Abstract:BACKGROUND: Recurrent pneumonia is uncommon in children and few studies investigate the clinical impact of underlying diseases on this issue. This study aimed to explore the difference in clinical manifestations, pathogens, and prognosis of recurrent pneumonia in children with or without underlying diseases.METHODS: We conducted a retrospective study of pediatric recurrent pneumonia from 2007 to 2019 in National Taiwan University Hospital. Patients under the age of 18 who had two or more episodes of pneumonia in a year were included, and the minimum interval of two pneumonia episodes was more than one month. Aspiration pneumonia was excluded. Demographic and clinical characteristics of patients were collected and compared.RESULTS: Among 8508 children with pneumonia, 802 (9.4%) of them had recurrent pneumonia. Among these 802 patients, 655 (81.7%) had underlying diseases including neurological disorders (N = 252, 38.5%), allergy (N = 211, 32.2%), and cardiovascular diseases (N = 193, 29.5%). Children without underlying diseases had more viral bronchopneumonia (p < 0.001). Children with underlying diseases were more likely to acquire Staphylococcus aureus (p = 0.001), and gram-negative bacteriae, more pneumonia episodes (3 vs 2, p < 0.001), a longer hospital stay (median: 7 vs. 4 days, p < 0.001), a higher ICU rate (28.8% vs 3.59%, p < 0.001), and a higher case-fatality rate (5.19% vs 0%, p < 0.001) than those without underlying diseases.CONCLUSION: Children with underlying diseases, prone to have recurrent pneumonia and more susceptible to resistant microorganisms, had more severe diseases and poorer clinical outcomes. Therefore, more attention may be paid on clinical severity and the therapeutic plan.

Abstract:Pneumonia is a significant cause of death for children, particularly those in developing countries. The records of children who were hospitalized because of pneumonia between January 2003 and December 2015 were retrospectively reviewed, and patients who met the recurrent pneumonia criteria were included in this study. During this 13-year period, 1395 patients were hospitalized with pneumonia; of these, 129 (9.2%) met the criteria for recurrent pneumonia. Underlying diseases were detected in 95 (73.6%) patients, with aspiration syndrome (21.7%) being the most common. Rhinovirus (30.5%), adenovirus (17.2%) and respiratory syncytial virus (13.9%) were the most frequent infectious agents. These results demonstrate that underlying diseases can cause recurrent pneumonia in children. Viruses are also commonly seen in recurrent pneumonia. Appropriate treatments should be chosen based on an analysis of the underlying disease, the patient's clinical condition and the laboratory and radiological data.

Abstract:Background Pediatric pneumonia remains a significant health challenge, while the viral risk factors for adverse outcomes in pediatric pneumonia are not yet fully clear. Methods A matched case-control study of pediatric patients with pneumonia was carried out in Beijing, China, between 2007 and 2015. The study enrolled 334 intensive care unit patients who developed life-threatening diseases and 522 controls matched to the sex, age, ethnicity, admission dates, and residing district of the cases suffered from pneumonia. Nasopharyngeal aspirates were taken from all participants and tested by PCR for 18 common respiratory viruses. Results At least, one virus was detected in 257 (77%) of the cases and 409 (78%) of the controls. We observed no difference in the prevalence of 17 respiratory viruses between cases and controls but found a higher frequency of influenza A virus (IFV-A) in the cases than in the controls (7% vs 4%, P = .036). After adjusting for comorbid conditions and a history of reactive airway diseases, IFV-A was associated with an increase in life-threatening pneumonia (adjusted odds ratio = 2.55, 95% CI = 1.24-5.24). Young age and congenital heart disease (aOR = 10.16-10.27, P < .001) were also independent risk factors. Conclusions The prevention of IFV infection is critical in decreasing the risk of life-threatening pneumonia in children.

