Abstract:BACKGROUND AND IMPORTANCE: A subset of invasive pituitary adenomas invade not only the medial wall of the cavernous sinus but can progress superiorly through the cavernous sinus roof at the oculomotor triangle and reach the subarachnoid parapeduncular space. We describe a series of 2 of 3 cases where an endoscopic endonasal approach was used to reach the parapeduncular space through the oculomotor triangle for tumor decompression. Images of the third case are presented. CLINICAL PRESENTATION: Case 1: We present a 2-dimensional surgical video of a recurrent corticotroph adenoma post gamma knife radiotherapy which was invading the left cavernous sinus and extending into the left parapeduncular space. Histopathological examination revealed densely granulated corticotrophin adenoma. The patient had reduction in the serum cortisol level postoperatively and was induced into remission medically. Postoperative third nerve palsy recovered partially, and sixth nerve palsy recovered completely at the 3-month follow-up. Case 2 : A case of recurrent silent corticotrophin adenoma invading the right parapeduncular space through the right cavernous sinus was operated through the same approach as case 1. Only a subtotal excision of the tumor in the cisternal space was possible. The patient developed a complete right third cranial palsy in the immediate postoperative period with near total recovery at the 6-month follow-up. CONCLUSION: Endoscopic endonasal approach to the parapeduncular space through a transcavernous transoculomotor route is reasonably safe and effective, as long as key anatomic landmarks and structures are identified and preserved while using natural tumor corridors to achieve tumor clearance.

Abstract:Invasion of the cavernous sinus by pituitary adenomas impedes complete surgical resection, compromises biochemical remission, and increases the risk of further tumor recurrence. Accurate preoperative MRI-based diagnosis or intraoperative direct inspection of cavernous sinus invasion are essential for optimal surgical planning and for tailoring postoperative therapeutic strategies, depending on whether a total resection has been achieved, or tumoral tissue has been left in surgically inaccessible locations. The molecular mechanisms underlying the invasive behavior of pituitary adenomas remain poorly understood, hindering the development of targeted therapies. Some studies have identified genes overexpressed in pituitary adenomas invading the cavernous sinus, offering insights into the acquisition of invasive behavior. Their main limitation however lies in comparing purely intrasellar specimens obtained from invasive and non-invasive adenomas. Further, precise anatomical knowledge of the medial wall of the cavernous sinus is crucial for grasping the mechanisms of invasion. Recently, alongside the standard intrasellar surgery, extended endoscopic intracavernous surgical procedures with systematic selective resection of the medial wall of the cavernous sinus have shown promising results for invasive secreting pituitary adenomas. The first- and second-generation somatostatin agonist ligands and cabergoline are used with variable efficacy to control secretory activity and/or growth of intracavernous remnants. Tumor regrowth usually requires surgical reintervention, sometimes combined with radiotherapy or radiosurgery which is applied despite their benign nature. Unraveling the molecular pathways driving invasive behavior of pituitary adenomas and their tropism to the cavernous sinuses is the key for developing efficient innovative treatment modalities that could reduce the need for repeated surgery or radiotherapy.

Extended Transsphenoidal Approach for Pituitary Adenomas Invading the Anterior Cranial Base, Cavernous Sinus, and Clivus: a Single-Center Experience with 126 Consecutive Cases

Abstract:Object The standard transsphenoidal approach has been successfully used to resect most pituitary adenomas. However, as a result of the limited exposure provided by this procedure, complete surgical removal of pituitary adenomas with parasellar or retrosellar extension remains problematic. By additional bone removal of the cranial base, the extended transsphenoidal approach provides better exposure to the parasellar and clival region compared with the standard approach. The authors describe their surgical experience with the extended transsphenoidal approach to remove pituitary adenomas invading the anterior cranial base, cavernous sinus (CS), and clivus. Methods Retrospective analysis was performed in 126 patients with pituitary adenomas that were surgically treated via the extended transsphenoidal approach between September 1999 and March 2008. There were 55 male and 71 female patients with a mean age of 43.4 years (range 12–75 years). There were 82 cases of macroadenoma and 44 cases of giant adenoma. Results Gross-total resection was achieved in 78 patients (61.9%), subtotal resection in 43 (34.1%), and partial resection in 5 (4%). Postoperative complications included transient cerebrospinal rhinorrhea (7 cases), incomplete cranial nerve palsy (5), panhypopituitarism (5), internal carotid artery injury (2), monocular blindness (2), permanent diabetes insipidus (1), and perforation of the nasal septum (2). No intraoperative or postoperative death was observed. Conclusions The extended transsphenoidal approach provides excellent exposure to pituitary adenomas invading the anterior cranial base, CS, and clivus. This approach enhances the degree of tumor resection and keeps postoperative complications relatively low. However, radical resection of tumors that are firm, highly invasive to the CS, or invading multidirectionally remains a big challenge. This procedure not only allows better visualization of the tumor and the neurovascular structures but also provides significant working space under the microscope, which facilitates intraoperative manipulation. Preoperative imaging studies and new techniques such as the neuronavigation system and the endoscope improve the efficacy and safety of tumor resection.

