Abstract:BackgroundDynamic contrast-enhanced MRI (DCE-MRI) estimates vascular permeability of brain tumors, and susceptibility-weighted imaging (SWI) may demonstrate tumor vascularity by intratumoral susceptibility signals (ITSS). This study assessed volume transfer constant (Ktrans) accuracy, the volume of extravascular extracellular space (EES) per unit volume of tissue (Ve) derived from DCE-MRI, and the degree of ITSS in glioma grading.MethodsThirty-two patients with different glioma grades were enrolled in this retrospective study. Patients underwent DCE-MRI and non-contrast enhanced SWI by three-tesla scanning. Ktrans values, Ve, and the degree of ITSS in glioma were compared. Receiver operating characteristic (ROC) curve analysis determined diagnostic performances of Ktrans and Ve in glioma grading, and Spearman’s correlation analysis determined the associations between Ktrans, Ve, ITSS, and tumor grade.ResultsKtrans and Ve values were significantly different between low grade gliomas (LGGs) and both high grade gliomas (HGGs) and grade II, III and IV gliomas (P < 0.01). The degree of ITSS of LGGs was lower than HGGs (P < 0.01), and the ITSS of grade II gliomas was lower than grade III or IV gliomas. Ktrans and Ve were correlated with glioma grade (P < 0.01), while ITSS was moderately correlated (P < 0.01). Ktrans values were moderately correlated with ITSS in the same segments (P < 0.01).ConclusionKtrans and Ve values, and ITSS helped distinguish the differences between LGGs and HGGs and between grade II, III and IV gliomas. There was a moderate correlation between Ktrans and ITSS in the same tumor segments.

Quantitative Evaluation of Ki-67 Labeling Index in Glioma with Transfer Constant Derived from Dynamic Contrast-Enhanced MRI

Abstract:Objective To study the value of transfer constant(Ktrans)derived from dynamic contrast-enhanced MRI (DCE-MRI) for quantitative evaluation of Ki-67 labeling index (Ki-67 LI) in glioma. Methods Twenty patients with glioma who underwent DCE-MRI and operation were retrospectively reviewed. The Ktrans value and Ki-67 LI were acquired and correlated using the Spearman correlation test. Also, the Ktrans values were compared between high(larger than 10%)and low(no more than 10%)Ki-67 LI group with Mann-Whitney U test, receiver operating characteristic curves was performed to evaluate the diagnostic value. Results The Ktrans value(0.0165 to 0.8048, median 0.1252)was significantly associated with Ki-67 LI(5%to 50%, median 20%) (r=0.721,P<0.001), and the Ktrans value was significantly higher in high Ki-67 group(0.0810 to 0.8048, median 0.1810)than that in low Ki-67 LI group(0.0165 to 0.1456, median 0.0697)(Z=-3.209, P=0.001). The most predictive Ktrans value differentiated high Ki-67 LI and low Ki-67 LI with an area under the curve(AUC) of 0.945 at a sensitivity of 92.3% and specificity of 85.7%. Conclusion Ktrans value could be used for quantitative evaluation of Ki-67 LI in glioma.

Abstract:BACKGROUND:The degree of pathological microvascular proliferation is an important element in evaluation of the astrocytoma grade. Analysis of Semi Quantitative Susceptibility Weighted Imaging and Dynamic Enhanced Perfusion in Gliomas Grading:Comparative Study of Pathological Results

