Abstract:Although the default mode network (DMN) is known to be abnormal in schizophrenia (SZ) patients with auditory verbal hallucinations (AVHs), it is still unclear whether AVHs that occur in SZ are associated with certain information flow in the DMN. This study collected resting-state functional magnetic resonance imaging data from 28 first-episode, drug-naïve SZ patients with AVHs, 20 SZ patients without AVHs, and 38 healthy controls. We used Granger causality analysis (GCA) to examine effective connectivity (EC) of two hub regions [posterior cingulate cortex (PCC) and anteromedial prefrontal cortex (aMPFC)] within the DMN. We used two-sample t-tests to compare the difference in EC between the two patient groups, and used Spearman correlation analysis to characterize the relationship between imaging findings and clinical assessments. The GCA revealed that, compared with the non-AVHs group, EC decreased from aMPFC to left inferior temporal gyrus (ITG) and from PCC to left cerebellum posterior lobe, ITG, and right middle frontal gyrus in SZ patients with AVHs. We also found significant correlations between clinical assessments and mean strengths of connectivity from aMPFC to left ITG and from PCC to left ITG. Moreover, receiver operating characteristic analysis revealed that the above-mentioned effective connectivities had a diagnostic value for distinguishing SZ patients with AVHs from non-AVHs patients. These findings suggest that AVHs in SZ patients may be associated with the aberrant information flows of the DMN, and the left ITG may probably serve as a potential biomarker for the neural mechanisms underlying AVHs in SZ patients.
Aberrant Development of Cross-Frequency Multiplex Functional Connectome in First-Episode, Drug-Naive Major Depressive Disorder and Schizophrenia.

Abstract:Impact statementRecent methodological advances in network neuroscience allow integrating connectivity information from different scales or aspects through multilayer network models. Utilizing such models, we found common and specific neurodevelopmental abnormalities in multiband functional brain networks between major depressive disorder (MDD) and schizophrenia (SCH), and the abnormalities cannot be uncovered by individually analyzing brain networks within each frequency interval. These findings have important implications for understanding shared and unique neural mechanisms underlying MDD and SCH and highlight the necessity of integrating connectivity information from different frequency intervals to search for new biomarkers of the diseases. Introduction: Both major depressive disorder (MDD) and schizophrenia (SCH) are characterized by neurodevelopmental abnormalities; however, transdiagnostic and diagnosis-specific patterns of such abnormalities have rarely been examined, particularly in large-scale functional brain networks via advanced multilayer network models.Methods: Here, we collected resting-state functional magnetic resonance imaging data from 45 MDD patients, 64 SCH patients, and 48 healthy controls (HCs; 13-45 years old), and we constructed functional networks in different frequency intervals. The frequency-dependent networks were then fused by multiplex network models, followed by graph-based topological analyses.Results: We found that functional networks of the patients showed common neurodevelopmental abnormalities in the right ventromedial parietooccipital sulcus (opposite correlations with age to HCs), whereas functional networks of the MDD patients exhibited specific alterations in the left superior parietal lobule and right precentral gyrus with respect to cross-frequency interactions. These findings were quite different from those from brain networks within each frequency interval, which revealed SCH-specific neurodevelopmental abnormalities in the right superior temporal gyrus (opposite correlations with age to the other two groups) in 0.027-0.073 Hz, and SCH-specific alterations in the left superior temporal gyrus and bilateral insula in 0.073-0.198 Hz. Finally, multivariate analysis of age prediction revealed that the subcortical network lost prediction ability in both patient groups, whereas the visual network exhibited additional prediction ability in the MDD patients.Discussion and Conclusion: Altogether, these findings demonstrate transdiagnostic and diagnosis-specific neurodevelopmental abnormalities and alterations in large-scale functional brain networks between MDD and SCH, which have important implications for understanding shared and unique neural mechanisms underlying the diseases.
Decreased Spontaneous Low-frequency BOLD Signal Fluctuation in First-episode Treatment-naive Schizophrenia

Abstract:The default mode of brain function hypothesis has been readdressed from the perspective of the presence of low-frequency blood oxygenation level-dependent (BOLD) functional magnetic resonance imaging (fMRI) signal changes (0.01-0.1Hz) in the resting healthy brain. In this study, a newly reported cosine- statistical parameter mapping (SPM) approach is presented to measure the temporal low-frequency blood oxygenation level-dependent (BOLD) signal for exploring the brain activity of schizophrenia in a resting state with subjects' eyes closed. Firstly, the nine cosine functions with frequency between 0.01and 0.1Hz are specified as the design matrix of the general linear model, then fMRI data is analyzed by SPM method. Nine first-episode treatment-naïve schizophrenia patients and nine healthy subjects participated in this study. The results showed diminished low-frequency spontaneously activity in schizophrenia.