Abstract:Background: Pneumonia is an acute lung inflammation caused by both infectious and non-infectious sources. Recurrent pneumonia is caused by weakened local or systemic host defenses, or by underlying lung diseases. Early diagnosis and treatment of the underlying cause should minimize pneumonia-related hospitalizations, morbidity, and death. Children hospitalized to the Pediatric Intensive Care Unit (PICU) with severe pneumonia have significant morbidity and death rate. Aim of The Work: To identify the underlying causes and clinical profile of children with sRP admitted to the Pediatric Intensive Care Unit. Patients and Methods: This research included 100 children with sRP hospitalized to PICU at Al-Hussein &Bab El-Shaarya University hospitals during the period from Feb. 2020 to Feb 2022. The research protocol was approved by the committee for research ethics at Al-Azhar faculty of medicine .All patients completed informed consent forms before the study began. We recruited all patients with severe recurrent pneumonia. Results: The average age of sRP diagnosis was 14.8 months, the age of the first pneumonia episode was 12.1. Males were twice of females to be diagnosed with sRP. Wheezing was the most prevalent symptom with 99% of cases then wheezing (63%) and fever (60%). Respiratory abnormalities were the most prevalent (28%) as underlying causes of sRP patients, then Immune disorders (22%) and congenital heart diseases (16%). Pulmonary hemorrhagic syndrome was the least prevalent cause as it was observed in only 2 cases. Conclusion: Severe recurrent pneumonia is common in pediatric critical care units. About ninety percent of individuals with recurrent severe pneumonia were also suffering from some other disease, including respiratory, immunological, congenital heart, and aspiration syndrome.

Abstract:Background Recurrent pneumonia (RP) accounts for 7.7%–9% of childhood pneumonia. Identifying the cause of RP is crucial for infection control and reducing mortality and morbidity. This study aimed to investigate the etiology, risk factors, and diagnostic procedures of RP based on the literature using a diagnostic algorithm. Methods Our study included RP patients aged 0–18 years who were followed up as outpatients or inpatients between 2018 and 2021. Patients were analyzed retrospectively using the national health database. Patients with RP were identified by ICD diagnosis codes. Etiology and risk factors were determined based on the occurrence of RP in the same or different areas. Results The rate of RP was found to be 14.4% among the cases of recurrent lower respiratory tract infection. Of these, 27.6% occurred in the same area and 72.4% in different areas. The underlying etiology was identified in 224 of 250 RP patients (89.6%). RP in different areas was mainly due to neuromuscular diseases, whereas asthma, right middle lobe syndrome, and congenital lung or airway structural disease were common causes of RP in the same area. Malnutrition, passive smoke exposure, and prematurity were common risk factors in both groups. Conclusions Systematic etiological investigations should take into account the characteristics of the patient population and geographical region. The use of diagnostic algorithms based on recurrence in the same or different areas is particularly beneficial. Whether asthma is causally related to RP episodes or is a coincidental association due to inadequate differential diagnosis remains unclear.

Abstract:Background: Inflammatory response, oxidative stress, and immunologic mechanism are involved in the pathogenesis of Mycoplasma pneumoniae pneumonia (MPP). However, the role of immune system of pediatric interstitial pneumonia due to M. pneumoniae infections remains poorly understood. The aim of this study was to analyze the immunologic features of pediatric interstitial pneumonia due to Mycoplasma pneumoniae (M. pneumoniae). Methods: A retrospective study was conducted on a primary cohort of children with MPP. Propensity score analysis was performed to match interstitial pneumonia and pulmonary consolidation children. Results: The clinical characteristics strongly associated with the development of interstitial pneumonia were boys, age >5 years, wheezing history, hydrothorax free, lymphocytes (>3.0 × 109/L), CD19+ (>0.9 × 109/L), CD3+ (>2.5 × 109/L), CD4+ (>1.5 × 109/L), CD8+ (>0.9 × 109/L), interleukin-6 (IL-6, <30 pg/ml), IL-10 (<6 pg/ml), and interferon-γ (IFN-γ, <15 pg/ml). After propensity score analysis, children with interstitial pneumonia showed significantly higher CD19+, CD3+, and CD4+ T cell counts, and lower serum IL-6, IL-10, and IFN-γ levels. The final regression model showed that only CD4+ T cells (>1.5 × 109/L, OR = 2.473), IFN-γ (<15 pg/ml, OR = 2.250), and hydrothorax free (OR = 14.454) were correlated with the development of interstitial pneumonia among children with MPP. Conclusions: The M. pneumoniae-induced interstitial pneumonia showed increased CD4+ T cells and lower serum IFN-γ level. Specific immunologic profiles could be involved in the development of pediatric interstitial pneumonia due to M. pneumoniae infections.
Necrotizing pneumonia caused by refractory Mycoplasma pneumonia pneumonia in children

