Abstract:ZCH-2B8a (IgG2a) is a novel monoclonal antibody (McAb) generated in laboratory of Children Hospital of Medical College, Zhejiang University recently using human myeloblastic leukemia cell line KG1a as immunogen. This antibody has been submitted to the 8th International Workshop and Conference on Human Leukocyte Differentiation Antigens (HLDA8) and the results showed that the antibody recognized an unknown molecule on the surface of some blood cells. The aim of this study was to investigate the reactivity of this antibody on normal blood cells and malignant cell lines and to explore its possible application in clinical practice. The multi-parameter flow cytometry was used to analyze the expression pattern of 2B8a antigen in triplicate on normal blood components including T cells, B cells, natural killers (NK), neutrophils, monocytes, dendritic cells (DC), red blood cells (RBC), platelets (Plt), hematopoietic stem/progenitor cells derived from either bone marrow or G-CSF mobilized peripheral blood CD34(+) cells and malignant cell lines including 14 hematopoietic, 5 neuroblastoma, 1 colon cancer and 1 amniotic epithelium cell lines. The amount of positive cells > or = 20% was considered as positivity. The results showed that 2B8a antibody reacted to 3/3 specimens of blood B cells with a positive rate of 26.29% and 2/3 specimens of monocytes with an average positive rate of 59.84%. 2B8a was weakly reactive to neutrophils (23.72%) and negative for T cells, NK, DC, RBC and Plt. The antibody reacted to all 3 marrow CD34(+) cells with an average positive rate of 39.33% while it was negative for G-CSF-mobilized CD34(+) peripheral blood stem/progenitor cells (PBSC, 1.25%). Cell line analysis showed that the antibody notably reacted to three out of 4 cell lines (Raji, SMS-SB, Nalm-6 and Nall-1) with the positive rates of 98.78%, 98.61%, 94.93% respectively and weakly to one of them with 5.68% in B lineage cell lines and monoblastic cell line (U937, 67.78%) while it was only weakly positive or negative for other myeloid leukemia cell lines including Meg01 (33.40%), HL-60 (29.70%), K562 (28.19%), KG1a (16.23%) and HEL92.1.7 (8.02%). Among 4 T lineage leukemia, 5 neuroblastoma and 1 colon cancer cell lines tested, only Molt-3 was found weakly positive (31.40%) for 2B8a, while the remaining 3 T cell lines (Molt4, JM and CCRF-CEM), 5 neuroblastoma cell lines (LA-N1, KCNR, BE, SK-N-SH, SK-N-AS) and the colon cancer cell line (HR8348) tested were negative. An amniotic epithelium cell line (FL) was showed positive for the antibody (45.03%). It is concluded that 2B8a antibody primarily reacts to B lineage and monocytic lineage cells which may bear the diagnostic and therapeutic applications among different types of hematopoietic malignancies.

Monoclonal Antibodies in Immunodiagnosis and Immunotherapy

Abstract:From the proteomic analysis, we identified hundreds of novel proteins that have never been characterized for their expression profile and function on human embryonic stem (hES) cells. In this study, we produced a group of monoclonal antibodies against the GRAM domain-containing protein 1A, which was found on hES cells. Using these antibodies, we analyzed the expression of GRAMD1A in various tissues and tumor cell lines. The results showed that GRAMD1A is expressed in the nucleus and cytoplasm of hES cells, cancer cell lines and ectoderm, mesoderm, and endoderm tissues. The development of the monoclonal antibody to GRAMD1A and the characterization of the expression pattern of this protein could have significant implications in the functional characterization of this novel protein.
Preparation and Characterization of a Monoclonal Antibody Against Human B Lymphocyte Stimulator

