Abstract:Objective:To investigate the clinical manifestation and pathological characteristics of Oligomeganephronia(OMN). Methodology:From June 2003 to July 2008,eight patients with OMN were reviewed retrospectively.The clinical and pathological feature of those patients has been analyzed.Eight age-and sex-matched patients with microhematuria served as control group.The number,diameter,area and volume of glomerulus were measured calculated in both two groups. Results:(1) Patients with OMN including 7 male and 1 female were characterized by early-onset with age from 5 to 27.All of them were negative family history of kidney disease.(2)Proteinuria was obviousely noted as the nonnephrotic range in all patients,with negative hematuria.There were 2 patients with hypertension and 6 with impaired renal function.The positive ratios of urine NAG and urine osmolarity were 100% and 75.0%.(3)Histological observation: the glomerular numbers in patients with OMN were decreased significantly compared with that in control group(P0.01).The average volume of glomerulus in OMN was 2.93 times lager than that in control group.The global or segmental scleroisis was noted in all patients.The tubluer hypertrophy and interstitial fibrosis were found in 87.5% and 75% cases. Conclusion:OMN is renal hypoplasia with early-onset-age and negative family history.Proteinuria,chronic renal insufficiency and tubular dysfunction were main clinical manifestations.The histological characteristics was prominent decrease in number and augmentation in volume of glomerulus,combined with tubluer hypertrophy and interstitial fibrosis.

Abstract:A 23-year-old woman was admitted to the hospital with proteinuria and mildly elevated creatinine, and a renal biopsy confirmed the diagnosis of oligomeganephronia (OMN). OMN is an extremely rare bilateral renal hypoplastic disease, and its diagnosis mainly relies on the pathological results obtained from renal biopsy. At present, there is no effective treatment for OMN. Here, we report a case with mild renal insufficiency and proteinuria as the main symptom and present a summary of the clinical characteristics of ON along with a review of the literature on OMN.

DIFFERENTIAL GREY AND WHITE MATTER MICROSTRUCTURAL ABNORMALITIES IN EARLY AND LATE-ONSET ALZHEIMER’S DISEASE AND MILD COGNITIVE IMPAIRMENT

Abstract:Early-onset Alzheimer's disease (EOAD) has different pathological and clinical patterns and disease course from late-onset AD (LOAD). Diffusion tensor imaging (DTI) studies have demonstrated white-matter (WM) and grey-matter (GM) microstructural changes that could be markers for AD progression. We therefore sought to demonstrate distinct patterns of brain microstructural in patients with early- and late-onset AD and mild cognitive impairment (MCI). 44 young healthy-controls (YHC, ≤65 years-old), 41 older HC (>65 years-old), 31 early-onset MCI (EOMCI), 59 late-onset MCI (LOMCI), 21 EOAD and 72 LOAD underwent T1 and diffusion MRI imaging. The free-water (FW) method was applied to derive individual FW (GM and WM) and tissue compartment fractional anisotropy (FAt) maps from DTI data. To assess age, stage, and interactions effects, we carried out ANCOVA analyses on WM-FW, WM-FAt, GM-thickness, and GM-FW images. We further tested the associations of brain-measures with Montreal Cognitive Assessment (MoCA) scores. In early-onset patients, minimal WM and GM microstructural changes were seen at the MCI stage, followed by steep damage from the MCI to AD stage. In late-onset patients, progressive microstructural alterations were demonstrated along the AD continuum (Fig. 1Band3B). Across all groups, older subjects had greater WM-FW and lower WM-tissue FAt than the younger counterparts (Fig.1). For GM-thickness, there were no age-related differences at pre-dementia stages, although EOAD had lower thickness than LOAD in the parietal regions (precuneus), while LOAD had lower thickness than EOAD in the precentral gyrus and superior-temporal gyrus (Fig. 2). In contrast to GM-thickness, GM-FW increases appeared early at the pre-dementia stage (LOMCI>EOMCI). EOAD had greater GM-FW increases in the parietal/precuneus and middle-temporal regions than LOAD (Fig. 3). Lastly, parietal GM-FW increases contributed to global cognitive impairment in MCI while parietal and temporal WM abnormalities and GM thinning related to lower MoCA in dementia (Table 1).

