Abstract:High serum free fatty acid (FFA) levels are associated with metabolic syndrome (MS). This study aimed to assess the association of fasting serum FFAs with nonalcoholic fatty liver disease (NAFLD) in a Chinese population. A total of 840 subjects fulfilled the diagnostic criteria of NAFLD and 331 healthy control participants were enrolled in this cross-sectional study. Fasting serum FFA levels and other clinical and laboratory parameters were measured. NAFLD patients had significantly higher serum FFA levels than controls (P < 0.001). Serum FFA levels were significantly and positively correlated with parameters of MS, inflammation indexes and markers of hepatocellular damage. Elevated serum FFA levels were found in NAFLD subjects with individual components of MS (obesity, hypertriglyceridaemia and hyperglycaemia). Stepwise regression showed that serum FFA levels were an independent factor predicting advanced fibrosis (FIB-4 ≥ 1.3) in NAFLD patients. Serum FFA levels correlated with NAFLD and could be used as an indicator for predicting advanced fibrosis in NAFLD patients.

Abstract:Objective To explore the relationship between fatty liver and metabolic syndrome, and to evaluate the value of fatty liver in predicting the clustering risk factors in metabolic syndrome. Methods A cross-sectional survey with multiple-stages and random samplings was performed in 3175 adult residents aged over 16. Questionaire and 75 g oral glucose tolerance test were conducted; plasma glucose, lipid-profile as well as body mass index (BMI), and blood pressure were measured. Diagnosis of fatty liver was based on established real-time ultrasonographic criteria. The diagnosis of metabolic syndrome and its components followed the United States National Cholesterol Education Program-Adult Treatment PanelⅢ (NCEP-ATPⅢ) criteria with the exception of waist circumference (WC) and BMI. Results The prevalences of metabolic syndrome and fatty liver were 22.9% (726/3175) and 20.8%(661/3175) respectively. Compared with obesity (BMI≥25 kg/m~2) and abdominal obesity (WC≥90 cm for males and 80 cm for females), fatty liver had the highest percentage in clustering risk factor and attributable risk percentage with high specificity and positive predictive value both in males and females. Compared with the normalcontrols,theriskforfatty liver was increased by 32.8-fold (95%CI 14.9-72.4) in abdominal obesity participants, 31.6-fold (14.2-70.4) in diabetes mellitus, 22.6-fold (10.3-50.0) in dyslipidemia, 23.3-fold (10.5-51.3) in hypertension, and 39.3-fold (17.8-87.1) in patients with metabolic syndrome. After 661 patients with fatty liver were stratified by BMI, the prevalence of central obesity increased with BMI, however, the prevalence of hypertriglyceridemia, impaired fasting glucose, low HDL-C and hypertension showed no significant increase with BMI. Moreover, the prevalences of one and five risk factors in patients with fatty liver and normal BMI were 86.1% and 2.8%, respectively. Conclusion There is ahigh prevalence of the metabolic syndrome and fatty liver in Shanghaiadults. Metabolic syndrome isclosely related with fatty liver.Fatty liver could be used to predict clustering risk factors in metabolic disorders.

