Abstract:Esophageal metastasis from a primary lesion is considered rare, especially when deriving from endometrial adenocarcinomas. Here, we describe a case of a 58-year-old woman presenting with a 3-month history of progressive dysphagia and lower abdominal pain. Subsequently, a poorly differentiated esophageal carcinoma was confirmed by endoscopic biopsy. The pathological and immunohistochemical analyses of the esophageal lesion were similar to that of the endometrial adenocarcinoma that had been removed 2 years earlier, suggesting that the esophageal tumor derived from her previous endometrial adenocarcinoma. The patient received 6 cycles of paclitaxel plus carboplatin first-line chemotherapy and 2 cycles of epirubicin plus cisplatin second-line chemotherapy, with continued disease progression. After switching to apatinib, computed tomography (CT) scans revealed stable disease after 2 months of treatment. Our goal in this case report was to increase the awareness of clinicians and pathologists regarding the possibility of esophageal metastasis originating from endometrial adenocarcinoma, although it is a rare occurrence. Apatinib may be effective for treating metastatic endometrial carcinoma. However, further investigations are warranted to determine the optimal targeted agent for this condition.
Taxanes in Combination with Platinum Derivatives for the Treatment of Ovarian Cancer During Pregnancy: A Literature Review

Abstract:Ovarian cancer is one of the most common types of solid carcinoma diagnosed during pregnancy. Taxane plus a platinum derivative is a combination therapy that is predominantly used in the treatment of ovarian cancer in non-pregnant women. Pregnancy adds various complexities to a course of treatment. In pregnant patients diagnosed with cancer during the first trimester, the risks of fetal malformations and fetal loss increase following the administration of cytotoxic drugs, and this is higher with multi-agent vs. single-agent chemotherapy (similar to 25 vs. 10%). Exposure during the second and third trimester has little influence on teratogenic effects but increases the risk of intrauterine growth retardation, prematurity, low birth weight, and bone marrow toxicity. The present study aimed to review the maternal and fetal safety of treatment with taxane plus platinum derivatives for ovarian cancer during pregnancy. Relevant literature was retrieved from the Embase and PubMed databases using the search terms "ovarian cancer", "pregnancy", "taxane", "paclitaxel", "docetaxel", "platinum", "cisplatin", and "carboplatin". All available data up until September 2016 was synthesized, with no language restrictions. A total of 11 articles (including 13 pregnancies and 14 newborns) were retrieved that reported on the use of standard-dose taxane and platinum chemotherapy, including 9 cases treated with paclitaxel and carboplatin, 3 cases treated with paclitaxel and cisplatin, and 1 case treated with docetaxel and cisplatin. In 13 of the 14 (92.9%) births included, a healthy neonate was born, with follow-up ranging from 2 to 160 months. The average weight of the neonates at the time of delivery was 2,442.1 g. In 7 of 9 the case reports that provided survival data, the mother was alive and disease-free at the end of follow-up (ranging from 2 to 40 months). In conclusion, combination therapy with taxanes and a platinum derivative may play a significant role in the management of pregnant patients with ovarian cancer during the second and third trimester. Exposure to this combination of agents during the second and third trimester does not appear to have a significant bearing on fetal mortality and abortion.
Application of Arsenic Trioxide-Based Combined Sequential Chemotherapy in Recurrent Resistant and Refractory Ovarian Cancers: A Single-Center, Open Phase II Clinical Study

Abstract:Objective. Arsenic trioxide (ATO) has been effectively used for the treatment of hematological malignancies and some solid tumors, while ATO effects were not tested clinically in epithelial ovarian cancer (EOC). Methods. Patients with primary or secondary platinum-resistant EOC were enrolled from October 2015 to December 2019. Patients were mainly treated with ATO-based combined sequential chemotherapy as follows: Regimen 1 (ATO combined taxanes weekly therapy); Regimen 2 (ATO + taxanes + 5-fluorouracil + adriamycin ± bevacizumab sequential chemotherapy), for 5 patients platinum-free interval >12 months, added oxaliplatin). Prespecified end points in this cohort included confirmed best overall response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS), and safety. Results. A total of 33 patients were enrolled in this study. After a median follow-up time of 22.1 months (range 5.5–42.9 months), ORR was 42% and DCR was 85%. The overall PFS was 9.5 months (range 1–38.4 months). The main side effect was myelosuppression. Conclusions. ATO-based sequential combined chemotherapy is effective for primary and recurrent drug-resistant EOC patients in clinical phase II trials. The associated side effects could be controlled, while further study is needed.

