Abstract:AbstractDisease relapse remains the major clinical challenge in treating T-cell acute lymphoblastic leukemia (T-ALL), particularly those with PTEN loss. We hypothesized that leukemia-initiating cells (LIC) are responsible for T-ALL development and treatment relapse. In this study, we used a genetically engineered mouse model of Pten−/− T-ALL with defined blast and LIC-enriched cell populations to demonstrate that LICs are responsible for therapeutic resistance. Unlike acute and chronic myelogenous leukemia, LICs in T-ALL were actively cycling, were distinct biologically, and responded differently to targeted therapies in comparison with their differentiated blast cell progeny. Notably, we found that T-ALL LICs could be eliminated by cotargeting the deregulated pathways driven by PI3K and Myc, which are altered commonly in human T-ALL and are associated with LIC formation. Our findings define critical events that may be targeted to eliminate LICs in T-ALL as a new strategy to treat the most aggressive relapsed forms of this disease. Abstract:Background: Acute lymphocytic leukemia (ALL) is a common blood cancer which induces high mortality in children. Bromodomains and extra-terminal (BET) protein inhibitors, such as JQ1 and ARV-825, are promising cancer therapeutic agents that can be used by targeting c-Myc. A recent work reported that JQ1 effectively attenuates ALL in vitro by suppressing cell proliferation and accelerating apoptosis. The purpose of this research was to probe into the potential mechanism of how JQ1 inhibits ALL cell proliferation in vitro. Material/Methods: Cell viability of ALL cells were measured by CTG after treatment by JQ1. Cell cycle analysis was done by EdU and PI staining. Cell apoptosis was assessed by Annexin V/PI staining. Glycolysis was detected using Seahorse and LC-MS kits. The expression of glycolytic rate-limiting enzymes was assessed by RNA-seq, qRT-PCR, and Western blot. Results: JQ1 suppressed cell proliferation by arresting the cell cycle and inducing the apoptosis of acute lymphocytic leukemia cells. JQ1 inhibited cell proliferation of B-ALL cells by restraining glycolysis. Conversely, the cell cycle block of B-ALL cells induced by JQ1 was partially abolished after pretreatment with 2-Deoxy-D-glucose (2-DG), an inhibitor of glycolysis. Furthermore, JQ1 restrained the glycolysis of B-ALL cell lines by remarkably downregulating the rate-limiting enzymes of glycolysis, such as hexokinase 2, phosphofructokinase, and lactate dehydrogenase A. Moreover, the cell cycle arrest was reversed in B-ALL cells with overexpressed c-Myc treated by JQ1, which is involved in the enhancement of glycolysis. Conclusions: The BET inhibitor JQ1 suppresses the proliferation of ALL by inhibiting c-Myc-mediated glycolysis, thus providing a new strategy for the treatment of ALL.