Abstract:Background and Aim: To explore the relationship between fatty liver and the metabolic syndrome in the adults of Shanghai and evaluate the value of fatty liver as a marker for risk factor clustering. Methods: Questionnaires, physical examinations, laboratory tests (blood lipid and glucose) and real-time liver ultrasonographies were performed in Shanghai adults and analyzed using randomized, multistage, stratified cluster sampling. Prevalence of the metabolic syndrome was defined by the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATPIII) criteria with the exception of abdominal obesity (waist circumference > 90 cm in men and > 80 cm in women); fatty liver was diagnosed in accordance with the presence of an ultrasonographic pattern consistent with 'bright' liver (brightness and posterior attenuation of liver). Results: The study population consisted of 3175 subjects (1218 men) with a mean (+/- SD) age of 52.4 +/- 15.1 years. Metabolic syndrome and fatty liver were found in 726 (22.87%) and 661 (20.82%) of sampled cases, respectively. After adjustment by age and sex, the prevalence of the metabolic syndrome and fatty liver in the general population of Shanghai were 15.30 and 17.29%, respectively. The risk for fatty liver in subjects with abdominal obesity, diabetes, dyslipidemia and hypertension increased 32.78-fold (95% confidence interval (CI) 14.85-72.35), 31.58-fold (95% CI 14.18-70.35), 22.64-fold (95% CI 10.26-49.99) and 23.25-fold (95% CI 10.54-51.30), respectively, compared with controls, whereas the risk for fatty liver in subjects with metabolic syndrome was increased by 39.33-fold (95% CI 17.77-87.05). After the 661 patients with fatty liver had been stratified by body mass index (BMI), the prevalence of abdominal obesity, hypertension and the metabolic syndrome were increased from 25.0, 47.2 and 36.1%, respectively, in people with normal BMI to 81.0, 73.8 and 55.4%, respectively, in obese persons. However, the prevalence of hypertriglyceridemia, high fasting glucose and low high-density lipoprotein-cholesterol showed no significant changes with increased BMI. Moreover, among fatty liver patients with normal BMI, the detection rate for one or more features of metabolic disorders was as high as 83.3% and that for five features was 2.8%. Compared with obesity (BMI >= 25 kg/m(2)) and abdominal obesity, fatty liver had the highest clustering rate, specificity, positive predictive value and attributable risk percentage in detecting risk factor clustering in both sexes. Conclusions: There is a high prevalence of metabolic syndrome and fatty liver among Shanghai adults. Metabolic disorders are closely related to fatty liver; moreover, fatty liver appears to be a good predictor for the clustering of risk factors for metabolic syndrome. Abstract:OBJECTIVE:The aim of this study was to investigate the prevalence of nonalcoholic fatty liver disease (NAFLD) and metabolic syndrome (MS) in subjects who underwent a routine health checkup. We intended to establish a clinical association between NAFLD and MS as well as to compare the diagnostic criteria of MS based on the definitions set forth by the International Diabetes Federation (IDF), the US National Cholesterol Education Program Adult Treatment Panel III (2001) (NCEP/ATP-III) and the Metabolic Syndrome Study Group of Chinese Diabetes Society (CDS).METHODS:Weight, height, waist circumference, hip circumference, percentage of body fat, blood pressure and ultrasound of liver were performed on subjects undergoing routine health checkup. Serum triglyceride, total cholesterol, high density lipoprotein cholesterol and fasting plasma glucose level were measured.RESULTS:A total of 2394 subjects were included in this analysis and 437 had NAFLD. The prevalence of MS in the whole sample according to IDF, NCEP/ATP-III and CDS definitions was 11.11%, 8.48% and 5.30%, respectively. The total degree of agreement between IDF, NCEP/ATP-III and CDS definition was 87.76%. The prevalence of MS in NAFLD subjects is much higher than that in non-NAFLD subjects. The prevalence of NAFLD in MS subjects is also much higher than that in non-MS subjects.CONCLUSION:The prevalence of MS varied depending on the diagnostic criteria used. NAFLD was strongly associated with the MS, although it remains unknown whether NAFLD is a cause or effect of MS.
The Prevalence and Risk Factors on Different Degrees of Non-Alcoholic Fatty Liver Disease in Shanghai

Abstract:Abstract [Background and aims]NAFLD is a common chronic liver disease. The purpose of this study is to carry out a current large-scale epidemiological survey and analyse factors related to the incidence and the severity of NAFLD.[Methods]Three communities of Shanghai were selected by stratified random sampling in 2019.The residents older than 18 years old underwent medical history taking, physical examination, laboratory examination and liver ultrasonography examination. The prevalence of different degrees of NAFLD, the prevalence of metabolic factors, and factors associated with the severity of NAFLD was analysed. [Results]A total of 19250 subjects were enrolled. The total prevalence of NAFLD in Shanghai was 45.55%, 62.00% in males and 27.54% in females. Age, FLI, BMI, WC, ALT, AST, AKP, GGT, CR, FPG, UA, TC, TG and LDL in NAFLD patients were higher than those in non-NAFLD people. The distribution of NAFLD prevalence was different in different age groups. The prevalence of diabetes, obesity and hypertriglyceridemia in the NAFLD group were higher than that in the non-NAFLD group. In males, FLI, weight, WC, ALT, UA, TG, GGT and metabolic syndrome were positively correlated with the severity of NAFLD; while HDL was negatively correlated with it. In females, age, FLI, weight, WC, ALT, FPG, UA, TG, LDL, GGT, AKP and metabolic syndrome were positively correlated with the severity of NAFLD; while HDL was negatively correlated with it.[Conclusions]The prevalence of NAFLD in Shanghai is high. The distribution of NAFLD prevalence was different in different age groups. Many factors are related to the incidence of NAFLD and the severity of NAFLD.