Abstract:Arsenic compounds, including various arsenic-containing compounds such as intravenous use of arsenic trioxide (ATO) and an oral tetra-arsenic tetra-sulfide (As4S4) -containing formula named the Realgar-Indigo naturalis formula (RIF). RIF, also known as Compound Huangdai Tablets, which exert anti-tumor effects through a variety of mechanisms, such as the induction of programmed cell death, induction of G1 or G2/M phase arrest, epigenetic regulation of miRNAs, and suppression of cancer stem cell properties. International multicenter clinical studies have shown that ATO and RIF have anti-tumorigenic effects on hematological tumors and some solids. Arsenic compounds also been used in the treatment of cervical cancer, endometrial cancer, ovarian cancer, especially advanced drug-resistant ovarian cancer. This article introduces the progress of ATO in combination chemotherapy, such as ATO with paclitaxel, adriamycin, cisplatin, etc in solid tumors and gynecological malignant tumors. The side effects associated with arsenic treatment, including cardiac disorders, skin and bone marrow suppression, etc. are also discussed in this review. Oral arsenic drugs also have a good therapeutic effect, it may be an outpatient oral chemotherapy new model for RIF to treatment recurrent ovarian cancer.
Complete Remission of Platinum-Refractory Primary Fallopian Tube Carcinoma with Third-Line Gemcitabine Plus Cisplatin: A Case Report and Review of the Literature

Abstract:Primary Fallopian tube carcinoma (PFTC) is a rare but highly aggressive disease. Currently, treatments are similar to those used in epithelial ovarian carcinoma (EOC), however, there are distinct differences between the two diseases. PFTC tends to recur in the retroperitoneal nodes and distant sites more often than EOC. Limited literature with regard to effective agents in platinum-resistant and -refractory (Pt-R) disease exists, particularly after two lines of consecutive treatment. In this case report, a 47-year-old female with PFTC exhibited recurrence in the liver after postoperative chemotherapy. The patient received paclitaxel and cisplatin combination as first‑line chemotherapy and topotecan as a second-line treatment, which is considered platinum-refractory. After the second‑line treatment failed, this patient received a gemcitabine plus cisplatin combination as third-line chemotherapy for a total of 6 cycles. The liver metastases regressed rapidly and completely. The patient's progression-free survival (PFS) was 10 months and overall survival (OS) was 45 months. In conclusion, gemcitabine and cisplatin combination is an effective regimen for refractory PFTC even after the failure of two previous lines of consecutive chemotherapy and this warrants further independent investigation.

Abstract:Objective To study the effect and mechanism of arsenic trioxide on endometrial cancer cells and drug resistant cells.Methods ① The effect of AS2O3 on growth andapoptosis rateof ISK were detected by MTS and Annexin V-FITC/PI double staining flow cytometryat different concentrationsand different time.② After the establishment of MPA resistant cells by the method of increasing the concentration gradient,MTS and Annexin V-FITC / PI double staining flow cytometry were used to detect the growth and apoptosis rate of ISK/MPA after different concentrations and time AS2O3 cultured,compared with the group of ISK.③ After AS2O3 cultured,western-blot was used to detect changes of proteins included p-AKT、p-ERK1/2、Caspase-3、Bcl-2 and Bax.④ To establish a subcutaneous tumor model in nude mice,intraperitoneal injection ofAS2O3 (2 mg/kg),observed the volume change of tumor and toxic effects during the period of drug injection.Results ① AS2O3 had inhibitory growth effect on ISK,the inhibitory effect had time-dependence and concentration-dependence.After treated with AS2O3,the apoptosis rate of ISK showed concentration dependent increasing,the apoptosis rate of 48 h was higher than that at 24 h.② The resistant endometrial carcinoma cells was successfully established.AS2O3 had inhibitory growth and apoptosis promoting effect on ISK/MPA cells.The inhibitory and apoptosis promoting effect had time and concentration dependence.The apoptosis promoting effect of ISK group was stronger than that at ISK/MPA group (P ＜ 0.05),while there was no significant difference on inhibitory effect (P ＞ 0.05).③After AS2O3 effect,the expression level of p-AKT、p-ERK1/2 protein and Caspase-3 were decreased,Bcl-2 decreased and Bax were increased (P ＜ 0.05).④ Successfully established subcutaneous endometrial carcinoma model in nude mice.After injection of AS2O3,the tumor volume was decreased.Conclusion AS2O3 can inhibit proliferation and promote apoptosis of ISK/MPA cells.The mechanism may be AS2O3 could lead to the phosphorylation levels decrease of Akt and ERK 1/2,inhibition of activation of P13K/AKT pathway and MPAK/ERK pathway,while decreased the expression of Bcl-2 at the same time increased the expression of Bax,then regulate the expression of Caspase-3,whichmay be play its role in proliferation inhibition and promoting apoptosis.
Looking beyond carboplatin and paclitaxel for the treatment of advanced/recurrent endometrial cancer

