Abstract:ObjectiveTo review drug policies, health technology assessment (HTA), and HTA’s use in drug policies in China, to further improve the quality and efficiency of drugs.MethodsThis study draws on multiple methods. A systematic review of the literature, review of Chinese government documents and statistical handbooks, and authors’ experiences in drug policies and HTA in China were combined to achieve the objective.ResultsOf 571 studies identified in the initial search, 14 eligible articles (6 English, 8 Chinese) were finally included. On the Web site of the National Health and Family Planning Commission, the National Development and Reform Commission, and the Ministry of Human Resources and Social Security, we found that HTA or pharmacoeconomics evaluation is mentioned in recent years and its frequency has been increasing; however, there was not one hit about HTA or PE on the Web site of China Food and Drug Administration.ConclusionsThe decision makers have realized the importance and value of HTA and have tried to integrate HTA into drug policies and regulations. However, the application of HTA findings to drug policymaking is not yet widespread and there are a number of challenges in using HTA in China. Therefore, it is necessary to establish a national HTA commission and develop pharmacoeconomics guidelines to support the use of HTA in decision making. Moreover, the most important steps are to encourage technology innovation, groom more HTA experts, and build reliable databases in China.

Abstract:Health technology assessment (HTA) is a field of scientific policy research that adopts multidisciplinary approaches to conduct systematic evaluation of health technologies and inform decision making. Although achievements have been made by HTA activities among academics, providers, and policy makers, development of the field of HTA in China is fragmented and not yet formally integrated in health policy making processes. All stakeholders need to make more efforts to strengthen HTA knowledge translation and facilitate a decision making process that is based on evidence including HTA findings. This article reviews how the field of HTA has developed in China, analyzes what factors have been influencing China's HTA development, and proposes policy recommendations.

Abstract:Objectives: This study is to review health technology assessment (HTA) development in China during the past two decades, and to facilitate further development of HTA and its integration into policy making.Methods: The study depends very much on the extensive experiences of the authors in involvement of HTA in China. In addition, literature review and Web site searching have been used to trace the process of growth of HTA in China.Results: With its rapid economic development, the Chinese health system has had many achievements in the past 30 years. However, there are some weaknesses that have developed during this period. HTA is traced back to the early 1990s in China. First, HTA knowledge transfer and establishment of HTA units were effective ways to develop HTA in China. By the end of 1990s, the policy makers of the Ministry of Health (MoH) made efforts to merge HTA with policy making to improve the quality and efficiency of health care. The main government authorities related to health technology are the State Food and Drug Administration, the Ministry of Labor and Social Security, and the MoH. Their involvements in HTA are varied.Conclusion: A technology licensure mechanism based on HTA, including technology permission for use, institution licensure, and workforce licensure, is being gradually carried out by the MoH in China. Moreover, HTA can play an important role in technology market entry, insurance benefit coverage, formulary, clinical pathway, reimbursement, and so on. There is a great opportunity for HTA to be an important part of health reform, especially to help policy makers within the health sector to make difficult decisions.

Abstract:Health technology assessment is recognized internationally and is increasingly important in health policy-making. This paper discusses the stakeholders in health technology,reviews the international experience on knowledge translation from health technology assessment to health policy,and proposes a conceptual model of the potential influencing factors. After reviewing the current situation of knowledge translation in China,this paper has also proposed some research ideas for further studies,such as a measurement on the extent of knowledge translation from health technology assessment to health policy,an identification of the influencing factors during the knowledge translation process and the extent to which they influence knowledge translation.

Abstract:Objective: This paper is to explore the correspondence between health technology assessments and policies on hip replacements in China by comparing research on hip replacements with policies issued by the Ministry of Health. Methods: This study retrieved systematic reviews and meta-analyses on hip replacements from 1975 to 2012 and collected Ministry of Health policies on hip replacements,and analyzed these policies in light of the research done. Results: The Ministry of Health released its first policy on hip replacements in 2005 for approving hip replacement applications. However,the influence of health technology assessments in this policy was not evident. In 2009,two policies related to single-disease quality control were released,and there is evidence that these policies were partly informed by meta-analysis of hip replacements. The latest policies on hip replacements were released in 2012. These policies reflect meta-analyses published between 2009 and 2012. Conclusions: Although it has not always been the case,health technology assessments-including systematic reviews and meta-analyses-are now being considered by the Ministry of Health when they formulate national policies on hip replacements.