Abstract:Objective To investigate the prevalence and major risk factors of fatty liver disease in elderly population of Shanghai community.Methods A total of 7 534 residents aged 60 and above who came from Meilong community of Minhang District were enrolled to participate in the survey.They underwent regular physical examination,laboratory tests and ultrasonographic examination.Results Among the 7 534 residents 4 198 were females and 3 336 were males with a F/M ratio of 1.26.The mean age of the participants was 68.8±7.0 years.Fatty liver was detected with ultrasound examination in 3 074 participants(40.80%).The prevalence of fatty liver in Shanghai elderly declines with the increment of age.Ages ranging from 60 to 64 have the highest prevalence of 45.20%.The prevalence of fatty liver in males was significantly higher than that in males younger than 80 years.The mean body mass index(BMI),waist circumference,waist hip circumference ratio(WHR),triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),systolic blood pressure(SBP),alanine aminotransferase(ALT) and γ-glutamyl transferase(GGT) in the fatty liver group were significantly higher than those in the group without fatty liver,but the high-density lipoprotein cholesterol(HDL-C) level was lower in the fatty liver group.Obesity,central-obesity and hyperlipidemia increased the risk for fatty liver by 15.2-fold,10.8-fold and 8.6-fold,respectively(P<0.001),Logistic regression analysis demonstrated that the prevalence of fatty liver was positively correlated to nine risk factors,including female sex,BMI,SBP,HDL-C,LDL-C,TG,TC,ALT and GGT.Conclusion There is a high prevalence of fatty liver among community elderly in Shanghai,to which overweight and hyperlipidemia are closely relevant.

Abstract:Backgrounds Nonalcoholic fatty liver disease (NAFLD) has become the most common chronic liver disease in Asians. However, data on prevalence and factors associated with NAFLD in Asians are lacking. The aim of this study is to investigate the prevalence of NAFLD in Shanghai employees to assess the relationship between NAFLD and age, gender, metabolic risk factors in this studied population. Methods We selected 7152 employees of Shanghai work-units. Each of them underwent detailed medical history-taking, physical examination, laboratory assessments and abdominal ultrasonography. The diagnosis of NAFLD was done according to established criteria. Receiver operating characteristics (ROC) curves were applied to detect areas under the ROC curves for each index. Nominal logistic regression analysis was used to estimate the odds ratio for risk factors of NAFLD. Results About 38.17% employees had NAFLD, more in men than in women. The prevalence of NAFLD increased with increasing age. In both genders, the prevalence of metabolic factors was higher in the NAFLD group. Body max index, waist circumference, weight-to-height ratio, blood pressure, blood glucose, total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein and uric acid were found to have a diagnostic value for NAFLD. Body max index is a better index for diagnosing NAFLD. Uric acid is a new diagnosing index not inferior to lipid metabolic factors. Metabolic factors can increase the risk of NAFLD up to 1.5 ~ 3.8 times. Conclusions Older age, male gender, metabolic factors such as obesity, abdominal obesity, dyslipidemia, hypertension or type 2 diabetes are risk factors for NAFLD. Prevalence of NAFLD in Shanghai employees is high. Prevention is extremely important. Those achieve the critical point should have early intervention.