Abstract:B lymphocyte stimulator (BLyS) is a member of the tumor necrosis factor (TNF) family. It is required for B cell development. Deregulation of BLyS was involved in the pathogenesis of B cell-related autoimmune diseases and multiple myeloma. To prepare monoclonal antibodies (MAbs) against BLyS, cDNA encoding soluble BLyS (sBLyS) was first amplified by reverse transcriptase-polymerase chain reaction (RT-PCR) with specific primers, and then inserted into a prokaryotic expression vector pET-30a. Right recombinant plasmid was expressed in Escherichia coli strain BL21(DE3), purified by nickel affinity chromatography. Isolated sBLys was used as an antigen to immunize mice. Splenocytes of one immunized mouse were fused with NS-1. Hybridomas secreting antibodies against sBLyS were identified by ELISA. One positive clone was selected to produce antibody by injecting the hybridoma into the peritoneal cavity of mice. After collecting ascites, the antibody was purified by protein A affinity chromatography. Western blot and immunoflourescence demonstrated that the antibody could bind recombinant sBLyS and genuine membrane-bound BLyS (mBLyS) on U937. This MAb can be used as a detecting reagent to analyze the serum level of BLyS in patients with autoimmune diseases and the expression profile of mBLyS on multiple myeloma cells.

Abstract:Objective B7-H4, a member of the B7 family of immunoregulatory receptors, may participate in the negative regulation of cell-mediated immunity. Aberrant B7-H4 expression is detected in some tumors and it plays a role in the occurrence and development of tumors. The aim of this study was to elucidate the functional and structural properties of B7-H4. Methods We developed a monoclonal antibody (mAb) against the extracellular domain of B7-H4 through immunization of Balb/c mice with 3T3-mB7-H4 cells which expressed extrinsic B7-H4. A stable hybridoma cell line was established. Then, we analysised the characterization of the mAb through Enzyme linked immunosorbent assay (ELISA), Immunoprecipitation (IP), western blotting, Immunohistochemical (IHC), and tested the biological activity of the mAb. Results ELISA, IP, and western blotting analyses indicated that the mAb specifically recognized B7-H4. In addition, flow cytometry demonstrated that the mAb exhibits excellent reactivity when applied to leukemic cells. IHC staining revealed that the mAb stained in a predominantly diffuse plasmalemmal or cytoplasmic pattern when applied to certain tumor tissues. The preliminary results of the mAb's biological activity showed that the mAb could effectively inhibit the function of B7-H4 in the inhibition of T cell, while promotingg the growth of T cells and the secretion of Interleukin-2 (lL-2), Interleukin-4 (IL-4), Interleukin10 (IL-10) and Interferon-γ (IFN-γ). Conclusion This mAb will be a valuable tool for the further investigation of B7-H4 function.

Abstract:Antibody-targeting therapy has drawn great interests to the hematologists and oncologists. 3A4, a novel antibody recognizing human CD45RA antigen, is a new target molecule for leukemias and holds a therapeutic potential for myeloid lineage leukemias. However, murine antibodies cannot be safely used in patients because of their strong immune reaction, humanization of the antibodies interested will be an important development step for therapeutic purpose. The aim of this study was to engineer the mouse 3A4 and to investigate the biological activity of its chimeric form. The humanized antibody composed of the 3A4 single-chain fragment of variable region and the human IgG1 Fc region, which was named human-mouse chimeric antibody 3A4 (Hm3A4). The function and biological activities of Hm3A4 were characterized using a variety of biological approaches. The results showed that Hm3A4 retained a strong binding activity to its antigen and could significantly block the binding of parental 3A4 to the antigen. In vitro experiments revealed that Hm3A4 could kill the target cells through complement-dependent cytotoxicity and antibody-dependent cell-mediated cytotoxicity function. In vivo, Hm3A4 showed efficient antileukemia activity outperforming the nontreated mice. In conclusion, the chimeric antibody has an excellent biological activity after humanization and holds targeting therapeutic potential for myeloid leukemia, which warrants further development of this agent.
S3Ab, a Novel Antibody Targeting B Lymphocytes, is a Potential Therapeutic Agent for B-lineage Malignancies

