Abstract:INTRODUCTION: Factor VIII has been shown to play an important role in cirrhotic patients with hypercoagulable phenotype. However, the role of factor VIII in portal vein thrombosis (PVT) among cirrhosis has not been well studied. We aimed to evaluate the association between increased level of factor VIII and PVT occurrence, as well as the extent of thrombosis in cirrhotic patients with clinical portal hypertension. METHODS: All cirrhotic patients aged 18–75 with confirmed gastroesophageal varices (GEV) seen at a regional referral center between January 1, 2018 and December 31, 2019 were enrolled. Patients with a history of liver transplantation, TIPS, malignant tumor, or cavernous transformation of the portal vein were excluded. A total of 424 patients were included and divided into PVT group (n = 96) vs non-PVT group (n = 328) based on the presence of PVT on computed tomography angiography report. RESULTS: The mean age of all included patients with cirrhosis and GEV was 57.0 ± 10.8 years, 61.1% (259) were male, 22.6% (96) with PVT, 20.5% (87) with a history of splenectomy. Factor VIII level was significantly higher in the PVT group compared with the non-PVT group (181.5 ± 65.9% vs 160.5 ± 53.4%, P = 0.005). PVT group was also noted to have significantly higher Child-Pugh grade, MELD score, splenectomy rate, prothrombin time, and significantly lower hemoglobin and albumin level compared with non-PVT group (Table 1). There were 71 cases with thrombosis in the main trunk, 26 in the left branch, 49 in the right branch, 28 in the superior mesenteric vein, and 24 in the splenic vein. Elevated factor VIII level was found to associate with increased occurrence of thrombosis in the main trunk (HR 1.007, 95% CI 1.003–1.011, P = 0.001), left branch (HR 1.011, 1.001–1.017, P = 0.001), right branch (HR 1.009, 1.004–1.014, P < 0.001), and superior mesenteric vein (HR 1.009, 1.003–1.015, P = 0.003); however, it had no effect on the occurrence of splenic vein thrombosis (HR 0.999, 0.992–1.007, P = 0.887). The level of factor VIII was positively correlated with the number of sites (0–4; main trunk, left branch, right branch, superior mesenteric vein) involved with thrombosis. (Figure 1) CONCLUSION: Elevated level of factor VIII was associated with occurrence of PVT and positively correlated with the number of sites involved with thrombosis in the portal system. Abstract:Objective To investigate the association of ABO blood types with VWF, FⅧ and ADAMTS13 levels and disease severity in liver cirrhosis (LC) patients. Methods Two hundred and twenty eight patients with liver cirrhosis and 168 healthy controls were enrolled in the study. Plasma VWF:Ag and ADAMTS13:Ag levels were assessed by ELISA; Fibrinogen, D- dimmer and F VIII:Ac were measured by an automated coagulometric method; ALT, AST, TB were measured by an automated biochemical analyzer; PLT were measured by blood analyzer, and blood ABO phenotyping was performed by the indirect technique. Results Increased VWF:Ag, F VIII:Ac and decreased ADAMTS13:Ag were observed in LC patients compared with healthy controls (all P＜0.01). Moreover, higher VWF:Ag and F VIII:Ac (both P＜0.05) were observed in non- O LC patients, and significant results remained when patients were stratified according to CTP and MELD score (P＜0.05). However, no difference was observed in ADAMTS13:Ag between LC patients with O and non- O blood types (P＞0.05). Conclusion LC patients with non- O blood type have increased VWF: Ag and F VIII: Ac, which may lead to dysfunction of microcirculation, and are likely to have more severe conditions.