Abstract:Objective To evaluate the value of high-field susceptibility weighted imaging(SWI) in glioma grading before surgery and the correlationship between intratumoral susceptibility signal intensity(ITSS) and maximum relative cerebral blood volume(rCBVmax).Materials and Methods Thirty two patients with glioma received both SWI and DSC examination at 3T MR system.The patients were divided into 3 groups according to the histopathologic grading.The intratumoral susceptibility signal intensity(ITSS) was graded as 0-3 for semiquantitative analysis.Spearman correlation analysis were used to determine the correlation between ITSS,rCBVmax,and histopathologic grading.Receiver operating characteristic(ROC) curve analysis were performed to determine the diagnostic accuracy for glioma grading.Results On SWI,the conglomerated mixed with fine linear and dot like ITSSs were demonstrated in all gliomas.These ITSSs were usually located in the inner portion of tumors with rim enhancement showed on contrast T1 weighted images.Significant difference of ITSSs was found between different pathologic grading of gliomas(P0.0001,Wilcoxon signed-rank test).There is significant correlation between ITSS and histopathologic grading(r=0.961,P0.0001).Great difference of rCBVmax was found between low grade glioma and high grade glioma.Significant correlation was found between ITSS and rCBVmax(r=0.846,P0.0001),but densely prominent ITSSs did not correspond with regions of visual rCBVmax.The sensitivity,specificity,positive predictive value,and negative predictive value of ITSS in the glioma grading were 94.7%,84.63%,90%,and 91.7% respectively.Conclusion High field SWI is useful in the preoperative assessment of the histopathologic grading of glioma,and ITSS shows significant correlation with the rCBVmax in the same tumor section.
Correlation of the Indices of Susceptibility Weighted Imaging and Perfusion Imaging with the Expression of Microvessel Density and Vascular Endothelial Growth Factor in Astrocytic Tumor

Abstract:Objective To detect the correlation of the expression of microvessel density(MVD) and vascular endothelial growth factor (VEGF) with the semi-quantivative indices of susceptibility weighted imaging(SWI) and perfusion imaging (PI)in astrocytic tumor.Methods SWI and PI were performed in 98 patients with varing grades of astrocytic tumors.According to the World Health Organization (WHO) classification of central nervous system tumors and grading criteria:8 cases of pilocytic astrocytoma (grade Ⅰ,1 case of pleomorphic xanthoastrocytoma (grade Ⅱ),23 cases of astrocytoma (grade Ⅱ),22 cases of anaplastic astrocytoma (grade Ⅲ) and 44 cases of glioblastoma (grade Ⅳ) were included.Intra-tumor susceptibility hypo intensity area (ITSHIA) acquired by SWI was observed and semi-quantitative data were calculated.Maximum relative rCBV values of solid part of the tumor (rCBVintra),surrounding area of tumor (rCBVperi) were calculated.The MVD and VEGF expression were quantified from the excised tumor tissues and were correlated with PI and SWI indices.Kruskal-Wallis H test was used to assess the difference of the MVD and VEGF in different grade astrocytic tumor.Results The MVD count (r =0.550,P ＜ 0.01) and VEGF expression(r =0.456,P ＜ 0.01) were positively correlated with pathologic grading of astrocytic tumor.Spearman correlation analysis indicated that semi-quantivative indictors of SWI (r =0.340 to 0.497,P＜0.01),as well as rCBVintra(r =0.467,P ＜0.01) and rrCBVperi(r =0.374,P ＜0.01)of PI were positively correlated with MVD.All indices of SWI (r =0.202 to 0.334,P ＜ 0.01),except for "ITSHIA frequency" and "ITSHIA area ratio score",were correlated with VEGF expression,while the rCBVintra (r =0.301,P ＜ 0.01) and rCBVperi (r =0.311,P ＜ 0.01) of PI were positively correlated with VEGF.Conclusion The indices of PI and SWI are obviously correlated with MVD and VEGF in astrocytic tumor.PI and SWI could be effective in evaluating angiogenesis preoperatively.
Quantitative measurement of blood-brain barrier permeability with DSC-MRI and its primary application in diagnosis of gliomas

