Abstract:Objectives: To explore the feasibility of texture analysis based on T2-weighted fluid-attenuated inversion recovery (T2-FLAIR) images and apparent diffusion coefficient (ADC) maps in the assessment of the severity and prognosis of ischaemic stroke using the National Institutes of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) scores, respectively. Methods: Overall, 116 patients diagnosed with subacute ischaemic stroke were included in this retrospective study. Based on T2-FLAIR images and ADC maps, 15 texture features were extracted from the ROIs of each patient using grey-level co-occurrence matrix (GLCM) and local binary pattern histogram Fourier (LBP-HF) methods. The correlations of NIHSS score on admission (NIHSSbasline), NIHSS score 24 h after stroke onset (NIHSS24h) and mRS score with the texture features were evaluated using Spearman's partial correlations. The receiver operating characteristic (ROC) curve was used to compare the performance of the selected texture features in the evaluation of stroke severity and prognosis. Results: Texture features derived from the T2-FLAIR images and ADC maps were correlated with NIHSS score and mRS score. Entropy(ADC) and 0.75Quanrile(T)(2-)(FLAIR) showed the best diagnostic performance for assessing stroke severity. The combination of Entropy(ADC) and 0.75Quantile(T2-FLAIR) achieved a better performance in the evaluation of stroke severity (AUC = 0.7, p = 0.01) than either feature alone. Only 0.05Quantile(T2-FLAIR) was found to be correlated with mRS score, and none of the texture features were predictive of mRS score. Conclusion: Texture features derived from T2-FLAIR images and ADC maps might serve as biomarkers to evaluate stroke severity, but were insufficient to predict stroke prognosis.

Abstract:Abstract Background This study aims is to explore whether it is feasible to use magnetic resonance texture analysis (MRTA) in order to distinguish favorable from unfavorable function outcomes and determine the prognostic factors associated with favorable outcomes of stroke. Methods The retrospective study included 103 consecutive patients who confirmed unilateral anterior circulation subacute ischemic stroke by computed tomography angiography between January 2018 and September 2019. Patients were divided into favorable outcome (modified Rankin scale, mRS ≤ 2) and unfavorable outcome (mRS > 2) groups according to mRS scores at day 90. Two radiologists manually segmented the infarction lesions based on diffusion-weighted imaging and transferred the images to corresponding apparent diffusion coefficient (ADC) maps in order to extract texture features. The prediction models including clinical characteristics and texture features were built using multiple logistic regression. A univariate analysis was conducted to assess the performance of the mean ADC value of the infarction lesion. A Delong’s test was used to compare the predictive performance of models through the receiver operating characteristic curve. Results The mean ADC performance was moderate [AUC = 0.60, 95% confidence interval (CI) 0.49–0.71]. The texture feature model of the ADC map (tADC), contained seven texture features, and presented good prediction performance (AUC = 0.83, 95%CI 0.75–0.91). The energy obtained after wavelet transform, and the kurtosis and skewness obtained after Laplacian of Gaussian transformation were identified as independent prognostic factors for the favorable stroke outcomes. In addition, the combination of the tADC model and clinical characteristics (hypertension, diabetes mellitus, smoking, and atrial fibrillation) exhibited a subtly better performance (AUC = 0.86, 95%CI 0.79–0.93; P > 0.05, Delong’s). Conclusion The models based on MRTA on ADC maps are useful to evaluate the clinical function outcomes in patients with unilateral anterior circulation ischemic stroke. Energy obtained after wavelet transform, kurtosis obtained after Laplacian of Gaussian transform, and skewness obtained after Laplacian of Gaussian transform were identified as independent prognostic factors for favorable stroke outcomes.
Prediction of Infarct Lesion Volumes by Processing Magnetic Resonance Apparent Diffusion Coefficient Maps in Patients with Acute Ischemic Stroke