Abstract:Abstract T-cell acute lymphoblastic leukemia (T-ALL) is a common hematological malignancy associated with poor prognosis and significant risk of relapse. Loss of PTEN function due to mutations or deletions is common in primary T-ALL samples and is associated with resistance to therapeutic agents. We previously developed a VEC-Cre+;Ptenloxp/loxp (Pten null) T-ALL model to investigate the molecular and cellular mechanisms underlying T-ALL pathogenesis and resistance to therapy. Pten-null T-ALL develops after acquisition of a T cell receptor α/δ-c-myc translocation that causes c-Myc over-expression, and the leukemia-initiating cells, or leukemia stem cells (LSCs), are enriched in a c-kitmidCD3+ subpopulation. The goal of our current study is to develop improved therapeutic approaches to target LSCs and eliminate T-ALL. We hypothesize that small molecule inhibitors that target molecular lesions and pathways required for the development and survival of Pten null LSCs and T-ALL may be potent in eliminating LSCs and abolishing T-ALL. Rapamycin, an inhibitor of the PI3K downstream kinase mTOR, can suppress leukemia development in Pten null pre-leukemic mice, but is insufficient to eliminate LSCs and T-ALL. Therefore, we are investigating the therapeutic utility of combinational therapies that co-target multiple deregulated pathways that are critical for LSC formation and T-ALL survival. We found that combination treatment of Pten null T-ALL mice with rapamycin and VX-680, an Aurora kinase inhibitor shown to have synthetic lethality with Myc over-expression, causes robust elimination of leukemia blasts and significantly diminishes the LSC population. Functional studies for leukemia-initiating-cell activity using transplantation of treated bone marrow into NOD-SCID-IL2rγ-/-(NSG) recipients showed that dual treatment reduced leukemia-initiating-cell activity in bone marrow by at least 100-fold in comparison to single agent alone. Importantly, a large portion of Pten null T-ALL LSCs are actively cycling (~50% in S-phase), which may contribute to the mechanism underlying the efficacy of VX-680. We have developed a T-ALL in vitro culture system derived from primary Pten null T-ALL mice to conduct more detailed evaluation of the cellular and biochemical effects of rapamycin and VX-680, perform mechanistic analysis, and screen additional cell cycle-directed and Myc-targeted agents. We found that Pten null T-ALL is highly sensitive to killing by the CDK1 inhibitor purvalanol and JQ1, a bromodomain inhibitor reported to down-regulate c-myc expression that is suitable for use in vivo. Rapamycin and JQ1 combination treatment of Pten null T-ALL mice causes a significant reduction in both the leukemia blast and LSC populations, similar to the effects of rapamycin and VX-680 combination therapy. Our results highlight the importance of understanding the mechanisms underlying T-ALL development and survival and show potent synthetic lethality of PI3K and Myc pathway co-inhibition in PTEN null T-ALL LSCs. Abstract:Abstract Abstract 4618 c-Myc (hereafter Myc) overexpression has been recognized in aggressive B-cell lymphomas and linked to adverse prognosis. Myc activation results in widespread repression of miRNA expression and lymphoma aggressive progression. Our recent study identified a Myc-miRNAs-EZH2 feed-forward loop linking over-expression of Myc, EZH2, and miRNA repression. Here, using a novel small-molecule bromodomain inhibitor, JQ1, to silence Myc and the EZH2 inhibitor DZNep, we demonstrated that combined inhibition of Myc and EZH2 cooperatively disrupted Myc activation, resulting in restoration of miR-26a expression, which subsequently suppressed lymphoma growth and clonogenicity in aggressive lymphoma cells. Furthermore, Myc and EZH2 cooperatively regulated miR-26a expression in aggressive lymphoma cells. MiR-26a functioned as a tumor suppressor miRNA and mediated the combinatorial effect of JQ1 and DZNep. These findings represent a promising, novel approach for silencing Myc with JQ1 for combinatorial therapy of aggressive B-cell lymphomas. Disclosures: No relevant conflicts of interest to declare.

Abstract:T-acute lymphoblastic leukemia (T-ALL) is an aggressive hematologic malignancy with complicated heterogeneity. Although expression profiling reveals common elevated genes in distinct T-ALL subtypes, little is known about their functional role(s) and regulatory mechanism(s). We here show that SHQ1, an H/ACA snoRNP assembly factor involved in snRNA pseudouridylation, is highly expressed in T-ALL. Mechanistically, oncogenic NOTCH1 directly binds to the SHQ1 promoter and activates its transcription. SHQ1 depletion induces T-ALL cell death in vitro and prolongs animal survival in murine T-ALL models. RNA-Seq reveals that SHQ1 depletion impairs widespread RNA splicing, and MYC is one of the most prominently downregulated genes due to inefficient splicing. MYC overexpression significantly rescues T-ALL cell death resulted from SHQ1 inactivation. We herein report a mechanism of NOTCH1–SHQ1–MYC axis in T-cell leukemogenesis. These findings not only shed light on the role of SHQ1 in RNA splicing and tumorigenesis, but also provide additional insight into MYC regulation.

