Abstract:About a third of all deaths worldwide are associated with atherosclerosis. Reduction of the local inflammation at atherosclerotic plaques could serve as an attractive strategy for combating the sequels associated with atherosclerosis. Statins class of drugs exert direct anti-inflammatory activities, however, higher doses are required for statins therapy due to their low bioavailabilities and potential side effects hence rendering them unrealistic. By creating intelligent nanostructures with a variety of functionalization, nanoparticles offer an excellent platform to cure atherosclerosis at the molecular scale. Owing to significant advancements in technological inventions, targeted ligands spontaneously transit the circulation, over-come biological barriers, and reach the target lesion. In recent decades, CD44 has received extensive research attention for several cancer types, where it controls tumor development and invasiveness. Whereas adhesion and migration are its primary activities, CD44 additionally has an impact on macrophage activation and localization, internalization, structural IP 2038 10920 On Tue 27 J n 2023 11 30 16 remodeling, proliferative, and apoptosis. Abstract:Atherosclerosis, a chronic disease associated with metabolism, poses a significant risk to human well-being. Currently, existing treatments for atherosclerosis lack sufficient efficiency, while the utilization of surface-modified nanoparticles holds the potential to deliver highly effective therapeutic outcomes. These nanoparticles can target and bind to specific receptors that are abnormally over-expressed in atherosclerotic conditions. This paper reviews recent research (2018-present) advances in various ligand-modified nanoparticle systems targeting atherosclerosis by specifically targeting signature molecules in the hope of precise treatment at the molecular level and concludes with a discussion of the challenges and prospects in this field. The intention of this review is to inspire novel concepts for the design and advancement of targeted nanomedicines tailored specifically for the treatment of atherosclerosis.

Abstract:Atherosclerosis continues to be a leading cause of morbidity and mortality globally. The precise evaluation of the extent of an atherosclerotic plaque is essential for forecasting its likelihood of causing health concerns and tracking treatment outcomes. When compared to conventional methods used, nanoparticles offer clear benefits and excellent development opportunities for the detection and characterisation of susceptible atherosclerotic plaques. In this review, we analyse the recent advancements of nanoparticles as theranostics in the management of atherosclerosis, with an emphasis on applications in drug delivery. Furthermore, the main issues that must be resolved in order to advance clinical utility and future developments of NP research are discussed. It is anticipated that medical NPs will develop into complex and advanced next-generation nanobotics that can carry out a variety of functions in the bloodstream.

Abstract:Atherosclerosis is a common form of cardiovascular disease, which is one of the most prevalent causes of death worldwide, particularly among older individuals. Surgery is the mainstay of treatment for severe stenotic lesions, though the rate of restenosis remains relatively high. Current medication therapy for atherosclerosis has limited efficacy in reversing the formation of atherosclerotic plaques. The search for new drug treatment options is imminent. Some potent medications have shown surprising therapeutic benefits in inhibiting inflammation and endothelial proliferation in plaques. Unfortunately, their use is restricted due to notable dose-dependent systemic side effects or degradation. Nevertheless, with advances in nanotechnology, an increasing number of nano-related medical applications are emerging, such as nano-drug delivery, nano-imaging, nanorobots, and so forth, which allow for restrictions on the use of novel atherosclerotic drugs to be lifted. This paper reviews new perspectives on the targeted delivery of nanoparticles to blood vessels for the treatment of atherosclerosis in both systemic and local drug delivery. In systemic drug delivery, nanoparticles inhibit drug degradation and reduce systemic toxicity through passive and active pathways. To further enhance the precise release of drugs, the localized delivery of nanoparticles can also be accomplished through blood vessel wall injection or using endovascular interventional devices coated with nanoparticles. Overall, nanotechnology holds boundless potential for the diagnosis and treatment of atherosclerotic diseases in the future.

Abstract:Atherosclerosis is one of the major causes of death worldwide, and it is closely related to many cardiovascular diseases, such as stroke, myocardial infraction and angina. Although traditional surgical and pharmacological interventions can effectively retard or slow down the progression of atherosclerosis, it is very difficult to prevent or even reverse this disease. In recent years, with the rapid development of nanotechnology, various nanoagents have been designed and applied to different diseases including atherosclerosis. The unique atherosclerotic microenvironment with signature biological components allows nanoplatforms to distinguish atherosclerotic lesions from normal tissue and to approach plaques specifically. Based on the process of atherosclerotic plaque formation, this review summarises the nanodrug delivery strategies for atherosclerotic therapy, trying to provide help for researchers to understand the existing atherosclerosis management approaches as well as challenges and to reasonably design anti-atherosclerotic nanoplatforms.

