Abstract:AIM:To analyze the quantitative differences between hepatitis B virus markers (HBV M) and HBV DNA in population with high-level or low-level HBsAg, and to reveal the characteristics of HBV DNA and HBV M in population with low-level HBsAg. METHODS:A total of 264 chronic HBV-infected patients were enrolled in our study, and were divided into low-level HBsAg group (147 cases) and high-level HBsAg group (117 cases) based on the HBsAg level or were divided into immune tolerance stage, immune clearance stage and non-active stage based on the natural history. Real-time PCR and microparticle enzyme immunoassay (MEIA) were used to determine the content of HBV DNA and HBV M in serum samples of high-level and low-level HBsAg patients, respectively, then the quantitative results were compared. RESULTS:There were statistically significant differences between HBV DNA and HBV M (anti-HBs, HBeAg and anti-HBe) of low-level HBsAg patients in immune tolerance stage (7 cases) and immune clearance stage(4 cases) (t=2.531-9.181, P<0.01-0.05). HBV DNA lower than 10(5) copies/L was found in 94.1% (128/136) of serum samples in non-active stage, and the detection rates of direct PCR and enriching PCR were 10.3% (14/136) and 10.3% (47/136) in low-level HBsAg group, respectively, and there were no correlations between HBV DNA and each of HBV M (P>0.05). In high-level HBsAg group, HBV DNA was positively correlated with HBeAg and anti-HBe in 25 cases of immune tolerance stage (r=0.744-0.772, t(r)=3.858-4.207, P<0.01), and was only negatively correlated with anti-HBs in 46 cases of immune clearance stage and 46 cases of non-active stage (r=-0.693 - -0.598, t(r)=-4.616 - -3.936, P<0.01-0.05). CONCLUSION:Population with low-level HBsAg have specific serological characteristics, which may be correlated with organic immune tolerance and individualized immune responses.

Abstract:To understand clinical features and immunity of chronic HBV-infected patients with high or low level HBsAg, the differences of serum two-component-determined circulating immune complexes (TCIC) and peripheral blood lymphocyte subsets between these patients were analyzed. 126 patients with chronic hepatitis B virus infection were divided into two groups including 44 with low-level HBsAg and 82 with high-level HBsAg, and three phases including non-active phase, immune active phase and immune tolerant phase by level of HBsAg and natural history of chronic hepatitis B virus infection. Antibody capture-ELISA method and flow cytometry were used to assay serum TCIC and peripheral blood lymphocyte subsets in 44 patients with low-level HBsAg (Group A), 82 patients with high-level HBsAg (Group B) and 22 healthy volunteers (Group C), respectively, and the results were analyzed and compared. Among these 44 patients in Group A, 40 patients (90.9%) were stable in non-active phase and the positive rate of HBsAg/IgG-CIC was the highest, accounting for 15.9% (7/44), while the positive rate of HBsAg/C3-CIC was the highest in Group B, accounting for 46.3% (38/82). The positive rates of serum TCIC, A value and non-active CD3(+)CD8(+) percentages in Group A were all lower than those in Group B with corresponding phases (P<0.01-0.05), however, the percentages of CD4(+)/CD8(+) were higher than those in the corresponding phases of Group B(P<0.01). Patients with low-level HBsAg were correlated to low capability of TCIC synthesis and elimination and there was a low dose-induced immunotolerance phenomenon.
Study on hepatitis B virus genotypes in Wenzhou district by PCR using type-specific primers and its clinical significance

Abstract:Objective To investigate the distribution of hepatitis genotypes in Wenzhou district detected by using type-specific primers PCR and its clinical significance. MethodsSerum samples from 202 HBV DNA positive patients with chronic HBV infection,including 52 chronic asymptomatic carriers(ASC),103 chronic hepatitis B(CHB),27 liver cirrhosis(LC) and 20 hepatocellular carcinoma(HCC) patients were collected and detected for HBV genotypes by type-specific primers PCR.ResultsOf all 202 patients,37 cases(18.3%) were genotype B,123 cases(60.9%) were genotype C and 41 cases were mixtures of genotype B and C.Genotype C was more prevalent in the CHB and HCC than genotype B and mixtures of genotype B/C(P﹤0.05);the ALT value and HBV DNA was significantly higher in genotype C than that in genotype B,mixtures of genotype B/C patients(P﹤0.05).The positive rate of HBeAg was also significantly higher in genotype C than in genotype B,mixtures of genotype B/C patients(P﹤0.05).ConclusionGenotype B,genotype C,genotype D and BC combination exist in Wenzhou district,and genotype C is the major genotype in this area,which may induce more severe the liver inflammation.

