Comparison of chronic cardiovascular and non-cardiovascular disease burden in elderly patients with different types of CHF

Abstract:Objective To analyze the differences in burdens of common chronic cardiovascular and non-cardiovascular diseases among the elderly inpatients with 3 different types of CHF.MethodsA retrospective analysis was performed on 4650 senior inpatients(≥60 years old) admitted to the First Medical Center of Chinese PLA General Hospital between February 2008 and December 2019.According to their left ventricular ejection fraction(LVEF),they were divided into heart failure with reduced ejection fraction group(HFrEF,LVEF<40%,n=1627),heart failure with mildly reduced ejection fraction group(HFmrEF,LVEF 40%~49%,n=723) and heart failure with preserved ejection fraction group(HFpEF,LVEF≥50%,n=2300).The prevalences of 18 common chronic cardiovascular and non-cardiovascular diseases, the number and spectrum of multimorbidity were analyzed and compared in the 3 groups.Results The senior CHF patients had averagely 1.12±0.73 cardiovascular diseases and 2.90±1.32 non-cardiovascular diseases.The numbers of cardiovascular, non-cardiovascular and all chronic diseases were 1.17±0.70,3.06±1.37 and 4.23±1.67,respectively in the HFpEF group, which were significant higher than those in HFmrEF and HFrEF groups(P<0.05).Dyslipidemia & anemia & chronic kidney disease and dyslipidemia & coronary heart disease & hypertension & anemia were the most prevalent patterns of ternary non-cardiovascular multimorbidity and quaternary chronic multimorbidity, respectively.Conclusion There are significant differences in the disease spectrum and burden of cardiovascular and non-cardiovascular diseases among the elderly CHF patients, and the HFpEF patients have the highest burden of non-cardiovascular multimorbidity.Attention should be paid to comprehensive prevention and treatment of chronic illnesses, such as dyslipidemia, coronary heart disease, hypertension, anemia, and chronic renal disease for the elderly CHF patients.