Abstract:Objective:To introduce the principles and methods of quantitative measurement of blood-brain barrier permeability with modified Tofts-Kermode model based on DSC-MRI and application in the differentiation of high-grade and low-grade gliomas.Methods:Bloodbrain barrier permeability of both brain lesions and normal tissues from 12 patients with different grades of gliomas were analyzed quantitatively.Parameters(Ktrans,Vp,Kep) were calculated,analyzed statistically and parameter images were reconstructed.Results:Ktrans of high-grade gliomas was larger than that of low-grade gliomas(P=0.031),while no significant difference was found between Vp and Kep of gliomas with different grade(P=0.126,P=0.756).Ktrans was a valuable indicator for the differentiation of high-grade and low-grade gliomas.A sensitivity of 100% and specificity of 58.3% were acquired when Ktrans=0.0274.Ktrans and Kep of gliomas were significantly larger than those of normal brain tissues(P<0.05).Conclusion:Blood-brain barrier permeability can be analyzed quantitatively with DSC-MRI,which represents potential values in diagnosis of gliomas.
Quantitative in Vivo Imaging of Tissue Factor Expression in Glioma Using Dynamic Contrast-Enhanced MRI Derived Parameters.

Abstract:OBJECTIVE:Tissue Factor (TF) has been well established in angiogenesis, invasion, metastasis, and prognosis in glioma. A noninvasive assessment of TF expression status in glioma is therefore of obvious clinical relevance. Dynamic contrast-enhanced (DCE) MRI parameters have been used to evaluate microvascular characteristics and predict molecular expression status in tumors. Our aim is to investigate whether quantitative DCE-MRI parameters could assess TF expression in glioma.MATERIALS AND METHODS:Thirty-two patients with histopathologically diagnosed supratentorial glioma who underwent DCE-MRI were retrospectively recruited. Extended Tofts linear model was used for DCE-MRI post-processing. Hot-spot, whole tumor cross-sectional approaches, and histogram were used for analysis of model based parameters. Four serial paraffin sections of each case were stained with TF, CD105, CD34 and α-Sooth Muscle Actin, respectively for evaluating the association of TF and microvascular properties. Pearson correlation was performed between percentage of TF expression area and DCE-MRI parameters, multiple microvascular indexes.RESULTS:Volume transfer constant (Ktrans) hot-spot value best correlated with TF (r=0.886, p<0.001), followed by 90th percentile Ktrans value (r=0.801, p<0.001). Moreover, histogram analysis of Ktrans value demonstrated that weak TF expression was associated with less heterogeneous and positively skewed distribution. Finally, pathology analysis revealed TF was associated with glioma grade and significantly correlated with these two dynamic angiogenic indexes which could be used to explain the strong correlation between Ktrans and TF expression.CONCLUSION:Our results indicate that Ktrans may serve as a potential clinical imaging biomarker to predict TF expression status preoperatively in gliomas.

Abstract:This study was designed to quantitatively analyse neovascular permeability in glioma by dynamic contrast-enhanced MRI (DCE–MRI). Forty-four patients with glioma were included in this study. The highest value of volume transfer constant (Ktrans) and volume of extravascular extracellular space per unit volume of tissue (Ve) were obtained and the differences in Ktrans and Ve between low-grade glioma (LGG) and high-grade glioma (HGG) were analyzed. The correlations between Ktrans, Ve and glioma grade were performed. Receiver operating characteristic (ROC) curve analyses were conducted. The values of Ktrans and Ve of LGG were significantly lower than those of HGG. The correlation analysis demonstrated statistically significant relationships between Ktrans and glioma grade, between Ve and glioma grade, and between Ktrans and Ve. The ROC curve analyses of Ktrans (0.035/min) and Ve (0.130) for differentiating LGG from HGG were statistically significant. Thus, DCE–MRI can be used to estimate neovascular permeability and for pre-operative grading of glioma.
Correlation Between the Semiquantitative Evaluations of Susceptibility-Weighted Imaging and Dynamic Susceptibility- Weighted Contrast-Enhanced Perfusion-Weighted Magnetic Resonance Imaging in Patients with Astrocytic Tumor