Abstract:Objective To analyze the current status of decision translation of health technology assessment(HTA) in China in health policy-making, explore the factors that may influence HTA knowledge translation, and provide references for improving decision-making translation of HTA. Methods Conducting questionnaire survey to health policy-makers using HTA results, and doing both descriptive and statistical analysis. Results 112 questionnaires were included. The survey indicated that research outcomes were to some extent embed in the policymakers' mind, and were also adopted by the policymakers to underpin the policy, but in terms of implementation, the result was not so favorable. There was limited training and funding. However, policy-makers were confident in the future development of HTA. And they were more likely to read research summaries. Conclusion The role of HTA in health technology policy-making should be improved, the funding for HTA be increased to improve the ability to use the HTA evidence, and the evidence-based policy-making in China be improved.

Abstract:Many studies of health technology assessment ( HTA) have been carried out to evaluate policy changes associated with ChinaK's health reform. How to successfully use the knowledge generated by such studies to inform health policy and decision-making is an important issue. By reviewing the definition of knowledge translation and three knowledge translation models that are commonly used,this study has proposed a theoretical model of knowledge translation from health technology assessment to health policy in China,which focuses on the influencing factors in the aspects of researchers and policymakers. Furthermore,this study has provided some recommendations on promoting HTA-related knowledge translation,such as enhancing communication between the researchers and policymakers,establishing incentive mechanisms to encourage knowledge translation.

Abstract:Abstract Background Health Technology Assessment (HTA) in China has recently expanded from purely academic research to include policy or decision-oriented practice, especially after HTA evidence was used to update the National Reimbursement Drug List for the first time in 2017. This study aims to identify the progress and challenges of HTA development from 2016 to 2021 and inform policies and decisions to promote further HTA development in China. Methods We conducted a cross-sectional web-based survey with policy makers, researchers and industry-providers in China in 2016 and 2021 respectively. The ‘Mapping of HTA Instrument’, was utilized to assess the HTA development across eight domains: Institutionalization, Identification, Priority setting, Assessment, Appraisal, Reporting, Dissemination of findings and conclusions, and Implementation in policy and practice. To reduce the influence of confounders and compare the mapping outcomes between the 2016 and 2021 groups, we conducted 1:1 Propensity Score Matching (PSM). Univariate analysis was conducted to compare the differences between the two groups. The overall results were further compared with those of a mapping study that included ten countries. Results In total, 212 and 255 respondents completed the survey in 2016 and 2021, respectively. The total score of the HTA development level in China in 2021 was higher than that in 2016 before PSM (89.38 versus 83.96). Following PSM, 183 respondents from the 2016 and 2021 groups were matched. Overall, the mean scores for most indicators in the Institutionalization domain and Dissemination domain in 2021 were higher than those in 2016 ( P < 0.05). The Appraisal domain in 2021 was more explicit, transparent and replicable than that in 2016 ( t = −3.279, P < 0.05). However, the mean scores of most indicators in the Assessment domain were higher in 2016 than those in 2021 ( P < 0.05). Conclusions Our study suggest that the level of HTA development in China progressed significantly from 2016 to 2021. However, before engaging in HTA activities, further efforts are required to enhance the assessment process. For instance, it is important to establish a clear goal and scope for HTA; adapt standardized methodologies for evaluating the performance of systematic reviews or meta-analyses; and provide comprehensive descriptions of the safety, clinical effectiveness, cost, and cost-effectiveness of the assessed technologies, thus improving the development of HTA in China.

Abstract:Objectives: The aim of this study was to examine the gaps between researchers and policy makers in perceptions and influencing factors of knowledge translation (KT) of health technology assessment (HTA) in China.Methods: A sample of 382 HTA researchers and 112 policy makers in China were surveyed using structured questionnaires. The questionnaires contained two sections: perceptions of HTA research and assessments of six-stage KT activities. Wilcoxon rank sum test was applied to compare the differences in these two sections between HTA researchers and policy makers. Multivariate linear regression was performed to explore KT determinants of HTA for researchers and policy makers separately.Results: Policy makers and researchers differed in their perceptions of HTA research in all items except collaboration in research development and presentation of evidence in easy-to-understand language. Significant differences in KT activities existed in all the six stages except academic translation. Regarding KT determinants, close contact between research unit and policy-making department, relevance of HTA to policy making, and importance of HTA on policy making were considered facilitators by both groups. For researchers, practicality of HTA report and presentation of evidence in easy-to-understand language can facilitate KT. Policy makers, on the other hand, considered an overly pedantic nature of HTA research as an obstacle to effective KT.Conclusions: Substantial gaps existed between HTA researchers and policy makers regarding the perceptions of HTA research and KT activities. There are also some differences in KT determinants by these two groups. Enhancing collaboration, promoting practicality and policy relevance of HTA research, and making HTA findings easily understood are likely to further the KT of HTA evidence.

