Abstract:Abstract Bladder cancer is common worldwide, with most patients presenting with nonmuscle invasive disease. Multiple intravesical recurrences lead to reduced quality of life and high costs for patients with this form of bladder cancer. Intravesical chemotherapy aimed at reducing recurrence is the standard-of-care but has significant side effects from nonspecific cytotoxicity to normal urothelium. Importantly, toxicity limits doses that can be administered. Thus, tumor-specific drug targeting could reduce toxicity and enhance effectiveness by allowing higher doses. Here, using cell internalization systematic evolution of ligands by exponential enrichment (SELEX), we identify a novel bladder cancer-specific, chemically modified nucleic acid aptamer that can be preferentially internalized into tumor cells but not normal urothelial cells. The 35-nucleotide B1 aptamer is internalized into bladder cancer cells through clathrin-mediated endocytosis and macropinocytosis. As proof of principle, a B1-guided DNA nanotrain delivery vehicle for epirubicin was constructed as a targeted intravesical chemotherapy. The B1-nanotrain-epirubicin construct exhibited selective cytotoxicity towards bladder cancer cells and outperformed epirubicin in murine orthotopic xenograft models of human bladder cancer. This aptamer-based delivery system makes targeted chemotherapy possible for bladder cancer, providing a compelling rationale for clinical development. Significance: These findings identify a bladder cancer–specific aptamer that can be used for targeted delivery of chemotherapy, potentially reducing toxicity and enhancing therapeutic efficacy.

Abstract:Bladder cancer is one of most common malignant urinary tract tumor types with high incidence worldwide. In general, transurethral resection of non-muscle-invasive bladder cancer followed by intravesical instillation of chemotherapy is the standard treatment approach to minimize recurrence and delay progression of bladder cancer. However, conventional intravesical chemotherapy lacks selectivity for tumor tissues and the concentration of drug is reduced with the excretion of urine, leading to frequent administration and heavy local irritation symptoms. While nanomedicines can overcome all the above shortcomings and adhere to the surface of bladder tumors for a long time, and continuously and efficiently release drugs to bladder cancers. The rapid advances in targeted therapy have led to significant improvements in drug efficacy and precision of targeted drug delivery to eradicate tumor cells, with reduced side-effects. This review summarizes the different available nano-systems of targeted drug delivery to bladder cancer tissues. The challenges and prospects of targeted therapy for bladder cancer are additionally discussed.

Abstract:The treatment of advanced urothelial cancer of the bladder has evolved substantially during recent years. Chemotherapy has been the mainstay of treatment and confers survival advantage. Despite such advances, the chemotherapy of bladder cancer is far from satisfactory due to severe side effects. Targeted therapy with novel drugs directed at specific molecular pathways opens promising new avenues to improve patient outcome. A systematic review examined the clinical data for novel targeted agents in 10 phase trials, with a focus on bevacizumab, aflibercept, sunitinib, sorafenib, gefitinib, lapatinib and trastuzumab. Besides, we present studies on other novel, promising targeted agents, including pazopanib, cetuximab and everolimus. Although bevacizumab and trastuzumab have shown promising results for patients with advanced bladder cancer, other targeted agents have not achieved the same clinical benefit in this disease as seen in other common epithelial cancers. Ultimately, combination targeted therapy, sequential therapy, adjuvant and neoadjuvant therapy may yield the best outcomes.
A non‐G‐Quadruplex DNA Aptamer Targeting NCL for Diagnosis and Therapy in Bladder Cancer

