Abstract:Objective To study the diagnosis and treatment of Ewing's sarcoma in mandible.Methods 4 cases with Ewing's sarcoma of Mandible were analyzed and the related articles were reviewed and discussed.Results All the cases occurred in mandible, and were treated with synthetic therapy. Their survival time was less than 5 years. Conclusion Ewing' s Sarcoma of Mandible was one of the most malignent tumors in bone. It was often misdiagnosed, so it must be treated with synthetic therapy.
Ewing's Sarcoma of Maxillary Sinus

Abstract:Ewing's sarcoma is a small round cell tumor commonly involving long bones. Head and neck involvement is seen less frequently with maxillary sinus being the rarest site. Here we report a case of a12-year-old female patient presented with left side facial swelling for 3 weeks duration which was diffuse and hard on palpation. CT and MRI of Nose & para nasal sinus revealed a mass lesion in the left maxillary sinus infiltrating and eroding the orbital wall and extending intracranially. Biopsy from the lesion revealed Ewing's sarcoma with strong positivity for CD-99 marker. Patient was referred to radiotherapy due to extensive disease at younger age. The CT and MRI along with HPE and IHC markers enables early diagnosis and prompt treatment that leads to better survival and prognosis, as unlike Ewing's sarcoma of long bones, that metastasize early. The combined chemoradiotherapy is now the standard line of treatment along with close follow up for at least 2 years.

Abstract:To present the neuroradiological and clinical characteristics of Ewing's sarcoma family of tumours (ESFTs) and to increase awareness of this neoplasm.The magnetic resonance imaging (MRI) features and clinical presentations of seven patients with pathologically documented ESFTs were retrospectively analysed. The tumour location, morphological features, signal intensity, contrast enhancement characteristics, involvement of the paraspinal soft tissues, and adjacent bony structures were assessed.Most of the ESFTs in young adults were well-circumscribed. The present study demonstrated that ESFTs often have a hypo- or iso-intense signal on T1-weighted imaging and an iso-intense signal on T2-weighted imaging. Spinal ESFTs tended to present homogeneous signal intensity and diffuse enhancement. ESFTs are more likely to occur in the thoracic spine and later to infiltrate into the paraspinal area or vertebral bone. A broad dural attachment is another common feature in the cases presented here.ESFT is a rare neoplasm that can have significant overlap in imaging appearance compared with other spinal neoplasms. A well-demarcated extradural mass invading the paraspinal soft or vertebral bones, with iso-intense on T2 weighted imaging and homogeneous enhancement could facilitate the diagnosis of spinal ESFT.
Ewing's Sarcoma of Mandible: A Case Report with Review of Literature

Abstract:Background: Ewing sarcoma (ES), a rare malignancy, comprises whatever the age, 4-15% of all primary bone tumors. It represents 1% of all malignant tumors in children and is the fourth most common bone malignancy after myeloma, osteosarcoma, and chondrosarcoma. Case description: A 12-year-old boy came to the Oral Surgery Department of Bretonneau Hospital referred by his dentist with a rapidly evolving swelling in the left mandibula for 6 weeks, which was initially diagnosed as a facial cellulitis. Cone beam computed tomography (CBCT) showed a poorly defined, expansile, and osteolytic tumor on the left side of the mandible. Clinical and radiographic findings were in favor of an aggressive primitive bone tumor. A mandibular biopsy under general anesthesia was performed in the Department of Surgical Oncology at Institut Curie in Paris, revealing an ES. Conclusion: Mandibular ES can mimic dental infections when swelling is the main clinical manifestation, which can lead to a delayed diagnosis. A correlation between clinical, radiological, histopathological, and immunohistochemical with cytogenetics is needed to confirm the diagnosis. Moreover, smaller tumors have better survival.Dentists must therefore be aware of the clinical signs of ES in order to quickly refer patients to a specialized department. Extraskeletal Ewing's Sarcoma: a Report of 18 Cases Andliterature Review