Abstract:Objective To investigate clinical and pathological features of idiopathic membranous nephropathy (IMN) accompanied by mesangial dense deposit.Methods Clinical data of 46 patients who were diagnosed as IMN accompanied by mesangial dense deposit admitted to Zhejiang Provincial People's Hospital from January 2013 to December 2014 were retrospectively analyzed.They were compared with those of 29 patients who were diagnosed as IMN without mesangial dense deposit during the same period in the hospital.Analysis of their clinical and pathological features was conducted.Results The IMN accompanied by mesangial dense deposit accounted for IMN 61.3％,and had more hyaline changes of arteriole (43.5％ vs 6.9％,P=0.001) and more obvious arteriolar wall thickening (78.2％ vs 51.7％,P=0.016) than IMN without mesangial dense deposit.Furthermore,the positive rate of IgA deposition in IMN accompanied by mesangial dense deposit was greatly higher than that in IMN without mesangial dense deposit (21.7％ vs 0,P=0.007).In other index,such as serum biochemical parameters,urine protein,glomerular lesion,tubulointerstitium pathological damage and other immunopathologic changes,no statistically significant differences were found between these two groups.Conclusions IMN patients accompanied by mesangial dense deposit have severe intrarenal artery lesions,and high positive rate of IgA deposition.

Abstract:Distal hereditary motor neuropathy (dHMN) is a group of clinically and genetically heterogeneous disorders characterized by progressive distal weakness and atrophy. The onset of dHMN is at mid-adulthood or early childhood, and the symptoms are mainly present in the lower limbs. Besides weakness and atrophy of distal limb muscles, some patients may develop bulbar paralysis, and some may also present with mild sensory disturbance. Decreased or absent tendon reflexes may be discovered. Electromyography may show neurogenic damages. Muscular biopsy may reveal neurogenic amyotrophy. An increasing number of genes have been associated with dHMN. Pathogenesis of dHMN may include formation of protein aggregates, impairment of autophagy pathway, RNA processing, translation synthesis, axonal transport, endoplasmic reticulum stress, calcium channel and neuroprotection. A review for recent progress made on clinical characterization and molecular genetics of dHMN is provided.
Different Iron Deposition Patterns in Early- and Middle-Late-onset Parkinson's Disease.

Abstract:Background: Iron deposition may contribute to the clinical symptoms in Parkinson's disease (PD). With partial different clinical manifestations, the iron deposition patterns between patients with early-onset Parkinson's disease (EOPD) and middle-late-onset Parkinson's disease (M-LOPD) are still unclear. This study was designed to investigate the patterns of iron deposition and their clinical relevance in EOPD and M-LOPD patients, using quantitative susceptibility mapping technique. Materials and methods: Thirty-five EOPD patients and 24 matched young controls, 33 M-LOPD patients and 22 matched older controls were recruited in the study. The iron content in the deep grey matter nuclei in the basal ganglia and midbrain were measured, and compared between patients and their corresponding controls. The correlations of regional iron content and clinical features were explored in patient groups. Results: Both M-LOPD and EOPD patients showed increased iron content in the substantia nigra (SN) pars compacta and SN pars reticulata. Increased iron content in the putamen was only observed in M-LOPD patients. The relationship between the increased iron content and disease severity (H&Y stages, UPDRS II scores and UPDRS III scores) was observed in M-LOPD patients, but not in EOPD patients. Conclusion: Our study suggested that the iron deposition pattern was greatly influenced by the age of PD onset, which increases our understanding of the different pathological underpinnings of EOPD and M-LOPD patients. Abstract:Early-onset Parkinson’s disease (EOPD) has a clinical course and characteristics distinct from middle-late-onset Parkinson’s disease (M-LOPD). Although many studies have investigated these differences, the neural mechanisms of these characteristics remain unclear. This study aimed to investigate the morphological differences, and their related clinical significance, between EOPD and M-LOPD patients. We recruited two groups of patients, 28 EOPD patients and 37 M-LOPD patients, and two age- and sex-matched control groups (23 controls in each group). The voxel-based morphometry (VBM) technique was used to examine changes in gray matter (GM) density between patients and their corresponding controls. Compared with controls, EOPD patients had lower GM density in the left putamen, inferior frontal gyrus and insula, and higher GM density in the right occipital lobe and bilateral cerebellum posterior lobes. M-LOPD patients had lower GM density in the left cerebellum posterior lobe, occipital lobe and right supplementary motor area (SMA), and higher GM density in the left middle temporal gyrus. Correlation analyses showed that GM density values in the right cerebellum posterior lobe positively correlated with the Unified Parkinson’s Disease Rating Scale (UPDRS) motor scores and the Hoehn–Yahr stages in EOPD patients. Our results reveal different patterns of structural changes in EOPD and M-LOPD patients. A probable compensatory effect of the cerebellum was observed and may partly explain the slower decline of motor function in EOPD patients.