Abstract:Objective: We aimed to investigate the diagnostic value of apparent diffusion coefficient (ADC) maps in magnetic resonance imaging (MRI) in the volume of acute cerebral infarction (ACI). Methods: A total of 207 ACI patients were selected in our study. The cerebral infarction (CI) volume in the initial diffusion-weighted imaging examination, minimum ADC value, relative apparent diffusion coefficient (rADC) value, and mean ADC value were measured. The correlations between age, smoking, drinking, hypertension, diabetes, coronary heart disease, clinical stage, the lowest ADC value, the mean ADC value, and the mean rADC value with CI volume were analyzed by logistic regression analysis. A receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of the ADC value in the ACI volume. Results: There was a significant difference in the distribution of the CI volume in ACI patients (P <.05). A significant difference was found in the signal intensity and percentage distribution of ADC map in patients of different CI groups with different CI volumes (P <.05). The signal of the ADC map was positively correlated with the CI volume. The mean ADC and rADC values had significant differences between different CI volumes (all P <.05). Logistic regression analysis revealed that the mean ADC value was significantly correlated with the CI volume (P <.05). Analysis of the ROC curve showed that the quantitative value of ADC has a diagnostic value for the ACI volume. Conclusion: This study has shown that the signal intensity change on the ADC map in MRI and quantitative analysis of the ADC value can be used as a reference for predicting the ACI volume.
[Cerebral Lesions of DWI Hyperintensity in Patients with Subacute Stroke Assessed by Intravoxel Incoherent Motion Technique].

Abstract:OBJECTIVETo investigate the cerebral lesions of diffusion weighted imaging (DWI) hyperintensity in patients with subacute stroke with intravoxel incoherent motion (IVIM) technique.METHODSThe clinical data of 20 patients with ischemic stroke (3 to 7 d after onset) who underwent DWI and IVIM scanning between June 2014 and July 2015, were retrospectively analyzed. The parameters from IVIM including slow diffusion coefficient (D), fast diffusion coefficient (D(*)) and perfusion fraction (f) were processed. DWI hyperintensity was segmented by its signal intensity greater than the mean+2 standard deviations of the value in the homologous contralateral region. Then, DWI hyperintensity was classified into two regions of interest (ROIs): infarction core and peri-core with the ADC threshold of 0.55 × 10⁻³ mm²/s. The mirrored ROIs of infarction core and peri-core were also obtained. Then, we measured the values of ADC and D, D(*) and f in these ROIs. The ratios of ADC (rADC), D (rD), D(*) (rD(*)) and f (rf) were also calculated (e.g., rADC=ADCinfarction core/ADCmirrored region).RESULTSCompared with mirrored region, ADC, D and f in the infarction core region decreased by 45% (P<0.001), 42% (P<0.001) and 32% (P<0.001), respectively; while ADC, D and f in the peri-core region decreased by 22% (P<0.001), 32% (P<0.001) and 8% (P=0.009), respectively. The values of rADC, rD, rD(*) and rf in the infarction core region were significantly lower than those in the peri-core region (all P<0.001). Pearson analysis showed that rADC was positively correlated with rf in the peri-core region (r=0.467, P=0.038).CONCLUSIONDuring subacute stage of stroke, compared to the infarction core region within DWI hyperintensity, D and f increase in the peri-core region of DWI hyperintensity, reflecting the increased water diffusion in microstructure and perfusion volume in microvasculature. This result shows that the potential reason for the heterogeneous ADC signal is associated with the disappearance of cellular edema and microvascular compensatory with increased blood volume.
Apparent diffusion coefficient value in predicting different outcomes of brain tissues in patients with acute ischemic stroke

