Abstract:AbstractPrimary malignant melanoma of the esophagus (PMME) is a rare and aggressive disease with high tendency of metastasis. To characterize the genetic basis and intratumor heterogeneity of PMME, we performed multiregion exome sequencing and whole genome SNP array genotyping of 12 samples obtained from a patient with PMME. High intratumor heterogeneity was observed in both somatic mutation and copy-number alteration levels. Nine geographically separate samples including two normal samples were clonally related and followed a branched evolution model. Most putative oncogenic drivers such as BRAF and KRAS mutations as well as CDKN2A biallelic inactivation were observed in trunk clones, whereas clinically actionable mutations such as PIK3CA and JAK1 mutations were detected in branch clones. Ancestor tumor clones evolved into three subclonal clades: clade1 fostered metastatic subclones that carried metastatic features of PIK3CA and ARHGAP26 point mutations as well as chr13 arm-level deletion, clade2 owned branch-specific JAK1 mutations and PTEN deletion, and clade3 was found in two vertical distribution samples below the primary tumor area, highlighting the fact that it is possible for PMME to disseminate by the submucosal longitudinal lymphatic route at an early stage of metastasis. These findings facilitate interpretation of the genetic essence of this rare melanoma subtype as well as the pattern of cancer evolution, thus reinforcing the therapeutic challenges associated with PMME.Significance: This study highlights the use of multiregion exome sequencing and whole genome SNP array genotyping to comprehensively characterize the genetic landscape of a rare type of esophogeal melanoma. Abstract:Mixed adenoneuroendocrine carcinoma (MANEC) in the esophagus is an infrequent but highly malignant cancer with few known genomic alterations. We conducted whole-exome sequencing and whole-genome SNP genotyping for 4-6 tumor subregions and 5-6 adjacent normal tissue sites and 1-3 lymph node metastases in two esophageal MANECs to detect somatic mutations and copy number alterations, and to explore their spatial heterogeneity and underlying clonal structure. TP53 mutation, RB1 deletion or LOH, and PIK3CA, PTEN, KRAS, SOX2, DVL3, TP63 amplification appeared in all regions in both tumors. Mutations falling in known cancer genes tended to show higher variant allele frequencies than those not falling in these genes in at least one of the cases. Phylogenetic analyses of the samples and underlying subclones suggested extensive migration across different tumor regions and from some regions to the lymph nodes. Lymph node metastases appeared to have been seeded by both early founder cells as well as subsequent, locally emerging daughter clones. A phenotypically normal tissue site carried most of the mutations found in neighboring tumor samples, implying field cancerization. Understanding such complex genetic heterogeneity within each patient will be important for guiding clinical decisions.

Abstract:To reveal the clonal architecture of melanoma and associated driver mutations, whole genome sequencing (WGS) and targeted extension sequencing were used to characterize 124 melanoma cases. Significantly mutated gene analysis using 13 WGS cases and 15 additional paired extension cases identified known melanoma genes such as BRAF, NRAS, and CDKN2A, as well as a novel gene EPHA3, previously implicated in other cancer types. Extension studies using tumors from another 96 patients discovered a large number of truncation mutations in tumor suppressors (TP53 and RB1), protein phosphatases (e.g., PTEN, PTPRB, PTPRD, and PTPRT), as well as chromatin remodeling genes (e.g., ASXL3, MLL2, and ARID2). Deep sequencing of mutations revealed subclones in the majority of metastatic tumors from 13 WGS cases. Validated mutations from 12 out of 13 WGS patients exhibited a predominant UV signature characterized by a high frequency of C->T transitions occurring at the 3' base of dipyrimidine sequences while one patient (MEL9) with a hypermutator phenotype lacked this signature. Strikingly, a subclonal mutation signature analysis revealed that the founding clone in MEL9 exhibited UV signature but the secondary clone did not, suggesting different mutational mechanisms for two clonal populations from the same tumor. Further analysis of four metastases from different geographic locations in 2 melanoma cases revealed phylogenetic relationships and highlighted the genetic alterations responsible for differential drug resistance among metastatic tumors. Our study suggests that clonal evaluation is crucial for understanding tumor etiology and drug resistance in melanoma.

Abstract:Acral melanoma (AM) is an extremely aggressive subtype of melanoma that is prevalent in eastern Asia. AM exhibits high intertumoral and intratumoral heterogeneities with poor prognosis. To associate the genomic heterogeneities with phenotypic traits and efficacy of treatments, a method is needed to recover genomic information from limited samples with high specificity and sensitivity from early stage AM specimens. We performed laser capture microdissection to isolate single micro-tumor nests, containing only dozens of cells, from stained tissue slices and then applied multiple annealing and looping-based amplification cycles, a highly efficient whole-genome amplification method originally developed for single cells, to amplify the whole genome of each tumor nest for sequencing. We were able to accurately profile the landscape of copy number alterations and single nucleotide variations of every single micro-tumor nest and to quantitatively characterize the heterogeneities at different levels, between tumor and nevi, among patients, among different phenotypes within a same tumor, and among adjacent tumor cell clusters with identical phenotypic appearance. We have found that genomic heterogeneity exists extensively and that branched evolution happens in the early stage of AM development. We are able to build the phylogenetic tree among these phenotypically addressable cell clusters.