Abstract:Objective:To investigated the clinical, histological and ultrastructural features of obesity-related glomerulopathy (ORG). We reviewed the native renal biopsies from Aug. 2000 to Aug. 2002, 15 patients, whose BMI more than 28 kg/m 2, were diagnosed ORG. They were 13 males and 2 female with a mean of (32 64±8.06) years old. According to the morphologic lesions, two groups of ORG were divided. They were glomerulomegaly alone (O-GM, group Ⅰ , n =8) and focal segmental glomerulosclerosis with glomerulomegaly (O-FSGS, group Ⅱ, n =7). Their clinical and laboratory findings were analyzed and compared. non-nephrotic range protienuria comprised of medium size molecule was encountered in 80% patients, microscopic hematuria in 33% cases. No anemia was found in any patient. Hypertriglyceridemia was more prevalent than hypercholesterolemia in ORG patients (respectively 53 3% and 40%), Elevated serum uric acid occurred in 7 patients (46 7%), hyperinsulinemia in 3/5 cases, elevated AST in 5 cases (33 3%). Liver sonography showed fatty liver in all patients. Glomerulomegaly was the specific feature in all sections with or without segmental sclerosis. Hyaline and foam cell located in glomerular capillary lumen and tip lesion occurred in some cases with O-FSGS, occasionally in patients with O-GM. Ultrastructural changes indicated mild podocyte effacement and microvilli, but the thickened glomerular basement membrane existed in early stage.ORG manifests with non-nephrotic range proteinuria. The morphologic features include glomerulomegaly without/with FSGS and thickened GBM in the early stage.
[Clinical characteristics of early-onset type 2 diabetes and risk factors associated with its chronic complications].

Abstract:OBJECTIVE:To investigate the clinical characteristics of early-onset type 2 diabetes (EOD) and the risk factors associated with its chronic complications. METHODS:A total of 2713 T2DM patients were recruited and divided into two groups: early- and late-onset groups EOD group (diagnosed at the age <or= 40) and late-onset type 2 diabetes (LOD) group (diagnosed at the age > 40). The clinical characteristics were compared between two groups, and the risk factors were analyses within the early-onset group. RESULTS:(1) The EOD patients were characterized by significantly higher levels of triglycerides (2.5 +/- 2.4 mmol/L vs 1.9 +/- 1.5mmol/L, P < 0.01) and BMI (24.4 kg/m(2) +/- 4.6 kg/m(2) vs 23.6 kg/m(2) +/- 3.7 kg/m(2), P < 0.01), higher rates of smoking (29.2% vs 22.0%, P < 0.01), family history of diabetes (28.6% vs 13.1%, P < 0.01), and higher prevalence of metabolic syndrome (18.9% vs 10.7%, P < 0.01), higher proportion of patients with unacceptable HbA1c (>7.0%) (77.3% versus 72.7%, P < 0.05) and fasting blood glucose >or= 7.0 mmol/L, (80.8% versus 74.6%, P < 0.05). (2) The prevalence rates of microvascular complications were higher than those of macrovascular complications in the EOD group, and the prevalence rates of pain, anesthesia, diarrhea, and nephropathy were 10.6%, 6.20%, 6.00%, and 13.00% respectively, all significantly higher than those of the LOD group (7.10%, 2.80%, 3.70%, and 8.00% respectively, P < 0.05, < 0.01, < 0.05, and < 0.01). Linear regression analysis showed that certain subsets of chronic complications were associated with onset age, diabetes duration, hypertension, family history and high level of triglyceride. CONCLUSION:Early-onset DM patients present the clinical features of poorer metabolic profiles, which may contribute to the development of certain subsets of microvascular complications. Late-onset patients tend to be complicated with macrovascular diseases, such as cerebrovascular disease, angiocardiopathy, etc. Multiple factor comprehensive control is important for the prevention of the complications.