Abstract:Objective:To explore the predictive ability of apparent diffusion coefficient (ADC) value in magnetic resonance imaging (MRI) mapping sequences in outcomes of brain tissues in patients with acute ischemic stroke (AIS) who had successful recanalization after endovascular treatment.Methods:A total of 45 patients with AIS who received endovascular treatment and successful recanalization in our hospital from January 2019 to December 2019 were selected. Post-processing software was used to analyze the images of these patients by MRI before surgery and one week after surgery, and the differences of ADC value in the core area of cerebral infarction, lesion reversal area and increased cerebral infarction area displayed by diffusion weighted imaging (DWI) before surgery were measured and compared. Receiver operating characteristic (ROC) curve was used to analyze the predictive ability of preoperative ADC value in the reversal of lesions showed by DWI.Results:Lesion reversal area and increased cerebral infarction area indicated by preoperative DWI existed in all patients after successful recanalization. The preoperative ADC values of the infarct core, lesion reversal area and increased cerebral infarction area were 0.555×10 -3 mm 2/s (0.462, 0.648), 0.637×10 -3 mm 2/s (0.509, 0.765) and 0.948×10 -3 mm 2/s (0.905, 0.991), respectively, with significant differences ( P<0.05). The optimal cut-off point to predict DWI lesion reversal after successful recanalization was 0.57×10 -3mm 2/s, and the accuracy was 87.1% (area under curve=0.871; 95%CI: 0.868-0.875, P=0.000), with sensitivity of 97.2% and specificity of 68.3%. Conclusion:In patients with AIS after successful recanalization, the preoperative ADC values are obviously different in brain tissues with different outcomes, which can be used to predict the final imaging outcomes of the brain tissues.
Diffusion-weighted Imaging and Arterial Spin Labeling for Prediction of Cerebral Infarct Volume in Acute Atherothrombotic Stroke

Abstract:Objectives: This study aims to investigate the usefulness of diffusion-weighted imaging (DWI) and arterial spin labeling (ASL) for predicting final infarct volume in patients with acute atherothrombotic subtype cerebral infarction (AT-type stroke). Methods: The data of 77 patients with AT-type stroke were retrospectively analyzed. ASL and DWI values of minimum apparent diffusion coefficient (min ADC), mean ADC (mean ADC), minimum cerebral blood flow (min CBF), and mean CBF (mean CBF) of the infarction lesions were measured. Changes in cerebral infarction volume (ΔVolume) were determined by DWI reexamination on the 7th day after onset. Correlations of ADC and CBF with Δ Volume were analyzed. Receiver operating characteristic (ROC) curve analysis was used to determine the usefulness of ADC and CBF values for predicting final infarct volume. Results: There was a significant difference in the distribution of the ΔVolume in AT-type stroke (P<0.0001). The ADC and min CBF values were negatively correlated with the infarct ΔVolume (P<0.05); mean CBF and ΔCBF were not correlated with ΔVolume. When min ADC was ≤0.303 × 10-3 mm2/s, min CBF 1.5 ≤2.415 mL/100 g/min, or min CBF2.5 ≤4.25 mL/100 g/min, ΔVolume was likely to be large. The ROC curve showed the highest predictive value for min ADC and min CBF. Conclusion: Distinctive patterns of quantitative ADC and CBF can be used as a simple and rapid method for predicting change in infarction volume in AT-type stroke.

Abstract:The current study aimed to investigate the associations between diffusion dynamics of ischemic lesions and clinical functional outcome of acute and early subacute stroke. A total of 80 patients with first ever infarcts in the territory of the middle cerebral artery underwent multi-b-values diffusion-weighted imaging and diffusion kurtosis imaging. Multiple diffusion parameters were generated in postprocessing using different diffusion models. Long-term functional outcome was evaluated with modified Rankin scale (mRS) at 6 months post-stroke. Good functional outcome was defined as mRS score ≤ 2 and poor functional outcome was defined as mRS score ≥ 3. Univariate analysis was used to compare the diffusion parameters and clinical features between patients with poor and good functional outcome. Significant parameters were further analyzed for correlations with functional outcome using partial correlation. In univariate analyses, standard-b-values apparent diffusion coefficient (ADCst) ratio and fractional anisotropy (FA) ratio of acute stroke, ADCst ratio and mean kurtosis (MK) ratio of early subacute stroke were statistically different between patients with poor outcome and good outcome (P < 0.05). When the potential confounding factor of lesion volume was controlled, only FA ratio of acute stroke, ADCst ratio and MK ratio of early subacute stroke remained correlated with the functional outcome (P < 0.05). Diffusion dynamics are correlated with the clinical functional outcome of ischemic stroke. This correlation is independent of the effect of lesion volume and is specific to the time period between symptom onset and imaging. More effort is needed to further investigate the predictive value of diffusion-weighted imaging.