Abstract:Cardiovascular diseases as a primary driver of global morbidity and mortality. Despite the array of therapeutic avenues in clinical practice, predominantly pharmaceutical and surgical interventions, they often fall short of fully addressing the clinical exigencies of cardiovascular patients. In recent years, nanocarriers have shown great potential in the treatment and diagnose of cardiovascular diseases. They can enhance drug targeting and bioavailability while reducing side effects. Additionally, by improving imaging and detection technologies, they enhance early diagnosis and disease monitoring capabilities. These advancements in technology offer new solutions for precision medicine in cardiovascular diseases, advancing treatment efficacy and disease management. Crafted from biomaterials, metals, or their amalgamations, these nanocarriers approximate the dimensions of biologically active molecules like proteins and DNA. Cardiovascular nanomedicine, in its infancy, has only recently burgeoned. Yet, with continual refinement in nanocarrier architecture, drug delivery mechanisms, and therapeutic outcomes, the potential of nanomedical technologies in clinical contexts becomes increasingly evident. This review aims to consolidate the strides made in nanocarrier research concerning the treatment and diagnose of cardiovascular diseases.

Currently available therapies are inadequate and warrant the demand for improved technologies for more effective treatment. Although primarily the domain of antitumor therapy, recent advances have shown the considerable potential of nanomedicine to advance atherosclerosis treatment. This Review details the arsenal of nanocarriers and molecules available for selective targeting in atherosclerosis, and emphasize the challenges in atherosclerosis treatment.

Abstract:To further minimize the risk of cardiovascular disease, nanotechnology may help us better understand the mechanism of atherosclerosis and contribute to the development of innovative diagnostic and treatment approaches. Therefore, inflammatory indicators have been popular in recent years as potential therapeutic and diagnostic targets. Inflammation contributes to the initiation and progression of several diseases globally. Adhesion molecules expressed by leukocytes and endotheliocytes cause the immune cells infiltration to inflamed area. These molecules serve as mediators for tethering, firm adhesion, rolling, and diapedesis processes. VCAM-1, an adhesion molecule with low intrinsic expression and easy access on the endothelium, has been investigated as a potential target. It also plays a critical role in the early stages of the development of several pathologies, including, but not limited to, atherosclerosis, Parkinson’s, Alzheimer’s diseases, and cancer. A therapeutic or diagnostic tool that specifically targets this protein would thus enable accurate identification. The development of numerous targeted technologies with potential applications for drug delivery and imaging of VCAM-1 over-expressing diseases has recently been encouraged by the emergence of monoclonal antibodies against VCAM-1. In this review article, we first discuss the origin, structure, and function of antibodies, followed by a summary of the VCAM-1 targeted nanosystems, including siRNA targeting carriers, for the therapy and diagnosis of various inflammatory-related pathologies. Finally, to promote the clinical nanomedicine application for the treatment of atherosclerosis, we suggest prospects for novel diagnostic and therapeutic approaches and provide solutions to challenges in this field.

Abstract:Atherosclerosis, a highly prevalent cardiovascular disease, has significantly contributed to morbidity and mortality worldwide. Despite the availability of various treatment options in clinical practice, complete reversal of the atherosclerotic pathological process remains challenging. Traditional anti-arteriosclerosis drugs often suffer from poor solubility, limited targeting ability and obvious adverse effects. Additionally, novel therapeutics such as genetic and protein-based drugs inherently suffer from instability issues, thereby compromising their actual curative efficacy and impeding further clinical application. Recently, nanomedicine-based drug delivery strategies have emerged as promising approaches for treating atherosclerosis due to their ability to specifically transport various drugs to target cells, microstructures and specific molecules within lesion sites. These advanced features enhance drug properties and bioavailability while minimizing off-target effects. Moreover, numerous studies have demonstrated that nanomedicine not only inhibits plaque progression but also reduces lesional areas and eventually reverses atherosclerosis. The utilization of nanomedicine is considered an important therapeutic tactic for further mitigating residual risks associated with atherosclerosis. This review provides a comprehensive overview of emerging nanomedicine-based strategies for targeted delivery of anti-atherosclerosis drugs, elucidating their distinct mechanisms and highlighting their remarkable efficacy in combating atherosclerosis. Additionally, critical challenges and future perspectives associated with the development and application of anti-arteriosclerosis nanomedicine are thoroughly discussed.

