Abstract:Background: Repair of traumatic alar defect is challenging because poor blood supply is caused by contracture scars, which sometimes extend beyond the alar groove. However, few studies have investigated the reconstruction results of severe traumatic cases. This study aimed to examine the clinical outcomes of severe traumatic alar defect reconstruction using either pedicled nasolabial or forehead flaps combined with conchal cartilage.Methods: This retrospective study investigated the clinical characteristics and treatment effects of 17 patients with severe traumatic alar defects treated in a single plastic surgery center from March 1, 2015, to September 1, 2018. All cases were scored and graded with regard to the size and depth of the alar defect and the surrounding scar according to the Alar Defect Severity Score (ADSS). Surgical outcomes were evaluated on the basis of the severity of defect before repair, donor site distortion, and postoperative nasal symmetry, especially shape and color.Results: The average ADSS of the cases was 8.10.8 (highest score, 9.0). No flap necrosis or any complications were observed postoperatively. The symmetry of the bilateral alae was satisfactory. No color distinction between grafts and surrounding tissues, retraction, or inferior displacement of the ala was observed at an average follow-up of 24.2 +/- 10.4 months (range, 8-42 months). The average postoperative surgeon-based evaluation score was 4.3 +/- 0.2 (highest score, 5.0). Esthetic and functional results were satisfactory in all cases.Conclusions: A pedicled flap combined with conchal composite grafts should be considered for the treatment of severe traumatic alar defect. This is a reproducible technique that enables a predictably decent outcome for severe traumatic alar defect, especially in Asian patients.
Alar Flap Combined With Free Auricular Composite Flap For The Reconstruction Of Nasal Alar Defect

Abstract:Background: The nasal ala plays an important role in the aesthetic appearance of the nose. Repairing the nasal alar defect, especially full-thickness alar defect, is one of the difficulties of plastic surgeons. In this article, a new surgical method is introduced about repairing full-thickness alar defect with free auricular composite flaps and local nasal alar rotation and advancement tissue flaps.Materials and Methods: We retrospectively reviewed 6 patients with a diagnosis of full-thickness alar defect between 2010 and 2013. All of them accepted this new surgical method. The patients were followed up for 6 to 12 months. The method used local nasal alar rotation and advancement tissue flap to form new nasal rim and made the defect of nasal rim shift to alar groove region. After that, we harvested free auricular composite flap to repair the new defect.Result: All patients attained relatively symmetrical nostrils with a natural, smooth, integrated alar rim and inconspicuous scars. All the reconstructed nasal alae were aesthetically satisfactory. Slight atrophy of grafts was observed in all patients. The shape of the donor ear changed a little. Two patients experienced hyperpigmentation change and one patient presented slight hypertrophic scar in the suture site. No other complications were observed.Conclusions: This simple method not only reconstructed almost normal nasal alar rims and alar grooves but also improved the survival rate of the composite grafts without any free edges. This method also changed the traditional free auricular tissue flap location from the nasal alar rim to the upper alar groove region, which would help achieve more aesthetic appearance. This new method is a creative and useful technique for the repair of full-thickness alar defect.

Abstract:Background A basal cell carcinoma can invade the cartilage in the nasal ala and can reach the nasal vestibule skin. A full-thickness reconstruction of the nasal ala is required to repair the tissue damage after tumor removal. We combined an auricular composite graft with a nasolabial flap to repair the defects in the nasal ala. Methods Eleven patients with full-thickness infiltration of the nasal ala by basal cell carcinoma were included in the study. The outer defect area after tumor removal ranged from 1.5 x 2.0 to 2.0 x 2.5 cm(2). Skincartilage composite grafts 1.5 x 1.5 to 1.5 x 2.0 cm(2) in size were harvested from the helix at the top of the ear. The composite graft was used as a support scaffold to reconstruct the defects. A nasolabial flap was used to cover the cartilage. Results All cases were treated successfully. No flap necrosis, skin color changes, or blistering occurred. The shape of the flap was stable, and the skin color was consistent with that of the surrounding tissue. The reconstructed nasal ala was symmetrical. Conclusions Combining an auricular composite graft with a nasolabial flap can repair defects up to 2.0 x 2.5 cm(2) in size. The reconstructed nasal ala matches the original skin color and maintains the appropriate shape.
The Application of Nasolabial Fold Flap and Auricle Compound Tissue in Repairing Nasal Alar Defect