Abstract:In the last 20 years,the population of overweight and obesity has been growing rapidly. And obesity-related glomerulopathy (ORG),which can be directly induced by obesity,has received more and more attention.The pathogenesis of ORG is complex,including hemodynamic dysfunction,kidney hypoxia,insulin resistance,over-activation of sympathetic nerve,and so on,in which podocyte injury appears to play a central role.The typical renal histologic features of ORG consisted of simple obesity-associated glomerular hypertrophy,and obesity-associated focal segmental glomerulosclerosis.The most striking clinical characteristic of ORG is the existence of massive proteinuria with very low incidence of nephrotic syndrome.The treatment for ORG mainly aims at the proper control of the associated risk factors, such as weight control,ameliorating insulin resistance,inhibiting over-activation of RAAS,etc,in order to delay the deterioration of renal functions.
Clinical Characteristics of Chinese Male Patients with Aquaporin-4 Antibody-Positive Late-Onset Neuromyelitis Optica Spectrum Disorder

Abstract:Background and Objective: Limited studies are available for male patients with anti-aquaporin-4 antibody (AQP4-Ab)-positive late-onset neuromyelitis optica spectrum disease (LONMOSD). The aim of this study was to investigate the clinical characteristics of Chinese male patients with AQP4-Ab-positive LONMOSD. Methods: We retrospectively reviewed the medical records of 12 male patients with LONMOSD, 16 male patients with early-onset NMOSD (EONMOSD), and 64 female patients with LONMOSD. These enrolled patients were classified according to the age of onset: LONMOSD (≥50 years of age at onset) versus EONMOSD (<50 years of age at onset). Clinical characteristics and magnetic resonance imaging (MRI) findings were collected. All included patients were positive for AQP4 antibody. Results: Compared with female LONMOSD patients, male LONMOSD patients had less frequent transverse myelitis (TM) at onset (8.33 vs. 53.13%, p = 0.004) and lower Expanded Disability Status Scale (EDSS) scores (median 1 vs. 4, p = 0.036). Compared with male EONMOSD patients, male LONMOSD patients had a shorter time from onset to diagnosis (0.85 months vs. 6.00 months, p = 0.04). Conclusion: Less common TM at onset, less disease severity, and shorter time from onset to diagnosis probably occur in male LONMOSD patients.

Abstract:Objective: To study the clinical features of late-onset neuromyelitis optica spectrum disorders(LONMOSD). Methods: Twenty-eight patients with LONMOSD and fifty-one patients with early-onset neuromyelitis optica spectrum disorders(EONMOSD) hospitalized in Navy General Hospital from January 2014 to May 2017 were enrolled and were followed up by telephone or outpatient visiting. The clinical manifestations, laboratory examinations and imaging features of the two groups were compared. Results: (1)The average age of onset in group LONMOSD was(59±6) years old, and 24 cases were female(85.7%). (2) The history of prodromal infection in LONMOSD patients was less reported than that in group EONMOSD(14.3 vs 37.3%, P<0.05), but concomitant diseases were more common in LONMOSD patients(53.6% vs 3.9%, P<0.05). (3) In group of LONMOSD, the patients with transverse myelitis(TM )as the first symptom were less than that of EONMOSD group (39.3% vs 64.7%, P<0.05). (4) There was no significant difference in EDSS score either in acute or remission stage, laboratory and imaging findings between the two groups. Conclusion: Patients with LONMOSD have less history of prodromal infection, and those with TM as the first symptom are less than EONMOSD patients.