Abstract:Abstract As nanotechnologies advance into clinical medicine, novel methods for applying nanomedicine to cardiovascular diseases are emerging. Extensive research has been undertaken to unlock the complex pathogenesis of atherosclerosis. However, this complexity presents challenges to develop effective imaging and therapeutic modalities for early diagnosis and acute intervention. The choice of ligand-receptor system vastly influences the effectiveness of nanomedicine. This review collates current ligand-receptor systems used in targeting functionalized nanoparticles for diagnosis and treatment of atherosclerosis. Our focus is on the binding affinity and selectivity of ligand-receptor systems, as well as the relative abundance of targets throughout the development and progression of atherosclerosis. Antibody-based targeting systems are currently the most commonly researched due to their high binding affinities when compared with other ligands, such as antibody fragments, peptides, and other small molecules. However, antibodies tend to be immunogenic due to their size. Engineering antibody fragments can address this issue but will compromise their binding affinity. Peptides are promising ligands due to their synthetic flexibility and low production costs. Alongside the aforementioned binding affinity of ligands, the choice of target and its abundance throughout distinct stages of atherosclerosis and thrombosis is relevant to the intended purpose of the nanomedicine. Further studies to investigate the components of atherosclerotic plaques are required as their cellular and molecular profile shifts over time.

Abstract:Cardiovascular disease (CVDs) is the first killer of human health, and it caused up at least 31% of global deaths. Atherosclerosis is one of the main reasons caused CVDs. Oral drug therapy with statins and other lipid-regulating drugs is the conventional treatment strategies for atherosclerosis. However, conventional therapeutic strategies are constrained by low drug utilization and non-target organ injury problems. Micro-nano materials, including particles, liposomes, micelles and bubbles, have been developed as the revolutionized tools for CVDs detection and drug delivery, specifically atherosclerotic targeting treatment. Furthermore, the micro-nano materials also could be designed to intelligently and responsive targeting drug delivering, and then become a promising tool to achieve atherosclerosis precision treatment. This work reviewed the advances in atherosclerosis nanotherapy, including the materials carriers, target sites, responsive model and treatment results. These nanoagents precisely delivery the therapeutic agents to the target atherosclerosis sites, and intelligent and precise release of drugs, which could minimize the potential adverse effects and be more effective in atherosclerosis lesion.

Abstract:Atherosclerosis is a sort of chronic inflammatory disease mainly caused by the accumulation of lipoproteins on the blood vessel walls. Many cardiovascular diseases with high mortality, like ischemic cerebral stroke and ischemic heart disease, are promoted by atherosclerosis. Currently, there is much space for improvement in clinical monitoring and treatment of atherosclerosis. The development of nanotechnology and nanomaterials provides solutions to certain shortcomings of current programs. The present article first reviews the nanomaterials-based therapeutic strategies as well as the related pathogenesis and influencing factors of atherosclerosis. In addition, the main recent developed nanomaterials-based protocols in order to early diagnosis and better monitor for atherosclerosis have been classified and summarized. Subsequently, we list the clinical progress of nanomaterials in the treatment and diagnosis of atherosclerosis in recent years and analyze the current main dilemmas, as well. This review aims to reveal the essential role of nanomaterials in atherosclerosis management and thus provide new insights to promote the further development of nanomaterials-based medicines thus better service in the field of human life and health.

Abstract:This review offers an up‐to‐date standpoint on advanced nanomaterials in atherosclerotic diagnostic imaging and/or therapy, biosafety, challenges, and prospects based on the pathophysiology and optional animal models of atherosclerosis and the rational design concepts of nanoplatforms. With the changing disease spectrum, atherosclerosis has become increasingly prevalent worldwide and the associated diseases have emerged as the leading cause of death. Due to their fascinating physical, chemical, and biological characteristics, nanomaterials are regarded as a promising tool to tackle enormous challenges in medicine. The emerging discipline of nanomedicine has filled a huge application gap in the atherosclerotic field, ushering a new generation of diagnosis and treatment strategies. Herein, based on the essential pathogenic contributors of atherogenesis, as well as the distinct composition/structural characteristics, synthesis strategies, and surface design of nanoplatforms, the three major application branches (nanodiagnosis, nanotherapy, and nanotheranostic) of nanomedicine in atherosclerosis are elaborated. Then, state‐of‐art studies containing a sequence of representative and significant achievements are summarized in detail with an emphasis on the intrinsic interaction/relationship between nanomedicines and atherosclerosis. Particularly, attention is paid to the biosafety of nanomedicines, which aims to pave the way for future clinical translation of this burgeoning field. Finally, this comprehensive review is concluded by proposing unresolved key scientific issues and sharing the vision and expectation for the future, fully elucidating the closed loop from atherogenesis to the application paradigm of nanomedicines for advancing the early achievement of clinical applications.