Portal Vein Thrombosis Can Aggravate Esophageal Variceal Bleeding After Endoscopic Variceal Ligation

Abstract:Background: We aimed to evaluate whether portal vein thrombosis (PVT) and other factors influence esophageal variceal bleeding (EVB) after endoscopic variceal ligation (EVL) for liver cirrhosis patients. Methods: We retrospectively enrolled 255 patients with EVL in our hospital and parameters of age, gender, blood platelet, albumin, total bilirubin (TBil), D-dimer, activated partial thromboplastin time (APTT), prothrombin time (PT), Child-Pugh classification, portal vein thrombosis (PVT) and splenectomy were collected. Results: EVB patients had higher PVT diagnosed rate than non-EVB patients (P=0.009) and the rate of EVB had an increasing trend with the severity of ascites (P=0.034). On multivariate analysis, we still found PVT and ascites obviously affected incidence rate of EVB (P=0.026, P=0.027, respectively). Conclusion: PVT and ascites per se appeared to be the cause of EVB subsequent to EVL. If detected, anticoagulation or treatments refer to PVT and ascites needed to conducted in order to reduce the incidence of EVB after EVL.
Increased Portal Vein Diameter is Predictive of Portal Vein Thrombosis Development in Patients with Liver Cirrhosis

Abstract:Background: Cirrhotic patients with portal vein thrombosis (PVT) may have a high risk of hepatic decompensation and increased mortality. This study aimed to investigate if increased portal vein diameter is associated with PVT development. Methods: A total of 174 cirrhotic patients were enrolled between February 1 and August 31, 2017. All participants were divided into PVT (n=62) and non-PVT (n=112) groups based on the thrombus that was detected by ultrasonography and confirmed by computed tomography angiography (CTA). Results: The study participants, aged 54.7 +/- 10.5 years (PVT) and 55.8 +/- 11.6 years (non-PVT), were included in this analysis. The Child-Pugh score of PVT or non-PVT was 6.6 +/- 1.3 and 5.8 +/- 0.9, respectively. Hepatitis B virus (HBV) is the primary etiological agent of cirrhosis. Logistic regression, receiver operating characteristic (ROC), and nomograph analysis designated portal diameter as the strongest independent risk factor for predicting PVT development [odds ratio (OR): 3.96, area under the ROC curve (AUC): 0.88; P 0.01], and the cutoff with predictive value for PVT development was 12.5 mm. No differences were observed in the overall survival (OS) in cirrhosis with or without PVT or stratifying on portal diameter based on the cutoff value. Conclusions: Increased portal diameter is associated with an increased risk of PVT development. Patients with cirrhosis and increased portal diameter are a high-risk subgroup that may need thromboprophylaxis.
Platelet-derived TGF-β1 is Related to Portal Vein Thrombosis in Cirrhosis by Promoting Hypercoagulability and Endothelial Dysfunction

Abstract:BackgroundPortal vein thrombosis (PVT) is a serious complication of cirrhosis accompanied by unclear pathogenesis. Transforming growth factor-beta (TGF-β) has been implicated in atherosclerosis and venous thrombosis whereas study regarding its part in PVT is lacking. The aim of this study was to explore the role of cytokine TGF-β1 in PVT and the potential mechanism.Materials and methodsWe included patients with cirrhotic gastroesophageal varices and divided them into two groups according to the presence of PVT. Serum levels of TGF-β1 were detected using Cytometric Bead Array kit and compared between two groups. Coagulation status was assessed using thromboelastography (TEG). Primary liver sinusoidal endothelial cells were treated with TGF-β1 and evaluated for endothelial dysfunction by RT-PCR.ResultsOur results uncovered that TGF-β1 (6,866.55 vs. 3,840.60 pg/ml, P = 0.015) significantly increased in the PVT group. Splenectomy might promote PVT by increasing platelet-derived TGF-β1 levels. Other cytokines showed no difference between PVT and non-PVT groups. Besides, TGF-β1 was correlated with platelet, fibrinogen, TEG-CI, TEG-MA, and TEG-α (coef = 0.733, 0.494, 0.604, 0.608, and 0.511; P < 0.001, 0.027, 0.004, 0.004, and 0.021, respectively), which indicated a hypercoagulable state in PVT patients. RT-PCR of liver sinusoidal endothelial cells showed a markable increment of von Willebrand Factor (vWF), thrombomodulin(TM), intercellular adhesion moleclar-1(ICAM-1), and vascular endothelial growth factor(VEGF) after TGF-β1 treatment, suggesting the involvement of endothelial dysfunction.ConclusionElevated platelet-derived TGF-β1 exhibited association with hypercoagulability and promoting effect on endothelial dysfunction, closely related with PVT in cirrhotic patients.