Abstract:CD79 alpha protein together with the related CD79 beta protein forms the B-cell antigen receptor (BCR). It remains present when B cells transform into active plasma cells, and is also present in virtually all B cell neoplasms. Monoclonal antibody (mAb) S3 (S3Ab) is a novel anti-CD79 alpha antibody generated by using Raji cells as an immunogen. Herein, we conducted a study on S3Ab using various cellular and immunocytological techniques. The results showed that S3Ab could recognise CD79 alpha in living cells. The molecular weights of the heavy and the light chains of S3Ab were 55 and 26 kDa, respectively. S3 antigen is only expressed on more mature B cells and negative on blast B cells. It could partially block the binding of anti-CD79 alpha (Hm47, recognising the cytoplasmic domain of CD79 alpha) to target cells. Immunoprecipitation experiment showed that S3 antigen is about 33 kDa and S3 can specifically bind to the recombinant extracellular segment of CD79 alpha. The internalisation rate of S3Ab to the target cells was as high as 74.0% after incubation at for 3 h. In conclusion, S3Ab is probably a new target molecule for B cells and can be an excellent antibody in targeting treatment of haematopoietic malignancies, warranting further development of this agent.

Abstract:OBJECTIVE:To analyze the reactive pattern and its clinical significance of ZCH-7-2D3 monoclonal antibody on leukemia cells.METHODS:Bone marrow samples from 100 leukemia patients (male 59: female 41, 34 cases of children and 66 adults, aged 11 months - 77 year) were collected. A CD45 gating strategy and multi-parameter flow cytometry were used to analyze the leukemia cells.RESULT:The positive rate (10/31) of 2D3 antibody on acute lymphocytic leukemia (ALL) patients was significantly lower than that (39/55) of acute myelogeneous leukemia (AML) (P <0.01). 2D3 was positive for all three cases of B/myeloid mixed lineage leukemia, but negative in 6 cases of chronic myelogeneous leukemia (CML), significantly lower than that in AML (39/55, P<0.01) patients. There was no difference between the positive rates of chronic lymphocytic leukemia (CLL, 3/5) and B lineage ALL (9/28, P = 0.2389). The antibody was not found to recognize the differentiation stages of either ALL and AML subtypes.CONCLUSION:2D3 monoclonal antibody primarily recognizes AML cases. Further studies on cloning of gene encoding the antigen protein and its biological functions are warranted.

