Abstract:Employment has historically provided a person with value and the ability to live a self-determined and independent life. Interagency collaborations have been found to play a key role in promoting employment for those with special needs. However, there is little research on identifying the needs of the field for collaborations between vocational rehabilitation and educational agencies. We conducted a series of eight focus groups in a large state in the United States and identified needs from educators and vocational rehabilitation counselors. Participants regularly noted that they needed increased collaboration and a need for additional training and education for all stakeholders. They also identified many barriers to this increased collaboration and training, including access to the community that they served, limitations on time for training, collaboration, and access, challenges in communication, including language limitations and trust, and limitations around the support that they received. On the other hand, they often felt supported by their administration, and, when collaboration was possible, their co-workers and external providers served as great resources.

Career Development and Transition for Exceptional Individuals

Abstract:Family involvement in transition planning has been associated with improved outcomes for youth with disabilities. However, limited research exists on providers' in-depth views regarding family-professional collaborations in pre-employment transition services (Pre-ETS). The present qualitative study examined the perspectives of Pre-ETS providers on the challenges and assistance provided to students with disabilities during their transition from high school to postsecondary education or work, with a specific focus on family-professional collaborations. We conducted 24 in-person focus groups across six regions in Texas with 162 Pre-ETS providers (i.e., vocational rehabilitation counselors and transition educators). We used thematic analysis to analyze the data, which unveiled three themes about barriers and facilitators to family-professional collaborations: The Need to Support Family Directly, The Need to Promote Family-Professional Communication, and The Need to Promote Community Support. Findings underscore the imperative for Pre-ETS providers to acknowledge and address varying familial emotions, feelings, and expectations regarding their children's employment, influenced by diverse cultural backgrounds. We discuss limitations and recommendations for future research and practice.
Crossing the Age Divide: Cross-Age Collaboration Between Programs Serving Transition-Age Youth

Abstract:Programs that serve transition-age youth with serious mental health conditions typically reside in either the child or the adult system. Good service provision calls for interactions among these programs. The objective of this research was to discover programmatic characteristics that facilitate or impede collaboration with programs serving dissimilar age groups, among programs that serve transition-age youth. To examine this “cross-age collaboration,” this research used social network analysis methods to generate homophily and heterophily scores in three communities that had received federal grants to improve services for this population. Heterophily scores (i.e., a measure of cross-age collaboration) in programs serving only transition-age youth were significantly higher than the heterophily scores of programs that served only adults or only children. Few other program markers or malleable program factors predicted heterophily. Programs that specialize in serving transition-age youth are a good resource for gaining knowledge of how to bridge adult and child programs.

Abstract:Young children with special needs and their families often experience transitions across multiple environments in the early childhood years. Many transitions are identified as stressful for children and families. In the present study, a series of focus groups involving administrators, practitioners, and family members were held around the United States to identify transition practices that have been implemented effectively for children, families, staff, administrators, and communities. Two major themes emerged from the data: critical interagency variables, defined as strategies that support an interagency process involving multiple parties; and transition practices and activities, defined as practices that address child, family, staff, program, and community-specific activities. Outcomes from these focus groups included identification of transition strategies for young children with special needs that are considered valuable by parents, providers, and administrators and that are consistent with theoretical frameworks described in the transition literature.

Promoting school to work transition for students with emotional/behavioral disorders

Abstract:Background: The successful transition of students from high school to adult occupations, including work and independent living, has been a major focus of the educational reform movement in the United States. However, despite good intentions, students with disabilities have less favorable transition outcomes than do students in the general population and employment opportunities are even less promising for students with emotional/behavioral disorders (EBD). Objective: The purpose of this model demonstration project, funded by the US Department of Education, was to develop a program based on identified best practices to facilitate successful school to work transition for students with EBD. Participants: 47 students with EBD between the ages of 14 and 19 were participants in this project. The majority of participants in the project were white males with a generic diagnosis of a disruptive personality disorder. Methods: Five cohort groups of students participated in a four phase program; Phase 1 - School-based Learning, Phase 2 - Community Involvement, Phase 3 - Paid, Supported Work Experience, and Phase 4 - Competitive Employment and Follow-Up. Results: At the conclusion of the project, 30% of the project participants were competitively employed. Students had the most difficulty in Phase 3, but 81% of those who successfully completed Phase 3 obtained and retained competitive employment for a least one year. Four case studies are presented to illustrate findings. Conclusions: The School to Work Transition Program model shows promise for providing interventions to promote successful transition for students with EBD. Further research on the effectiveness of the overall model, as well as specific components of the model, is needed.

