Abstract:Prof. Lanjuan Li Editor-in-ChiefThe 20th century has witnessed a steep decline of the global burden of infectious diseases due to improved hygiene conditions in large parts of the world and the introduction of vaccines and antimicrobials. However, in today's global landscape infectious diseases still remain a major concern. According to the 2017 Global Burden of Disease Study, communicable diseases still accounted for approximately 8.14 million deaths, which was 14.6% of all age deaths for that year.1 Furthermore, it accounted for 24% of all age years of life lost. Infectious disease outbreaks have been rising since the 1980s2 and pathogenic microbes are now spreading faster geographically due to international traveling. Simultaneously, emerging and re-emerging infectious diseases are being identified faster than ever before. The resurgence of these emerging and re-emerging diseases might be caused by climate change, changing international food chains, and a rise in antimicrobial resistance. According to a recent report on antimicrobial resistance issued by the UK government,3 unless proper action is taken, drug-resistant diseases might cause 10 million deaths per year by 2050, costing the global economy up to 100 trillion dollar. This is a daunting future scenario that we should be able to avert with joined global efforts and extensive surveillance, reporting and intensified fundamental and applied research on infectious microbes and diseases. Over the past decades, China has transformed itself into an international powerhouse with an economic aggregate that has become the second-largest in the world. As a result, China's science and technology have entered a stage of great outbreak, and the growth rate of its scientific research has attracted worldwide attention. However, according to Web of Science, the number of English journals in China is only just over 300. Despite its rapid growth, the number of English journals still lags behind the overall scientific research output of China. Therefore, there is a serious imbalance between the scientific research level and the number of international journals in China. One of the fields that have shown rapid growth in China is in the field of infectious microbes and diseases. China has gradually moved to the forefront in our global efforts to combat infectious diseases. Of the 32 new infectious diseases discovered in the world over the past 20 years, about half have emerged in China. Furthermore, China has been at the center of efforts to contain pandemics of Severe Acute Respiratory Syndrome in 2003,4 H1N1 influenza in 20095 and H7N9 influenza in 2013.6 Many of the Chinese scientists responsible for pushing the field teamed-up to jointly establish the Collaborative Innovation Centre for Diagnosis and Treatment of Infectious Diseases (CCID) in 2012. The CCID was housed at the State Key Laboratory for Diagnosis and Treatment of Infectious Diseases in China and has attracted a large number of domestic and foreign outstanding talents. Since its establishment, authors affiliated with CCID have published over 2500 high-quality international research papers, which are estimated to be approximately 1/25th of all high-level publications in the field of infectious diseases containing Chinese authors. Now, CCID decided to launch a new journal in the field of infectious microbes and diseases to help Chinese international publishing grow up to its potential. The new journal “Infectious Microbes & Diseases” will take the open-access model, and will be co-published by Lippincott Williams & Wilkins and Zhejiang University Press on behalf of the CCID and the State Key Laboratory for Diagnosis and Treatment of Infectious Diseases in China. CCID hopes to run a world-class academic journal that promotes the timely and widespread dissemination of research on infectious microbes and diseases, improves the level of diagnosis, treatment, prevention, and control of infectious microbes and diseases, and safeguard human health. The research field of infectious microbes and diseases deserves an excellent platform based in China to publish and rapidly disseminate excellent research, and which is committed to solving the public health and safety problems caused by infectious microbes and diseases globally.
Preparing the Developing World for the Next Pandemic: Evidence from China’s R&D Blueprint for Emerging Infectious Diseases

Various generalized and innovative platform technologies were also highlighted for enhancing overall capacities of EID preparedness and response, covering basic research, experiment, detection, prevention and control, surveillance and information sharing. Conclusions This is the first study in developing countries that established an R&D Blueprint of prioritized diseases, countermeasures and technologies. Our findings could help to drive pre-emptive scientific and technological actions toward emerging pathogens that may cause the next epidemic and could provide evidence-based strategies for developing countries to establish their national health research agenda tailored to health and research context under resource-limited settings.