Abstract:Objective To evaluate the correlations between the indexes of susceptibility-weighted imaging( SWI) and those of perfusion-weighted magnetic resonance imaging( PI) in astrocytic tumors before the operation. Methods Ninety-eight patients were performed conventional, contrast MR scan, SWI and PI scan by Siemens 3. 0T magnetic resonance imaging system. Intratumor susceptibility hypo-intensity area ( ITSHIA) in SWI was observed and semiquantitative data were acquired. Maximum relative rCBV values of solid part within the tumor ( rrCBVintramax ) and surrounding area of tumor ( rrCBVperimax ) acquired by PI were calculated. Comparison of the rrCBVintramax and rrCBVperimax in astrocytic tumor of different grade were conducted. The corresponding situation between hot spot of PI and ITSHIA were evaluated and correlation between SWI and PI were compared. Results rrCBVintramax(rs =0. 662,P<0. 01) of astrocytic tumors was related positively linearly with pathological grade. rrCBVperimax(rs =0. 794,P <0. 01) was also markedly related. The rrCBVintramax of pilocytic astrocytoma was higher than that of astrocytoma(Ⅱ) ,which was similar to that of gradeⅢ, whereas rrCBVperimax showed no differences compared with astrocytoma(Ⅱ) and markedly lower than the tumor of high grade. The semiquantitative data of ITSHIA had a typical positive correlation with rrCBVintramax and rrCBVperimax respectively. The area including hot spots in PI was not to be able completely correspondent with ITSHIA. Conclusions Semiquantitative indexes of SWI are closely related to that of PI. Both PI and SWI may be used as sensitive indexes of evaluating pathological grade of astrocytic tumor. The incompletely correspondent between hot spot and ITSHIA may be due to an association with different machismos of PI and SWI.
Susceptibility-weighted imaging in grading brain astrocytomas

Abstract:OBJECTIVE: To evaluate the value of intratumoral vessels and micro-hemorrhage shown in susceptibility weighted imaging (SWI) for grading brain astrocytomas and to analyze the difference between SWI and conventional imaging techniques.METHODS: 22 patients with astrocytomas were diagnosed with surgical specimens, 9 of which were grades I-II, and 13 were grades III-IV. All examinations were performed on Signa DEx 3.0 T MRI scanner. Conventional imaging techniques (T1WI, T2WI, T2FLAIR, CE-T1WI) and SWI sequence were used. The parameters of SWI sequence were the following: TR=35 ms, TE=20 ms, FA=15 degrees, slice thickness=2 mm. The small vessels and blood products of the tumors in SW images were analyzed. The differences between the two groups in SW images were analyzed statistically.RESULTS: The findings in SW images of brain astrocytomas were correlated strongly with pathology. SWI was more sensitive compared to conventional imaging techniques for showing small vessels and micro-hemorrhage in brain astrocytomas. Statistical comparison showed that the small vessels and micro-hemorrhage of two groups of brain astrocytomas in SW images differed significantly.CONCLUSION: SWI is superior to conventional imaging techniques at showing the small vessels and micro-hemorrhage in brain astrocytomas, which plays an important role in the tumor grading.