Abstract:OBJECTIVE:To investigate the prevalence and major risk factors of fatty liver among adult residents in Shanghai.METHODS:A cross-sectional survey with multiple-stage stratified cluster and random sampling was performed. All residents aged 16 and above were invited to participate in the survey; they came from four communities of Yangpu District and Pudong New District. Questionnaire, physical examination, serum lipid-profile, and 75 gram oral glucose tolerance test and ultrasonographic examination of liver were undertaken. Analysis of data was performed through SPSS 11.0 for Windows statistical package.RESULTS:A total of 3175 residents took part in the survey, which was 75% of adult residents of the investigated communities and 2.26/10 000 of Shanghai municipal residents. Of the 3175, 1218 were males and 1957 were females. The mean age of the participants was 52.4+/-15.1 years and ranged from 16 to 88 years. Fatty liver was detected with ultrasound examination in 661 participants (20.82%), among which 3.48% had alcoholic fatty liver, 4.08% had suspected alcoholic fatty liver, and 92.43% had nonalcoholic fatty liver. The age-adjusted, sex-adjusted prevalence of fatty liver in Shanghai adult residents was 17.29%, the prevalence of alcoholic fatty liver, suspicious alcoholic fatty liver, and nonalcoholic fatty liver in Shanghai adult residents were 0.79%, 1.15%, and 15.35%, respectively. The prevalence of fatty liver was increased with aging in males and in females. Among participants younger than 50 years old, the prevalence of fatty liver in males was significantly higher than that in females, but in participants older than 50 years the case was just the opposite, higher in females. The mean age (years), body mass index (BMI), waist circumference, blood pressure, fasting and two hour serum glucose level, triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and the presence of obesity, diabetes mellitus, hypertension, dyslipidemia, and gallstones in the fatty liver group was significantly higher than those in the group without fatty liver, but the high-density lipoprotein cholesterol (HDL-C) level and the educational level were both lower in the fatty liver group. Logistic regression analysis demonstrated that the prevalence of fatty liver was only positively correlated to nine risk factors, including male sex, educational level, waist circumference, BMI, fasting glucose level, HDL-C, TG, hypertension and diabetes mellitus. In heavy drinkers, obesity increased the risk for fatty liver by 4.8-fold, but heavy drinking only increased the risk for fatty liver 1.5-fold (95% CI 0.9-2.6, P=0.1685).CONCLUSION:There is a high prevalence of fatty liver among adult residents in Shanghai, and nonalcoholic fatty liver is the major type. Metabolic disorders such as obesity and diabetes mellitus, hypertension and hyperlipidemia are more closely associated with fatty liver than heavy drinking in Shanghai.

Abstract:Objective To investigate the prevalence and risk factors of fatty liver and its relationship with the disorder of glucose metabolism in middle-aged and elderly people in Chongming area,Shanghai.Methods We conducted a population-based cross-sectional study in the residents aged 40-70 years from 8 urban communities and 4 rural communities of Chongming area,Shanghai.A questionnaire interview and an oral glucose tolerance test (OGTT) were carried out for each participant and the anthropometric measurements,blood pressure,plasma lipids,liver enzyme profiles were also obtained from each of them.Fatty liver was diagnosed by hepatic ultrasound.Results A total of 10 061 subjects were recruited in this survey.Complete investigation data were collected from 9 913 subjects.There were 3 218 males and 6 695 females with a mean age of (56.0±7.8) years.Totally 4 185 subjects (42.22 ％) were diagnosed as fatty liver by abdominal ultrasound examination.The subjects with fatty liver were older than those without fatty liver.The prevalence of obesity,abdominal obesity,metabolic syndrome,diabetes,prediabetes,isolated impaired glucose tolerance (i-IGT),and a combination of impaired fasting glucose (IFG) and impaired glucose tolerance (IGT) in fatty liver group were significantly higher than those in non-fatty liver group.The systolic blood pressure,diastolic blood pressure,waist circumference,hip circumference,waist hip ratio,body mass index (BMI),insulin resistance index (HOMA-IR),low density lipoprotein cholesterol (LDL-C),triglyceride (TG),total cholesterol (TC),OGTT 2-hour blood glucose,fasting blood glucose (FPG),glycosylated hemoglobin (HbA1c),alanine aminotransferase (ALT),aspartate aminotransferase (AST),and gamma glutamyltransferase (γ-GT) levels in fatty liver group were also significantly higher than those in non-fatty liver group (P＜0.01 or 0.05).But the constituent ratio between subjects with i-IFG and normal glucose tolerance (NGT),HDL-C level and AST/ALT in fatty liver group were significantly lower than those in non-fatty liver group (all P＜0.01).The prevalence of fatty liver in prediabetes subjects was significantly higher than that in NGT population (P＜0.001).The.prevalence of fatty liver in subjects with isolated IFG (i-IFG) was significantly higher than that in NGT subjects (P＜0.001),but significantly lower than that in subjects with i-IGT (P＜0.001).The prevalence of fatty liver in subjects with a combination of IFG and IGT was significantly higher than that in subjects with i-IGT (P＜0.001),but was much lower than that in subjects with diabetes (P＜ 0.001).Logistic regression analysis showed that waist circumference (OR =1.042,95％ CI:1.033-1.051,P＜0.001),BMI (OR =1.222,95 ％ CI:1.190-1.256,P＜0.001),OGTT 2 h glucose (OR =1.106,95 ％ CI:1.084-1.130,P＜0.001),HbA1c (OR=1.230,95％CI:1.125-1.345,P＜0.001),HOMA-IR (OR=1.163,95％CI:1.113-1.215,P＜ 0.001),HDL-C (OR=0.689,95％CI:0.497-0.954,P=0.025),LDL-C (OR=3.236,95％CI:2.542-4.120,P＜0.001) andTG (OR=1.905,95％CI:1.726-2.101,P＜0.001) were independent risk factors of fatty liver diseases after adjustment of potential cofounder;FPG (OR =16.054,95 ％ CI:12.997-19.831,P＜0.00 1),OGTT 2 h glucose (OR =2.744,95 ％ CI:2.573-2.927,P ＜0.001),HbA1c (OR=2.314,95％CI:1.832-2.922,P＜0.001) and HDL-C (OR=2.196,95％CI:1.184-4.073,P =0.013) were independent risk factors of diabetes after adjustment of potential covariate factors.Conclusions Fatty liver is highly prevalent in Chongming area,Shanghai.Fatty liver is associated with diabetes and prediabetes.