Abstract:Schizophrenia has been associated with abnormal intrinsic brain activity, involving various cognitive impair-ments. Qualitatively similar abnormalities are seen in individuals at ultra-high risk (UHR) for psychosis. In this study, resting-state fMRI (rs-fMRI) data were collected from 44 drug-naive first-episode schizophrenia (Dn-FES) patients, 48 UHR individuals, and 40 healthy controls (HCs). The fractional amplitude of low-frequency fluc-tuations (fALFF), regional homogeneity (ReHo), and functional connectivity (FC), were performed to evaluate resting brain function. A support vector machine (SVM) was applied for classification analysis. Compared to HCs, both clinical groups showed increased fALFF in the central executive network (CEN), decreased ReHo in the ventral visual pathway (VVP) and decreased FC in temporal-sensorimotor regions. Excellent performance was achieved by using fALFF value in distinguishing both FES (sensitivity=83.21%, specificity=80.58%, accu-racy=81.37%, p=0.009) and UHR (sensitivity=75.88%, specificity=85.72%, accuracy=80.72%, p<0.001) from HC group. Moreover, the study highlighted the importance of frontal and temporal alteration in the pathogenesis of schizophrenia. However, no fMRI features were observed that could well distinguish Dn-FES from UHR group. To conclude, fALFF in the CEN may provide potential power for identifying individuals at the early stage of schizophrenia and the alteration in the frontal and temporal lobe may be important to these individuals.

Abstract:Background Schizophrenia (SCZ) is a severe and chronic psychiatric disorder with premature age-related physiological changes. However, numerous previous studies examined the epigenetic age acceleration in SCZ patients and yielded inconclusive results. In this study, we propose to explore the epigenetic age acceleration in drug-naive first-episode SCZ (FSCZ) patients and investigate whether epigenetic age acceleration is associated with antipsychotic treatment, psychotic symptoms, cognition, and subcortical volumes. Methods We assessed the epigenetic age in 38 drug-naive FSCZ patients and 38 healthy controls by using three independent clocks, including Horvath, Hannum and Levine algorithms. The epigenetic age measurements in SCZ patients were repeated after receiving 8 weeks risperidone monotherapy. Results Our findings showed significantly positive correlations between epigenetic ages assessed by three clocks and chronological age in both FSCZ patients and healthy controls. Compared with healthy controls, drug-naive FSCZ patients have a significant epigenetic age deceleration in Horvath clock ( p = 0.01), but not in Hannum clock ( p = 0.07) and Levine clock ( p = 0.43). The epigenetic ages of Hannum clock ( p = 0.002) and Levine clock ( p = 0.01) were significantly accelerated in SCZ patients after 8-week risperidone treatment. However, no significant associations between epigenetic age acceleration and psychotic symptoms, cognitive function, as well as subcortical volumes were observed in FSCZ patients. Conclusion These results demonstrate that distinct epigenetic clocks are sensitive to different aspects of aging process. Further investigations with comprehensive epigenetic clock analyses and large samples are required to confirm our findings.
Altered Coupling of Spontaneous Brain Activities and Brain Temperature in Patients with Adolescent-Onset, First-Episode, Drug-Naïve Schizophrenia

Abstract:PURPOSE:A recent study has reported that schizophrenia patients show an uncoupled association between intraventricular brain temperature (BT) and cerebral blood flow (CBF). CBF has been found to be closely coupled with spontaneous brain activities (SBAs) derived from resting-state BOLD fMRI metrics. Yet, it is unclear so far whether the relationship between the intraventricular BT and the SBAs may change in patients with adolescent-onset schizophrenia (AOS) compared with that in healthy controls (HCs).METHODS:The present study recruited 28 first-episode, drug-naïve AOS patients and 22 matched HCs. We measured the temperature of the lateral ventricles (LV) using diffusion-weighted imaging thermometry and measured SBAs using both regional homogeneity and amplitude of low-frequency fluctuation methods. A nonparametric Wilcoxon rank sum test was used to detect the difference in intraventricular BT between AOS patients and HCs with LV volume, age, and sex as covariates. We also evaluated the relationship between the intraventricular BT and the SBAs using partial correlation analysis controlling for LV volume, age, and sex.RESULTS:We found that HCs showed a significant negative correlation between the intraventricular BT and the local SBAs in the bilateral putamina and left superior temporal gyrus, while such a correlation was absent in AOS patients. Additionally, no significant difference between the two groups was found in the intraventricular BT.CONCLUSION:These findings suggest that AOS patients may experience an uncoupling between intraventricular BT and SBAs in several schizophrenia-related brain areas, which may be associated with the altered relationships among intraventricular BT, CBF, and metabolism.