Abstract:Zhejiang Children's Hospital (ZCH)-2B8a antibody was recently generated in our laboratory using leukemia cell line KG1a as immunogen. This antibody was submitted to the 8th International Workshop and Conference on Human Leukocyte Differentiation Antigens (HLDA8) and the results showed that the antibody recognized an unknown molecule on the surface of some blood cells. In this study, the reactivity of the antibody on normal and malignant hematopoietic cells was investigated and its diagnostic and therapeutic significances were explored. Materials and Methods 3 specimens each of normal peripheral blood, bone marrow/G-CSF mobilized peripheral blood CD34+ stem/progenitor cells (PBSCs), 13 hematopoietic cell lines and the cells from 100 leukemia patients (male 58: female 42, 33 cases of children and 67 adults, aged 11 month ~ 77 yrs) were enrolled into this study. A multi-parameter flow cytometry with CD45 gating strategy was used to analyze the cells. Results 2B8a weakly reacted with a few B lymphocytes (34.3±15.91%), monocytes(44.4±37.77%), granulocytes (26.4±22.40%), but not reacted with other blood cells such as T cells (0.6±0.37%), NK cells (2.6±1.56%), DC cells (8.4±8.18%), erythrocytes (0.4±0.21%) and platelets (0.3±0.28%). It was reactive with 38.0±9.09% of bone marrow CD34+ cells with 21.0±22.00% of CD38+ and 25.3±22.6% of CD38- subsets (P = 0.5). In G-CSF mobilized PBSCs, 2B8a recognized 11.7±18.73% of CD34+ PBSCs with 9.7±15.20% of CD38+ and 33.3±57.74% of CD38- subsets. The positive rates of 2B8a on a panel of hematopoietic cell lines were shown in Table 1. The fresh leukemia cell analysis showed that the antibody primarily reacted with lymphoid leukemias, of which acute lymphocytic leukemia (ALL) and chronic lymphocytic leukemia (CLL) accounted for 29%(9/31) and 40%(2/5), respectively. It did not react with most cases of myeloid leukemias, with only 5%(3/57) of acute myeloid leukemia(AML) and 0% (0/6) of chronic myelogeneous leukemia (CML) cases, in which significant difference between myeloid and lymphoid leukemias (χ2 = 10.29, P < 0.01) was found; In ALL, 2B8a only reacted with B lineage (32%, 9/28), while not with T and NK lineage leukemias. In B lineage leukemia, early Pre-B ALL was the most reactive type of disease, positive in 60% of cases, significantly higher than cALL cases (5/21, 24%, P = 0.0173) with declining expression pattern with cell maturation. There was no reactivity in mature B-ALL but it was strongly reactive with lymphoma cells suggesting the possibility for this antibody as a potential targeting therapy for patients with B lineage lymphoma. Conclusions 2B8a may be used in distinguishing lymphoid from myeloid leukemia. It might hold a potential application for targeting therapy for patients with leukemia and malignant lymphoma. Abstract:OBJECTIVE:Acute monocytic leukemia (AML)-M5 is the common type of acute myeloid leukemias in children. Studies have shown that there are abundant lipopolysaccharide (LPS) receptor (designated as CD14) molecules on the cell membrane of M5 cells and they play an important role in the diagnosis of M5, since they can be recognized and bound by mouse-anti-human CD14 monoclonal antibody (McAb). However, mouse-originated antibodies are largely not suitable for clinical application due to the severe side effects, thus "humanized antibody" is desired. As the first step for developing humanized antibody, the construction and expression of single chain antibody (scFv) with functional protein are necessary. The present study aimed to express ZCH-7-2F9 ScFv (scFv(2F9)) in eukaryotic cells and obtain the scFv(2F9) protein with a high biological activity.METHODS:Four primers were synthesized to construct the eukaryotic expressional vector, which included SfiI and EcoRI enzyme cleaving site, 6 x His and stop code TGA sequences. scFv(2F9) gene was amplified through splicing by overlap extension (SOE) using the high fidelity Taq polymerase. Positive recombinants (pSectag2A/scFv(2F9)) were identified through enzyme cleaving and sequenced before expression and were transformed into Chinese hamster ovary (CHO) cells for expression. Western-Blot and flow cytometry (FCM) were carried out to determine the relative molecular mass (Mr) and binding activity of scFv(2F9).RESULTS:The cloned scFv(2F9) gene was identified to be functional by sequencing and expressing. The interested protein could be detected in the culture supernatant of transformed CHO cells with an Mr of 31 000. The blocking test showed that the positive cell percentages, the mean fluorescence intensity (MFI) and the peak of channel (peak Ch) were reduced by 90.02%, 63.30% and 63.38%, respectively after blocking with scFv(2F9), while those were 4.55%, 10.09% and 5.02% after blockage using the supernatant from the CHO cells transfected with blanked vector pSectag2A.CONCLUSIONS:The scFv(2F9) against human CD14 antigen was successfully expressed in eukaryotic cells and showed a high biological activity, which may be useful for the further studies on its humanized antibodies.
Recombinant expression of the beta-subunit of HLA-DR10 for the selection of novel lymphoma targeting molecules.

Abstract:Selective high-affinity ligands (SHALs) were selected as substitutes for monoclonal antibodies (mAbs) to deliver radioisotopes to malignant tumors. Because a SHAL (5 KD) is considerably smaller in comparison to an antibody (150 KD), a significant therapeutic index (TI) enhancement for radioimmunotherapy (RIT) is anticipated. The antibody-antigen (Ab-Ag) model system chosen for the development of SHALs consists of Lym-1, a MAb with proven selectivity in non-Hodgkin's lymphoma (NHL) patients and its well-characterized Ag, the beta subunit of HLA DR10. Whereas Lym-1 is readily available, the subunit of HLA-DR10 is not. Native, heterodimeric (alpha and beta subunits) HLA-DR10 can be purified from Raji cells, which are known to overexpress this Ag. Inconsistent homogeneity between preparations of HLA-DR10 solubilized in the presence of detergents prompted us to express a recombinant form of the beta subunit of HLA-DR10 in Escherichia coli. Negligible production yields (<or=50 microg/L) were achieved by the expression of the full-length protein in a soluble form. By contrast, yields of 240 mg/L were obtained by expressing only the extracellular domain (ED) of the beta subunit of HLA-DR10 in an insoluble form (inclusion bodies). The recovery yield of refolded protein was 75%. Circular dichroism (CD) and Lym-1 binding studies indicated that the recombinant ED of the beta subunit of HLA-DR10 was properly folded. Therefore, this recombinant protein can be used as a surrogate for native heterodimeric HLA DR10 for the in vitro selection of SHALs and related targeting molecules.
3A4, a New Potential Target for B and Myeloid Lineage Leukemias