Abstract:Objective To investigate the prevalence and risk factors of fatty liver among adult residents in Chongming District,Shanghai.Methods A cross-sectional survey with multiple stage stratified cluster and random sampling was performed.All residents aged from 40 to 70 were invited to participate the survey.Questionnaire,physical examination,serum lipid-profile,75 g oral glucose tolerance test and ultrasonography of liver were undertaken.Analysis of data was performed through SPSS 13.0 for windows statistical package.Results A total of 3 017 residents participated the survey,which was 80.15/10 000 of the residents.Of the 3 017 residents surveyed,882 were males and 2 135 were females.The mean age of the participants was(55.6±8.0) years and ranged from 40-70 years.Fatty liver was detected by ultrasound examination in 1 262 participants(41.83%).The age and gender adjusted prevalence of fatty liver was 21.25%.The standardized prevalence of fatty liver in males(24.53%) was similar to that in females(20.40%)(P>0.05).The prevalence of non-alcoholic fatty liver was 88.75%(1 120/1 262) in fatty liver.Mean age,fasting plasma glucose(FPG),2 h plasma glucose(2hPG),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),and alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyltransferase(GGT) in fatty liver group was significantly higher than that in non-fatty liver group(all P<0.000 1);high density lipoprotein cholesterol(HDL-C) level was lower in fatty liver group than in non-fatty liver group(P<0.000 1).Logistic regression analysis demonstrated that the prevalence of fatty liver was positively correlated with age,postprandial glucose level,TG,LDL-C,SBP,ALT,GGT.Conclusions There was a high prevalence of fatty liver among adult residents in Chongming district,Shanghai,and non-alcoholic fatty liver was the major type.Aging,diabetes mellitus,hypertension and dyslipidemia were the risk factors of fatty liver.

This study aimed to investigate the role of obesity and metabolic abnormalities in NAFLD among elderly Chinese.Methods: A cross-sectional study was performed among elderly residents who took their annual health checkups during 2016 in Keqiao District, Shaoxing, China. Results: A total of 3359 elderly adults were retrospectively included in this study. The overall prevalence of NAFLD was 28.7％. The prevalence of NAFLD were 7.14％, 27.92％, 34.80％, and 61.02％ in participants with metabolically healthy normal weight (MHNW), metabolically abnormal normal weight (MANW), metabol- ically healthy obese (MHO), and metabolically abnormal obese (MAO), respectively. NAFLD patients in MHO group had more unfavorable metabolic profiles than those in MHNW group. Logistic regression analysis showed that sex, body mass index (BMI), fasting blood glucose, and serum uric acid were the risk factors of NAFLD. Conclusions: Both obesity and metabolic health were significantly associated with NAFLD in elderly Chi- nese. Screening for obesity and other metabolic abnormalities should be routinely performed for early risk stratification of NAFLD.
Prevalence of Nonalcoholic Fatty Liver among Administrative Officers in Shanghai: an Epidemiological Survey.