Abstract:Objective To explore the application of nasolabial fold flap and auricle compound tissue in repairing nasal alar defect. Methods From October 2011 to March 2016, 32 patients with nasal alar defect were treated with nasolabial fold flap graft and auricle compound tissue free graft. The defect state of nasal alar, repair methods, incision scar, symmetry and postoperative appearance were all observed and analyzed. Results Nineteen patients received the treatment of nasolabial fold flap graft and 13 patients received the treatment of auricle compound tissue free graft. Nasal alar defect was well-repaired, and the outcome appearance was satisfactory without obvious scars around wound surface and donor tissue's incision. Conclusion Both nasolabial fold flap graft and auricle compound tissue free graft are favorable treatments for repairing nasal alar defect. Auricle compound tissue free graft should first be considered for patients without severe nasal alar defect, while nasolabial fold flap free graft should be employed for patients with severe nasal alar defect; Both methods have its advantages and disadvantages, the choice should be made base on the comprehensive consideration of the actual conditions of each patient.
A Combined Therapy for the Repair of Alar Defects That Consists of a Modified Spiral Flap and Postoperative Nasal Stent

Abstract:Background: The repair of alar defects is challenging in clinics. Although skin grafts and nasolabial flaps are reliable, they can cause secondary post-surgical deformities. Methods: In this report, we describe an alar spiral advancement flap with a subcutaneous pedicle combined with postoperative nasal stent therapy for the repair of alar defects. Results: All cases showed slight asymmetry immediately after surgery, but at a median follow-up of 24 months, no cases of dissatisfaction with the nose shape or abnormal respiratory function were found. Almost all incision scars subsided within 12 months. In addition, all flaps that were dissected and rotated during the surgeries healed, and no signs of necrosis or development of vascular compromise were observed. Conclusion: Our preliminary experience suggests that the alar spiral flap followed by nasal stent therapy provides stable recovery of appearance and function for alar defects during follow-ups and shows promise for future therapies.
Single-stage Reconstruction of a Large Alar Full Thickness Defect with the Combination of Island Forehead Flap and Nasolabial Flaps

Abstract:Objective To discuss the methods and effects of large alar full-thickness defect reconstruction with combined application of island forehead flap and nasolabial flaps.Methods We retrospectively reviewed the clinical and follow-up data of 4 patients who underwent single-stage reconstruction of large alar full-thickness defects with combined application of island forehead flaps and 2 nasolabial flaps from January 2008 to January 2011.All patients were male and the patients′ age ranged from 62 to 76 years,and the mean age was 64 years.3 cases of defects resulted from resection of a skin malignant tumor(basal cell carcinoma in 2 cases,and acanthoma canceration in 1 case) and traumatic defect in 1 case.The average diameter of defects was 3.3 cm(2.5-4.5 cm).Defects involved two subunits(tip + alar) in one case,three subunits(alar + tip+ nasal lateral side) in one case and four subunits(tip + alar + lateral side + dorsal) in 2 cases.In all patients the risk factors of peripheral vascular disease had been excluded.The first step,a trapezoidal flap of the medial nasolabial sulcus based on piriform aperture was harvested and turned into the nasal cavity to reconstruct the lining.Then the triangle flap of the lateral nasolabial sulcus was raised on a subcutaneous pedicle and was medially moved and sutured around the alar region.The third step,an island forehead flap,based on the subcutaneous pedicle containing the supratrochlear artery and the parallel terminal angular artery,was harvested and inferiorly transferred through the glabella subcutaneous tunnel to cover the nasal skin defect.Results All defects were successfully repaired.All tissue flaps survived and the scar was not conspicuous.One patient had nostril stenosis with mild nasal congestion complaint 6 months postoperatively.There was no tumor recurrence during 6 months to 3 year follow-up.Conclusions In select cases,the paramedian island forehead flap in combination with two nasolabial flaps can be safely used and is an effectively method to repair a large alar full thickness defect.