Abstract:Aim:To investigate the clinical and pathological features of late-onset glycogen storage disease type Ⅱ(GSD type Ⅱ).Methods: Eleven GSD type Ⅱ patients were followed up,and their clinical profiles and pathological data were reviewed retrospectively.Results: Eleven late-onset cases presented with a slowly progressive proximal myopathy,some with predominant respiratory insufficiency.Serum creatine kinase was mildly to moderately elevated(262~1 802 U.L-1).Electromyography indicated a definite myopathy in 9 patients,and abnormal irritability in 3 patients.The common muscle pathological feature was a vacuolar myopathy with high glycogen content and acid phosphatase acitvity in the vacuoles.Conclusion: Late-onset GSD type Ⅱ often presented slowly progressive myopathy,which often affected the limb and respiratory muscles.The CK level was usually mildly to moderatly elevated,muscle pathology showed remarkable vacuolar myopathy.The α-glucosidase activity was significantly reduced,which could help to diagnose.

Abstract:Background: Two typical pathological changes can develop in the type 2 diabetic nephropathy: one is diffuse glomerulosclerosis (DIF) and the other is nodular glomerular lesion (NOD). The aim of this study is to retrospectively investigate these two types of diabetic nephropathy and compare the characteristics of clinical findings and prognosis.Methods: One hundred and twenty-four patients with biopsy-proven diabetic nephropathy, including 58 DIF and 66 NOD were investigated in this study. Clinical data were registered for each patient: age, sex, diabetes duration, hypertension duration, body mass index (BMI), and laboratory tests included: glycosylated hemoglobin, plasma albumin, plasma cholesterol and triglycerides, serum creatinine, proteinuria, urine osmole, urine N-acetyl-beta-D-glucosaminidase (NAG) enzyme and creatinine clearance rate (Ccr). Furthermore, the 1- and 5-year renal survival rates were analysed and compared.Results: (1) Compared with DIF patients, NOD patients had a longer duration of diabetic mellitus, and higher levels of proteinuria and NAG enzyme, along with lower BMI, Ccr and plasma albumin levels (p < 0.01). (2) In the NOD cases, there were 90% presented diabetic retinopathy; while in the DIF cases, there were 14% presented diabetic retinopathy. (3) In the DIF patients, the 1- and 5-year renal survival rates were 96 and 78%, respectively, while in the NOD patients, the rates were 86 and 18%, respectively (p < 0.01).Conclusion: The NOD patients had longer diabetic durations, more severe renal damage, and exhibit a poorer renal prognosis, Furthermore, the NOD patients revealed more frequently evidence of diabetic retinopathy. Abstract:Acquired sporadic late onset nemaline myopathy (SLONM) and inherited nemaline myopathy (iNM) both feature accumulation of nemaline rods in muscle fibers. Unlike iNM, SLONM is amenable to therapy. The distinction between these disorders is therefore crucial when the diagnosis remains ambiguous after initial investigations. We sought to identify biomarkers facilitating this distinction and to investigate the pathophysiology of nemaline rod formation in these different disorders. Twenty-two muscle samples from patients affected by SLONM or iNM underwent quantitative histological analysis, laser capture microdissection for proteomic analysis of nemaline rod areas and rod-free areas, and transcriptomic analysis. In all iNM samples, nemaline rods were found in subsarcolemmal or central aggregates, whereas they were diffusely distributed within muscle fibers in most SLONM samples. In SLONM, muscle fibers harboring nemaline rods were smaller than those without rods. Necrotic fibers, increased endomysial connective tissue, and atrophic fibers filled with nemaline rods were more common in SLONM. Proteomic analysis detected differentially expressed proteins between nemaline rod areas and rod-free areas, as well as between SLONM and iNM. These differentially expressed proteins implicated immune, structural, metabolic, and cellular processes in disease pathophysiology. Notably, immunoglobulin overexpression with accumulation in nemaline rod areas was detected in SLONM. Transcriptomic analysis corroborated proteomic findings and further revealed substantial gene expression differences between SLONM and iNM. Overall, we identified unique pathological and molecular signatures associated with SLONM and iNM, suggesting distinct underlying pathophysiological mechanisms. These findings represent a step towards enhanced diagnostic tools and towards development of treatments for SLONM.
Clinical and psychopathological features of very late onset of schizophrenia-like psychosis