Abstract:AIM:To determine the prevalence of nonalcoholic fatty liver in a specific population in Shanghai by an epidemiological survey, and to analyze risk factors of fatty liver.METHODS:Total 4009 administrative officers who denied regular alcohol drinking participated in the survey, and underwent physical examination and laboratory tests. The important parameters were body mass index (BMI), waist hip circumferences ratio (WHR) and levels of serum lipids. Diagnosis of fatty liver was based on established real-time ultrasonographic criteria, the presence of an ultrasonographic pattern consistent with "bright liver", with evident ultrasonographic contrast between hepatic and renal parenchyma, vessel blurring, and narrowing of the lumen of the hepatic veins. Analysis of data was performed through SPSS for Windows statistical package.RESULTS:The overall prevalence of fatty liver was 12.9 %, 15.8 % in males and 7.5 % in females, and the prevalence of fatty liver in males younger than 50 years old, was significantly higher (13.3 %) than that of in females (2.7 %). But the difference between the sexes became less significant in people older than 50 years (19.1 % vs 18.1 %). The prevalence of fatty liver was increased with age; this was markedly presented in females younger than 50 years. Multiple variant regression analysis demonstrated that the prevalence of fatty liver was positively correlated to several risk factors, including male, aging (>50yr), hyperlipidemia, impaired glucose tolerance/diabetes mellitus, hypertension and overweight/obesity.CONCLUSION:There is a high prevalence of nonalcoholic fatty liver among certain population in Shanghai, to which overweight and hyperlipidemia are closely relevant.

Abstract:Background/Aims: To determine the prevalence and risk factors of fatty liver (FL) among Shanghai adults.Methods: A cross-sectional ultrasonographic survey with randomized multistage stratified cluster sampling was used.Results: The study included 3175 subjects (1218 men) with a mean age of 52 years. FL was found in 661 (20.82%) subjects. After adjustment by age and sex, FL prevalence was found to be 17.29%, and the prevalences of alcoholic, suspected alcoholic and nonalcoholic FL were determined to be 0.79, 1.15 and 15.35%, respectively. Generally, age, body mass index (BMI), waist circumference, blood pressure, and the prevalences of obesity, diabetes, hypertension and dyslipidernia were all significantly higher in FL patients than in controls; In contrast, the levels of high-density-lipoprotein cholesterol (HDL-C), education and physical activity were markedly lower. Multiple regression analyses showed that only nine factors (male, educational level, waist circumference, BMI, HDL-C, triglyceride, fasting plasma glucose, diabetes and hypertension) were closely related to FL. In excessive drinkers, obesity increased the risk for FL by 4.8-fold, but excessive drinking was associated with only a 1.5-fold increased risk in obese subjects.Conclusions: FL in Shanghai is highly prevalent and mainly related to multiple metabolic disorders. Abstract:Objective To investigate relation of fatty liver with alcohol drinking and obesity. Methods Using randomized multistage stratified cluster sampling, investigations including questionnaires, physical examinations, oral glucose tolerance tests, blood lipid tests, and real-time liver ultrasonographics subjects were adults age over the 16 in 8 residential areas of Shanghai city. Results 3 175 adults were enrolled consisted of 1 218 males and 1 426 females, with a mean age of 52.4±15.1 years. The prevalence of drinking monthly and obesity were 7.62% and 41.9%, respectively. Ultrasonography detected fatty liver in 661 (20.82%) cases: 3.48% were alcoholic fatty liver, and 92.43% were nonalcoholic fatty liver. Compared with controls, the risk for fatty liver was higher by 3.6 fold(95% CI , 1.1-11.7) in heavy drinkers, 11.6 fold(95% CI , 8.2-16.5) in obese persons, and 17.1 fold(95% CI , 9.1-32.4 ) in persons who were both obese and heavy drinking. In heavy drinkers, obesity increased the risk for fatty liver by 4.8 fold (95% CI ,1.4-16.6), but heavy drinking not significantly (95% CI , 0.9-2.6) increased the risk for fatty liver in obese subjects. Conclusion There is a high prevalence of fatty liver, predominantly nonalcoholic fatty liver, in the adult population of Shanghai. Obesity and associated multiple metabolic disorders are more closely related to fatty liver than excessive drinking.

Abstract:Objectives To investigate the prevalence of non-alcoholic fatty liver(NAFLD) disease and its correlation with chronic metabolic diseases in two primary school students in Shanghai. Methods One thousand five hundred and thirty-two 7-11 year-old students from two primary schools were enrolled in Septamber-October 2011. The anthropometric indices, blood pressure, screening for pseudoacanthosis nigricans and liver ultrasonography of all subjects were recorded. Results The overall prevalence of NAFLD, obesity, abdominal obesity, pseudoacanthosis nigricans, high systolic blood pressure and high diastolic blood pressure was 6.5%, 26.7%, 16.3%, 5.1%, 1.7% and 1.9%, respectively. The prevalence of NAFLD and abdomi-nal obesity in students of central urban area was significantly higher than that in suburban area(P<0.01). The binary regression analysis revealed a significant association between NAFLD in students with sex, age, obesity, abdominal obesity, pseudoacan-thosis nigricans and high systolic blood pressure(P<0.05). Conclusions NAFLD has close correlation with chronic metabolic disease in children. It is time to adopt prevention, detection and treatment in NAFLD children with symptoms of chronic meta-bolic diseases.
Association Between Insulin Resistance, Metabolic Syndrome and Nonalcoholic Fatty Liver Disease in Chinese Adults.