Abstract:OBJECTIVE Tumors located in the retrochiasmatic region with extension to the third ventricle might be difficult to access when the pituitary-chiasmatic corridor is narrow. Similarly, tumor extension into the interpeduncular and retrosellar space poses a major surgical challenge. Pituitary transposition techniques have been developed to gain additional access. However, when preoperative pituitary function is already impaired or the risk of postoperative panhypopituitarism (PH) is considered to be particularly high, removal of the pituitary gland (PG) might be the preferred option to increase the working corridor. The aim of this study was to describe the relevant surgical anatomy, operative steps, and clinical experience with the endoscopic endonasal pituitary sacrifice (EEPS) and transsellar approach. METHODS This study comprised anatomical dissections to highlight the relevant surgical steps and a retrospective case series reporting clinical characteristics, indications, and outcomes of patients who underwent EEPS. The surgical technique is as follows: both lateral opticocarotid recesses are exposed laterally, the limbus sphenoidale superiorly, and the sellar floor inferiorly. After opening the dura, the PG is detached circumferentially and mobilized off the medial walls of the cavernous sinuses. The descending branches of the superior hypophyseal artery are coagulated, and the stalk is transected. After removal of the PG, drilling of the dorsum sellae and bilateral posterior clinoidectomies are performed to gain access to the hypothalamic region, interpeduncular, and prepontine cisterns. RESULTS From 2018 to 2023, 11 patients underwent EEPS. The cohort comprised mostly tuberoinfundibular craniopharyngiomas (n = 8, 73%). Seven (64%) patients had partial or complete anterior PG dysfunction preoperatively, while 4 (36%) had preoperative diabetes insipidus. Because of the specific tumor configuration, the chance of preserving endocrine function was estimated to be very low in patients with intact function. The main reasons for pituitary sacrifice were impaired visibility and surgical accessibility to the retrochiasmatic and retrosellar spaces. Gross-total tumor resection was achieved in 10 (91%) patients and near-total resection in 1 (9%) patient. Two (18%) patients experienced a postoperative CSF leak, requiring surgical revision. CONCLUSIONS When preoperative pituitary function is already impaired or the risk for postoperative PH is considered particularly high, the EEPS and transsellar approach appears to be a feasible surgical option to improve visibility and accessibility to the retrochiasmatic hypothalamic and retrosellar spaces, thus increasing tumor resectability.

Endoscopic-assisted Transsphenoidal Approach for the Resection of Pituitary Adenoma

Abstract:Objective To explore the technique of transsphenoidal approach surgery under endoscopic assistance for pituitary adenoma.Methods Totally 83 patients with pituitary adenoma,who were admitted to our hospital from June 2004 to June 2012,were enrolled into this study.Endoscopic-assisted procedure was performed on the patients via a single-nostril approach through the middle nasal septum.After exploring the opening of the sphenoidal sinus,we extended the gap at the sella floor,and then removed the adenoma starting from the posterior part,followed by the bilateral parts and then the anterior part.Residual tumor tissues were detected by endoscopy at the end of the procedure.Results The procedure was completed in all the 83 patients,and none of them died.Among the 83 cases,55 out of the 62 patients who had vision and visual field change,were improved.The level of hormones were recovered after two weeks of operation in 26 patients,and improved in 30.Follow-up by MRI after three months of operation showed no residual tumor tissues in 49 patients,residual tissues <50% in 30,and >50% in 4.Conclusions Transsphenoidal approach suits most of the operations for pituitary adenoma.The keys of the procedure include the extent of seller floor resection,and the order of tumor resection.Endoscopic-assisted procedure is superior in keeping surgical filed clear and removing the tumor completely.

Abstract:Objective: Previous anatomical studies have divided the cavernous sinus (CS) into four compartments. Among these, the lateral compartment is the most common location for residual tumor given the risk of neurovascular injury. Our goal was to delineate the anatomical landmarks in the lateral component of CS and illustrate the technical nuances of the lateral transcavernous approach for invasive pituitary adenomas and other parasellar lesions.