Abstract:Introduction Very late onset schizophrenia-like psychosis takes the 3 rd place among late-life psychosis, after dementia and affective disorders associated psychosis. It's still unknown the real place of this psychosis. Objectives to investigate the clinical and psychopathological features and short-terms outcomes of late-onset schizophrenia and schizophrenia-like psychosis Methods 45 patients, mean age 70,6 ±8,70 years, median age of manifestation psychosis - 68 [61; 75] years with late-onset schizophrenia (n=19, 42,2%), late-onset schizoaffective disorder (n=9, 20%), late-onset delusional disorder (n=7, 15,5%) and late-onset organic schizophrenia-like disorder (n=10, 22,3%) underwent clinical examination. Psychopathological, psychometric (PANSS, HAMD, CDSS, MoCA) and statistical methods were applied. Results 3 clinical groups were allocated. The 1 st group included 15 patients (33%) and was characterized with severe polymorphic psychotic symptoms, included catatonic and paraphrenic signs with mental disorganization. They had the highest score of PANSS (105,46±17,99, p=0,002) and the lowest score of MoCA (14,2±2,16, p=0,05) in compare with 2 nd and 3 rd groups. They also had symptoms of depression (CDSS 6,28±5,29), compared with the 3 rd group (HAMD 21,00±5,92, p=0,05). In short-terms outcomes was formed negative symptoms and cognitive impairment with decreasing social and daily activity. The 2 nd group (22 cases, 49%) included patients with prevalence of delusions of persecution, more rare auditory hallucinations and more often acoasms. They had medium score PANSS (90,22±16,79), with minimal cognitive declare (MoCA average score 20,33±4,27). The short-term outcomes were characterized with formation of residual positive and negative symptoms, that impact on daily and social activity. The 3 rd group included 8 patients (8%) with prevalence of delusion symptoms, such as misidentification, persecutory and reference delusions, which were mood-congruent. They had medium PANSS score (89,75±18,90) with more severe depressive symptoms by HAMD scale in compare with 2 nd group (22,00±10,00, p=0,07) and minimal cognitive declare (MoCA average score 25,00±1,00, p=0,05) in compare with 1 st group. This group was characterized with high level of reduction of productive symptoms and restoration of premorbid social and daily activity in short-term outcomes. Conclusions features of clinical characteristics, including the nature and severity of cognitive impairment at the onset of disease, are significant for prognosis and outcomes of disease. The data obtained could be served for the development of personalized therapeutic approaches that take into account the syndromic features and course of late-onset psychosis. Abstract:BACKGROUND: Pompe disease is a rare, progressive, and life-threatening autosomal recessive disorder. In its late-onset form, the disease is primarily characterised by mild progressive proximal limb and respiratory muscle weakness. Mutations in the acid alpha-glucosidase (<i>GAA</i>) gene cause lysosomal enzyme GAA to be significantly reduced or missing altogether, for which supplementation can be given through enzyme replacement therapy.METHODS: Fourteen patients diagnosed with late-onset Pompe disease (LOPD) in the First Affiliated Hospital of Nanjing Medical University from 2017 to 2021 were enrolled. GAA activity was measured based on enzymatic activity in dried blood spots, and next-generation sequencing was used to detect mutations in the <i>GAA</i> gene. The impacts of novel missense variants were determined by five different prediction algorithms. The structural figures of novel variants and their wide types were processed with PyMOL.RESULTS: The study included 14 patients with LOPD (male-to-female ratio, 1:1) from eastern China. The median age at symptom onset and diagnosis was 15.0 years (7-36 years) and 21.5 years (8-47 years), respectively. The median diagnostic delay from onset was 3.0 years (0-22 years). Proximal muscle weakness was the first prominent symptom in 8 patients, while the other 6 patients experienced respiratory failure, chest congestion and asthma, and scoliosis. The most frequent mutation of the GAA gene was c.2238G>C (p.W746C), which was observed at an allele frequency of 14.3% (4/28) and in 28.6% of patients (4/14). Four novel variants potentially related to the pathogenicity of LOPD were found: c.1299G>C (p.Q433H), c.1409A>G (p.N470S), c.2242delG (p.E748Rfs*16), and c.2832delA (p.E945Sfs*78).CONCLUSIONS: The c.2238G>C (p.W746C) mutation was the most common mutation in 14 patients with LOPD from eastern China. This study has identified four novel variants in patients with LOPD. Predicting the pathogenicity of these novel variants may increase the understanding of the genetic mutation spectrum in LOPD. Our findings may also improve recognition of the characteristics of Chinese patients with LOPD.

