Abstract:Background. Although the measurement of central venous pressure (CVP) is a common clinical tool, the role of CVP monitoring in the outcome of sepsis is controversial because threshold values of CVP are uncertain, and there are only limited data on short-term survival of patients with septic acute kidney injury (AKI). Methods. This retrospective cohort study was based on the Medical Information Mart for Intensive Care IV (MIMIC-IV) database (source of the training dataset). Multivariate regression analysis was performed to clarify the relation between CVP measurement and clinical outcomes, and a univariate regression model after propensity score matching was utilized to validate our findings. A mortality prediction model for septic AKI and a risk stratification scoring approach were developed, and the emergency intensive care unit (eICU) database was used for external validation. Results. Of the 9170 patients in the training set, 2446 (26.7%) underwent CVP measurement. No significant association was found between CVP monitoring and 28-day mortality among patients with septic AKI (odds ratio = 0.479 ; 95% confidence interval 0.213-1.076, P = 0.075 ), even after adjustments (propensity score matching; P = 0.178 ). Length of ICU stay and hospital stay was markedly reduced in patients undergoing CVP measurement within 3 hours (median 6.2 and 10.9 days, respectively, P < 0.001 ). The addition of the mean perfusion pressure initial, CVP, and the magnitude of the CVP change within 48 hours to the model significantly increased model discrimination (area under the receiver operating characteristic curve: 0.867 and 0.780, respectively, P < 0.001 ). Conclusions. These findings suggest that CVP measurement alone has little effect on the outcome of septic AKI. Nonetheless, initial CVP levels and the dynamic changes in CVP within the first 48 hours after ICU admission and the mean perfusion pressure initial can improve the accuracy of outcome prediction models.

Abstract:Objectives: The aim of this study is to assess Heparin-binding protein (HBP) as a diagnostic and prognostic biomarker of severe sepsis in the pediatric intensive care unit (PICU).Methods: A multicenter, prospective study was conducted among children with sepsis in nine PICUs in China from October 2019 to June 2021. Plasma levels of HBP, procalcitonin (PCT), C-reactive protein (CRP), lactate, and white blood cell (WBC) count were determined at enrollment and 72 h after enrollment.Results: Of 355 included patients, 132 patients were diagnosed with non-severe sepsis (referred to as sepsis), 223 patients had severe sepsis. Patients with severe sepsis had significantly elevated levels of HBP compared with sepsis (median 170.5 vs. 74.1 ng/mL, P < 0.001). Adding HBP to a diagnostic model with PCT and lactate could significantly improve the diagnostic capability for severe sepsis. The plasma levels of HBP correlated positively with the number of dysfunctional organs. After adjusting for confounding factors, the declined levels of HBP at 72 h had a significant association with decreased in-hospital mortality (adjusted odds ratio (aOR) 0.242, P < 0.001). The levels of HBP showed weak positive correlations with PCT, CRP, WBC, and no correlation to lactate.Conclusions: HBP at enrollment can be an independent indicator for severe sepsis and the dynamic changes at 72 h can be a predictor for in-hospital mortality in PICU.

Abstract:Introduction Sepsis commonly results in acute kidney injury (AKI), whereas about 50% of AKI cases are due to sepsis. Sepsis-associated acute kidney injury (SA-AKI) increases morbidity and mortality especially among critically ill patients. This study aims to monitor renal microcirculation perfusion during sepsis using contrast-enhanced ultrasonography (CEUS), and to explore whether CEUS is useful for predicting the development of SA-AKI. Methods and analysis This prospective observational study will enrol patients who were diagnosed with sepsis-3 definition. The total of septic or septic shock patients were stratified into AKI (including stages 1, 2 and 3) and non-AKI groups according to Kidney Disease Improving Global Outcomes criteria on days 0, 1, 2 and 7 after admission to the emergency intensive care unit, meanwhile, the CEUS technique will be performed to monitor renal microcirculation perfusion. A multivariable model including all CEUS variables were expected to create for predicting the development of AKI during sepsis. Ultrasonography results, demographic information, therapeutic interventions, survival outcomes, laboratory and other clinical datas will also be collected for further analysis. Ethics and dissemination The study protocol was approved on 2 August 2017 by the Ethics Committee of Sir Run Run Shaw Hospital (Zhejiang University Medical College) (approval number: 2016C91401). The results will be published in a peer-reviewed journal and shared with the worldwide medical community within 2 years after the start of the recruitment. Trial registration number ISRCTN14728986