Abstract:Antibody-targeting therapy has drawn great interests to the hematologists and oncologists. Many antibodies have been studied for their potential targeting for hematopoietic malignancies. A few have been proved to be very effective for patients with these diseases. However, more antibodies are needed for clinical use. CD45 and its isoforms may convey clinical potential in terms of targeting therapy. Zhejiang Children's Hospital (ZCH)-6-3A4 (3A4), a novel antibody that can recognize an isoform of CD45 has been found to react with restricted cell components in hematopoietic system, which may have the potential for targeting therapy. Herein, we conducted an in vitro study of our newly prepared antibody 3A4 using various cellular and immunocytological methods. The results showed that the antibody 3A4 (murine IgG1 kappa) was a new clone of anti-CD45RA. It could block the binding to an epitope of CD45RA recognized by a standard anti-CD45RA antibody (Clone name L48). The reactivity of the 3A4 to both fresh leukemia cells from patients and well-defined leukemia cell lines was largely similar to those of L48, but the former recognized more leukemia cells than the latter. Cytometric analysis after papain treatment showed that the internalization rate of the 3A4 antibody to the target cells was as high as 71.3% after incubation at 37 degrees C for 4 h, which was significantly higher than that of L48 (20.4%). The norcantharidin (NCTD)-conjugated immunotoxin (NCTD-3A4) was generated using an active ester method. The targeting inhibition rate on KG1a was as high as 61.10% after 96 h incubation in a dose-dependent manner, which was significantly higher than that (3.56%, P < 0.01) with 3A4-negative Nalm-6 cells. In conclusion, our new anti-CD45RA antibody 3A4 is probably a new target molecule of leukemia cells and holds a targeting therapeutic potential for hematopoietic malignancies, which warrants further development of this agent.

Abstract:Leukemia stem cells (LSCs) play a critical role in the initiation, recurrence, and resistance to treatment of leukemia. Eradicating LSCs is crucial for the complete elimination of the disease. CD45RA is identified as an important marker for LSC subsets in acute myeloid leukemia (AML), providing a strategic target for therapy. In this report, we introduce Hu3A4, an innovative humanized CD45RA antibody devised to target LSCs expressing this antigen. Hu3A4 retains the antigen-recognition ability of its parental antibody while removing sequences from the variable region that could elicit human anti-mouse immune reactions. The modified variable regions of the heavy and light chains were intricately fused with the constant regions of human IgG1 heavy and light chains, respectively, producing a humanized antibody that emulates the structure of natural IgG. Hu3A4 was produced through recombinant expression in Chinese Hamster Ovary (CHO) cells, which ensured stable gene integration. In vitro tests revealed that Hu3A4 could effectively target and lyse the cells. Further, in vivo studies highlighted Hu3A4's substantial anti-leukemic activity, significantly prolonging survival times in treated animal models compared to controls (P < 0.01). To summarize, Hu3A4 exhibits remarkable bioactivity and offers a promising therapeutic potential for the treatment of leukemia patients. Progressing Hu3A4 through additional preclinical and clinical studies is crucial to validate its efficacy as a therapeutic agent for leukemia.
Analysis of an anti-B lymphocyte stimulator monoclonal antibody B7 and its binding activity to myeloma and lymphoma cell lines.