Abstract:To adequately prepare for potential hazards caused by emerging and reemerging infectious diseases, the WHO has issued a list of high-priority pathogens that are likely to cause future outbreaks and for which research and development (R&D) efforts are dedicated, known as paramount R&D blueprints. Within R&D efforts, the goal is to obtain effective prophylactic and therapeutic approaches, which depends on a comprehensive knowledge of the etiology, epidemiology, and pathogenesis of these diseases. In this process, the accessibility of animal models is a priority bottleneck because it plays a key role in bridging the gap between in-depth understanding and control efforts for infectious diseases. Here, we reviewed preclinical animal models for high priority disease in terms of their ability to simulate human infections, including both natural susceptibility models, artificially engineered models, and surrogate models. In addition, we have thoroughly reviewed the current landscape of vaccines, antibodies, and small molecule drugs, particularly hopeful candidates in the advanced stages of these infectious diseases. More importantly, focusing on global trends and novel technologies, several aspects of the prevention and control of infectious disease were discussed in detail, including but not limited to gaps in currently available animal models and medical responses, better immune correlates of protection established in animal models and humans, further understanding of disease mechanisms, and the role of artificial intelligence in guiding or supplementing the development of animal models, vaccines, and drugs. Overall, this review described pioneering approaches and sophisticated techniques involved in the study of the epidemiology, pathogenesis, prevention, and clinical theatment of WHO high-priority pathogens and proposed potential directions. Technological advances in these aspects would consolidate the line of defense, thus ensuring a timely response to WHO high priority pathogens.

Abstract:The escalating global changes have fostered conditions for the expansion and transmission of diverse biological factors, leading to the rise of emerging and reemerging infectious diseases. Complex viral infections, such as COVID-19, influenza, HIV, and Ebola, continue to surface, necessitating the development of effective vaccine technologies. This review article highlights recent advancements in molecular biology, virology, and genomics that have propelled the design and development of innovative molecular tools. These tools have promoted new vaccine research platforms and directly improved vaccine efficacy. The review summarizes the cutting-edge molecular engineering tools used in creating novel vaccines and explores the rapidly expanding molecular tools landscape and potential directions for future vaccine development. The strategic application of advanced molecular engineering tools can address conventional vaccine limitations, enhance the overall efficacy of vaccine products, promote diversification in vaccine platforms, and form the foundation for future vaccine development. Prioritizing safety considerations of these novel molecular tools during vaccine development is crucial.

Abstract:Abstract Objective: The R&D explosion for Ebola virus disease (EVD) during the 2014-2016 outbreak led to the successful development of high-quality vaccines performed by China and the U.S. This study aims to compare the R&D activities of Ebola-related medical products in two countries, as a way to present the influential factors of R&D for emerging infectious disease (EID) and to provide suggestions for timely and efficient R&D response to the COVID-19 pandemic.Methods: In this comparative study, R&D activities were analyzed in terms of research funding, scientific research outputs, R&D timeline, and government incentive and coordinated mechanisms. Quantitative analysis was performed using data retrieved from national websites, clinical trial registries and databases.Qualitative semi-structured interviews were conducted to explore perspectives of fifteen key informants from EID field, especially of those involved in Ebola product development. Findings: The funding gap between China and the U.S. was significant before 2014 and narrowed after the Ebola outbreak. Both research teams started basic studies prior to the Ebola outbreak; however, the U.S. got FDA approval for clinical trials 5 months earlier than China. The underlying gap reveals the lack of participation and support by private sectors, the stagnant transformation platform, and the inadequate government incentives in China. Conclusion: R&D pre-planning and resource deployment mechanisms are crucial for EID preparedness and response. Funding should be allocated to support diversified R&D institutions for coping with the risks. Building private and public collaboration, and strengthening government support for clinical trials may accelerate the translation of basic research.
A bibliometric insight into nanomaterials in vaccine: trends, collaborations, and future avenues