Abstract:Sepsis is defined as an organ dysfunction syndrome caused by the over-activation of inflammatory response (1,2). Kidney is one of the most commonly involved organs in sepsis, and there is a large body of evidence showing that even mild increase in serum creatinine contributes to significantly increased risk of death (3,4). Thus, strenuous efforts have been made to improve renal function during sepsis, including the avoidance of renal toxic agents, treatment with sodium bicarbonate to correct metabolic acidosis (5), and restriction of chloride-rich fluid and starch (3,6,7). However, the risk of acute kidney injury (AKI) is still high in the intensive care unit and a substantial number of patients requires renal replacement therapy to avoid life-threatening metabolic derangements.
Effect of the Intensity of Continuous Renal Replacement Therapy in Patients with Sepsis and Acute Kidney Injury: a Single-Center Randomized Clinical Trial

Abstract:INTRODUCTION:Acute kidney injury (AKI) is a major complication in patients with sepsis and is an independent predictor of mortality. However, the optimal intensity of renal replacement therapy for such patients is still controversial. METHODS:From 1 January 2004 to 30 September 2009, we randomly assigned 280 patients with sepsis and AKI to continuous renal replacement therapy by high-volume hemofiltration (50 mL/kg/h, HVHF) or extra high-volume hemofiltration (85 mL/kg/h, EHVHF). The primary study outcome was death from any cause within 28, 60 and 90 days. Results were analyzed by univariate and multivariate methods and by Kaplan-Meier survival curves. RESULTS:A total of 141 patients were given EHVHF and 139 were given HVHF. The two groups had similar baseline characteristics and received treatment for an average of 9.38 days (EHVHF group) and 8.88 days (HVHF group). There were no significant differences between the groups in number of deaths at 28, 60 or 90 days. There were also no differences between the groups in renal outcome of survivors at 90 days. Multivariate analysis indicated that inotropic support by norepinephrine, time in hospital of >7 days, blood platelet count <8 × 10(9)/L, Acute Physiological and Chronic Health Evaluation (APACHE) II score >25, total bilirubin >100 μmol/L, prothrombin time >18 s, serum creatinine <250 μmol/L and blood urea nitrogen >20 mmol/L were independent risk factors for death at 90 days after initiation of renal replacement therapy. CONCLUSIONS:In patients with sepsis and AKI, increasing the intensity of renal replacement therapy from 50 (HVHF) to 85 mL/kg/h (EHVHF) had no effect on survival at 28 and 90 days.

Abstract:BACKGROUND: Acute kidney injury (AKI) is common in patients with sepsis and may result in death. Systemic immune inflammation index (SII) is associated with kidney injury, but its predictive value for AKI in patients with sepsis remains unclear.OBJECTIVE: This study aimed to explore the predictive value of SII in sepsis patients with AKI.METHODS: From January 2020 to December 2022, 221 patients with sepsis treated in our hospital were retrospectively collected. The patients were divided into AKI group (n = 61) and control group (n = 160). Clinical characteristics and SII level were compared between the two groups, and the predictive value of SII for the occurrence of AKI was analysed.RESULTS: The SII level (724.72 ± 235.50 vs. 522.38 ± 205.62, p < 0.001), the serum procalcitonin level (8.13 ± 15.52 vs. 4.52 ± 10.34 µg/L, p < 0.001), and the acute physiology and chronic health evaluation II score (14.26 ± 2.90 vs. 11.62 ± 2.26, p < 0.001) significantly increased in the AKI group compared with the control group, whereas the albumin level significantly decreased (30.60 ± 5.41 vs. 32.49 ± 5.31 g/L, p = 0.019). The receiver operating characteristic curve showed that SII was valuable in predicting AKI in patients with sepsis, with an area under the curve of 0.733 (95% confidence interval: 0.657-0.810, p < 0.001). The continuous renal replacement therapy intervention rate (88.52% vs. 0.00%, p < 0.001), the intervention rate of vasoactive drugs (34.43% vs. 3.75%, p < 0.001), and the hospital mortality rate (16.39% vs. 2.50%, p < 0.001) significantly increased in the AKI group compared with the control group.CONCLUSIONS: AKI was associated with poor prognosis in patients with sepsis. SII, procalcitonin and acute physiology and chronic health evaluation II (APACHE II) score were valuable in predicting the occurrence of AKI. SII may serve as a new marker in patients with sepsis.