Abstract:The serological markers with coexistence of hepatitis B surface antigen (HBsAg) and antibody to HBsAg (anti-HBs) of hepatitis B virus (HBV) infection were rare pattern. The virological significance, immune response and clinical outcome of these patients remain largely unknown.This research explores the relationship between this serological profile and HBV genome variants.We studied 35 patients both carrying HBsAg and anti-HBs (group I), and 70 patients with HBsAg positive but anti-HBs negative (group II, served as control). The HBV genome sequences were obtained by direct sequencing of polymerase chain reaction (PCR) products.The amino acid (aa) variation within major hydrophilic region (MHR), especially in the first loop (aa124-137) of "a" determinant in group I is significantly higher than those in group II. The aa variation of cytotoxic lymphocyte (CTL) epitope in HBsAg (aa87-aa95) in group I is also significantly higher than that in group II. Interestingly, the basal core promoter (BCP) double mutations (A1762T/G1764A) in group I is significantly higher than those in group II as well.In patients with HBV infection, the coexistence of HBsAg and anti-HBs is associated with an increased aa variability in several key areas of HBV genome. The molecular characteristic of HBV in HBsAg and anti-HBs positive patients is distinct and worth further studies.
Hepatitis B surface antigen levels during natural history of chronic hepatitis B: a Chinese perspective study.

Abstract:AIM: To determine the baseline hepatitis B surface antigen (HBsAg) levels during the different phases of chronic hepatitis B (CHB) patients in China. METHODS: Six hundred and twenty-three hepatitis B virus or un-infected patients not receiving antiviral therapy were analyzed in a cross-sectional study. The CHB patients were classified into five phases: immune-tolerant (IT, n = 108), immune-clearance (IC, n = 161), hepatitis B e antigen negative hepatitis (ENH, n = 149), low-replicative (LR, n = 135), and liver cirrhosis (LC, n = 70). HBsAg was quantified (Abbott ARCHITECT assay) and correlated with hepatitis B virus (HBV) DNA, and serum alanine aminotransferase/aspartate aminotransferase (ALT/AST) in each phase of CHB was also determined. RESULTS: Median HBsAg titers were different in each phase of CHB (P < 0.001): IT (4.85 log(10) IU/mL), IC (4.36 log(10) IU/mL), ENH (2.95 log(10) IU/mL), LR (3.18 log(10) IU/mL) and LC (2.69 log(10) IU/mL). HBsAg titers were highest in the IT phase and lowest in the LC phase. Serum HBsAg titers showed a strong correlation with HBV viral load in the IC phase (r = 0.683, P < 0.001). No correlation between serum HBsAg level and ALT/AST was observed. CONCLUSION: The mean baseline HBsAg levels differ significantly during the five phases of CHB, providing evidence on the natural history of HBV infection. Abstract:AIMTo investigate hepatitis B surface antigen (HBsAg) levels in patients with HBeAg-positive chronic hepatitis B (CHB) and different immune conditions.METHODSHBeAg-positive CHB patients with different immune conditions were enrolled in this cross-sectional study. These patients were grouped according to the following criteria: immune-tolerant patients, IT group; patients with a mild immune response in the immune clearance phase, IC-Mild group; and patients with a dramatic immune response in the immune clearance phase and exhibiting acute on chronic liver failure (ACLF), ACLF group. All these patients had not previously received antiviral therapy and were enrolled at a pre-settled ratio of 2:2:1. Serum HBsAg levels and the correlation between serum HBsAg level and serum hepatitis B virus (HBV) DNA level were evaluated in these groups.RESULTSIn total, 180 HBeAg-positive CHB patients [IT group (n = 72), IC-Mild group (n = 72), and ACLF group (n = 36)] were enrolled in this study. The median serum HBsAg levels varied among the groups (P < 0.001): IT, 4.86 log10 IU/mL; IC-Mild, 3.97 log10 IU/mL; and ACLF, 3.57 log10 IU/mL. Serum HBsAg level showed a moderate positive correlation with serum HBV-DNA level in the IC-Mild group (r = 0.60, P < 0.001), but exhibited a weaker correlation in the IT (r = 0.52, P < 0.001) and ACLF groups (r = 0.51, P = 0.001). The ratio of HBsAg/HBV DNA did not differ significantly among the IT, IC-Mild, and ACLF groups (medians: 0.56, 0.55, and 0.56, respectively; P = 0.179).CONCLUSIONSerum HBsAg levels varied significantly in HBeAg-positive patients with different immune conditions. These findings may have important implications for understanding the immune clearance of HBV in HBeAg-positive CHB patients.

