Interactive Effects of Meteorological Factors and Particulate Pollutants on Childhood Asthma: A Time Series Analysis in Shanghai, China

Abstract:Abstract Background : Previous studies have revealed that meteorological factors or ambient air pollutants were associated with childhood asthma, but their interactive effects are scarcely known. Methods: In order to investigate the potential interactions between meteorological factors and air pollutants on childhood asthma, we obtained daily data on meteorological factors (mean temperature, mean relative humidity, and mean air pressure), air particulate matter (PM) with diameter < 2.5 μm (PM 2.5 ) and < 10 μm (PM 10 ), and outpatient visits for childhood asthma during 2007-2017 in Shanghai, China. Independent effects of meteorological factors and PM on childhood asthma were estimated using distributed lag non-linear model (DLNM), and then relative excess risk due to interaction (RERI) was calculated to assess their potential interactions. Results: A total of 976, 350 outpatient visits for childhood asthma were included in this study. Significant lagged and non-linear effects of meteorological factors and PM exposure on childhood asthma were observed in DLNM analyses. Further, in interaction analysis, we found statistically significant antagonistic interaction between mean temperature and PM (RERI: -0.14, 95%CI: -0.15, -0.12 for PM 2.5 ; RERI: -0.09, 95%CI: -0.10, -0.08 for PM 10 ) and synergistic interaction between mean air pressure and PM (RERI: 0.16, 95%CI: 0.14, 0.18 for PM 2.5 ; RERI: 0.17, 95%CI: 0.15, 0.19 for PM 10 ) on childhood asthma. Subgroup analyses by sex and age as well as sensitivity analyses of alternative degree of freedoms for calendar time, meteorological factors and PM are performed and confirmed the robustness of our results. Conclusion: Our findings provide supporting evidence that meteorological factors and PM might interactively affect childhood asthma, including antagonistic interaction between temperature and PM and synergistic interaction between air pressure and PM.

Abstract:Objective: To synthesize evidence regarding the relationship between outdoor air pollution and risk of asthma exacerbations in single lag0 and lag1 exposure patterns.Methods: We performed a systematic literature search using PubMed, Embase, Cochrane Library, Web of Science, ClinicalTrials, China National Knowledge Internet, Chinese BioMedical, and Wanfang databases. Articles published until August 1, 2020 and the reference lists of the relevant articles were reviewed. Two authors independently evaluated the eligible articles and performed structured extraction of the relevant information. Pooled relative risks (RRs) and 95% confidence intervals (CIs) of lag0 and lag1 exposure patterns were estimated using random-effect models.Results: Eighty-four studies met the eligibility criteria and provided sufficient information for meta-analysis. Outdoor air pollutants were associated with increased risk of asthma exacerbations in both single lag0 and lag1 exposure patterns [lag0: RR (95% CI) (pollutants), 1.057(1.011, 1.103) (air quality index, AQI), 1.007 (1.005, 1.010) (particulate matter of diameter ≤ 2.5 μm, PM2.5), 1.009 (1.005, 1.012) (particulate matter of diameter, PM10), 1.010 (1.006, 1.014) (NO2), 1.030 (1.011, 1.048) (CO), 1.005 (1.002, 1.009) (O3); lag1:1.064(1.022, 1.106) (AQI), 1.005 (1.002, 1.008) (PM2.5), 1.007 (1.004, 1.011) (PM10), 1.008 (1.004, 1.012) (NO2), 1.025 (1.007, 1.042) (CO), 1.010 (1.006, 1.013) (O3)], except SO2 [lag0: RR (95% CI), 1.004 (1.000, 1.007); lag1: RR (95% CI), 1.003 (0.999, 1.006)]. Abstract:Background: Early life is suggested to be a critical time in determining subsequent asthma development, but the extent to which the effect of early-life exposure to ambient air pollution on childhood asthma is unclear.Objectives: We investigated doctor-diagnosed asthma in preschool children due to exposure to ambient air pollution in utero and during the first year of life.Methods: In total 2490 children aged 3-6 years participated in a questionnaire study regarding doctor-diagnosed asthma between September 2011 and January 2012 in China. Children's exposure to critical air pollutants, sulfur dioxide (SO2) as proxy of industrial air pollution, nitrogen dioxide (NO2) as proxy of traffic pollution, and particulate matter <= 10 mu m in diameter (PM10) as a mixture, was estimated from the concentrations measured at the ambient air quality monitoring stations by using an inverse distance weighted (IDW) method. Logistic regression analysis was employed to determine the relationship between early-life exposure and childhood asthma in terms of odds ratio (OR) and 95% confidence interval (CI).Results: Association between early-life exposure to air pollutants and childhood asthma was observed. SO2 and NO2 had significant associations with adjusted OR (95% CI) of 1.45 (1.02-2.07) and 1.74 (1.15-2.62) in utero and 1.62 (1.01-2.60) and 1.90 (1.20-3.00) during the first year for per 50 mu g/m(3) and 15 mu g/m(3) increase respectively. Exposure to the combined high level of SO2 and NO2 in China significantly elevated the asthmatic risk with adjusted OR (95% CI) of 1.76 (1.18-2.64) in utero and 1.85 (1.22-2.79) during the first year compared to the low level exposure. The associations were higher for males and the younger children aged 3-4 than females and the older children aged 5-6.Conclusions: Early-life exposure to ambient air pollution is associated with childhood asthma during which the level and source of air pollution play important roles. The high level and nature of combined industrial and traffic air pollution in China may contribute to the recent rapid increase of childhood asthma. (C) 2015 Elsevier Inc. All rights reserved.

