Predictive value of tumor infiltrating lymphocytes and programmed cell death-ligand 1 expression in stage Ⅱ-Ⅲ colon cancer metastasis after radical operation

Abstract:Objective:To investigate the predictive value of tumor infiltrating lymphocytes (TILs) and programmed cell death-ligand 1 (PD-L1) expression in colon cancer after radical resection.Methods:The clinical and follow-up data of 111 patients with stage Ⅱ-Ⅲ colon cancer undergoing radical operation were collected. According to the follow-up results, the patients were divided into tumor metastasis group and non tumor metastasis group, and TILs and PD-L1 were detected by immunohistochemistry. The best cut-off value of TILs and PD-L1 were determined by the receiver operating characteristics (ROC) curve. Logistic regression model was used to analyze the high risk factors of tumor metastasis. Kaplan Meier method was used to evaluate 5-year overall survival (OS) and disease-free survival (DFS).Results:The positive rates of CD3 +, CD8 + TILs and PD-L1 in tumor metastasis group and non metastasis group after radical resection were 22.2% and 66.7%, 11.1% and 61.1%, 60.0% and 20.0%, respectively ( P<0.05). The positive rates of CD4 + TILs were 44.4% and 27.8%, respectively ( P>0.05). The results of multivariate analysis showed that the low scores of CD3 + and CD8 + TILs H-score were independent risk factor for colon cancer metastasis after radical resection, while the low scores of PD-L1 were independent protective factor ( P<0.05). The results of survival analysis showed that the CD8 + TILs and PD-L1 were independent prognostic factors of OS and DFS in patients with colorectal cancer. Conclusion:TILs and PD-L1 can be used to predict the prognosis of patients with stage Ⅱ-Ⅲ colon cancer.