Abstract:Background To investigate the clinical features of necrotizing pneumonia (NP) caused by refractory Mycoplasma pneumoniae pneumonia (RMPP). Methods A retrospective observational study was carried out in patients with NP caused by RMPP who were admitted to our hospital from January 2008 to December 2015, and the clinical manifestations, laboratory data, imaging performances, hospital courses and outcomes were analyzed. Results Twenty-five patients with NP caused by RMPP were collected, with a median age of 5.1 (4.0–7.9) years. The mean duration of fever and hospital stay was 21.0 ± 8.9 and 19.9 ± 9.9 days, respectively. The levels of lactate dehydrogenase (LDH), C-reactive protein, interleukin (IL)-6, IL-10 and interferon-gamma were elevated. Meanwhile, the pleural fluid cell count, LDH and protein were also increased. 80.0% of the patients had pleural effusion; and a high incidence of lobar atelectasis and pulmonary consolidation was found the patients. The mean duration from the onset of symptoms to the discovery of necrotic lesions was 21.0 ± 6.9 days. 80.0% of the patients were administrated corticosteroids, and bronchoalveolar lavage was extracted separately from all patients. Of the 20 patients who presented with pleural effusion, 11 underwent thoracocentesis alone and 2 underwent chest drainage. All patients received prolonged courses of antibiotics (32.2 ± 8.7 days). All patients were dischaged home and recovered without surgical intervention; and chest lesions were resolved or only minimal residual fibrotic changes were residual within 3.0 (2.0–6.0) months. Conclusions Necrotizing pneumonia caused by RMPP is severe, however, self-limiting and reversible. Good outcomes can be achieved with appropriate management.

Abstract:Community-acquired pneumonia (CAP) is one of the leading causes of morbidity and mortality in children worldwide. In this study, we aimed to describe the aetiology of viral infection of pediatric CAP in Chinese mainland. During November 2014 to June 2016, the prospective study was conducted in 13 hospitals. The hospitalized children under 18 years old who met the criteria for CAP were enrolled. The throat swabs or nasopharyngeal aspirates (NPAs) were collected which were then screened 18 respiratory viruses using multiplex PCR assay. Viral pathogens were present in 56.6% (1539/2721) of the enrolled cases, with the detection rate of single virus in 39.8% of the cases and multiple viruses in 16.8% of the cases. The most frequently detected virus was respiratory syncytial virus (RSV) (15.2%, 414/2721). The highest detection rate of virus was in < 6-month-age group (70.7%, 292/413). RSV, human metapneumovirus (HMPV), human parainfluenza viruses (HPIVs) and influenza B virus (Flu B) showed the similar prevalence patterns both in north and south China, but HPIVs, Flu A, human bocavirus (HBoV), human adenovirus (HAdV) and human coronaviruses (HCoVs) showed the distinct circulating patterns in north and south China. Human enterovirus/human rhinovirus (HEV/HRV) (27.6%, 27/98), HBoV (18.4%, 18/98), RSV (16.3%, 16/98) and HMPV (14.3%, 14/98) were the most commonly detected viruses in severe pneumonia cases with single virus infection. In conclusion, viral pathogens are frequently detected in pediatric CAP cases and may therefore play a vital role in the aetiology of CAP. RSV was the most important virus in hospitalized children with CAP in Chinese mainland.
THE CLINICAL CHARACTERISTICS AND PREDICTORS OF REFRACTORY MYCOPLASMA PNEUMONIAE PNEUMONIA IN CHILDREN

