Abstract:Introduction: Tooth loss can have a wide range of adverse consequences on patients. Caffeine is the main active ingredient in the world's most popular beverages. This study aims to explore any possible connection between the two. Methods: This study analyzed data from the NHANES collected from 2017 to March 2020. Multivariate logistic regression models to examine the association between the caffeine intake and tooth loss. Odds ratios (OR) with 95% confidence intervals (95%CI) was used to evaluate the significance of association. Results: A total of 5,450 participants were finally included in the trial. Adjusted Multivariate logistic regression result showed that tooth loss was strongly linked with higher levels of caffeine intake (OR=1.74; 95% CI 1.40-2.17; p<0.001). Stratified logistic regression analysis showed that only in the subgroups of age (p=0.01) and PIR (p=0.02), the results showed statistical difference. Caffeine intake does not influence tooth loss in ≥ 60 group (p<0.05). Conclusion: In conclusion, increased caffeine consumption may be a factor in tooth loss.

Abstract:Rationale and objectiveA causal relationship concerning coffee intake and diabetic nephropathy (DN) is controversial. We conducted a Mendelian randomization study to assess the causal nature of these associations.Methods40 independent single nucleotide polymorphisms (SNPs) associated with coffee intake were selected from the UK Biobank study. Summary-level data for diabetic nephropathy were obtained from publicly available genome-wide association studies (GWAS) and the FinnGen consortium. Inverse variance weighted (IVW), MR-Egger, and weighted median (WM) methods were used to examine a causal association. Sensitivity analyses included Cochran’s Q test, the intercept of MR-Egger, MR-PRESSO, and the Outlier method. Leave-One-Out sensitivity analyses were also conducted to reduce the heterogeneity.ResultsOur current study demonstrated positive associations of genetically predicted coffee intake with diabetic nephropathy (OR=1.939; P = 0.045 and type 2 diabetes with renal complications (OR = 2.787, P= 0.047). These findings were robust across several sensitivity analyses.ConclusionsThis study found a positive correlation between coffee consumption and the risk of diabetic nephropathy using genetic data. For a more accurate and trustworthy conclusion, subgroup analysis on coffee intake, including preparing method, variety of coffee, and quantity, is required.
The Association Between Caffeine Intake and Risk of Kidney Stones: A Population-Based Study.

Abstract:Background:Although many studies have proven the beneficial effects of caffeine on human health, the association between caffeine intake and the risk of kidney stones is limited in large epidemiologic studies.Objectives:We aimed to investigate the association between caffeine intake and the risk of kidney stones.Methods:A total of 30,716 participants (with weight numbers of 204, 189, and 886) with a history of kidney stone were included in this analysis. All data were survey-weighted, and corresponding logistic regression models were performed to examine the associations between caffeine intake and the risk of kidney stones.Results:In a fully adjusted model, a per-quartile increase in caffeine intake was associated with a 5.32% decreased risk of kidney stones. In the subgroup analysis, the multivariate-adjusted odds ratios (95% confidence intervals) of the risk of kidney stones for per-quartile increments in caffeine intake were 0.9650 (0.9643, 0.9656) for men, 0.9320 (0.9313, 0.9327) for women, 0.9384 (0.9378, 0.9389) for white race individuals, 1.0281 (1.0270, 1.0292) for nonwhite race individuals, 0.9460 (0.9455, 0.9465) for overweight/obese individuals, and 0.9314 (0.9303, 0.9324) for non-overweight individuals, 0.9100 (0.9094, 0.9105) for caffeine from coffee, and 1.0021 (1.0013, 1.0029) for caffeine from non-coffee sources.Conclusion:Caffeine intake was negatively associated with the risk of kidney stones. In subgroup analyses, the negative association of caffeine with kidney stone risk was only found in white individuals. In addition, the decreased risk was found higher in women and non-overweight individuals. Especially for women, white individuals and non-overweight individuals. The protective effect of caffeine intake from coffee on stone formation was more significant than that of caffeine from non-coffee sources.