Abstract:BackgroundIt remains unclear whether the topological deficits of the white matter network documented in cross-sectional studies of chronic schizophrenia patients are due to chronic illness or to other factors such as antipsychotic treatment effects. To answer this question, we evaluated the white matter network in medication-naive first-episode schizophrenia patients (FESP) before and after a course of treatment.MethodWe performed a longitudinal diffusion tensor imaging study in 42 drug-naive FESP at baseline and then after 8 weeks of risperidone monotherapy, and compared them with 38 healthy volunteers. Graph theory was utilized to calculate the topological characteristics of brain anatomical network. Patients’ clinical state was evaluated using the Positive and Negative Syndrome Scale (PANSS) before and after treatment.ResultsPretreatment, patients had relatively intact overall topological organizations, and deficient nodal topological properties primarily in prefrontal gyrus and limbic system components such as the bilateral anterior and posterior cingulate. Treatment with risperidone normalized topological parameters in the limbic system, and the enhancement positively correlated with the reduction in PANSS-positive symptoms. Prefrontal topological impairments persisted following treatment and negative symptoms did not improve.ConclusionsDuring the early phase of antipsychotic medication treatment there are region-specific alterations in white matter topological measures. Limbic white matter topological dysfunction improves with positive symptom reduction. Prefrontal deficits and negative symptoms are unresponsive to medication intervention, and prefrontal deficits are potential trait biomarkers and targets for negative symptom treatment development.
Alteration of the Gut Microbiome in First-Episode Drug-Naïve and Chronic Medicated Schizophrenia Correlate with Regional Brain Volumes.

Abstract:The human gut microbiome plays an important role in the basic neurodevelopmental processes of the central nervous system and has been implicated in several neuropsychiatric disorders. However, the connection between the gut microbiome and the underlying pathogenesis of schizophrenia (SCZ) is poorly defined. Here we analyzed the faecal samples from 40 first-episode drug-naïve SCZ (FSCZ) patients, 85 chronically antipsychotic-treated SCZ (TSCZ) patients and 69 healthy controls (HCs) using 16S rRNA gene sequence to determine whether the alterations of the gut microbiome were associated with SCZ or antipsychotic treatment. In addition, we acquired the T1-weighted brain imaging data by using structural magnetic resonance imaging to test whether microbial composition correlated with structural brain signatures. Our analyses revealed low microbiome alpha-diversity indexes in TSCZ patients but not in FSCZ patients as compared to HCs. Importantly, both FSCZ and TSCZ patients had distinct changes in gut microbial composition at certain taxa including Christensenellaceae, Enterobacteriaceae, Pasteurellaceae, Turicibacteraceae at the family level and Escherichia at genus level as compared to HCs. We also found significant disturbances of gut microbial composition in TSCZ versus FSCZ patients (eg. Enterococcaceae and Lactobacillaceae). Most interestingly, our exploratory analyses found specific SCZ-associated microbiota to be correlated with the right middle frontal gyrus (rMFG) volume which was aberrant in SCZ patients. Our findings extend prior work and suggest a possible link between the gut microbiome and brain structure which may be implicated in the pathology of SCZ.