Abstract:Intravoxel incoherent motion (IVIM) diffusion‐weighted MRI can simultaneously measure diffusion and perfusion characteristics in a non‐invasive way. This study aimed to determine the potential utility of IVIM in characterizing brain diffusion and perfusion properties for clinical stroke. The multi‐b‐value diffusion‐weighted images of 101 patients diagnosed with acute/subacute ischemic stroke were retrospectively evaluated. The diffusion coefficient D, representing the water apparent diffusivity, was obtained by fitting the diffusion data with increasing high b‐values to a simple mono‐exponential model. The IVIM‐derived perfusion parameters, pseudodiffusion coefficient D*, vascular volume fraction f and blood flow‐related parameter fD*, were calculated with the bi‐exponential model. Additionally, the apparent diffusion coefficient (ADC) was fitted according to the mono‐exponential model using all b‐values. The diffusion parameters for the ischemic lesion and normal contralateral region were measured in each patient. Statistical analysis was performed using the paired Student t‐test and Pearson correlation test. Diffusion data in both the ischemic lesion and normal contralateral region followed the IVIM bi‐exponential behavior, and the IVIM model showed better goodness of fit than the mono‐exponential model with lower Akaike information criterion values. The paired Student t‐test revealed significant differences for all diffusion parameters (all P < 0.001) except D* (P = 0.218) between ischemic and normal areas. For all patients in both ischemic and normal regions, ADC was significantly positively correlated with D (both r = 1, both P < 0.001) and f (r = 0.541, P < 0.001; r = 0.262, P = 0.008); significant correlation was also found between ADC and fD* in the ischemic region (r = 0.254, P = 0.010). For all pixels within the region of interest from a representative subject in both ischemic and normal regions, ADC was significantly positively correlated with D (both r = 1, both P < 0.001), f (r = 0.823, P < 0.001; r = 0.652, P < 0.001) and fD* (r = 0.294, P < 0.001; r = 0.340, P < 0.001). These findings may have clinical implications for the use of IVIM imaging in the assessment and management of acute/subacute stroke patients. Copyright © 2016 John Wiley & Sons, Ltd.
Assessment of MRI-based Anomalous Diffusion Changes in Brain Ischemic Stroke with a Fractional Motion Model.

Abstract:The actual diffusion process in human brain has been shown to be anomalous comparing to that predicted with traditional diffusion MRI (dMRI) theory. Recently, dMRI based on fractional motion (FM) model has demonstrated the potential to accurately describe anomalous diffusion in vivo. In this work, we explored the potential value of FM model-based dMRI in quantificational identification of ischemic stroke and compared that with the traditional apparent diffusion coefficient (ADC). We included 23 acute stroke patients, 8 of whom finished a follow-up scan, and 22 matched healthy controls. The dMRI images were acquired by using a Stejskal-Tanner single-shot spin-echo echo-planar-imaging sequence (diffusion gradients were applied in three orthogonal directions with 25 non-zero b values ranging from 248 to 4474 s/mm(2)) at 3.0 T MRI. We calculated the coefficient of variation (CV) for FM-related parameters in stroke lesions, and compared the mean values for FM-related parameters and ADC by using twosample t-tests. Correlation analysis was achieved using Pearson correlation coefficient test. In acute stroke lesions, CV for FM-related parameters showed significant increase compared with normal tissues (P < 0.01), while those of ADC didn't appear statistical difference. Mean values for FM-related parameters showed significant decrease in acute lesion (P < 0.01) and their changing pattern during follow-up was positively correlated with ADC (P < 0.005). Texture Analysis Based on Vascular Ultrasound to Identify the Vulnerable Carotid Plaques

