Abstract:Gastrointestinal cancer (GIC) is a group of malignant tumors, including esophagus, stomach, liver, pancreas, as well as colorectal region. GIC accounts for more than 27% of the total newly diagnosed tumors worldwide, and 37% of tumor-related death events among all types of tumors. Totally, about 4.8 million new cases are yearly diagnosed and 3.4 million people lost their lives due to those malignancies every year (1). Moreover, the amount of either new or death cases are predicted to increase by 58% and 73% to 7.5 million and 5.6 million by 2040 due to the changes in the age composition and growth of the world population (2). In fact, the current poor prognosis and high mortality rate of GIC is mainly due to a high risk of metastasis, with its occurrence accompanied by lymphatic metastases and distant metastases (3–5). This serious pending burden forces us to reconsider how we deal with this challenge. In purpose of averting many future diagnoses and deaths, it is necessary to dedicate more work to the fundamental studies of these diseases, seeking reliable biomarkers benefiting to early diagnosis and preventative actions, and the development of innovative treatment strategies (6–9). The Research Topic of this special issue takes an in-depth look at novel molecular mechanisms for gastrointestinal tumorigenesis and metastasis, as well as report the latest progress related to new biomarkers, therapeutic targets and development of new treatment strategies which will contribute to improved therapeutic efficacy and prognosis for GIC patients. A collection of original articles, systematic reviews, case reports and perspectives will provide readers with news on exciting breakthroughs in research into GICs including gastric cancer (GC), hepatocellular carcinoma (HCC), Cholangiocarcinoma (CCA), pancreatic ductal adenocarcinoma (PDAC), and colorectal cancer (CRC). The majority of early stage of GIC metastases cannot be detected by computed tomography (CT), magnetic resonance imaging (MRI) or other traditionally morphological imaging approaches since the lesions are too small or they have not yet formed cancer nodules (10). However, there is still a rather limited number of useful biomarkers available that represents the risk of misdiagnosis leading to an ineffective therapeutic strategy and substantial problem for the management of GIC. Next generation sequencing (NGS) technology would appear to be a promising tool for GIC diagnosis. Abstract:Gastric cancer (GC) is one of the most common malignancies worldwide. Most patients are diagnosed at advanced stages due to the subtle symptoms of earlier disease and the low rate of regular screening. Systemic therapies for GC, including chemotherapy, targeted therapy and immunotherapy, have evolved significantly in the past few years. For resectable GC, perioperative chemotherapy has become the standard treatment. Ongoing investigations are exploring the potential benefits of targeted therapy or immunotherapy in the perioperative or adjuvant setting. For metastatic disease, there have been notable advancements in immunotherapy and biomarker-directed therapies recently. Classification based on molecular biomarkers, such as programmed cell death ligand 1 (PD-L1), microsatellite instability (MSI), and human epidermal growth factor receptor 2 (HER2), provides an opportunity to differentiate patients who may benefit from immunotherapy or targeted therapy. Molecular diagnostic techniques have facilitated the characterization of GC genetic profiles and the identification of new potential molecular targets. This review systematically summarizes the main research progress in systemic treatment for GC, discusses current individualized strategies and presents future perspectives.

New agents on the horizon in gastric cancer

Abstract:Background: Conventional cytotoxic chemotherapy has been the backbone of advanced gastric cancer treatment for decades and still represents a key element of the therapeutic armamentarium. However, only small increments in survival outcomes have been reached. A better understanding of genetic alterations and molecular signatures of gastric cancer has been reached in the last years. It will serve as a roadmap for better treatment stratification and future drug development. Materials and methods: We reviewed preclinical and clinical studies that assessed novel treatment targets and emerging drug therapies in gastric cancer. We performed research via PubMed, and the congress webpages of the American Society of Clinical Oncology, European Society of Medical Oncology and the Japanese Society of Medical Oncology. Results: HER2-targeting with trastuzumab is effective in HER2-positive metastatic gastric cancer; combined HER2 targeting strategies are being investigated. Studies assessing the role of HER2 targeting in the perioperative setting are ongoing. Novel treatment targets include inhibition of cancer stemness-related signaling pathways like STAT3. DNA damage repair and Claudin 18.2, a tight junction protein with high expression in gastric cancers are also novel molecular drug targets. Modification of the tumor microenvironment, including activation of immune response by PD-1/PD-L1 checkpoint inhibitors and stroma modification by matrix metalloproteinase-9 inhibition, led to first promising treatment results. Conclusion: Novel treatment options for gastric cancer patients are emerging. They involve novel mechanisms of action, and are based on our constantly increasing understanding of tumor biology and better molecular stratification of gastric cancer patients.