Abstract:Portal vein thrombosis (PVT) is a significant issue in cirrhotic patients, necessitating early detection. This study aims to develop a data-driven predictive model for PVT diagnosis in chronic hepatitis liver cirrhosis patients. We employed data from a total of 816 chronic cirrhosis patients with PVT, divided into the Lanzhou cohort ( n = 468) for training and the Jilin cohort ( n = 348) for validation. This dataset encompassed a wide range of variables, including general characteristics, blood parameters, ultrasonography findings and cirrhosis grading. To build our predictive model, we employed a sophisticated stacking approach, which included Support Vector Machine (SVM), Naïve Bayes and Quadratic Discriminant Analysis (QDA). In the Lanzhou cohort, SVM and Naïve Bayes classifiers effectively classified PVT cases from non-PVT cases, among the top features of which seven were shared: Portal Velocity (PV), Prothrombin Time (PT), Portal Vein Diameter (PVD), Prothrombin Time Activity (PTA), Activated Partial Thromboplastin Time (APTT), age and Child–Pugh score (CPS). The QDA model, trained based on the seven shared features on the Lanzhou cohort and validated on the Jilin cohort, demonstrated significant differentiation between PVT and non-PVT cases (AUROC = 0.73 and AUROC = 0.86, respectively). Subsequently, comparative analysis showed that our QDA model outperformed several other machine learning methods. Our study presents a comprehensive data-driven model for PVT diagnosis in cirrhotic patients, enhancing clinical decision-making. The SVM–Naïve Bayes–QDA model offers a precise approach to managing PVT in this population.

Clinical characteristics and predictors of benign portal vein thrombosis in patients with liver cirrhosis: A retrospective single-center study

Abstract:Portal vein thrombosis (PVT) is a common thrombotic complication of cirrhosis. It can lead to variceal bleeding and bowel ischemia and also complicate liver transplantation. Identifying the possible risk factors associated with PVT can aid in identifying patients at high risk, enabling their screening and potentially preventing PVT through the rational use of anticoagulants. This study focuses on examining the clinical characteristics of PVT in cirrhotic patients and identifying the clinical and biochemical factors that are linked to the development of PVT. Consecutive hospitalized cirrhotic patients between 2015 and 2023 were identified through the hospital's computerized medical records based on the Tenth Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10) coding system and retrospectively analyzed. 928 individuals were included in this study; 783 (84.3%) without PVT and 145 (15.7%) with benign PVT. Hepatitis B virus (HBV) was significantly more common in the PVT group (P-value = .02), while alcohol and primary sclerosing cholangitis (PSC) were less common in this group (P-value = .01 and .02, respectively). Hepatocellular carcinoma (HCC) (P-value < .01), ascites (P-value = .01), and spontaneous bacterial peritonitis (SBP) (P-value = .02) were more common in the PVT group. Patients with PVT had a higher international normalized ratio (INR) level (P-value = .042) and lower plasma albumin (P-value = .01). No differences were identified in white blood cell, hemoglobin, platelet, and bilirubin levels. However, patients with PVT had higher model for end-stage liver disease (MELD) (P-value = .01) and Child-Pugh scores (P-value = .03). This study demonstrated a higher likelihood of PVT presence in cirrhotic patients with advanced age, HBV, and HCC, along with ascites, SBP, splenomegaly, hypoalbuminemia, elevated INR, and a higher MELD score. Nevertheless, additional research endeavors are necessary to accurately ascertain and validate supplementary risk factors within a broader demographic.