Overlapping obesity-related glomerulopathy and immunoglobulin A nephropathy: clinical and pathologic characteristics and prognosis

Abstract:Background: In this study, we investigated the clinical and pathologic characteristics and prognosis of overlapping obesity-related glomerulopathy (ORG) and immunoglobulin A nephropathy (IgAN) (ORG + IgAN), which is rare in the clinic. Methods: We included 62 cases of ORG + IgAN, 110 cases of ORG without other glomerulopathy (ORG alone) and 124 cases of IgAN without other glomerulopathy (IgAN alone). The clinical, pathologic and prognostic data were collected and compared. Results: ORG + IgAN patients showed a higher incidence of body mass index (BMI), higher incidence of hyperuricemia, higher incidence of hypertriglyceridemia and higher blood glucose than the IgAN alone(all P < 0.05). ORG + IgAN patients presented with higher incidence of microscopic hematuria, greater mesangial cell proliferation and a higher proportion of crescents than the ORG alone (all P < 0.05). The ORG + IgAN patients who received corticosteroid or immunosuppressive therapy achieved a higher cumulative rate of partial or complete remission (PR or CR, P = 0.009). However, there was no significant difference in the cumulative renal survival rate between the ORG + IgAN patients in the glucocorticoids/immunosuppressors and non-glucocorticoids/immunosuppressors groups (P = 0.356). Obesity-related focal segmental glomerulosclerosis (O-FSGS) and body mass index (BMI) were significantly associated with poor prognosis (all P < 0.05). Conclusions: ORG + IgAN should be considered in obese patients who present with metabolic abnormalities and microscopic hematuria. Although corticosteroid or immunosuppressive therapy achieves higher cumulative incidence rates of PR or CR, there is no benefit to long-term prognosis but an increased risk of infection. Moreover, O-FSGS and BMI are significantly associated with poor prognosis.
Facial Onset Sensory and Motor Neuronopathy (FOSMN Syndrome): Cases Series and Systematic Review

Abstract:OBJECTIVE:To provide new and comprehensive evidence for diagnosis and management of FOSMN syndrome.METHODS:We reviewed our database to identify patients with FOSMN syndrome. Online database including PubMed, EMBASE, and OVID were also searched for relevant cases.RESULTS:We identified a total of 71 cases, including 4 cases from our database and 67 ones from online searching. A predominance of male was observed [44 (62.0%)] with median onset age of 53 (range: 7-75) years old. The median (range) disease duration was 60 (3-552) months at the time of the visit. The initial symptoms could be sensory deficits in face (80.3%) or oral cavity (4.2%), bulbar paralysis (7.0%), dysosmia (1.4%), dysgeusia (4.2%), weakness or numbness of upper limbs (5.6%), or lower limbs (1.4%). Abnormal blink reflex was presented in 64 (90.1%) patients. CSF tests showed elevated protein level in 5 (7.0%) patients. Six (8.5%) patients had MND-related gene mutation. Five (7.0%) patients showed transient responsiveness to immunosuppressive therapy, then deteriorated relentlessly. Fourteen (19.7%) patients died, with an average survival time of around 4 years. Among them, five patients died of respiratory insufficiency.CONCLUSION:The age of onset, progress of disease course, and prognosis of FOSMN syndrome could be varied significantly. The prerequisites of diagnosis were progressive and asymmetric lower motor neuron dysfunction, with sensory dysfunction which usually showed in face at the onset. Immunosuppressive therapy could be tried in some patients with suspected inflammatory clues. In general, FOSMN syndrome tended to be motor neuron disease with sensory involvement.