Abstract:Digestive malignancies remain one of the leading causes of cancer-related death worldwide despite the fact that increasing clinical and biological knowledge has emerged. Among all the newly discovered cancer cases, colorectal cancer, stomach cancer, liver cancer, and esophagus cancer rank in the front and the mortality rate for them also tops the list. The poor prognosis of digestive tumors is partially due to late diagnosis, delayed initiation of treatment, and unsatisfactory reaction to cancer therapies. Many molecular markers have been discovered for early diagnosis and better therapeutic outcomes from which we do benefit a lot. However, diagnosis and treatment of alimentary cancers require further optimization. We believe further study of novel molecular targets for early detection and better treatment would be helpful to reduce mortality rate and to improve the prognosis of malignant diseases of digestive system.

Abstract:In this editorial, we provide commentary on six articles recently published in the World Journal of Gastrointestinal Oncology . These articles collectively present the latest findings in the field of gastric and colorectal cancer (CRC) research. The global incidence of gastric cancer varies based on geographical location, age, and sex. The disease predominantly affects middle-aged and elderly individuals, with a slightly higher prevalence in men than in women. CRC is characterized by a low 5-year survival rate and high mortality rate. It primarily affects individuals over the age of 50, and the risk of disease increases with age. Both gastric and CRC pose significant health threats, thus requiring more effective diagnostic, therapeutic, and supportive care strategies to improve patient outcomes. The articles discussed in this editorial encompass topics such as screening, diagnosis, mechanisms of progression, and postoperative recovery in gastric and CRC, and the findings offer valuable insights for clinical decision-making in the diagnosis, treatment, and prognosis of gastrointestinal cancers.

Abstract:Gastrointestinal cancer (GIC) is the most common cancer with a poor prognosis. Currently, surgery is the main treatment for GIC. However, the high rate of postoperative recurrence leads to a low five-year survival rate. In recent years, immunotherapy has received much attention. As the only immunotherapy drugs approved by the Food and Drug Administration (FDA), immune checkpoint blockade (ICB) drugs have great potential in cancer therapy. Nevertheless, the efficacy of ICB treatment is greatly limited by the low immunogenicity and immunosuppressive microenvironment of GIC. Therefore, the targets of immunotherapy have expanded from ICB to increasing tumor immunogenicity, increasing the recruitment and maturation of immune cells and reducing the proportion of inhibitory immune cells, such as M2-like macrophages, regulatory T cells and myeloid-derived suppressor cells. Moreover, with the development of nanotechnology, a variety of nanoparticles have been approved by the FDA for clinical therapy, so novel nanodrug delivery systems have become a research focus for anticancer therapy. In this review, we summarize recent advances in the application of immunotherapy-based nanoparticles in GICs, such as gastric cancer, hepatocellular carcinoma, colorectal cancer and pancreatic cancer, and described the existing challenges and future trends.

Abstract:Gastric cancer is a malignant tumor that ranks third in cancer-related deaths worldwide. Early-stage gastric cancer can often be effectively managed through surgical resection. However, the majority of cases are diagnosed in advanced stages, where outcomes with conventional radiotherapy and chemotherapy remain unsatisfactory. Immunotherapy offers a novel approach to treating molecularly heterogeneous gastric cancer by modifying the immunosuppressive tumor microenvironment. Immune checkpoint inhibitors and adoptive cell therapy are regarded as promising modalities in cancer immunotherapy. Food and Drug Administration-approved programmed death-receptor inhibitors, such as pembrolizumab, in combination with chemotherapy, have significantly extended overall survival in gastric cancer patients and is recommended as a first-line treatment. Despite challenges in solid tumor applications, adoptive cell therapy has demonstrated efficacy against various targets in gastric cancer treatment. Among these approaches, chimeric antigen receptor-T cell therapy research is the most widely explored and chimeric antigen receptor-T cell therapy targeting claudin18.2 has shown acceptable safety and robust anti-tumor capabilities. However, these advancements primarily remain in preclinical stages and further investigation should be made to promote their clinical application. This review summarizes the latest research on immune checkpoint inhibitors and adoptive cell therapy and their limitations, as well as the role of nanoparticles in enhancing immunotherapy.

Abstract:OBJECTIVE:To summarize the present literature about the therapy of advanced gastric cancer.METHODS:The periodic full-text databases of both Medline and CBM disc from January 1991 to September 2009 in the internet were used to retrieve chapters of the corresponding materials.The index words were composed of stomach neoplasms and drug therapy.A total of 1809 aricles were obtained from PubMed and 131 from CBMdisc.The adoptive standards were as the following:1)the chemotherapy of advanced gastric cancer.2)the targeted treatment of advanced gastric cancer.According to the criteria,81 papers were selected and finally 34 were included.RESULTS:The treatment of advanced gastric cancer is essentially palliative.Most gastric cancer responses to chemotherapy are partial and of short duration.There is no standard treatment.Several targeted drugs are currently undergoing clinical trials.CONCLUSION:The present study of the medical treatment of advanced gastric cancer focuses on the following aspects:1)to improve the reqimens of combination chemotherapy;2)to keep on evaluating the effective reqimens nowadays.3)to study the application of the new chemotherapeutics and targeted drugs.