Abstract:Head and neck melanomas are a heterogeneous group of tumors that may arise from abnormal melanocytes in various sites, such as scalp of the head, conjunctiva, nasal pharyngeal and oral cavity.Despite their close anatomical location, melanomas from different sites of the head and neck have widely different histological structures, biological behaviors, genetic characteristics, and prognosis. However, researches on genomic landscape of head and neck melanoma in East Asian patients are still sparse. We performed whole-genome sequencing (WES) and clinical profiling on 40 head and neck melanoma samples, including 8 conjunctival melanoma of the eyes, 7 scalp of the head, 10 nasal and 15 oral cavity. All the samples were derived from East Asian populations. We show that tumors from eyes and head tended to harbor more substitution/indel (SNVs) than those from nasal and oral. Percent of the genome affected by copy number variants was no significant differences among the four sites. Head and neck melanomas samples had more genetic aberations in genes within the RTK-RAS pathway and NOTCH pathway. BRAF, NRAS were identified as significantly mutated genes (SMGs).Eyes and head tumors had consistent mutational signature, while the oral and nasal tumors had similar mutationnal signature. The number and type of structural rearrangements varied dramatically among the four sites, suggesting the implication of distinctive mutational processes. Somatic arm level CNV alterations including amplification at chromosomes 1q, 6p, 7 and deletions at chromosomes 6q, 10, 13q, 16q, 18p, 19.Significantly amplified regions, included those containing 1q22(MUC1), 3p13(MITF), 4q12(KIT), 5p15.33 (TERT), 6p23, 7p22.1(RAC1), 8q24.3(MYC), 9q34.3(NOTCH1),11q14.1(PAK1,GAB2), 12q14.1(CDK4), 17q25.1, 20q13.2. as well as deleted regions 3q22.2, 6q26, 9p21.3-24.3(CDKN2A,CDKN2B), 10q26.13, 11q23.3, 16p12.1, 11q22.3(ATM). Here, we provided a detailed view of the genomic landscape of East Asian head and neck melanomas at different sites, which might extend our current understanding of head and neck melanoma.

Abstract:Background Chromosomal and genomic aberrations are common features of human cancers. However, chromosomal numerical and structural aberrations, breakpoints and disrupted genes have yet to be identified in esophageal squamous cell carcinoma (ESCC). Methods Using multiplex-fluorescence in situ hybridization (M-FISH) and oligo array-based comparative hybridization (array-CGH), we identified aberrations and breakpoints in six ESCC cell lines. Furthermore, we detected recurrent breakpoints in primary tumors by dual-color FISH. Results M-FISH and array-CGH results revealed complex numerical and structural aberrations. Frequent gains occurred at 3q26.33-qter, 5p14.1-p11, 7pter-p12.3, 8q24.13-q24.21, 9q31.1-qter, 11p13-p11, 11q11-q13.4, 17q23.3-qter, 18pter-p11, 19 and 20q13.32-qter. Losses were frequent at 18q21.1-qter. Breakpoints that clustered within 1 or 2 Mb were identified, including 9p21.3, 11q13.3-q13.4, 15q25.3 and 3q28. By dual-color FISH, we observed that several recurrent breakpoint regions in cell lines were also present in ESCC tumors. In particular, breakpoints clustered at 11q13.3-q13.4 were identified in 43.3% (58/134) of ESCC tumors. Both 11q13.3-q13.4 splitting and amplification were significantly correlated with lymph node metastasis (LNM) ( P = 0.004 and 0.022) and advanced stages ( P = 0.004 and 0.039). Multivariate logistic regression analysis revealed that only 11q13.3-q13.4 splitting was an independent predictor for LNM ( P = 0.026). Conclusions The combination of M-FISH and array-CGH helps produce more accurate karyotypes. Our data provide significant, detailed information for appropriate uses of these ESCC cell lines for cytogenetic and molecular biological studies. The aberrations and breakpoints detected in both the cell lines and primary tumors will contribute to identify affected genes involved in the development and progression of ESCC.