Abstract:Background/Aims: To investigate the correlation of hepatitis B virus (HBV) genotypes and basal core promoter (BCP) and precore (PC) mutations in patients with chronic hepatitis B. Methods: HBV genotyping, nucleotide mutation, serum HBV DNA level and serological markers were analyzed in 121 patients with chronic HBV infection using INNO-LiPA HBV genotyping, polymerase chain reaction (PCR) product-based sequencing, fluorescence quantitative PCR and enzyme-linked immunosorbent assays respectively. Results: Forty (33.0%), 77 (63.6%), two (1.7%) and two (1.7%) patients had genotypes B, C, B/C and D infections respectively. Significant differences were found in serum HBV DNA levels (log(10) copies/ml: 6.18 vs. 5.61, P=0.042) and mutations at nucleotide (nt) 1762/1764 (71.4% vs. 42.5%, P=0.002) between genotypes C- and B-infected patients. There were significant differences in the mean age, serum biochemical parameter levels and mutation rates in BCP/PC among hepatitis e antigen (HBeAg)-positive and -negative chronic hepatitis B (CHB) and liver cirrhosis (LC) groups. Conclusion: Genotypes C and B are predominant in China, and the frequent nt 1762/1764 mutation, which occurs commonly in HBeAg-negative CHB, especially in genotype C patients, may be associated with the progress of chronic HBV infection.
HBsAg as an important predictor of HBeAg seroconversion following antiviral treatment for HBeAg-positive chronic hepatitis B patients

Abstract:Background Serum quantitative hepatitis B surface antigen (HBsAg) levels may be an important predictor of hepatitis B e antigen (HBeAg) seroconversion (SC) in HBeAg-positive chronic hepatitis B (CHB) patients during antiviral treatment. The pattern of HBsAg variation in CHB patients either with or without SC following tenofovir disoproxil fumarate (TDF) treatment is not clearly understood. Methods Twenty patients with full experimental data were enrolled, and liver biochemistry, serum HBV DNA, and circulating CD4 + CD25 + regulatory T cell (Treg) levels were determined at baseline and every 12weeks after the initiation of TDF treatment (for a total of 96weeks). In addition, the relationship between HBsAg or HBeAg and alanine aminotransferase (ALT), HBV DNA and Treg levels in SC and non-SC patients was analyzed. Results In all, 9 patients had undergone HBeAg seroconversion by week 72 of TDF treatment, and biochemical and virological indexes and Treg percentages declined to normal levels. Furthermore, the positive correlation between HBsAg and ALT, HBV DNA and Treg levels was significant for SC patients, but not for non-SC patients. However, for HBeAg, significant positive correlations were or not observed for both SC and non-SC patients. Conclusions The quantitation of HBsAg is a more useful indicator than HBeAg for distinguishing SC and non-SC patients during TDF treatment. Moreover, HBsAg may be related to immune regulatory property of CHB patients during antiviral treatment.
Association of hepatitis B virus mutations in basal core promoter and precore regions with severity of liver disease: an investigation of 793 Chinese patients with mild and severe chronic hepatitis B and acute-on-chronic liver failure