Abstract:Gastric adenocarcinoma (GC) and gastroesophageal junction adenocarcinoma represent frequent and severe diseases whose management has radically changed over the last 10 years. With the advent of second- and third-line standard therapies for metastatic GC patients in the 2010s, the molecular dismemberment of the disease and positive trials with immunotherapy and targeted agents will mark the 2020s. New treatment options have emerged in the neoadjuvant, adjuvant, and metastatic setting. In addition to improved multimodal treatment in operable patients, new subgroups have emerged depending on molecular alterations (HER2, Microsatellite instability) or expression of specific proteins in the tumour (PDL1, Claudin 18.2) making immunohistochemistry central in profiling the tumour for an optimal individualised management. The aim of this review is to describe the current standards of management of early and late stage GC and the molecular markers needed today to optimally manage our patients together with future perspectives on this disease.

Abstract:Globally, in 2018, 4.8 million new patients have a diagnosis of gastrointestinal (GI) cancers, while 3.4 million people died of such disorders. GI malignancies are tightly relevant to 26% of the world-wide cancer incidence and occupies 35% of all cancer-associated deaths. In this article, we principally investigated molecular and cellular mechanisms of tumorigenesis in five major GI cancers occurring at esophagus, stomach, liver, pancreas, and colorectal region that illustrate high morbidity in Eastern and Western countries. Moreover, through this investigation, we not only emphasize importance of the tumor microenvironment in development and treatment of malignant tumors but also identify significance of M2PK, miRNAs, ctDNAs, circRNAs, and CTCs in early detection of GI cancers, as well as systematically evaluate contribution of personalized precision medicine including cellular immunotherapy, new antigen and vaccine therapy, and oncolytic virotherapy in treatment of GI cancers.

Abstract:Gastric cancer is one of the most common malignancies in the digestive system. Due to the low rate of diagnosis, most of the patients with gastric cancer have advanced disease at the time of diagnosis. Chemotherapy is still the mainstay of treatment for advanced gastric cancer, but efficacy of combination chemotherapy is modest. Novel treatment options are urgently needed to improve the outcome of patients with advanced gastric cancer. Molecular targeted therapies have emerged as a novel approach to the treatment of both hematological and solid tumors in recent years. The study of molecular biological mechanisms underlying the formation, progression and metastasis in advanced gastric cancer has enabled us to use the new approach to treat this disease in clinical practice. These therapeutic strategies include targeting EGFR signal transduction pathway, HER2 monoclonal antibody, anti-angiogenesis, multityrosine kinase inhibitors, cyclin dependent kinase (CDK) inhibitors c-Met inhibitors, PI3k/Akt/mTOR pathway inhibitors, Matrix metalloproteinase, IGF-1R inhibitors, and HSP 90 inhibitors. The aim of this review is to summarize the most recent publications on targeted therapies in advanced gastric cancer.

Abstract:Gastric cancer is one of the most common cause of cancer related deaths worldwide which results in malignant tumors in the digestive tract. The only radical treatment option available is surgical resection. Recently, the implementation of neoadjuvant chemotherapy resulted in 5-year survival rates of 95% for early gastric cancer. The main reason of treatment failure is that early diagnosis is minimal, with many patients presenting advanced stages. Hence, the greatest benefit of radical resection is missed. Consequently, the main therapeutic approach for advanced gastric cancer is combined surgery with neoadjuvant chemotherapy, targeted therapy, or immunotherapy. In this review, we will discuss the various treatment options for advanced gastric cancer. Clinical practice and clinical research is the most practical way of reaching new advents in terms of patients' characteristics, optimum drug choice, and better prognosis. With the recent advances in gastric cancer diagnosis, staging, treatment, and prognosis, we are evident that the improvement of survival in this patient population is just a matter of time.

Abstract:Chemotherapy-based comprehensive treatment is the main therapy for advanced gastric cancer (AGC). Although the signifcant progress has been made in the treatment of AGC, the standard first-line chemotherapy regimen for AGC remains controversy due to the low complete response rate of chemotherapy and the short duration of response after chemotherapy. In recent years, with the rapid development of tumor molecular biology, the understanding of various related molecular signaling pathways and the targeted molecules in the occurrence and development of gastric cancer has become clear, this has led to the development of new targeted molecular agents which targeting the critical aspects of oncogenic pathways.