Abstract:Objective To study the value of extended transsphenoidal approach to invasive pituitary adenoma.Method Between September 1999 and March 2007,117 patients with invasive pituitary adenoma underwent surgery via extended transsphenoidal approach.The clinical data and follow-up of this series were retrospectively analyzed.Resuits 14 adenomas extended anteriorly to anterior cranial base,103 laterally to cavernous sinus,27 posteriorly to elivus.Total tumor removal was achieved in 73 cases,subtotal removal in 40,and partial removal in 4.Transient postoperative complications included 7 cases of CSF fistulae,5 cases of partial cranial nerve palsy,and 5 cases of acute panhypopituitarism.Permanent neurological complications included 2 cases of carotid artery injury,2 cases of monocular blindness and one case of diabetes insipidus.There was no perioperative mortality.The follow-up period ranged from 3 months to 8 years.The tumor recurrence was observed in two patients and then was treated with gamma knife.There was no reoperation.Conclusion The extended transsphenoidal approach could provide excellent exposure of tumors.The rate of total tumor removal is high,while the complication is few.This procedure has been proved to be an effective and perfect way to remove invasive pituitary adenomas.Operation,radiotherapy and medicine treatment are comprehensive treatment for invasive pituitary adenomas.
Neuroendoscopic microsurgical treatment of pituitary adenomas using bilateral endonasal approach with fixed arm

Abstract:Objective To explore the efficacy and safety of the neuroendoscopic microsurgical treatment of pituitary adenomas using bilateral endonasal approach with fixed arm.Methods A total of 57 patients with pituitary adenomas underwent neuroendoscopic microsurgical operations using bilateral endonasal transsphenoid approach with fixed arm at Department of Neurosurgery,the 2nd Affiliated Hospital of Nanjing Medical University from January 2015 to December 2015.Their clinical data were retrospectively analyzed.Extracapsular resection was performed for lesions with pseudocapsules,and the diving technique was applied to improve the rate of gross total resection (GTR).Results Among the 57 patients,GTR was achieved in 48(84％) cases,subtotal resection in 7 (12％) cases,and partial resection in 2 (4％).The hormone levels were retested at 12 hours post operations,and the results revealed that the prolactin levels in 28(88％,28/32) cases and the levels of growth hormone and insulin-like growth factor 1 in 8 (80％,8/10) cases recovered to normal.Postoperative improvement of vision and visual field occurred in 30 (88％,30/34) cases.Three patients developed cerebrospinal fluid rhinorrhea 2 weeks to 2 months post operations,which resolved after endoscopic repair of saddle bottom and lumbar cistem drainage.No serious complications such as intracranial infection or rupture of internal carotid artery were observed.Conclusion Neuroendoscopic microsurgery using bilateral endonasal approach with fixed arm,combined with pseudocapsule-based extracapsular resection and the diving technology,could be performed to significantly increase the GTR rate of pituitary adenomas and improve the surgical safety.
Simultaneous combined keyhole mini-transcranial approach and endoscopic transsphenoidal approach to remove multi-lobulated pituitary neuroendocrine tumor with suprasellar extension

Abstract:Background: Transsphenoidal surgery (TSS) is the main method to remove pituitary neuroendocrine tumor (PitNET), but large or multi-lobulated one is still challenging. Objective: The less invasive simultaneous combined keyhole mini-transcranial approach and standard transsphenoidal approach was used to remove multi-lobulated or hourglass-shaped non-functioning PitNET to avoid the specific complications related to extended TSS. Methods: Twenty-one patients of large multi-lobulated or hourglass-shaped PitNET (6 patients were recurrent cases after TSS or transcranial approach) were treated by this simultaneous combined approach. The supraorbital or midline keyhole approach was performed under the operating microscope so that the fine arteries feeding optic apparatus were dissected from the tumor capsule safely and securely and the suprasellar part of the tumor was removed in the extra-capsular fashion in combination with transsphenoidal approach. Results: Visual function improved after the surgery in 14 (74 %) of 19 patients with preoperative disturbance of the visual fields. Four patients (19 %) developed postoperative deterioration of pituitary endocrine functions requiring permanent hormonal replacement. One patient (4.2 %) suffered cortical laminar necrosis without permanent disorders and another patient (4.2 %) suffered cerebrospinal fluid rhinorrhea but neither was permanent. Postoperative computed tomography detected no subarachnoid hemorrhage in any patient. Gross total removal was achieved in 15 patients (71 %). Five patients (24 %) with subtotal removal and one patient (4.2 %) with partial removal had lateral tumor extension with Knosp classification 3 or 4. Conclusion: The combined method is an effective treatment option to remove complex PitNET safely and securely with dissecting fine arteries on the tumor capsule.
Endoscopic Transnasal Transsphenoidal Surgery for Pituitary Adenoma