Abstract:Introduction: The reported relationship between coffee intake and renal function is poorly understood. By applying two-sample Mendelian randomization (MR) and systematic review and meta-analysis we investigated the association of caffeine and coffee intake with prevalent CKD and markers of renal function. Material and methods: For the individual data analysis we analyzed the National Health and Nutrition Examination Surveys (NHANES) data on renal function markers and caffeine intake. MR was implemented by using summary-level data from the largest ever genome-wide association studies (GWAS) conducted on coffee intake (N = 91,462) and kidney function (N = 133,413). The inverse variance weighted method (IVW), weighted median-based method, MR-Egger, MR-RAPS, and MR-PRESSO were applied. Random effects models and generic inverse variance methods were used to synthesize quantitative and pooled data for the meta-analysis, followed by a leave-one-out method for sensitivity analysis. Results: Finally, we included the data of 18,436 participants; 6.9% had prevalent CKD (based on eGFR). Caffeine intake for the general population was 131.1 ±1.1 mg. The percentage of participants with CKD, by caffeine quartile, was 16.6% in the first (lowest) quartile, 13.9% in the second, 12.2% in the third and 11.0% in the top quartile (p < 0.001). After adjustment, for increasing quartiles for caffeine consumption, mean urine albumin, albumin-creatinine ratio and estimated glomerular filtration rate (GFR) did not change significantly (p > 0.234). In fully adjusted logistic regression models, there was no significant difference in chances of CKD prevalence (p-trend = 0.745). In the same line, the results of MR showed no impact of coffee intake on CKD (IVW: β = -0.0191, SE = 0.069, p = 0.781) or on eGFR (overall = IVW: β = -0.0005, SE = 0.005, p = 0.926) either in diabetic (IVW: β = -0.006, SE = 0.009, p = 0.478) or non-diabetic patients (IVW: β = -6.772, SE = 0.006, p = 0.991). Results from the meta-analysis indicated that coffee consumption was not significantly associated with CKD (OR = 0.85, 95% CI: 0.71-1.02, p = 0.090, n = 6 studies, I 2 = 0.32). These findings were robust in sensitivity analyses. Conclusions: Implementing different strategies, we detected no significant association between coffee consumption and renal function or risk of CKD.
The Association Between Caffeine Intake and Mortality among Patients with Chronic Kidney Disease: A Population-Based Study

Abstract:Background The benefits of caffeine to human health have been widely reported, but the association between caffeine intake and mortality among patients with chronic kidney disease (CKD) has been rarely studied in large epidemiologic studies. Thus, we aimed to investigate the association between caffeine intake and mortality among CKD patients.Methods Our study was based on non-dialysis CKD patients in 2003–2016 National Health and Nutrition Examination Survey (NHANES). Weighted COX regression analysis was applied to explore the linear relationship between caffeine intake and mortality (including all-cause mortality, cardiovascular mortality, cancer mortality, cerebrovascular mortality, nephropathy mortality and influenza and pneumonia mortality). Restricted cubic spline analysis was performed to explore the nonlinear relationship. Finally, threshold effects were analyzed with two-piecewise linear regression.Results In the fully adjusted model, there was no significant linear association between caffeine intake and mortality. However, a U-shaped nonlinear association between caffeine intake and all-cause mortality (inflection point = 277mg) was identified. Moreover, there was a J-shaped association between caffeine intake and cardiovascular mortality (inflection point = 252mg) and cancer mortality (inflection point = 79mg).Conclusion Moderate caffeine intake shows a protective effect on the prognosis of CKD patients. However, excessive caffeine intake was associated with increased all-cause mortality, cardiovascular mortality and cancer mortality.
Association between urinary caffeine and caffeine metabolites and stroke in American adults: a cross-sectional study from the NHANES, 2009–2014