Abstract:Reconstruction of the nasal ala presents surgical challenges, including loss of the nasofacial junction and vasculature compromise, in addition to achieving a cosmetically satisfactory result. The reconstructive surgeon has a variety of closure techniques to employ, but few allow for acceptable cosmesis in a single-stage procedure. The objective of this study is to discuss a novel approach to alar reconstruction using a melolabial-based transposition island pedicle flap, an alternative to traditional interpolated melolabial flaps and inferiorly based interpolated paranasal flap methods. Our reconstruction method utilizes an island pedicle flap harvested from the nasolabial fold and rotated 165˚ medially and superiorly into a surgical defect on the adjacent ala. The pedicle is placed within the alar facial sulcus for a slight trap-dooring effect, recreating the sulcus. The harvest site is closed linearly, resulting in a fusiform scar line to take advantage of the nasolabial fold. Although delicate care is required while dissecting and positioning the flap, it is an otherwise straightforward procedure. The ideal candidate for this technique presents with loss of the alar subunit with an intact alar rim. The only limitation to this style of flap is that the patient has undergone prior procedures involving the ipsilateral nasolabial fold. The transposition island pedicle flap is a well-tolerated alternative to patient cases that require grafting or more involved multi-step reconstructions to efficiently repair nasal alar defects. This technique provides the patient with a presentable cosmetic result using local tissue with minimal post-surgical complications and alar compromise.
Nasofacial Groove Pedicled Flap for the Reconstruction of Lateral Alar Defect

Abstract:Background: The repair of nasal alar defects is challenging for plastic surgeons, and there is currently no standard operation. Herein, the authors reported the clinical outcomes of a nasofacial groove pedicled flap for the reconstruction of alar defect. Methods: This retrospective study included patients who underwent the nasofacial groove pedicled flap for the reconstruction of alar defect between January 2018 and June 2020. Photographs of standard facial postures were taken before and after surgery to record the surgical results of the patients. The patient’s medical history was reviewed retrospectively. Self-reported satisfaction of patients on scar morphology and reconstructive effect were evaluated with a questionnaire survey. Results: There were 26 eligible patients enrolled, and all patients were followed up for more than 1 year after surgery. All flaps were free of ischemia and necrosis and healed well. No patient experienced restricted nostril ventilation. Eight patients underwent reoperation to trim the flap pedicle and the scar. Eight patients (8/26) reported “very satisfied,” and 17 patients (17/26) reported “satisfied” with the repair effect and scar morphology. One patient went through multiple laser treatments to improve her scars but still remained visible hyperpigmentation. She was dissatisfied with postoperative flap pigmentation but was satisfied with the correction effect. Conclusions: The clinical results indicated that the nasal groove flap was safe for the treatment of the lateral alar defect, and the patients were satisfied with the clinical results. The authors believe that this flap can be used as an alternative method for repairing the lateral alar defect. Level of Evidence: Level —IV, therapeutic study.