Abstract:Background The prevalence of atopic dermatitis (AD) in children has increased substantially in China over past decades. The ongoing rise in the prevalence stresses the important role of the environmental factors in the pathogenesis of AD. However, studies evaluating the effects of air pollution on AD in children are scarce. Objective To quantitatively assess the association between air pollution and outpatient visits for AD in children. Methods In this time-series study, we collected 214,747 children of AD from January 1, 2015 to December 31, 2019 through the electronic data base in the Children's Hospital of Chongqing Medical University. The number of daily visits was treated as the dependent variable, and generalized additive models with a Poisson like distribution were constructed, controlling for relevant potential confounders and performing subgroup analyses. Results Each 10 μg/m 3 increase in PM 2.5 , PM 10 , SO 2 , NO 2 and each 1 mg/m 3 increase in CO concentrations was significantly associated with a 0.7% (95% CI: 0.2, 1.3%), 0.9% (95% CI: 0.5, 1.4%), 11% (95% CI: 7.5, 14.7%), 5.5% (95% CI: 4.3, 6.7%) and 10.1% (95% CI: 2.7, 18.2%) increase of AD outpatient visits on the current day, respectively. The lag effect was found in SO 2 , PM 10 , and NO 2. The effects were stronger in cool season and age 0–3 group. Conclusions Our study suggests that short-term exposure to ambient air pollution contributes to more childhood AD outpatient visits in Chongqing, China.

Abstract:There is limited evidence regarding the relationship between air pollution and pediatric asthma in developing countries. This study aimed to investigate the association between short-term exposure to ambient air pollutants and pediatric asthma emergency department (ED) visits in Shanghai, China. We collected data on six criteria air pollutants (PM2.5, PM10, NO2, SO2, CO, and O3) and daily ED visits for pediatric asthma patients from 66 hospitals in Shanghai from 2016 to 2018. The generalized additive model combined with polynomial distributed lag model was applied to explore the associations. We fitted two-pollutant models and stratified the analyses by sex, age, and season. In total, we identified 108,817 emergency department visits for pediatric asthma. A 10 μg/m3 increase in the concentrations of PM2.5, NO2, SO2, and O3 was significantly associated with increased risks of pediatric asthma ED visits, with relative risk of pediatric asthma of 1.011 [95% confidence interval (CI): 1.002, 1.021], 1.030 (95%CI: 1.017, 1.043), 1.106 (95%CI: 1.041, 1.174), and 1.009 (95%CI: 1.001, 1.017), respectively. The associations of NO2 remained robust in the two-pollutant models. There were stronger associations for older children (6–18 years) and in warm seasons. The concentration-response curves for pediatric asthma and PM2.5, NO2, SO2, and O3 were steeper at lower and moderate concentrations but became flatter at higher concentrations. This analysis provided evidence that short-term exposure to air pollutants (PM2.5, NO2, SO2, and O3) could increase the risk of asthma exacerbations among children, and health benefits would be gained from improved air quality.