Abstract:This study investigates the potential correlation between urinary caffeine levels and the occurrence of stroke, a serious cerebrovascular disease that can lead to disability or death. The data used in this study was obtained from the National Health and Nutrition Examination Survey conducted between 2009 and 2014. The study analyzed a total of 5,339 individuals, divided into a control group (n = 5,135) and a stroke group (n = 162). The researchers utilized multiple logistic regression and smoothed curve fitting to examine the relationship between urinary caffeine and caffeine metabolites and the incidence of stroke. The study found that higher urinary caffeine levels were associated with a lower risk of stroke in Mexican American participants (odds ratio [OR] = 0.886, 95% confidence interval [CI]: (0.791, 0.993), P = 0.037). After adjusting for certain participant characteristics, it was also found that higher urinary paraxanthine levels were associated with a lower risk of stroke incidence (OR = 0.991, 95% CI (0.984, 0.999), P = 0.027). Meanwhile, the highest urinary paraxanthine levels group had 43.7% fewer strokes than the lowest level group (OR = 0.563, 95% CI (0.341, 0.929), P = 0.025). In this study, we showed a negative link between urine paraxanthine levels and the risk of stroke. Meanwhile, urinary caffeine levels were negatively associated with the incidence of stroke in Mexican Americans, but no correlation in other populations. Our findings may have predictive and diagnostic implications in clinical practice. Further extensive prospective investigations are still needed to validate our conclusions.

Inverted U-shaped Relationship Between Coffee Consumption and Serum Uric Acid in American Chronic Kidney Disease Population

Abstract:ObjectiveThe objective of this study was to examine the contentious relationship between coffee consumption and serum uric acid (SUA) levels, specifically within American population with chronic kidney disease (CKD).MethodsA total of 23,381 participants from the 2001–2018 National Health and Nutrition Examination Survey (NHANES) were selected for inclusion in this investigation. Linear regression models and generalized additive models with smooth curve fitting were employed to analyze the association between log coffee consumption and serum uric acid levels. Furthermore, the robustness of the findings was assessed across diverse populations.ResultsThe results of the linear regression analysis indicate a positive and marginally statistically significant (p = 0.0307) association between log coffee consumption and SUA, even after controlling for other variables. Furthermore, the generalized additive model revealed a nonlinear relationship characterized by an inverted U-shape between log coffee consumption and SUA. The inflection point, identified as 11.43 g/day, marks the point at which this relationship changes direction. Moreover, this inverted U-shaped relationship was consistently observed across various subgroups, including gender, age (<60 and ≥ 60 years), hypertensive and non-hypertensive individuals, those with and without cardiovascular disease, non-diabetic individuals, and those who consumed coffee with or without caffeine or sugar.ConclusionAn inverse U-shaped correlation has been observed between log coffee consumption and SUA levels. This finding implies that once coffee consumption surpasses a specific threshold, it promotes a decline in SUA levels.
Association of Urinary Caffeine and Caffeine Metabolites with Cardiovascular Disease Risk in Adults

Abstract:Objectives: Studies have shown that the consumption of a moderate amount of caffeine is associated with a reduced risk of cardiovascular disease (CVD) and may even be protective against CVD. The purpose of the current study was to evaluate the association between urinary caffeine and its related metabolites and CVD risk in a national representative sample of US adults. Methods: We analyzed cross-sectional data from the US National Health and Nutrition Examination Survey from 2009 and 2010. The associations between the levels of urinary caffeine metabolites and self-reported CVD, including congestive heart failure, coronary heart disease, angina, heart attack, and stroke, were examined separately in men and women using multivariate logistic regression models adjusted for covariates. Results: In total, 1916 participants (910 men and 1006 women) were included in the analysis. Among women, the odds ratios of CVD in the highest quartiles of 1,3-dimethylxanthine and 1,3,7-trimethylxanthine were 0.33 (95% confidence interval [CI], 0.12-0.92) and 0.35 (95% CI, 0.13-0.93), respectively, compared with the lowest quartiles. Each one-unit (mmol/L) increase in theophylline concentration was associated with a 0.24mg/dL increase in high-density lipoprotein cholesterol in the fully adjusted model. Among men, no significant association was observed between urinary caffeine metabolites and CVD. Regarding the subtypes of CVD, compared with women in the lowest quartile for 1,3-dimethylxanthine and 1,3,7-trimethylxanthine, the odds of coronary heart disease decreased by 90% (95% CI,-99% to-11%) and 97% (95% CI,-99% to-47%), respectively, in those in the highest quartile. Conclusions: Urinary 1,3-dimethylxanthine and 1,3,7-trimethylxanthine were significantly and inversely associated with CVDs in women. Coffee consumption with different additives and types, genetic variation in caffeine metabolism and new-onset acute kidney injury