Abstract:CONTEXT:Most of what we know about antipsychotic drug effects is at the receptor level, distal from the neural system effects that mediate their clinical efficacy. Studying cerebral function in antipsychotic-naive patients with schizophrenia before and after pharmacotherapy can enhance understanding of the therapeutic mechanisms of these clinically effective treatments.OBJECTIVE:To examine alterations of regional and neural network function in antipsychotic-naive patients with first-episode schizophrenia before and after treatment with second-generation antipsychotic medication.DESIGN:Case-control study.SETTING:Huaxi MR Research Center and Mental Health Centre of the West China Hospital.PARTICIPANTS:Thirty-four antipsychotic-naive patients with first-episode schizophrenia were scanned using gradient-echo echo-planar imaging while in a resting state. After 6 weeks of antipsychotic treatment, patients were rescanned. Thirty-four matched healthy control subjects were studied at baseline for comparison purposes.MAIN OUTCOME MEASURES:The amplitude of low-frequency fluctuations (ALFF) of blood oxygen level-dependent signals, believed to reflect spontaneous neural activity, was used to characterize regional cerebral function. Functional connectivity across brain regions was evaluated using a seed voxel correlation approach and an independent component analysis. Changes in these measures after treatment were examined to characterize effects of antipsychotic drugs on regional function and functional integration.RESULTS:After short-term treatment with second-generation antipsychotic medications, patients showed increased ALFF, particularly in the bilateral prefrontal and parietal cortex, left superior temporal cortex, and right caudate nucleus. Increased regional ALFF was associated with a reduction of clinical symptoms, and a widespread attenuation in functional connectivity was observed that was correlated with increased regional ALFF.CONCLUSIONS:We demonstrate for the first time, to our knowledge, that widespread increased regional synchronous neural activity occurs after antipsychotic therapy, accompanied by decreased integration of function across widely distributed neural networks. These findings contribute to the understanding of the complex systems-level effects of antipsychotic drugs.
A Study of Fractional Amplitude of Low Frequency Fluctuation in Schizophrenia Patients with Persistent Verbal Auditory Hallucinations

Abstract:Backgroundï¼ Auditory verbal hallucinations (AVHs), one of the hallmark positive symptoms of schizophrenia (SCZ), affect 60-80% of SCZ patients. However, the neuroimaging mechanisms underlying AVHs, particularly persistent AVHs, are currently unknown. The aim of this study was to explore the neuroimaging mechanisms of persistent AVHs by comparing the characteristics of changes in fractional amplitude of low frequency fluctuations (fALFF) values of subjects. Methodsï¼ 64 SCZ patients with persistent AVHs, 39 patients with non-AVHs, and 60 healthy controls (HC) underwent a resting-state functional magnetic resonance imaging (MRI) study. The fALFF was used to assess localized spontaneous neural activity in the brain of three groups of subjects. The Positive and Negative Symptoms scale (PANSS) was used to rate the severity of patients' psychiatric symptoms; the P3 item on the PANSS scale was used to assess the severity of persistent AVHs. Resultsï¼ SCZ patients with persistent AVHs had lower fALFF values in the right lingual gyrus, right postcentral, and left supplementary movement area (SMA) than in the HC, and higher fALFF values in the right medial superior frontal, left inferior parietal lobule, left precuneus, and right supramarginal than in the HC. SCZ patients with non-AVHs had lower fALFF values in the left lingual gyrus, right postcentral, and left supplementary motor area than in the HC group, while higher fALFF values were found in the right medial superior frontal only. However, there was no difference in fALFF values between patients with persistent AVHs and non-AVH patients. Conclusionsï¼ These results suggest that local spontaneous neural activity was significantly impaired in the brains of patients in the AVH and non-AVH groups compared to those in HC, and that altering spontaneous neural activity in these brain regions may be of value in the treatment of SCZ.