Abstract:Sepsis is a leading cause of acute kidney injury in clinical practice. The diagnosis of sepsis-induced acute kidney injury requires the diagnosis of sepsis and subsequent occurrence of acute kidney injury. The current definition for acute kidney injury is based on Scr and urine output, which is limited by the delayed identification of such patients. Numerous novel biomarkers have been found to be up-regulated in kidney injury, among which cystatin C and neutrophil gelatinase-associated lipocalin are the most studied. In the management of sepsis-induced acute kidney injury, early goal directed therapy may be potentially useful, but requires further validation in large clinical trials. It is well known that fluid overload is harmful in septic patients with established acute kidney injury and should be avoided. Renal replacement therapy is the mainstay treatment for the severe form of sepsis-induced acute kidney injury. However, there is still no consensus on the definition of timing and dosing in clinical practice, and the optimal timing and dosing are still unknown.

Abstract:Abstract Backgroud To explore whether monitoring of non-invasive urine oxygen tension (PuO2) for 3 consecutive days can reflect renal medulla microcirculation injury early and whether changes in PuO2 (△PuO2) have reliable early diagnostic value for sepsis-associated acute kidney injury (SA-AKI). Methods Twenty-four patients who were hospitalised for more than 48 h in our hospital and were diagnosed with sepsis were retrospectively divided into non-SA-AKI group and SA-AKI group according to the occurrence of acute kidney injury (AKI). The general and oxygen metabolism data of the two groups were compared. The results of urine analysis (PuO2, PuCO2, and urinary lactic acid) for 3 consecutive days were compared with common clinical kidney injury indices, and a changing trend was observed. Results In the PICU, a total of 24 patients with sepsis were included (14 males, 58.33%; mean age, 5 years), with 15 cases in the non-SA-AKI group and 9 cases in the SA-AKI group, with incidence of SA-AKI being 37.5%. PuO2 gradually decreased from D0 to D2 in the SA-AKI group; PuCO2 increased naturally. The variation trend of PuO2 was statistically significant in D1-D0, D2-D0, and D2-D1 (P<0.05), and the △PuO2 of D2-D0 (10.83±25.69 vs. -41.13±34.98, P=0.001). The early diagnostic value of PuO2 was significant (AUC=0.885, 95% CI: 0.704–1.000). Conclusions Three consecutive days of PuO2 monitoring can reflect early renal medullary microcirculation injury, and △PuO2 has early diagnostic value for SA-AKI.
Impact of central venous pressure on the mortality of patients with sepsis-related acute kidney injury: a propensity score-matched analysis based on the MIMIC IV database

Abstract:Background: Sepsis has long been a life-threatening organ dysfunction. Sepsis associated acute kidney injury (SA-AKI) is an important complication of sepsis, as an important hemodynamic index, the impact of central venous pressure (CVP) on sepsis patients needs to be explored. Thus this study aimed to investigate the relationship between CVP and the mortality of SA-AKI.Methods: Clinical data of adult patients with sepsis-related acute kidney injury, defined as met both the Sepsis 3.0 criteria and the Kidney Disease Improving Global Outcomes Clinical Practice Guideline (KDIGO) criteria, were obtained from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. The included cohort was divided into a high CVP and a low CVP group were determined based on the cuf-off value from receiver operating characteristic curve, with propensity score-matched analysis of the 28-day mortality for both groups and sensitivity analysis using inverse the probability-weighting model, multifactorial regression, and doubly robust estimation, patients acquired chronic coronary syndrome (CCS) and diabetes were also taken into consideration.Results: Of 1,377 patients with sepsis-related acute kidney injury, low CVP group (<13 mmHg) was 67.4% (n=928) and high CVP group (≥13 mmHg) was 32.6% (n=449). The two groups were matched 1:1 by propensity score to obtain a matched cohort (n=288). The mortality rates in the low versus high CVP group (19.4% vs. 34.7%) were statistically difference (odds ratio OR: 0.454; 95% confidence interval 0.263, 0.771). Moreover, the bistable analysis of logistic regression of the matched cohort (OR: 0.434; 95% CI: 0.244, 0.757), propensity score inverse probability weighting (IPW) (OR: 0.547; 95% CI: 0.454, 0.658), and multifactorial logistic regression (OR: 0.352; 95% CI: 0.127, 0.932) all yielded the same results.Conclusions: In patients with sepsis-related acute kidney injury, a lower CVP level (<13 mmHg) is an independent variable associated with decreased mortality. The threshold of CVP needs to be controlled in clinical work to improve the prognosis of patients with SA-AKI.