Abstract:This study aimed to evaluate the association of consumption of coffee with different additives, including milk and/or sweetener (sugar and/or artificial sweetener), and consumption of different types of coffee, including instant, ground, and decaffeinated coffee, with new-onset acute kidney injury (AKI), and examine the modifying effects of genetic variation in caffeine metabolism. 194,324 participants without AKI at baseline in the UK Biobank were included. The study outcome was new-onset AKI. During a median follow-up of 11.6 years, 5,864 participants developed new-onset AKI. Compared with coffee non-consumers, a significantly lower risk of new-onset AKI was found in coffee consumers adding neither milk nor sugar to coffee (HR, 0.86; 95%CI, 0.78-0.94) and adding only milk to coffee (HR,0.83; 95%CI, 0.78-0.89), but not in coffee consumers adding only sweetener (HR,1.14; 95%CI, 0.99-1.31) and both milk and sweetener to coffee (HR,0.96; 95%CI, 0.89-1.03). Moreover, there was a U-shaped association of coffee consumption with new-onset AKI, with the lowest risk at 2-3 drinks/d, in unsweetened coffee (no additives or milk only to coffee), but no association was found in sweetened coffee (sweetener only or both milk and sweetener to coffee). Genetic variation in caffeine metabolism didn't significantly modify the association. A similar U-shaped association was found for instant, ground, and decaffeinated coffee consumption in unsweetened coffee consumers, but not in sweetened coffee consumers. In conclusion, moderate consumption (2-3 drinks/d) of unsweetened coffee with or without milk was associated with a lower risk of new-onset AKI, irrespective of coffee type and genetic variation in caffeine metabolism.

Abstract:Caffeine is a psychoactive substance widely consumed worldwide, mainly via sources such as coffee and tea. The effects of caffeine on kidney function remain unclear. We leveraged the genetic variants in the CYP1A2 and AHR genes via the two-sample Mendelian randomization (MR) framework to estimate the association of genetically predicted plasma caffeine and caffeine intake on kidney traits. Genetic association summary statistics on plasma caffeine levels and caffeine intake were taken from genome-wide association study (GWAS) meta-analyses of 9876 and of >47,000 European ancestry individuals, respectively. Genetically predicted plasma caffeine levels were associated with a decrease in estimated glomerular filtration rate (eGFR) measured using either creatinine or cystatin C. In contrast, genetically predicted caffeine intake was associated with an increase in eGFR and a low risk of chronic kidney disease. The discrepancy is likely attributable to faster metabolizers of caffeine consuming more caffeine-containing beverages to achieve the same pharmacological effect. Further research is needed to distinguish whether the observed effects on kidney function are driven by the harmful effects of higher plasma caffeine levels or the protective effects of greater intake of caffeine-containing beverages, particularly given the widespread use of drinks containing caffeine and the increasing burden of kidney disease.
Urinary caffeine and its metabolites in association with advanced liver fibrosis and liver steatosis: a nationwide cross-sectional study

Abstract:Aims: This study used urinary caffeine and its metabolites to evaluate its relationship with liver fibrosis and steatosis. Methods: A total of 2069 adult participants with needed data were filtrated from the 2009-2014 National Health and Nutrition Examination Survey (NHANES) cycles. Logistic regression models, weighted quantile sum regression (WQS) models, quantile g-computation (QG-Comp) models, and the restricted cubic spline regression (RCS) models were used to assess the relations of the major caffeine metabolites with liver steatosis and advanced liver fibrosis. Results: Most of the urinary caffeine and its metabolites were found to be negatively associated with advanced liver fibrosis. The joint effects of the major caffeine metabolites on anti-fibrosis were significant only in females. In the WQS regression model, the mixture of major caffeine metabolites was associated with a higher risk of hepatic steatosis, with OR=1.18 (95% CI=1.02-1.35), but it disappeared after gender stratification. In addition, the QG-Comp regression model showed no association between the major caffeine metabolites and liver steatosis in the overall population or gender-specific populations. Conclusion: There may be a potential promising effect on anti-fibrosis of caffeine and its metabolites. Further prospective and experimental studies are urgently needed to verify our results and identify the possible molecular biological mechanisms.

