Smoking, Subdivided Blood Lipids and Bladder Cancer: A Multivariable and Mediation Mendelian Randomization Study

Abstract:Background: We chose Mendelian randomization (MR) to explore the association between smoking, a wide range of subdivided blood lipids and bladder cancer (BLCA), which has rarely been examined in previous studies.Methods: Genome‑wide association study data of smoking, blood lipids and BLCA were obtained from GSCAN consortium (~1.2 million participants), a UK Biobank subset (~120,000 participants), FinnGen consortium (2,072 cases and 307,082 controls), respectively. Firstly, inverse variance weighted (IVW), complementary and sensitivity analysis, multivariable MR and meta-analysis were conducted to validate the association between blood lipids and BLCA. Secondly, a mediation MR was conducted to clarify the association between smoking, blood lipids and BLCA.Findings: Five lipids were validated to be robustly and inversely associated with BLCA, including triglycerides in very large HDL [odds ratio [OR] (95% confidence interval [CI]) 0.85(0.76-0.96), P = 0.009], cholesterol in small VLDL [0.81(0.68-0.98), P = 0.03], free cholesterol in very large HDL [0.82(0.70-0.96), P = 0.02], total free cholesterol [0.84(0.72-0.97), P = 0.02], apolipoprotein B [0.86(0.74-0.99), P = 0.040]. All the five lipids had no heterogeneity or pleiotropy, and presented consistent direction and magnitude in IVW, weighted median and MR-Egger. In mediation MR, we observed that triglycerides in very large HDL and cholesterol in small VLDL could attenuate the effect of smoking on BLCA with a mediated proportion of -4.3% and -4.5%, respectively.Interpretation: We discovered five lipids to be robustly and inversely associated with BLCA, two of which could attenuate the effect of smoking on BLCA.Funding: This study was supported by the grants from National Natural Science Foundation of China (81902603), Fundamental Research Funds for the Central Universities (4206-413100049).Declaration of Interest: All authors declare that the research was conducted with no commercial or financial relationships that could be construed as a potential conflict of interest.