[Correction of severe alar retraction with alar rotation flap]

Abstract:Objective: To investigate the therapeutic effect of alar rotation flap for severe alar retraction. Methods: Patients with severely retracted alar underwent ala reconstruction using alar rotation flaps and autogenous cartilage batten grafts. First, costal cartilage was used to reshape the nasal tip and nasal dorsum. Then cartilage patch was used to extend and thicken the retracted alar. Then the alar rotation flap was transferred to correct retracted alar. Results: Fourteen patients with severe alar retraction underwent alar reconstruction with alar rotation flap and alar batten grafts. The alar retraction was corrected in all cases, with improvements functionally and aesthetically. No recurrence of alar retraction was noted. The incision healed with acceptable cosmetic results, with obvious scar in only one patient (one side). Conclusions: The alar rotation flap is an effective and reliable surgical option to correct severe alar retraction. Scar can be kept inconspicuous by precise placement of the incision within the junction of the ala and the nasal dorsum, following principles of the aesthetic nasal subunits.
[Microsurgical Reconstruction of the Nasal Subunit Defects].

Abstract:OBJECTIVE:The nose is composed of several delicate subunits, some of which are difficult to reconstruct if they have been injured. The paper presents the microsurgical technique to repair the nasal subunit defects with the free combined preauricular and auricular flap which well match the nasal tissues in texture, contour and color.METHODS:The nasal subunit defects were repaired with the combined preauricular and auricular flap which were vascularized by the superficial temporal vascular system. The flap was harvested from the contralateral preauricular and the region of helix crus. The superficial temporal vessels were anastomosed to facial vessels via the vascular grafts harvested from the lateral circumfluent femoral vessels, which were about 10 to 14 cm in length. The helix crus of donor ear was reconstructed with the post-auricular flap.RESULTS:28 cases were treated, including 3 cases of nasal tip defects, 9 cases of combined nasal alar and sidewall defects, and 16 cases of nasal alar defects. In these cases, the size of the subunit defects varied from 2.5 cm x 1.5 cm to 4 cm x 2.5 cm. 27 cases were successfully repaired with satisfactory results. The contour of reconstructed helix crus in donor site was acceptable. No walking dysfunction of the donor thigh was complained. There is one failed case, and the possible reason is the insufficient blood perfusion to the flaps, which may due to the patient's longtime-smoking status and the hypertension.CONCLUSIONS:The technique of free combined preauricular and auricular flap is ideal for the reconstruction of nasal subunit defects.
Case Report: A Preferred Reconstructing Modality to Restore Neoplastic Nasal Alar Subunit Defects: Sequential Facial Artery Perforator Flaps

Abstract:BackgroundAchieving perfect repair of a nasal defect with the recovery of cosmetic subunits has become a challenge to plastic, dermatologic, and head and neck surgeons. The study aimed to evaluate the effect of reconstructing neoplastic nasal alar subunit defects with sequential facial artery perforator flaps produced from nasolabial groove tissue.MethodsA retrospective analysis of 20 patients who had undergone reconstruction for neoplastic nasal alar defects with this technique from January 2017 to October 2019 was performed. The reconstruction procedure used sequential facial artery perforator flaps. The surgical procedure used and follow-up results achieved have been documented photographically for all patients.ResultsThe aesthetic and functional results of surgery were satisfactory in all the 20 patients. After all surgeries, the reconstructed alar tissues were compliant, bilateral symmetries of the alae and nasolabial grooves were satisfactory, and no patients exhibited color mismatches between the flaps and surrounding tissues. During a mean follow-up period of 22 months, none of the patients exhibited alar retraction, inferior displacement, deformation, or hypertrophic scarring.ConclusionsThe sequential facial artery perforator flap technique created with nasolabial groove tissue to reconstruct neoplastic nasal alar defects is a simple single-stage procedure that provides excellent surgical outcomes.
Application of Nasofacial Groove Pedicle Flap in Repairing Nasal Subunit Defects