Abstract:BackgroundFor health technology assessment (HTA) to be more policy relevant and for health technology-related decision-making to be truly evidence-based, promoting knowledge translation (KT) is of vital importance. Although some research has focused on KT of HTA, there is a dearth of literature on KT determinants and the situation in developing countries and transitional societies remains largely unknown.ObjectiveTo investigate the determinants of HTA KT from research to health policy-making from the perspective of researchers in China.DesignCross-sectional study.MethodsA structured questionnaire which focused on KT was distributed to HTA researchers in China. KT activity levels in various fields of HTA research were compared, using one-way ANOVA. Principal component analysis was performed to provide a basis to combine similar variables. To investigate the determinants of KT level, multiple linear regression analysis was performed.ResultsBased on a survey of 382 HTA researchers, it was found that HTA KT wasn't widespread in China. Furthermore, results showed that no significant differences existed between the various HTA research fields. Factors, such as attitudes of researchers toward HTA and evidence utilization, academic ranks and linkages between researchers and policy-makers, had significant impact on HTA KT (p-values<0.05). Additionally, collaboration between HTA researchers and policy-makers, policy-relevance of HTA research, practicality of HTA outcomes and making HTA reports easier to understand also contributed to predicting KT level. However, academic nature of HTA research was negatively associated with KT level.ConclusionKT from HTA to policy-making was influenced by many factors. Of particular importance were collaborations between researchers and policy-makers, ensuring policy relevance of HTA and making HTA evidence easier to understand by potential users.
Key Issues of Economic Evaluations for Health Technology Assessment in China: A Nationwide Expert Survey

Abstract:Objectives: Health technology assessment (HTA) is increasingly crucial in medicine price negotiations in China, yet previous appraisals revealed national discrepancies on key economic evaluation issues: willingness-to-pay threshold, pricing models for multi-indication medicines, and comparator selection principles. This study aimed to collect expert opinions on these issues for future HTA evaluations. Methods: A nationwide anonymous web-based survey encompassing experts across academia, HTA, consultancy/contract research organization/industry, service provider, and payer. In 2023, a generic invitation containing a web link to the questionnaire was disseminated via WeChat using convenience and snowball sampling. Agreement rates for questionnaire views were analyzed using descriptive statistics. The relationship between participants' responses and demographics was examined using appropriate logistic models. Results: A total of 303 responses were received from experts in 34 cities. Key expert views include a suggested base willingness-to-pay threshold ranging from 0.5 to 1.5 times gross domestic product (52.1% agreement); elevated thresholds for childhood diseases, rare diseases, end-of-life diseases, and first-in-class medicines (>78.0% agreement); a single pricing model for multi-indication medicines (60.4% agreement); consideration of multiple medicines as comparators (79.9% agreement); and avoiding the use of centrally procured medicines as comparators for medicines with a time to market of less than 3 years (71.0% agreement). Participants who are service provider had lower odds of selecting higher thresholds (odds ratio 0.26; P < .01) than responders from consultancy/contract research organization/industry. Conclusions: Expert views indicate the need for substantial changes in China's current HTA methods, highlighting the need for increased investment in HTA processes and expertise cultivation.
Does stents related health technology assessment have an impact on health policies in China: Based on the literature review

Abstract:Objective: This study aimed to examine the relationship between researches on stents and related policies in China. Methods: The study reviewed and compared published scientific articles and policy documents related to stents. We retrieved literature and guidelines from China National Knowledge Infrastructure ( CNKI) and PubMed databases using"stent"as the key word,and searched policy documents in websites of the Chinese Ministry of Health and related websites of Beijing,Shanghai and Guangzhou. Results: Numbers of stent related papers are increasing both in Chinese and English. The number of Chinese systematic reviews and meta-analysis is too small to draw a conclusion. These papers are mainly about safety and effectiveness,but lack of content about cost effectiveness. We also found six policy documents regarding tendering procurement,price management and technique management standard. The first two types of policies may have some connection with cost related articles; however,the third one has no connection with stent related articles. Conclusions: Although there are many studies on stents,there is still a lack of cost-related studies to address this important health policy aspect in China. The indication of stents, especially drug eluting stents ( DES) in guideline is consistent with the evidence from the technology evaluations. However,other policy documents were still lack of evidence.