Abstract:Background: Children are vulnerable to the respiratory effects of air pollution, and their lung function has been associated with long-term exposure to low air pollution level in developed countries. However, the impact of contemporary air pollution level in developing countries as a result of recent efforts to improve air quality on children's lung function is less understood. Methods: We obtained a cross-sectional sample of 617 schoolchildren living in three differently polluted areas in Anhui province, China. 2-year average concentrations of air pollutants at the year of spirometry and the previous year (2017–2018) obtained from district-level air monitoring stations were used to characterize long-term exposure. Forced vital capacity (FVC), forced expiratory volume in 1 second (FEV 1 ), and forced expiratory flow between 25 and 75% of FVC (FEF 25−75 ) were determined under strict quality control. Multivariable regression was employed to evaluate the associations between air pollution level and lung function parameters, overall and by demographic characteristics, lifestyle, and vitamin D that was determined by liquid chromatography tandem mass spectrometry. Results: Mean concentration of fine particulate matter was 44.7 μg/m 3 , which is slightly above the interim target 1 standard of the World Health Organization. After adjusting for confounders, FVC, FEV 1 , and FEF 25−75 showed inverse trends with increasing air pollution levels, with children in high exposure group exhibiting 87.9 [95% confidence interval (CI): 9.5, 166.4] mL decrement in FEV 1 and 195.3 (95% CI: 30.5, 360.1) mL/s decrement in FEF 25−75 compared with those in low exposure group. Additionally, the above negative associations were more pronounced among those who were younger, girls, not exposed to secondhand smoke, non-overweight, physically inactive, or vitamin D deficient. Conclusions: Our study suggests that long-term exposure to relatively high air pollution was associated with impaired lung function in children. More stringent pollution control measures and intervention strategies accounting for effect modification are needed for vulnerable populations in China and other developing countries.

The Association Between High Ambient Air Pollution Exposure and Respiratory Health of Young Children: A Cross Sectional Study in Jinan, China

Abstract:There is growing concern about health effects of high air pollution in less-developed countries. Children represent a population at increased risk for air pollution-related respiratory conditions. This study investigated the relationship between high ambient air pollution exposure and respiratory health of young schoolchildren. From 2014 to 2016 in Jinan, China, 2532 primary school children in grades three to five from two different schools with different levels of air pollution were included in the study. Levels of ambient air pollution exposure including PM10, PM2.5, NO2, SO2, CO and O-3 were measured continuously at the two schools. A questionnaire about children's respiratory health was conducted every year. Among them, about 150 randomly selected children also performed lung function tests two times a year at the beginning of November and middle of December. Annual average exposure levels of PM2.5 (66.8-79.1 vs 90.0-107.7 mu g/m(3)), PM10 (129.5-177.3 vs 198.1-218.6 mu g/ m(3)), NO2 (45.3-53.2 vs 45.0-56.2 mu g/m(3)), SO2 (29.8-56.5 vs 40.5-80.3 mu g/m(3)), CO (1.3-1.5 vs 1.4-1.7 mg/m(3)) and O-3 (84.8-120.2 vs 61.1-128.1 mu g/m(3)) in the heavy pollution primary school were significantly higher than the light one. The higher air pollution exposure was related to increased prevalence of respiratory diseases of young children in the last year, especially allergic rhinitis. The increased odds of lung function impairment associated with exposure to higher air pollution, could be up to 171.5% (aOR = 2.715; 95% CI = 1.915-3.849) for PEF < 75% predicted in 2014. However, after short-termexposure for 1.5 month or a week, paired comparison for parameters of the same child showed different results. The association between high ambient air pollution exposure and respiratory health of young children is closely related to exposure time and dose and may be fluctuate and complex. Abstract:Abstract Background The prevalence of allergic respiratory disease (ARD) is increasing worldwide during the last few decades, causing a great disease burden especially for children. Air pollution has been increasingly considered as a potential contributor to this trend, but its role in ARD induced by house dust mite (HDM-ARD) remains unclear, especially in time-series study. Methods A positive reporting of respiratory allergy to named allergens was included by serum specific IgE testing. A time series Quasi-Poisson regression with distributed lag non-linear model, combined with generalized linear model was used to examine the effects of air pollutants on ARD, HDM-ARD and ARD induced by non-house dust mite (NHDM-ARD). Results A total of 16,249 cases of ARD, including 8,719 HDM-ARD and 8,070 NHDM-ARD from 1 Jan 2013 to 31 Dec 2017 were involved in this study. Air pollutants were significantly associated with clinical visits for childhood ARD and HDM-ARD. Exposure to higher O 3 and interquartile range (IQR) increment in O 3 (40.6 μg/m 3 ) increased the risks of clinical visits for childhood HDM-ARD (RR lag0-5 for the 95th percentile of O 3 : 1.26, 95% confidence interval (CI): 1.03, 1.55; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.09, 95% CI: 1.01, 1.17) and ARD (RR lag0-5 for the 95th percentile of O 3 : 1.19, 95% CI: 1.03, 1.38; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.06, 95% CI: 1.01, 1.12). In addition, higher O 3 was associated with increased RR of boys with ARD (RR lag0-5 for the 95th percentile: 1.26, 95% CI: 1.05, 1.51; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.09, 95% CI: 1.02, 1.16) and HDM-ARD (RR lag0-5 for the 95th percentile: 1.36, 95% CI: 1.06, 1.75; RR lag0-5 for IQR increment (40.6 μg/m 3 ): 1.11, 95% CI: 1.02, 1.22), but not in girls. Conclusions Exposure to O 3 appeared to be a trigger of clinical visits for childhood ARD, especially for HDM-ARD and boys. These findings provide novel evidence on the impact of air pollution on HDM-ARD, which may have significant implications for designing effective intervention programs to control and prevent childhood ARD, especially HDM-ARD, in China and other similar developing countries.