Abstract:ObjectiveTo investigate the features of hepatitis B virus (HBV) basal core promoter/precore (BCP/PC) mutations and genotypes in a large number of mild/severe chronic hepatitis B (CHB-M/CHB-S), and acute-on-chronic liver failure (ACLF) patients and analyze the clinical implications of the virologic features.Patients and methodsSera of 793 (325 CHB-M, 170 CHB-S, and 298 ACLF) patients admitted to or who had visited Beijing 302 Hospital from January 2005 to December 2008 were collected and successfully amplified for the HBV BCP/PC and a 1225-bp-long S/Pol (nt 54–1278) gene regions. Biochemical and serological parameters and HBV DNA level were routinely performed. Viral DNA was extracted and subjected to a nested PCR. Genotypes/subgenotypes were determined based on complete genomic sequence or on analysis of the 1225-bp-long S/Pol-gene sequence. HBV genotyping was performed by direct PCR sequencing followed by molecular evolutionary analysis of the viral sequences. A P value of <0.05 (two-sided) was considered to be statistically significant.ConclusionsOur findings suggest that CHB patients infected with BCP/PC mutant viruses are more susceptible to severe hepatitis and ACLF than those with the BCP/PC wild-type virus and that ACLF patients with PC mutant viruses have an increased risk of death. As such, the HBV PC mutation is a potential predictive indicator of ACLF outcome.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background & Aims</h3><p>During chronic hepatitis B virus (HBV) infection, suppressed functionality of natural killer (NK) cells might contribute to HBV persistence but the underlying mechanisms remain elusive. A peculiar feature of HBV is the secretion of large amount of hepatitis B surface antigen (HBsAg). However, the effect of HBsAg quantities on NK cells is unclear. The aim was to determine the effects of HBsAg quantities on NK cell functionality in patients with chronic hepatitis B (CHB).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>80 CHB patients were included and categorized into four groups based on their HBsAg levels. As a control, 30 healthy donors were enrolled. NK cell frequency, phenotype and function were assessed using flow cytometry and correlated with HBsAg levels and liver enzymes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Compared to the healthy controls, a reshaping of NK cell pool towards more CD56<sup>bright</sup> NK cells was observed during CHB infection. Importantly, NK cells in patients with low HBsAg levels (<100 IU/ml) displayed an activated phenotype with increased expression of activation makers CD38, Granzyme B and proliferation marker Ki‐67 while presenting with defective functional responses (MIP‐1β, CD107a) at the same time. Furthermore, NK cell activation was negatively correlated with patient HBsAg levels while NK function correlated with patient age. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>The differential regulation of NK cell phenotype and function suggests that activation of NK cells in patients with low serum HBsAg levels may contribute to HBV clearance.</p></section>

Prevalence of Basal Core Promoter and Precore Mutations in Chinese Chronic Hepatitis B Patients and Correlation with Serum HBeAG Titers

Abstract:The A1762T and G1764A mutations in the basal core promoter (BCP) region and the G1896A mutation in the precore (PC) region of hepatitis B virus (HBV) genome are found commonly in HBeAg-negative patients. Experiments in vitro suggest that BCP and PC mutation reduce and abolish HBeAg expression, respectively. In the present study, the prevalence of the BCP and PC mutations were determined in 207 patients with HBeAg positive chronic hepatitis B from China and correlated with the titers of serum HBeAg. None of the patients received antiviral therapy. The HBV genotype was determined by direct sequencing of the HBsAg gene. The BCP and PC mutations were detected by the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and confirmed by DNA sequencing. The HBeAg titer was measured by the microparticle enzyme immunoassay. Fifty-one of the 207 patients (24.6%) were infected with genotype B and the remainder with genotype C. The BCP mutations were detected in 103 patients (50%) while the PC mutation was present in 43 (20.8%). Thirteen patients (6.3%) harbored both BCP and PC mutations. No significant difference in the titers of HBeAg was found between patients infected with the two HBV genotypes, but the presence of either the BCP or PC mutation was associated with reduced HBeAg titer (P < 0.05). The presence of both the BCP and PC mutations was accompanied by even lower HBeAg titer (P < 0.05). These findings confirm that in patients with HBeAg, the BCP and PC mutations reduced the expression of HBeAg.