Abstract:Sepsis is the leading cause of acute kidney injury (AKI) in pediatric intensive care unit.Development of AKI during sepsis increased patient morbidity,predicts higher mortality and days of stay in the intensive care unit.The mechanisms behind AKI in sepsis remain controversial but were believed to be complex and multi-factorial.The pathophysiology of AKI in sepsis involved intrarenal hemodynamic changes,endothelial dysfunction,infiltration of inflammatory cytokines.The new markers of neutrophil gelatinase-associated lipocalin as the representative is helpful for early diagnosis of AKI.Renal replacement therapy (RRT)is the main treatment of sepsis related AKI.At present,the model,dose and exact timing of RRT is not well defined.A widely accepted viewpoint is that the injury stage of RIFLE diagnostic criteria and fluid overload up to 10％～ 20％ is the beginning of the most appropriate chance of RRT.

Abstract:Purpose: With the proper insights, measurement of central venous pressure (CVP) can be a useful clinical aid. However, the formal utility of CVP measurement on mortality in septic patients has never been proved.Methods: The Medical Information Mart for Intensive Care III (MIMIC-III) was applied to identify septic patients who had and did not have CVP measured. The primary outcome was 28-day mortality. The statistical approaches including multivariate regression, propensity score matching (PSM) and an inverse probability of treatment weighing (IPTW) and causal mediation analysis (CMA) were utilized to elucidate the relationship between CVP measurement and 28-day mortality.Results: A total of 10275 patients were included in our study, of which 4516 patients (44%) had CVP measured within 24 h after ICU admission. A significant beneficial effect of CVP measurement in terms of 28-day mortality was observed (OR 0.60 (95% CI 0.51–0.70; p<0.001)). Patients in CVP group received more fluid on day 1, had a shorter duration of mechanical ventilation and vasopressor use, and the reduction of serum lactate was higher than that in the no CVP group. The mediation effect of serum lactate reduction was significant for the whole cohort (p=0.04 for average causal mediation effect (ACME)) and patients in the CVP group with an initial CVP level below 8mmHg (p=0.04 for ACME).Conclusion: CVP measurement is associated with a lower risk-adjusted 28-day mortality among patients with sepsis, which is proportionally mediated through serum lactate reduction.

Abstract:Abstract Background Pediatric sepsis is a common and complex syndrome characterized by a dysregulated immune response to infection. Aberrations in the renin-angiotensin system (RAS) are factors in several infections of adults. However, the precise impact of RAS dysregulation in pediatric sepsis remains unclear. Methods Serum samples were collected from a derivation cohort (58 patients with sepsis, 14 critically ill control subjects, and 37 healthy controls) and validation cohort (50 patients with sepsis, 37 critically ill control subjects, and 46 healthy controls). Serum RAS levels on day of PICU admission were determined and compared with survival status and organ dysfunction. Results In the derivation cohort, the serum renin concentration was significantly higher in patients with sepsis (3678 ± 4746) than that in healthy controls(635.6 ± 199.8) (p < 0.0001). Meanwhile, the serum angiotensin (1-7) was significantly lower in patients with sepsis (89.7 ± 59.7) compared to that in healthy controls(131.4 ± 66.4) (p < 0.01). These trends were confirmed in a validation cohort. Non-survivors had higher levels of renin (8207 ± 7903) compared to survivors (2433 ± 3193) (p = 0.0001) and lower levels of angiotensin (1-7) (60.9 ± 51.1) compared to survivors (104.0 ± 85.1) (p < 0.05). A combination of renin, angiotensin (1-7) and procalcitonin achieved a model for diagnosis with an area under the receiver operating curve (AUROC) of 0.87 (95% CI: 0.81–0.92). Conclusion Circulating renin and angiotensin (1-7) have predictive value in pediatric sepsis.