Abstract:OBJECTIVE: Permeability parameters from dynamic contrast-enhanced MRI (DCE-MRI) and apparent diffusion coefficient (ADC) value on diffusion-weighted imaging (DWI) can be quantitative physiologic metrics for gliomas. The transfer constant (K(trans)) has shown efficacy in grading gliomas. Volume fraction of extravascular extracellular space (ve) has been underutilized to grade gliomas. The purpose of this study was to evaluate ve in its ability to grade gliomas and to assess the correlation with other permeability parameters and ADC values.MATERIALS AND METHODS: A total of 33 patients diagnosed with pathologically-confirmed gliomas were examined by 3 T MRI including DCE-MRI and ADC map. A region of interest analyses for permeability parameters from DCE-MRI and ADC were performed on the enhancing solid portion of the tumors. Permeability parameters form DCE-MRI and ADC between low- and high-grade gliomas; the diagnostic performances of presumptive metrics and correlation among those metrics were statistically analyzed.RESULTS: High-grade gliomas showed higher K(trans) (0.050 vs. 0.010 in median value, p = 0.002) and higher ve (0.170 vs. 0.015 in median value, p = 0.001) than low-grade gliomas. Receiver operating characteristic curve analysis showed significance in both K(trans) and ve for glioma grading. However, there was no significant difference in diagnostic performance between K(trans) and ve. ADC value did not correlate with any of the permeability parameters from DCE-MRI.CONCLUSION: Extravascular extracellular space (ve) appears to be comparable with transfer constant (K(trans)) in differentiating high-grade gliomas from low-grade gliomas. ADC value does not show correlation with any permeability parameters from DCE-MRI.

Abstract:Objective To evaluation of diffusion heterogeneity and vascular perfusion in tumor and peritumoral areas for prognostic prediction in high-grade glioma (HGG, WHO III/IV grade). Methods Forty patients with HGG underwent diffusion kurtosis imaging (DKI), intravoxel incoherent motion (IVIM), and arterial spin labeling (ASL) MRI before operation. After normalization, the parameters were divided into diffusion heterogeneity parameters (rD, rMD, rMK, rKr, rKa) and vascular perfusion parameters (rD*, rF, rCBF). Univariate and multivariate Cox regression analyses were used to evaluate associations between overall survival (OS) and the above parameters, clinical factors, and IDH1 status. The Mann-Whitney test was used to evaluate differences in the parameters between different IDH1 states. Results In the univariate Cox regression analysis, OS was significantly associated with tumor resection range, IDH1 status, tumor heterogeneity parameters (rD, rMD, rMK, rKr, rKa), and rCBF in tumor area(all p < 0.05). In addition, rD and rCBF measured in the peritumoral region were also predictors of poor OS (both p < 0.01). Multivariate Cox regression analysis indicated that rMK in the tumor area and rCBF in the peritumoral area (hazard ratio = 7.900 and 5.466, respectively, for each 0.1 increase in the normalized value) were independent predictors of OS. Conclusion The rMK of tumor area and rCBF of peritumoral area had independent predictive value for OS in patients with HGG. Advances in knowledge This study explored useful imaging biomarkers from the diffusion heterogeneity and vascular perfusion of tumor and peritumoral areas in HGG, which is useful to help clinician to make precise therapeutic plans, and predict the prognostic for glioma patients.