Abstract:Background: The aim of this study was to assess the association between insulin resistance, metabolic syndrome and nonalcoholic fatty liver disease (NAFLD) in Chinese adults.Methods: Fifty five subjects with NAFLD and 55 controls were enrolled for the study. Waist circumference, blood pressure, plasma triglyceride, high density lipoprotein cholesterol and fasting plasma glucose concentrations and homeostasis model assessment of insulin resistance (HOMA-IR) values as an index used to quantify insulin resistance were measured and analyzed. Logistic regression was analyzed to predict independent risk factors of NAFLD.Results: The prevalence of metabolic syndrome in NAFLD group was obviously higher than in controls group (47.3% VS 3.6%, P<0.001). There were all significant differences of each component of metabolic syndrome and HOMA-IR values in comparison of nonalcoholic fatty liver disease (NAFLD) and controls group. In a logistic regression analysis, age, diastolic blood pressure, waist circumference and HOMA-IR were the covariates independently associated with the presence of NAFLD (Odds Ratio=1.107, 1.083, 1.218 and 16.836; 95% Cl: 1.011 (similar to) 1.211, 1.001 similar to 1.173, 1.083 (similar to) 1.370 and 3.626 similar to 78.168, respectively; P<0.05)Conclusion: NAFLD was closely associated with metabolic syndrome and insulin resistance was a very strong predictor of NAFLD.

Abstract:OBJECTIVE: Non-alcoholic fatty liver disease (NAFLD) is a major public health burden in China, and its prevalence is increasing. This study aimed to determine the risk factors and biomarkers of NAFLD.DESIGN: An observational cross-sectional primary survey.SETTING: Central China.PARTICIPANTS: The study included 1479 participants aged over 18 and below 80 years, not currently being treated for cancer or infectious disease or no surgery in the previous year, and no history of cancer or an infectious disease. Participants underwent clinical examination, metabolomic assay and anthropometric assessment. Univariate and logistic regression analyses were used to evaluate associations between covariates and NAFLD.MAIN OUTCOME MEASURES: Risk factors and metabolic biomarkers including sex, body mass index, hypertension, body fat ratio, blood triglycerides, blood fasting glucose, liver enzyme elevation, uric acid and oleic acid-hydroxy oleic acid (OAHOA).RESULTS: Data from the 447 participants (mean age 44.3±11.9 years) were analysed, and the prevalence of NAFLD was 24.7%. Male sex (OR 3.484, 95% CI 2.028 to 5.988), body mass index ≥24 kg/m2 (OR 8.494, 95% CI 5.581 to 12.928), body fat ratio (≥25 for women, ≥20 for men) (OR 1.833, 95% CI 1.286 to 2.756), triglycerides ≥1.7 mmol/L (OR 1.340, 95% CI 1.006 to 1.785), fasting glucose ≥6.1 mmol/L (OR 3.324, 95% CI 1.888 to 5.850), blood pressure ≥140/90 mm Hg or antihypertensive drug treatment (OR 1.451, 95% CI 1.069 to 1.970), uric acid (≥357 μmol/L for women, ≥416 μmol/L for men) (OR 2.755, 95% CI 2.009 to 3.778) and OAHOA (<5 nmol/L) (OR 1.340, 95% CI 1.006 to 1.785) were independent predictors of NAFLD (all P<0.05). These results were verified by all 1479 participants.CONCLUSIONS: NAFLD was common among the study participants. In particular, NAFLD was correlated with uric acid. We identified OAHOA as a novel marker of NAFLD prevalence. It provides a reference on the prevention of NAFLD and related metabolic diseases with the rapid urbanisation, technological advancement and population ageing in China over the recent decades.