Abstract:Background: The emergence of nanotechnology has injected new vigor into vaccine research. Nanovaccine research has witnessed exponential growth in recent years; yet, a comprehensive analysis of related publications has been notably absent. Objective: This study utilizes bibliometric methodologies to reveal the evolution of themes and the distribution of nanovaccine research. Methods: Using tools such as VOSviewer, CiteSpace, Scimago Graphica, Pajek, R-bibliometrix, and R packages for the bibliometric analysis and visualization of literature retrieved from the Web of Science database. Results: Nanovaccine research commenced in 1981. The publication volume exponentially increased, notably in 2021. Leading contributors include the United States, the Chinese Academy of Sciences, the "Vaccine", and researcher Zhao Kai. Other significant contributors comprise China, the University of California, San Diego, Veronique Preat, the Journal of Controlled Release, and the National Natural Science Foundation of China. The USA functions as a central hub for international cooperation. Financial support plays a pivotal role in driving research advancements. Key themes in highly cited articles include vaccine carrier design, cancer vaccines, nanomaterial properties, and COVID-19 vaccines. Among 7402 keywords, the principal nanocarriers include Chitosan, virus-like particles, gold nanoparticles, PLGA, and lipid nanoparticles. Nanovaccine is primarily intended to address diseases including SARS-CoV-2, cancer, influenza, and HIV. Clustering analysis of co-citation networks identifies 9 primary clusters, vividly illustrating the evolution of research themes over different periods. Co-citation bursts indicate that cancer vaccines, COVID-19 vaccines, and mRNA vaccines are pivotal areas of focus for current and future research in nanovaccines. "candidate vaccines," "protein nanoparticle," "cationic lipids," "ionizable lipids," "machine learning," "long-term storage," "personalized cancer vaccines," "neoantigens," "outer membrane vesicles," "in situ nanovaccine," and "biomimetic nanotechnologies" stand out as research interest. Conclusions: This analysis emphasizes the increasing scholarly interest in nanovaccine research and highlights pivotal recent research themes such as cancer and COVID-19 vaccines, with lipid nanoparticle-mRNA vaccines leading novel research directions.

In this review, we depict the latest panorama of global COVID-19 vaccine research and development based on different technology platforms, and summarize key characteristics and available evidence on vaccines authorized for emergency use, in order to provide insights into improve coordination in the COVID-19 outbreak response for related stakeholders.

A Summary on Tuberculosis Vaccine Development-Where to Go?

Abstract:Background: Tuberculosis (TB) is an old infectious disease caused by Mycobacterium tuberculosis infection. Vaccination is the most effective way to prevent and control TB. However, there is relatively little literature that systematically analyzes the progress of new TB vaccine research from a bibliometric perspective. This study was conducted to examine the development of TB vaccines over the past 20 years and to identify research priorities and directions for the future. Methods: The Science Citation Index Expanded (SCI-E) of the Web of Science Core Collection (WOSCC) database was selected to search the literature related to TB vaccines. The countries, institutions, authors, journals, references, and keywords of each publication were analyzed and visualized using the VOSviewer, CiteSpace, and Bibliometrix software. Furthermore, GraphPad Prism and Microsoft Excel 365 were also used for statistical analysis. Results: As of 20 October 2022, 7960 publications related to TB vaccines were identified with 288,478 citations. The United States of America (USA) accounted for the largest share (2658, 33.40%), followed by the United Kingdom (UK, 1301, 16.34%), and China (685, 8.6%). Regarding affiliations, the University of London had the most publications (427) and shared the highest H-index (76) with the Statens Serum Institut of Denmark. In terms of the number of articles for the journals and authors, the journal Vaccine ranked first with 629 articles. Professor Peter Anderssen has published the highest number of papers (160). The burst keywords and thematic maps analysis showed that future trends in TB vaccine development would focus on exploring the interaction mechanisms between M. tuberculosis and the host. Conclusion: The number of publications on TB vaccines has grown over the past two decades. Developed countries play a significant role in TB vaccine research, and developing countries are fast catching up. We believe that future research will be aimed at understanding the fine molecular mechanisms of host–pathogen interaction, leading to the development of better TB vaccines.