Abstract:It is unclear whether abnormal spontaneous neural activation patterns found in chronic schizophrenia patients (CSP) are part of the pathogenesis of disease, consequences of chronic illness, or effects of antipsychotic treatment. We performed a longitudinal resting-state functional magnetic resonance imaging (fMRI) study in 42 treatment-naïve first-episode schizophrenia patients (FESP) at baseline and then after 8-weeks of risperidone monotherapy and compared the findings to 38 healthy volunteers. Spontaneous brain activity was quantified using the fractional amplitude of low-frequency fluctuations (fALFF) and regional homogeneity (ReHo) and compared between patients and controls. Pretreatment, patients exhibited higher fALFF in left caudate compared with controls. After treatment, patients had elevated fALFF in bilateral putamen and right caudate and increased ReHo in right caudate and left putamen. Greater increase of fALFF in the left putamen correlated with less improvement in positive symptoms. Thus, abnormalities of spontaneous neural activity in chronic schizophrenia is at least partly due to a medication effect. The observed post-treatment increase in striatal intrinsic activity may reflect counter-therapeutic functional adaptation to dopamine D2 receptor occupancy required for medication effects on psychosis.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>With the development of imaging techniques, evidence of abnormal neural activity has been implicated in patients with somatization disorder (SD). It remains unclear whether abnormal spontaneous neural activities are related to specific frequency bands. In this study, resting-state functional magnetic resonance imaging (fMRI) using the frequency-specific amplitude of low frequency fluctuation (ALFF) approach was applied to investigate changes in spontaneous neural activity in different frequency bands in patients with SD.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Twenty-five first-episode, medication-naive patients with SD and 28 age-, sex-, education-matched healthy controls (HCs) underwent resting-state fMRI. The ALFF method with the classical low-frequency (0.01 - 0.08 Hz), slow-5 (0.01 - 0.027 Hz) and slow-4 (0.027 - 0.08 Hz) bands was employed to analyze the data.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>With the classical low-frequency and slow-5 bands, patients with SD showed significantly increased ALFF in the left orbitofrontal cortex (OFC) and reduced ALFF in the right cerebellum compared with HCs. With the slow-4 band, patients with SD exhibited significantly reduced ALFF in the right cerebellum compared with HCs. However, no significant correlation was observed between the ALFF value in the left OFC or right cerebellum and clinical/cognitive variables.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Our findings indicate that there are abnormal regional activities of the left OFC and right cerebellum in first-episode, treatment-naive patients with SD, suggesting that these alterations occur early in the course of the disease and are independent of medication status. Our study provides novel evidence that different regional activities of the frontal-cerebellar circuit may be involved in the pathophysiology of SD.</p>

Abnormal Resting-State Connectivity in a Substantia Nigra-Related Striato-Thalamo-Cortical Network in a Large Sample of First-Episode Drug-Naïve Patients With Schizophrenia

Abstract:Objective: The dopamine hypothesis is one of the most influential theories of the neurobiological background of schizophrenia (SCZ). However, direct evidence for abnormal dopamine-related subcortical-cortical circuitry disconnectivity is still lacking. The aim of this study was therefore to test dopamine-related substantia nigra (SN)-based striato-thalamo-cortical resting-state functional connectivity (FC) in SCZ. Method: Based on our a priori hypothesis, we analyzed a large sample resting-state functional magnetic resonance imaging (fMRI) dataset from first-episode drug-naïve SCZ patients (n = 112) and healthy controls (n = 82) using the SN as the seed region for an investigation of striato-thalamo-cortical FC. This was done in the standard band of slow frequency oscillations and then in its subfrequency bands (Slow4 and Slow5). Results: The analysis showed in SCZ: (1) reciprocal functional hypo-connectivity between SN and striatum, with differential patterns for Slow5 and Slow4; (2) functional hypo-connectivity between striatum and thalamus, as well as functional hyper-connectivity between thalamus and sensorimotor cortical areas, specifically in Slow4; (3) correlation of thalamo-sensorimotor functional hyper-connectivity with psychopathological symptoms. Conclusions: We demonstrate abnormal dopamine-related SN-based striato-thalamo-cortical FC in slow frequency oscillations in first-episode drug-naive SCZ. This suggests that altered dopaminergic function in the SN leads to abnormal neuronal synchronization (as indexed by FC) within subcortical-cortical circuitry, complementing the dopamine hypothesis in SCZ on the regional level of resting-state activity.