Abstract:Endometrial cancer incidence and mortality are rising among all ethnic groups. Carboplatin plus paclitaxel is the established frontline treatment for advanced/recurrent disease; however, subsequent treatment with traditional cytotoxic chemotherapy is challenging. The molecular characterization of endometrial cancer has provided important insights into the biological drivers of carcinogenesis, which has allowed for the development of newer precision immunotherapies and targeted therapies, including pembrolizumab, dostarlimab, and lenvatinib. Until recently, platinum rechallenge was often considered at the time of recurrence, given the lack of other available therapeutic options; however, "platinum sensitivity" in endometrial cancer is subjective and largely based on expert opinion and/or practitioner experience. Small retrospective studies have tried to provide guidance on the utility of platinum rechallenge, but they are limited by variable patient characteristics and small sample sizes. The applicability of these retrospective studies to contemporary clinical practice is difficult in the setting of changing patient demographics, a better understanding of endometrial cancer drivers, and the recent approvals of immune checkpoint inhibitors and the combination of lenvatinib plus pembrolizumab in the second-line setting. The primary focus of this review is to distill the available data regarding platinum-doublet chemotherapy rechallenge and highlight recent pivotal developments in endometrial cancer treatment, as well as future directions.
XRCC1 Polymorphism and Overall Survival in Ovarian Cancer Patients Treated with Platinum-Based Chemotherapy

Abstract:Objectives: Although platinum-based chemotherapy is widely used for advanced ovarian cancer (OC), genetic polymorphisms can influence the chemotherapeutic response. This study investigated the association between XRCC1 polymorphisms Arg194Trp, Arg280His, and Arg399Gln, and overall survival (OS) in OC patients who received platinum-based chemotherapy. Methods: We systematically searched PubMed, Embase, the Cochrane library, Chinese National Knowledge Infrastructure, Wanfang, and Weipu databases for relevant studies from inception to October, 2017. OS was calculated using a random-effects model. Sensitivity, subgroup, and publication bias analyses were also performed. Results: Five studies involving 1159 OC patients were included. When compared with 194ArgArg, 194TrpTrp (hazard ratio [HR] 1.09, 95% confidence interval [CI] 0.71–1.69, P = .69) and 194TrpArg (HR 1.00, 95% CI 0.78–1.28, P = .98) carriers were not associated with OS. Similarly, compared with 280ArgArg carriers, neither 280HisHis (HR 1.39, 95% CI 0.82 to −2.34, P = .22) nor 280HisArg (HR 0.98, 95% CI 0.73 to −1.31, P = .90) affected OS. Furthermore, there were no significant differences in OS between 399GlnGln (HR 1.00, 95% CI 0.46–2.16, P > .99), 399GlnArg (HR 1.05, 95% CI 0.81–1.37, P = .70), and 399ArgArg. Finally, subgroup analysis suggested that 399GlnGln significantly decreased OS when the percentage of III or IV cases was >80.0% (HR 1.79, 95% CI 1.22–2.62, P = .003), while OS was increased when this percentage was <80.0% (HR 0.47, 95% CI 0.28–0.79, P = .004). Conclusions: This study indicated that XRCC1 Arg194Trp, Arg280His, and Arg399Gln did not affect OS after platinum-based chemotherapy in OC patients. However, disease status could affect the relationship between Arg399Gln and OS in these patients.
Arsenic trioxide re-sensitizes ERα-negative breast cancer cells to endocrine therapy by restoring ERα expression in vitro and in vivo.