Abstract:Objective To evaluate the efficacy of endoscopic transnasal transsphenoidal surgery for the treatment of pituitary adenoma and the prevention of surgical complications.Methods From September 2002 to September 2008,22 patients with pituitary adenoma were treated by endoscopic transnasal transsphenoidal surgery in our hospital.The anterior wall and ostium of the sphenoid sinus were exposed during the operation so that to enlarge the floor of the sella turcica,and then the dura was incised,and the adenoma was exposed and resected.Results Nineteen cases(86.4%)had total resection and three cases(13.6%)received subtotal resection(> 90%).Two cases were identified with intraoperative cerebrospinal leakage,which were then repaired immediately.Two cases showed transient diabetes insipidus after the operation,the symptom disappeared in 7 days with conservative treatments.All of the 22 patients were followed up for 6-60 months with a mean of 21.6 months.During the period,the symptoms of headache,visual disturbance,amenorrhea,and galactorrhea were cured or improved;one patient(4.5%)had recurrence in 3 months after the operation and then was cured by re-operation without recurrence.Conclusions Endoscopic transnasal transsphenoidal surgery provides a straight approach to the sellar region,where it offers a multi-angled and close-up view of all the relevant neurovascular structures.The technique is a safe,valuable and effective procedure for removing pituitary adenomas.
Endoscopic Endonasal Transsphenoidal Surgery for Treating Pituitary Adenoma Via a Sub-Septum Mucosa Approach.

Abstract:A novel sub-suptum mucusa approach was used to investigate the surgical method via an endonasal sub-septum-mucosa approach to pituitary adenoma under endoscopy. In this work, we aim to ensure the quality of operation and to reduce the operation trauma and complications. By endoscopy, the nasal mucosa was incised in the nasal septum, and all mucosal flaps were retained, a sub-septum-mucosa surgical corridor was made where the endoscope was used in the mucosa cavity for operation and to remove the pituitary adenoma. 52 patients (28 women, mean age 46.76 years) underwent endoscopic endonasal transsphenoidal sub-septum-mucosa approach for treating pituitary adenoma. 46 patients (88.5%) underwent gross total removal (GTR) for the tumor; 6 patients (11.5%) went through partial tumor removal (PTR). After operation, all nasal mucosa was retained, no complications such as nasal bleeding, loss of sense of smell or cerebrospinal fluid rhinorrhea. Our results showed that treating pituitary adenoma using endonasal transsphenoidal surgeries by keeping nasal mucosa under neuroendoscopy tend to have smaller trauma. This novel method more advanced since it provides a clear operation field, a flexible transformation of operation modes and leads to less postoperative complications.
Endoscopic Endonasal Pituitary Adenomas Surgery: the Surgical Experience of 178 Consecutive Patients and Learning Curve of Two Neurosurgeons

Abstract:Background: We aim to study surgical technique and analyze the related factors affecting tumor total removal and postoperative endocrinological remission for endoscopic endonasal pituitary adenomas surgery.Methods: We retrospectively analyzed 178 endoscopic endonasal pituitary adenomas surgery from March 2011 to May 2014. Endonasal approach included the routine transnasal-sphenoidal approach, transnasal- maxillary sinus approach in four cases and transnasal- clivus approach in one case.Results: According to postoperative imaging data and endocrine examination results, total removal was achieved in 129 patients (72.5%), and endocrinological remission was achieved in 38 patients with functional adenomas (44. 1%). Statistical analysis of the clinical data showed that total removal rate was much closely related to tumor volume (P = 0.006), and tumor invasiveness (P < 0.001).Conclusions: In this study, we found tumor sizes and invasion of cavernous sinus were related to total removal rate and endocrinological remission rate; the direction and degree of tumor invasion, and the surgeon's experience were the key influence factors of the endocrinological remission rate for invasive functional pituitary adenomas.
Resection of Pituitary Adenoma by Unilateral Endonasal Transsphenoidal Approach Microsurgery