Abstract:Background This study aimed to explore the molecular mechanism of the coexistence of hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (HBsAb) serological pattern via intensive characterization of HBV s gene in both chronic hepatitis B (CHB) and hepatocellular carcinoma (HCC) patients. Method A total of 73 HBsAg+/HBsAb+ patients (CHB = 36, HCC = 37) and 96 HBsAg+/HBsAb− patients (CHB = 47, HCC = 49) were enrolled from 13 medical centers in China. The sequence features were elaborated based on the combination of next-generation sequencing (NGS) and multidimensional bioinformatics analysis. Results The 16 high-frequency missense mutations, changes of stop codon mutation, clustering, and random forest models based on quasispecies features demonstrated the significant discrepancy power between HBsAg+/HBsAb+ and HBsAg+/HBsAb− in CHB and HCC, respectively. The immunogenicity for cytotoxic T lymphocyte (CTL) epitope Se and antigenicity for the major hydrophilic region (MHR) were both reduced in HBsAg+/HBsAb+ patients (CTL Se: p < 0.0001; MHR: p = 0.0216). Different mutation patterns were observed between HBsAg+/HBsAb+ patients with CHB and with HCC. Especially, mutations in antigenic epitopes, such as I126S in CHB and I126T in HCC, could impact the conformational structure and alter the antigenicity/immunogenicity of HBsAg. Conclusion Based on NGS and bioinformatics analysis, this study indicates for the first time that point mutations and quasispecies diversities of HBV s gene could alter the MHR antigenicity and CTL Se immunogenicity and could contribute to the concurrent HBsAg+/HBsAb+ with different features in HCC and CHB. Our findings might renew the understanding of this special serological profile and benefit the clinical management in HBV-related diseases.

The linear regression and multivariate logistic regression analysis were performed to determine associated factors of serum HBV pgRNA levels.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> RESULTS</h3><p>In this cohort, the median serum HBV pgRNA level was 4.12 log<sub>10</sub> copies/mL and 33.33%(68/204) of them had serum HBV pgRNA under low limit of detection(LLD) (<500 copies/mL); and the percentage of patients with serum HBV pgRNA under LLD in HBeAg‐positive patients was significantly lower than that in HBeAg‐negative patients (15.75%[23/46] <i>vs.</i>77.59%[45/58], <i>P</i><0.001). Overall, serum HBV pgRNA strongly correlated with HBcrAg (<i>r</i>=0.760, <i>P</i><0.001), and moderately correlated with HBV DNA (<i>r</i>=0.663, <i>P</i><0.001) and HBsAg (<i>r</i>=0.670, <i>P</i><0.001). As compared to HBsAg and HBV DNA, only HBcrAg showed stable correlation with serum HBV pgRNA both in HBeAg‐positive and HBeAg‐negative patients. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> CONCLUSIONS</h3><p>Serum HBV pgRNA level differed between HBeAg‐positive and HBeAg‐negative patients; and it had better and more stable correlation with serum HBcrAg than serum HBV DNA and HBsAg, irrespective of HBeAg status.</p><p>This article is protected by copyright. Evolutionary Analysis of Pre-S/S Mutations in HBeAg-Negative Chronic Hepatitis B with HBsAg < 100 IU/ml

Abstract:Background: Hepatitis B surface antigen (HBsAg) and viral load are important clinical indicators for antiviral therapy. Few studies have evaluated viral sequence biomarkers predicting the risk of hepatocellular carcinoma (HCC) in the stage, which show a low serological response (HBsAg < 100 IU/ml) and high viral levels (HBV DNA > 2,000 IU/ml). This study aims to determine the trend of the biological prevalence within the pre-S/S regions of special model of inactive CHB infection. Methods: We used Sanger sequencing, quantitative HBV serology (HBeAg and HBsAg), and liver function index to identify whether HBV genome sequences are associated with long-term risk of further HCC progression in special inactive CHB infection. Results: HBV sequencing analysis of 28 CHB patients with special infectious pattern showed higher genetic diversity among four opening reading frames (ORFs) (p < 0.001). However, dN/dS ratios of HBsAg and pre-C/C regions in the experimental group showed no significantly different from those in the HCC group (p = 0.06), while significantly lower in polymerase and HBxAg regions of the experimental group (p < 0.001). In addition, seven positively selected sites were identified in pre-S1, five in pre-S2, and four in S, in which five sites (128H/135Q/135R/139L/141P) were among “α” determinant. Conclusions: These mutations in the pre-S/S region might be associated with the HCC phenotype of low HBsAg expression, with the P region possibly impacting high viral loads. Increased viral diversity across the HBV genome is also associated with low levels of HBsAg. The cumulative evolutionary changes in the HBV pre-S/S regions shows that facilitate immune evasion should be monitored individually. Due to the similarity of evolutionary characteristics in HCC, low serological responses and high viremia may be associated with the risk of further disease progression.