Abstract:ObjectiveTo analyze the clinical characteristics of refracory Mycoplasma pneumoniae pneumonia (RMPP), and explore the related factors predicting RMPP.MethodsRetrospective analysis was performed on 634 children with Mycoplasma pneumoniae pneumonia (MPP) hospitalized in our hospital between January 1, 2011 and December 31, 2014. The clinical features, laboratory data, radiological findings between the RMPP group and the general Mycoplasma pneumoniae pneumonia (GMPP) group were compared and the predictive values of related factors were analyzed.ResultsThe median age of the RMPP patients (n = 145) was much older than that of the GMPP patients (n = 489) (P<0.01). We also found more severe presentations, higher incidence of extra-pulmonary complications and more serious radiological findings in RMPP group, which needed oxygen more often, longer antibiotics administration and intensive care (P<0.05). Meanwhile, the levels of C-reactive protein (CRP), lactic dehydrogenase (LDH), immunoglobulin A (IgM), interleukin (IL)-6, IL-10, interferon gamma (IFN-gamma) and the percentage of neutrophils, CD8+ in RMPP group were significantly higher than those in GMPP group (P<0.05); while the levels of prealbumin (PAB) were lower than that in GMPP group (P<0.01). In ROC curve analysis, the percentage of neutrophil, CRP, LDH, PAB, IL-6, IL-10 and IFN-gamma were useful for differentiating patients with RMPP from those with GMPP. Multiple logistic regression analysis showed that the CRP >= 16.5mg/L, LDH >= 417IU/L and IL-6 >= 14.75pg/ml were significant predictors regarding to RMPP.ConclusionsCRP >= 16.5mg/L, LDH >= 417IU/L and IL-6 >= 14.75pg/ml might be the significant predictors of RMPP in children, which can aid in early recognition of RMPP.
Early Discrimination of Refractory Mycoplasma Pneumoniae Pneumonia in Children: A Multicenter Prospective Study in Zhejiang, China

Abstract:Objective: To explore potential predictors of RMPP in early stage.Study design: The prospective study, multicenter study was conducted in Zhejiang, China from May 1st, 2019 to January 31st, 2020. Children aged 29 days to 14 years old, with fever time during 48 to 120 hours were included. A total of 1428 children completed the study. A questionnaire was designed to collect patients’ information. Pharyngeal swab samples were collected. M. pneumoniae DNA in pharyngeal swab specimens were detected. Whether the patients develop to RMPP were assessed. Logistic regression analyses were used to examine associations between clinical data and RMPP.Results: The ages of the patients ranged from 34 days to 13.9 years with a median 4.3 years. The positive rate of M. pneumoniae -DNA was 37.4% (534/1428), and 446 cases were Mycoplasma pneumoniae pneumonia (MPP). In MPP patients, 55 cases were RMPP (12.3%), others were general MPP (GMPP) patients (n=391, 87.7%). Only the peak body temperature before the first visit and LDH level in RMPP patients were higher than that in GMPP [39.6 (39.1-40.0) °C vs. 39.2 (38.9-39.7) °C, p=0.003, and 332.5 (278.8-392.1) U/L vs. 310.5 (259.0-358.8) U/L, p=0.024]. Logistic regression also only included the above two parameters in the prediction probability. The area under ROC curve of the prediction probability π of RMPP was 0.682 (95% CI, 0.593-0.771), P<0.01. The cut-off value was 0.12. Sensitivity and specificity of the prediction probability π in cut-off value was 0.64 and 0.70, respectively.Conclusions: AND RELEVANCE A prediction probability, calculating from the peak body temperature before the first visit and LDH level for early identifying RMPP from other MPP within 2-5 days of fever duration, with a cut-off value of 0.12 may be helpful in clinical practice.

Abstract:Objective To analyze the associated factors of refractory Mycoplasma pneumoniae ( Mp ) pneumonia. Methods The cases of Mono-Mp pneumonia admitted in Children's Hospital of Fudan University from September 2012 to August 2013 were collected,and divided into two groups,refractory Mp pneumonia( RMPP ) and usual Mp pneumonia( UMPP ) . Laboratory outcomes collected by literature review were performed on the next day of admission. Chest X-ray and/or CT data were also collected in the first 3 days after admission. The laboratory outcomes and chest images were analyzed for the associated factors of refractory Mp pneumonia by univariate and multivariate logistic regression analysis. Results 653 cases of Mono-Mp pneumonia were collected,covered 51. 7%(653/1257)of the total number of pneumonia. There were 300 cases in refractory pneumonia group,171 boys and 129 girls. And there were 353 cases in normal pneumonia group,221 boys and 132 girls. ①Univariate analysis showed the age in RMPP group was significantly higher than that of UMPP group,(66. 8 ± 37. 5)vs(51. 4 ± 34. 4) months,P<0. 01;the proportion of cases over 3 years old in RMPP group was also significantly higher than that in UMPP group (234/300 vs 224/353,P<0. 01);the hospitalized days and febrile days in RMPP group were significantly higher than those in UMPP group(P<0. 01,respectively);laboratory outcomes in RMPP group,CK,LDH,HBDH,ALT,AST,CRP,PCT,IL-6, were significantly higher than those in UMPP group( P <0. 01,respectively);infiltration area of chest images in RMPP was significantly greater than that in UMPP,(1. 95 ± 1. 12)vs(1. 55 ± 0. 97),P <0. 01. ② Those significant outcomes in univariate analysis were chosen to perform stepwise logistic regression,and the analysis showed that febrile days( OR=1. 954, 95%CI:1. 403_2. 722),serum LDH level(OR=1. 009,95%CI:1. 001_1. 018),and infiltration area of chest images(OR=2. 422,95%CI:1. 111_5. 279)were the independent associated factors of RMPP. Conclusion Mp became a major pathogen of hospitalized children with CAP. Severe cases often occurred in children over 3 years old. The persistent fever,the large area of infiltration in chest images,high level of serum LDH in the early stage suggested that it may progress into severe pneumonia.
[Etiology and epidemic characteristics of hospital acquired pneumonia in children]