Abstract:Vulnerable carotid plaques are closely related to the occurrence of ischemic stroke. Therefore, accurate and rapid identification of the nature of carotid plaques is essential. This study aimed to determine whether texture analysis based on a vascular ultrasound can be applied to identify vulnerable plaques. Data from a total of 150 patients diagnosed with atherosclerotic plaque (AP) by carotid ultrasound (CDU) and high-resolution magnetic resonance imaging (HRMRI) were collected. HRMRI is the in vivo reference to assess the nature of AP. MaZda software was used to delineate the region of interest and extract 303 texture features from ultrasonic images of plaques. Following regression analysis using the least absolute shrinkage and selection operator (LASSO) algorithm, the overall cohort was randomized 7:3 into the training (n = 105) and testing (n = 45) sets. In the training set, the conventional ultrasound model, the texture feature model, and the conventional ultrasound-texture feature combined model were constructed. The testing set was used to validate the model's effectiveness by calculating the area under the curve (AUC), accuracy, sensitivity, and specificity. Based on the combined model, a nomogram risk prediction model was established, and the consistency index (C-index) and the calibration curve were obtained. In the training and testing sets, the AUC of the prediction performance of the conventional ultrasonic-texture feature combined model was higher than that of the conventional ultrasonic model and the texture feature model. In the training set, the AUC of the combined model was 0.88, while in the testing set, AUC was 0.87. In addition, the C-index results were also favorable (0.89 in the training set and 0.84 in the testing set). Furthermore, the calibration curve was close to the ideal curve, indicating the accuracy of the nomogram. This study proves the performance of vascular ultrasound-based texture analysis in identifying the vulnerable carotid plaques. Texture feature extraction combined with CDU sonogram features can accurately predict the vulnerability of AP.
Association of cerebral microvascular perfusion and diffusion dynamics detected by intravoxel incoherent motion-diffusion weighted imaging with initial neurological function and clinical outcome in acute ischemic stroke

Abstract:Background This work aimed to explore the association of cerebral microvascular perfusion and diffusion dynamics measured by intravoxel incoherent motion (IVIM) imaging with initial neurological function and clinical outcome in acute stroke. Methods In total, 39 patients were assessed with admission National Institutes of Health Stroke Scale (NIHSS) and day-90 modified Rankin Scale (mRS). The parametrical maps of IVIM were obtained, including apparent diffusion coefficient (ADC), pseudo-diffusion coefficient (D*), true diffusion coefficient (D) and perfusion fraction (f). The fD* was the product of f and D*. Moreover, the ratios of lesioned/contralateral parameters (rADC, rD, rD*, rf and rfD*) were also obtained. The differences of these parameters between the poor outcome group and good outcome group were evaluated. Partial correlation analysis was used to evaluate the correlations between the admission NIHSS/day-90 mRS and each parameter ratio, with lesion volumes controlled. Results The ADC, D, D*, f and fD* values of lesions were significantly reduced than those of the contralateral regions. The rADC and rD were significantly decreased in the poor outcome group than good outcome group (all p < 0.01). With lesion volume controlled, rADC showed a weak negative correlation ( r = −0.340, p = 0.037) and a notable negative correlation ( r = −0.688, p < 0.001) with admission NIHSS score and day-90 mRS score, respectively. In addition, rD showed a strong negative correlation ( r = −0.731, p < 0.001) with day-90 mRS score. Conclusion Significant negative correlations were revealed between IVIM derived diffusion dynamics parameters and initial neurological function as well as clinical outcome for patients with acute ischemic stroke. IVIM can be therefore suggested as an effective non-invasive method for evaluating the acute ischemic stroke.