Abstract:Abstract Background In recent five years, air quality in China has been gradually improved; however, the prevalence of asthma in China has still increased significantly. This study aims to examine whether air pollution exposure is a significant contributor to childhood asthma and to explore the mechanisms using exposure and response biomarkers. Methods The generalized additive model was applied to evaluate the effects of air pollutants on asthma hospitalization. Urinary 1-hydroxypyrene (1-OHPyr) was determined as PAHs exposure biomarker. Metabolic profiles and microbial diversity were screened and selected as response biomarkers. Results Daily average concentrations of the six common air pollutants were not significantly associated with daily childhood asthma hospitalization. Urinary 1-OHPyr level showed large variability among asthmatic children and were positively correlated with the abundance of Prevotella (genus level) and purine metabolism. Conclusions The biomarker of PAHs exposure is associated with metabolic and microbiological biomarkers of childhood asthma. As the potential health risk, we suggest that PAHs should be included in the national monitoring system aiming to reduce the exposure.
A study on the short-term effect of particulate matter pollution on hospital visits for asthma in children in Shanghai, China

Abstract:Recently, particulate matter pollution has been worsening, which has been affecting the asthma visits in children. In this study, we assessed the short-term effects of PM10 and PM2.5 on asthma visits in children in Shanghai, China from January 1, 2009 and December 31, 2010, using a generalized additive model. We controlled the confounding factors, such as long-term trends, week day effect, and weather elements. The lag effects of different age subgroups (≤ 2 yr, 3–5 yr, and 6–18 yr subgroups) were performed. The results showed significant effects of PM10 and PM2.5 on asthma visits in children, though the seasonal lags varied for the three age subgroups. In general, the effect of PM2.5 on asthma visits in children was stronger and more acute than that of PM10. PM2.5 showed the highest relative risk of 1.192 at lag 0 day in summer; and PM10 showed the highest relative risk of 1.073 at lag 3 day in autumn. Overall, particulate matter pollution showed a greater effect on relatively younger children. In particular, the ≤ 2 yr subgroup showed the highest seasonal relative risk of PM10. Especially, seasonal relative risk of PM10 in autumn for the ≤ 2 yr subgroup was much higher than that for the other two subgroups. The 3–5 yr and 6–18 yr subgroups showed the highest seasonal relative risk of PM2.5 in summer and winter, respectively. But the pediatric visits data we obtained cannot reflect the true prevalence of asthma and multiple visits. Thus, selection bias may exist in our analysis.