Abstract:Objective: To investigate the etiology and epidemic characteristics of hospital acquired pneumonia (HAP) in children. Method: A retrospective hospital infection study was performed in 52,639 children admitted to our hospital from January 2005 to December 2008. Result: Six hundred and ninety eight patients were diagnosed as HAP. The incidence of HAP was 1.33%, among which, 108 (15.47%) cases were early-onset HAP and 590 (84.53%) were late-onset HAP. The HAP patients aged 3 days to 15 years (503 male and 195 female). The proportion of patients younger than 1 year was 51.4%. Main underlying diseases were cytomegalovirus hepatitis, congenital heart disease, malignant tumor, granulocytopenia or agranulocytosis, prematurity and low birth weight. There was significant difference in the incidences among different departments with the highest one seen in ICU, followed by departments of infectious diseases, hematology and digestive diseases. Two hundred and thirty one stains of pathogens were identified from sputum of 355 cases. One hundred and fifty six (67.5%) strains were Gram-negative bacteria, which accounted for the highest proportion. There were 30 (13.0%) Gram-positive bacterial strains, and 29 (12.6%) respiratory tract virus strains, and 15 (6.5%) fungal strains, and 1 (0.4%) mycoplasma strain. The predominant bacterial pathogens were Klebsiella pneumoniae, followed by Stenotrophomonas maltophilia, Burkholderia cepacia, Escherichia coli and Acinetobacter baumannii. The isolation rates of Klebsiella pneumoniae and Escherichia coli with positive extended-spectrum beta-lactamases (ESBLs) were 94.8% and 85.7%, respectively. Those two bacteria were universally resistant to the third and forth generations cephalosporins. The main pathogens of early-onset HAP were respiratory syncytial virus (RSV), Streptococcus mitis, Streptococcus pneumonia, Haemophilus influenzae and Klebsiella pneumoniae, while the main pathogens of late-onset HAP were ESBLs-positive Klebsiella pneumonia, Stenotrophomonas maltophilia, Burkholderia cepacia, Escherichia coli and Acinetobacter baumannii. Conclusion: HAP in children is most common in children younger than 1 year and with underlying diseases. The main pathogens are Gram-negative bacteria. RSV was an important pathogen of HAP. The pathogens of early-onset HAP are different from those of late-onset HAP. These results may be of some help in prevention and control of HAP in children and in guiding for rational application of antibiotics, especially the empirical antibiotic choice.

Abstract:Background: Respiratory syncytial virus (RSV) is the most common pathogen of acute bronchiolitis in children, which sometimes triggers the development of recurrent wheezing and increases the risk of childhood asthma. Methods: We enrolled 425 children who were diagnosed with RSV-infected bronchiolitis at the department of pulmonology, Children's Hospital Zhejiang University School of Medicine in 2011. Long-term follow-up was performed to explore the consequence of bronchiolitis on subsequent recurrent wheezing and asthma. Results: Of 425 patients, 266 cases completed the entire follow-up, the mean age of onset was 4.9 (3.3) months, and the male-to-female ratio was 2.5. The mean birth weight of all patients was 3.22 (0.63) kg, and the number of patients who had a history of cesarean section was 148. According to the outcome of follow-up, 36 were in the recurrent wheezing (RW) group, 65 were in the asthma (AS) group, and the remaining 165 were in the completely recovered (CR) group. The age of onset was older and the birth weights were higher in the AS group than those in the CR group (P < 0.05). And the higher proportion of cesarean sections was higher in the RW group than that in the CR group (P < 0.05). Furthermore, we found a remarkable increasing of serum IgE in the AS groups than that in the CR group (P < 0.01). Multiple logistic regression analysis showed that the cesarean section was the risk factor for the development of recurrent wheezing and the higher birth weight was the risk factor for the development of asthma. Conclusion: RSV bronchiolitis might increase the incidence of recurrent wheezing and asthma. Allergic constitution was an important prerequisite for the occurrence of asthma, and related risk factor such as cesarean section can only increase recurrent wheezing to a certain extent within a certain period of time. And we also find higher birth weight and older onset age for those who develop asthma, which should be verified in the future.
Etiology and Clinical Epidemiology of Children with Acute Pneumonia in Shanghai Area