Association of caffeine consumption with all-cause and cause-specific mortality in adult Americans with hypertension

Abstract:Coffee is an important beverage that is widely consumed, of which caffeine is the main active ingredient. However, the long-term relationship between caffeine consumption and mortality in hypertensive patients has rarely been studied. This study analyzed a cohort of 12,093 US adults from the National Health and Nutrition Examination Survey from 1999 to 2018. Caffeine consumption was divided into five groups: no intake, >0 to ≤100, >100 to ≤300, >300 to ≤400 and >400 mg/day. Using multivariable-adjusted Cox proportional hazards models, this study performed a 20-year follow-up analysis (1999-2018). In the fully adjusted model, all caffeine consumers had lower all-cause mortality compared with no intake, especially in the >300 to ≤400 mg/day group (hazard ratio (HR) 0.71, 95% confidence interval (CI) 0.60-0.84). The result of restricted cubic spline also showed a nonlinear association between caffeine consumption and all-cause mortality. For cardiovascular disease, mortality decreased only at >400 mg/day (HR 0.63, 95% CI 0.47-0.85). For cancer, diabetes, and kidney disease, only >300 to ≤400 mg/day was significantly associated with decreased mortality: (HR 0.60, 95% CI 0.42-0.67), (HR 0.22, 95% CI 0.07-0.75), and (HR 0.32, 95% CI 0.10-0.96), respectively. Lower all-cause mortality was observed in non-Hispanic White, African American, population aged 40 or above, and people with a body mass index <25 kg/m2. Our findings indicate a nonlinear association between average caffeine consumption and all-cause mortality, suggesting that hypertensive patients may benefit from moderate caffeine intake.
Sex-specific Association Between Coffee Consumption and Incident Chronic Kidney Disease: a Population-Based Analysis of 359,906 Participants from the UK Biobank

Abstract:Background: The risk for chronic kidney disease (CKD) is influenced by genetic predisposition, sex, and lifestyle. Previous research indicates that coffee is a potentially protective factor in CKD. The current study aims to investigate whether sex disparity exists in the coffee-CKD association, and whether genetic risk of CKD or genetic polymorphisms of caffeine metabolism affect this association. Methods: A total of 359,906 participants from the UK Biobank who were enrolled between 2006 and 2010 were included in this prospective cohort study, which aimed to estimate the hazard ratios for coffee intake and incident CKD using a Cox proportional hazard model. Allele scores of CKD and caffeine metabolism were additionally adjusted for in a subsample with qualified genetic data (n = 255,343). Analyses stratified by genetic predisposition, comorbidities, and sex hormones were performed. Tests based on Bayesian model averaging were conducted to ascertain the robustness of the results. Results: Coffee was inversely associated with CKD in a dose-dependent manner. The effects of coffee did not differ across different strata of genetic risk for CKD, but were more evident among slower genetically predicted caffeine metabolizers. Significant sex disparity was observed (P value for interaction = 0.013), in that coffee drinking was only associated with the risk reduction of CKD in females. Subgroup analysis revealed that testosterone and sex hormone-binding globulin (SHBG), but not estradiol, modified the coffee-CKD association. Conclusions: In addition to the overall inverse coffee-CKD association that was observed in the general population, we could also establish that a sex disparity existed, in that females were more likely to experience the benefit of the association. Testosterone and SHBG may partly account for the sex disparity.
A Prospective Investigation of Coffee Drinking and Bladder Cancer Incidence in the United States