Abstract:Portal vein thrombosis (PVT), one of the most prevalent hepatic vascular conditions in patients with liver cirrhosis (LC), is associated with high mortality rates. An imbalance between a disintegrin-like metalloproteinase with thrombospondin type-1 motifs 13 (ADAMTS-13) enzyme and von Willebrand factor (VWF) is responsible for hypercoagulability, including spontaneous thrombus formation in blood vessels. Herein, we aimed to identify potential prognostic and diagnostic biomarkers in Japanese patients with LC and PVT. In total, 345 patients were divided into two groups: 40 patients who developed PVT (PVT group) and 305 who did not develop PVT (NPVT group). Among the 345 patients with LC, 81% (279/345) were deemed ineligible due to the presence of preventive comorbidities, active or recent malignancies, and organ dysfunction. The remaining 66 patients were divided into two groups: the PVT group (n = 33) and the NPVT group (n = 33). Plasma ADAMTS-13 activity (ADAMTS-13:AC) and the vWF antigen (VWF:Ag) were measured using enzyme-linked immunosorbent assays. Contrast-enhanced, three-dimensional helical computed tomography (CT) was used to detect and characterize PVT. ADAMTS-13:AC was significantly lower in the PVT group than in the NPVT group. No significant differences in plasma vWF:Ag or liver stiffness were observed between the two groups. ADAMTS-13:AC of <18.8 was an independent risk factor for PVT on multivariate analyses (odds ratio: 1.67, 95% confidence interval: 1.21–3.00, p < 0.002). The receiver operating characteristic analysis of ADAMTS-13:AC revealed an area under the curve of 0.913 in PVT detection. Patients with PVT having ADAMTS-13:AC ≥18.8 (n = 17) had higher albumin levels and better prognoses than those with ADAMTS-13:AC <18.8 (n = 16). No significant correlations of ADAMTS-13:AC levels with either fibrin degradation product or D-dimer levels were observed. ADAMTS-13:AC levels could be potential diagnostic and prognostic biomarkers for PVT in Japanese patients with LC.

Abstract:Portal vein thrombosis (PVT) is divided into cirrhotic and non-cirrhotic PVTs. The incidence rate of PVT varies greatly among different clinical stages of cirrhosis, with an overall incidence rate of about 13.92%, and the prevalence of cirrhotic PVT following splenectomy is as high as 60%. The pathogenesis of cirrhotic PVT is still unclear. However, the activation of Janus kinase/signal transduction and activator transcription signaling pathways, the rise in the expression of von Willebrand factor, and the gut microbiota along with its metabolite trimethylamine-N-oxide play an important role in the injury of vascular endothelial cells and the formation of PVT in cirrhosis. Therefore, these could be a new target for cirrhotic PVT prevention and treatment.

Abstract:BACKGROUND & AIMS: Portal vein thrombosis (PVT) is a common and serious complication in patients with cirrhosis. However, little is known about PVT in patients with cirrhosis and acute decompensation (AD). We investigated the prevalence and clinical significance of PVT in nonmalignant patients with cirrhosis and AD. METHODS: We performed a retrospective study of 2 cohorts of patients with acute exacerbation of chronic liver disease who participated in the Chinese AcuTe on CHronic LIver FailurE study, established by the Chinese Chronic Liver Failure Consortium, from January 2015 through December 2016 (n = 2600 patients) and July 2018 through January 2019 (n =1370 patients). We analyzed data on the prevalence, clinical manifestations, and risk factors of PVT from 2826 patients with cirrhosis, with and without AD. RESULTS: The prevalence of PVT in patients with cirrhosis and AD was 9.36%, which was significantly higher than in patients with cirrhosis without AD (5.24%) (P = .04). Among patients with cirrhosis and AD, 63.37% developed PVT recently (the first detected PVT with no indication of chronic PVT). Compared with patients without PVT, a significantly higher proportion of patients with PVT had variceal bleeding (47.33% vs 19.63%; P < .001) and patients with PVT had a significantly higher median serum level of D-dimer (2.07 vs 1.25; P < .001). Splenectomy and endoscopic sclerotherapy were independent risk factors for PVT in patients with cirrhosis and AD. The 1-year mortality rate did not differ significantly between patients with vs without PVT. CONCLUSIONS: In an analysis of data from 2826 patients with cirrhosis, a significantly higher proportion of those with AD had PVT than those without AD. PVT was associated with increased variceal bleeding, which would increase the risk for AD. Strategies are needed to prevent PVT in patients with cirrhosis, through regular screening, to reduce portal hypertension.
Analysis of Related Influencing Factors of Portal Vein Thrombosis After Hepatectomy