Abstract:Objective: To investigate the etiology and the characteristics of clinical epidemiology of acute pneumonia in children. Methods; Sputum specimens were collected from 724 children with acute pneumonia by nasotracheal aspiration and detection of pathogens by monoclonal antibody bridge linked enzyme assay, sputum culture and polymerase chain reaction (PCR) technique. Results; Microbial etiology were confirmed in 441 (60. 9% ) of 724 patients, among them viral infections were 160 cases (36. 3%), bacterial infections 125 cases (28. 3%), Mycoplasma pneumoniae 81 cases (18. 4%) and Chlamydia pneumoniae 25 cases (5. 7%). Pathogens detected in 724 cases were respiratory syncycial virus 129 (17.8%), adenovirus 19 cases (2.6%), parainfluen-za virus type 1, 3 13 cases (1.8%) and type2 2 cases (0.3%), Klebsiella pneumoniae 24 cases (3. 3%) , Streptococcus pneumoni-ae 23 cases (3.2%), Moraxella catarrhalis 23 cases (3.2%), Haemophilia influenzae 22 cases (3.0%), Staphylococcus aureus 19 cases (2. 6%), Haemophilus parahaemolyticus 18 cases (2. 5%), coagulase-negative Staphlococci 8 cases (1. 1%) and Es-cherichia coli 4 cases (0.6%), Mycoplasma pneumoniae 103 cases (14.2%) and Chlamydia pneumonias 21 cases (3.7%) respectively. Conclusions: Viral infection is the most common cause of acute pneumonia in children, followed by bacteria, Mycoplasma pneumoniae and Chlamydia pneumoniae. Respiratory syncycial virus is the most common pathogen, and the incidence of adenovirus pneumonia is decreasing. There was no evidence of outbreak of influenze and parainfluenzae virus infection. Haemophilus influenza and Streptococcus pneumoniae are the prominent pathogens of bacterial pneumonia, Klebsiella pneumoniae and Moraxella catarrhalis infection are increasing. Viral and Mycoplasma pneumoniae infection are more common in winter and spring while bacterial pneumonia are common in the early summer, however all can be seen in the whole year round. . The average age of patients with Mycoplasma pneumoniae infection become younger and Chlamydia pneumoniae infection is more common in infants.
Risk factors for mortality in children with pneumonia admitted to the pediatric intensive care unit

Abstract:AIMS: To describe the epidemiology of children with severe pneumonia and identify risk factors for poor outcomes.METHODS: We conducted a retrospective study of children admitted to pediatric intensive care unit (PICU) from 2010 to 2014 with a diagnosis of pneumonia. Clinical microbiological, ventilation and other pertinent PICU data were collected. Primary outcome was PICU mortality. Univariate and multivariate logistic regression model were used to identify risk factors for mortality.RESULTS: Severe pneumonia consisted of 237/3539 (6.7%) of PICU admissions. Of these, 162/237 (68.4%) required mechanical ventilation. 32/237 (13.5%) patients died. The majority of patients had no organisms identified 82/237 (34.6%). A sole bacterial or viral pathogen was identified in 48/237 (20.1%) and 41/237 (17.9%) patients, respectively. Patients with viral pneumonias were more likely to develop acute respiratory distress syndrome compared to other etiologies (7/41[17.1%] vs 8/196 [4.0%]; P = 0.006). Bacterial pneumonias were associated with lung abscess (4/48 [8.3%] vs 2/189 [1.5%]; P = 0.016) and necrotizing pneumonia (18/48 [37.5%] vs 15/189 [7.9%]; P < 0.001) compared to other etiologies. Co-detections (>1 respiratory pathogens isolated) occurred in 62/237 (26.2%) patients and were associated a higher rate of mechanical ventilation, and decreased ventilator and PICU free days. After adjusting for severity of illness, risk factors for mortality were: hospital acquired pneumonia (HAP) (aOR: 2.92 [95%CI 1.15, 7.40]; P = 0.024) and bacteremia (aOR: 5.03 [95%CI 1.77, 14.35]; P = 0.003).CONCLUSIONS: Severe pediatric pneumonia accounts for a significant number of PICU admissions and is associated with significant mortality risk. The presence of co-morbidities, HAP and bacteremia were early prognostic variables independently associated with poor clinical outcomes.