Abstract:Objective To summarize the experience in treating pituitary adenoma by microsurgery through unilateral endonasal transsphenoidal approach.Methods From January 2002 to October 2006,microsurgical resection through unilateral endonasal transsphenoidal approach was performed in our hospital on 32 patients with pituitary adenomas including microadenomas(diameter10 mm)in 2,small adenomas(10～20 mm)in 9,middle adenomas(20～30 mm)in 16,macroadenomas(30 mm)in 5.Results Of 32 patients with pituitary adenoma,30 received total resection of the tumors and 2 subtotal.Pathological examination revealed prolactinomas(PRL)-secreting adenomas in 27,growth hormone(GH)-secreting adenomas in 3,and PRL+GH-secreting adenomas in 2,all were improved by treatment.No patients died of operation.There was temporal diabetes insipidus in 6 patients and cerebrospinal fluid rhinorrhea in 2.The following-up for mean 4 months showed that the patients were improved in the clinical symptoms,and the hormone level decreased in 32,and the operative effect was good.Conclusion The microsurgery through unilateral endonasal transsphenoidal approach for the pituitary adenomas had advantages such as minimal invasion,short operative time,few complications, good preservation of pituitary function and fast rehabilitation as well as very low mortality.The unilateral endonasal transsphenoidal microsurgery should be first selected for the patients with pituitary adenomas.

Abstract:Object Opening the oculomotor triangle and removing the posterior fossa lesion by endoscopic endonasal approach (EEA) is challenging for even an experienced endoscopic neurosurgeon. We summarize the treatment experience and technical nuances with EEA for resection of pituitary neuroendocrine tumors and cavernous sinus (CS) meningiomas invading through the oculomotor triangle (OT). Method Between 2018 and 2022, eight cases, including five pituitary neuroendocrine tumors (PitNET) including three nonfunctioning and two somatotroph tumors with elevated growth hormone, and 3 CS meningiomas were treated using an endoscopic endonasal transoculomotor triangle approach. The critical surgical technique is continuously opening the diaphragma sellae from medial to lateral towards the interclinoidal ligament (ICL) and transecting it to enlarge the OT. We evaluated preoperative tumor size, previous surgical history, preoperative symptoms, extent of tumor resection, histopathology, and postoperative complications for all patients. Results The GTR(gross total resection)- defined as complete removal in 3 patients (38%), near-total resection(NTR) - defined as >95% removal in 4 patients (50%), and subtotal resection (STR)- defined as up to 90 % removal in one patient (12%)and gross-total resection of tumor invading through the OT was achieved in all patients through pure EEA. Two of three patients with visual deficit in Non-Functioning-PitNET improved, and the other remained stable postoperatively. One patient showed transient oculomotor nerve palsy. The Growth Hormone level of the two patients with somatotroph tumors declined to normal. For three patients with CS meningiomas, cranial nerve palsy improved in two patients, while the other patient developed increased facial numbness after surgery. Conclusion Endoscopic endonasal transoculomotor triangle approach is an efficient surgical option for the tumor with CS invasion and OT penetration.

Endoscopic Transcranial Transdiaphragmatic Approach in a Single-Stage Surgery for Giant Pituitary Adenomas.

Abstract:BackgroundThe treatment for giant pituitary adenomas (GPAs, maximal diameter >4 cm) remains challenging, with remarkable mortality and morbidity, and there is no consensus on the optimal surgical approach. Gross total resection (GTR) for GPAs is difficult to achieve through a single transsphenoidal or transcranial approach. Any residual tumor is at risk for postoperative apoplexy. In this study, we propose a new surgical technique for resecting the GPAs in a sing-stage transcranial surgery.MethodsA retrospective review of 4 patients with complicated GPAs, who had been treated via an endoscopic transcranial transdiaphragmatic approach in a single-stage surgery after routine transcranial resection, was performed. The following data was analyzed: clinical characteristics, preoperative imaging studies, resection rate, perioperative morbidity and mortality, as well as postoperative outcomes.ResultsAll patients had nonfunctioning GPAs and preoperative visual disturbances. In three patients, GTR was achieved, and in one patient, near-total resection (90%-100% of the tumor) was achieved. Three patients attained improved postoperative visual function, while one patient’s vision remained unchanged. One patient suffered a deficiency in adrenocorticotropic hormone along with thyroid-stimulating hormone, and one patient developed diabetes insipidus. Notably, none of the patients suffered cerebrospinal fluid leakage. However, one patient developed an epidural hematoma and underwent decompressive craniectomy.ConclusionsThe endoscopic transcranial transdiaphragmatic approach in a single-stage surgery can be efficiently and safely performed for maximal excision of GPAs with extensive suprasellar extension. Furthermore, relative to the conventional combined or staged approaches, this innovative surgical strategy provides neurosurgeons with a clear operative field with reduced invasiveness.