Abstract:Background Sepsis is common and frequently fatal condition in critically ill patients and is a major cause of acute kidney injury (AKI). In this retrospective study, we sought to develop a comprehensive risk score model of sepsis associated-AKI (SA-AKI). Methods A total of 2617 patients were randomly assigned to a development (1554 patients) and a validation group (777 patients). The risk score model for SA-AKI was developed with multivariate regression analysis in development group and the model was further evaluated on validation group. Results We identified 16 independent predictors of SA-AKI in development group (age ≥ 60 years, hypertension/coronary heart disease, diabetes, chronic kidney disease, heart failure, chronic obstructive pulmonary disease, acute severe pancreatitis, hypotension, hypoproteinemia, lactic acidosis, the length of stay in intensive care unit(ICU), 60 g/L<hemoglobin < 90 g/L, hemoglobin ≤ 60 g/L, and ≥ 2 failed organs. This model had excellent performance characteristics in validation cohort( c statistic 0.857, 95% CI 0.839–0.874). Conclusion The novel risk score model for SA-AKI in ICU can identify patients at high risk to develop AKI. Application of this model could help clinicians to stratify patients for primary prevention, surveillance and early therapeutic intervention to improve care and prognosis of sepsis patients in ICU.
A New Prediction Model for Acute Kidney Injury in Patients with Sepsis

Abstract:Background: Acute kidney injury is common in patients with sepsis and contributes to poor prognosis and mortality. Early identification of high-risk patients can provide evidence for clinical decision-making. Methods: We developed a prediction model based on a cohort of 15,726 patients with sepsis from the Medical Information Mart for Intensive Care III critical care database. Logistic regression analysis was applied to develop a prediction model incorporating the selected risk factors. Discrimination and calibration of the prediction model were assessed using the C-index and calibration plot. Results: Risk factors in the prediction model included diabetes mellitus, chronic kidney disease, congestive heart failure, chronic liver disease, hyperbicarbonemia, hyperglycemia, low blood pH, prolonged clotting time, hypotension, and hyperlactatemia. The model showed great discrimination with a C-index of 0.711 (95% CI, 0.702-0.721) and appropriate calibration. A medium C-index value of 0.712 (95% CI, 0.697-0.727) could still be reached in the validation cohort. Negative and positive predictive values for the optimal cutoff value of >= 6 points were 56.8% and 72.3% in the training cohort and 57.3% and 72.6% in the validation cohort, respectively. Conclusions: This prediction model allows clinicians to quickly assess the risk of sepsis-associated acute kidney injury (SA-AKI) at an early stage. Accordingly, clinicians can implement more medical measures that are considered beneficial to patients with sepsis.
Association Between Admission Serum Procalcitonin and the Occurrence of Acute Kidney Injury in Patients with Septic Shock: A Retrospective Cohort Study

Abstract:Background: Procalcitonin (PCT) is an effective and sensitive diagnostic biomarker that can facilitate the early detection of infection and septicemia, but whether it can similarly be utilized to predict the development of acute kidney injury (AKI) in patients suffering from septic shock remains to be established. Herein, the relationship between serum PCT at admission and the onset of AKI in septic shock patients was thus evaluated following adjustment for other potential covariates. Methods: This was a retrospective cohort study of 303 septic shock patients treated in a Chinese hospital between May 2015 and May 2019. All patients in whom PCT levels were measured on admission and who did not exhibit AKI or chronic kidney disease at the time of admission were assessed for AKI development within one week following intensive care unit (ICU) admission as per the KDIGO criteria. The relationship between serum PCT at admission and AKI incidence was then assessed for these patients. Results: These 303 patients were an average of 64 years old, and were 59.7% male. Of these patients, 50.5% developed AKI within the first 7 days following ICU admission. A dully-adjusted binary logistic regression analysis revealed PCT levels at admission to be associated with AKI following adjustment for potential confounding factors (odds ratio (OR) = 1.01, 95%CI (1.01,1.02), p = 0.0007). Receiver operating characteristic curve analysis further indicated that a PCT cutoff level of 52.59 ng/ml at admission was able to predict the incidence of AKI with respective sensitivity and specificity values of 50% and 84%. Interaction analysis revealed no significant interactive relationship between PCT and AKI, suggesting that serum PCT levels represent an early predictor of AKI incidence in septic shock patients. Conclusions: Serum PCT at the time of admission can be used as a predictor of AKI in patients suffering from septic shock.

Abstract:SEPSIS is a complex clinical syndrome involving both infection and a deleterious host immune response. Actue Kidney Injury (AKI) is a frequent and serious complication of sepsis in Intensive Care Unitpatient (ICU). Despite recent impressive advances in the pathophysiology, diagnostic procedures, and appropriate therapeutic interventions in sepsis, sepsis-induced AKI fails to reduce the overwhelmingly high mortality rate and longer hospital stay. A major obstacle for effective treatment of sepsis-induced AKI is the relative lack of specific early and effective diagnostic tools. Current clinical Serum Creatinine (Scr) is unable to detect the early AKI, in the past ten decades, studies have identified several biomarkers for the early diagnosis of AKI, but only limited information exists concerning their values in the diagnosis of sepsis-induced AKI. In this article, we have made a comprehensive review to discuss the clinical value of biomarkers in the diagnosis of sepsis-induced AKI.
[Value of pancreatic stone protein/regenerating protein in severity evaluation and prognosis prediction for children with sepsis]

