Predictive value of pre-pregnancy BMI and first trimester weight gain on the risk of gestational diabetes mellitus

Abstract:Objective To investigate the predictive value of pre-pregnancy body mass index(BMI) and weight gain in early pregnancy on the risk of gestational diabetes mellitus(GDM).Methods A total of 386 pregnant women who underwent prenatal examination in Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from September 2017 to March 2018 were selected.All pregnant women received 75g oral glucose tolerance test(OGTT) at 24-28 weeks of gestation, and were divided into GDM group(n=165) and non-GDM group(n=221),according to the OGTT results.Relevant clinical data were collected to compare and analyze the factors related to the occurrence of GDM in pregnant women and the predictive value of weight gain in early pregnancy on the occurrence of GDM.Results Among the 386 pregnant women, 42.75%(165/386) were diagnosed with GDM and 57.25%(221/386) were not diagnosed with GDM.There were no significant differences in ages and height between the two groups(both P>0.05).Family history of diabetes mellitus(first degree relatives) distribution, pre-pregnancy BMI,early pregnancy weight gain, early pregnancy fasting blood glucose, early pregnancy fasting insulin and insulin resistance(HOMA-IR) in GDM group were higher than those in non-GDM group, and the differences were statistically significant(χ~2=5.966,t=3.690-10.322,respectively, P<0.05).Binary Logistic regression analysis showed that family history of diabetes mellitus(first degree relatives) distribution, pre-pregnancy BMI and early pregnancy weight gain were independent risk factors for the occurrence of GDM(OR=4.674,1.187,1.157,respectively, P<0.05).ROC analysis showed that the diagnostic cut-off values of weight gain in early pregnancy predicting the occurrence of GDM were 2.55kg and 1.90kg, respectively, in pregnant women with normal prepregnancy BMI and overweight and obese prepregnancy(the Yoden index were 0.211,0.292 respectively, the sensitivities were 0.480,0.536 respectively, the specificities wre 0.731,0.756 respectively).Conclusion Pre-pregnancy BMI and early pregnancy weight gain are closely related to the risk of GDM in pregnant women.Therefore, timely prevention and intervention measures should be given to pregnant women who gain too much weight during pregnancy preparation or early pregnancy examination.