Abstract:Background: Allergic rhinitis (AR) is a common chronic inflammatory disease with bothersome symptoms. However, the effect of air pollution on the prevalence of AR in children is controversial. Objective: This study aimed to investigate the association between air pollution and the prevalence of AR in Chinese children. Methods: This study, in China, included 160,356 students ages 0-18 years who completed a questionnaire about the accuracy of the International Study of Asthma and Allergies in Childhood (ISAAC). The effect of different air pollutants on the prevalence rate were evaluated by meta-analysis. Also, it evaluated the effect of different air pollutants on the prevalence rate. Results: The differences in the effects of sulfur dioxide (SO₂) exposure (combined odds ratio [ORcombined] 1.03 [95% confidence interval {CI}, 1.01-1.05]; p = 0.010) and nitrogen dioxide (NO₂) exposure (ORcombined 1.11 [95% CI, 1.05-1.18]; p = 0.0006) on the risk of childhood AR was statistically significant. The effect of particulate matter with aerodynamic diameter of <10 μm (PM10) exposure on the risk of childhood AR was statistically significant (ORcombined 1.02 [95% CI, 1.01-1.03]; p < 0.001), the effect of particulate matter with aerodynamic diameter of <2.5 μm (PM2.5) exposure on the risk of childhood AR was statistically significant (ORcombined 1.15 [95% CI, 1.03-1.29]; p = 0.02), and the effect of ozone exposure on the risk of childhood AR was not statistically significant (ORcombined 0.98 [95% CI, 0.67-1.41]; p = 0.13). Conclusion: NO₂, SO₂, PM2.5, and PM10 were associated with the prevalence of AR in Chinese children. PM2.5 had the highest correlation with AR prevalence.
Interaction effect of prenatal and postnatal exposure to ambient air pollution and temperature on childhood asthma

Abstract:Background Although mounting evidence has associated air pollution and environmental temperature with children's health problems, it is unclear whether there is an interaction between these factors on childhood asthma. Objectives To explore the effects of temperature-pollution interactions during pre- and post-natal periods on asthma among pre-schoolers. Methods A retrospective cohort study of 39,782 pre-schoolers was performed during 2010-2012, in seven cities in China. Exposure to three temperature indicators (TI) and three critical ambient air pollutants, including particulate matter with aerodynamic diameter ≤ 10 μm (PM 10 ), sulfur dioxide (SO 2 ) and nitrogen dioxide (NO 2 ) as proxies of industrial and vehicular air pollution, was estimated by an inverse distance weighted (IDW) method. Two-level logistical regression analysis was used to examine the association between both pre- and post-natal exposure and childhood asthma in terms of odds ratio (OR) and 95% confidence interval (CI). Results Asthma prevalence in pre-schoolers at age of 3-6 years (6.9%) was significantly associated with traffic-related air pollutant (NO 2 ) exposure, with ORs (95% CI) of 1.17 (1.06, 1.28), 1.19 (1.05-1.34) and 1.16 (1.03-1.31) for an IQR increase in NO 2 exposure during lifetime, pregnancy, and entire postnatal period respectively. Furthermore, childhood asthma was positively associated with exposure to increased temperature during lifetime, pregnancy, and entire postnatal period with ORs (95% CI) = 1.89 (1.66, 2.16), 1.47 (1.34, 1.61), and 1.15 (1.11, 1.18) respectively, while was negatively associated with decreased temperatures. Childhood asthma was positively related with exposure to extreme heat days (EHD) during postnatal period particularly in first year of life respectively with ORs (95% CI) = 1.23 (1.04, 1.46) and 1.26 (1.07, 1.47), but was not related with extreme cold days (ECD) exposure. A combination of high air pollutant levels and high temperatures significantly increased the risk of asthma during both pre- and post-natal periods. Strikingly, we found a significantly positive interaction of temperature and PM 10 or SO 2 on asthma risk among boys and younger children. Conclusions Prenatal and postnatal exposure to ambient air pollution and high temperatures are independently and jointly associated with asthma risk in early childhood.