Abstract:Barriers to seamless service delivery between workforce development and mental health systems of care have kept both entities from maximizing their potential in regards to employment for job seekers with mental illness who are capable of work and seeking employment. Using a multiple case study design, this study examined the nature of collaboration between workforce development and mental health systems to understand the policies and practices in place to assist individuals with mental illness to find and keep work. The paper presents innovative strategies that involved staff from both workforce development and mental health agencies. Findings from this research identified the following collaborative strategies: (a) the creation of liaison positions and collaborative teams; (b) staff training on mental health and workforce issues; and (c) multi-level involvement of individuals with mental illness. Implications for workforce professionals are offered as a way to stimulate implementation of such strategies.

An Assessment of Collaborations Between Centers for Independent Living and Vocational Rehabilitation Agencies Around Transition-Age Youth

Abstract:Centers for independent living (CILs) and vocational rehabilitation (VR) agencies offer transition services that many youth with disabilities could benefit from, but little is known about the existence of and potential for collaborations between these agencies. In this study, we examined data from VR agencies to assess their relationships with CILs involving transition-age youth at the national and state levels, focusing on two types of collaborations: referrals from CILs to VR agencies and VR youth's use of CIL services. Although many CILs and VR agencies do work together, their relationships vary, and these collaborations reflect only a small fraction of VR youth who are transition age. Further, for each collaboration type, the VR youth involved in these collaborations differ from other people who apply for or use VR services in terms of their characteristics and service use patterns. The findings point to opportunities for greater collaboration between CILs and VR agencies. They also indicate a need for more research to understand the optimal formats and contexts for such collaborations and the criteria for effectively promoting them at the person and agency levels.

Abstract:In the United States, families of young children with developmental disabilities may encounter many challenges and obstacles when advocating for their child’s developmental services and supports. Research has highlighted that the transition from early intervention services to school-based services is especially confusing and difficult for many families. To limit confusion, the context and procedures of the transition must be clearly understood by all parties involved, including the family, early intervention providers, and school personnel. This exploratory qualitative pilot study highlights different practices used in a transition classroom for young children with developmental disabilities and the experiences of those involved in the transition process. The transition classroom was created through a collaboration between a non-profit early intervention organisation and a public Early Childhood Special Education Program. A teacher, the director of the non-profit organisation, and a parent of a child in the transition classroom were interviewed. Implications emphasising effective partnerships between families, schools, and community organisations, and how to potentially build those partnerships are discussed.

Abstract:Effective transition programming is a crucial component in ensuring that individuals with disabilities receive an appropriate education. The current study addressed transition practices as viewed by special education lead teachers and district-level transition personnel in South Carolina. These individuals reported their various responsibilities. They also reported that teacher, parent, and student participation in transition planning meetings ranged from 82% to 89% and that the majority of schools offered school-based work experiences as well as an array of other school-based functional learning opportunities. Transition personnel rated their district's transition services better than the lead teachers did. The study also found that schools addressed certain areas of the transition process better than others. Several areas of transition services were not adequately addressed and need to be strengthened, including supported employment, advocacy and legal services, medical services management, community residential life, and approaches to providing life skills and functional skills instruction in relationship to state standards.

Abstract:Students with sensory impairments comprise a heterogeneous group that is often underserved by rehabilitation and other social services. This underserved status has been attributed to poor interagency collaboration and eligibility issues, the low incidence nature of these disabilities, and the shortage of trained professionals to address the specialized needs of these individuals. This article examines important educational and vocational characteristics of this population to assist rehabilitation professionals in supporting more effective transitions into employment and other adult roles. Specific transition considerations include unique learning challenges, assessment issues, assistive technology, and accommodations. The article concludes with strategies for transition planning, community integration, and evaluating transition outcomes.

