Abstract:Exercise-induced tachycardia-dependent atrioventricular block with a normal electrocardiogram at rest is rare. Herein, we present a case of a 65-year-old woman with exercise-related chest suppression and treadmill exercise test-induced second- and third-degree atrioventricular blocks with narrow QRS wave and normal resting electrocardiogram. High atrioventricular block leads to tissue and organ insufficiency, resulting in exercise intolerance, dyspnea, dizziness, and syncope. Ths diagnosis was exercise-induced high-degree atrioventricular block. For lack of effective medicines, the patient received a permanent dual chamber pacemaker to ensure atrioventricular sequential pacing during exercise. No exercise-related discomfort occurred during follow up.

Abstract:A case of torsade de pointes (TdP) with complete atrioventricular block and pacemaker failure that was misdiagnosed as epilepsy is presented herein. An 82-year-old female with recurrent seizure-like attacks showed epileptiform discharge during an electroencephalogram recording. A long QT interval and severe hypokalemia induced runs of TdP, which was related to pacemaker lead fracture, was detected during Holter recording and accompanied with episodes of seizures. After a DDD pacemaker with a new ventricular lead was replaced, there was no recurrence of any seizure-like attacks. Bradycardia-mediated TdP associated with complete atrioventricular block should not be missed in patients with recurrent seizure-like attacks even after pacemaker implantation.

Abstract:Background: Widely split P waves in sinus rhythm have been reported previously. However, widely split P' waves in focal atrial tachycardia (AT) on a surface electrocardiogram (ECG) have rarely been reported. The electrophysiological mechanism is relatively difficult to clarify, requiring a electrophysiological study. Case summary: A 67-year-old patient, who had undergone two radiofrequency ablations for atrial fibrillation, presented with recurrent palpitation. During the palpitation episode, the 12-lead ECG showed AT with a 3:1 atrioventricular conduction rate. P' waves were markedly prolonged in duration and widely split in morphology. An electrophysiological study showed that the tachycardia arose from the left atrial appendage (LAA) and was conducted through two distinct pathways. The impulse of one pathway was transmitted solely via the superior part of the atrium, including the Bachmann bundle. The second pathway was conducted via the coronary sinus and transmitted the impulse from the LAA to the ventricle. After the site showed that the earliest activation was ablated, repeated intravenous infusion of isoprenaline and programmed atrial stimulation did not induce tachycardia. Conclusion: Widely split P' waves in AT indicate intra- and interatrial conduction blocks, which can be easily overlooked due to the presence of low-voltage areas. Therefore, an electrophysiological study is crucial for identifying the origin of the tachycardia and elucidating the mechanistic details.

Rare Presentation of Wide QRS Tachycardia in a Patient in Their 40s

Abstract:This article describes the case of a 40-year-old individual who presented with fulminant myocarditis. Initial ECG displayed sinus tachycardia with a heart rate of 117 bpm, QS complexes in leads V1-V3, ST-segment depression in leads II, III, aVF, V5-V6, and ST-segment elevation >0.2 mV in leads V1 through V3. The initial clinical assessment suggested an acute anteroseptal myocardial infarction. However, subsequent diagnostic evaluation through coronary angiography disclosed that the coronary arteries were normal. Therefore, clinicians should carefully consider the differential diagnosis between these conditions, as their management strategies differ markedly. Two hours after admission, the patient unexpectedly developed syncope. The ECG findings were consistent with the typical characteristics of bidirectional ventricular tachycardia. Our report described the appearance and morphology as well as mechanism of bidirectional ventricular tachycardia in detail. Additionally, we delineate differential diagnoses for disease that can cause bidirectional ventricular tachycardia, such as aconite poisoning, digoxin overdose, immune checkpoint inhibitor (ICI), myocardial ischemia, and hereditary channelopathies, such as catecholaminergic polymorphic ventricular tachycardia (CPVT) and Andersen-Tawil syndrome. Therefore, clinicians should recognize this ECG finding immediately and initiate appropriate treatment promptly as these measures may be vital in saving the patient's life.

Abstract:A woman in her 50s presented to the emergency department in a comatose condition. One hour prior to presentation, she had consumed 50 tablets each of doxepin (25 mg/tablet) and amitriptyline (25 mg/tablet) as a suicide attempt due to depression. She had no history of cardiovascular disease but had been diagnosed with depression in the previous year. Her blood pressure was 115/75 mm Hg, and heart rate was 126 bpm. Her oxygen saturation level was 97% on room air. Abstract:Clinical introduction A man in his 50s presented to the emergency department with palpitations. He had a background of arrhythmic right ventricular cardiomyopathy. He had previously undergone the placement of a dual-chamber implantable cardioverter-defibrillator for atrioventricular block and ventricular tachycardia (VT). Abstract:Introduction: It is rare to find that wide QRS tachycardia automatically changes to narrow QRS tachycardia, and it is more difficult to clarify the mechanism. Case Report: A 3-month-old infant with recurrent paroxysmal supraventricular tachycardia underwent transesophageal cardiac electrophysiological examination. The wide QRS tachycardia was induced by atrial RS 2 stimulation, and it soon changed to narrow QRS tachycardia automatically. By the accurate measurement of esophageal lead, it was found that the electrocardiogram changes completely conform to Coumel law. The mechanism of wide and narrow QRS tachycardia was orthodromic atrioventricular reentrant tachycardia with or without ipsilateral functional bundle branch block, and the accessory pathway was defined as the left free wall-concealed accessory pathway. Conclusion: Transesophageal cardiac electrophysiological examination can reveal some special electrophysiological phenomena, and its non-invasive nature is especially suitable for infants.