Abstract:Background/Aim: Few studies examined the effects of specific PM2.5 constituents on childhood asthma and allergy in China. We aimed to investigate the effects of various PM2.5 constituents on asthmatic and allergic symptoms in preschool children in China.Methods: A cross-sectional study was conducted among 48219 preschool children in six cities (Shanghai, Nanjing, Chongqing, Changsha, Urumqi and Taiyuan) based on China , Children ,Home and Health (CCHH) project in China. Information on allergic symptoms as well as demography were collected by questionnaires. Data for ambient PM2.5 concentration and its 5 constituents: BC, NH4+, SO42-, NO3- and OC were obtained from the global burden disease project to 205 kindergartens at a spatial resolution of 0.01×0.01(1km×1km) in six cities. A hierarchical logistic regression model was performed to explore the associations between kindergarten-level PM2.5 components and individual-level outcomes of asthmatic and allergic symptoms controlling for demography and other covariates.Results: A total of 30759 participants (51.7% male) were finally completed the study due on the missing data. Both the single-pollutant and two-pollutants model showed that ambient PM2.5 per 10ug/m3 increase positively associated with the prevalence of diagnosed asthma by an adjusted odds ratio (AOR) and 95% confidence interval(CI) of 1.18(1.01-1.35) and 1.13(1.02-1.24), respectively. Furthermore, the concentration of SO42- and NO3- were significantly associated with the prevalence of asthma and current rhinitis by the AOR for each 1ug/m3 increase in SO42-, and NO3- concentrations of 1.09(1.03-1.15) for asthma and 1.04(1.00-1.07) for current rhinitis, respectively.Conclusion: Components of PM2.5, especially NO3- and SO42- exposure were associated with asthmatic and allergic diseases in preschool children in China. Considering the related sources and combustion, it may represents an preventable risk factor for childhood asthma and allergic diseases.

Abstract:BACKGROUND: Allergic rhinitis (AR) is one of the most common allergic diseases in the world, and usually persists throughout the activity. Epidemiological studies have shown a positive association between air pollution and allergic rhinitis. However, we could not find any meta-analysis of the risk of air pollutants (PM2.5, PM10, NO2, SO2, O3 and CO) on the prevalence of AR in people of all ages.OBJECTIVES: Carry out a meta-analysis on the results of recent studies (up to 2020) to present valid information about exposure to air pollution and risk of prevalence of AR.METHODS: We systematically searched three databases for studies up to December 17, 2020, including air pollution and AR. Random effect models were conducted to estimate the pooled odds ratios (ORs) and 95% confidence intervals (95% CIs). Subgroup analysis, funnel plot, Egger's test, and the trim-and-fill method were also conducted.RESULTS: Thirty-five studies across 12 countries, including a total of 453,470 participants, were included. The OR per 10 μg/m3 increase of pollutants was 1.13 (1.04-1.22) for PM10 and 1.12 (1.05-1.20) for PM2.5. The OR per 10 μg/m3 increment of gaseous pollutants were 1.13 (1.07-1.20) for NO2, 1.13 (1.04-1.22) for SO2 and 1.07 (1.01-1.12) for O3. No significant association was observed between CO and AR. Children or adolescents are more sensitive to air pollution than adults. The effects of PM10 and SO2 were significantly stronger in Europe than Asia. The effects of air pollutants were more significant and higher in developing countries than in developed countries, except for PM10. A significant difference of subgroup test was found between developed and developing countries of NO2.CONCLUSION: This meta-analysis showed a positive association between air pollution and the prevalence of allergic rhinitis, and identified geographic area and economic level as the potential modifiers for the association.