Abstract:BACKGROUND AND OBJECTIVE:Extraskeletal Ewing's sarcoma (EES) is a rare, rapidly growing, round-cell, malignant tumor that can develop in the soft tissues at any location. This study was to analyze the clinical features, diagnosis and treatment of EES.METHODS:Clinical data of 18 patients with EES, treated at between Cancer Center of Sun Yat-sen University between 1995 and 2007, were analyzed.RESULTS:Of the 18 patients, 13 were male and 8 were female, aged from 8 months to 60 years. Twelve (66.7%) patients were between 5-25 years of age. Eight (44.4%) patients had tumors originated from low extremities.Sixteen patients had masses at their first visit. Sixteen patients were treated by the combined modality therapy, and 2 patients were treated by the single modality therapy. The 1-, 3- and 5- year actuarial survival rates were 82.4%, 64.2% and 32.1%, respectively. The presence of metastatic disease at the time of diagnosis and the mode of treatment were prognostic factors.CONCLUSIONS:EES is common in adolescent. It often manifests as a localized mass. The combined modality therapy is recommended for this disease. The presence of metastatic disease at the time of diagnosis and the mode of treatment are prognostic factors.

Abstract:Objective To analyze the clinical,pathological and MR imaging features of Ewing's sarcoma of vertebral column.Methods Nine cases of Ewing's sarcoma of vertebral column was proved by surgical or biopsy pathology.6 of 9 patients underwent MR imaging.Their clinical and pathological features and MR imaging manifestations were retrospectively analyzed.Results In 6 patients with MR imaging,2 tumors arose each in cervical,thoracic and lumbar vertebrae,respectively,the remaining 3 tumors in sacral vertebrae.The tumor showed isointensity in 4 cases and slight hypointensity in 2 cases on T1-weighted imaging,and heterogeneous hyperintensity signal on T2-weighted imaging and heterogeneously enhanced after contrast enhancement.Soft tissue mass were found in 6 tumors and no calcification and periosteal reaction were seen.Hematoxylin-Eosin staining demonstrated uniform small round cells in all cases and without Homer-Wright(H-W) rosettes.PAS staining were positive in 6 cases.2 cases were slightly positive for NSE.Conclusions The Ewing's sarcoma of vertebral column has some characteristic manifestations but not pathognomonic on MR imaging.The Ewing's sarcoma of vertebral column should be reminded when the patients are children or adolescent,and the lesions show obviously osteolysis of vertebral body and a concurrent large soft tissue mass.

Abstract:Objective To investigate the imaging features of Ewing's sarcoma and primitive neuroectodermal tumor (ES and PNET) in sacrum.Methods Imaging data were retrospectively collected and reviewed in 17 patients with ES and PNET confirmed by pathology.X-ray was performed in all patients,CT scans in 13 patients,and MR scans in 10 patients.Results All lesions were detected solitary and lateral.X-ray and CT features were lytic bone destruction in 12 cases,2 of them with bone sclerosis; one patient showed diffuse sclerosis.Soft tissue mass in pelvic cavity can be seen in 11 cases,9 of them with adjacent large blood vessels invasive and 4 patients with sacroiliac joints invasive.On post-contrast images,mild enhancement was found in 8 cases,and 5 patients with marked enhancement.MR images showed intermediate signal intensity on T1WI in 8 cases and heterogeneous moderate to high signal intensity on T2WI/fat suppression in 10 cases.DWI showed high signal intensity in 4 cases.Unlike CT scan,contrastenhanced MR images showed significant enhancement in 7 cases.Conclusions Ewing's sarcoma and primitive neuroectodermal tumor (ES and PNET) in sacrum have some specific imaging features.Imaging examination could provide reliable evidence for clinical diagnosis.