Abstract:Objective To analyze the serological patterns of both HBsAg and HBsAb positive patients with chronic HBV infection, and to investigate the virus gene sequence variation or missing pre-S/S ifeld for genotyping and in order to explore its clinical signiifcance. Methods Total of 100 cases with chronic HBV infection whom with both HBsAg and HBsAb positive at the same time were screened by enzyme linked immunoabsorbent assay analysis method and the results were reconfirmed by chemiluminessence micro-particle immunoassay. HBV DNA content were detected by real-time lfuorescent quantitative PCR. There were 60 cases with HBV DNA positive and 40 cases with HBV DNA negative. Among the 100 cases, 60 cases with both HBsAg and HBsAb positive as experimental group, and 60 cases with chronic hepatitis B whom with HBsAg (+) and HBsAb (-) as control group. HBV pre-S/S gene sequences and sequencing analysis of the two groups were detected by polymerase chain reaction (PCR) method with in vitro ampliifcation. Compared the two groups of pre-S/S gene variants according to the sequencing results of genetic classiifcation, thus combined with clinical data to explore its clinical signiifcance. Results Among the experimental group, genotypes B were 19 cases and genotype C were 41 cases;among the control group, of genotype B were 18 cases and genotype C were 42 cases. Patients with genotype B were older than cases with genotype C in experimental group (50.0 ± 16.3 vs 34.0 ± 13.4, F=31.6, P=0.0432). The amino acid mutation rate in S area of cased with genotype B and C in experimental group were 10.4%and 21.6%, respectively (F=24.31, P=0.046). The amino acid mutation rate in S area of cases of genotype C in experimental group was 41.6%, which significantly higher than those in control group (3.2%), with significant difference (F=85.68,P=0.006). Conclusions The serum HBV DNA positive rate in patients with HBsAg (+) and HBsAb (+) accompanied by HBeAg (+) increased signiifcantly. The phenomenon of both HBsAg and HBsAb positive genetic mutations and pre-S/S area has signiifcant relevance, while the mutation rate of genotype C higher than that of genotype B.

Abstract:Objective To discuss the relationship between the HBV genotype and clinical outcomes.Methods 410 cases of northern and western chinese population with HBV persistent infection were enrolled,whose HBV DNA levels were more than 104 copies/ml.HBV genome DNA was extracted by boiling method.The S gene was amplified by PCR and sequenced by ABI 3730.Results Genotype C in 377 cases corresponded to 91.95%,genotype B in 28 cases correspond-ed to 6.83%,mixed genotype of C and B only in one case corresponded to 0.24%,genotype D in four cases corresponded to 0.98%.Disease progression for genotype C and genotype B was studied through One-way ANOVA for 2 statuses,which were CHB vs.LC vs.HCC and LC vs.(CHB+HCC).There was statistical significance between CHB and LC or HCC.But it had no statistical significance between LC and HCC.Conclusions The HBV genotype C is the predominate infection in north chinese population and the genotype C infection may develop into more severe condition than genotype B.