Abstract:Employment has been identified as a critical need for individuals with disabilities (IWDs), given that it is an essential component of quality of adult life (Rogan, Grossi, & Gajewski, 2002).Nevertheless, despite the extensive support by federal legislation, public policy, federal initiatives and programs, the employment outcomes for IWDs are still disappointing and they continue to encounter employment problems such as lower employment rates and lower annual earnings.For example, people with disabilities have experienced significantly lower employment rates than those without a disability.According to findings from the NLTS2 (Newman, Wagner, Knokey, Marder, Nagle, Shaver, & Wei, 2011), the approximate rate of employment, at the time of the study, for individuals with disabilities ages 18-24 was 63% and was impacted heavily by the identified disability label and level of training/education.The Bureau of Labor Statistics (2022) reports 19.1% of people with disabilities were employed compared to 63.7% of their peers without a disability.Research has shown that long-term career outcomes for individuals with disabilities can drop to a rate as long as 17.2% (Baker, Lowrey, & Wennerlind, 2018).Research on evidence-based practices indicated that early employment experiences, supported internships, and collaboration are effective in preparing and supporting IWDs for their transition to employment (Bellman, Burgstahler, & Ladner, 2014;Lee & Carter, 2012).Longitudinal data demonstrates that schools can improve employment outcomes for youth with disabilities through job readiness instruction (Park, Bouck, & Duenas, 2020).

Abstract:Improving adolescent to adult health care transitions (HCTs) would benefit all young people in the United States but has particular importance for the more than 120,000 adolescents and young adults (AYA) placed in foster care. Leveraging clinical guidelines and evidence-based interventions, HCT efforts inclusive of and specific to AYA placed in foster care can be strengthened through greater collaboration between child welfare, health care, and other AYA-serving systems. This Health Policy Brief advances the importance of structured Health Care Transition services for adolescents and young adults placed in foster care. Additionally, it highlights opportunities for health care providers, youth-serving systems, and health policy stakeholders to collaborate with child-welfare involved young people in order to prioritize evidence-based health care transition interventions. Abstract:The homeless population in the US has dramatically increased in the past two decades. People who are homeless often lack skills sets such as stress management and social skills, independent living skills, and skills for vocational and leisure engagement. Best practice vocational education and training programs for individuals who are homeless recognize that success in the worker role often hinges on a person's capacity to manage day-to-day living. Life skills and pre-employment training are essential components of vocational programs but no more important than interpersonal skill development including anger management, developing self esteem and motivation, building goal setting capacity, and skills for money management, personal budgeting and self-advocacy. These areas of performance have all been traditionally included in occupational therapy's domain of practice. This article describes Project Employ, a grant funded supportive employment program that has grown out of collaboration between Duquesne University's Department of Occupational Therapy and Bethlehem Haven, an emergency shelter and residential recovery program and primary service provider for homeless people in Pittsburgh, Pennsylvania. The purpose of this article is to describe the history, structure and outcomes of Project Employ.

Abstract:Access to content and services online is increasingly important for everyone, including people with disabilities. National commitments, including the Americans with Disabilities Act, and international resolutions, including the United Nations Declaration of the Rights of Persons with Disabilities, call for work to ensure that people with disabilities can participate fully in the online world. Gains in education, employment and health, as well as in civic engagement, social participation, and personal independence will follow from enhanced inclusion online. Research in many areas of computer science, including recognition technology, natural language processing, personalization, software architecture, and others, is needed to secure these benefits. Organizing this research calls for partnerships among academic researchers, federal agencies, and commercial organizations, as well as effective division of labor and cooperation between computer scientists, behavioral scientists, advocacy groups, and consumers.

Abstract:Background: Adults with intellectual disabilities have high rates of unemployment and underemployment. Despite benefits to employers and employees, some groups may be hesitant to implement inclusive employment programs due to lack of knowledge, absence of well-defined strategies, and limited exposure to successful examples. Objective: To address this gap, the Down Syndrome Program (DSP) in a New England tertiary pediatric hospital established an inclusive employment program that supports and trains young adults with Down syndrome in the development of foundational job skills within a hospital-based clinic. Methods: This case study examines strategies and lessons learned from the employment program's implementation and evolution. Results: Successful implementation required iterative, tailored approaches to meet diverse needs. Conclusion: The DSP developed a framework and collection of best practices for other organizations to adopt for successful employment of individuals with disabilities under an inclusive employment model.