Abstract:Objective: To describe the difference of the clinical symptoms and radiologic features between Ewing sarcoma(ES) and peripheral primitive neuroectodermal tumors(pPNETs) of bone, and to benefit from the differential diagnosis and treatment. Materials and Methods: We retrospectively reviewed 18 cases of ES and 10 cases of pPNETs all were confirmed pathologically. Radiologic and clinical studies were reviewed by two musculoskeletal radiologists. In ES, radiographs(n=18), CT(n=12), MRI(n=9) were performed. In pPNETs, radiographs(n=10), CT(n=7), MRI(n=10) were performed too. The evaluation included clinical presentation,lesion location and size,bone erosion/destruction,periosteal reaction, soft tissue involvement and intrinsic characteristic findings on CT and MRI. Results: Patients' average age was 18.5 years (range 10～28) and 24.4 years(range 8～86) in ES and pPNETs respectively. For the ES, the lesion locations included femur(n=6), humerus(n=4), clavicle(n=2), tibia(n=1), fibula(n=1), ulna(n=1), ilium(n=1), sacrum(n=1) and acetabulum(n=1); Radiographs(n=18) showed lytic lesion(7/18), with irregular sclerosis and matrix mineralization(11/18), soft tissue mass(10/18), periosteal reaction(10/18), eccentric(6/18), scalloping of cortex(3/18); CT(n=12) showed lytic lesion(4/12), lesion with irregular sclerosis and matrix mineralization(8/12), soft tissue mass(10/12), periosteal reaction(6/12); On MRI(n=9), T1WI showed isointensity(8/9) and slightly hyperintensity(1/9), T2WI and STIR images showed heterogeneous hyperintensity(9/9), soft tissue mass(8/9), the size ranged from 2cm×3cm to 3cm×10cm. For the pPNETs, the lesion locations included ilium(n=3), humerus(n=2), corpus vertebrae(n=1), arcus vertebrae(n=1), scapula(n=1), sacrum(n=1), sphenoid(n=1); Radiographs(n=10) showed lytic lesion(8/10), with irregular sclerosis and dilation(1/10), and normal(1/10), soft tissue mass(7/10), periosteal reaction(0/10). CT(n=7) showed lytic lesion(6/7), lytic lesion with irregular sclerosis and dilation (1/7), soft mass(7/7), speculate calcification(2/7), periosteal reaction(0/10); On MRI(n=10), T1WI showed isointensity(9/10) and slightly hyperintensity(1/10), T2WI and STIR images showed heterogeneous hyperintensity (8/10), uniform hyperintensity(2/10), soft tissue mass(10/10), the size ranged from 2.5cm×4cm to 7.5cm×13.5cm. Conclusion: The clinical and radiologic features of ES and pPNETs were different, compared with pPNETs, the patients with ES were younger, the commoner locationswere long tubular bones, radiography and CT showed periosteal reaction and matrix mineralization, MRI showed the similar intensity but with smaller soft tissue mass, these features were helpful in differential diagnosis if combining with pathology.
Clinicopathologic Study of Epithelioid Sarcoma

Abstract:Objective To review the clinicopathologic features and differential diagnosis of epithelioid sarcoma(ES).Methods Ten cases of ES,6 males and 4 females aged 21～50 years(mean 35 years) were included in the study.Tissue specimens were investigated by light microscope and immunohistochemistry(En Vision method).The clinical data were analyzed.Results Clinically,all patients presented a slowly growing mass,4 tumors were located in the hands,2 in forearm,and others in arm,foot,lower leg and back.Grossly,ES presented as indurated ill-defined nodules,measuring 2.0 to 8.4 cm(mean 5.2 cm).The cut surface showed a gray-white lesion with focal necrosis.Histologically,ES showed a nodular pattern,which underwent central necrosis resulting in a pseudogranulomatous appearance.The tumors consisted of large polygonal cells and plump spindle-shaped cells with deeply eosinphilic cytoplasm.Immunohistochemically,all cases were immunoreactive with CK,Vim.Nine cases were positive for epithelial membrane antigen,6 for CD34.S-100 and smooth muscle actin were focally expressed only in one case.Myoglobin,FⅧRAg,CD31 and HMB45 were negative.Conclusions ES is an uncommon soft tissue tumor and mostly occurs in the distal extremities with undetermined differentiation and complex immune phenotype.