Abstract:Purpose: To analyze the related factors of portal vein thrombosis (PVT) after hepatectomy. Methods: A retrospective analysis was made on 1029 patients who underwent partial hepatectomy in the first affiliated Hospital of Chongqing Medical University from March 2018 to March 2023, including PVT group (n = 24) and non-PVT group (n = 1005). The general and clinical data of the two groups were collected. Univariate and multivariate logistic regression analysis was used to analyze the clinical information of the two groups. Result: The proportion of preoperative hepatitis B, liver cirrhosis, ascites, intraoperative blood transfusion, postoperative hemostatic drugs, preoperative prothrombin time, intraoperative portal occlusion time, operation time, international standardized ratio of prothrombin time on the first day after operation, D-dimer on the first day after operation, fibrin degradation products on the first day after operation and postoperative hospital stay in the PVT group were all higher than those in the control group (P < .05). The preoperative platelet and albumin in the PVT group were lower than those in the control group. Intraoperative blood transfusion, liver cirrhosis, ascites, international standardized ratio of postoperative prothrombin time, postoperative fibrin degradation products, hilar occlusion time and albumin were independent risk factors for PVT. Conclusion: There are many influencing factors of PVT after hepatectomy. Clinical intervention should be taken to reduce PVT. Clinical Registration Number: K2023-348.

Abstract:Objective To observe the risk factors for portal vein thrombosis (PVT) in cirrhotic patients prior to receiving endoscopic treatment to prevent gastroesophageal varices rebleeding.Methods A retrospective analysis was conducted on cirrhotic patients admitted to Zhongshan Hospital,Fudan University from 2008 to 2013 for secondary prevention of gastroesophageal varices bleeding via endoscopic treatment.Relevant information and data were collected,followed by an univariate analysis and multiple Logistic regression analysis in attempt to identify potential factors affecting the formation of PVT.Results Totally 591 patients were enrolled in the present study and were classified as present-PVT group (n =122,20.64 ％) and absent-PVT group (n =469,79.36 ％).Univariate analysis showed that MELD score,hemoglobin,platelet count,total bilirubin,alanine aminotransferase,blood urea nitrogen,splenectomy ratio all achieved statistical significance between the two groups (P＜0.05).The multiple Logistic regression showed that MELD score,lowered blood urea nitrogen,elevated D-dimer and history of splenectomy were independent factors associated with the formation of PVT (P values were separately 0.016,0.026,0.014 and ＜0.001).Conclusions Cirrhotic patients should receive regular surveillance for MELD score,liver function,D-dimer and portal vein ultrasonography,especially in patients received with a previous history of splenectomy.
A Diagnostic Prediction Model of Acute Symptomatic Portal Vein Thrombosis.