Abstract:ObjectiveTo investigate the etiology and epidemiological characteristics of pneumonia in children of different ages, to better characterize the co-infection patterns of pneumonia and their association with severe diseases.MethodsChildren aged 28 days to 13 years with pneumonia who were hospitalized in the Department of Pediatrics, Dongyang People's Hospital, Zhejiang Province from April 1 to December 28, 2023 were selected as the research subjects. Oropharynx swabs were collected from the patients within 24 hours of hospital admission, and PCR tests were conducted for 18 respiratory pathogens. Binary multivariate logistic regression analysis was used to analyze the status of viral and bacterial infection, patterns of co-infection in patients with different ages, and the risk factors for the outcome of severe pneumonia.ResultsA total of 2 191 hospitalized children with pneumonia were enrolled in the study. Severe cases were more common in children aged 5 years and older (53.3%) and in the second quarter of the year (46.5%). An average of (1.31±0.90) pathogens were detected in severe cases. Mycoplasma pneumoniae (44.4%, 973 cases) had the highest detection rate of pathogens. Streptococcus pneumoniae (21.7%, 476 cases) and rhinovirus (10.1%, 222 cases) were the most common bacteria and viruses, respectively, in hospitalized children with pneumonia in Dongyang City. Multivariate logistic regression analysis indicated positive interactions between different viral and bacterial pathogens. The adjusted OR (aOR) values for different respiratory pathogens in children with severe pneumonia varied significantly (all P<0.04). Among them, Chlamydia pneumoniae (aOR=9.74, 95%CI=2.36‒49.32, P<0.01), Mycoplasma pneumoniae (aOR=2.62, 95%CI=2.04‒3.37, P<0.01), and RSV (aOR=1.69, 95%CI=1.12‒2.54, P<0.01) were the risk factors for severe pneumonia.ConclusionIn the pathogen spectrum of children with pneumonia in Dongyang City, Zhejiang Province from April to December 2023, most viruses and bacteria exhibited positive interactions. Chlamydia pneumoniae, Mycoplasma pneumoniae, and RSV maybe the significant risk factors for severe pneumonia.