Abstract:Background: In 1991, coffee was classified as a group 2B carcinogen, possibly carcinogenic to humans, based on limited epidemiologic evidence of a positive association with bladder cancer. In 2016, the International Agency for Research on Cancer downgraded this classification due to lack of evidence from prospective studies particularly for never smokers. Methods: Baseline coffee drinking was assessed with a food frequency questionnaire in the NIH-AARP prospective cohort study. Among 469,047 US adults, who were cancer free at baseline, 6,012 bladder cancer cases (5,088 men and 924 women) were identified during >6.3 million person-years of follow-up. Multivariable-adjusted Cox proportional hazards models were used to estimate hazard ratios (HR) and 95% confidence intervals (CI), with non-coffee drinkers as the reference group. Results: Coffee drinking was positively associated with bladder cancer in models adjusted for age and sex (HR for ≥4 cups/d relative to coffee nondrinkers = 1.91, 95% CI = 1.70, 2.14; P trend < 0.0001). However, the association was substantially attenuated after adjustment for cigarette smoking and other potential confounders (HR for ≥4 cups/d relative to coffee nondrinkers = 1.18, 95% CI = 1.05, 1.33; P trend = 0.0007). Associations were further attenuated after additional adjustment for lifetime smoking patterns among the majority of the cohort with this available data (P trend = 0.16). There was no evidence of an association among never smokers (P trend = 0.84). Conclusions: Positive associations between coffee drinking and bladder cancer among ever smokers but not never smokers suggest that residual confounding from imperfect measurement of smoking or unmeasured risk factors may be an explanation for our positive findings.

Abstract:BackgroundThe relationship between hypertension and kidney disease is complicated. Clinical trials found intense blood pressure control was not associated with alterations in glomerular filtration rate (GFR) in all patients but did slow the rate of GFR decline among those with a higher baseline proteinuria. However, the underlying mechanism has been unclear.MethodsWe tested the hypothesis that the association between high blood pressure and renal function is modified by albuminuria status by conducting analyses in a cross-sectional study with 12,440 adult participants without known kidney diseases, diabetes or cardiovascular diseases, participating in the National Health and Nutrition Examination Survey (NHANES) 1999-2006.Results1226 out of 12440 were found to have unknown high blood pressure and 4494 were found to have reduced renal function. Overall, a moderate association was found between high blood pressure and renal function insufficiency in all participants analyzed. However, among participants with albuminuria, the prevalence of moderate-severe renal insufficiency substantially and progressively increased from normal subjects to prehypertensive and undiagnosed hypertensive subjects (1.43%, 3.44%, 10.96%, respectively, P for trend<0.0001); on the other hand, the prevalence of undiagnosed hypertension was also significantly higher among subjects with moderate-severe renal insufficiency than those with mild renal insufficiency (35.54% Vs 19.09%, P value <0.05), supporting an association between hypertension and renal function damage. In contrast, no association between hypertension and renal insufficiency was observed among those without albuminuria in this population. Similar findings were observed when the CKD-EPI equation was used.ConclusionsThe association between high blood pressure and reduced renal function could be dependent upon the albuminuria status. This finding may provide a possible explanation for results observed in clinical trials of intensive blood pressure control. Further studies are warranted to confirm our findings.

Abstract:Background: Previous experimental studies have pointed to the benefits of flavonoid intake on kidney function, yet a comprehensive analysis of their role in modulating albuminuria risk within population-based epidemiological frameworks remains limited. This study aimed to fill this gap by examining the correlation between flavonoid intake and albuminuria risk, while further elucidating the potential mediating role of inflammation in this relationship. Methods: Utilizing data from 29,940 participants spanning three cycles (2007-2008, 2009-2010, and 2017-2018) of the National Health and Nutrition Examination Survey (NHANES). We employed rigorous analytical methods, encompassing logistic regression, Restricted Cubic Splines (RCS), and Mediation analysis, to assess the link between dietary flavonoid intake and albuminuria risk. Results: Our findings reveal consistent negative associations between total flavonoids and specific subclasses, including flavonols, flavones, flavan-3-ols, anthocyanidins, and catechins, and the risk of albuminuria. Specifically, after adjusting for confounding factors, each logarithmic unit increase in these flavonoid categories was associated with a decrease in albuminuria risk ranging from 3% to 12%. The mediating proportion of C-reactive protein (CRP) in the relationships between total flavonoids, flavonols, flavones, catechins, and flavan-3-ols intake and albuminuria risk was 7.3%, 5.6%, 2.3%, 7.7%, and 4.6%, respectively. Conclusions: These results indicate that increased consumption of flavonoid-rich foods contribute to a reduced risk of albuminuria.

