Abstract:To the editor: Desmoid tumors (DTs) are rare benign soft-tissue neoplasms composed of mature fibroblasts and dense intercellular collagen matrixes.1 Intra-abdominal DTs could involve the mesentery and retroperitoneum.2 Although DTs do not metastasize, its evolution can be life-threatening because of aggressive local invasion. We report the result of a case with a ruptured pancreatic DT leading to hemorrhagic shock. A 24-year-old male patient was admitted to the hospital with crampy abdominal pain and nausea. On arrival at the emergency department, he complained of abdominal pain and was found to have hypotension and tachycardia. The blood count showed a rapid decrease in hemoglobin levels from 11.9 to 6.3 g/dl. Abdominal ultrasonography revealed the presence of fluid around the liver and spleen. Computed tomographic scans (Figure 1A) revealed massive intra and retroperitoneal hemorrhage and a 5-cm solid mass in the pancreas corpus. The patient was taken to the operation theater. Splenopancreatectomy was performed. The macroscopic examination (Figure 1B) revealed a poorly circumscribed and fasciculated tumor with local hemorrhage. The transection margins were tumor-free. Histological analysis (Figure 1C) showed that the mass had infiltrated the surrounding pancreatic parenchyma and consisted of spindle cells with a regular nucleus, separated by large amounts of collagen fibers. Immunohistochemical analysis for cluster of differentiation 117 (CD117), delay of germination-1 (DOG-1), CD34, Desmin, B-cell CLL/lymphoma 2 (BCL2), cytokeratin 19 (CK19), electrophoretic mobility and agglutinibility (EMA), anaplastic lymphoma receptor tyrosine kinase (ALK), and S-100 was negative, and the proliferation marker Ki67 was less than 5%.Figure 1. Histological analysis showed that the mass had infiltrated the surrounding pancreatic parenchyma and consisted of spindle cells with a regular nucleus, separated by large amounts of collagen fibers.DTs include abdominal and nonabdominal DTs; among abdominal DTs, a further distinction is made between abdominal-wall and intraabdominal DTs.3 While intra-abdominal DTs are mostly associated with familial adenomatous polyposis,2 such an association was not found in our case. Pancreatic DTs are extremely rare. Whereas it has no metastatic potential, local invasion of blood vessels could result in bleeding. Therefore, the patient underwent surgery. The diagnosis was confirmed by pathological examination. Given the relatively benign course of pancreatic DTs and the young age at presentation, surgical resection should be performed as it could invade the vessels and may, thus, be life-threatening.
