Abstract:Editor, Ocular trauma remains a common cause of severe ocular morbidity in the industrialized countries (Bull 2010; Rasmussen et al. 2010; Wang et al. 2011). Closed globe injury, in which the eye wall does not have a full-thickness wound in Birmingham Eye Trauma Terminology (BETT) (Kuhn et al. 1996), occurs frequently among all types of ocular trauma. However, there are no reports with regard to clinical characteristics and prognostic indicators in closed globe injuries presenting with no light perception (NLP). Some clinicians still do not give surgery to closed globe patients because of the presentation of NLP (Yeung et al. 2006). This letter describes the clinical features and anatomical and visual outcomes of 14 closed globe injuries with initial NLP and identifies the prognostic factors for traumatized eyes with NLP vision following closed globe injury. Fourteen injured eyes (14 patients) were selected from the Eye Injury Vitrectomy Study database, which is a hospital-based multicentre prospective cohort study. The mean age was 28.8 (median, 27; range, 3–51) years. The mean follow-up period was 15.6 (median, 11; range, 6–67) months. The mean time from injury to exploratory surgery was 17.3 (median, 15; range, 6–33) days. In this series cases, hyphaema was noted in seven eyes (50%), which was the most frequent anterior segment presentation in the series cases. Retinal detachment was the most frequent posterior segment manifestation, which occurred in 11 (78.6%) eyes. Optic nerve damage, including avulsion of the optic nerve, traumatic optic neuropathy and optic atrophy, was noted in six (42.9%) eyes. Significant proliferative vitreoretinopathy developed in four eyes (28.6%). Seven (50%) injured eyes involved choroid (Table 1). In this letter, a severe vitreous haemorrhage was defined as a haemorrhage too dense to allow visualization of the optic disc and identification of the intraocular tissues. Anatomically restored eyes were determined after a period of follow-up no < 6 months, IOP ≥ 8 mmHg, vitreous cavity filled with aqueous humour, retinal attachment or only local detachment that does not require surgery. Silicone oil–sustained eyes refer to eyes injected with silicone oil because of an incomplete treatment, lower IOP (< 8 mmHg), unrecovered retinal detachment or inoperable retinal detachment. Six (42.9%) eyes were anatomically restored with final NLP or better vision after vitreoretinal surgery. Silicone oil cannot be evacuated in five silicone oil–sustained eyes because of hypotony or detached retina. Seven (50%) eyes achieved hand movement or better vision after a period of follow-up (Table 1). Chi-square analysis demonstrated that the most important prognostic factor for final NLP was optic nerve damage (p = 0.03). Retinal detachment (p = 0.02) and choroidal damage (p = 0.03) were the predictors for poor anatomical outcomes, which included silicone oil–sustained eyes, phthisis bulbi and enucleation or evisceration. With the development of microsurgical technique, vitreoretinal surgery can enable injured patients to retain their own eye globes even if light perception or better vision cannot be restored. However, evidence regarding the effect of vitreoretinal surgery on prognosis of closed globe injuries presenting with NLP is far from being conclusive. Based on our clinical survey, closed globe injury presenting with NLP is frequently accompanied by various ocular pathologies in anterior and posterior segments. The NLP eyes after closed globe injury are retrievable. We suggest that exploratory surgery is necessary for closed globe injuries presenting with NLP, and we do not recommend the routine use of the procedures, such as primary enucleation or observation.