Abstract:Objective:To improve the clinical recognition of extraskeletal Ewing's sarcoma and avoid the misdiagnosis.Method:Clinical manifestation of a patient with extraskeletal Ewing's sarcoma was analyzed and the literatures were reviewed.The diagnosis,differential diagnosis and treatment of tumors were investigated.Result:Early diagnosis is difficult and the diagnosis should mainly rely on the pathological.Microscopically,undifferentiation small round cells were arranged in variable patterns of diffuse growth,lobular or nested.Immunohistochemistry,CD99 was expressed in all the tumor cells.Conclusion:Extraskeletal Ewing′s sarcoma is an uncommon malignant round cell tumor that arises from the soft tissue,and it have poor prognosis and can be effectively treated with combination therapy.
Clinicopathologic and Molecular Cytogenetic Analysis of 8 Cases with Uterine Cervical Ewing Sarcoma: Case Series with Literature Review.

Abstract:Ewing sarcoma (ES) is a highly malignant tumor that rarely occurs in the uterine cervix. Herein, we report 8 cases with ES arising primarily in the uterine cervix by focusing on clinicopathologic and molecular cytogenetic features and differential diagnoses. Eight cases of cervical ES were diagnosed between February, 2012, and September, 2018. The age of patients ranged from 13 to 47 years. Abnormal vaginal bleeding and lower abdominal pain were the most common symptoms. Histologically, the tumor was composed of uniform, round, and oval cells with a narrow rim of eosinophilic cytoplasm. Fibrous septa were observed between tumor cell nests. The tumors showed brisk mitotic activity and areas of coagulative necrosis. According to immunohistochemical studies, 50% (4/8) of the cases were positive for cytokeratin (AE1/AE3), and 87.5% (7/8) were positive for synaptophysin, which resulted in a diagnostic confusion with small cell carcinoma, primarily when dealing with small cervical biopsies. Molecular testing demonstrated the rearrangement of the EWSR1 gene in all of the 8 cases, which confirmed the diagnosis of ES. Although rare, ES should be considered as indicators of cervical small round cell neoplasms. Molecular analysis may greatly contribute to the final diagnosis of ES occurring in this unusual location.

Abstract:Ewing sarcoma (ES) usually arises from long bones and affects the head and neck region in only 1%–4% of cases. We reported clinical, radiographic, cytomorphologic, and histomorphologic findings of the ES in the mandible, because of its rarity and radiologically misinterpreted as a parotid gland tumor. A 26‐year‐old male patient presented with a history of painfull cheek swelling. On magnetic resonance imaging, a mass measuring 50 × 48 × 45 mm was found eroding mandible and pushing back the parotid gland. Aspiration cytology was performed with suspicion of parotid gland tumor. Small, nucleated cells with nuclear indentation, inconspicuous nucleoli, and occasionally rosette‐like arrangement were observed. Neuroendocrine immune markers were positive on cell block. It was diagnosed as small round cell neoplasm with neuroendocrine differentiation and biopsy was suggested. The differential diagnosis considered soft tissue and parotid gland tumors. The small round cell tumor morphology was seen on biopsy specimen and immunostaining was applied. The diagnosis for this case was ES of the mandible. ES of the mandible is unusual. Although the histogenesis is still unknown, various cells have been proposed as cells of origin namely, endothelial, hematopoietic, fibroblastic, mesenchymal stem cells or neural derived mesenchymal stem cells. Small cell morphology, CD99, CD56, neuron specific enolase, and synaptophysin expressions confirmed the diagnosis of ES. The differentiation of the ES from other small cell tumors may be difficult and requires awareness for histological and immunohistochemical features. It should be kept in mind that the diagnosis can be challenging due to uncommon locations and radiological misinterpreted.