Abstract:Successful job search results from job seekers' well-shaped social communication. While well-known differences in communication exist between people with autism and neurotypicals, little is known about how people with autism collaborate with their social surroundings to strive in the job market. To better understand the practices and challenges of collaborative job seeking for people with autism, we interviewed 20 participants including applicants with autism, their social surroundings, and career experts. Through the interviews, we identified social challenges that people with autism face during their job seeking; the social support they leverage to be successful; and the technological limitations that hinder their collaboration. We designed four probes that represent major collaborative features found from the interviews--executive planning, communication, stage-wise preparation, and neurodivergent community formation--and discussed their potential usefulness and impact through three focus groups. We provide implications regarding how our findings can enhance collaborative job seeking experiences for people with autism through new designs.

Abstract:Transition policy in the United States has endorsed work-based learning experiences, including internships, for students with intellectual and developmental disabilities (IDD) in order to combat poor rates of post-school employment. Workforce Innovation and Opportunity Act (WIOA) state plans have reported use of internships as a way to provide Pre-Employment Transition Services (Pre-ETS). Internships are complex programs that vary in terms of length, payment, funding, and collaborative partnerships, and are intended to provide an educational experience for students to develop vocational skills. The purpose of this scoping review was to examine the impact of internship participation on employment for youth and young adults (aged 16 to late 20s) with IDD. Systematic searches of electronic databases containing peer-reviewed sources were conducted. Nineteen articles were included in the final sample. Findings identified specific internship programs that have been examined within the peer-reviewed literature base, the level of evidence each provides concerning the impact on employment outcomes, and pertinent internship characteristics observed across studies. Implications for future research and practice are discussed.

Abstract:Background: Underserved and unserved people are individuals who lack necessary health services. They can be found in both high- and low-income countries with disabilities in areas such as speech, language, hearing, swallowing, physical, and neurological, as well as other health problems. In addition, academic programs that prepare students to serve underserved or unserved communities do not exist in some countries. Collaborative projects allow those with expertise to share their knowledge, strategies, methods, and technologies with health practitioners, administrators, educators, students, and families. The goal is to assist populations who require special services. Collaborative projects can be interdisciplinary, international, or involve academic institutions or organizations. It is also essential that collaboration projects consider the strengths and assets that are present in the environment and in the population. This asset-based approach supports sustainability as long-term solutions can take advantage of existing strengths. Sustainability is essential so that the goals of projects can continue to contribute to a population. Objectives: The primary objective of this article is to present collaborative projects that positively impact underserved and unserved populations. The strengths of these projects are also presented as a model for further work. Summary: This article presents several collaborative projects, along with the impact of these efforts.
Appealing Features of Vocational Support Services for Hispanic and non-Hispanic Transition Age Youth and Young Adults with Serious Mental Health Conditions

Abstract:Transition age youth and young adults (TAYYAs) diagnosed with serious mental health conditions (SMHCs) are at greater risk of being unemployed compared to their peers without SMHCs. Job counseling and job placement services are the greatest predictor of competitive employment, yet we have limited knowledge about what TAYYAs believe they need to obtain gainful employment. In person, qualitative interviews were conducted with 57 non-Hispanic and Hispanic TAYYAs with SMHCs enrolled in three vocational support programs in MA (Vocational Rehabilitation, Individual Placement and Support; the Clubhouse Model as described by the International Center for Clubhouse Development). Six themes emerged from the data: three themes were identified as social capital (supportive relationships, readily available workplace supports, and vocational preparation), two themes related to human capital (effective educational supports and work experience), and one theme related to cultural capital (social skills training). Unique features (Spanish-speaking staff and/or familiar in Latino culture, familial-like staff support) were frequently noted by Hispanic TAYYAs.

Abstract:Youth and young adults (YYA) with intellectual and developmental disabilities (IDD) have high rates of co-occurring mental health (MH) conditions. The time during transition from pediatric to adult health and mental health care can be a very challenging, with risk of loss of services leading to poor outcomes. This study aimed to explore barriers to transition from pediatric to adult health and mental health care and services for individuals with IDD and co-occurring MH conditions, by eliciting the view of stakeholders, including disability advocates. Qualitative analysis was conducted using grounded theory, and themes were coded based upon the social-ecological model (SEM). We generated themes into multiple levels: the individual level, the family level, the provider level, the systems of care level, and the societal level. Stakeholders expressed a critical need to improve coordination between systems, and to increase provider availability to care for YYA with IDD and co-occurring MH conditions.