Abstract:Objective To evaluate three different tracer-kinetic models used for dynamic contrast-enhanced MRI (DCE-MRI) data processing in the prediction of the histopathologic grade of cerebral gliomas.Methods Forty-one patients with histopathologically graded gliomas (grade Ⅱ 13,grade Ⅲ 14,grade Ⅳ 14)were imaged with DCE-MRI from March,2013 to November,2014.The values of Ktrans of gliomas were obtained by three tracer-kinetic models,which were Patlak model,Tofts model and Extended Tofts Linear model.All data were analyzed statistically by a Graphpad 6.0 statistical software.Comparison of the differences of Ktrans among different grades of gliomas was conducted using the Kruskal-Wallis test and Dunn's multiple comparisons test for the data not conform to normal distribution.Correlations of Ktrans values among those three models were analyzed using linear regression analysis,The differences of Ktrans between low grades and high grades of gliomas was conducted using the Mann-Whitney U test.Receiver operating characteristic (ROC) curve analyses were performed to determine the cut-off values for Ktrans to distinguish different low grades and high grades of gliomas.Results The Ktrans values obtained by Patlak model was 0.008(0.004-0.043) min-1for grade Ⅱ,0.021(0.005-0.088) min-1 for grade Ⅲ,and 0.035(0.017-0.061) min-1 for grade Ⅳ.The Ktrans values obtained by Tofts model was 0.085 (0.041-0.158)min 1for grade Ⅱ,0.140 (0.063-0.315) min-1for grade Ⅲ,0.229 (0.126-0.419)min 1 for grade Ⅳ.The Ktrans values obtained by Extended Tofts Linear model was 0.012 (0.004-0.092) min 1 for grade Ⅱ,0.048 (0.010-0.188) min-t for grade Ⅲ,0.094 (0.036-0.215)min 1 for grade Ⅳ.All the Ktrans values obtained by three models increased when the histological grades increased,with statistical significance between grade Ⅱ and Ⅲ (H=18.31,18.09,20.18,P＜0.05).Ktrans values among the three models had good linear correlations.The Ktrans obtained by Extended Tofts Linear model had good linear correlations with both Patlak model and Tofts model (r=0.933,0.893,P＜0.05),and the Ktrans obtained by Patlak model had less linear correlation with Tofts model (r=0.822,P＜0.05).The K values were statistically different between LGG and HGG (P＜0.01).The cut-off value of K provided good combination of sensitivity and specificity in the differentiation between LGG and HGG,and the K obtained by Extended Tofts Linear model had the best sensitivity and specificity among the three models (sensitivity 92.3％,specificity 85.7％).Conclusions K from DCE-MRI has a high performance in predicting the histopathologic grade of brain glioma,and the Extended Tofts Linear model is more suitable for the evaluation of cerebral glioma.
Quantitative Analysis of Permeability for Glioma Grading Using Dynamic Contrast-Enhanced Magnetic Resonance Imaging

Abstract:The objective of the present study was to quantitatively analyze the permeability of tumor entity and peritumor edema in glioma grading, using dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). In the present retrospective study, 80 patients underwent T1-weighted DCE-MRI examination at 3.0 T and the pathological results (including astrocytoma and oligodendroglioma) were obtained between January 2012 and June 2015. All cases were surgically validated as grade I-IV gliomas. The original DCE-MRI data were analyzed using dual compartment modified Tofts model. The forward volume transfer constant (Ktrans), backflux rate (kep) and fractional volume (ve) were calculated with the region of interest selected on the highest permeability area of the tumor entity and peritumor edema. Analysis of variance with the Bonferroni correction was used to compare the values of Ktrans, kep, and ve of the tumor entity and peritumor edema in different glioma grades. The results of the present study revealed that the Ktrans, kep, and ve values in each stage were associated with the pathological grading (r=0.951, 0.804 and 0.766, respectively). There were significant differences identified between different tumor grades in Ktrans, kep, with the exception being between grades II and III in kep. In addition, there was a significant difference revealed between grade I/II and grade III/IV in ve. Receiver operator characteristics curve analysis was used to evaluate the diagnosis accuracies of permeability parameters. Ktrans was demonstrated to exhibit the highest sensitivity and specificity for evaluating the tumor grade. With the threshold values of 0.160, 0.420 and 0.935 in Ktrans on tumor, glioma grades I vs. II, II vs III and III vs. IV may be differentiated with sensitivities of 0.900, 0.950 and 0.950, and specificities of 0.950, 0.950 and 0.850, respectively. Furthermore, associations were observed between the Ktrans, kep and ve values of peritumor edema and the pathological grading in glioma (Ktrans r=0.438, P<0.001; Kep r=0.385, P<0.001; Ve r=0.397, P<0.001, respectively). Ktrans values in peritumoral edema revealed significant differences between low-grade and high-grade glioma. The sensitivity and specificity for Ktrans of peritumor edema were 0.975 and 0.950, with a threshold value of 0.007. Therefore, the DCE-MRI parameters of Ktrans of tumor entity and peritumor edema in gliomas may be used to accurately differentiate glioma grades.

