and anastomotic leakage might have no long-term impact on oncological outcomes for mid and low rectal cancer with robotic surgery.
Robot Assisted Esophagectomy for Esophageal Squamous Cell Carcinoma

Abstract:Background: This study aims to report our experience with robot assisted esophagectomy (RAE) for the treatment of resectable esophageal squamous cell carcinoma (ESCC).Methods: A series of 249 consecutive patients diagnosed with ESCC who underwent RAE from November 2015 to December 2017 at Shanghai Chest Hospital were evaluated, and their clinical data were reviewed retrospectively.One hundred patients were equally divided into four groups according to the surgery order, and the short-term outcomes in each group were analyzed.Results: Overall, 249 patients (201 males and 48 females) with a mean age of 63.4±7.3 years who underwent RAE were analyzed.The thoracic procedure was successfully performed with the assistance of a robot.The mean total duration was 250.6±58.4mins, and the estimated blood loss was 215.5±87.6 mL.R0 resection was performed in 232 (93.2%) patients with a mean total number of dissected lymph nodes of 18.5±9.1 and mean yield of lymph nodes along the recurrent laryngeal nerve (RLN) of 4.4±3.2.The median postoperative hospital stay was 11 days, and no 90-day mortality was observed.Forty-five (18.1%) patients experienced pulmonary complications, and the recurrent laryngeal nerve injury were observed in 38 (15.3%) patients.A significant reduction in thoracic duration was observed after the initial 25 cases (P<0.001).After 50 cases, the dissection of total lymph nodes, mediastinum lymph nodes and lymph nodes along the RLN were significantly improved (P<0.001,P<0.001, P=0.001, respectively) with a shorter postoperative hospital stay (P=0.005).Conclusions: RAE is a safe and feasible alternative surgical approach for resectable esophageal carcinoma and is associated with a large yield of lymph nodes, especially along the RLN.The surgeon will reach a plateau of operative duration after 25 cases and a plateau of lymphadenectomy after 50 cases.