Health technology assessment in Australia began from pharmaceutical benefit schedule and developed as time went on. Now health technology assessment is applied into more areas,and the whole process is about 20 years. The success has common in characteristics of other countries,however,it has unique characteristic at the same time, and there still exists space for improvement,which also hints that the development of health technology assessment in China needs to learn from international successful experiences and make it localized.

Abstract:Objective: To compare the differences in health technology assessment ﹙HTA﹚ knowledge translation between different types of research institutes and provide some scientific evidences to expand HTA impact in the process of policy-making. Methods: A self-administered questionnaire survey was implemented to domestic HTA researchers. Likert scores were applied to describe the situation of HTA knowledge translation. Rank-sum test was conducted to compare the differences. Results: While the differences in research utilization were not statistical significant, some significant differences were found between researchers from university and researchers not from university in the aspects of organizational knowledge translation support, research uptake process, cognition of knowledge translation, output forms of research outcomes, quality of HTA research. Conclusion: It is recommended to establish the knowledge translation support mechanism for HTA research outcomes, strengthen the collaborations and communications between HTA researchers and policy-makers, promote their cognition of HTA knowledge translation in the process of decision-making, and adopt various output forms of research outcomes more appropriately and effectively.

Abstract:HTA already has a key role in South Korea and Taiwan, with current systems undergoing important changes. In contrast, in mainland China and Japan, HTA is not yet formally utilized, although this appears likely to change. Several elements are important for HTA to be meaningful and impactful for DM therapies, including a clear, transparent analytical framework for HTA that includes all relevant costs and outcomes; availability of local DM epidemiologic, economic, and quality-of-life data; acceptance of modeling as a core methodology; availability of real-life patient data; and recognition of specific evidence requirements associated with biosimilars. HTA has the potential to assist payors in making informed decisions about the coverage of DM medications.

Abstract:Background In China, health technology assessment (HTA) has recently been adopted in pricing negotiation for medicine listing in the National Reimbursement Drug List. At present, how HTA is applied to inform the decision-making process remains underreported. In order to explore how the adoption of HTA was translated into listing and price negotiation results in light of the confidential nature of the negotiating process, this study aimed to compare the negotiated price and the clinical benefit of selected targeted anticancer medicines (TAMs) involved in the 2019 negotiation. Main text Among 16 TAMs successfully negotiated, only four TAMs representing four indication groups had appropriate reference medicines for comparison and were, therefore, included in the analysis. The price and clinical benefit of the four TAMs were compared against one or two reference medicines with the same initial indications. The sales prices for nine TAMs before and after the negotiation were extracted from the centralized medication procurement system. Clinical benefits were evaluated based on evidence from published articles and clinical guidelines. The results suggested that, despite the application of HTA, both rational and irrational decisions had been made about the reimbursement of TAMs in the 2019 negotiation, warranting further investigation. Conclusion While the development and adoption of HTA has seen significant progress in China, actions are needed to ensure that the adoption of HTA is effectively applied in decisions on the reimbursement of medicines.

Abstract:Unreasonable growth of total health expenditure at various extent has been found to be prevalent among many countries for a long time. Thus how to deal with this global public health challenge has become a hot topic among the consumers, providers, and payers alike. Echoing the global trend of value-based healthcare, value-based management of total health expenditure could be a direction of cost containment in the future. Through promoting the rational development of healthcare industry, assisting the decision-making of health insurance authority, improving health authority′s supervision of health technology allocation and utilization, maximizing resource utilization efficiency at hospitals, standardizing physician practices, guiding patients′ medication preferences and behaviors, health technology assessment could mobilize stakeholders′participation in the value-based management of total health expenditure and serve as an important decision-making tool to optimize the allocation and utilization of scarce health resources, reducing and avoiding waste in healthcare sector, and promoting high-value and sustainable development of total health expenditure.