Abstract:This systematic review aims to identify the association between prenatal exposure to air pollutants and allergic diseases in children, focusing on specific pollutants, timing of exposure, and associated diseases. We searched PubMed, Scopus, and Web of Science for English articles until May 1, 2023, examining maternal exposure to outdoor air pollutants (PM 1 , PM 2.5 , PM 10 , NO, NO 2 , SO 2 , CO, and O 3 ) during pregnancy and child allergic diseases (atopic dermatitis (AD), food allergy (FA), asthma (AT) and allergic rhinitis (AR)/hay fever (HF)). The final 38 eligible studies were included in the meta-analysis. Exposure to PM 2.5 and NO 2 during pregnancy was associated with the risk of childhood AD, with pooled ORs of 1.34 (95% confidence interval (CI) , 1.10–1.63) and 1.10 (95% CI , 1.05–1.15) per 10 μg/m 3 increase, respectively. Maternal exposure to PM 1 , PM 2.5 , and NO 2 with a 10 μg/m 3 increase posed a risk for AT, with pooled ORs of 1.34 (95% CI , 1.17–1.54), 1.11 (95% CI , 1.05–1.18), and 1.07 (95% CI , 1.02–1.12), respectively. An increased risk of HF was observed for PM 2.5 and NO 2 with a 10 μg/m 3 increase, with ORs of 1.36 (95% CI , 1.17–1.58) and 1.26 (95% CI , 1.08–1.48), respectively. Traffic-related air pollutants (TRAP), particularly PM 2.5 and NO 2 , throughout pregnancy, pose a pervasive risk for childhood allergies. Different pollutants may induce diverse allergic diseases in children across varying perinatal periods. AT is more likely to be induced by outdoor air pollutants as a health outcome. More research is needed to explore links between air pollution and airway-derived food allergies.

Abstract:Obesity has become a worldwide epidemic; 340 million of children and adolescents were overweight or obese in 2016, and this number continues to grow at a rapid rate. Epidemiological research has suggested that air pollution affects childhood obesity and weight status, but the current evidence remains inconsistent. Therefore, the aim of this meta-analysis was to estimate the effects of childhood exposure to air pollutants on weight. A total of four databases (PubMed, Web of Science, Embase, and Cochrane Library) were searched for publications up to December 31, 2021, and finally 15 studies met the inclusion criteria for meta-analysis. Merged odds ratios (ORs), coefficients (β), and 95% confidence intervals (95% CIs) that were related to air pollutants were estimated using a random-effects model. The meta-analysis indicated that air pollutants were correlated with childhood obesity and weight gain. For obesity, the association was considerable for PM10 (OR = 1.12, 95% CI: 1.06, 1.18), PM2.5 (OR = 1.28, 95% CI: 1.13, 1.45), PM1 (OR = 1.41, 95% CI: 1.30, 1.53), and NO2 (OR = 1.11, 95% CI: 1.06, 1.18). Similarly, BMI status increased by 0.08 (0.03–0.12), 0.11 (0.05–0.17), and 0.03 (0.01–0.04) kg/m2 with 10 μg/m3 increment in exposure to PM10, PM2.5, and NO2. In summary, air pollution can be regarded as a probable risk factor for the weight status of children and adolescents. The next step is to conduct longer-term and large-scale studies on different population subgroups, exposure concentrations, and pollutant combinations to provide detailed evidence. Meanwhile, integrated management of air pollution is essential.

Abstract:Introduction: Air pollution is widely acknowledged as a significant factor in respiratory outcomes, including coughing, wheezing, emergency department (ED) visits, and even death. Although several literature reviews have confirmed the association between air pollution and respiratory outcomes, they often did not standardize associations across different studies and overlooked other increasingly impactful pollutants such as trace metals. Recognizing the importance of consistent comparison and emissions of non-exhaust particles from road traffic, this study aims to comprehensively evaluate the standardized effects of various criteria pollutants and trace metals on respiratory health. Methods: We conducted a comprehensive meta-analysis of peer-reviewed journal articles on air pollution and respiratory outcomes published between 1 January 2000, and 1 June 2024. The study included children (age < 18 years), adults (age ≥ 18 years), and all age groups exposed to criteria pollutants established by the US Environmental Protection Agency National Ambient Air Quality Standards and over 10 trace metals. Using databases, such as PubMed, MEDLINE, Web of Science Core Collection, and Google Scholar, we identified 579 relevant articles. After rigorous screening and quality assessment using the Newcastle-Ottawa Scale, 50 high-quality studies were included. We converted various reported outcomes (e.g., odds ratios, relative risk, and percent increase) to a standardized odds ratio (OR) for comparability and performed meta-analyses using R 4.4.0 and related packages, ensuring the robustness of our findings. Results: Our meta-analysis indicated significant associations between air pollutants and respiratory outcomes. For particulate matter with diameter ≤ 2.5 μm (PM 2.5 ), the overall ORs for children, adults, and combined age groups were 1.31, 1.10, and 1.26, respectively, indicating a consistent positive association. Similar positive associations were observed for particulate matter with diameter ≤ 10 μm (PM 10 ) and other pollutants, with children showing higher susceptibility than adults. The analysis of trace metals also showed significant associations; however, these findings require cautious interpretation due to the small number of studies. Conclusion: Our study supports associations between air pollutants, including non-exhaust trace metals, and respiratory outcomes across different age groups. The findings underscore the need for stringent environmental health policies and further research, especially in regions with higher pollution levels. The future studies should consider long-term and short-term exposures separately and include diverse populations to improve the accuracy and generalizability of the results.