Abstract:Coherence in the phase of oscillatory neuronal activity indicates functional interaction among brain regions at rest. Infra-slow fluctuations in BOLD signal (band 5 - 0.01 to 0.027 Hz - and band 4 - 0.027 to 0.073 Hz) has been observed using resting state fMRI when employing frequency-domain analysis, and has been previously shown to be altered in schizophrenia. In the current study, we examined the strength and the dynamic variance of phase coherence in these 2 bands (using a sliding window approach) among 6 large-scale brain networks (default-mode, fronto-parietal, salience, sensorimotor, visual, cerebellar) in 129 drug-naïve patients with schizophrenia and 197 healthy controls. Our motivation was to exploit the large-scale resting-state slow-wave oscillatory dynamics to parse the heterogeneity of schizophrenia. Four 6*6 matrices depicting patient vs. control differences in dynamic variance and mean of phase coherence (vPC and mPC respectively) in the slow 4 and slow 5 bands were constructed from resting state fMRI time series obtained from 6 networks based on 160 nodes of Dosenbach’s atlas. Deviations in mean/variance of phase coherence among the 6 networks were identified after FDR correction for each matrix (p<0.05). A latent profile analysis (LPA) was undertaken on the basis of the identified deviant features in the patient group. LPA is a Finite Mixture Modelling approach to identify naturally occurring sub-groups of patients on the basis of multivariable data. The identified subgroups were compared in terms of the severity of clinical symptoms across van der Gaag’s 5 factors of PANSS scale. Patients with schizophrenia showed increased vPC between salience-sensorimotor and visual-cerebellar networks in band 5; decreased vPC between DMN-sensorimotor and DMN-cerebellar networks in band 4. Patients also had a decrease in mPC between DMN-visual and DMN-cerebellar networks in band 5. We were able to identify 3 subgroups of patients using LPA. SZ1 (n=28) and SZ2 (n=45) had higher overall burden of symptoms compared to SZ3 (n=56). SZ2 had the highest burden of negative syndrome score and showed most deviance from healthy controls (5 out of 7 features significantly different from the healthy cohort). SZ1 had the highest burden of positive syndrome scores and had 4 out of 7 features deviant from HC. In contrast, SZ3 had least deviation from HC (3 out of 7 features) and also had less symptom burden across all symptom dimensions. Various abnormalities have been reported in the interactions among the large-scale networks in schizophrenia, with lack of consistency ascribed to syndromic heterogeneity. We illustrate how deviations in time-varying nature of slow-wave oscillations in resting state fMRI can be exploited to meaningfully reduce heterogeneity of this illness. The 3 subgroups thus identified not only show differential symptom burden but also exhibit hierarchical deviation from a normative group of healthy controls (SZ2>SZ1>SZ3>HC). To our knowledge this is the first attempt to stratify ‘neurotypes’ among drug-naïve patients with schizophrenia on the basis of large-scale network dynamics. Given the widespread availability of resting-fMRI data, we anticipate independent replication of our results in the near future.
Two Patterns of White Matter Abnormalities in Medication-Naive Patients With First-Episode Schizophrenia Revealed by Diffusion Tensor Imaging and Cluster Analysis.

Abstract:IMPORTANCE Accumulating evidence supports the hypothesis that cerebral white matter abnormalities are involved in the pathophysiology of schizophrenia; however, findings from in vivo neuroimaging studies have been inconsistent. Besides confounding factors, including age, illness duration, and medication effects, an additional cause for the inconsistent results may be heterogeneity in the nature of white matter alterations associated with the disorder. OBJECTIVE To investigate whether different patterns of white matter abnormalities exist in a large cohort of medication-naive patients with first-episode schizophrenia and the relationship between such patterns and clinical parameters. DESIGN, SETTING, AND PARTICIPANTS A cross-sectional diffusion tensor imaging study of 113 medication-naive patients with first-episode schizophrenia and 110 demographically matched healthy control individuals. The study was conducted in the mental health center of West China Hospital, Sichuan University, Chengdu, China, from January 2006 to June 2014. MAIN OUTCOMES AND MEASURES The patterns of white matter abnormalities revealed by tract-specific analysis in conjunction with hierarchical clustering. RESULTS With diffusion features extracted from 18 fiber tracts, cluster analysis revealed 2 patterns of abnormalities. One pattern (42.5% of patient sample) showed widespread white matter abnormalities compared with matched healthy control individuals, while another pattern (57.5% of patient sample) only showed circumscribed regional white matter abnormalities, mainly in the left superior longitudinal fasciculus. Patients in these subgroups did not differ in demographic features; however, negative symptoms were more severe in patients with widespread white matter abnormalities. CONCLUSIONS AND RELEVANCE Two distinct patterns of white matter abnormalities exist at the early phase of schizophrenia, with those having global abnormalities experiencing more severe negative symptoms. The finding that distinct subgroups of patients with schizophrenia have different forms of white matter pathology may reflect qualitatively distinct genetic influences or neurodevelopmental alterations and thus represents a promising strategy for resolving neurobiological heterogeneity in the schizophrenia syndrome.
Amplitude of Low-Frequency Oscillations in First-Episode, Treatment-Naive Patients with Major Depressive Disorder: a Resting-State Functional MRI Study.