Abstract:Purpose To evaluate two advanced diffusion models, diffusion kurtosis imaging and the newly proposed mean apparent propagation factor-magnetic resonance imaging, in the grading of gliomas and the assessing of their proliferative activity. Methods Fifty-nine patients with clinically diagnosed and pathologically proven gliomas were enrolled in this retrospective study. All patients underwent DKI and MAP-MRI scans. Manually outline the ROI of the tumour parenchyma. After delineation, the imaging parameters were extracted using only the data from within the ROI including mean diffusion kurtosis (MK), return-to-origin probability (RTOP), Q-space inverse variance (QIV) and non-Gaussian index (NG), and the differences in each parameter in the classification of glioma were compared. Receiver operating characteristic (ROC) curve analysis was used to evaluate the diagnostic performance of these parameters. Results MK, NG, RTOP and QIV were significantly different amongst the different grades of glioma. MK, NG and RTOP had excellent diagnostic value in differentiating high-grade from low-grade glioma, with largest areas under the curve (AUCs; 0.929, 0.933 and 0.819, respectively; P < 0.01). MK and NG had the largest AUCs (0.912 and 0.904) when differentiating grade II tumours from III tumours (P < 0.01) and large AUCs (0.791 and 0.786) when differentiating grade III from grade IV tumours. Correlation analysis of tumour proliferation activity showed that MK, NG and QIV were strongly correlated with the Ki-67 LI (P < 0.001). Conclusion MK, RTOP and NG can effectively represent the microstructure of these altered tumours. Multimodal diffusion-weighted imaging is valuable for the preoperative evaluation of glioma grade and tumour proliferative activity.

Abstract:Purpose: To evaluate the roles of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and optimum tracer kinetic parameters in the noninvasive grading of the glial brain tumors with histopathological grades (IIV). Materials and Methods: Twenty-eight patients with histopathologically graded gliomas were imaged. Images with five flip angles were acquired before injection of gadolinium-DTPA and were processed to calculate the T1 value of each region of interest (ROI). All the DCE-MRI data acquired during the injection were processed based on the MRI signal and pharmacokinetic models to establish concentrationtime curves in the ROIs drawn within the tumors, contralateral normal areas, and area of the individual artery input functions (iAIF) of each patient. A nonlinear least-square-fitting method was used to obtain tracer kinetic parameters. KruskalWallis H-test and MannWhitney U-test were applied to these parameters in different histopathological grade groups for statistical differences (P < 0.05). Results: Volume transfer coefficient (Ktrans) and extravascular extracellular space volume fraction (Ve) calculated using iAIFs can be used not only to distinguish the low (ie, I and II) from the high (ie, III and IV) grade gliomas (P(?Ktrans) < 0.001 and PVe < 0.001), but also grade II from III (P(?Ktrans) = 0.016 and PVe = 0.033). Conclusion: Ktrans is the most sensitive and specific parameter in noninvasive grading, distinguishing the high (III and IV) from the low (I and II) grade and high grade III from low grade II gliomas. Abstract:Background The application of conventional magnetic resonance imaging (MRI) in glioma grading is limited and non-specific. Purpose To investigate the application values of MRI, texture analysis (TA) of dynamic contrast-enhanced MRI (DCE-MRI) and intratumoral susceptibility signal (ITSS) on susceptibility weighted imaging (SWI), alone and in combination, for glioma grading. Material and Methods Fifty-two patients with pathologically confirmed gliomas who underwent DCE-MRI and SWI were enrolled in this retrospective study. Conventional MRIs were evaluated by the VASARI scoring system. TA of DCE-MRI-derived parameters and the degree of ITSS were compared between low-grade gliomas (LGGs) and high-grade gliomas (HGGs). The diagnostic ability of each parameter and their combination for glioma grading were analyzed. Results Significant statistical differences in VASARI features were observed between LGGs and HGGs ( P < 0.05), of which the enhancement quality had the highest area under the curve (AUC) (0.873) with 93.3% sensitivity and 80% specificity. The TA of DCE-MRI derived parameters were significantly different between LGGs and HGGs ( P < 0.05), of which the uniformity of K trans had the highest AUC (0.917) with 93.3% sensitivity and 90% specificity. The degree of ITSS was significantly different between LGGs and HGGs ( P < 0.001). The AUC of the ITSS was 0.925 with 93.3% sensitivity and 90% specificity. The best discriminative power was obtained from a combination of enhancement quality, K trans - uniformity, and ITSS, resulting in 96.7% sensitivity, 100.0% specificity, and AUC of 0.993. Conclusion Combining conventional MRI, TA of DCE-MRI, and ITSS on SWI may help to improve the differentiation between LGGs and HGGs.