Abstract:Recent epidemiological studies pointed out that air pollution has a significant impact on pediatric asthma. Shanghai is one of the biggest cities in China, and the short-term effect of atmospheric particulate matter on the incidence of pediatric asthma has become a hot topic. From January 1, 2009, to December 31, 2010, we used daily measurements of pollutant concentrations, daily weather data, and daily records of pediatric asthma hospital visits from local authorities to evaluate the short-term effect of air pollution on pediatric asthma incidence in Shanghai, China. We used a generalized additive model (GAM) in the analysis, and the controlled confounding factors include long-term trends, day-of-the-week effects, and weather elements. We divided the entire study group into different age-subgroups. In addition, we took a variety of lag models into consideration. The results showed a strong connection between concentrations of fine particulate matter (PM2.5) and pediatric asthma hospital visits from 2009 to 2010 in Shanghai, China. For the entire study group, the greatest relative risk (RR) of PM2.5 on pediatric asthma hospital visits was 1.060 on a lag of 4 days. As for the three different age-subgroups, the greatest RR of PM2.5 on pediatric asthma hospital visits was 1.061 (at a lag of 5 days), 1.071 (at a lag of 4 days), and 1.052 (at a lag of 2 days), for the under-2-year-olds, 3-to-5-year-olds, and the 6-to-18-year-olds, respectively. The overall short-term effect of PM2.5 on pediatric asthma hospital visits was relatively stronger in younger children. Within the year, we detected the strongest seasonal effect of PM2.5 on pediatric asthma hospital visits in Summer. When adding other air pollutants in the analysis model, RR of PM2.5 on pediatric asthma hospital visits would be increased.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Aims</h3>The association of long-term ambient air pollution and hypertension has been inconsistently reported. We performed an updated systematic review and meta-analysis to assess the association between long-term exposure to ambient air pollution and risk of hypertension in adults and in children.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3>PubMed, EMBASE, and Web of Science were searched up to August 7, 2020 for published articles examining the association of long-term exposure to ambient air pollution, including particulate matter (PM; ultrafine particles, PM1, PM1–2.5, PM2.5, PM2.5–10 and PM10), nitrogen dioxide (NO2), nitrogen oxides (NOx), sulfur dioxide (SO2), ozone (O3), carbon monoxide (CO) and hypertension. Pooled odds ratios (ORs) and 95% confidence intervals (CIs) for hypertension with each 10-μg/m3 increase in air pollutants were calculated by random-effects models.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3>We included 57 studies (53 of adults and 4 of children) in the meta-analysis. Risk of hypertension was significantly increased in adults with each 10-μg/m3 increase in exposure to PM2.5 (OR 1.10, 95% CI 1.07–1.14; I2 = 93.1%; n = 37), PM10 (1.04, 1.02–1.07; I2 = 44.8%; n = 22), and SO2 (1.21, 1.08–1.36; I2 = 96.6%; n = 3). Hypertension was not significantly associated with PM1 (n = 2), PM2.5–10 (n = 16), NO2 (n = 27), or NOx (n = 17). In children, the summary ORs (95% CIs) for each 10-μg/m3 increase in PM2.5, PM10, SO2 and O3 were 2.82 (0.51–15.68; I2 = 83.8%; n = 2), 1.15 (1.01–1.32; I2 = 0; n = 2), 8.57 (0.13–575.58; I2 = 94.2%; n = 2), and 1.26 (0.81–1.09, I2 = 91.6%; n = 2), respectively.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3>Long-term ambient air pollution is a potential risk factor for hypertension in adults. More studies are needed to explore the effects of long-term air pollution on hypertension in children.