Abstract:The clonal evolution which drives esophageal squamous cell carcinoma (ESCC) from initiation in normal cell to primary carcinoma and metastases is poorly understood. In this study, multi-region whole-exome sequencing (WES) (284X) and whole-genome single nucleotide polymorphism genotyping were performed on a total of 109 samples of ESCC from 10 patients. This included 42 apparently normal samples of esophageal mucosa at increasing distances from the upper or lower boundaries of the primary tumor to the surgical margins of resection, 43 spatially separated tissue samples within primary tumor and 24 regional lymph node metastases. Phylogenetic analysis was performed to reconstruct ancestor-descendant relationships of clones and the clonal composition of multi-region samples. Mutations of cancer-related genes were validated by deep targeted sequencing (1,168X). Both inter- and intra-tumoral genetic heterogeneity were obvious across multi-region samples among ESCC patients. Clones varying in number from one to seven were discovered within each regional tumor or metastatic sample. Phylogenetic analysis demonstrated complex clonal evolution patterns. Regional lymph node metastases had characteristics of early initiation and polyclonal spreading, and could be derived from carcinoma in situ (CIS) directly. TP53 was the only gene harboring non-silent mutations identified across all multi-region tumor samples of all ten patients. Mutations of TP53 were also found in histologically normal mucosa in sites away from primary tumor.

Abstract:Primary malignant melanoma of the esophagus (PMME) is an exceedingly rare disease with a poor prognosis. The etiology of PMME remains largely unknown and genetic characteristics are yet to be clarified, essential for identifying potential therapeutic targets and defining treatment guidelines. The genomic features of PMME were comprehensively characterized, and comparative genomic analysis was further performed between these specimens and 398 skin cutaneous melanomas (SKCM), 67 non-esophagus mucosal melanomas (NEMM), and 79 uveal melanomas (UVM). In the PMME cohort, recurrently mutated driver genes, such as MUC16, RANBP2, NRAS, TP53, PTPRT, NF1, MUC4, KMT2C, and BRAF, were identified. All RANBP2 mutations were putatively deleterious, and most affected samples had multipoint mutations. Furthermore, RANBP2 showed parallel evolution by multiregional analysis. Whole-genome doubling was an early truncal event that occurred before most driver mutations, except for in TP53. An ultraviolet radiation-related mutational signature, SBS38, was identified as specific to epithelial melanomas and could predict inferior survival outcomes in both PMME and SKCM patients. Comparing the mutational and copy number landscapes between PMME and other subtypes of melanoma revealed that PMME has a similar genomic pattern and biological characteristics to SKCM. In summary, we comprehensively defined the key genomic aberrations and mutational processes driving PMME and suggested for the first time that PMME may share similar genomic patterns with SKCM; therefore, patients with rare melanomas, such as PMME, may benefit from the current treatment used for common cutaneous melanoma.

Abstract:Background Anti-PD-1 monoclonal antibody monotherapy achieves poor outcomes in mucosal melanoma, but our previous study has demonstrated that primary malignant melanoma of esophagus (PMME) has a dramatically high response rate (ORR 75%). The genomic landscape of PMME and reasons for this finding have not been extensively investigated. Methods Whole exome sequencing were performed in seven treatment-naive PMME tumor samples and matched normal tissues, and genomic data of eight mucosal melanomas was obtained from the International Cancer Genome Consortium. Comprehensive profiling was performed to analyze the of genetic differences of PMME with mucosal melanoma. Results The mean depth of coverage was 268X (ranged from 100X to 400X), 1203 SNV/InDels were detected in seven PMME samples, with a mean value of 172 mutations. The median tumor mutation burden (TMB) was 3.58 mutations/Mb (range 0.19-9.58 mutations/Mb), which was significantly than mucosal melanoma (P = 0.021). The mutation spectrum of SNVs displayed more frequency of T>A transition while less frequency of C>T transversion compared to mucosal melanoma. There were 36 genes found to be mutated in more than two PMME tumor samples, and the most significantly mutated genes were CFH (42.9%, 3/7) and MUC16 (42.9%, 3/7). Both BRAF and NRAS were only found in one sample, and no NF1, KIT and SF3B1 mutation were detected. In total, 671 CNVs were detected, and the most frequently amplification chromosomal regions were 10q and 1p, while most frequently deletion in 1q and 8q. Genetic aberrations were mainly enriched in RTK/RAS and WNT pathways. Conclusions The TMB is higher in PMME compared to mucosal melanoma, with the highest mutation frequency in CFH and MUC16. CFH has a critical role in the regulation of complement activation, and MUC16 has been reported to be associated with higher tumor mutation load and outcomes in gastric cancer. In summary, our study reveals that the genomic landscape of PMME is different from mucosal melanoma originates from other mucosal membrane sites, and indicates that CFH and MUC16 mutation might be associated with better response to anti-PD-1 monoclonal antibody in PMME patients.