Abstract:BACKGROUND: Resting-state fMRI is a novel approach to measure spontaneous brain activity in patients with major depressive disorder (MDD). Although most resting-state fMRI studies have focused on the examination of temporal correlations between low-frequency oscillations (LFOs), few studies have explored the amplitude of these LFOs in MDD. In this study, we applied the approaches of amplitude of low-frequency fluctuation (ALFF) and fractional ALFF to examine the amplitude of LFOs in MDD. METHODOLOGY/PRINCIPAL FINDINGS: A total of 36 subjects, 18 first-episode, treatment-naive patients with MDD matched with 18 healthy controls (HCs) completed the fMRI scans. Compared with HCs, MDD patients showed increased ALFF in the right fusiform gyrus and the right anterior and posterior lobes of the cerebellum but decreased ALFF in the left inferior temporal gyrus, bilateral inferior parietal lobule, and right lingual gyrus. The fALFF in patients was significantly increased in the right precentral gyrus, right inferior temporal gyrus, bilateral fusiform gyrus, and bilateral anterior and posterior lobes of the cerebellum but was decreased in the left dorsolateral prefrontal cortex, bilateral medial orbitofrontal cortex, bilateral middle temporal gyrus, left inferior temporal gyrus, and right inferior parietal lobule. After taking gray matter (GM) volume as a covariate, the results still remained. CONCLUSIONS/SIGNIFICANCE: These findings indicate that MDD patients have altered LFO amplitude in a number of regions distributed over the frontal, temporal, parietal, and occipital cortices and the cerebellum. These aberrant regions may be related to the disturbances of multiple emotion- and cognition-related networks observed in MDD and the apparent heterogeneity in depressive symptom domains. Such brain functional alteration of MDD may contribute to further understanding of MDD-related network imbalances demonstrated in previous fMRI studies.

Abstract:Objective To investigate asymmetric changes of the prefrontal lobes and hippocampus before and after treatment in first-episode schizophrenia patients using proton magnetic resonance spectroscopy(1H-MRS).Method 21 first-episode schizophrenia patients and 21 healthy subjects matched for age,gender,and years of education were included in the study.1H-MRS was performed in two sides of the hippocampus and frontal lobes regions before and one year after treatment The ratios of N-Acetylaspartate(NAA),choline(Cho) to creatine(Cr) were measured.The patients were also assessed by(positive and negative symptoms scale,PANSS) and(global assessment fumction,GAF).One-sample T test and paired-samples T test were used for statistic process.Results The NAA / Cr was significantly higher in the left hippocampus than in the right(1.41 ± 0.09 vs.1.32 ± 0.10) for healthy control subjects(P < 0.05).There are no significant difference in brain metabolites between the left and right hippocampus in the first episode schizophrenia patients(P > 0.05).One year following treatment,the NAA / Cr ratio was significantly higher in the left hippocampus than in the right(1.41 ± 0.09 vs.1.32 ± 0.10) for the schizophrenia patents concomitant with the improvements of PANSS(58.76 ± 23.07 vs 28.05 ± 8.99) and GAR(68.52 ± 27.74 vs.97.95 ± 13.81)(P < 0.05).There was no significant difference at brain metabolites between the left and right frontal lobes regions in the first episode schizophrenia patients before and after treatment and healthy control subjects.Conclusion Asymmetry of NAA / Cr disappears in the hippocampus in the first-episode schizophrenia whereas treatment of atypical antipsychotics can attenuate the asymmetry of NAA / Cr.

Abstract:BACKGROUND:Previous studies have demonstrated that abnormities of both resting-state brain activity and cognitive dysfunction are frequently observed in patients with major depressive disorder (MDD). However, the underlying relationship between these two aspects is less investigated. In this context, the aim of the present study was to investigate the association between cognitive dysfunction and altered resting-state brain function in first-episode drug-naïve MDD patients.METHODS:Twenty-five drug-naïve MDD patients and twenty-six age-, sex-, and education-matched normal controls were recruited in this study. Cognitive function was evaluated by using a series of validated test procedures. The resting-state functional magnetic resonance imaging data were obtained on a Philips 3.0 Tesla scanner and analysed using the fractional amplitude of low frequency fluctuation (fALFF) method. Correlations of fALFF values with cognitive dysfunction were further analysed.RESULTS:Compared with healthy controls, MDD patients showed significantly fewer completed categories in the Wisconsin Card Sorting Test (WCST) and decreased scores in the first and second subtests of the Continuous Performance Test (CPT). However, the two groups did not differ in their performance on the Stroop Colour Word Test and Trail-making Test. MDD patients exhibited significantly decreased fALFF values in the left superior frontal gyrus (SFG), left middle frontal gyrus, and left inferior frontal gyrus, as well as increased fALFF values in the left inferior temporal gyrus (ITG), bilateral parahippocampal gyrus, and the right caudate. Finally, the correlation analyses revealed that fALFF values in the left SFG and left ITG were associated with the number of WSCT completed categories and scores on the second subtest of the CPT in MDD, respectively.CONCLUSIONS:The present findings suggest that there is little evidence of an association between regional abnormalities in resting-state brain function and cognitive deficits in MDD.