Abstract:Objective: To evaluate the value of pancreatic stone protein/regenerating protein (PSP/reg) in severity evaluation and prognosis prediction for children with sepsis. Methods: In this prospective case-control study, 159 children with sepsis (106 cases in the sepsis group; 53 cases in the severe sepsis group, including 12 cases of septic shock) and 20 children without sepsis (control group) were enrolled. ELISA was applied to measure plasma PSP/reg levels on days 1, 3, and 7 of admission to the PICU. The Spearman rank correlation test was applied to assess the correlations between plasma PSP/reg level and serum procalcitonin (PCT), CRP, WBC count, and pediatric critical illness score (PCIS). The area under the receiver operating characteristic curve (AUC) was used to assess the value of each index in determining severity and predicting prognosis for children with sepsis. Results: On day 1 of admission to the PICU, plasma PSP/reg levels in the sepsis and severe sepsis groups were significantly higher than in the control group (P<0.05), and the severe sepsis group had a significantly higher plasma PSP/reg level than the sepsis group (P<0.05). On day 1 of admission to the PICU, the survival group (n=132) had a significantly lower plasma PSP/reg level than the non-survival group (n=27) (P<0.05). On day 1 of admission to the PICU, plasma PSP/reg level in children with sepsis was positively correlated with WBC count and serum PCT level (rs=0.212 and 0.548, respectively; both P<0.05), and negatively correlated with PCIS score (rs=-0.373; P<0.05). The AUCs of plasma PSP/reg level and serum PCT for determination of severe sepsis, septic shock, and death were higher than 0.7 (P<0.05). Conclusions: PSP/reg is closely related to infection, and has a certain clinical value in risk stratification of sepsis and prognosis evaluation.

Abstract:Background: Sepsis-associated acute kidney injury (SA-AKI) is associated with high morbidity, with no current therapies available beyond continuous renal replacement therapy (CRRT). Systemic inflammation and endothelial dysfunction are key drivers of SA-AKI. We sought to measure differences between endothelial dysfunction markers among children with and without SA-AKI, test whether this association varied across inflammatory biomarker-based risk strata, and develop prediction models to identify those at highest risk of SA-AKI. Methods: Secondary analyses of prospective observational cohort of pediatric septic shock. Primary outcome of interest was the presence of ≥ Stage II KDIGO SA-AKI on day 3 based on serum creatinine (D3 SA-AKI SCr). Biomarkers including those prospectively validated to predict pediatric sepsis mortality (PERSEVERE-II) were measured in Day 1 (D1) serum. Multivariable regression was used to test the independent association between endothelial markers and D3 SA-AKI SCr. We conducted risk-stratified analyses and developed prediction models using Classification and Regression Tree (CART), to estimate risk of D3 SA-AKI among prespecified subgroups based on PERSEVERE-II risk. Results: A total of 414 patients were included in the derivation cohort. Patients with D3 SA-AKI SCr had worse clinical outcomes including 28-day mortality and need for CRRT. Serum soluble thrombomodulin (sTM), Angiopoietin-2 (Angpt-2), and Tie-2 were independently associated with D3 SA-AKI SCr. Further, Tie-2 and Angpt-2/Tie-2 ratios were influenced by the interaction between D3 SA-AKI SCr and risk strata. Logistic regression demonstrated models predictive of D3 SA-AKI risk performed optimally among patients with high- or intermediate-PERSEVERE-II risk strata. A 6 terminal node CART model restricted to this subgroup of patients had an area under the receiver operating characteristic curve (AUROC) 0.90 and 0.77 upon tenfold cross-validation in the derivation cohort to distinguish those with and without D3 SA-AKI SCr and high specificity. The newly derived model performed modestly in a unique set of patients (n = 224), 84 of whom were deemed high- or intermediate-PERSEVERE-II risk, to distinguish those patients with high versus low risk of D3 SA-AKI SCr. Conclusions: Endothelial dysfunction biomarkers are independently associated with risk of severe SA-AKI. Pending validation, incorporation of endothelial biomarkers may facilitate prognostic and predictive enrichment for selection of therapeutics in future clinical trials among critically ill children.