Abstract:T-acute lymphoblastic leukemias (T-ALLs) are aggressive hematologic tumors resulting from the malignant transformation of T cell progenitors. In this context, constitutive activation of NOTCH1 signaling is the most prominent oncogenic pathway in T cell transformation. Yet functional role(s) of NOTCH1 in T-ALL pathogenesis and precise mechanism(s) of action remain to be fully determined. SHQ1 is an essential assembly factor for H/ACA ribonucleoproteins which are required for spliceosomal small nuclear RNA (snRNA) maturation. We here identify SHQ1 as a NOTCH1 downstream target that plays a pivotal role in maintaining MYC splicing fidelity and promoting T-ALL cell proliferation in vitro and in vivo. We identified SHQ1 as a NOTCH1-regulated gene from multiple human T-ALL genome-wide expression studies. To validate this finding, we analyzed SHQ1 expression in a spectrum of human T-ALL cells upon NOTCH1 pathway inhibition. NOTCH1 inactivation caused a marked downregulation of SHQ1 in all T-ALL cell lines tested. We further demonstrated NOTCH1 bound to the canonical CSL binding sites in the SHQ1 promoter and directly activated the transcription. We next systematically analyzed the SHQ1 expression in 174 T-ALL primary samples and found that SHQ1 expression was significantly elevated in T-ALL compared with normal peripheral blood. To explore the functional role of SHQ1, we knocked it down in T-ALL using specific shRNAs. SHQ1 depletion profoundly inhibited T-ALL cell proliferation and induced massive apoptotic cell death. Consistent with these in vitro findings, SHQ1 depletion in a human T-ALL xenograft significantly delayed leukemia onset and prolonged survival. To decipher the molecular mechanism whereby SHQ1 contributes to T cell leukemogenesis, we performed genome-wide RNA-Seq and analyzed alternative splicing across the genome. Approximately 70% of genes exhibited abnormal intron retention upon SHQ1 depletion. Among those whose expression levels and mRNA splicing were prominently altered by SHQ1, MYC gained our attention because of its vital role in T-ALL. Depletion of SHQ1 resulted in marked downregulation of mature MYC mRNA and protein. Endogenous mRNA analysis and mini-gene experiments showed aberrant accumulation of MYC pre-mRNA in SHQ1-depleted cells. Expectedly, SHQ1 knockdown had minimal effects on T-ALL cells constitutively expressing a human MYC protein. In addition to the well-documented transcriptional control of MYC by NOTCH1, we herein report a previously unsuspected mechanism in which NOTCH1 modulates MYC splicing by activation of SHQ1. Our findings not only shed new insights in the molecular pathology of NOTCH1-induced T-ALL but also provide a new layer of regulation of oncogene MYC at the post-transcriptional level, mechanism of which may apply to other MYC involved tumors. Abstract:Acute lymphoblastic leukemia (ALL) is the most common cancer in children. Current chemotherapy is quite toxic in growing children and more directed therapeutics are being sought. The IL-7R pathway is a major driver of ALL and here we evaluate two drugs directed to that pathway using a model of T cell ALL. Mutant gain-of-function IL-7Rα was transduced into an IL-7-dependent murine thymocyte line conferring ligand-independent survival and growth. JAK1 is associated with IL-7Rα and mediates signaling from the mutant receptor. , treating the transformed cell line with the JAK1/2 inhibitor ruxolitinib inhibited ligand-independent signaling and induced cell death. Transfer of the transformed cell line into mice resulted in aggressive leukemia and untreated mice succumbed in about three weeks. Treatment with ruxolitinib incorporated into chow showed a potent therapeutic benefit with reduction in leukemic burden and extension of survival. BCL-2 is an anti-apoptotic downstream mediator of the IL-7R survival mechanism. Venetoclax, an inhibitor of BCL-2, showed activity against the transformed cell line and could be combined with ruxolitinib . These findings support the therapeutic potential of treating T-ALL by targeting the IL-7R pathway.

Abstract:T cell acute lymphoblastic leukaemia (T-ALL) is an aggressive malignancy with poor prognosis, but a decisive marker and effective treatment for leukaemia stem cells (LSCs) remain unclear. Here, using lineage tracing, limiting dilution assays and in vivo live imaging approaches, we identify rare inhibitory receptor programmed cell death 1 (PD-1)-expressing cells that reside at the apex of leukaemia hierarchy for initiation and relapse in T-ALL. Ablation of PD-1-expressing cells, deletion of PD-1 in T-ALL cells or blockade of PD-1 or PD-1 ligand 1 significantly eradicated LSCs and suppressed disease progression. Combination therapy using PD-1 blockade and chemotherapy substantially extended the survival of mice engrafted with mouse or human T-ALL cells. Mechanistically, PD-1+ LSCs had high NOTCH1-MYC activity for disease initiation. Furthermore, PD-1 signalling maintained quiescence and protected LSCs against T cell receptor-signal-induced apoptosis. Overall, our data highlight the hierarchy of leukaemia by identifying PD-1(+) LSCs and provide a therapeutic approach for the elimination of LSCs through PD-1 blockade in T-ALL.

Abstract:To explore possible targets for T-ALL therapy, we achieved knockdown of Notch3 expression using novel tumor-specific shRNA driven by survivin promoters. Background: As a highly conserved system, the activation of the Notch pathway has been implicated in the tumorigenesis of various hematologic diseases, including leukemias, lymphomas, and multiple myeloma. The Notch3 receptor is frequently expressed in T-cell acute lymphoblastic leukemia (T-ALL). Methods: To explore its possibility as a therapeutic target for T-ALL, we investigated the effect of Notch3 silencing on Jurkat and SupT1 cells using a novel tumor-specific short hairpin RNA (shRNA) driven by survivin promoters. Conclusion: These facts suggest that downregulating expression of Notch3 could attenuate the Notch signaling activity in T-ALL. All these results indicate that inhibition of Notch3 expression can result in potent antitumor activity in T-ALL.