Abstract:AIM: To study the roles of isotropic diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)mapping in diagnosing early cerebral infarction.METHODS: 21 patients with cerebral in farction (8hyperacute,13 acute)were imaged with both conventional MRI and single-shot echo-planar isotropic diffusion weighted imaging.Among them 12 patients had CT scanning simultaneously within 24 hours after onset.The positive rate of early infarction was comparted on CT,T 2WI and DEI.The change of the infarct lesion in DWI and T 2WI was also analysed.The average ADC,relative ADC(rADC)and the ADC from center to periphery of the lesion were calculated.RESULTS: 8 hyperacute cerebral ischemic regions were revealed at DWI and ADC mapping,but CT and conventional MR were not.Hyperacute and acute infarcts appeared as areas of hyperintensity on DWI,and their average ADC was significantly depressed comparted with homologous contralateral tissue (0 698±0 104 υs 0 990±0 161×10 -3 mm 2/s; P0 01 ).ADC value in 21 hyperacute had gradient sign. CONCLUSION:Isotropic diffusion weighted imaging and ADC mapping have greater senstitivity for acute and hyperacute cerebral infarction than conventional MRI and CT,and may be used to defined the core and penumbra of ischemic lesion.

Abstract:Background The apparent diffusion coefficient (ADC) from diffusion-weighted imaging (DWI) can quantify alterations in water diffusivity resulting from microscopic structural changes from amnestic mild cognitive impairment (aMCI) and Alzheimer's disease (AD). Purpose To investigate the ADC value for aMCI and AD using Brain Search (BS) software based on anatomical volumes of interest (AVOI). Material and Methods In total, 174 aged people were screened, and 25 patients with AD, 26 patients with aMCI, and 18 normal controls (NCs) were recruited. DWI was performed at 1.5 T with a fluid-attenuated inversion recovery (FLAIR), and the independent ADC mapping was generated after imaging acquisition. Ninety regional parcellations were adopted in a Brain Search (BS) based on the automated anatomic labeling atlas. The gray scale intensities (water diffusivity) from the collected ADC mappings were analyzed with BS. The mean value of each anatomical brain region was compared among aMCI, AD, and NC. The statistically significant ( P < 0.05) group differences are displayed in color. Results During the pathological process of AD, the changes of water diffusivity appeared first in the left hippocampus, then gradually progressed to the bilateral sides and eventually displayed right lateralization. The ADC values from aMCI were obviously elevated compared to the values from the NC group in the left limbic cortex. Between the AD and NC groups, the significantly different brain areas included the bilateral hippocampus, the Cingulum_Mid, the ParaHippocampal_R, and the Temporal and Frontal lobes. There was a negative correlation between the ADC values and the scores from MMSE, MoCA, the Digit test, Raven's IQ, and WAIS IQ. Additionally, the ADC values were positively correlated with the scores from CDR, ADL, and ADAS-Cog. Conclusion The water diffusivity for aMCI and AD displays asymmetric anatomical lateralization. The water diffusivity alterations can be analyzed and visualized with our newly designed analytic imaging software, BS, which can be used as a good reference for examining and diagnosing aMCI and AD patients.

Abstract:Objective: To compare perfusion-weighted imaging (PWI) and apparent diffusion coefifcient (ADC) maps in prediction of infarct lesion volumes and growth in patient with hyperacute cerebral artery in-farct.Methods: Twenty hyperacute cerebral artery stroke patients who underwent multimodal magnetic reso-nance imaging (MRI) in hyperacute (<6 h) phase and the follow-up MRI in the subacute (days 5-7) phase were retrospectively reviewed. Diffusion- and perfusion-weighted imaging lesion volumes were semi-automatically measured by a post-processing software of GE Healthcare 3.0 T MR scanner (Signa Twin Speed, GE Medical System, Milwaukee, Wisconsin, USA). Finally, to compare the two methods in infarct growth prediction, correla-tion and regression analysis and receiver operating characteristic curves were used.Results: The lesion volume of follow-up FLAIR was highly correlated with volumes predicted with ADC-, CBF-, CBV-, MTT-, and TTP-maps (allP<0.05). The correlation was higher for ADC-predicted volume (r=0.954, 95%CI: 0.804-1.000) than that for PWI- predicted volume (P<0.05). Besides, The infarct growth was correlated with ADC-DWI mismatch (r=0.744,P=0.001), but we didn’t ifnd any correlation among infarct growth and CBF-DWI, CBV-DWI, MTT-DWI, TTP-DWI mismatches (allP>0.05).Conclusion: Data showed that by using the post-processing software provided by MR equipment, the ADC-based method is more feasible than PWI-based method for evaluating in-farct growth and volume in the subacute phase, which makes early estimation of prognosis possible.