Abstract:Although there is convincing evidence of a link between B lymphocyte stimulator ( BLyS) and the proliferation and survival of malignant B cells, previous observations about BLyS expression on B lymphoma cells were contradictory. In this study, BLyS expression on human lymphoma and myeloma cell lines was evaluated by Fluorescence-Activated Cell Sorter (FACS). First, specificity of a monoclonal antibody (MAb) against BLyS, was analyzed. The results showed that MAb B7 was immunoglobulin G3 (IgG3) and recognized recombinant human BLyS specifically. In addition, MAb B7 bound to the histiocyte lymphoma cell line U937 in dosage-dependent manner, but not the T lymphoma cell line Jurkat, suggesting that the cellular binding of MAb B7 was specific. Using this MAb, BLyS expression on two multiple myeloma cell lines (XG-7 and SKO-007) and two Burkitt's lymphoma cell lines (Daudi and Raji) was evaluated. MAb B7 did bind to XG7 and SKO-007 (66.84% and 79.38% positive cells, respectively). But MAb B7 did not bind Burkitt's lymphoma cell lines, Daudi and Raji (4.09% and 3.02% positive cells, respectively). It will be interesting to further analyze the expression of BLyS on B malignant cells of multiple myeloma and lymphoma patients, and to evaluate the correlation of its expression and patient prognosis.
Antigenic expression of B-cell chronic lymphocytic leukemic cell lines

Abstract:A flow cytometric analysis of five B cell chronic lymphocytic leukemic (B-CLL) cell lines was undertaken using 129 unknown reagents from the blind panel (BP) and 72 reagents from the known CD panel obtained from the Fourth International Leucocyte Differentiation Conference and Workshop, B cell section (Vienna, 1989). The five cell lines examined were: SeD (PNAS 75, 5706, 1978), B-CLL-LCL (BLOOD 71, 9, 1988), JVM-HH and JVM-2(INT J CAN 38, 531, 1986), and WR#1 (TH and BD). The reagents were #1-129 (blinded panel) and reagents 1-44 and 53-84 (CD panel with CD23 reagents missing). Positivity was defined as greater than 30% of the cells having a three fold increase or more in mean channel fluorescence. Fourty-three reagents of the blinded panel were negative by these criteria while all remaining reagents were positive on all five lines. SeD showed the lowest reactivity; B-CLL-LCL and JVM-2 showed the most reactivity; JVM-HH and WR#1 were intermediate. The known CD panel confirmed the reactivity of the blinded panel. An average immunophenotype was constructed and compared to published normal EBV lymphoblastoid cell lines and several differences were noted. There was an absence or significant decrease in the expression of CD19, CD21, CD22 and CD37 while there was an increased expression of CD38, CD54, CD74 and CD76. The heterogeneity observed between the B-CLL lines may in part be due to polymorphisms but is more likely to represent the underlying heterogeneity seen in common and familial B-CLL.2+öff
Monoclonal antibody BB9 raised against bone marrow stromal cells identifies a cell-surface glycoprotein expressed by primitive human hemopoietic progenitors

Abstract:Objective: The identification of cell-surface antigens whose expression is limited to primitive hematopoietic progenitor cells (HPC) is of major value in the identification, isolation, and characterization of candidate stem cells in human hemopoietic tissues. Based on the observation that bone marrow stromal cells and primitive HPC share several cell-surface antigens, we sought to generate monoclonal antibodies to HPC by immunization with cultured human stromal cells. Methods: BALB/c mouse were immunized with human bone marrow (BM)-derived stromal cells. Splenocytes isolated from immunized mice were fused with the NS-1 murine myeloma cell line and resulting hybridomas selected in HAT medium, then screened for reactivity against stromal cells, peripheral blood (PB), and BM cells. Results: A monoclonal antibody (MAb), BB9, was identified based on its binding to stromal cells, a minor subpopulation of mononuclear cells in adult human BM, and corresponding lack of reactivity with leukocytes in PB. BB9 bound to a minor subpopulation of BM CD34(+) cells characterized by high-level CD34 antigen and Thy-1 expression, low-absent expression of CD38, low retention of Rhodamine 123, and quiescent cycle status as evidenced by lack of labeling with Ki67. CD34(+)BB9(+) cells, in contrast to CD34(+)BB9(-) cells, demonstrated a capacity to sustain hematopoiesis in pre-CFU culture stimulated by the combination of IL-3, IL-6, G-CSF, and SCF. BB9 also demonstrated binding to CD34(+) cells from mobilized PB. Conclusion: Collectively, these data therefore demonstrate that MAb BB9 identifies an antigen, which is selectively expressed by hierarchically primitive human HPC and also by stromal cells.