Abstract:Atherosclerosis is the leading inducement of cardiovascular diseases, which ranks the first cause of global deaths. It is an arterial disease associated with dyslipidemia and changes in the composition of the vascular wall. Besides invasive surgical strategy, the current conservative clinical treatment for atherosclerosis falls into two categories, lipid regulating-based therapy and antiinflammatory therapy. However, the existing strategies based on conventional drug delivery systems have shown limited efficacy against disease development and plenty of side effects. Nanomedicine has great potential in the development of targeted therapy, controlled drug delivery and release, the design of novel specific drugs and diagnostic modalities, and biocompatible scaffolds with multifunctional characteristics, which has led to an evolution in the diagnosis and treatment of atherosclerosis. This paper will focus on the latest nanomedicine strategies for atherosclerosis diagnosis and treatment as well as discussing the potential therapeutic targets during atherosclerosis progress, which could form the basis of development of novel nanoplatform against atherosclerosis.

Abstract:Abstract Atherosclerosis and atherothrombosis, the major contributors to cardiovascular diseases (CVDs), represent the leading cause of death worldwide. Current pharmacological therapies have been associated with side effects or are insufficient at halting atherosclerotic progression effectively. Pioneering work harnessing the passive diffusion or endocytosis properties of nanoparticles and advanced biotechnologies in creating recombinant proteins for site-specific delivery have been utilized to overcome these limitations. Since CVDs are complex diseases, the most challenging aspect of developing site-specific therapies is the identification of an individual and unique antigenic epitope that is only expressed in lesions or diseased areas. This review focuses on the pathological mechanism of atherothrombosis and discusses the unique targets that are important during disease progression. We review recent advances in site-specific therapy using novel targeted drug-delivery and nanoparticle-carrier systems. Furthermore, we explore the limitations and future perspectives of site-specific therapy for CVDs. Graphical Abstract

Biomimetic nanomedicines (bNMs) are moving closer to fulfilling this need, pushing back the frontier of nano-based drug delivery systems design. This review seeks to outline how these nanomedicines (NMs) might work to diagnose and treat atherosclerosis, to trace the trajectory of their development to date and in the coming years, and to provide a foundation for further discussion about atherosclerotic theranostics.

Abstract:Cardiovascular diseases (CVDs) and the underlying process of atherosclerosis are considered to be the most frequent causes of mortality and morbidity in developed societies. Atherosclerosis constitutes a systemic, chronic and progressive inflammatory disease that is initiated by early endothelial dysfunction and is subsequently amplified by oxidative stress, lipid deposition and monocyte recruitment. An interplay occurs among diverse cells, chemoattractants, adhesion molecules and low-density lipoproteins in the subendothelium. Due to the complexity of its pathogenesis, effective therapeutic strategies have not yet been applied in routine clinical practice. With the advent of nanotechnology, nanoparticulate systems with diagnostic and therapeutic moieties for the site-specific targeting of atherosclerotic lesions as well as nanomaterials that are suitable for cardiovascular implants may offer possible solutions to certain shortfalls of current treatment regimens. This article describes the recent advances that involve different types of nanoparticles for the early detection and concurrent therapy of atherosclerotic lesions. Moreover, it provides a state-of-the-art overview of stent technology in the restoration of normal blood flow to ischemic myocardial sites and underscores its drawbacks in light of substantial nanotechnology-based improvements. Emphasis is placed on the contribution of nanomedicine to the development of novel and effective therapies for atherosclerosis, ranging from 'nanotheranostic' strategies for vulnerable plaques to the nanoporous and nanoparticulate drug-delivery platforms that have been applied to stent technology. By striking a balance between the efficacy and the potential toxicity of nanotechnology-enabled systems, new frontiers in atherosclerosis treatment will emerge.

Abstract:Nanotechnology could improve our understanding of the pathophysiology of atherosclerosis and contribute to the development of novel diagnostic and therapeutic strategies to further reduce the risk of cardiovascular disease. Macrophages have key roles in atherosclerosis progression and, therefore, macrophage-associated pathological processes are important targets for both diagnostic imaging and novel therapies for atherosclerosis. In this Review, we highlight efforts in the past two decades to develop imaging techniques and to therapeutically manipulate macrophages in atherosclerotic plaques with the use of rationally designed nanoparticles. We review the latest progress in nanoparticle-based imaging modalities that can specifically target macrophages. Using novel molecular imaging technology, these modalities enable the identification of advanced atherosclerotic plaques and the assessment of the therapeutic efficacy of medical interventions. Additionally, we provide novel perspectives on how macrophage-targeting nanoparticles can deliver a broad range of therapeutic payloads to atherosclerotic lesions. These nanoparticles can suppress pro-atherogenic macrophage processes, leading to improved resolution of inflammation and stabilization of plaques. Finally, we propose future opportunities for novel diagnostic and therapeutic strategies and provide solutions to challenges in this area for the purpose of accelerating the clinical translation of nanomedicine for the treatment of atherosclerotic vascular disease.