Abstract:The trans-sphenoidal approach, commonly used for removing pituitary adenomas, has become a widely accepted and successful method. In recent years, the endoscopic trans-sphenoidal technique has emerged as a minimally invasive surgical approach for pituitary adenoma removal. The majority of pituitary adenomas exhibit a soft consistency and can be successfully extracted with aspiration and curettage using the trans-sphenoidal approach. However, a subset of around 5–15% of these adenomas possess a solid and fibrous texture. The occurrence of firm and fibrous adenomas is relatively common; unfortunately, there are no reliable predictors to identify them preoperatively.

Abstract:Pituitary adenomas are very common representing 18.1% of all brain tumors and are the second most common brain pathology. Transsphenoidal surgery is the mainstay of treatment for all pituitary adenomas except for prolactinomas which are primarily treated medically with dopamine agonists. A thorough endocrine evaluation of pituitary adenoma preoperatively is crucial to identify hormonal compromise caused by the large sellar mass, identifying prolactin-producing tumors and comorbidities associated with Cushing and acromegaly to improve patient care and outcome. Transsphenoidal surgery is relatively safe in the hands of experienced surgeons, but still carries a substantial risk of causing hypopituitarism that required close follow-up in the immediate postoperative period to decrease mortality. A multidisciplinary team approach with endocrinologists, ophthalmologists, and neurosurgeons is the cornerstone in the perioperative management of pituitary adenomas.

Abstract:Introduction Endoscopic endonasal surgery has globally improved postoperative results in pituitary adenomas. Material and Methods We retrospectively analyzed 101 patients who underwent endonasal endoscopic surgery for pituitary adenomas in the period from 2016 to 2021. Data on epidemiological variables, preoperative radiological factors including tumor volume, tumor appearance, cavernous sinus invasion (modified Knosp scale), degree of extension according to the SIPAP (stands for the five directions in which a pituitary adenoma can extend: suprasellar, infrasellar, parasellar, anterior, and posterior) classification, and preoperative visualization of the healthy gland on magnetic resonance imaging (MRI) were collected as well as intra- and postoperative cerebrospinal fluid (CSF) leak. As variables of interest, data on the degree of tumoral resection and preservation of hormonal function were collected. Results Among the preoperative factors related to greater tumoral resection, we found a lesser tumoral extension according to the SIPAP scale, and the absence of a postoperative CSF leak had a statistically significant relation with greater hormonal preservation. Conclusion The SIPAP classification is a simple-to-measure preoperative radiological variable that could predict the extent of resection, and, conversely, the occurrence of a postoperative CSF leak has been associated with an inferior endocrinological outcome in this type of surgery.

Abstract:Objective To investigate the significance of the concept of the pseudocapsule-based extra-capsular dissection technique in endoscopic transsphenoidal surgery for pituitary adenomas.Methods We performed a retrospective analysis on clinical data of 287 patients with pituitary adenomas who underwent endoscopic endonasal transsphenoidal surgeries at Neurosurgery Department of Zhongshan Hospital,Fudan University from October 2011 to December 2015.Patients were divided into two groups:pseudocapsule-based extra-capsular dissection group (157 cases) and intra-capsular resection group (130 cases).We compared their surgical and endocrinological outcomes and complications.Results The total resection rate in pseudocapsule-based extra-capsular dissection group was 89.8％ (141/157) while that in intra-capsular resection group was 81.5％ (106/130) (P =0.044).Furthermore,the recurrence rates in the extra-capsular and intra-capsular resection groups were 1.9％ (3/157) and 6.9％ (9/130),respectively (P =0.035).Besides,with the application of extra-capsular dissection technique,the remission rate of functional adenoma in terms of biochemical parameters increased [56.5％ (26/46) vs.86.2％ (50/58),P =0.001].In addition,there was no difference in the rate of hypopituitarism,CSF leakage,intracranial infection or temporary polyuria (P ＞ 0.05).Conclusions The application of pseudocapsulebased dissection technique in endoscopic transsphenoidal surgery for pituitary adenomas could lead to a high rate of gross total tumor resection and endocrinological remission.Extra-pseudocapsular resection does not seem to cause additional postoperative complications.