Abstract:Bladder cancer (BC) is a highly aggressive malignant tumor affecting the urinary system, characterized by metastasis and a poor prognosis that often leads to limited therapeutic success. In this study, we aimed to develop a novel DNA aptamer for the diagnosis and treatment of BC using a tissue‐based systematic evolution of ligands by exponential enrichment (SELEX) process. SELEX involves the in vitro evolution of aptamers through iterative selection, allowing the identification of high‐affinity aptamers that specifically bind to target molecules from a large pool of oligonucleotides. By employing SELEX, we successfully generated a new aptamer named TB‐5, which demonstrated a remarkable affinity for nucleolin (NCL) in BC tissues, surpassing the binding affinity observed in normal tissues by over four‐fold. Furthermore, TB‐5 displayed marked biocompatibility both in vitro and in vivo. To validate NCL as a reliable tissue‐specific biomarker, immunohistochemical staining was performed. Circular dichroism spectroscopy revealed that TB‐5 adopts a non‐G‐quadruplex structure, distinguishing it from the current aptamer AS1411, which also binds to NCL. Interestingly, TB‐5 exhibited a distinct binding region on NCL compared to AS1411. Notably, our study revealed that TB‐5 activates NCL function by promoting autophagy and suppressing the migration and invasion of BC cells, shedding light on the interplay between steric hindrance and NCL biological function. Most importantly, we discovered that TB‐5 disrupts mRNA transcription processes in cancer cells by binding to NCL, leading to the inhibition of cell proliferation and migration. These findings highlight the critical role of NCL in the pathological examination of BC and warrant more comprehensive investigations on anti‐NCL aptamers in BC imaging and treatment. Abstract:Bladder cancer (BC) is one of the most common types of cancers in the world. Despite various treatments are currently in place, the prognosis of BC does not seem to improve. Therefore, this prompts us to search for alternative treatments. The understanding of the molecular changes in cancer offers the prospect of targeting the key elements responsible for cancer development and progression, and thus, helps to develop a more effective and robust targeted therapy that does not endanger normal cells. At present, protein-based therapeutic targets such as programmed death ligand 1, epidermal growth factor receptor and aberrantly glycosylated integrin-α3β1, and genetic targets such as p53, human EGF receptor-2, and miR-23b are promising targets for BC treatment. In addition, predictive markers of treatment efficacy for BC, such as meiotic recombination 11, excision repair cross-complementing group 1, and multidrug resistance 1 gene are essential in facilitating clinicians to design and implement suitable and effective therapies for BC patients. This review discusses the pivotal role of these markers in the development of targeted therapy and the estimation of the clinical outcome of BC.

Abstract:Cancer is still a global public health problem. Although remarkable success has been achieved in cancer diagnosis and treatment, the high recurrence and mortality rates remain severely threatening to human lives and health. In recent years, peptide nanomedicines with precise selectivity and high biocompatibility have attracted intense attention in biomedical applications. In particular, there has been a significant increase in the exploration of peptides and their derivatives for malignant tumor therapy and diagnosis. Herein, we review the applications of peptides and their derivatives in the diagnosis and treatment of bladder cancer, providing new insights for the design and development of novel peptide nanomedicines for the treatment of bladder cancer in the future.

TB‐5 demonstrates potential as a diagnostic and therapeutic tool due to its high specificity and affinity for bladder cancer tissues, elucidating the significant role of nucleolin in bladder cancer pathology.

Abstract:Photodynamic therapy (PDT) is often applied in a clinical setting to treat bladder cancer. However, current photosensitizers report drawbacks such as low efficacy, low selectivity, and numerous side effects, which have limited the clinical values of PDT for bladder cancer. Previously, we developed the first bladder cancer-specific aptamer that can selectively bind to and be internalized by bladder tumor cells versus normal uroepithelium cells. Here, we use an aptamer-based drug delivery system to deliver photosensitizer chlorine e6 (Ce6) into bladder tumor cells. In addition to Ce6, we also incorporate catalase into the drug complex to increase local oxygen levels in the tumor tissue. Compared with free Ce6, an aptamer-guided DNA nanotrain (NT) loaded with Ce6 and catalase (NT-Catalase-Ce6) can specifically recognize bladder cancer cells, produce oxygen locally, induce ROS in tumor cells, and cause mitochondrial apoptosis. In an orthotopic mouse model of bladder cancer, the intravesical instillation of NT-Catalase-Ce6 exhibits faster drug internalization and a longer drug retention time in tumor tissue compared with that in normal urothelium. Moreover, our modified PDT significantly inhibits tumor growth with fewer side effects such as cystitis than free Ce6. This aptamer-based photosensitizer delivery system can therefore improve the selectivity and efficacy and reduce the side effects of PDT treatment in mouse models of bladder cancer, bearing a great translational value for bladder cancer intravesical therapy.

Abstract:Bladder cancer is one of the most common malignancies of the urinary tract, with an estimated 550,000 new cases occurring worldwide each year. Despite aggressive treatment of these patients, including surgery, chemotherapy, radiotherapy, and immunotherapy, 70% of patients have tumor recurrence within 5 years. This suggests that conventional treatments have a limited role in inhibiting the progression of bladder cancer. As a result, novel diagnostic strategies and treatment options are being explored. Significant advancements in the design and use of nanotechnology for the treatment of cancer have been done recently. The advantage of nano drug delivery systems is that they are more penetrating and targeted than conventional therapeutic modalities. The application of a variety of drug carriers derived from nanoplatforms has opened up new horizons for bladder cancer precision treatment. This review first summarises recent advances in smart nanotechnology for the diagnosis of bladder cancer, including urinalysis and imaging. Ongoing research on therapeutic approaches based on nano-targeted drug delivery technology is then discussed.