Abstract:Approximately one-third of breast cancers lack estrogen receptor α (ERα) because of the hypermethylation of the CpG island in the receptor's promoter. These tumors are associated with poorer histological differentiation, a higher growth fraction, are rarely responsive to endocrine therapy and have a worse clinical outcome. Thus, re-expression of ERα in ERα-negative breast cancers may restore the sensitivity of antiestrogen therapy. The ERα-negative breast cancer cell line MDA-MB-435s was treated with different concentrations of arsenic trioxide (As2O3). MS-PCR was used to detect the change in the methylation status of ERα. RT-PCR, immunohistochemistry and Western blot analyses were used to detect changes in the mRNA and protein expression of DNA methyl-transferase-1 (DNMT1) and ERα. Cell proliferation was examined using the MTT assay. A xenograft model in nude mice was used to further examine the results we observed in vitro. The ERα gene was demethylated after As2O3 treatment of MDA-MB-435s cells. RT-PCR, immunohistochemistry and Western blot analyses revealed that DNMT1 expression was inhibited and ERα was re-expressed in a concentration-dependent manner after As2O3 treatment. The MTT assay showed that cell proliferation was significantly suppressed after exposure to different concentrations of As2O3. Addition of tamoxifen (TAM) further suppressed levels of cell proliferation. In vivo, the xenograft tumor volumes of As2O3-treated mice were smaller than those observed in untreated and TAM-treated mice. Treatment with a combination of As2O3+TAM resulted in further suppression. As2O3 can act as a demethylation agent to restore ERα expression in ERα-negative breast cancer cells and re-sensitize these cells to endocrine therapy in vitro and in vivo.

An Evidence-based Perspective of Arsenic Trioxide (As<sub>2</sub>O<sub>3</sub>) for Cancer Patients

Abstract: The ancient drug, arsenic, has remarkable efficacy in the treatment of relapsed acute promyelocytic leukemia (APL) and it is highly likely that a regimen for treatment of APL that does not require any traditional chemotherapy drugs will be developed in the future. Arsenic trioxide (white arsenic or As2O3) was approved by the United States Food and Drug Administration for being used in the treatment of relapsed/refractory APL in 2000. This success has led to exploration of its use in other malignancies. As2O3 interacts with multiple molecular targets and signaling pathways. The resultant effect depends on factors, including cell type, and the dose and duration of As2O3 exposure. Understanding the molecular and biological basis of these effects will promote the rational and optimal application of As2O3 in diseases other than APL. A series of clinical trials with As2O3 has confirmed its benefit in the therapy of APL, although its role in the treatment of other malignancies remains to be determined. Careful attention to the clinical management of patients on As2O3 therapy can significantly lessen the risk of major side effects. The administration of As2O3 can be done safely if careful attention to electrolyte abnormalities and electrocardiographic monitoring is undertaken. Here we provide an overview of the mechanism of action of arsenic and summarize its development in the treatment of APL and other malignant disorders.

Abstract:Drug resistance is a major concern in the successful treatment of ovarian cancer. In the present study we report a combinational drug regime using arsenic trioxide (ATO) and cisplatin (CDDP) to increase therapeutic potentiality in ovarian cancer cells. ATO-mediated growth inhibition and apoptosis in human suspension ovarian cancer COC1 cells were evaluated by MTT assay and annexin V assay using flow cytometry, respectively. cDNA arrays were performed to screen ATO-mediated gene expression. Treatment of COC1 cells with ATO alone resulted in growth inhibition and apoptosis with a dose-and time-dependent fashion; further cDNA arrays showed that 34 genes (23 up-regulated genes and 11 down-regulated genes) may strongly associate with the antiproliferative and pro-apoptotic effects induced by ATO. Furthermore, Chou-Talalay analysis was used to evaluate the combinational effect of ATO and CDDP as well as dose-reduction index (DRI) in a panel of ovarian cancer cells including CDDP-sensitive and -resistant cell lines. The combination index (CI) analysis indicated that the interaction effect of ATO/CDDP exhibited a wide range of synergism in all the adherent ovarian cancer cells (A2780, IGROV-1, SKOV-3, and R182) as well as 0.93 to 0.69 for IC50 to IC90 in suspension COC1 cells where CI < 1, =1, and > 1, define synergism, additive effect, and antagonism, respectively. More intriguingly, the combination of ATO and CDDP yielded favorable DRIs ranging from 1.23-fold to 13.51-fold dose reduction. These results suggest that ATO and its combination with CDDP present therapeutic potential for ovarian cancer, and deserve further preclinical and clinical studies. (Cancer Sci 2009; 100: 2459-2464).
Three-step chemotherapy treatment for platinum-sensitive recurrent ovarian cancer: a review of 20 cases