Abstract:Confronted with the Coronavirus disease 2019 (COVID-19) pandemic, China has become an asset in tackling the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission and mutation, with several innovative platforms, which provides various technical means in this persisting combat. Derived from collaborated researches, vaccines based on the spike protein of SARS-CoV-2 or inactivated whole virus are a cornerstone of the public health response to COVID-19. Herein, we outline representative vaccines in multiple routes, while the merits and plights of the existing vaccine strategies are also summarized. Likewise, new technologies may provide more potent or broader immunity and will contribute to fight against hypermutated SARS-CoV-2 variants. All in all, with the ultimate aim of delivering robust and durable protection that is resilient to emerging infectious disease, alongside the traditional routes, the discovery of innovative approach to developing effective vaccines based on virus properties remains our top priority.

The research foundation for COVID-19 vaccine development

Abstract:The development of effective vaccines in <1 year to combat the spread of coronavirus disease 19 (COVID-19) is an example of particularly rapid progress in biomedicine. However, this was only made possible by decades of investment in scientific research. Many important research commentaries and reviews have been provided to describe the various contributions and scientific breakthroughs that led to the development of COVID-19 vaccines. In this work, we sought to complement those efforts by adding a systematic and quantitative study of the research foundations that led to these vaccines. Here, we analyzed citations from COVID-19 vaccine research articles to determine which scientific areas of study contributed the most to this research. Our findings revealed that coronavirus research was cited most often, and by a large margin. However, significant contributions were also seen from a diverse set of fields such as cancer, diabetes, and HIV/AIDS. In addition, we examined the publication history of the most prolific authors of COVID-19 vaccine research to determine their research expertise prior to the pandemic. Interestingly, although COVID-19 vaccine research relied most heavily on previous coronavirus work, we find that the most prolific authors on these publications most often had expertise in other areas including influenza, cancer, and HIV/AIDS. Finally, we used machine learning to identify and group together publications based on their major topic areas. This allowed us to elucidate the differences in citations between research areas. These findings highlight and quantify the relevance of prior research from a variety of scientific fields to the rapid development of a COVID-19 vaccine. This study also illustrates the importance of funding and sustaining a diverse research enterprise to facilitate a rapid response to future pandemics.

Methods R&D activities were analyzed in terms of research funding, scientific research outputs, R&D timeline, and governmental mechanisms. Quantitative analysis was performed using data retrieved from national websites, clinical trial registries and databases. Qualitative semi-structured interviews were conducted to explore perspectives of key informants involved in Ebola product development. Results The funding gap between China and the U.S. was significant before 2014 and narrowed after the Ebola outbreak. Both research teams started basic studies prior to the outbreak; however, the U.S. got FDA approval for clinical trials 5 months earlier than China. The underlying gap reveals the lack of participation and support by private sectors, stagnant transformation platform, and inadequate government incentives in China. Conclusions R&D pre-planning and resource deployment mechanisms are crucial for EID preparedness and response. Building private and public collaboration, and strengthening government support for clinical trials may accelerate the translation of basic research.

Abstract:Immunization is a cornerstone of public health. Despite great success, China's National Immunization Program (NIP) faces challenges, such as the integration of several World Health Organization-recommended vaccines and other systemic issues. The Innovation Laboratory for Vaccine Delivery Research (VaxLab), supported by the Bill & Melinda Gates Foundation and established in 2021 at Duke Kunshan University, focuses on enhancing China's NIP through research and policy advocacy. This editorial aims to summarize the key findings of the manuscripts published in the collection contributed by VaxLab team and set the future research agenda.

Abstract:Vaccines play a critical role in the prevention of life-threatening infectious disease. However, the development of effective vaccines against many immune-evading pathogens such as HIV has proven challenging, and existing vaccines against some diseases such as tuberculosis and malaria have limited efficacy. The historically slow rate of vaccine development and limited pan-variant immune responses also limit existing vaccine utility against rapidly emerging and mutating pathogens such as influenza and SARS-CoV-2. Additionally, reactogenic effects can contribute to vaccine hesitancy, further undermining the ability of vaccination campaigns to generate herd immunity. These limitations are fuelling the development of novel vaccine technologies to more effectively combat infectious diseases. Towards this end, advances in vaccine delivery systems, adjuvants, antigens and other technologies are paving the way for the next generation of vaccines. This Review focuses on recent advances in synthetic vaccine systems and their associated challenges, highlighting innovation in the field of nano- and nucleic acid-based vaccines.