Abstract:Objective: To investigate the etiology of necrotizing pneumonia (NP) in children and the clinical characteristics of NP caused by different pathogens in China. Methods: A retrospective, case-control study was performed in children with NP who were admitted to 13 hospitals in China from January 2008 to December 2019. The demographic and clinical information, laboratory data, etiological and radiological findings were analyzed. The data were divided into three groups based on the following years: 2008-2011, 2012-2015 and 2016-2019, and the distribution characteristics of the pathogens in different period were compared. Meanwhile, the pathogens of pediatric NP in the southern and northern China were compared. And the clinical characteristics of the Mycoplasma pneumoniae (MP) NP and the bacterial NP were also compared. T-test or Mann-Whitney nonparametric test was used for comparison of numerical variables, and χ2 test was used for categorical variables. Results: A total of 494 children with NP were enrolled, the median ages were 4.7 (0.1-15.3) years, including 272 boys and 222 girls. Among these patients, pathogens were identified in 347 cases and the pathogen was unclear in the remaining 147 cases. The main pathogens were MP (238 cases), Streptococcus pneumoniae (SP) (61 cases), Staphylococcus aureus (SA) (51 cases), Pseudomonas aeruginosa (13 cases), Haemophilus influenzae (10 cases), adenovirus (10 cases), and influenza virus A (7 cases), respectively. MP was the most common pathogen in all three periods and the proportion increased yearly. The proportion of MP in 2016-2019 was significantly higher than that in 2012-2015 (52.1% (197/378) vs. 36.8% (32/87), χ2=6.654, P=0.010), while there was no significant difference in the proportion of MP in 2012-2015 and that in 2008-2011 (36.8% (32/87) vs. 31.0% (9/29), χ²=0.314, P=0.575).Regarding the regional distribution, 342 cases were in the southern China and 152 in the northern China. Also, MP was the most common pathogen in both regions, but the proportion of MP was higher and the proportion of SP was lower in the north than those in the south (60.5% (92/152) vs. 42.7% (146/342), χ2=13.409, P<0.010; 7.9% (12/152) vs. 14.3% (49/342), χ2=4.023, P=0.045). Comparing the clinical characteristics of different pathogens, we found that fever and cough were the common symptoms in both single MP and single bacterial groups, but chest pain was more common (17.0% (34/200) vs. 6.1% (6/98), χ2=6.697, P=0.010) while shortness of breath and wheezing were less common in MP group (16.0% (32/200) vs. 60.2% (59/98), χ2=60.688, P<0.01; 4.5% (9/200) vs. 21.4% (21/98), χ2=20.819, P<0.01, respectively). The white blood cell count, C-reactive protein and procalcitonin in the bacterial group were significantly higher than those in the MP group (14.7 (1.0-67.1)×109/L vs. 10.5 (2.5-32.2)×109/L, 122.5 (0.5-277.3) mg/L vs. 51.4 (0.5-200.0) g/L, 2.13 (0.05-100.00) μg/L vs. 0.24 (0.01-18.85) μg/L, Z=-3.719, -5.901 and -7.765, all P<0.01). Conclusions: The prevalence of pediatric NP in China shows an increasing trend during the past years. MP, SP and SA are the main pathogens of NP, and the most common clinical symptoms are fever and cough. The WBC count, C-reactive protein and procalcitonin in bacterial NP are significantly higher than those caused by MP.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>The epidemiology of <em>Mycoplasma pneumoniae</em> (MP) and local dominant etiologies of pathogens that cause respiratory tract infections (RTIs) among central China children (≤14 years old) hospitalized are poorly understood.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A total of 10,429 specimens were analyzed, and IgM antibodies against 9 respiratory pathogens including MP were detected using indirect immunofluorescence assay from serum.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>It showed that 59.3% of the enrolled children were positive for at least 1 pathogen; highest detection rates included those between 3 and <6 years of age (70.4%), female (63.2%), and who were hospitalized in 2014 (80.9%). The most predominant pathogen was MP (45.6%), followed by Parainfluenza viruses (PIVs) (22.6%) and influenza B viruses (IFVB) (14.7%). Coinfection was observed in 2,907 specimens (27.9%); the coinfection combination containing MP and PIVs had the highest detection rate of 15%, followed by MP and IFVB as well as IFVB and PIVs.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>MP was the most commonly detected bacteria among hospitalized children, which should be included in the differential diagnosis for hospitalized children with RTI. These findings will contribute to the effective prevention and therapeutic approaches of pathogens among local children suffering from RTI.</p>

Abstract:OBJECTIVE:To understand the etiology of pneumonia in hospitalized patients less than 3 years of age.METHODS:A total of 316 children with pneumonia admitted to the Children's Hospital of Suzhou University in Jiangsu Province from March, 2006 to January, 2007 were enrolled in this study. Sputum samples were obtained by deep nasotracheal aspiration technique for bacterial and viral cultures.RESULTS:Of the 316 samples, specific microbial etiology was obtained in 192 cases (60.8%). Bacterial infection was found in 162 cases (51.3 %), viral infection in 19 cases (6.3%)and compound infection with virus and bacteria in 11 cases (3.5 %). Haemophilus influenzae was the most common agent (46 cases; 14.6%) in bacterial infection, followed by Streptococcus pneumoniae (32 cases; 10.1%). Respiratory syncycial virus (RSV) was the most common agent (12 cases; 4.0%) in viral infection, followed by adenovirus (11 cases; 3.6%).CONCLUSIONS:Bacterial infection was a leading cause of pneumonia in children less than 3 years of age in Suzhou area. Haemophilus influenzae was the most common agent, followed by Streptococcus pneumoniae.