Abstract:Background and purpose: Over 80% of advanced ovarian cancer will recur after primary treatment with a median progression-free survival time of 20 months. Patients with recurrent ovarian cancer can’t be cured, but can achieve longer life span and improved life of quality. This study proposed to design a three-step chemotherapy regimen for firstly relapsed platinum-sensitive ovarian cancer to improve survival outcomes. Methods: From May 2010 to December 2016, 20 patients with platinum-sensitive first relapsed ovarian cancer were treated in the Department of Gynecology and Oncology, Fudan University Shanghai Cancer Center. All patients received three-step chemotherapy. The three-step chemotherapy regimen included: ① paclitaxel+carboplatin, administered once every 3 weeks, a total of 6 cycles; ② nedaplatin+etoposide+cyclophosphamide, administered once every 4 weeks, total 5 cycles followed by paclitaxel+carboplatin, administered once every 4-6 weeks, a total of 4 cycles; ③ nedaplatin+etoposide+ cyclophosphamide, administered once every 8 weeks, a total of 5 cycles. Results: The 4-year disease-free survival (DFS) rate and the 5-year DFS rate were 65.0% (13/20) and 61.5% (8/13), respectively. The 4-year and 5-year survival rates were both 100% (20/20 and 13/13). Conclusion: Three-step chemotherapy regimen is correlated to improved survival outcomes for patients with platinum- sensitive recurrent ovarian cancer. Due to its promising effects and low costs, potential value of this treatment strategy is worth further exploration.

Abstract:Background Treatment for platinum-resistant ovarian cancer is challenging. Currently, platinum-resistant ovarian cancer is typically treated with non-platinum single-agent chemotherapy ± bevacizumab, but the prognosis is often extremely poor. In the treatment of platinum-resistant ovarian cancer patients, reports of triple therapy with interstitial implantation radiotherapy combined with immunotherapy and granulocyte-macrophage colony-stimulating factor (GM-CSF) (PRaG for short) are relatively rare. Case description Here, we report a patient with oligometastatic platinum-resistant ovarian cancer. The patient achieved partial response (PR) of the lesion and sustained benefit for more than six months after receiving interstitial implantation radiotherapy combined with immunotherapy along with GM-CSF. Conclusion This triple therapy may provide additional options for these patients.

Abstract:Despite being rare cancers, ovarian cancer rank as the fifth leading cause of cancer mortalities in women in the United States. Most patients are detected late, with most patients diagnosed in Stages 3 and 4. While patients often initially respond to standard of care chemotherapy, which includes platinum-based chemotherapy such as cisplatin and carboplatin, they recur with resistant phenotypes. While a large effort has characterized mechanisms of resistance to platinum-based chemotherapy, it is still challenging to identify these patients and identify more promising therapies. While the field understands that a quiescent, stem cell-like state can serve as an intermediate from which chemoresistant recurrences can arise, the epigenetics of this state are less well understood. In fact, we do know that embryonic and pluripotent stem cells differ significantly in enhancers and other epigenetic features that contribute significantly to their phenotypes. We performed histone post-translational modifications by CUT&Tag, as well as ATACseq and RNA-seq, on four isogenic cell line models of cisplatin resistance. We show that these models are resistant to both cisplatin and carboplatin. While we see paradoxical quiescent cell markers in these isogenic lines, we have characterized differential features of their epigenetics that may contribute to the resistant phenotype. We are continuing to explore transcription factors that may drive this resistance. We are also examining super-enhancers that are altered between chemo-sensitive and -resistant cell lines. Similarly, we have examined whether inhibitors of components of super-enhancers may be a viable therapeutic option for platinum-resistant high-grade serous ovarian cancer patients. We see moderate synergy between triptolide (targets TFIIH subunit XPB1) and JQ-1 (targets BRD4) with cisplatin in resistant models. Together, we hope our studies can contribute to the knowledge of how epigenetic features might contribute and be targets for chemoresistance in ovarian cancer. Abstract:More than 80% of the patients with ovarian cancer chemotherapy relapse due to drug resistance, which makes the highest ovarian cancer mortality rates among the gynecologic malignancies. Compared with the highly heterogeneous tumor cell differentiation, tumor stroma cells have genetic stability, and it is unlikely to produce acquired mutations and lead to drug resistance. In view of the treatment strategies for tumor interstitium, ovarian cancer is reviewed in this paper. At the same time, we pointed out the direction for reducing toxicity, preventing the cancer recurrence and metastasis, improving the survival rate in the treatment of gynecologic malignancies.
