Curative effect of hemispherectomy on drug-refractory epilepsy

Abstract:Objective To explore the clinical efficacy and related factors of peri-insular hemispherotomy in treating drug-refractory epilepsy with hemispheric lesions. Methods A retrospective analysis was performed on 15 patients with drug-refractory epilepsy who underwent peri-insular hemispherotomy at the Epilepsy Center of Xinqiao Hospital of Army Medical University from June 2017 to April 2022, including 8 males and 7 females, aged 0.9 to 29 years old. Relevant clinical data were documented, including clinical symptoms, imaging, electroencephalogram(EEG), pathological results, etc. The patients were followed up after surgery to evaluate the effect of controlling seizure and complications. Results All patients underwent a thorough pre-operative evaluation, showed clear indications for surgery and had a successful peri-insular hemispherotomy. The average postoperative follow-up time was 28.8 months, among which, 13 patients(87%) were seizure free after surgery. Postoperative pathological findings showed 9 patients with diffuse encephalomalacia, 2 patients with cortical dysplasia combined with gyral malformation, 2 patients with focal cortical dysplasia, 1 patient with dysembryoplastic epithelioid neoplasia and 1 patient without pathological examination. Among these patients, there was no aphasis, no decrease in neuropsychological development compared with preoperative conditions and no long-term complications such as hydrocephalus and hemosiderosis after surgery. The limb dysfunction in the early postoperative period was aggravated in 2 patients compared with before surgery and improved during follow-up. Conclusion Peri-insular hemispherotomy is an effective surgery in treating hemispheric or multi-lobar lesions-associated drug-refractory epilepsy with fewer postoperative complications.