Abstract:Ewing sarcomas (EWS) are highly malignant neoplasms of mesenchymal origin that are rare in the head and neck. Only a few laryngeal EWS have been reported in the literature. We report a 47 years-old man who visited our hospital for medical help after 5 months of hoarseness and sore throat. Computed tomography (CT) showed uneven thickening of the epiglottis fold, right vocal cord, and anterior union. In addition, fluoro-18-fluorodeoxyglucose positron emission tomography ( 18 F-FDG PET)/CT has confirmed high activity in the already known laryngeal and nodal lesions, and has revealed otherwise unknown skeletal metastases. We also reviewed the published clinical features, histopathology, and imaging findings of nine patients with laryngeal EWS confirmed by pathology. The main clinical manifestations of laryngeal EWS are rapidly growing lumps, hoarseness, acute respiratory distress, and aphonia. The EWS tumor cells usually express CD99, vimentin, synaptophysin (Syn), and neuron-specific enolase (NSE) but do not express common antigen (LCA), CD20, and chromaffin granin (CgA). Laryngeal EWS’ CT imaging characteristics are mainly homogeneous, well-bounded soft-tissue masses. Our case suggests that EWS should be considered a differential diagnosis of laryngeal cancer, especially when PET/CT reveals distant bone metastasis, which is more likely to indicate EWS.

Abstract:Objective To improve the clinical recognition of Ewing's sarcoma and to avoid the misdiagnosis and mistreatment.Methods Clinical manifestation of a 31-year-old male patient with Ewing's sarcoma was analyzed and literature was reviewed.The etiology,diagnosis,differential diagnosis and treatment of tumors were investigated.Results Ewing's sarcoma,a rare primary benign tumor of bone,had no specific clinical manifestations and was often confused with osteomyelitis,bone tuberculosis and lymphocyte leukemia.Therefore,the diagnosis of Ewing's sarcoma depended on the pathology.The mortality rate is high.Because of the high degree of malignancy and high mortality,the chemotherapeutics,radiotherapy and surgical operation should be performed to increase survival in patients with Ewing's sarcoma.Conclusion Ewing's sarcoma is a rare bone tumor with high degree of malignancy and bad prognosis and can be effectively treated with combination therapy.Moreover,Ewing's sarcoma should be distinguish with lymphoma and lymphocyte leukemia in the morphology to avoid misdiagnosis.
A Rare Case of Ewing Sarcoma of the Left Ethmoid Sinus With Orbital Extension

Abstract:Background Ewing sarcoma (ES) of the ethmoid sinus with orbital involvement in an adult is very rare, with 16 reported cases in the literature. Immunohistochemical studies show small blue round cells positive for CD99 and fluorescence in situ hybridization (FISH) testing reveals positivity for the EWSR1 gene. Methods A 38‐year‐old male with a diagnosis of ES of the ethmoid sinus presented with left‐sided periorbital pain and edema, rhinorrhea, and proptosis. The patient underwent neoadjuvant chemotherapy, surgical resection of the left skull base, and postoperative proton radiotherapy. Results The patient tolerated chemotherapy, surgical resection, and adjuvant proton radiotherapy well with resolution of proptosis, diplopia, and pain. Due to local recurrence, he is currently undergoing adjuvant chemotherapy. Conclusion Our findings provide insight on the clinical presentation and appropriate management of extraosseous ES, specifically in the ethmoid sinus in the adult population.

Osteosarcoma of the jaws: a clinicopathological analysis of three cases

Abstract:Objective To report three cases of rare osteosarcoma of the jaws.Methods A retrospective clinicopathological study was made on three cases of osteosarcoma of the jaws in past three years in our hospital.The clinical characteristics,radiological appearance,pathological,immunohistochemical and molecular pathological findings were analyzed,and also the literatures were reviewed.Results Three cases of osteosarcoma of the jaws occurred in patients at the age of 49 years,46 years and 51 years,respectively,and two of the tumors located in the left maxilla and one in right maxilla.All the pathological diagnoses were osteosarcoma of maxilla,and one case was secondary osteosarcoma.Conclusion There are some differences including age of onset,clinical features and morphological characteristics between osteosarcoma of the jaws and ordinary osteosarcoma of long bones.Intergrative analysis of clinical,radiology and pathological findings and application of some necessary ancillary techniques such as immunohistochemistry will help inaccuracy and timely diagnosis of such rare osteosarcoma of the jaws.
[The Ultrasonographic Features of Female Reproductive System Extraosseous Ewing's Sarcoma].