Abstract:BACKGROUND:The aim of this study was to develop a diagnostic prediction model to improve identification of acute symptomatic portal vein thrombosis (PVT). METHODS:We examined 47 patients with PVT and 94 controls without PVT in the Second Affiliated Hospital of Soochow University and Suqian People's Hospital of Nanjing, Gulou Hospital Group. We constructed a prediction model by using a support vector machine (SVM) classifier coupled with a least absolute shrinkage and selection operator (LASSO). We applied a 10-fold cross-validation to estimate the error rate for each model. RESULTS:The present study indicated that acute symptomatic PVT was associated with 11 indicators, including liver cirrhosis, D-Dimer, splenomegaly, splenectomy, inherited thrombophilia, ascetic fluid, history of abdominal surgery, bloating, C-reactive protein (CRP), albumin, and abdominal tenderness. The LASSO-SVM model achieved a sensitivity of 91.5% and a specificity of 100.0%. CONCLUSIONS:We developed a LASSO-SVM model to diagnose PVT. We demonstrated that the model achieved a sensitivity of 91.5% and a specificity of 100.0%.
Clinical analysis of portal vein thrombosis in patients with liver cirrhosis

Abstract:Objective To evaluate the etiology,relative risk factors,clinical manifestations and treatment of portal vein thrombosis in liver cirrhosis patients.Methods Twenty-nine patients of liver cirrhosis with PVT diagnosed at ShengJing hospital affiliated to China medical university from March 2007 to August 2009 were enrolled in this study.The data were collected and retrospective analysed.Results The clinical manifestations of liver cirrhosis with PVT are various lack of specificity.The increased level of D-dimer and platelet are independent risk factors for PVT in liver cirrhosis patients.Conclusion Understanding deeply,early diagnose and appropriate treatment regimen are the key for the prognosis for patients of liver cirrhosis with PVT.
Coagulation imbalance may not contribute to the development of portal vein thrombosis in patients with cirrhosis.

Abstract:Introduction: The relationship between the imbalance in pro- and anti-coagulant factors and portal vein thrombosis (PVT) in individuals with cirrhosis is unclear. The aim of this study was to determine whether the imbalance in pro-and anti-coagulant factors contributes to the development of PVT in cirrhotic patients. Materials and methods: Blood samples were collected from 30 consecutive cirrhotic patients with PVT and 30 age-, sex-, and Child-Pugh score-matched cirrhotic patients without PVT (controls), and the plasma levels of coagulation factors II, V, VII, VIII, IX, X, XI and XII and of protein C (PC), protein S (PS) and antithrombin (AT) were analyzed. The ratios of pro-vs. anti-coagulant factors were further investigated. Results: The levels of pro-and anti-coagulant factors were not statistically different between the PVT and control groups. Similar results were obtained when the patients were divided according to Child-Pugh classification. No difference was observed for the ratios of pro-vs. anti-coagulant factors between the two groups but the ratios of factor II-to-PC and factor VII-to-PC which were significantly decreased in the PVT group. Most of the ratios did not reach statistical significance in each Child-Pugh category except the followings: factor VIII-to-PS, factor XII-to-PC and factor XII-to-PS in class A patients; factor II-to-PS, factor VII-to-PC and factor VII-to-PS in class B patients. But the difference might not be so convincing. Conclusions: PVT in cirrhotic patients may not result from coagulation imbalance. (C) 2012 Elsevier Ltd. All rights reserved.