Abstract:Objective To analyze the clinical and virological characteristics of hepatitis B patients with co-existence of HBsAg and anti-HBs antibodies in serum.Methods Of 13 080 patients with positive HBsAg from January 2008 to April 2010,476(3.64%) were positive for HBsAg and anti-HBs.The serum HBV markers,HBV DNA quantification,anti-HBc IgM and alanine aminotransferase(ALT) were simultaneously examined in the patients of research group(113 patients with complete data) and control group(79 age-and sex-matched HBV-infected patients with positive HBsAg and negative anti-HBs).Results There was no statistical difference of HBeAg-positive rates between the research and control group(P0.05).The HBsAg levels of patients in the research group were much lower than those of control group.The positive rate of the patients with anti-HBc IgM was 30.9% in the research group.The levels of HBsAg,HBV DNA and ALT in the patients with positive anti-HBc IgM were significantly higher than those in the patients with negative anti-HBc IgM.Conclusion The level of HBsAg in the research group was significantly lower than that in control group.There were significant differences of HBsAg levels between the patients with coexistence of HBsAg/anti-HBs and hepatitis B patients with only HBsAg-positive.The results suggested that anti-HBs antibody may play certain role for HBV infection.About 30% patients with coexistence of HBsAg/anti-HBs were in the state of acute flare of chronic hepatitis B.

Abstract:We aimed to determine the prevalence of the coexistence of HBsAg and anti-HBs and to analyze the clinical and virological features of infection, including amino acid (aa) patterns of the S gene and reverse transcriptase (RT) region in Chinese chronic hepatitis B (CHB) patients. Fifty-four (2.90%) CHB patients who were positive for both HBsAg and anti-HBs were tested, and sequences were obtained from 52 of them as well as 48 patients from a control group. S gene and RT region sequences were amplified and sequenced using in-house protocols. There was no significant difference between patients with and without anti-HBs with regard to age, gender, alanine aminotransferase level, and the proportion positive for HBeAg and HBcAb. The occurrence of genotype C (P = 0.001) and anti-HBeAb positivity (P = 0.027) was significantly higher in HBsAg+/anti-HBs+ individuals. In the S gene, the number of mutated residues in the HBsAg+/anti-HBs+ group was markedly higher than in control patients (1.88 versus 1.02 substitutions per 100 amino acids, P = 0.022). The amino acid exchange occurred mostly within the N-terminal region (2.15 versus 0.87 substitutions per 100 amino acids, P = 0.023) and the “a” determinant (3.61 versus 1.56 substitutions per 100 amino acids, P = 0.049) in the two groups. In the RT region, the mean number of substitution per 100 aa showed a tendency to be significantly higher in HBsAg+/anti-HBs+ patients than in controls (2.34 versus 1.46, P = 0.040). This study showed a prevalence of coexistence of anti-HBs in HBsAg-positive patients and an increased frequency of genotype C and aa variability within both HBsAg and RT involving functionally important regions of those proteins.

Additionally, point mutations within the pre S/S regions of HBsAg have frequently been reported to be associated with diagnostic failure, vaccine escape and immunotherapy escape. However, the prevalence of escape mutations with low levels of HBsAg (<100 IU/ml) in inactive HBsAg carriers has not been systematically studied within the past decade. Methods: This study aims to determine the trend of the biological prevalence of escape mutations within the pre S/S regions of special model of inactive CHB infection. Increased diversity over the complete HBV genome at baseline was associated with evolutionary characteristics. We used Sanger sequencing, quantitative HBV serology (HBeAg and HBsAg) and liver function index (aspartate alanine aminotransferase, ALT; alanine aminotransferase, AST) to identify whether HBV genome sequences are associated with the serological response to long-term host immunity in special inactive CHB infection.Results: Compared to HBV in HCC, HBV sequencing analysis of 28 LR/RA CHB patients with genotypes B and C showed higher genetic diversity among four ORFs, but the two groups presented similar selective pressure. Seven positive selection sites in the pre-S1 region are potentially associated with immune evasion. These mutations in the pre-S/S region might be associated with the HCC phenotype of low HBsAg expression, with the P region possibly impacting high viral loads.Conclusion: According to the results of this study, LR/RA CHB is characterized by not only genetic diversity but also positive evolutionary pressure within the pre-S/S regions. Increased viral diversity across the HBV genome is also associated with low levels of HBsAg. In conclusion, the cumulative evolutionary changes in the HBV pre-S/S regions that facilitate immune evasion should be monitored individually. Due to the similarity of evolutionary characteristics in HCC, low serological responses and high viremia may be associated with the risk of HCC.