Abstract:OBJECTIVE:This study investigated the clinicopathological traits and ultrasound features of female reproductive system extraosseous Ewing's sarcoma (EES) and explored the diagnostic value of ultrasonography for this condition.METHODS:Cases of female pelvic EES diagnosed and treated at our hospital between June 2009 and June 2019 were included in this study. Pathology data and ultrasound manifestations were assessed retrospectively to summarize the clinical traits and ultrasound features of female reproductive system EES. Based on the results, recommendations for the ultrasonography-based diagnosis of this disease were proposed.RESULTS:During the 10-year study period, 13 female patients were diagnosed with EES in the pelvic cavity based on the results of postoperative pathology tests. The age of the patients ranged from 8 mouth to 40 years, and no patients demonstrated specific clinical symptoms. However, an examination of tumor biomarkers revealed that certain patients had elevated levels of CA125. In the 13 patients, 19 lesions were identified, including 16 that involved the reproductive system. The primary ultrasound manifestation was uneven, low-echo solidity or cystic solidity, exhibiting large size, irregular shape, and unclear boundary. A few patients had concurrent ascites. Although some lesions lacked blood supply, the blood supply of most lesions was medium to abundant, and the blood flow was mostly characterized by low resistant. Almost none of the lesions were definitively diagnosed preoperatively.CONCLUSIONS:Preoperative definitive diagnosis of EES in the female reproductive system remains a great clinical challenge. Although certain clinical traits and ultrasound features are associated with this disease, and color Doppler ultrasonography might provide vital information indicating the presence of EES, the final diagnosis still depends on the pathological test results of the patients.
Clinical and Pathological Features of Epithelioid Sarcoma

Abstract:Objective: To study the clinical and pathological features of epithelioid sarcoma (ES) and to outline its main points of differential diagnosis. Methods: Nine patients with ES were collected, and their clinical features were analyzed with pathological aspects observed by haematoxylin and eosin staining and immunohistochemical technique (LSAB method). Results: The tumor consisted of epithelioid cells and spindle cells. Cytological atypia and mitotic activity could be identified. Cytokeratin(CK), epithelial membrane antigen (EMA), vimentin(Vim), S-100, CD34 and smooth muscle actin(SMA)were found positive staining in the tumor cells. Conclusions: ES is a kind of malignant tumor. Epithelial derivation can be confirmed by immunohistochemical staining. Considering its clinical and pathological aspects, ES should be differentiated from other tumors and granuloma.

Abstract:An 89-year-old male presented with syncope and worsening difficulty in breathing through the left nostril. Computed tomography demonstrated a tumor in the anterior ethmoid air cells and maxillary sinus, which extended into the frontal lobe. Magnetic resonance imaging similarly demonstrated an aggressive lesion. This mass was difficult to differentiate from more commonly seen lesions at this location such as an esthesioneuroblastoma or nasopharyngeal carcinoma. Direct visualization, biopsy, and subsequent pathologic analysis eventually confirmed the diagnosis of malignant Ewing sarcoma (EWS). Our case explores the radiological findings of EWS originating from the ethmoid sinus, compares EWS with other common carcinomas in the same location, confirms the diagnosis through pathological correlation, and investigates the prognosis and treatment of these lesions. This case highlights the importance of a multidisciplinary approach to diagnose EWS when it occurs in an atypical location. The clinical team relied on input from the radiology, surgery, ENT, neurology, and pathology departments to make an accurate diagnosis and plan treatment for this aggressive tumor.