Abstract:Objective:To investigate the aberrant structural covariance network and its relationship with clinical symptoms in first-episode drug-na?ve patients with schizophrenia.Methods:Structural MRI data were acquired from 163 first-episode drug-na?ve schizophrenia patients and 189 health controls (HCs). SPM12 toolbox was used to preprocess data and extract the data from the region of interest. The structural covariance network was constructed based on GMV. Finally, network topological indexes were calculated by graph theoretical approaches and statistical analysis was performed using permutation test with 10 000 repetitions to test the difference between the topological parameters of the two groups. The results of principal component analysis of Positive and Negative Syndrome Scale (PANSS) scores were processed by factor rotation and weighting. According to the final weight scores, first-episode drug-na?ve schizophrenia patients were divided into high PANSS scores group and low PANSS scores group, and the differences of network topological indexes between the two groups were compared by using the method that was same as what had heen used in comparison between the groups of schizophrenia and HCs.Results:At the global level, compared to the HCs, the length of path in first-episode drug-na?ve schizophrenia patients was significantly increased ( P<0.05,FDR corrected), while local efficiency ( P<0.01,FDR corrected) and global efficiency ( P<0.01,FDR corrected) were significantly decreased. Compared to the patients with low PANSS scores, the length of path in patients with high PANSS scores was significantly increased ( P<0.05,FDR corrected), while the local efficiency ( P<0.05,FDR corrected) and the global efficiency ( P<0.05, FDR corrected) were significantly decreased. Conclusions:Structural covariance network of first-episode drug-na?ve schizophrenia patients was disrupted compared to HCs, and the more severe the clinical symptoms, the more damage to the covariance network.
Aberrant Functional Connectivity of the Orbitofrontal Cortex Is Associated With Excited Symptoms in First-Episode Drug-Naïve Patients With Schizophrenia

Abstract:Background: Schizophrenia (SZ) is associated with the highest disability rate among serious mental disorders. Excited symptoms are the core symptoms of SZ, which appear in the early stage, followed by other stages of the disease subsequently. These symptoms are destructive and more prone to violent attacks, posing a serious economic burden to the society. Abnormal spontaneous activity in the orbitofrontal cortex had been reported to be associated with excited symptoms in patients with SZ. However, whether the abnormality appears in first-episode drug-naïve patients with SZ has still remained elusive. Methods: A total of 56 first-episode drug-naïve patients with SZ and 27 healthy controls underwent resting-state functional magnetic resonance imaging (rs-fMRI) and positive and negative syndrome scale (PANSS). First, differences in fractional amplitude of low-frequency fluctuations (fALFF) between first-episode drug-naïve patients with SZ and healthy controls were examined to identify cerebral regions exhibiting abnormal local spontaneous activity. Based on the fALFF results, the resting-state functional connectivity analysis was performed to determine changes in cerebral regions exhibiting abnormal local spontaneous activity. Finally, the correlation between abnormal functional connectivity and exciting symptoms was analyzed. Results: Compared with the healthy controls, first-episode drug-naïve patients with SZ showed a significant decrease in intrinsic activity in the bilateral precentral gyrus, bilateral postcentral gyrus, and the left orbitofrontal cortex. In addition, first-episode drug-naïve patients with SZ had significantly reduced functional connectivity values between the left orbitofrontal cortex and several cerebral regions, which were mainly distributed in the bilateral postcentral gyrus, the right middle frontal gyrus, bilateral paracentral lobules, the left precentral gyrus, and the right median cingulate. Further analyses showed that the functional connectivity between the left orbitofrontal cortex and the left postcentral gyrus, as well as bilateral paracentral lobules, was negatively correlated with excited symptoms in first-episode drug-naïve patients with SZ. Conclusion: Our results indicated the important role of the left orbitofrontal cortex in first-episode drug-naïve patients with SZ and suggested that the abnormal spontaneous activity of the orbitofrontal cortex may be valuable to predict the occurrence of excited symptoms. These results may provide a new direction to explore the excited symptoms of SZ.
