Anterior vitrectomy and second stage phacoemulsification for the treatment of acute angle-closure glaucoma with extremely shallow anterior chamber and cataract

Abstract:Objective:To evaluate the efficacy of anterior vitrectomy and second stage phacoemulsification combined with intraocular lens implantation for the treatment of acute angle-closure glaucoma with extremely shallow anterior chamber and cataract.Methods:The clinical data of 35 eyes of 28 patients with acute angle-closure glaucoma and cataract from Sep. 2018 to Jul. 2019 in Shanghai Tongji Hospital were analyzed retrospectively. All cases with extremely shallow anterior chamber underwent anterior vitrectomy and second stage phacoemulsification after IOP stabilization. The therapeutic efficacy was observed during 1 year of follow-up.Results:At 1 year after surgery, the intraocular pressure decreased significantly from (54.47±5.01) mmHg to (17.70±2.35) mmHg (1 mmHg =0.133 kPa) ( t=34.519, P<0.001), and the anterior chamber depth increased significantly from (0.47±0.34) mm to (2.79±0.47) mm( t=0.890, P<0.001). All patients showed significant improvement in BCVA postoperatively and no intra- or post-operative complications occurred. Conclusion:Anterior vitrectomy and second stage phacoemulsification combined with intraocular lens implantation is safe and effective for acute angle-closure glaucoma with cataracts and extremely shallow anterior chamber.