Susceptibility-Weighted Imaging of Glioma: Update on Current Imaging Status and Future Directions

Abstract:Susceptibility-weighted imaging (SWI) provides invaluable insight into glioma pathophysiology and internal tumoral architecture. The physical contribution of intratumoral susceptibility signal (ITSS) may correspond to intralesional hemorrhage, calcification, or tumoral neovascularity. In this review, we present emerging evidence of ITSS for assessment of intratumoral calcification, grading of glioma, and factors influencing the pattern of ITSS in glioblastoma. SWI phase imaging assists in identification of intratumoral calcification that aids in narrowing the differential diagnosis. Development of intratumoral calcification posttreatment of glioma serves as an imaging marker of positive therapy response. Grading of tumors with ITSS using information attributed to microhemorrhage and neovascularity in SWI correlates with MR perfusion parameters and histologic grading of glioma and enriches preoperative prognosis. Quantitative susceptibility mapping may provide a means to discriminate subtle calcifications and hemorrhage in tumor imaging. Recent data suggest ITSS patterns in glioblastoma vary depending on tumoral volume and sublocation and correlate with degree of intratumoral necrosis and neovascularity. Increasingly, there is a recognized role of obtaining contrast-enhanced SWI (CE-SWI) for assessment of tumoral margin in high-grade glioma. Significant higher concentration of gadolinium accumulates at the border of the tumoral invasion zone as seen on the SWI sequence; this results from contrast-induced phase shift that clearly delineates the tumor margin. Lastly, absence of ITSS may aid in differentiation between high-grade glioma and primary CNS lymphoma, which typically shows absence of ITSS. We conclude that SWI and CE-SWI are indispensable tools for diagnosis, preoperative grading, posttherapy surveillance, and assessment of glioma.
Values of Tumor Vein Detected by Susceptibility Weighted Imaging in Evaluation of Gliomas Grade

Abstract:Objective To study the value of tumor vein detected by susceptibility weighted imaging (SWI) in evaluation of glioma grade.Methods Thirty-nine cases of gliomas were confirmed pathologically,including 18 cases of low-grade gliomas (WHO Ⅰ-Ⅱ grade) and 21 cases of high-grade gliomas (WHO Ⅲ-Ⅳ grade).All cases were underwent routine MR series and SWI scanning,and SWI images were obtained with minimum intensity projection on GE ADW 4.2 work-station.SWI images were observed,and veins within glioma were scored to analyze its statistical difference,and the correlation between tumor vein scores and glioma grade was also analyzed.Results On SWI,tumor vein detection rate was 28％ in low-grade gliomas group,and 100％ in high-grade gliomas group,the difference was statistically significant (x2=22.750,P＜0.01); Among the 18 cases of low-grade gliomas,the tumor vein score was 0 point in 13 cases(72％) and 1 point in 5 cases(28％),while it was 1 piont in 4 cases(19％) and 2 point in 17 cases (81％) among the 21 cases of high-grade gliomas.There was significant difference between two gliomas(Z=-5.411,P＜0.01); The tumor vein score was positively correlated with the pathological grade (r=0.878,P＜0.01).Conclusion SWI had remarkable auxiliary benefit in grading ofglioma by observing intratumoral veins.