Abstract:Objective To evaluate the effect on reconstructing nasal subunits using nasofacial groove pedicle flap and its advantages and disadvantages. Methods From January 2015, total 15 cases with nasal defects due to tumor or trauma on different parts of nasal subunits, including nasal ala and nasal tip were treated with nasofacial groove pedicle flap for nasal reconstruction. Each flap was designed specifically based on the area of defect and then rotated to cover the wound surface. Conditions of both donor and recipient sites were monitored and evaluation of the nasal area were carried out by utilizing three-dimensional (3D) laser scanning. Results All 15 cases shown positive results on the survival rates of flaps, and minimal scars on both donor and recipient sites. Patients' satisfaction rates were high. 3D laser scanning shown regular appearance on the nasal area postoperatively. Conclusion Using nasofacial groove pedicle flap to repair nasal subunit defects is a promising method due to good appearance of recipient site and minimal damage of donor site.
Resurfacing partial nose defects with a retroauricular skin/cartilage free flap.

Abstract:Background: The retroauricular skin has always received much attention by plastic surgeons for use in nose reconstruction because of its rich vascularity. Anatomically, the skin of this area lies hidden behind the ear and is very similar to that of the face. More importantly, the retroauricular skin flap along with the cartilage can be used to repair defective cartilage of the nose. All these reasons make this area an ideal donor site for nose reconstruction. This article presents the surgical procedures for reconstruction of partial nasal defects with the use of retroauricular skin/cartilage free flaps. Methods: Based on the anatomy and blood supply of the retroauricular region, a flap was designed and harvested with the retroauricular vessels as its pedicle. This flap was used to repair the partial nose defects in 10 patients, between the years 2002 and 2008. Of these, 4 patients presented with unilateral ala defects; 2 with nasal tip defects; 2 with defects on the nasal tip and bilateral ala; 1 with unilateral ala and tip defect; and 1 with unilateral ala and lateral defect. The size of the defect ranged from 2.5 x 1.5 cm to 4.0 x 3.0 cm. The flap varied between 3.0 x 2.0 cm and 4.5 x 3.5 cm. In all cases, the auricular cartilage of 1.5 x 1.0 cm to 1.0 x 0.5 cm was taken along with the flap. The retroauricular vessels were anastomosed with the facial vessels. The donor site was covered with full-thickness skin graft or local flap. Results: No flap failure was recorded. Excellent morphologic reconstruction was obtained with these flaps with no sequela at the donor site in terms of form and function. Postoperatively, the blood supply to the flaps was fairly good in 8 cases, although there was vein congestion in 2 cases on the fourth day. However, the postoperative results of the 10 cases were satisfactory after treatment. Conclusion: The retroauricular skin-cartilage free flap is a good choice for partial nasal reconstruction, having the merits of a well-hidden donor site, good color match, and 1-stage operation for cartilage support and nose reconstruction.

METHODS:Based on the anatomy and blood supply of the retroauricular region, a flap was designed and harvested with the posterior auricular vessels as its pedicle. The flap was used to repair nose defects in five patients. Of them, three had unilateral subtotal alinasal defects, one had nasal apex defect, and one had defects on nasal apex and bilateral ala. The size of the defect ranged from 2.5 cm x 1.5 cm to 3.0 cm x 4.0 cm. The flap varied between 3.0 cm x 2.0 cm and 4.5 cm x 3.5 cm. In 4 cases, the auricular cartilage of 2.0 cm x 1.5 cm to 1.5 cm x 1.0 cm was taken along with the flap. The posterior auricular vessels were anastomosed with the facial vessels. The donor site was covered with full-thickness skin graft. RESULTS:Postoperatively, the blood supply to the flaps was fairly good in 3 cases, while there was various venous stasis in 2 cases during the first 4 days. However, the postoperative results of the 5 cases were satisfactory. CONCLUSIONS:The retroauricular free flap is a good choice for partial nose reconstruction, which had the merits of well-hidden donor site, good color match, and one-stage operation.