Abstract:<h3> Background & Aims</h3><p>The role of portal vein thrombosis (PVT) in the natural history of cirrhosis is controversial. There are few prospective studies validating risk factors for development of PVT.</p><p>We analyzed the incidence, factors associated with PVT development and its influence on cirrhosis decompensations and orthotopic liver transplant (OLT) free survival.</p><h3> Methods</h3><p>In this prospective observational study between January 2014 and March 2019, 445 consecutive patients with chronic liver disease were screened and finally 241 with cirrhosis included. Factors associated with PVT development and its influence on cirrhosis decompensations and OLT free survival by time dependent covariate coding were analyzed.</p><h3> Results</h3><p>Majority of patients belonged to Child‐Pugh class A 184 (76.3%) and average MELD score was 10±5. Previous cirrhosis decompensations occurred in 125 (52.1%), 63 (26.1%) were on NSBB and 59 (27.2%) had undergone banding for bleeding prophylaxis. Median follow‐up was 29 (1–58) months. Cumulative incidence of PVT was 3.7% and 7.6% at 1 and 3 years. Previous decompensation of cirrhosis and and low platelet counts but not NSBB independently predicted development of PVT. During follow‐up, 82/236 (34.7%) patients developed cirrhosis decompensations. OLT free survival was 100% and 82.8% at 3 years, with and without PVT respectively. MELD score, but not PVT, independently predicted cirrhosis decompensations (HR 1.14; 95%C.I.:1.09–1.19) and OLT free survival (HR 1.16;95%C.I.:1.11–1.21).</p><h3> Conclusion</h3><p>Previous decompensations of cirrhosis and thrombocytopenia predict PVT development in cirrhosis suggesting a pathophysiologic role for severity of portal hypertension. Abstract:Background and aim Cirrhotic patients with gastroesophageal varices and non-tumoral portal vein thrombosis have a higher risk of re-bleeding and poor prognosis. This study aimed to analyze inflammatory biomarkers and thromboelastography in cirrhotic patients with portal vein thrombosis. Methods A total of 385 consecutive cirrhotic patients with gastroesophageal varices were prospectively enrolled between 1 December 2016, and 31 August 2017. Of these, 231 were eligible for analysis and were divided into portal vein thrombosis (n = 103) and non-portal vein thrombosis (n = 128) groups based on computerized tomography angiography findings. Results Patients with portal vein thrombosis generally had higher Child-Pugh scores than those without portal vein thrombosis (6.38 ± 0.12 vs. 5.81 ± 0.09, P < 0.001). The serum albumin levels were significantly lower in patients with portal vein thrombosis (35.90 ± 0.52 vs. 38.52 ± 0.43, P < 0.001). The portal vein thrombosis group had significant higher serum levels of interleukin 6 [4.85 (3.15–6.99) vs. 3.09 (2.06–5.20) pg/ml, P < 0.001] and tumor necrosis factor alpha [10.70 (7.60–15.20) vs. 9.07 (7.03–11.60) pg/ml, P = 0.020]. The interleukin 6 level was 2.5-fold higher in patients with portal vein thrombosis (adjusted odds ratio: 2.574; 95% confidential interval: 1.248–5.310). Thromboelastography showed that TEG-R, the reaction time, was significantly lower in the portal vein thrombosis group [5.20 (4.80–6.30) vs. 6.00 (5.20–6.95), P = 0.009], indicating enhanced coagulation activity. Conclusion This study confirmed the important role of systemic inflammation in portal vein thrombosis. Interleukin 6, an important inflammatory cytokine, is independently associated with portal vein thrombosis. The correlation between the interleukin 6 level and portal vein thrombosis requires further investigation.

We analyzed data on the prevalence, clinical manifestations, and risk factors of PVT from 2826 patients with cirrhosis, with and without AD.</AbstractText>RESULTS: <AbstractText Label="RESULTS">The prevalence of PVT in patients with cirrhosis and AD was 9.36%, which was significantly higher than in patients with cirrhosis without AD (5.24%) (P = .04). Among patients with cirrhosis and AD, 63.37% developed PVT recently (the first detected PVT with no indication of chronic PVT). Compared with patients without PVT, a significantly higher proportion of patients with PVT had variceal bleeding (47.33% vs 19.63%; P &lt; .001) and patients with PVT had a significantly higher median serum level of D-dimer (2.07 vs 1.25; P &lt; .001). Splenectomy and endoscopic sclerotherapy were independent risk factors for PVT in patients with cirrhosis and AD. The 1-year mortality rate did not differ significantly between patients with vs without PVT.</AbstractText>CONCLUSIONS: <AbstractText Label="CONCLUSIONS">In an analysis of data from 2826 patients with cirrhosis, a significantly higher proportion of those with AD had PVT than those without AD.