Abstract:Background/Objectives: T-cell lymphomas are often histologically indistinguishable from benign T-cell infiltrates, and diagnosis typically relies on slow, complex, and expensive multiplexed PCR reactions, requiring significant training and experience to interpret them. We aimed to raise highly specific antibodies against the two alternatively used and very similar T-cell receptor beta constant regions, TCRbeta1 and TCRbeta2, encoded by the TRBC1 and TRBC2 gene segments, respectively. We sought to demonstrate the feasibility of detecting TCRbeta1 and TCRbeta2 immunohistochemically in routine clinical (formalin-fixed, paraffin-embedded (FFPE)) tissue sections as a novel diagnostic strategy for T-cell lymphomas. Methods: Recombinant rabbit antibodies were validated using Western blotting and FFPE immunostaining of T-cell leukemia lines. The immunostaining of FFPE tissue containing benign and lymphomatous T-cell populations was undertaken, with corroboration by BaseScopeTM high-sensitivity in situ hybridization and quantitative real-time PCR (Q-PCR). An additional Q-PCR literature review and analysis of publicly available RNAseq data was used to determine the TCRbeta2/TCRbeta1 ratio cut-off to separate benign and malignant T-cell populations. Results: Our TCRbeta1/TCRbeta2 antibody pair gave highly specific FFPE tissue staining. All benign samples analyzed (immunohistochemically, by BaseScopeTM, by Q-PCR, and by RNAseq data analysis) had TCRbeta1/TCRbeta2 or TRBC1/TRBC2 ranges well within the previously published flow cytometric benign range (TCRbeta2/TCRbeta1 = 0.18:1–5.7:1), while samples of T-cell lymphoma did not. One out of thirteen (7.7%) lymphoma samples showed some detectable TCRbeta1/TCRbeta2 protein co-expression, and 4 out of 13 (30.8%) T-cell lymphomas showed a TRBC1/TRBC2 transcript co-expression using BaseScopeTM. Conclusions: Analyzing T-cell monotypia immunohistochemically, analogous to B-cell monotypia (kappa: lambda ratio for B-cell and plasma cell neoplasms), could make the diagnosis of T-cell lymphomas cheaper, quicker, and more accurate. Larger studies are needed to validate our antibodies for clinical use.

Abstract:The CD19-targeting bispecific T-cell engager blinatumomab has shown remarkable efficacy in patients with relapsed/refractory B-cell precursor acute lymphoblastic leukemia. However, several studies showed that blinatumomab has a short plasma half-life due to its low molecular weight, and thus its clinical use is limited. Furthermore, multiple trials have shown that approximately 30% of blinatumomab-relapsed cases are characterized by CD19 negative leukemic cells. Here, we design and characterize two novel antibodies, A-319 and A-2019. Blinatumomab and A-319 are CD3/CD19 bispecific antibodies with different molecular sizes and structures, and A-2019 is a novel CD3/CD19/CD20 trispecific antibody with an additional anti-CD20 function. Our in vitro, ex vivo, and in vivo experiments demonstrated that A-319 and A-2019 are potent antitumor agents and capable of recruiting CD3 positive T cells, enhancing T-cell function, mediating B-cell depletion, and eventually inhibiting tumor growth in Raji xenograft models. The two molecules are complementary in terms of efficacy and specificity profile. The activity of A-319 demonstrated superior to that of A-2019, whereas A-2019 has an additional capability to target CD20 in cells missing CD19, suggesting its potential function against CD19 weak or negative CD20 positive leukemic cells.