Abstract:The effects of caffeine on cognitive impairment have not been conclusively determined. This study aimed to objectively assess the correlation between the urinary caffeine and caffeine metabolites and cognitive decline in older adults. Data on urinary caffeine and caffeine metabolites and the cognitive performance of participants aged 60 years and older were extracted from the National Health and Nutrition Examination Surveys 2011–2014. Binary logistic regression and restricted cubic splines (RCS) analyses were used to evaluate the association between urinary caffeine and caffeine metabolites and cognitive performance. Eight hundred twenty-seven individuals were enrolled in this cross-sectional study. We observed that 1-methylxanthine, 3-methylxanthine, 7-methylxanthine, 1,3-dimethylxanthine, 1,7-dimethylxanthine, and 3,7-dimethylxanthine levels were significantly and inversely associated with cognitive decline. The RCS results suggested an approximately linear dose–response relationship between the aforementioned metabolites and cognitive performance. Moreover, the effects of urinary caffeine and caffeine metabolites on cognitive function assessed using the AFT were more evident in men. Our study suggested that urinary caffeine and caffeine metabolite levels were associated with a reduced risk of cognitive impairment in a linear manner, especially in men.

Abstract:Recent large 3 cohorts have shown that caffeinated beverage consumption was associated with lower risk of kidney stone disease. However, its protective mechanisms remained unknown and had not been previously investigated. We thus evaluated protective effects of caffeine (1 μM–10 mM) on calcium oxalate monohydrate (COM) kidney stone formation, using crystallization, crystal growth, cell-crystal adhesion, Western blotting, and immunofluorescence assays. The results showed that caffeine reduced crystal number but, on the other hand, increased crystal size, resulting in unchanged crystal mass, consistent with crystal growth that was not affected by caffeine. However, caffeine significantly decreased crystal-binding capacity of MDCK renal tubular cells in a dose-dependent manner. Western blotting and immunofluorescence study of COM crystal-binding proteins revealed significantly decreased level of annexin A1 on apical surface and its translocation into cytoplasm of the caffeine-treated cells, but no significant changes in other COM crystal-binding proteins (annexin A2, α-enolase, HSP70, and HSP90) were observed. Moreover, caffeine decreased intracellular [Ca2+] but increased [Ca2+] secretory index. Taken together, our findings showed an in vitro evidence of the protective mechanism of caffeine against kidney stone formation via translocation of annexin A1 from apical surface into cytoplasm to reduce the crystal-binding capacity of renal tubular epithelial cells.

The potassium puzzle: exploring the intriguing connection to albuminuria

Abstract:Background: Studies have revealed a relationship between dietary potassium intake and albuminuria, despite the fact that the human body needs a lot of potassium. Our study concentrated on the link between dietary potassium intake and albuminuria. Methods: This study used subgroup analysis and weighted multivariate regression analysis. Data from the National Health and Nutrition Examination Survey (NHANES) were examined to determine the urinary albumin-to-creatinine ratio (ACR) and participant age (20 years or older). ACR >30 mg/g was the threshold for albuminuria. Results: 7,564 individuals in all were included in the study. The link between the two was significant in both our original model (OR = 0.99; 95% CI, 0.98-0.99, p < 0.0001) and the minimum adjusted model (OR = 0.99; 95% CI, 0.98-0.99, p < 0.0001). A fully adjusted model did not change the significance of the negative correlation between potassium consumption and albuminuria (OR = 0.99; 95% CI, 0.98-1.00, p = 0.0005), indicating that each unit increase in potassium intake was related with a 1% decrease in the chance of developing albuminuria. The negative correlation between potassium intake and albuminuria was not significantly influenced by sex, age, BMI, hypertension, diabetes, or smoking, according to interaction tests (p for interaction >0.05). Conclusion: Reduced risk of albuminuria was linked to higher dietary potassium intake. The particular mechanism linking the two still has to be explained by several inventive and prospective studies.