Abstract:BACKGROUND AND PURPOSE:The application of a fluid-attenuated inversion-recovery pulse with a conventional diffusion-weighted MRI sequence (FLAIR DWI) decreases the partial volume effects from cerebrospinal fluid on apparent diffusion coefficient (ADC) measurements. For this reason, FLAIR DWI may be more useful in the evaluation of ischemic stroke, but few studies have looked at the effect of FLAIR on ADC measurements in this setting. This study quantitatively compares FLAIR DWI and conventional DWI in ischemic stroke of varying ages to assess the potential advantages of this technique.METHODS:We respectively analyzed 139 DWI studies in patients with ischemic stroke with and without FLAIR at varying time points ranging from hyperacute to chronic. ADC values were measured in each lesion, as well as in the contralateral normal side. Comparisons were made between the ADC values obtained from the DWI sequences with and without FLAIR for both the lesion and the normal contralateral side.RESULTS:The ADC measurements within the ischemic lesion were very similar on FLAIR DWI and conventional DWI for lesions less than 14 days old (p>0.05), but were significantly decreased on FLAIR DWI for lesions between 15 and 30 days old and in lesions >31 days old (chronic stage) (p<0.01). The contralateral ADC values were all significantly decreased on the FLAIR DWI sequence compared with conventional DWI (p<0.01).CONCLUSIONS:The application of an inversion pulse does not significantly affect the ADC values for early stage ischemic stroke (less than 14 days from symptom onset), but results in a more accurate relative ADC measurement by reducing the cerebrospinal fluid partial volume effects of the normal contralateral side. In addition, combined with the conventional DWI, FLAIR DWI may be helpful in determining the age of ischemic lesions.

Abstract:Objective： To study the changes of water molecular diffusion in the ischemic region by using MR dephase technique and discuss the potential mechanism of the diffusion changes at early stage. Methods: Totally 43 cases were studied retrospectively. There were 10 cases whose MRI examinations were performed within 6 hours，12 cases from 7－24 hours，7 cases from 2－7 days, 8 cases from 8－14 days, 6 cases from 15 days to 2 months. The apparent diffusion coefficients in the ischemic region were calculated. Results: The ADCav in the grey matter was 8.61×10－4mm2*s－1. The ADCav decreased to (4.72×10－4±1.51×10－4) mm2*s－1 in ischemic region at superacute stage, ADCav ratio to contralateral corresponding region was 0.55±0.18, and ADCav increased to (5.68×10－4±1.22×10－4) mm2*s－1 during the time range of 2-7 days, （9.22×10－4±2.07×10－4） during the time range of 8－14 days, and approaching （26.42×10－4+9.65×10－4） mm2*s－1 during the time range of 2 months. The pearson product- moment correlation between the changes of diffusion value and time was sighificent (r=0.95, P<0.001). ADCv increased at superacute stage and decreased over time. Conclusion: The diffusion of water molecules in ischemic region decreased at superacute stage, and the ADC increased over time. The anisotropy increased at superacute stage and decreased as the course developed. DWI could detect ischemic lesion much earlier than CT and routine MR examination. DWI has great value in the diagnosis of superacute stroke. The mechanism of the diffusion changes at early stage may be the intracellular toxicity edema.
[The Clinical Value of Diffusion Tensor Imaging in Ischemic Stroke].