Abstract:A middle-aged man with no previous cardiac history was admitted to the hospital being treated for thigh cellulitis, during his stay he developed palpitations and tachycardia which on initial ECG showed atrial flutter with a 2:1 AV block and evidence of an accessory pathway. He was subsequently given AV nodal blocking agents in the form of beta-blockers (bisoprolol) to slow his heart rate down; unfortunately, this led to hemodynamic instability due to 1:1 conduction of the atrial flutter down the accessory pathway. This case report demonstrates the importance of recognising pre-excitation on an ECG and the potential adverse effect of administering AV nodal blockade.

Abstract:A case of ventricular noncompaction with Giant P waves and focal atrial tachycardia is presented. A 36-year-old man was admitted to our hospital because of palpitations and a progressive worsening of heart failure. A 12 lead rest electrocardiogram showed large positive waves followed by smaller negative waves in limb leads and lead V-1 which seemed to represent QRS complexes followed by retrograde P waves at first glance. Electrocardiogram revealed supraventricular tachycardia when palpitations occurred in this patient without any obvious triggers. Intravenous administration of amiodarone decreased the ventricular response by depression of conduction across the AV node which confirmed the diagnosis of focal atrial tachycardia. Abstract:A 53-year-old male patient with dextrocardia and situs viscera inversus, a congenitally corrected transposition of the great arteries (c-TGA), and atrial septum defect (ASD), secundum type, post patch repair was referred to our laboratory for treatment of frequent episodes of arrhythmia. In the previous fourteen years, the patient had experienced multiple supraventricular tachycardias (SVTs), which were successfully ablated 14, 12, and 2 years prior, respectively. This time in the electrophysiology laboratory, a narrow QRS tachycardia (cycle length=380 msec) was induced by right ventricular and right atrial incremental pacing with concentric activation. A bi-atrial activation map was constructed using a NavX system (St. Jude Medical, St. Paul, MN, USA) via a transseptal procedure to approach the systemic left atrium, which showed a centrifugal activation from a focal origin with mapped cycle length 121 ms (32% of the tachycardia cycle length) on the systemic high left atrial septum where a large low voltage zone was observed. The bipolar electrograms at the origin of the focal atrial tachycardia (AT) demonstrated fractionated and low-amplitude potentials. Successful ablation was carried out at the AT origin resulting in the non-inducibility of the AT. No recurrence of the tachycardia was observed during one year of follow-up.

Abstract:This report describes a patient with atrial tachycardia who underwent radiofrequency catheter ablation. Two applications of radiofrequency energy were received at different sites within 1 week. Initially, right atrium mapping indicated that the earliest activation site was located at the His-bundle region (HBR). However, the first application of radiofrequency near HBR appeared to be ineffective. Moreover, a degree III atrioventricular block and inferior ST-segment elevation was temporally observed during the therapy, which recovered after drug treatment. As later aortic sinus mapping showed that the activation site located at the noncoronary sinus (NCS) preceded the one at HBR, and this activation terminated at atrial tachycardia, a second application of radiofrequency was performed at NCS without any complications. In addition, a literature review of the clinical features of atrial tachycardia at the NCS is also summarized in the present report.

Abstract:Background: Dextrocardia is a congenital heart malformation with a low incidence that occurs in only 1 in 10,000–12,000 people. Wolff-Parkinson-White (WPW) syndrome is a congenital condition with additional accessory pathways between the atria and the ventricle, which affects up to three in 1,000 people worldwide. Experience of radiofrequency catheter ablation in patients with WPW syndrome and dextrocardia is scarce due to its rare incidence. Case presentation: A 39-year-old female was hospitalized due to two episodes of palpitations in the latest 2 months. The morphology of the P-QRS-T complex of lead aVR and aVL, II, and III were presented invertedly as common conditions, and shortened P-R interval and a characteristic "delta" wave were shown on the electrocardiogram (EGM). The patient with dextrocardia and situs invertus malposition was confirmed by chest-X ray, cardiac color Doppler echocardiography. The patient was diagnosed with WPW syndrome with dextrocardia and underwent radiofrequency catheter ablation (RFCA) successfully. In this case, the key to the success of RFCA is to understand the anatomical structure of the heart and the great vessels before the operation and make a personalized operative plan. Conclusion: Catheter ablation for tachycardia patients with dextrocardia is efficient and safe. For patients with dextrocardia, the key to successful ablation was adjusting for projection angulation and different catheter manipulation compared with a standard case because of the mirror image of a normal heart.

Abstract:A 75-year-old man was admitted due to an electrical storm with appropriate recurrent implantable cardioverter defibrillator (ICD) discharges. The patient had had an extensive anterolateral myocardial infarction with associated severe left ventricular dysfunction 10 years earlier (left ventricular ejection fraction, 25%), and an ICD was placed 9 years before admission for primary prevention of sudden cardiac death. A first invasive study induced up to five ventricular tachycardias and an extensive endocardial substrate ablation was performed. Despite intravenous β-blockers, general anesthesia and procainamide infusion, the patient continued to have recurrent episodes of very slow sustained ventricular tachycardia with a right bundle branch block pattern. In a subsequent invasive study, no mid-diastolic activity was found despite careful mapping during the induced clinical ventricular tachycardia and ablation attempts inside the apical endocardial scar were unsuccessful. A percutaneous epicardial approach with navigation system support (EnSite Precision<sup>TM</sup> Cardiac Mapping System v. 2.0, St. Jude Medical, St. Paul, MN, USA) without antiarrhythmic infusion was planned. A wide QRS complex rhythm with alternating QRS morphology was readily induced by epicardial ventricular pacing trains (Fig. 1, top) that elicited both arrhythmia QRS patterns with very long stimulus QRS intervals (Fig. 1, bottom). What is the possible mechanism of this arrhythmia? Do we need further pacing maneuvers during the arrhythmia to localize critical sites at which ablation pulses can predictably be successful?