Abstract:OBJECTIVE:The plastic surgeons prefer to reconstruct nasal alar with free auricular composite flap because it well matches nasal tissue in contour, texture and color. However, the size of the free composite flap should be less than 1. 0 cm x 1.5 cm due to the limitation of revascularization. Our aim is to search for a surgical method which could be used to repair full-layer larger nasal alar defects.METHODS:A surgical technique was presented to repair nasal alar defect with a free auricular composite flap, which was vascularized by branches of superficial temporal artery. Briefly, the contralateral auricular composite tissue pedicled by superficial temporal vessels (3 to 4 cm in length) was harvested from region of helix crus and preauricular skin, which matched the arc of the nasal rim, and then transplanted onto the recipient area. The superficial temporal vessel pedicles were anastomosed to facial vessels via vessel grafts harvested from lateral circumflex femoral vessels, which were about 10 to 12 cm in length.RESULTS:Twelve cases were treated, among them, 11 cases were successfully repaired with satisfactory results, one case failed, possibly due to the bad condition of the patient's blood vessels. In our cases, the size of alar defect varied from 1.5 cm x 2.5 cm to 2.0 cm x 3.8 cm. The minimal auricular donor site deformities and inconspicuous scar were acceptable and could easily be hidden by hair.CONCLUSIONS:The technique of free auricular composite flap, which is vascularized by superficial temporal vessels, is ideal for nasal alar reconstruction.
Repairing Partial Nasal Defect with Different Strategies

Abstract:Objective Proper selection of reconstruction method is the key point to get a successful result in nasal reconstruction.The purpose of the article is to evaluate the results of repairing the partial nasal defect with different surgical strategies.Methods Different surgical methods were used according to the area and type of nasal defects of nasal defects,the different sub-unit of nasal de fects and the different tissue damage levels.Local skin flaps,auricular composite tissue flaps,nasolabial skin flaps and superficial temporal artery pedicle flaps were used in these patients.Results 23 cases suffering from partial nasal defects were repaired with flaps or tissue graft,including 4 auricular composite tissue flap,5 nasolabial skin flap,3 forehead island flap with a pedicle of superficial temporal vessels,7 local flap,and 1 skin graft.After 5-16 months of follow-up,the color and texture around the nose were basically consistent,the appearance of nose was satisfactory,and the postoperative effect turned out fine.Conclusions The method of surgical reconstruction of partial nasal defects should be considered carefully according to the area of nasal defects,sub-unit of nasal defects and the different tissue damage levels.

Abstract:Abstract Defects of the nasal ala are challenging to reconstruct, given its complex three-dimensional structure. Successful repair of these defects needs to provide aesthetic symmetry and preserve nasal function. A wide variety of reconstructive options have been described for nasal ala defects, ranging from skin grafts to locoregional flaps, and also includes the auricular composite graft. However, there are currently no comprehensive guidelines for nasal ala repair, and the versatile role of the auricular composite graft has not been well defined. In this review, we aim to provide a comprehensive algorithm to guide repair of nasal ala defects. Additionally, we compare our experience using the auricular composite graft with the available literature to better define its utility in nasal ala repair.
Repair of Through-and-Through Defects of the Nasal Ala with Nasolabial Fold Flap

Abstract:Objective: To discuss the clinical application of nasolabial fold flap in repairing through-and-through defects of the nasal ala. Methods: A retrospective study was conducted on 23 cases of through-and-through defects after resection of alae nasi malignant with nasolabial fold flap in our hospital from January 2014 to June 2018, and its clinical effect was ob-served. Results: All the 23 flaps survived. The follow-up period ranged from 6 months to 3 years. The scar was not obvious. The shape of ala nasi was symmetrical without swelling. The nostrils were not narrow. The sensory function of the skin in the repaired defect site was maintained. Patients were satisfied with the operation. Conclusion: The nasolabial fold flap with long vascular pedicle, which can be easily transferred, is easy to survive and has abundant blood supply. It can provide e-nough tissue to repair the through-and-through defects of the nasal ala. It serves as one of the good flaps for repairing the through-and-through defects after resection of alae nasi.