Abstract:OBJECTIVE:To assess the correlations between the change of parameters of diffusion tensor imaging and the clinical symptoms, volume of infarcts, and alteration of fiber tract in patients with ischemic stroke.METHODS:Ten patients diagnosed as acute infarcts underwent 3.0T conventional MRI and diffusion tensor imaging examinations 3 days after the onset of illness. The FA values of the lesions and the contralateral corresponding brain tissues were obtained and the volumes of infarcts were calculated. Visualization of the fiber tract was achieved by postprocessing the acquired diffusion tensor imaging data.RESULTS:All patients had a decrease in FA values of the lesions compared with the contralateral corresponding brain tissues (P=0.002). The percentage of decrease in FA values was correlated with the duration of the infarct attack (correlation coefficient = -0.818, P=0.004). The percentage of decrease in FA values of the infarct lesions was not correlated with the NIHSS scores (correlation coefficient = -0.554, P=0.097). There was a significant positive correlation between the volume of infarcts and the NIHSS scores (correlation coefficient = 0.843, P=0.002). Disruption and attenuation of the fiber tract was observed in the infarct lesions.CONCLUSION:The FA values of the infarct lesions decrease with the course of the disease in the acute and early subacute stage of ischemic stroke. The larger the infarct volume is, the more serious the clinical manifestation is. DTI can show the relationship between the direction and morphology of the fiber tract and the infarct lesions, which will play an important role for judging the prognosis of the patients and informing rehabilitation plans.

Abstract:Objective To reveal the transmiting discipline of parameters and define therapy time window of ischemia penumbra by continuous detection of diffusion tensor imaging(DTI) o Methods 35 patients with acute cerebral infarction in different phases were imaged with conventional DWI and DTI.To measure DCavg,FA,RA,1-VR,ADC in different regions of interest (ROI).Results Change in DCavg had tendency which appear reduced obviously,elevated,renormalized,increased.FA/RA/1-VR of lesions elevated in hyperacute stroke and unrecovered declined acute to sub-acute stroke.Difference of DCavg,FA,RA,1-VR between central and periphral parts of lesions were significant in hyperacute and acute stroke.ADC of lesions were significant in hyperacute and acute stroke.Difference of FA between central and peripheral parts of lesions were significant in acute stroke(P ＜ 0.01).Difference of ADC,DCavg between central and peripheral parts of lesions were significant in acute and subacute stroke(P ＜ 0.01).Conclusion It had characteristic discipline in changes of DCavg,FA,RA,1-VR,ADC,The peripheral parts of lesions in hyperacute and acute stroke maybe ischemia penumbra,DTI has important practical value for clinical.

Abstract:PURPOSE: To explore the potential of texture analysis based on apparent diffusion coefficient (ADC) maps, as a predictor of local invasion depth (stage pT1-2 versus pT3-4) and nodal status (pN0 versus pN1-2) of rectal cancer.MATERIALS AND METHODS: Sixty-eight patients with rectal cancer underwent preoperative magnetic resonance (MR) imaging including diffusion weighted imaging (DWI) at a 3.0 Tesla system. Routine ADC variables (ADCmean , ADCmin , ADCmax ), histogram features (skewness, kurtosis) and gray level co-occurrence matrix features (entropy, contrast, correlation) were compared between pT1-2 and pT3-4 stages, between pN0 and pN1-2 stages.RESULTS: Skewness, entropy, and contrast were significantly lower in patients with pT1-2 as compared to those with pT3-4 tumors (0.166 versus 0.476, P = 0.015; 3.212 versus 3.441 P = 0.004; 10.773 versus 13.596, P = 0.017). Furthermore, skewness and entropy were identified as independent predictors for extramural invasion of tumors (stage pT3-4). Significant differences were observed between pN0 and pN1-2 tumors with respect to ADCmean (1.152 versus 1.044, P = 0.029), ADCmax (1.692 versus 1.460, P = 0.006) and entropy (3.299 versus 3.486, P = 0.015). ADCmax. and entropy were independent predictors of positive nodal status.CONCLUSION: Texture analysis on ADC maps could provide valuable information in identifying locally advanced rectal cancer.LEVEL OF EVIDENCE: 3 J.