Abstract:BACKGROUND AND AIM:To evaluate changes in the prevalence rates of ultrasonographic fatty liver (FL) in a specific population.METHODS:An analysis of the medical records of BaoSteel Group (Shanghai, China) employees was done to evaluate the prevalence of FL in this population, in which health examinations were performed biennially between 1995 and 2002.RESULTS:The study reviewed a database of 59 131 employees, of which 27.1% received medical check-ups four times within the study period, 26.6% three times, and 24.0% twice. The prevalence rates of obesity and metabolic disorders were high at baseline and increased significantly with time. The prevalence of ultrasonographic FL increased from 3.87% to 14.04% in the overall population, while there was an increase from 4.44% to 14.64% in men and 1.56% to 11.37% in women over the 6-year study period. Increased rates of FL were also noted, from 25.88% to 51.39%, among patients with elevated serum alanine aminotransferase (ALT) levels (> 40 U/L). The highest overall prevalence rates of FL were found in individuals aged 50-60 years, with all age-associated prevalence significantly higher in males than females.CONCLUSION:The prevalence of FL increased rapidly over the study period with increased rates of obesity and metabolic disorders; FL is becoming a major cause of abnormal ALT levels in the specific population.

Abstract:Background and AimWith the changes in diet structure and lifestyle, the incidence of fatty liver disease is increasing in China, especially in cities. The goal of the present study was to accurately determine the prevalence and risk factors of fatty liver disease in Beijing residents, China.MethodsBy using random multistage stratification and cluster sampling, residents aged >20 years in Dongcheng District and Tongzhou District were recruited, and questionnaire survey, physical examination, detection of fasting glucose, blood lipids and liver biochemistry, and ultrasonography of the liver, gallbladder, and spleen were carried out. Database EpiData 3.0 was employed for data input, followed by statistical analysis with SPSS version 11.0.ResultsA total of 3762 residents were included in the present study including 2328 males and 1434 females with a mean age of 46.3714.28 years (range 20-92 years). Ultrasonography revealed fatty liver in 1486 residents with a prevalence of 39.5%. Moreover, non-alcoholic fatty liver disease (NAFLD) and alcoholic fatty liver disease were found in 1177 (31.3%) and 309 (8.2%) residents, respectively. After adjustment of prevalence based on the age and gender constituents of Beijing residents, the standardized prevalence of overall fatty liver disease, NAFLD, and alcoholic fatty liver disease was 35.1%, 31.0%, and 4.1%, respectively. Binary logistic regression analysis revealed waist-to-hip ratio, diastolic pressure, fasting blood glucose, triglyceride, high-density lipoprotein cholesterol, and low density lipoprotein cholesterol were closely related to NAFLD.ConclusionsThe Beijing residents have a high prevalence of fatty liver disease as much as 35.1%, which is characterized by NAFLD. Obesity, and glucose and lipid metabolism disorders are the main risk factors of fatty liver disease.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>This study aimed to assess the association between metabolic syndrome (MetS) and severity of non-alcoholic fatty liver disease (NAFLD), and to discuss the pathological relevance of the diagnostic criteria in metabolic (dysfunction) associated fatty liver disease (MAFLD).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This was a multicenter, cross-sectional study. Patients with NAFLD confirmed by liver biopsy were enrolled between July 2016 and December 2018 from 14 centers across the mainland of China. Anthropometric and metabolic parameters were collected to assess the pathological relevance.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Of 246 enrolled patients with NAFLD, 150 (61.0%) had the comorbidity of MetS. With the increase of metabolic components, the proportion of non-alcoholic steatohepatitis (NASH) and significant fibrosis were significantly increased. The comorbid three metabolic components significantly increased the proportion of NASH, and further increase of metabolic components did not increase the proportion of NASH. However, the increase of metabolic components was parallel to the increase of the proportion of liver fibrosis. Among the 246 patients, 239 (97.2%) met the diagnostic criteria of MAFLD. Although non-MAFLD patients had less NASH, they present with similar proportion of significant fibrosis and cirrhosis. In the diagnostic criteria of MAFLD, BMI ≥ 23 kg/m<sup>2</sup> was related to NASH (Mantel-Haenszel Common Estimate OR: 2.975; 95% CI: 1.037-8.538; <em>P</em> = 0.043), and T2DM was related to significant fibrosis (Mantel-Haenszel Common Estimate OR: 2.531; 95% CI: 1.388-4.613; <em>P</em> = 0.002). The homeostasis model assessment of insulin resistance (HOMA-IR) ≥ 2.5 was the most significant factor for NASH (OR: 4.100; 95% CI: 1.772-9.487; <em>P</em> = 0.001) and significant factor for liver fibrosis (OR: 2.947; 95% CI: 1.398-6.210; <em>P</em> = 0.004) after the adjustments of the BMI and diabetes.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Metabolic dysregulations are important risk factors in NAFLD progression. The insulin resistance status may play a predominant role in the progression in MAFLD patients.</p>
